Medulloblastoma is the most common pediatric malignant brain tumor, consisting of four molecular subgroups (WNT, SHH, Group 3, Group 4) and 12 subtypes. Expression of the cell surface poliovirus receptor (PVR), CD155, is necessary for entry of the viral immunotherapeutic agent, PVSRIPO, a polio:rhinovirus chimera. CD155, physiologically expressed in the mononuclear phagocytic system, is widely expressed ectopically in solid tumors. The objective of this study is to elucidate CD155 expression as both a receptor for PVSRIPO and a therapeutic target in medulloblastoma.PVR mRNA expression was determined in several patient cohorts and human medulloblastoma cell lines. Patient samples were also analyzed for CD155 expression using immunohistochemistry and cell lines were analyzed using Western Blots. CD155 was blocked using a monoclonal antibody and cell viability, invasion, and migration were assessed.PVR mRNA expression was highest in the WNT subgroup and lowest in Group 4. PVR expression in the subgroups of medulloblastoma were similar to other pediatric brain and non-brain tumors. PVR expression was largely not associated with subgroup or subtype. Neither PVR protein expression intensity nor frequency were associated with overall survival. PVR expression was elevated in Group 3 patients with metastases but there was no difference in paired primary and metastatic medulloblastoma. Blocking PVR resulted in dose-dependent cell death, decreased invasion in vitro, and modestly inhibited cell migration.CD155 is expressed across medulloblastoma subgroups and subtypes. Blocking CD155 results in cell death and decreased cellular invasion. This study provides rationale for CD155-targeting agents including PVSRIPO and antibody-mediated blockade of CD155.

Radiofrequency ablation (RFA) using a V-shaped active tip needle may offer a larger lesion of the medial branch nerves, improving clinical outcome. The aim of our study is to evaluate the efficacy and the feasibility of RFA using V-shaped active tip needles.Methods: This is a single-center observational retrospective study. Clinical records were screened and analyzed if they met the following inclusion criteria: adult patients (> 18 years), diagnosis of chronic lumbar zygapophyseal joint pain, failure of conservative treatments, ability to provide informed consent for data analysis and publication. Exclusion criteria: lumbar pain not related to zygapophyseal joints, previous spinal/lumbar surgery, incomplete data, absence or withdrawal of informed consent. The primary outcome of the study was a change in pain intensity at follow-up. The secondary outcomes were the evaluation of quality-of-life improvement, the occurrence of adverse events and the impact on post-procedural analgesic consumption. For these purposes, pre- and post-treatment numeric rating scale (NRS), neuropathic pain 4 questions (DN4), EuroQoL - EQ-5D-3L, EQ-VAS, EQ-index and North American Spine Society (NASS) index were retrieved and analysed.Results: Sixty-four patients were included. 7.8% of patients at 1-month (CI95% 0.026, 0.173), 37.5% at 3-month (CI95% 0.257, 0.505), 40.6% at 6-month (CI95% 0.285, 0.536) and 35.9% at 9-month (CI95% 0.243, 0.489) follow-up reported a reduction of more than 80% in NRS Statistical analysis indicated a significant change in NRS, DN4, EQ-index and EQ-5D-VAS (p-value < 0.001) at the different time-points.Conclusion: RFA using a V-shaped active tip needle might be a feasible and effective treatment for chronic lumbar zygapophyseal joint pain.Keywords: arthralgia, chronic pain, zygapophyseal joint, pain management, radiofrequency ablation, neuromodulation, lumbar facet joint

While numerous numerical relativity simulations adopt a 1+log slicing condition, shock-avoiding slicing conditions form a viable and sometimes advantageous alternative. Despite both conditions satisfying similar equations, recent numerical experiments point to a qualitative difference in the behavior of the lapse in the vicinity of the black-hole puncture: for 1+log slicing, the lapse appears to decay approximately exponentially, while for shock-avoiding slices it performs approximately harmonic oscillation. Motivated by this observation, we consider dynamical coordinate transformations of the Schwarzschild spacetime to describe small perturbations of static trumpet geometries analytically. We find that the character of the resulting equations depends on the (unperturbed) mean curvature at the black-hole puncture: for 1+log slicing it is positive, predicting exponential decay in the lapse, while for shock-avoiding slices it vanishes, leading to harmonic oscillation. Chronic pain is debilitating and profoundly affects health-related quality of life. Spinal cord stimulation (SCS) is a well-established therapy for chronic pain; however, SCS has been limited by the inability to directly measure the elicited neural response, precluding confirmation of neural activation and continuous therapy. A novel SCS system measures the evoked compound action potentials (ECAPs) to produce a real-time physiological closed-loop control system.To determine whether ECAP-controlled, closed-loop SCS is associated with better outcomes compared with fixed-output, open-loop SCS at 24 months following implant.The Evoke study was a double-blind, randomized, controlled, parallel arm clinical trial with 36 months of follow-up. Participants were enrolled from February 2017 to 2018, and the study was conducted at 13 US investigation sites. SCS candidates with chronic, intractable back and leg pain refractory to conservative therapy, who consented, were screened. Key eligibility criteria included overall, back, and leg pain visual analog scale score of 60 mm or more; Oswestry Disability Index score of 41 to 80; stable pain medications; and no previous SCS. Analysis took place from October 2020 to April 2021.ECAP-controlled, closed-loop SCS was compared with fixed-output, open-loop SCS.Reported here are the 24-month outcomes of the trial, which include all randomized patients in the primary and safety analyses. The primary outcome was a reduction of 50% or more in overall back and leg pain assessed at 3 and 12 months (previously published).Of 134 randomized patients, 65 (48.5%) were female and the mean (SD) age was 55.2 (10.6) years. At 24 months, significantly more closed-loop than open-loop patients were responders (≥50% reduction) in overall pain (53 of 67 [79.1%] in the closed-loop group; 36 of 67 [53.7%] in the open-loop group; difference, 25.4% [95% CI, 10.0%-40.8%]; P = .001). There was no difference in safety profiles between groups (difference in rate of study-related adverse events: 6.0 [95% CI, -7.8 to 19.7]). Improvements were also observed in health-related quality of life, physical and emotional functioning, and sleep, in parallel with opioid reduction or elimination. Objective neurophysiological measurements substantiated the clinical outcomes and provided evidence of activation of inhibitory pain mechanisms.ECAP-controlled, closed-loop SCS, which elicited a more consistent neural response, was associated with sustained superior pain relief at 24 months, consistent with the 3- and 12-month outcomes.

Inspired by the recent developments in the free-standing 2D complex oxide perovskite membranes and bulk 2D halide perovskite photoabsorbers, here, we put forward a new computational material database that has screened the stabilities and physical properties of 5,112 free-standing 2D nanostructures for 213 ABX3 cubic perovskites in 3 crystallographic orientations and 4 thicknesses, each with 2 different types of terminating surface chemistries. Our screening results show that many ABX3 compounds may form free-standing 2D structures that are both chemically and dynamically stable. Generally, the optimized 2D structures exhibit a shrinkage of the in-plane B-X bond lengths that are compensated by the elongation of the bonds normal to the 2D plane. As a result, the electron density distributions along the out-of-plane directions are strongly perturbed leading to a complex structural-dependent variation in the space-charge polarizations in these free-standing 2D perovskites. However, the out-of-plane ionic contributions to the dielectric permittivity are completely suppressed because of the structural confinement effect. The dominant contribution to the ionic dielectric permittivity thus comes from the in-plane optical phonons, the change in the frequencies of which from their bulk counterparts exhibits a complex dependency with the structures and chemistries of the 2D structures. Furthermore, without considering surface passivations and structural defects, the majority of the free-standing 2D perovskites exhibit metallic behavior. In particular, transition metal halides show exclusive spin-polarization at the Fermi level, which may be harnessed for applications such as nano spin-polarizer. Finally, unsupervised machine learning is further applied to discover the intriguing chemical and structural patterns among the computationally screened free-standing 2D perovskite structures, allowing us to reveal in a fully `data-driven' manner, the dependence of the energetic stabilities upon the surface termination, ionic radii and the layer thicknesses of these nanostructures. With fully open-sourced data and analysis code, we anticipate the current work can inspire the future theoretical and experimental works in the developments of free-standing 2D perovskites to harness their ubiquitous properties for new applications in nanoelectronics, sensing, energy harvesting and many others.

Spinal cord stimulation (SCS) is considered an effective interventional nonpharmacologic treatment option for several chronic pain conditions. Here we present the effects of the novel evoked compound action potential (ECAP) controlled closed-loop (ECAP-CL) SCS system on long-term sleep quality outcomes from the EVOKE study.The EVOKE study is a double-blind, randomized, controlled clinical trial conducted at 13 sites in the United States (N = 134 patients). The clinical trial utilized SCS to manage chronic pain and compared novel ECAP-CL technology to open-loop SCS. Additionally, sleep quality data was collected using the Pittsburgh Sleep Quality Index (PSQI) at baseline and all study visits.The mean PSQI global score for ECAP-CL patients at baseline was 14.0 (n = 62; ± 0.5, SD 3.8), indicating poor sleep quality. Clinically meaningful and statistically significant reductions (p 0.001) in the global PSQI scores were noted at 12 months (n = 55; 5.7 ± 0.6, SD 4.2). A total of 76.4% of ECAP-CL patients met or exceeded Minimal Clinically Important Difference from baseline in PSQI at 12 months. Additionally, 30.9% of ECAP-CL patients achieved "good sleep quality" scores (PSQI ≤ 5), and 29.1% achieved sleep quality remission. "Normative" sleep scores were observed in 29.6% of ECAP-CL patients at 12 months, and these scores were better than the US general population. Additionally, ECAP-CL patients achieved statistically significant changes from baseline (p 0.01) across all seven subcomponent scores of PSQI at 12 months.ECAP-CL SCS elicits consistent neural activation of the target leading to less variability in long-term therapy delivery. In the EVOKE study, this resulted in ECAP-CL patients demonstrating clinically superior and sustained pain relief. Results from this study provide new evidence of long-term improvement in sleep quality and quantity in patients with chronic pain resulting from the use of this novel ECAP-CL SCS technology.The Clinicaltrials.gov registration number for the study is NCT02924129.

Milk related materials are frequently used as a vehicle for drug product administration. Therefore, drug solubility information in milk related vehicles is desirable for prediction of how they may influencein vivodrug release and bioavailability. However, there are very limited data published on this topic. This study explored a practical method to address the key challenges associated with solubility assessment in milk, including the sample equilibration time and cleanup procedures. Amitriptyline, acetaminophen, dexamethasone, nifedipine, piroxicam, and prednisolone were selected as model drugs to represent a wide range of physicochemical properties. Their solubilities were determined at room temperature in pH 6.8 phosphate buffer, skim milk, whole milk, reconstituted whole milk powder, and unprocessed raw milk. The overall results confirmed that milk greatly improves the solubility of poorly water-soluble drugs. However, the extent of improvement and mechanism of solubilization appeared unique for each drug, highlighting the importance of evaluating milk solubility experimentally. The method used in this exploratory study can be applied in future investigations of a broader range of drugs and milk-related vehicles.

Neutron irradiation induced degradation of porous silica film is studied by Molecular Dynamics and Density-Functional theory-based methods. The degradation of microscopic structure, thermal property, and optical property of porous silica film are systematically investigated. Low-energy recoil is used to simulate the neutron irradiation effect. The pair and bond angle distributions, and coordination number distributions reveal that, under neutron irradiation, the microscopic structure of porous silica film is obviously modified, and the coordination defects are induced. We find that the higher recoil energy, the more coordination defects are formed in the film. The increased defects lead to a decrease in thermal conductivity. In addition, neutron irradiation induces additional optical absorption peaks in UV region and increasement in refractive index, resulting in a noticeable reduction in light transmittance. The detailed calculation of density of states reveals that these optical absorption peaks originate from the irradiation induced defect states in band gap. Our work shows that low-energy neutron irradiation can induce obvious defect density and degrade thermal and optical properties of porous silica film, which are responsible for subsequent laser-induced damage.

Two-dimensional transition metal dichalcogenide (TMDC) nanocrystals (NCs) exhibit unique optical and electrocatalytic properties. However, the growth of uniform and high-quality NCs of monolayer TMDC remains a challenge. Until now, most of them are synthesized via a solution-based hydrothermal process or ultrasonic exfoliation method, in which the capping ligands introduced from organic solution often quench the optical and electrocatalytic properties of TMDC NCs. Moreover, it is difficult to homogeneously disperse the solution-based TMDC NCs on a substrate for device fabrication, since the dispersed NCs can easily aggregate. Here, we put forward a novel CVD method to grow closely spaced MoS2 NCs around 5 nm in lateral size. TEM and AFM characterizations demonstrate the monolayer and high-crystalline nature of MoS2 NCs. An obvious blue-shift with 130 meV in photoluminescence signals can be observed. The MoS2 NCs also show an outstanding surface-enhanced Raman scattering for organic molecules due to their localized surface plasmon and abundant edge sites and exhibit excellent electrocatalytic properties for the hydrogen-evolution reaction with a very low onset potential of ∼50 mV and Tafel slope of ∼57 mV/decade. Finally, we further demonstrate this kind of CVD method as a versatile platform for the growth of other TMDC NCs, such as WSe2 and MoSe2 NCs.

BACKGROUND The goal of this study was to demonstrate that the paresthesia-independent 10 kHz spinal cord stimulation (SCS) can provide long-term pain relief in patients with peripheral polyneuropathy (PPN). Clinically diagnosed subjects with PPN refractory to conventional medical management were enrolled in this prospective, multicenter study between November 2015 and August 2016, after institutional review board approval and patient informed consent were obtained. METHODS Subjects underwent trial stimulation utilizing 2 epidural leads, and if successful, were implanted with a permanent 10 kHz SCS system and followed up for 12 months post-implant. Outcome measures included adverse events, pain, neurological assessments, disability, function, quality of life, pain interference, sleep, satisfaction, and global impression of change. Data are presented as descriptive statistics. Permanent implant population results are reported as mean ± standard error. RESULTS Twenty-one of the 26 trialed subjects had a successful trial and 18 received a permanent implant. All subjects had the leads placed anatomically without the need for paresthesia. Subjects experienced significant and sustained pain relief (at least 65% at all timepoints) whereas physicians noted improvements in neurological function. Significant improvements in disability, function, sleep, sensory, and affective dimensions of pain were reported at all timepoints. All adverse events were resolved without sequelae. CONCLUSION Findings from this study suggest that 10 kHz SCS may provide sustained pain relief and disability improvements in patients suffering from PPN.

Perovskite multiferroics have drawn significant attention in the development of next-generation multifunctional electronic devices. However, the majority of existing multiferroics exhibit ferroelectric and ferromagnetic orderings only at low temperatures. Although interface engineering in complex oxide thin films has triggered many exotic room-temperature functionalities, the desired coupling of charge, spin, orbital and lattice degrees of freedom often imposes stringent requirements on deposition conditions, layer thickness and crystal orientation, greatly hindering their cost-effective large-scale applications. Well-defined atomically dispersed metal catalysts (or single-atom catalysts) have been widely studied to fundamentally understand their catalytic mechanisms, improve the catalytic efficiency, increase the abundance of active components, enhance the catalyst utilization, and develop cost-effective catalysts to effectively reduce the usage of noble metals. Such single-atom catalysts have relatively higher selectivity and catalytic activity with maximum atom utilization due to their unique characteristics of high metal dispersion and a low-coordination environment. However, freestanding single atoms are thermodynamically unstable, such that during synthesis and catalytic reactions, they inevitably tend to agglomerate to reduce the system energy associated with their large surface areas. Therefore, developing innovative strategies to stabilize single-atom catalysts, including mass-separated soft landing, one-pot pyrolysis, co-precipitation, impregnation, atomic layer deposition, and organometallic complexation, is critically needed. Many types of supporting materials, including polymers, have been commonly used to stabilize single atoms in these fabrication techniques. Herein, we review the stabilization strategies of single-atom catalyst, including different synthesis methods, specific metals and carriers, specific catalytic reactions, and their advantages and disadvantages. In particular, this review focuses on the application of polymers in the synthesis and stabilization of single-atom catalysts, including their functions as carriers for metal single atoms, synthetic templates, encapsulation agents, and protection agents during the fabrication process. The technical challenges that are currently faced by single-atom catalysts are summarized, and perspectives related to future research directions including catalytic mechanisms, enhancement of the catalyst loading content, and large-scale implementation are proposed to realize their practical applications. Graphical Abstract Single-atom catalysts are characterized by high metal dispersibility, weak coordination environments, high catalytic activity and selectivity, and the highest atom utilization. However, due to the free energy of the large surface area, individual atoms are usually unstable and are prone to agglomeration during synthesis and catalytic reactions. Therefore, researchers have developed innovative strategies, such as soft sedimentation, one-pot pyrolysis, coprecipitation, impregnation, step reduction, atomic layer precipitation, and organometallic complexation, to stabilize single-atom catalysts in practical applications. This article summarizes the stabilization strategies for single-atom catalysts from the aspects of their synthesis methods, metal and support types, catalytic reaction types, and its advantages and disadvantages. The focus is on the application of polymers in the preparation and stabilization of single-atom catalysts, including metal single-atom carriers, synthetic templates, encapsulation agents, and the role of polymers as protection agents in the manufacturing process. The main feature of polymers and polymer-derived materials is that they usually contain abundant heteroatoms, such as N, that possess lone-pair electrons. These lone-pair electrons can anchor the single metal atom through strong coordination interactions. The coordination environment of the lone-pair electrons can facilitate the formation of single-atom catalysts because they can enlarge the average distance of a single precursor adsorbed on the polymer matrix. Polymers with nitrogen groups are favorable candidates for dispersing active single atoms by weakening the tendency of metal aggregation and redistributing the charge densities around single atoms to enhance the catalytic performance. This review provides a summary and analysis of the current technical challenges faced by single-atom catalysts and future research directions, such as the catalytic mechanism of single-atom catalysts, sufficiently high loading, and large-scale implementation.

Despite the efficacy of immune checkpoint inhibitors (ICIs), adverse events including hepatotoxicity limit their ongoing use. We investigated the outcomes and management of patients with immune-mediated hepatitis (IMH) and clinical predictors of toxicity resolution.Patients referred to our multidisciplinary immunotherapy-related toxicity group from August 2017 to December 2020 for IMH were evaluated. Toxicity was defined according to CTCAEv4.0. IMH resolution was defined as liver enzyme normalization after steroid initiation.Thirty-three patients were included in the study, 62% female, and 71% Caucasian. The most common ICI used was PD-1/PD-L1 (76%). Peak IMH occurred at a median of 89 [45,193] days, for which most patients received 1-2 mg/kg/day prednisone equivalent with 35% requiring MMF. Median follow-up was 123 [33,472] days with IMH resolution seen in 48% of patients at a median of 111 [41,214] days. While high-dose steroid use was not associated with IMH resolution, liver enzyme improvement one week after steroids predicted resolution in univariate analysis (p = 0.041). All 11 patients without IMH resolution died from cancer progression or complications with three patients having acute liver failure. Available liver biopsies showed bile duct injury, with varying degrees of portal and lobular inflammation.IMH improvement one week after steroid initiation may predict ultimate IMH resolution.

OBJECTIVE The Chiari Severity Index (CSI) and points-based algorithm of Thakar et al. are two prognostic tools that have been developed to predict the likelihood of improvement after suboccipital decompression in adult patients with Chiari malformation type I (CM-I). This study aimed to externally validate and critically evaluate these algorithms in the interest of guiding the development of improved prediction systems. METHODS A consecutive cohort of CM-I patients undergoing suboccipital decompression between September 2006 and September 2018 were included. The CSI and Thakar point score were computed for all patients, and associations with improvement were analyzed. The ability of both prediction systems to predict improvement as measured by different Chicago Chiari Outcome Scale (CCOS) cutoffs was assessed using receiver operating curve analysis. Post hoc correlations between the algorithms and different CCOS subcomponents were also assessed. RESULTS The surgical cohort was composed of 149 adult CM-I patients, of whom 39 (26%) had a syrinx. Most patients experienced improvement after surgery (80% CCOS ≥ 13; 96% CCOS ≥ 11). The proportion of patients improving decreased with increasing CSI, but the results were not statistically significant (p = 0.246). No statistically significant difference in the mean Thakar point score was identified between improved and nonimproved patients using both CCOS cutoffs (p = 0.246 for a cutoff of 13 and p = 0.480 for a cutoff of 11). The CSI had a poor ability in identifying improved patients at a CCOS cutoff of 13 (area under the curve [AUC] 0.582) and 11 (AUC 0.646). The Thakar point score similarly had poor discrimination at a cutoff of 13 (AUC 0.467) and 11 (AUC 0.646). Neither algorithm had significant correlation with any of the CCOS subcomponents except for CSI and nonpain symptom improvement (coefficient = −0.273, p = 0.001). CONCLUSIONS Previously published algorithms failed to provide prediction value with regard to clinically meaningful improvement following suboccipital decompression in adult CM-I patients. Future models and practical scoring systems are still required to improve the decision-making process.

Oxide perovskites are promising candidates for future solar energy conversion applications due to their intriguing properties, e.g. above bandgap photovoltage, which can be beneficial to overcome fundamental limitations of well-established semiconductor materials. Yet, their inherent wide bandgap remains a hurdle. This work revisited the long-standing concerns about the bandgap reduction in bismuth layered perovskite oxides through a combination of theoretical and experimental approaches. Ab initio calculations suggest that the reduced bandgap in cobalt-doped bismuth titanate can be attributed to the increase of density-of-states in the vicinity of the valence and conduction band edge. More importantly, comparison of the energy landscape for different cobalt substitution sites and its correlation with electronic bandgap shows that substituting Ti with cobalt equally in both inner and outer perovskite blocks with respect to the vicinity of Bi2O2 layer, is the most energetically favoured configuration with a significant 45% reduction of bandgap compared to the undoped bismuth layered oxide. The experimental replication of the bandgap tuning in cobalt modified Bi3.25La0.75Ti3O12 thin films prepared under optimal processing conditions strongly underpins the validity of our DFT prediction. Such compositional and structural engineering enabled a much narrower bandgap (∼1.7 eV) followed with an approximately 8-fold enhancement of the photocurrent density compared to the parental structure. This work will facilitate the design and synthesis route for tailoring the conventional wide bandgap ferroelectric oxides to be applied in photovoltaic and optoelectronic devices.

Sacroiliac joint disease is a prominent diagnosis across the world. A novel fixation technique employing a posterior approach, single point, bone allograft transfixation has proven to be helpful anecdotally. The purpose of this is study is to investigate prospectively the safety and efficacy of this approach.A multicenter, prospective, single arm study was performed after patient identification and treatment with the novel posterior fusion, single-point transfixation system and followed for 24 months. Target enrollment is 100 patients. Interim results on the first 69 consecutive patients at 6 months is presented. Primary endpoint at 6-month analysis was Pain Intensity reduction by visual analogue scale and functional improvement by Oswestry Disability Index. Adverse events were assessed for safety analysis.In total, 69 patients were identified for this analysis. At 6 months, a mean improvement of 34.9 was identified by a reduction in VAS and functional improvement was demonstrated by a mean reduction in ODI of 17.7. There were three adverse events, all unrelated to the device.The posterior single point transfixation is safe and efficacious for the treatment of sacroiliac joint dysfunction with statistical improvements in pain and function.
