Measurement-Based Care (MBC) is the use of patient-reported outcome measures repeatedly over the course of treatment to track progress and empower both providers and patients to collaboratively set goals and plan treatment. The Measurement-Based Care in Mental Health Initiative within the Department of Veterans Affairs' Office of Mental Health and Suicide Prevention partnered with the Post Traumatic Stress Disorder (PTSD) Mentoring Program to create an interdisciplinary field-based workgroup. The workgroup included psychologists, clinical social workers, and mental health counselors from PTSD Clinical Teams. The task of the workgroup was to create guidelines for best practice in delivery of MBC in PTSD Clinical Teams given anticipated policy requiring MBC to be used in PTSD Clinical Teams. Framed in the Strategic Action Field Framework for Policy Implementation Research, the current paper evaluates this hybrid top-down and bottom-up process of policy development. Major barriers included difficulty with the workgroup as an authentic bottom-up process, inability to reach the entire field (e.g., focus groups not widely attended by providers), and limited diversity in the workgroup. Facilitators included using consensus to make decisions, support provided to workgroup members by national operations partners, and collaboration and mutual respect among workgroup members. Workgroup members noted an equal, respectful partnership between operations partners and the workgroup; they reported feeling empowered and believed the viewpoints of the field were included in the guidelines. Further, due to the COVID-19 pandemic, the workgroup included more guidelines specific to telehealth into the guidelines. This hybrid model provides a process through which frontline workers can inform policy development and implementation.

The high yielding tenera is the commercial oil palm planting material of choice in Southeast Asia. Notwithstanding this, there is continuous effort to further improve the yield and one way to do this is by addressing the yield components (YCs ). Using 4,451 SNP and over 600 SSR markers , this study revealed quantitative trait loci (QTL) associated with YCs in two breeding populations, a Deli dura x Yangambi pisifera (P2) and a Deli dura x AVROS pisifera (KULIM DxP). Thirteen and 29 QTLs were identified in P2 and KULIM DxP, respectively . They were compared to other YC-linked QTLs reported previously for different genetic backgrounds by mapping the QTL-linked markers to the oil palm genome . The comparison revealedfour common chromosomes containing QTLs influencing various YCs . The results reveal the possible presence of closely linked loci or pleiotropic genes influencing YCs in oil palm. Exploiting the genome data has also facilitated the discovery of candidate genes within or near the QTL regions including those related to glycosylation, fatty acid and oil biosynthesis, and development of flower, seed and fruit .

Background Curcumin is the main active ingredient of the spice turmeric, investigated extensively for putative anticancer properties. Objectives This phase IIa open-labelled randomized controlled trial aimed to assess safety, efficacy, quality of life, neurotoxicity, curcuminoids, and C-X-C-motif chemokine ligand 1 (CXCL1) in patients receiving folinic acid/5-fluorouracil/oxaliplatin chemotherapy (FOLFOX) compared with FOLFOX + 2 g oral curcumin/d (CUFOX). Methods Twenty-eight patients aged >18 y with a histological diagnosis of metastatic colorectal cancer were randomly assigned (1:2) to receive either FOLFOX or CUFOX. Safety was assessed by Common Toxicity Criteria-Adverse Event reporting, and efficacy via progression-free survival (PFS) and overall survival (OS). Quality of life and neurotoxicity were assessed using questionnaires (European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-C30 and Functional Assessment of Cancer Treatment-Gynecologic Oncology Group-Neurotoxicity). Plasma curcuminoids were determined with liquid chromatography (LC) electrospray ionization tandem mass spectrometry and CXCL1 by ELISA. Results Addition of daily oral curcumin to FOLFOX chemotherapy was safe and tolerable (primary outcome). Similar adverse event profiles were observed for both arms. In the intention-to-treat population, the HR for PFS was 0.57 (95% CI: 0.24, 1.36; P = 0.2) (median of 171 and 291 d for FOLFOX and CUFOX, respectively) and for OS was 0.34 (95% CI: 0.14, 0.82; P = 0.02) (median of 200 and 502 d for FOLFOX and CUFOX, respectively). There was no significant difference between arms for quality of life (P = 0.248) or neurotoxicity (P = 0.223). Curcumin glucuronide was detectable at concentrations >1.00 pmol/mL in 15 of 18 patients receiving CUFOX. Curcumin did not significantly alter CXCL1 over time (P = 0.712). Conclusion Curcumin is a safe and tolerable adjunct to FOLFOX chemotherapy in patients with metastatic colorectal cancer. Supercritical water desalination (SCWD) is an alternative zero-liquid discharge desalination technique that can overcome many technical and environmental challenges in common desalination processes. Our recent techno-economic analyses on the process integration and intensification of an SCWD process suggest that SCWD can be an economically competitive zero-liquid discharge desalination technique for highly concentrated brines. In addition to these attractive features, this work explores the possibility of utilizing the SCWD process for the selective recovery of industrially essential materials as co-products. Model brines containing sodium chloride, neodymium chloride, and other sodium-containing salts, are examined as model feeds from 25 to 450 C at 25 MPa. The sodium contents in the effluent were not sensitive to the presence other salts, but that of neodymium was. When sodium acetate was added, water-insoluble precipitates were obtained. The characterization of these deposits using gas pycnometer, FT-IR, SEM/EDS, and XRD indicates that the precipitates mainly contain neodymium hydroxide and some low-concentration impurities. The present study searched for candidate genes in five linkage groups (LGs) - T2, T3, OT4, OT6 and T9 hosting the QTLs associated with iodine value (IV) and fatty acid composition (FAC) in an oil palm interspecific hybrid population. Each of the five LGs was successfully anchored to its corresponding chromosomal segment where, a wider repertoire of candidate genes was identified. This study further revealed a total of 19 candidate genes and four transcription factors involved in biosynthesis of fatty acids, lipids (including triacylglycerol) and acetyl-CoA, glycosylation and degradation of fatty acids. Their possible involvement in regulating the levels of saturation are discussed. In addition, 22 candidate genes located outside the QTL intervals were also identified across the interspecific hybrid genome. A total of 92 SSR markers were developed to tag the presence of these candidate genes and 50 were successfully mapped onto their respective positions on the genome. The data obtained here complements the previous studies, and collectively, these QTL-linked candidate gene markers could help breeders in more precisely selecting palms with the desired FAC.

This research aims to identify the critical challenges associated with restarting manufacturing organizations post-coronavirus disease 2019 (COVID-19). The authors conducted an expert-based survey among various industry leaders of manufacturing organizations to capture a holistic view of business continuity plans and the associated challenges. The selected individuals are responsible for making business continuity policies and plans at their respective organizations. They were asked to reflect on their experience of the present-day challenges in managing business continuity in their organizations. Expert interviews were reflective and provided candid inputs. Consequently, the keywords of the experts' feedback were synthesized by using the mind map qualitative approach, which helps in the visualization of the critical challenges at an abstract level. Further, the interrelation between them and the significance of each critical challenge is evaluated using fuzzy theory with the decision-making trial and evaluation laboratory (DEMATEL) technique. The findings of these evaluations will help to assess the existing policies/practices and to strengthen business continuity plans post-COVID-19. This study is a pioneering work that will help organizations to prepare action plans for kick-starting their broken-down economic engines.

Additional file 6. List of QTL associated with the compactness traits and FAC in the independent and the integrated maps determined via Genstat. For a particular trait (Ex. RL and C16:0), a number of markers are present in the QTL interval. Although almost the same genomic region (determined via position on LG) is linked to the QTL in independent populations, a different marker is at times located closest to the QTL peak, in the integrated map. The original marker identified in the independent populations remains significant.

Naraul Formation of the Larji Group in Kulu district of Himachal Pradesh contains the major strata bound Copper mineralization at many places as observed through old working near Naraul, Danala and Kanda in Sainj Valley of Kulu district exposing the Proterozoic Northwestern Himalayan belt. The same structural belt is also known for Silver mineralisation around Manikaran. At Naraul, a regional stratigraphic and mineralogical framework of the Naraul Formation for mineralogical resource assessment was viewed. The old workings within 4-5 km long and 200-300m wide along the NW- SE trending structural trend cover the main ore potential zones. The ore mineralisation seems to be alike the copper silver deposit of the Revett Formation of Montana and Idaho, USA. The deposit in Naraul needs further more drilling investigation and exploration for actual reserve assessment.&nbsp

The present study was conducted to assess the impact of structured teaching program on knowledge and skill regarding prevention of major complications of immobility in orthopaedic patients among staff nurses at SKIMS Medical College and Hospital, Bemina Srinagar. The association between pre test knowledge and skill score of study subjects with demographic variables was also assessed. Quantitative research approach with one group pre testpost test research design with purposive sampling technique to select study sample of 50 was used.Data was collected by using structured knowledge questionnaire and observational checklist. The result revealed that the structured teaching program was effective in improving the knowledge with the mean difference of 11.46(p=0.03) and skill with the mean difference of 9.20(p=0.001) at 0.05 level of significance. The result also revealed significant association between professional qualification and years of experience with pre test knowledge score and educational qualification withpre test skill score. Findings of the study concluded that structured teaching program was effective.

The expression of the tumor suppressor p53 can influence the bioactivation of, and DNA damage induced by, the environmental carcinogen benzo[a]pyrene, indicating a role for p53 in its cytochrome P450 (CYP)‐mediated biotransformation. The carcinogen 2‐amino‐1‐methyl‐6‐phenylimidazo[4,5‐b]pyridine (PhIP), which is formed during the cooking of food, is also metabolically activated by CYP enzymes, particularly CYP1A2. We investigated the potential role of p53 in PhIP metabolism in vivo by treating Trp53(+/+), Trp53(+/−) and Trp53(−/−) mice with a single oral dose of 50 mg/kg body weight PhIP. N‐(Deoxyguanosin‐8‐yl)‐2‐amino‐1‐methyl‐6‐phenylimidazo[4,5‐b]pyridine (PhIP‐C8‐dG) levels in DNA, measured by liquid chromatography‐tandem mass spectrometry, were significantly lower in liver, colon, forestomach and glandular stomach of Trp53(−/−) mice compared to Trp53(+/+) mice. Lower PhIP‐DNA adduct levels in the livers of Trp53(−/−) mice correlated with lower Cyp1a2 enzyme activity (measured by methoxyresorufin‐O‐demethylase activity) in these animals. Interestingly, PhIP‐DNA adduct levels were significantly higher in kidney and bladder of Trp53(−/−) mice compared to Trp53(+/+) mice, which was accompanied by higher sulfotransferase (Sult) 1a1 protein levels and increased Sult1a1 enzyme activity (measured by 2‐naphthylsulfate formation from 2‐naphthol) in kidneys of these animals. Our study demonstrates a role for p53 in the metabolism of PhIP in vivo, extending previous results on a novel role for p53 in xenobiotic metabolism. Our results also indicate that the impact of p53 on PhIP biotransformation is tissue‐dependent and that in addition to Cyp1a enzymes, Sult1a1 can contribute to PhIP‐DNA adduct formation., What's new? Many environmental carcinogens must be metabolized to form reactive electrophilic species capable of damaging DNA. Using a gene knockout mouse model, the authors show that the impact of p53 on PhIP‐DNA adduct formation is tissue‐dependent and that, in addition to cytochrome P450 enzymes, sulfotransferases contribute to the bioactivation of PhIP.

Background: Leukemia incidence has increased in recent decades among European children, suggesting that early-life environmental exposures play an important role in disease development. Objectives: We investigated the hypothesis that childhood susceptibility may increase as a result of in utero exposure to carcinogens and hormonally acting factors. Using cord blood samples from the NewGeneris cohort, we examined associations between a range of biomarkers of carcinogen exposure and hormonally acting factors with micronuclei (MN) frequency as a proxy measure of cancer risk. Associations with gene expression and genotype were also explored. Methods: DNA and protein adducts, gene expression profiles, circulating hormonally acting factors, and GWAS (genome-wide association study) data were investigated in relation to genomic damage measured by MN frequency in lymphocytes from 623 newborns enrolled between 2006 and 2010 across Europe. Results: Malondialdehyde DNA adducts (M1dG) were associated with increased MN frequency in binucleated lymphocytes (MNBN), and exposure to androgenic, estrogenic, and dioxin-like compounds was associated with MN frequency in mononucleated lymphocytes (MNMONO), although no monotonic exposure–outcome relationship was observed. Lower frequencies of MNBN were associated with a 1-unit increase expression of PDCD11, LATS2, TRIM13, CD28, SMC1A, IL7R, and NIPBL genes. Gene expression was significantly higher in association with the highest versus lowest category of bulky and M1dG–DNA adducts for five and six genes, respectively. Gene expression levels were significantly lower for 11 genes in association with the highest versus lowest category of plasma AR CALUX® (chemically activated luciferase expression for androgens) (8 genes), ERα CALUX® (for estrogens) (2 genes), and DR CALUX® (for dioxins). Several SNPs (single-nucleotide polymorphisms) on chromosome 11 near FOLH1 significantly modified associations between androgen activity and MNBN frequency. Polymorphisms in EPHX1/2 and CYP2E1 were associated with MNBN. Conclusion: We measured in utero exposure to selected environmental carcinogens and circulating hormonally acting factors and detected associations with MN frequency in newborns circulating T lymphocytes. The results highlight mechanisms that may contribute to carcinogen-induced leukemia and require further research. LINEAR SWEEP VOLTAMMETRY OF IRREVERSIBLE CHARGE TRANSFER COUPLED WITH IRREVERSIBLE CATALYTIC REACTION UNDER DIFFUSION-CONVECTION CONTROL : AN INTEGRAL EQUATION APPROACH

Background: Introduction of neoadjuvant chemotherapy (NACT) has dramatically changed the management of locally advanced breast cancer (LABC). However, very few randomized trials of NACT have been carried out specifically in LABC patients in our country. In this retrospective analysis, we presented our experience with NACT in LABC patients. Materials and Methods: Medical records of 148 patients of stage III LABC patients treated with NACT, followed by surgery and radiotherapy from January 2006 to December 2010 were reviewed. Clinical and pathological responses to different chemotherapy regimens were assessed according to World Health Organization criteria. Various factors influencing response to NACT and clinical outcome were identified and analyzed. Results: A total of 90 (60.8%) patients received anthracycline-based chemotherapy and 52 (35.1%) patients received mixed anthracycline and taxane-based chemotherapy.119 patients (80.4%) responded to NACT either in the form of complete or partial response (PR). Complete response was seen in 27 (18.2%) patients and 92 (62.2%) patients showed PR after NACT. Pathological complete response was seen in 24 (16.2%) patients-. At a median follow-up period of 44 months 36 patients (24.3%) developed relapse of which six patients developed locoregional recurrence, while 28 (18.9%) patients developed distant metastasis. Aristolochic acid (AA) causes aristolochic acid nephropathy (AAN), first described in women in Belgium accidently prescribed Aristolochia fangchi in a slimming treatment, and also Balkan endemic nephropathy (BEN), through probable dietary contamination with Aristolochia clematitis seeds. Both nephropathies have a high risk of urothelial cancer, with AA being the causative agent. In tissues of AAN and BEN patients, a distinct DNA adduct, 7-(deoxyadenosin-N-6-yl)-aristolactam I (dA-AAI), has been detected. DNA adducts can be removed through DNA repair, they can result in mutations through erroneous DNA replication or they can cause cell death. The dA-AAI adduct induces AT to TA transversions in the tumor-suppressor TP53 gene in experimental systems, matching TP53 mutations observed in urothelial tumors from AAN cancer cases. Using thin-layer chromatography P-32-postlabeling and mass spectrometric analysis we report the detection of dA-AAI in renal DNA from 11 Belgian AAN patients over 20 years after exposure toAA had ceased. Our results showed that dA-AAI is an established biomarker of AA exposure, and that this biomarker can be demonstrated to be persistent decades after a distinct AA exposure. Further, the persistence of dA-AAI adducts appears to be a critical determinant for the AA mutational fingerprint frequently found in oncogenes and tumor suppressor genes recently identified by whole genome sequencing of AA-associated urothelial tumors. The potential for exposure to AA worldwide is high; the unprecedented long-term persistence of dA-AAI provides a useful long-term biomarker of exposure and attests to the role of AA in human urothelial malignancy. What's new? Aristolochic acid (AA), present in Aristolochia-derived herbal drugs frequently used in traditional Chinese medicine, can cause aristolochic acid nephropathy and urothelial cancer. The true extent of AA exposure is unknown as reliable biomarkers are missing. This study demonstrates that premutagenic AA-DNA adducts, specifically 7-(deoxyadenosin-N6-yl)-aristolactam I adducts, have an exceptionally long-term persistence in the renal DNA of aristolochic acid nephropathy patients and thus can serve as biomarkers to assess exposure to AA, even decades later.
