In its typical form, spinal tuberculosis (TB) presents as destroyed contiguous vertebral bodies with involvement of intervertebral discs and paravertebral or psoas abscesses. Atypical forms are uncommonly reported. Here, we describe 8 patients with noncontiguous multisegmental spinal TB with no intervertebral disc involvement. From 2013 to 2014, we surgically treated 384 patients with spinal TB to relieve spinal cord compression, re-establish spinal stability, confirm the diagnosis, and debride the TB foci. Eight of these patients had noncontiguous multisegmental TB without intervertebral disc involvement. Seven of the 8 patients underwent short-segmental fixation and fusion at a single focus. Appropriate combinations of anti-TB medication were continued until final follow-up. They were followed at established intervals using plain radiography, 3-dimensional computed tomography, and magnetic resonance imaging of the surgical region to evaluate fusion and the condition of the foci. Mean follow-up was 26.6 months (range, 24–32 months), during which time all patients were prescribed the appropriate anti-TB medications. Satisfactory clinical and radiological results were obtained in all patients, without complications. Presentation of noncontiguous multisegmental spinal TB without the involvement of intervertebral disc resembles that of a neoplasm or other spinal infection. Differentiation requires the presence of a combination of general symptoms, laboratory test results, appropriate radiological results, and the physician's experience. For patients in whom surgery is indicated, the patient's general condition should be taken into consideration. Surgical intervention only focus on the responsible level is less invasive and can achieve satisfactory clinical and radiographic outcomes.

3. Management of limb fractures in a teaching hospital: comparison between Wenchuan and Yushu earthquakes

【Abstract】Objective: To comparatively analyze the medical records of patients with limb fractures as well as rescue strategy in Wenchuan and Yushu earthquakes so as to provide references for post-earthquake rescue. Methods: We retrospectively investigated 944 patients sustaining limb fractures, including 891 in Wenchuan earth-quake and 53 in Yushu earthquake, who were admitted to West China Hospital (WCH) of Sichuan University. Results: In Wenchuan earthquake, WCH met its three peaks of limb fracture patients influx, on post-earthquake day (PED) 2, 8 and 14 respectively. Between PED 3-14, 585 patients were transferred from WCH to other hospitals out-side the Sichuan Province. In Yushu earthquake, the maxi-mum influx of limb fracture patients happened on PED 3, and no one was shifted to other hospitals. Both in Wenchuan and Yushu earthquakes, most limb fractures were caused by blunt strike and crush/burying. In Wenchuan earthquake, there were 396 (396/942, 42.0%) open limb fractures, includ-ing 28 Gustilo I, 201 Gustilo II and 167 Gustilo III injuries. But in Yushu earthquake, the incidence of open limb frac-ture was much lower (6/61, 9.8%). The percent of patients with acute complications in Wenchuan earthquake (167/891, 18.7%) was much higher than that in Yushu earthquake (5/53, 3.8%). In Wenchuan earthquake rescue, 1 018 surgeries were done, composed of debridement in 376, internal fixation in 283, external fixation in 119, and vacuum sealing drainage in 117, etc. While among the 64 surgeries in Yushu earthquake rescue, the internal fixation for limb fracture was mostly adopted. All patients received proper treatment and sur-vived except one who died due to multiple organs failure in Wenchuan earthquake. Conclusion: Provision of suitable and sufficient medi-cal care in a catastrophe can only be achieved by construc-tion of sophisticated national disaster medical system, pre-diction of the injury types and number of injuries, and con-firmation of participating hospitals’ exact role. Based on the valuable rescue experiences after Wenchuan earthquake, the rescue was faster, more orderly and effective in Yushu earthquake. Nevertheless, there is still a long way to go in the development of a stronger emergent response to the disasters. Key words: Earthquakes; Extremities; Fractures, bone; Disaster medicine

4. Air Pod Target Location Method Research Using GPS Datum Mark in Vertical Down of View

Traditional UAS targeting technology uses space coordinate transformation theory to locate targets.It requires complicated calibration between INS benchmark,shock absorber base and optical turret coordinate.The accuracy of locating results is low due to the deviation of coordinate calibration and the latitude and longitude error of UAS.Errors brought by shock absorber can be eliminated thoroughly when INS and optical turret solid alliance installed in air pod.Using height value of airplane and laser distance measurement value,optical turret can work out the height of target in vertical down of view.It can also calculate GPS coordinate of target accurately using pixels relationship between target and GPS datum mark combined with the latitude and longitude of datum mark.Simulation test shows an accuracy result of ground target locating method based on GPS datum mark when air pod in vertical down of view status.Comparing with space coordinate transformation locating method,it saves complicated system calibration process,simplifies arithmetic formula,improves the locating accuracy and enhances the real-time feature of solution.

This paper bring forward one new method of target automatic orientation and we establish the target positioning system basing on dual antenna GPS receiver. We analysis the principle of target orientation and make an emulation of the system. The results show us that the method has high precision. Then, we make many times experiments. The results of experiments show us this system has high precision and has real-time characteristic.

7. Treatment for 332 cases of lower leg fracture in '5.12' Wenchuan earthquake

ObjectiveTo retrospectively analyze the medical treatment of 332 patients with lower leg fracture in Wenchuan earthquake admitted in West China Hospital.MethodsFrom May 12, 2008 to June 15, 2008, 332 patients with lower leg fracture injured in Wenchuan earthquake were treated in our hospital. The data on trauma condition and clinical treatment were collected and analyzed.ResultsAmong the 332 cases of lower leg fracture, there were 179 cases of open fracture, accounting for 53.9%, in which 91% belonged to Gustilo II or III injury with serious pollution. Many patients had posttraumatic complications, vascular and nerve injury, wound infection or osteofascial compartment syndrome. After medical treatment, blood vessels were reconnected, wound surface was repaired and wound infection was under control.ConclusionFor the patients with lower leg fracture in earthquake, we followed the principle of “complete debridement-restoring the continuity of bone bracket-timely recovering blood supply of limbs and repairing nerve damage-repair the wound surface at stage I or II” so as to reduce the incidence of amputation and infection.
