Abstract

Vitamin K deficiency is common among kidney transplant recipients (KTRs) and likely contributes to progressive vascular calcification and stiffness. In this single-center, randomized, double-blind, placebo-controlled trial, we aimed to investigate the effects of vitamin K supplementation on the primary end point, serum calcification propensity (calciprotein particle maturation time, T50), and secondary end points arterial stiffness (pulse wave velocity [PWV]) and vitamin K status in 40 vitamin K-deficient KTRs (plasma dephosphorylated uncarboxylated matrix Gla protein [dp-ucMGP] ≥500 pmol/L). Participants (35% female; age, 57 ± 13 years) were randomized 1:1 to vitamin K2 (menaquinone-7, 360 μg/day) or placebo for 12 weeks. Vitamin K supplementation had no effect on calcification propensity (change in T50 vs baseline +2.3 ± 27.4 minutes) compared with placebo (+0.8 ± 34.4 minutes; Pbetween group = .88) but prevented progression of PWV (change vs baseline -0.06 ± 0.26 m/s) compared with placebo (+0.27 ± 0.43 m/s; Pbetween group = .010). Vitamin K supplementation strongly improved vitamin K status (change in dp-ucMGP vs baseline -385 [-631 to -269] pmol/L) compared with placebo (+39 [-188 to +183] pmol/L; Pbetween group < .001), although most patients remained vitamin K-deficient. In conclusion, vitamin K supplementation did not alter serum calcification propensity but prevented progression of arterial stiffness, suggesting that vitamin K has vascular effects independent of calciprotein particles. These results set the stage for longer-term intervention studies with vitamin K supplementation in KTRs.TRIAL REGISTRY: EU Clinical Trials Register (EudraCT Number: 2019-004906-88) and the Dutch Trial Register (NTR number: NL7687).

2. Prehabilitation in Adult Solid Organ Transplant Candidates

Abstract

Purpose of Review To highlight the importance of biological age in the context of prehabilitation and to present relevant research regarding prehabilitation prior to solid organ transplantation. Recent Findings Studies on the effect of prehabilitation have been performed in kidney-, lung-, liver-, and heart transplant patient populations. Prior to kidney transplantation, exercise interventions have been shown to improve cardiopulmonary- and physical fitness and result in a decreased length of hospital stay postoperatively. Among lung transplant candidates, various methods of prehabilitation have been studied including home-based, outpatient and in-patient programs, consisting of physical training, psychological support, education, and nutritional interventions. Overall, prehabilitation seems to improve or maintain quality of life and exercise capacity in this patient population. Patients undergoing liver transplantation seem to benefit from prehabilitation as well. Not only does it seem safe and feasible, but significant improvements in aerobic and functional capacity have also been found. Regarding heart transplant candidates, both inpatient and outpatient, supervised prehabilitation programs show promising results with improvements in exercise capacities and quality of life. Summary Prehabilitation is an effective and safe intervention for improving functional outcomes of solid organ transplant patients. Future studies should evaluate whether prehabilitation translates into improved pre- and post-transplant clinical outcomes.

5. Influencing factors of interprofessional collaboration in multifactorial fall prevention interventions: a qualitative systematic review

Abstract

Background With the ageing population worldwide, falls are becoming a severe and growing health problem. Interprofessional multifactorial fall prevention interventions (FPIs) have effectively prevented falls in community-dwelling older adults. However, the implementation of FPIs often fails due to a lack of interprofessional collaboration. Therefore, gaining insight into the influencing factors of interprofessional collaboration in multifactorial FPI’s for older adults living in the community is essential. Consequently, our aim was to provide an overview of factors influencing interprofessional collaboration in multifactorial FPIs for community-dwelling older adults. Methods This qualitative systematic literature research was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement. Pubmed, CINAHL, and Embase electronic databases have been systematically searched for eligible articles, with a qualitative design. The quality was appraised using the Checklist for Qualitative Research by the Joann Briggs Institute. The findings were inductively synthesized using a meta-aggregative approach. Confidence in the synthesized findings was established using the ConQual methodology. Results Five articles were included. Analysis of the included studies resulted in 31 influencing factors for interprofessional collaboration, which were labelled as findings. These findings were summarized in ten categories and combined into five synthesized findings. Results showed that communication, role clarity, information sharing, organization, and interprofessional aim influence interprofessional collaboration in multifactorial FPIs. Conclusions This review provides a comprehensive summary of findings on interprofessional collaboration, specifically in the context of multifactorial FPIs. Knowledge in this area is considerably relevant given the multifactorial nature of falls, which demands an integrated, multidomain approach, including both health and social care. The results can be utilized as a fundament for developing effective implementation strategies aiming to improve interprofessional collaboration between health and social care professionals working in multifactorial FPIs in the community.

Abstract

OBJECTIVES: A high body mass index (BMI) is associated with an unfavorable disease course in COVID-19, but not among those who require admission to the ICU. This has not been examined across different age groups. We examined whether age modifies the association between BMI and mortality among critically ill COVID-19 patients. DESIGN: An observational cohort study. SETTING: A nationwide registry analysis of critically ill patients with COVID-19 registered in the National Intensive Care Evaluation registry. PATIENTS: We included 15,701 critically ill patients with COVID-19 (10,768 males [68.6%] with median [interquartile range] age 64 yr [55-71 yr]), of whom 1,402 (8.9%) patients were less than 45 years. INTERVENTIONS: None. MEASUREMENTS AND MAIN RESULTS: In the total sample and after adjustment for age, gender, Acute Physiology and Chronic Health Evaluation IV, mechanical ventilation, and use of vasoactive drugs, we found that a BMI greater than or equal to 30 kg/m2does not affect hospital mortality (adjusted odds ratio [ORadj] = 0.98; 95% CI, 0.90-1.06; p = 0.62). For patients less than 45 years old, but not for those greater than or equal to 45 years old, a BMI greater than or equal to 30 kg/m2was associated with a lower hospital mortality (ORadj= 0.59; 95% CI, 0.36-0.96; p = 0.03). CONCLUSIONS: A higher BMI may be favorably associated with a lower mortality among those less than 45 years old. This is in line with the so-called "obesity paradox" that was established for other groups of critically ill patients in broad age ranges. Further research is needed to understand this favorable association in young critically ill patients with COVID-19.

Abstract

Chronic inflammatory disease of the gastrointestinal (GI) tract is defined by several pathophysiological characteristics, such as dysbiosis of the microbiota, epithelial barrier hyperpermeability, systemic dissemination of endotoxins and chronic inflammation. In addition to well-reported environmental factors in non-communicable disease, such as smoking, diet, and exercise, humans are frequently exposed to myriads more environmental factors, from pesticides to food additives. Such factors are ubiquitous across both our diet and indoor/outdoor environments. A major route of human exposure to these factors is ingestion, which frequently occurs due to their intentional addition (intentional food additives) and/or unintentional contamination (unintentional food contaminants) of food products-often linked to environmental pollution. Understanding how this persistent, diverse exposure impacts GI health is of paramount importance, as deterioration of the GI barrier is proposed to be the first step towards systemic inflammation and chronic disease. Therefore, we aim to evaluate the impact of ingestion of environmental factors on inflammatory processes in the GI tract. In this review, we highlight human exposure to intentional food additives (e.g. emulsifiers, bulking agents) and unintentional food contaminants (e.g. persistent organic pollutants, pesticides, microplastics), then present evidence for their association with chronic disease, modification of the GI microbiota, increased permeability of the GI barrier, systemic dissemination of endotoxins, local (and distal) pro-inflammatory signalling, and induction of oxidative stress and/or endoplasmic reticulum stress. We also propose a link to NLRP3-inflammasome activation. These findings highlight the contribution of common environmental factors towards deterioration of GI health and the induction of pathophysiology associated with onset and maintenance of chronic inflammation in the GI tract.

Abstract

Optimal growth and development in childhood and adolescence is crucial for lifelong health and well-being1–6. Here we used data from 2,325 population-based studies, with measurements of height and weight from 71 million participants, to report the height and body-mass index (BMI) of children and adolescents aged 5–19 years on the basis of rural and urban place of residence in 200 countries and territories from 1990 to 2020. In 1990, children and adolescents residing in cities were taller than their rural counterparts in all but a few high-income countries. By 2020, the urban height advantage became smaller in most countries, and in many high-income western countries it reversed into a small urban-based disadvantage. The exception was for boys in most countries in sub-Saharan Africa and in some countries in Oceania, south Asia and the region of central Asia, Middle East and north Africa. In these countries, successive cohorts of boys from rural places either did not gain height or possibly became shorter, and hence fell further behind their urban peers. 11. Contextual determinants influencing the implementation of fall prevention in the community: a scoping review

Abstract

BackgroundSuccessful implementation of multifactorial fall prevention interventions (FPIs) is essential to reduce increasing fall rates in community-dwelling older adults. However, implementation often fails due to the complex context of the community involving multiple stakeholders within and across settings, sectors, and organizations. As there is a need for a better understanding of the occurring context-related challenges, the current scoping review purposes to identify what contextual determinants (i.e., barriers and facilitators) influence the implementation of FPIs in the community.MethodsA scoping review was performed using the Arksey and O'Malley framework. First, electronic databases (Pubmed, CINAHL, SPORTDiscus, PsycINFO) were searched. Studies that identified contextual determinants that influence the implementation of FPIs in the community were included. Second, to both validate the findings from the literature and identify complementary determinants, health and social care professionals were consulted during consensus meetings (CMs) in four districts in the region of Utrecht, the Netherlands. Data were analyzed following a directed qualitative content analysis approach, according to the 39 constructs of the Consolidated Framework for Implementation Research.ResultsFourteen relevant studies were included and 35 health and social care professionals (such as general practitioners, practice nurses, and physical therapists) were consulted during four CMs. Directed qualitative content analysis of the included studies yielded determinants within 35 unique constructs operating as barriers and/or facilitators. The majority of the constructs (n = 21) were identified in both the studies and CMs, such as “networks and communications”, “formally appointed internal implementation leaders”, “available resources” and “patient needs and resources”. The other constructs (n = 14) were identified only in the .DiscussionFindings in this review show that a wide array of contextual determinants are essential in achieving successful implementation of FPIs in the community. However, some determinants are considered important to address, regardless of the context where the implementation occurs. Such as accounting for time constraints and financial limitations, and considering the needs of older adults. Also, broad cross-sector collaboration and coordination are required in multifactorial FPIs. Additional context analysis is always an essential part of implementation efforts, as contexts may differ greatly, requiring a locally tailored approach.

Abstract

Knowledge about how weather conditions affect travel behavior in different user groups and contexts is relevant for planners and policymakers to facilitate sustainable transportation systems. We aimed to assess the influence of day-to-day weather on cycling for transportation among parents of young children with access to different bike types (e-bike vs non e-bike) in a natural study setting over nine months. We hypothesized less impact of weather variability on cycling when using an e-bike compared with a non e-bike. A randomized, controlled trial was conducted in Southern Norway. The intervention group (n = 18) was in random order equipped with an e-bike with trailer for child transportation (n = 6), a cargo (longtail) bike (n = 6) and a traditional bike with trailer (n = 6), each for three months. These 18 participants reported cycling on 832 out of 3276 person-days (25%). We used dynamic structural equation modeling for intensive longitudinal data to examine the relations between daily weather conditions, bike type (e-bike vs traditional bike), and cycling (dichotomized daily at yes or no). Air temperature (positively) and wind speed (negatively) were both credible predictors of cycling, whereas the other predictors (precipitation in the morning (yes or no) and presence of snow (yes or no) were not. We added interaction terms between bike type and weather conditions, but none of the interaction terms had a credible effect on cycling. Thus, the relations between weather conditions and cycling were not moderated by bike type among parents of young children.

Abstract

Wicked societal problems, such as environmental issues and climate change, are complex, networked problems involving many intertwined issues, no optimal solutions, and numerous stakeholders. Cities are problem owners and living labs for finding solutions through design-enabled innovation initiatives. However, to reach collective impact, it is paramount that these initiatives can learn from one another and align efforts through collaborative sensemaking. In the MappingDESIGNSCAPES project, we piloted a participatory collaboration mapping approach for cross-case sensemaking across design-enabled urban innovation initiatives. We used the CommunitySensor methodology for participatory community network mapping and the Kumu online network visualization tool to help representatives of three urban prototype cases share and collectively make sense of their design lessons. In this first of two papers, we describe how we set up the MappingDESIGNSCAPES project as part of the DESIGNSCAPES urban design innovations R&D program; how we created a conceptual model of the collaboration ecosystems around design-enabled urban innovations; and co-created a visual knowledge base centered around the case and cross-case maps grounded in this conceptual model. We end this paper with a discussion of participatory mapping lessons learned. Abstract

Background: COVID-19 patients were often transferred to other intensive care units (ICUs) to prevent that ICUs would reach their maximum capacity. However, transferring ICU patients is not free of risk. We aim to compare the characteristics and outcomes of transferred versus non-transferred COVID-19 ICU patients in the Netherlands. Methods: We included adult COVID-19 patients admitted to Dutch ICUs between March 1, 2020 and July 1, 2021. We compared the patient characteristics and outcomes of non-transferred and transferred patients and used a Directed Acyclic Graph to identify potential confounders in the relationship between transfer and mortality. We used these confounders in a Cox regression model with left truncation at the day of transfer to analyze the effect of transfers on mortality during the 180 days after ICU admission. Results: We included 10,209 patients: 7395 non-transferred and 2814 (27.6%) transferred patients. In both groups, the median age was 64 years. Transferred patients were mostly ventilated at ICU admission (83.7% vs. 56.2%) and included a larger proportion of low-risk patients (70.3% vs. 66.5% with mortality risk

J Strength Cond Res 35(8): 2327-2337, 2021-Youth with chronic diseases or physical disabilities (CDPD) often show reduced fitness and physical activity (PA) levels and participate less in organized sports compared with healthy peers. The purpose of this study was to examine the associations between participation in sports and health-related fitness and PA in youth with CDPD. A total of 163 subjects (mean age 14 years; range 8-19 years) with CDPD were included in this cross-sectional study, with 81 participating in organized sports and 82 not. Subjects were recruited between October 2014 and November 2016. Aerobic and anaerobic fitness, agility, and muscle strength were assessed in the laboratory, whereas PA was monitored in daily life using accelerometry during 1 week. Linear regression analyses were used to assess the associations of sports participation (independent variable) with health-related fitness and PA (dependent variables). Results show that youth with CDPD participating in organized sports 2 times a week performed better on all outcome measures. They reached a higher peak oxygen uptake (difference of 4.9 ml O2·kg-1·min-1, P = 0.001) compared with their peers not participating in sports. Also, anaerobic fitness, agility, muscle strength, and PA were all positively associated with sports participation. Moreover, the association between sports participation and aerobic fitness was mediated by PA for 31% (P = 0.045). In conclusion, participation in sports is associated with both higher levels of PA and health-related fitness in youth with CDPD. Promotion and stimulation of participation in sports seems a good way to promote health-related fitness as well as a healthy active lifestyle in youth with CDPD.

This article responds to dr. Harmen Jansen’s critique of the classic Reformed position on women in office, as maintained by the Gereformeerde Bond in the Protestant Church of the Netherlands. While men and women are created equal in God’s image, and thus share equally in Christ’s salvation, this does not exclude a functional difference in task and responsibility. An appeal to cultural and social patterns should not relativize Scripture’s authoritative teaching. The church welcomes the participation of women and their Spirit-given talents in various ministries, even if the offices of pastor and elder remain closed to them. The dynamics of mutual dedication and love that mirrors the relationship of Christ and his church can only function if the distinct identities of men and women are preserved.

Resume L’annonce du diagnostic de handicap aux parents de jeunes enfants est un moment difficile pour les parents comme pour le medecin, mais le faire est un imperatif autant moral que medical. La methodologie comme la maniere de le faire sont cependant importantes. Un defaut de communication entre les parents et les medecins peut affecter la sante psychologique des parents. Cet article constitue une adaptation et une explication de la maniere d’utiliser le protocole SPIKES pour annoncer la « mauvaise nouvelle » d’un diagnostic de trouble du developpement a des familles d’enfants de moins de douze mois, la paralysie cerebrale servant d’exemple. Dans un deuxieme temps, il decrit et explique l’eventail des reponses des parents a l’annonce de cette mauvaise nouvelle, depuis « attente vigilante » jusqu’a « acceptation ». Le besoin de connaissances des parents va des causes et pronostics aux traitements et leurs resultats. Les questions frequemment posees et les suggestions de reponses sont presentees en utilisant les scenarios cliniques d’enfants diagnostiques recemment.

Abstract

The notion of analogy plays an important role in Steven Duby’s project of theologia. Traditional Reformed theology understands analogy as an “analogy of attribution” based on the creature’s participation in God’s own perfections. Duby’s discussion of analogy addresses its grounds, main forms and variations, and limitations. In response, this article suggests supplementing Duby’s broadly Thomistic explanation with key elements from the Scotist theory of univocal predication. The first benefit of this integration is a clearer balance of apophatic and kataphatic tendencies in the doctrine of God. The second result is a more sophisticated account of the doctrine of divine simplicity, combining Thomas’ emphasis on the common ratio for predicating terms of God and creatures with the Scotist notions of disjunctive properties and distinctio formalis. While speaking about God’s essence by different concepts is necessary because of our limited understanding, it is also grounded in the reality of God Himself.

Abstract

Background SP140 is a bromodomain-containing protein expressed predominantly in immune cells. Genetic polymorphisms and epigenetic modifications in the SP140 locus have been linked to Crohn’s disease (CD), suggesting a role in inflammation. Results We report the development of the first small molecule SP140 inhibitor (GSK761) and utilize this to elucidate SP140 function in macrophages. We show that SP140 is highly expressed in CD mucosal macrophages and in in vitro-generated inflammatory macrophages. SP140 inhibition through GSK761 reduced monocyte-to-inflammatory macrophage differentiation and lipopolysaccharide (LPS)-induced inflammatory activation, while inducing the generation of CD206+ regulatory macrophages that were shown to associate with a therapeutic response to anti-TNF in CD patients. SP140 preferentially occupies transcriptional start sites in inflammatory macrophages, with enrichment at gene loci encoding pro-inflammatory cytokines/chemokines and inflammatory pathways. GSK761 specifically reduces SP140 chromatin binding and thereby expression of SP140-regulated genes. GSK761 inhibits the expression of cytokines, including TNF, by CD14+ macrophages isolated from CD intestinal mucosa. Conclusions This study identifies SP140 as a druggable epigenetic therapeutic target for CD.

Abstract

While the association of vascular calcification with inferior patient outcomes in kidney transplant recipients is well-established, the association with graft outcomes has received less attention. With this dual-centre cohort study, we aimed to determine the clinical impact of recipient pre-transplant aorto-iliac calcification, measured on non-contrast enhanced computed tomography (CT)-imaging within three years prior to transplantation (2005&ndash, 2018). We included 547 patients (61.4% male, age 60 (interquartile range 51&ndash, 68) years), with a median follow-up of 3.1 (1.4&ndash, 5.2) years after transplantation. The aorto-iliac calcification score (CaScore) was inversely associated with one-year estimated-glomerular filtration rate (eGFR) in univariate linear regression analysis (standard &beta, &minus, 3.3 (95% CI &minus, 5.1 to &minus, 1.5, p &lt, 0.0001), but not after adjustment for potential confounders, including donor and recipient age (p = 0.077). In multivariable Cox regression analyses, a high CaScore was associated with overall graft failure (p = 0.004) and death with a functioning graft (p = 0.002), but not with death-censored graft failure and graft function decline. This study demonstrated that pre-transplant aorto-iliac calcification is associated with one-year eGFR in univariate, but not in multivariable linear regression analyses. Moreover, this study underlines that transplantation in patients with a high CaScore does not result in earlier transplant function decline or worse death censored graft survival, although ongoing efforts for the prevention of death with a functioning graft remain essential.

BACKGROUND: Chronic kidney disease mineral and bone disorders (CKD-MBD) and vascular calcification are often seen in kidney transplantation recipients (KTR). This study focused on the bone-vascular axis hypothesis, the pathophysiological mechanisms driving both bone loss and vascular calcification, supported by an association between lower bone mineral density (BMD) and higher risk of vascular calcification. METHODS: KTR referred for a dual-energy X-ray absorptiometry procedure within 6 mo after transplantation were included in a cross-sectional study (2004-2014). Areal BMD was measured at the proximal femur, and abdominal aortic calcification (AAC) was quantified (8-points score) from lateral single-energy images of the lumbar spine. Patients were divided into 3 AAC categories (negative-AAC: AAC 0; low-AAC: AAC 1-3; and high-AAC: AAC 4-8). Multivariable-adjusted multinomial logistic regression models were performed to study the association between BMD and AAC. RESULTS: We included 678 KTR (51 ± 13 y old, 58% males), 366 (54%) had BMD disorders, and 266 (39%) had detectable calcification. Abstract

Religious texts represent a specific rhetorical genre. What does it mean to construct these texts? In order to answer this question we reflect upon our experience of co-authoring a religious text, the Ordinary Catechism (Gewone Catechismus), published in 2019. The findings include insights in collaborative spirituality, a dynamic interaction with tradition, and various levels of abstraction in theologizing. Further, we analyze these findings by using genre theory. It is concluded that religious texts are responses to a rhetorical situation; they are part of genre repertoires that religious communities use to express and to experience the Christian faith.

Abstract

Objective: Vascular calcification contributes to the cause of cardiovascular disease. The calciprotein particle maturation time (T 50 ) in serum, a measure of calcification propensity, has been linked with adverse outcomes in patients with chronic kidney disease, but its role in the general population is unclear. We investigated whether serum T 50 is associated with cardiovascular mortality in a large general population-based cohort. Approach and Results: The relationship between serum T 50 and cardiovascular mortality was studied in 6231 participants of the PREVEND (Prevention of Renal and Vascular End-Stage Disease) cohort. All-cause mortality was the secondary outcome. Mean (±SD) age was 53±12 years, 50% were male, and mean serum T 50 was 329±58 minutes. A shorter serum T 50 is indicative of a higher calcification propensity. Serum T 50 was inversely associated with circulating phosphate, age, estimated glomerular filtration rate, and alcohol consumption, whereas plasma magnesium was positively associated with serum T 50 ( P R 2 =0.281). During median (interquartile range) follow-up for 8.3 (7.8–8.9) years, 364 patients died (5.8%), of whom 95 (26.1%) died from a cardiovascular cause. In multivariable Cox proportional hazard models, each 60 minutes decrease in serum T 50 was independently associated with a higher risk of cardiovascular mortality (fully adjusted hazard ratio [95% CI], 1.22 [1.04–1.36], P =0.021). This association was modified by diabetes mellitus; stratified analysis indicated a more pronounced association in individuals with diabetes mellitus. Conclusions: Serum T 50 is independently associated with an increased risk of cardiovascular mortality in the general population and thus may be an early and potentially modifiable risk marker for cardiovascular mortality.

Abstract

This special issue is dedicated to the retirement of former editor in chief of the Tijdschrift voor Taalbeheersing Ton van Haaften. It presents an investigation of some aspects of the communicative activity type ‘political debate’ from both a discourse and a historical angle. A special focus is put on Dutch parliamentary debate, of which it is shown that its norms and conventions originate from nineteenth century parliamentary discussions on how to conduct such a debate. Ever since, the strategy of attacking politicians personally has both been employed for rhetorical purposes and fiercely criticized. It was also the (late) nineteenth century when a rhetorical style was introduced in politics, i.e. by former pastor and politician Abraham Kuyper, whose famous ‘Maranatha’ speech to his followers is analyzed for its rhetorical features. Today, even more than in Kuyper’s days, politicians claim to speak on behalf of ‘the people’. An inventory of the ways in which populist politician Geert Wilders presents his appeals to the people shows four stylistic features that may have a strategic function.

‘Parasitic Politics’. Thorbecke and Personal Attacks in the Dutch Lower House This contribution uses a famous personal attack in Dutch parliamentary history to discuss ad hominem in the context of the activity type of parliamentary debating. The case is Thorbecke, liberal leader and most prominent defender of pure parliamentary discussion, vs the then Prime Minister Van Hall, in December 1860. Thorbecke rejected Van Hall’s opportunist policies but he also disliked him intensely. His personal feelings transpired in his diatribe against Van Hall’s ‘parasitic politics’. Thorbecke’s adherents applauded what they considered a principled attack, Van Hall’s supporters criticized the vehement personal attack. The attack virtually ended the discussion, and it was the culmination of Thorbecke’s vendetta against Van Hall. It is hard to deny that Thorbecke’s diatribe was a personal attack, since he disregarded the arguments of his opponent and discredited him by attacking his personal credibility, but the interpretation of such a political case will always remain open to discussion.

MATERIALS AND METHODS: Data from the National-Intensive-Care-Evaluation-registry of COVID-19 patients admitted between February 15th and January 1th 2021 and other viral pneumonia patients admitted between January 1st 2017 and January 1st 2020 were used. Patients' characteristics, the unadjusted, and adjusted in-hospital mortality were compared. RESULTS: 6343 COVID-19 and 2256 other viral pneumonia patients from 79 ICUs were included. The COVID-19 patients included more male (71.3 vs 49.8%), had a higher Body-Mass-Index (28.1 vs 25.5), less comorbidities (42.2 vs 72.7%), and a prolonged hospital length of stay (19 vs 9 days). The COVID-19 patients had a significantly higher crude in-hospital mortality rate (Odds ratio (OR) = 1.80), after adjustment for patient characteristics and ICU occupancy rate the OR was respectively 3.62 and 3.58. CONCLUSION: Higher mortality among COVID-19 patients could not be explained by patient characteristics and higher ICU occupancy rates, indicating that COVID-19 is more severe compared to other viral pneumonia. Our findings confirm earlier warnings of a high need of ICU capacity and high mortality rates among relatively healthy COVID-19 patients as this may lead to a higher mental workload for the staff.

BACKGROUND AND OBJECTIVE Bobath therapy, or neurodevelopmental therapy (NDT) is widely practiced despite evidence other interventions are more effective in cerebral palsy (CP). The objective is to determine the efficacy of NDT in children and infants with CP or high risk of CP. METHODS Cumulative Index to Nursing and Allied Health Literature, Cochrane Library, Embase, and Medline were searched through March 2021. Randomized controlled trials comparing NDT with any or no intervention were included. Meta-analysis was conducted with standardized mean differences calculated. Quality was assessed by using Cochrane Risk of Bias tool-2 and certainty by using Grading of Recommendations Assessment, Development, and Evaluation. RESULTS Of 667 records screened, 34 studies (in 35 publications, 1332 participants) met inclusion. Four meta-analyses were conducted assessing motor function. We found no effect between NDT and control (pooled effect size 0.13 [−0.20 to 0.46]), a moderate effect favoring activity-based approaches (0.76 [0.12 to 1.40]) and body function and structures (0.77 [0.19 to 1.35]) over NDT and no effect between higher- and lower-dose NDT (0.32 [−0.11 to 0.75]). A strong recommendation against the use of NDT at any dose was made. Studies were not all Consolidated Standards of Reporting Trials-compliant. NDT versus activity-based comparator had considerable heterogeneity (I2 = 80%) reflecting varied measures. CONCLUSIONS We found that activity-based and body structure and function interventions are more effective than NDT for improving motor function, NDT is no more effective than control, and higher-dose NDT is not more effective than lower-dose. Deimplementation of NDT in CP is required.

Abstract

Increased aquaculture production can improve food and nutrition security and economic growth in developing countries, but comes with environmental risks. In recent years, aquaculture has increased tremendously in Lake Kariba, which is located on the border between Zambia and Zimbabwe. We calculated the carrying capacity of aquaculture production in Lake Kariba using the Beveridge P balance model with data for lake-wide total-P concentrations, river flows and data from aquaculture farms and feed producers in the Lake Kariba area. The maximum permissible P load for aquaculture farms in Lake Kariba is 1.2 × 106 kg P yr-1. Average loss of P to the environment in Lake Kariba through farming is 13.92 kg P t-1 for Nile tilapia Oreochromis niloticus and 92.5 kg P t-1 for Nile crocodile Crocodylus niloticus. Consequently, sustainable aquaculture carrying capacity in Lake Kariba is 86900 t yr-1 for O. niloticus and 13000 t yr-1 for C. niloticus. We expect aquaculture production in Lake Kariba to account for 71% of the total allowable aquaculture P load by 2028. The total-P concentration is expected to increase to 28.5 mg m-3 by 2028 due to growth in aquaculture, and its development should be carefully monitored. In future, we recommend that our predictions should be compared to observed changes in order to validate the model. We also illustrate that companies can greatly improve the carrying capacity of a system by improving the feed conversion ratio, the efficiency of nutrient uptake and the processing of mortalities.

Abstract

Background While [18F]-fluordeoxyglucose ([18F]FDG) uptake is associated with arterial inflammation, [18F]-sodium fluoride ([18F]NaF) is a marker for arterial micro-calcification. We aimed to investigate the prospective correlation between both PET markers over time and whether they are prospectively ([18F]FDG) and retrospectively ([18F]NaF) related to progression of systemic arterial disease in a longitudinal study in patients with type 2 diabetes mellitus (T2DM). Methods Baseline [18F]FDG PET/Low Dose (LD) Computed Tomography (CT) scans of ten patients with early T2DM without cardiovascular history (70% men, median age 63 years) were compared with five-year follow-up [18F]NaF/LDCT scans. Systemic activity was expressed as mean target-to-background ratio (meanTBR) by dividing the maximal standardized uptake value (SUVmax) of ten arteries by SUVmean of the caval vein. CT-assessed macro-calcifications were scored visually and expressed as calcified plaque (CP) score. Arterial stiffness was assessed with carotid-femoral pulse wave velocity (PWV). Five-year changes were expressed absolutely with delta (Δ) and relatively with %change. Results Baseline meanTBR[18F]FDG was strongly correlated with five-year follow-up meanTBR[18F]NaF (r = 0.709, P = .022). meanTBR[18F]NaF correlated positively with ΔCPscore, CPscore at baseline, and follow-up (r = 0.845, P = .002 and r = 0.855, P = .002, respectively), but not with %change in CPscore and PWV. Conclusion This proof-of-concept study demonstrated that systemic arterial inflammation is an important pathogenetic factor in systemic arterial micro-calcification development.

47. Early Diagnosis Implementation, Classification And Intervention For Infants With Cerebral Palsy

Abstract

Background: Early diagnosis of cerebral palsy (CP) is now standard of care although not fully implemented in practice. Irrespective of diagnosis age, parents and clinicians have three fundamental questions: 1) is it CP (diagnosis); 2) what type of CP (classification); and 3) what can be done to optimise outcomes (intervention)? This program of research explores early diagnosis, classification and effective interventions for infants and children with CP. Methods: A series of studies were conducted. Firstly, a literature review outlining the history of CP diagnosis. Implementation of an early diagnosis guideline was evaluated in a prospective observational case series. A second guideline described best practice for communicating a diagnosis. Next, early markers with highest accuracy for classification of motor severity, type and topography of CP were synthesised in a systematic review. Further, best available evidence of interventions was outlined in a systematic review. Lastly, the efficacy of one intervention, neurodevelopmental therapy (NDT), was determined through meta-analyses. Results: Diagnosis: Early diagnosis guidelines can be implemented with CP or high risk of CP diagnosis at an average age of 4.4 months. Guidelines for communicating a diagnosis are available. Classification: Tools with high accuracy to predict motor severity, type and topography are available from term age. Unilateral spastic CP can be predicted from 3-4 months. Further research would increase confidence in early prediction of bilateral spastic, dyskinetic and ataxic CP. Intervention: An increase in interventions for infants and children with CP in the last 10 years was found. Motor outcomes can be improved using motor-training interventions. Meta-analyses revealed no effect between NDT and control and no effect between higher and lower dose NDT. Activity-based approaches were superior to NDT for improving motor outcomes. Conclusion: Early diagnosis can be implemented in real-life settings and should be usual care. Best-evidence tools from term age to support early classification should be used. NDT is not recommended. However, multiple effective interventions for CP are available.

Abstract

Objective: To examine the effect of the intervention implemented in the ToyBox study on changes observed in age and sex specific BMI percentile and investigate the role of perinatal factors, parental perceptions and characteristics on this change. Design: A multicomponent, kindergarten-based, family-involved intervention with a cluster-randomized design. A standardized protocol was used to measure children’s body weight and height. Information was also collected from parents/caregivers via the use of validated questionnaires. Linear mixed effect models with random intercept for country, socioeconomic status and school were used. Setting: Selected preschools within the provinces of Oost-Flanders and West-Flanders (Belgium), Varna (Bulgaria), Bavaria (Germany), Attica (Greece), Mazowieckie (Poland) and Zaragoza (Spain). Participants: A sample of 6, 268 pre-schoolers aged 3.5-5.5 (51.9% boys). Results: There was no intervention effect on the change in children’s BMI percentile. However, parents’ underestimation of their children’s actual weight status, parental overweight and mothers’ pre-pregnancy overweight/obesity were found to be significantly and independently associated with increases in children’s BMI percentile in multivariate modelling. Conclusion: Before or as part of the implementation of any childhood obesity intervention initiative, it is important to assist parents/caregivers to correctly perceive their own and their children’s weight status. Recognition of excessive weight by parents/caregivers can increase their readiness to change and as such facilitate higher adherence to favourable behavioural changes within the family. © 2021 Lippincott Williams and Wilkins.
