Studies investigating the effects of language intervention on the re-organization of language networks in chronic aphasia have resulted in mixed findings, likely related to-among other factors-the language function targeted during treatment. The present study investigated the effects of the type of treatment provided on neural reorganization. Seventy individuals with chronic stroke-induced aphasia, recruited from three research laboratories and meeting criteria for agrammatism, anomia or dysgraphia were assigned to either treatment (N = 51) or control (N = 19) groups. Participants in the treatment group received 12-weeks of language intervention targeting sentence comprehension/production, naming, or spelling. At baseline and post-testing, all participants performed an fMRI story comprehension task, with blocks of auditorily-presented stories alternated with blocks of reversed speech. Participants in the treatment, but not control, group significantly improved in the treated language domain. FMRI region-of-interest (ROI) analyses, conducted within regions that were either active (or homologous to active) regions in a group of 22 healthy participants on the story comprehension task, revealed a significant increase in activation from pre-to post-treatment in right-hemisphere homologues of these regions for participants in the sentence and spelling, but not naming, treatment groups, not predicted by left-hemisphere lesion size. For the sentence (but not the spelling) treatment group, activation changes within right-hemisphere homologues of language regions were positively associated with changes in measures of verb and sentence comprehension. These findings support previous research pointing to recruitment of right hemisphere tissue as a viable route for language recovery and suggest that sentence-level treatment may promote greater neuroplasticity on naturalistic, language comprehension tasks, compared to word-level treatment.

Objective: Vascular risk factors (VRF) are known contributors to brain health in old age, with minority populations disproportionately impacted. The role of duration and severity of exposure to VRF remains unclear. We examined the association between cumulative exposures to VRF (cVRF) from young adulthood to midlife and midlife brain health, while addressing racial differences. Methods: 142 participants (39% black, 42% women) from the Coronary Artery Risk Development in Young Adults study with longitudinal VRF data over 30 years and a brain MRI at Y30 were included. Cumulative blood pressure (cBP), cholesterol, BMI, smoking, and glucose was calculated as the mean of two measures across two consecutive visits, multiplied by the number of years between visits from Y0 to Y30. Multivariate linear regression models including all VRF, demographics, and medications were used to assess the independent association of cVRF with brain MRI outcomes across races. Results: Higher cSBP was associated with lower total brain, gray matter, and hippocampal volumes in blacks, but not in whites (interaction p0.05). Conclusions: Exposure to higher BP across young adulthood is associated with midlife brain structure, possibly irrespective of race. Given higher level of cBP exposure in minorities, our observations should encourage clinicians to be aggressive in addressing hypertension in young adults, especially in young minority populations as a potential target to narrow disparities in brain health.

For people with disordered consciousness (DoC) after traumatic brain injury (TBI), relationships between treatment-induced changes in neural connectivity and neurobehavioral recovery have not been explored. To begin building a body of evidence regarding the unique contributions of treatments to changes in neural network connectivity relative to neurobehavioral recovery, we conducted a pilot study to identify relationships meriting additional examination in future research. To address this objective, we examined previously unpublished neural connectivity data derived from a randomized clinical trial (RCT). We leveraged these data because treatment efficacy, in the RCT, was based on a comparison of a placebo control with a specific intervention, the familiar auditory sensory training (FAST) intervention, consisting of autobiographical auditory-linguistic stimuli. We selected a subgroup of RCT participants with high-quality imaging data (FAST n = 4 and placebo n = 4) to examine treatment-related changes in brain network connectivity and how and if these changes relate to neurobehavioral recovery. To discover promising relationships among the FAST intervention, changes in neural connectivity, and neurobehavioral recovery, we examined 26 brain regions and 19 white matter tracts associated with default mode, salience, attention, and language networks, as well as three neurobehavioral measures. Of the relationships discovered, the systematic filtering process yielded evidence supporting further investigation of the relationship among the FAST intervention, connectivity of the left inferior longitudinal fasciculus, and auditory-language skills. Evidence also suggests that future mechanistic research should focus on examining the possibility that the FAST supports connectivity changes by facilitating redistribution of brain resources. For a patient population with limited treatment options, the reported findings suggest that a simple, yet targeted, passive sensory stimulation treatment may have altered functional and structural connectivity. If replicated in future research, then these findings provide the foundation for characterizing the unique contributions of the FAST intervention and could inform development of new treatment strategies. For persons with severely damaged brain networks, this report represents a first step toward advancing understanding of the unique contributions of treatments to changing brain network connectivity and how these changes relate to neurobehavioral recovery for persons with DoC after TBI. Transcranial direct current stimulation (tDCS) can influence performance on behavioral tasks and improve symptoms of brain conditions. Yet, it remains unclear precisely how tDCS affects brain function and connectivity. Here, we measured changes in functional connectivity (FC) metrics in blood-oxygenation-level-dependent (BOLD) fMRI data acquired during MR-compatible tDCS in a whole-brain analysis with corrections for false discovery rate. Volunteers (n = 64) received active tDCS, sham tDCS, and rest (no stimulation), using one of three previously established electrode tDCS montages targeting left dorsolateral prefrontal cortex (DLPFC, n = 37), lateral temporoparietal area (LTA, n = 16), or superior temporal cortex (STC, n = 11). In brain networks where simulated E field was highest in each montage, connectivity with remote nodes decreased during active tDCS. During active DLPFC-tDCS, connectivity decreased between a fronto-parietal network and subgenual ACC, while during LTA-tDCS connectivity decreased between an auditory-somatomotor network and frontal operculum. Active DLPFC-tDCS was also associated with increased connectivity within an orbitofrontal network overlapping subgenual ACC. Irrespective of montage, FC metrics increased in sensorimotor and attention regions during both active and sham tDCS, which may reflect the cognitive-perceptual demands of tDCS. Taken together, these results indicate that tDCS may have both intended and unintended effects on ongoing brain activity, stressing the importance of including sham, stimulation-absent, and active comparators in basic science and clinical trials of tDCS.

Urologic chronic pelvic pain syndrome (UCPPS) may be defined to include interstitial cystitis/bladder pain syndrome (IC/BPS) and chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). The hallmark symptom of UCPPS is chronic pain in the pelvis, urogenital floor, or external genitalia often accompanied by lower urinary tract symptoms. Despite numerous past basic and clinical research studies there is no broadly identifiable organ-specific pathology or understanding of etiology or risk factors for UCPPS, and diagnosis relies primarily on patient reported symptoms. In addition, there are no generally effective therapies. Recent findings have, however, revealed associations between UCPPS and “centralized” chronic pain disorders, suggesting UCPPS may represent a local manifestation of more widespread pathology in some patients. Here, we describe a new and novel effort initiated by the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) of the U.S. National Institutes of Health (NIH) to address the many long standing questions regarding UCPPS, the Multidisciplinary Approach to the Study of Chronic Pelvic Pain (MAPP) Research Network. The MAPP Network approaches UCPPS in a systemic manner, in which the interplay between the genitourinary system and other physiological systems is emphasized. The network’s study design expands beyond previous research, which has primarily focused on urologic organs and tissues, to utilize integrated approaches to define patient phenotypes, identify clinically-relevant subgroups, and better understand treated natural history and pathophysiology. Thus, the MAPP Network provides an unprecedented, multi-layered characterization of UCPPS. Knowledge gained is expected to provide important insights into underlying pathophysiology, a foundation for better segmenting patients for future clinical trials, and ultimately translation into improved clinical management. In addition, the MAPP Network’s integrated multi-disciplinary research approach may serve as a model for studies of urologic and non-urologic disorders that have proven refractory to past basic and clinical study. Functional MR (fMR) imaging offers unprecedented spatial and temporal resolution of brain cognition. The impact of this technology on neuroscience research is just now being explored. This article discusses blood oxygenation level-dependent-based fMR imaging. Topics include the origin of the MR-based signal, data acquisition and sequence dependence, and paradigm design issues--issues that need to be fully understood to properly design and interpret the fMR imaging experiment.

PURPOSE: To determine the importance of obtaining precontrast T1-weighted magnetization transfer (MT) MR images for better interpretation contrast-enhanced T1-weighted MT images. METHODS: One hundred fifty-five patients referred for MR imaging of the brain were examined prospectively with noncontrast T1-weighted imaging, noncontrast T1-weighted imaging with MT, contrast-enhanced T1-weighted imaging, and contrast-enhanced T1-weighted imaging with MT. In the patients who had abnormally increased signal intensity on postcontrast images (with or without MT), the four imaging sequences were evaluated with regard to number of lesions and lesional signal intensity. For each of the sequences, two experienced neuroradiologists subjectively graded the lesions on a scale of 1 to 4 (4 being the most conspicuous) with regard to abnormally increased signal intensity. RESULTS: Twenty-two of the 155 patients had increased signal intensity on one or more of the postcontrast sequences. Eight of these 22 patients had increased signal intensity of one or more lesions on images without MT. All these lesions were seen better on images obtained with MT. An additional six of the 22 patients had increased signal intensity of one or more lesions on images obtained with MT that was not detected on images obtained without MT. Eight of the 22 patients had no high signal intensity on noncontrast images with or without MT. One of the eight had increased number and conspicuity of lesions on postcontrast MT images. CONCLUSIONS: A significant number of patients had increased signal intensity on noncontrast T1-weighted images with MT that was not seen on noncontrast T1-weighted images without MT. This high signal intensity was also visible on postcontrast MT images, and would have been mistaken for pathologic enhancement if noncontrast MT images had not been available for comparison.
