Tuberculous peritonitis is one of the least common forms of extrapulmonary tuberculosis. Tuberculous peritonitis in pregnancy is a diagnostic challenge, especially in the absence of lung involvement. We report a case of peritoneal tuberculosis in a 32-Year-old female, a G2A1 with 30 weeks’ gestation came with chief complaint of profuse clear discharge from the umbilicus. USG abdomen and pelvis was suggestive of 1 cm sized wide hypoechoic track in the umbilical region likely communicating with anterior uterine wall, while MRI Fistulogram was suggestive of Oedematous sinus tract in the cutaneous and subcutaneous plane of the periumbilical anterior abdominal wall with no obvious extension beyond the rectus sheath. Anti-tubercular treatment was started for the patient immediately upon diagnosis. The flow of clear discharge ceased within 2 days and the symptoms resolved in 2 weeks. This case is being presented for the rarity of its occurrence specifically in pregnancy.

Non-Rigid Structure-from-Motion (NRSfM) reconstructs a deformable 3D object from the correspondences established between monocular 2D images. Current NRSfM methods lack statistical robustness, which is the ability to cope with correspondence errors.This prevents one to use automatically established correspondences, which are prone to errors, thereby strongly limiting the scope of NRSfM. We propose a three-step automatic pipeline to solve NRSfM robustly by exploiting isometry. Step 1 computes the optical flow from correspondences, step 2 reconstructs each 3D point's normal vector using multiple reference images and integrates them to form surfaces with the best reference and step 3 rejects the 3D points that break isometry in their local neighborhood. Importantly, each step is designed to discard or flag erroneous correspondences. Our contributions include the robustification of optical flow by warp estimation, new fast analytic solutions to local normal reconstruction and their robustification, and a new scale-independent measure of 3D local isometric coherence. Experimental results show that our robust NRSfM method consistently outperforms existing methods on both synthetic and real datasets., Accepted in TPAMI 2021

The paper demonstrates curcumin/β-cyclodextrin-based inclusion complex (IC) loaded polyvinyl alcohol (PVA) dip-coated and copolymer-compatibilized polylactic acid (PLA)/poly(ε-caprolactone) (PCL) blend-based electrospun mats (EMs) as antibacterial, and suture-resistant constructs, to overcome the present challenges in developing structurally-stable, biocompatible, pliable, and stand-alone multifunctional-biomedical-devices. The thermal, microstructural, and viscoelastic characterization confirmed the presence of H-bonding interactions between IC and PVA moieties and between IC incorporated PVA matrix with the copolymer-mediated nanotextured PLA/PCL blend-based EMs. IC release and surface PVA erosion induced a decrease in modulus (4-fold) and strength (2-fold) of constructs (post-release). Mechanistically new and architectural-framework-defined PVA-gelation induced bi-axially diverted suture-failure (post-release) and resulted in a significant enhancement in suture-retention-strength (3-fold), energy (5-fold), and displacement (2-fold) for ~20 wt% IC-loaded-PVA-coated EM-constructs. Precision medicine can put clinicians in a position where they must act more as resource allocators than their traditional role as patient advocates. In the allocation of transplantable organs and tissues, the use of eplet matching will enhance precision medicine but, in doing so, generate a tension with the present reliance on rule of rescue and justice-based factors for allocations. Matching donor and recipient human leukocyte antigens (HLA) is shown to benefit virtually all types of solid organ transplants yet, until recently, HLA-matching has not been practical and was shown to contribute to ethnic/racial disparities in organ allocation. Recent advances using eplets from the HLA molecule has renewed the promise of such matching for predicting patient outcomes. The rule of rescue in organ allocation reflects a combination of ethical, policy, and legal imperatives. However, the rule of rescue can impede the allocation strategies adopted by professional medical associations and the optimal use of scarce transplant resources. While eplet-matching seeks to improve outcomes, it may potentially frustrate current ethics-motivated initiatives, established patient-practitioner relationships, and functional conventions in the allocation of medical resources such as organ and tissue transplants. Eplet-matching allocation schemes need to be carefully and collaboratively designed with clear, fair and equitable guidelines that complement functional conventions and maintain public trust., La médecine de précision peut placer les cliniciens dans une position où ils doivent agir davantage comme des répartiteurs de ressources que dans leur rôle traditionnel de défenseurs des patients. Xantho-granulomatous oophoritis is an uncommon, non-neoplastic, chronic process affecting female genital tract. Its clinical and radiological features mimic ovarian neoplasm. Here we present a case report of 42-year-old female with chronic pelvic pain with adnexal mass with infertility and raised CA 125 levels. Histopathological examination revealed it to be a case of xantho-granuous oophoritis. Xantho-granulomatous oophoritis is very rare disease and only few cases have been reported so far and this case is being reported in view of its rarity.

Many tasks of Nurses are routine, quite difficult, disgusting, unpleasant, and not rewarding, nurses are likely to burn out due to job stress. Job stress affects nurses' health, increases absenteeism and attrition, errors in treating patients, infection rates, and increased cost of employment for hospitals.The aim of quantitative descriptive survey study was to find out the Job stress of 300 female nurses working in selected private hospitals in the Kottayam, selected by simple random sampling in December 2021. Socio-demographic tool and Nurses’ Occupational Stressor Scale (NOSS) were used for data collection.The majority (61%) of the subjects belonged to the age group of 31 to 40 years. The data showed that the majority of the subjects (89%) were married, 56% were from nuclear families and most had 1-2 children. Among the subjects, 55% of spouses had private jobs while 2% were unemployed and 9% worked abroad. Most of the subjects (78%) were GNM. The scope of work was divided equally among the staff, ie, medical 30%, surgical 17%, and critical care medicine/emergency medicine/OT 43%. Of 300 subjects, 47.6% had more than 5 years of experience. 82% of subjects underwent CNE and 73% participated in stress management programs at some stage of their professional life. Of the 300 subjects, 80.3% reported as they are receiving support from their immediate supervisors and Hospital management, while 53% reported that their job interfered with family life, and 44% were moderately satisfied in meeting their family needs. 81% of the subjects showed medium job stress. The mean was 51 (the mean percentage was 60.8%) with a standard deviation of 5.9.For nurses, job stress seems to vary according to individual and job characteristics and work-family conflicts. Interventions should be planned to prevent and manage stress, which in turn contributes to job performance and productivity.

The purpose of this research is to investigate the influence that marketing communication and information sharing have on the performance of businesses. The relationship between marketing communication, information sharing, client retention, and personalization, as well as the success of the firm, is investigated in this approach. In this research endeavour, a questionnaire was the instrument of choice for data collecting. A questionnaire was constructed with the help of previously discovered scale items relevant to marketing communication, information sharing, customer retention, personalization, and business performance. This led to the development of the questionnaire. The staff members of India's small and medium-sized firms (SMEs) each received one of the 500 questionnaires that were handed out to them. Partial Least Square was the statistical tool that was used to do the analysis of the data (PLS). It has been discovered that effective marketing communication has a beneficial effect on the retention of existing customers, which in turn leads to improved company performance. Additionally, the exchange of information has a beneficial effect on customisation, which ultimately leads to the performance of the company.

The nature of gene action and combining ability is valuable in determining whether heterosis is fixable or predictable. Thus, to know the nature of gene action of seed cotton yield and its components and fibre quality parameters, seventeen parents (15 males and two genetic male sterile lines) and 30 F1 crosses (genetic male sterility based) were evaluated in a line x tester mating design of Asiatic cotton (Gossypium arboreum L). Sufficient variability among hybrids and their parents was found as the mean squares of genotypes for all the traits including fibre quality parameters investigated were significant. The total genetic variability was partitioned to general combining ability (GCA) and specific combining ability (SCA) effects. GCA was significant for male parents HD-123, RG-270, CISA-59, CISA-64, CISA-65, CISA-66 and LD-694 for seed cotton yield (SCY); HD-123, RG-286 and DLSA-24 for bolls per plant; HD-123, CISA-59, PAIG-8/3, DLSA-9 and LD-694 for boll weight; female parent DS-5, male parents HD-123, RG-18, CISA-65, and PAIG-8/3 for ginning outturn (GOT%); RG--270, RG-286, PAIG-8/3, DLSA-9 and LD-694 for 2.5% span length (mm); RG-286, CISA-59, CISA-66, PAIG-8/3 and LD-694 for lower micronaire value having finenessand HD-400, RG-270, RG-286, RG-341 CISA-60, CISA-66, PAIG-8/3 and LD-694 for tenacity (g/tex) which were considered as good general combiners. Similarly among 30 cross combinations, SCA was positively significant for 10 crosses for seed cotton yield, 10 crosses for boll number, 10 crosses for boll weight, 9 crosses for GOT(%), 10 crosses for 2.5% span length and 8 crosses for fibre strength (gt/tex).Cluster III of yield attributing traits, consisted of 9 crosses ( C3, C6, C18, C20, C26, C22, C23, C24, C30, and C8) showed the highest average seed cotton yield where as Cluster II of fibre quality traits consisted of 3 crosses ( C2, C12, C15) showed highest average GOT(%) with highest tenacity (g/tex). Cross DS-5 x HD 123for SCY and bolls per plant, DS-5 x DLSA 9 for boll weight, DS-5 x CISA 65 for GOT % , CISA-2 x CISA 66 for lower micronaire value (fibre fineness) showed significant estimation of heterosis in desirable direction and could be further utilized in the future breeding programme for specific trait improvement.

Thestudy is to compare the effect of oral hydration in pregnant women with moderate isolated oligohydramnios (AFI 5-8 cm) over 28 weeks of gestation. The study was conducted on 50 pregnant women, randomized into two groups, each group having 25 women. Study group comprising 25 pregnant women with moderate oligohydramnios from study group were given 2 litres of water in 2 hours for 7 days apart from their routine water intake and ultrasound was done after 2 hours after completion of water intake, 24 hours and then after 7 days. While 25 pregnant women with moderate oligohydramnios from control group were give routine medications as per the Institute protocol and was explained to have routine water intake. Ultrasound was done after 2 hours after registering the case as control, 24 hours and 7 days.: Amniotic fluid index improved in 19 cases as compared to pretreatment levels in study group. Amniotic fluid index worsened in 21 pregnant women from control group. The difference was statistically significant (p Oral maternal hydration therapy with water improves the level of amniotic fluid index significantly in third trimester of pregnancy. Hydration with water is not associated with any adverse maternal outcomes.

Vitamin D deficiency was found to be more prevalent in India despite having sunny weather. Its deficiency was found to be correlated with glycosylated hemoglobin. We conducted this study in an effort to better understand the relationship between vitamin D and glycosylated hemoglobin in non-obese. A total of 119 type 2 diabetes patients were recruited in this cross-sectional study. Patient details and biochemical parameters were collected. Participants were divided into groups with vitamin D deficiency 25(OH) D 20 ng/mL, Further subgrouping was done into male and female-only groups. A growing antimicrobial crisis has increased demand for antimicrobial materials. It has become increasingly popular to convert polymeric macromolecules into carbonized polymer nanodots (CPDs) in order to achieve highly biocompatible materials with unique properties as a result of the ability to synthesize nanomaterials of the right size and add value to existing stable polymers. This work presents the tuning of polymeric carbon dots (PCDs) for antibacterial application by combining a biocidal polymer with one-pot solvothermal synthesis. PCDs displayed broad-spectrum antibacterial activity via various mechanisms, including inhibition of bacterial cell walls, ROS generation, and antibiotic resistance. Further, these biocidal PCDs were observed to show excitation-independent near-white light emission which on the other hand is generally possible due to mixed sizes, doping, and surface effects. As opposed to the parent biocidal polymer, CD added ROS-mediated bactericidal activity, increased cytocompatibility and nanofibers with anti-adhesive impact and the potential of imaging bacterial cells.

Inflammation in ulcerative colitis is typically restricted to the mucosal layer of distal gut. Disrupted mucus barrier coupled with microbial dysbiosis has been reported to occur prior to the onset of inflammation. Here, we show the involvement of vesicular trafficking protein Rab7 in regulating the colonic mucus system. We identified a lowered Rab7 expression in goblet cells of colon during human and murine colitis. In vivo Rab7 knocked down mice (Rab7KD) displayed a compromised mucus layer, increased microbial permeability and depleted gut microbiota with enhanced susceptibility to dextran sodium-sulfate induced colitis. These abnormalities emerged owing to altered mucus composition, as revealed by mucus proteomics, with increased expression of mucin protease Chloride channel accessory 1 (CLCA1). Mechanistically, Rab7 maintained optimal CLCA1 levels by controlling its lysosomal degradation, a process that was dysregulated during colitis. Overall, our work establishes a role for Rab7 dependent control of CLCA1 secretion required for maintaining mucosal homeostasis.

These programs are the reasons that employees are motivated and are performing beyond expectation. From primary data it was found that employees love the way Jeff allows them to take part in decision making program and to follow innovation ways of working. After the financial performance of the organization for past 5 years was analyzed, only growth is found. It means these programs are motivating employees. Motivating employees are performing better and it is increasing organizational performance.

E-learning has become one of the most debated concepts in today's global education industry. While some speak in favour of e-learning, others criticize e-learning and state that it cannot cultivate better results compared to traditional learning. In today’s COVID situation, e-learning has become necessary for universities around the world, this research has once again focused on this topic to identify whether e-learning can positively influence the performance of students or not. While identifying the same, three factors are selected that are technical competency, e-learning standards, and e-learning infrastructure. After collecting data and information from existing empirical research works, it was found that e-learning can cultivate positive results in terms of student’s performance in different universities and classes. However, it was found that for that positive performance, technical competency must be high and the standard of materials shared through e-learning must be effective enough. In terms of infrastructure, contradictory results are found as one research work stated that it is not vital for improved performance, while the other stated that it is important for better academic performance. Overall, this research identified that e-learning can increase academic performance with the right tools and techniques implemented with it.

Background In kidney transplant recipients (KTRs), observational data have reported conflicting findings about the utility of renal resistive index (RRI) in determining outcomes. This study aimed to synthesize the current literature and determine the prognostic role of RRI in KTRs. Methods The authors conducted a systematic review to assess the role of RRI in predicting death, graft failure, graft function and proteinuria. Of the 934 titles/abstracts reviewed, 26 studies were included. There was significant heterogeneity in RRI measurements and thresholds as well as in analytical methods, and a meta-analysis could not be performed. Results All included studies were observational and included 7049 KTRs. Eight studies analyzed death, of which five reported a significant association with higher RRI. In the remaining three, small sample sizes and lower/multiple RRI thresholds may have limited detection of a statistically significant difference. Three studies investigated all-cause graft failure, and an association with RRI was reported but varied by time of RRI measurement. Three out of five studies that analyzed a composite of patient and graft outcomes reported an association with RRI. Evidence analyzing death-censored graft failure, graft failure (unclear whether death-censored or all-cause), measures of graft function and proteinuria was conflicting. Most studies had a moderate to high risk of bias. Conclusions RRI likely has a prognostic role in predicting patient outcomes, reflecting patient systemic vascular disease burden rather than graft hemodynamics. Since cardiovascular diseases are a major cause of death and graft loss, RRI may be explored as a noninvasive tool to risk-stratify KTRs.

Bacteria-assisted infections on biomaterials used inside a body as an implant/device are one of the major threats to human health. Microbial-resistant coatings on biomaterials can potentially be considered to mitigate the biomaterial-associated infections. Usually biomaterials with leachable antimicrobial coatings, though economically attractive, provide only short-term protection of the surface against bacteria. Therefore, a stable, nonfouling or bactericidal, and biocompatible polymeric coating is highly desirable. In this regard, polymer brushes, defined as polymer chains tethered to a surface by one end, with suitable anti-infective functionality, represent a useful class of stable coatings which are covalently connected to the underlying surface, thus prolonging the infection resistance of the coated surface. Surface-initiated atom transfer radical polymerization (SI-ATRP) is a versatile technique for the generation of polymeric brushes via "grafting from" way. In this review, we have attempted to give a brief overview about the recent developments of surface coatings by infection-resistant polymer brushes synthesized via SI-ATRP and their applications in the biomedical field. On the basis of their charges, these anti-infective brushes can be classified into five different categories such as neutral, cationic, anionic, zwitterionic, and mixed brushes. The working mechanism of each type of brush in repelling (nonfouling/bacteriostatic) and/or killing (bactericidal) the bacteria has also been discussed. A brief summary of their future scope is also highlighted.

In patients with kidney failure, living donor kidney transplantation (LDKT) is the best treatment option; yet, LDKT rates have stagnated in Canada and vary widely across provinces. We aimed to identify barriers and facilitators to LDKT in a high-performing health system.This study was conducted using a qualitative exploratory case study of British Columbia. Data collection, conducted between October 2020 and January 2021, entailed document review and semistructured interviews with key stakeholders, including provincial leadership, care teams and patients. We recruited participants via purposive sampling and snowballing technique. We generated themes using thematic analysis.After analysis of interviews conducted with 22 participants (5 representatives from provincial organizations, 7 health care providers at transplant centres, 8 health care providers from regional units and 2 patients) and document review, we identified the following 5 themes as facilitators to LDKT: a centralized infrastructure, a mandate for timely intervention, an equitable funding model, a commitment to collaboration and cultivating distributed expertise. The relationship between 2 provincial organizations (BC Transplant and BC Renal Agency) was identified as key to enabling the mandate and processes for LDKT. Five barriers were identified that arose from silos between provincial organizations and manifested as inconsistencies in coordinating LDKT along the spectrum of care. These were divided accountability structures, disconnected care processes, missed training opportunities, inequitable access by region and financial burden for donors and recipients.We found strong links between provincial infrastructure and the processes that facilitate or impede timely intervention and referral of patients for LDKT. Our findings have implications for policy-makers and provide opportunities for cross-jurisdictional comparative analyses.

Objectives Intracerebroventricular injection of endothelin-A receptor antagonist BQ123 potentiates opioid analgesia and reverses analgesic tolerance. This study explores whether these effects can be replicated by injecting BQ123 intravenously. Methods Male Swiss-Webster mice were used. Morphine tolerance was induced using 3- or 7-day dosing. Intravenous BQ123 (8 mg/kg) was injected only once on Day 1, 2, 3 or 4 (3-day studies), and on Day 4, 6 or 8 (7-day studies). On Day 4 or 8, respectively, tail-flick and hot-plate latencies were measured following a morphine challenge dose. Key findings Intravenous BQ123 increased the potency and duration of morphine antinociceptive responses. In the 3-day study, the antinociceptive response was unaffected by BQ123 given on Days 1 or 2. BQ123 treatment on Day 3 or 4 (Day 4, BQ123 given 15-min before morphine) significantly potentiated antinociceptive response versus vehicle-treated tolerant mice. In 7-day studies, the antinociceptive response was unaffected by BQ123 given on Day 4. BQ123 given on Day 6 or 8 (Day 8, BQ123 given 15-min before morphine) produced a >100% increase in antinociceptive response versus vehicle-treated tolerant mice for at least 48 h. Conclusions Intravenous administration of BQ123 is effective in potentiating morphine analgesia and restoring antinociceptive response in morphine-tolerant mice.

Physician engagement in leadership leads to better delivery of care to patients and is crucial for the advancement of knowledge, understanding, and wisdom in transplantation. Despite this, many physicians do not think of themselves as leaders and not much is offered in the form of training and education in leadership. Those who want to embark on a path to leadership sometimes do not know how to engage and where to start. This paper proposes 6 potential pathways to consider while embarking on leadership in transplantation. These are clinical innovation, research, education, administration, advocacy, and ethics. The profiles of some of the major leaders in the field of transplantation are highlighted to exemplify them. In addition, some other emerging pathways are presented. These proposed pathways are meant to serve as a guide on where to start but are interdependent. Last, how to choose from these options is described using techniques such as self-reflection, mentorship, peer engagement, and participation in leadership programs.

COVID-19 illness is an evolving disorder, and many extrapulmonary manifestations have been reported. With this report, we are highlighting one of the neuro-ophthalmologic complications of Covid-19. We report a case of 16 years old boy who presented with complaints of rapid loss of vision and retroorbital pain in the Right eye during recovery from Covid -19 infection. Clinically, radiologically, CSF, Blood, and all other investigations suggest Probable Covid -19 Right Eye Optic Neuritis. The patient showed remarkable recovery following steroid therapy. This report aims to add a case to the limited literature available on Optic Neuritis following Covid-19 infection.

This paper is an ecocritical reading of Amitav Ghosh’s Ibis Trilogy which comprises of three historical fictions, Sea of Poppies (2008), River of Smoke (2011) and Flood of Fire (2015). It is a documentary of the opium trade between India and China and the trafficking of people as indentured labors by the East India Company during the mid-nineteenth century. The paper will examine the ecocentric dimensions in the work of Ghosh. Ecocriticism is a rapidly emerging field of literary study that considers the relationship which human beings share with the environment. The main issues that become very prominent in these novels are Ecological Imperialism and Ecocide. These terms are portrayed by Ghosh in his Ibis Trilogy in a very realistic manner with the grand scale of opium war. Opium war is one of the greatest incidents in the colonial history of India. This paper will analyse the issue of environmental degradation as found in the Ibis Trilogy in the light of ecocriticism.

The structural and characteristic features of HIV-1 broadly neutralizing antibodies (bnAbs) from chronically infected pediatric donors are currently unknown. Herein, we characterized a heavy chain matured HIV-1 bnAb 44m, identified from a pediatric elite-neutralizer. Interestingly, in comparison to its wild-type AIIMS-P01 bnAb, 44m exhibited moderately higher level of somatic hypermutations (SHM) of 15.2%. 44m neutralized 79% of HIV-1 heterologous viruses tested, with a geometric mean IC50titer of 0.36 μg/ml. The cryoEM structure of 44m Fab in complex with fully-cleaved glycosylated native-like BG505.SOSIP envelope trimer at 4.4 Å resolution revealed that 44m targets the V3-glycan N332-supersite and GDIR motif to neutralize HIV-1 with improved potency and breadth, plausibly attributed by a matured heavy chain as compared to that of wild-type AIIMS-P01 bnAb. This study improves our understanding on pediatric HIV-1 bnAbs and structural basis of broad HIV-1 neutralization by 44m may be useful blueprint for vaccine design in future.

Performing third or fourth kidney transplantation (3KT and 4KT) in older patients is rare due to surgical and immunologic challenges. We aimed to analyze and compare the outcomes of younger (18-64 years) and older (≥65 years) recipients of 3KT and 4KT. Between 1990 and 2016, we identified 5816 recipients of 3KTs (153 were older) and 886 recipients of 4KTs (18 were older). The incidences of delayed graft function (24.3% vs. 24.8%, p = .89), primary non-function (3.2% vs. 1.3%, p = .21), 1-year acute rejection (18.6% vs. 14.8%, p = .24), and 5-year death censored graft failure (DCGF) (24.8% vs. 17.9%, p = .06) were not different between younger and older recipients of 3KT. Fog computing offers better services to end users by bringing processing, storage, and networking closer to them. Recently, there has been significant research addressing architectural and algorithmic aspects of fog computing. In the existing literature, a systematic study of architectural designs is widely conducted for various applications. Algorithms are seldom examined. Algorithms play a crucial role in fog computing. This survey aims to performing a comparative study of existing algorithms. The study also presents a systematic classification of the current fog computing algorithms and highlights the key challenges and research issues associated with them.
