Abstract

Objective: In order to agree on the fundamental aspects related to the management of pregnancy in patients with systemic lupus erythematosus and antiphospholipid syndrome, the Spanish Societies of Gynaecology and Obstetrics, Internal Medicine and Rheumatology have set up a working group for the preparation of three consensus documents. Methods: Each of the Scientific Societies involved proposed five representatives based on their experience in the field of pregnancy control in patients with autoimmune diseases. The recommendations were developed following the Delphi methodology. Results: This third document contains the recommendations regarding the management of delivery, puerperium and lactation, including medication use during these periods and the care of the newborn. In addition, a section on contraception is included. Conclusions: These multidisciplinary recommendations are considered decision-making tools for clinicians involved in the care of patients with systemic lupus erythematosus/antiphospholipid syndrome during pregnancy. Abstract

OBJECTIVE: In order to agree on the fundamental aspects related to the management of pregnancy in patients with systemic lupus erythematosus (SLE) and antiphospholipid syndrome (APS), the Spanish Societies of Gynaecology and Obstetrics, Internal Medicine and Rheumatology set up a working group for the preparation of three consensus documents. METHODS: Each of the Scientific Societies involved proposed five representatives based on their experience in the field of pregnancy control in patients with autoimmune diseases. The recommendations were developed following the Delphi methodology. RESULTS: This second document contains the recommendations regarding the management of pregnancy in women with SLE and APS, including complications such as lupus activity, congenital heart block, thrombotic and obstetric manifestations of APS and placental vascular disease. CONCLUSIONS: These multidisciplinary recommendations are considered decision-making tools for clinicians involved in the care of patients with SLE/APS during pregnancy.

Abstract

The potentialities of In-Orbit Servicing (IOS) to extend the operational life of satellites and the need to implement Active Debris Removal (ADR) to effectively tackle the space debris problem are well known among the space community. Research on technical solutions to enable this class of missions is thriving, also pushed by the development of new generation sensors and control systems. Among private companies, space agencies and universities, the European Space Agency (ESA) has been developing technologies in this field for decades. Several solutions have been proposed over the years to safely capture orbital objects, the majority relying on robotic systems. A promising option is the employment of an autonomous spacecraft (chaser) equipped with a highly dexterous robotic arm able to perform the berthing with a resident space object. This operation poses complex technical challenges both during the approach phase and after contact. In this respect, the design of an effective, reliable, and robust Guidance, Navigation and Control (GNC) system, for which several algorithmic architectures and hardware configurations are possible, plays a key role to ensure safe mission execution. This work presents the outcomes of a research activity performed by a consortium of universities under contract with ESA with the goal to develop the navigation and control subsystems of a GNC system for controlling a chaser equipped with a redundant manipulator. Both the final approach until capture and the target stabilization phase after capture are considered in the study. The proposed solution aims at the implementation of a combined control strategy. Robust control methods are adopted to design control laws for the uncertain, nonlinear dynamics of the chaser and of the complete chaser–target stack after capture. Visual–based solutions, i.e., relying on active/passive electro– optical sensors, are selected for relative navigation. A complete sensor suite for relative and absolute navigation is part of the GNC system, including transducers for robot joint measurements. To properly validate the proposed solutions, a complete numerical simulator has been developed. This software tool allows to thoroughly assess the system performance, accounting for all the relevant external disturbances and error sources. A realistic synthetic image generator is also used for relative navigation performance assessment. This paper presents the design solutions and the results of preliminary numerical testing, considering three mission scenarios to prove the flexibility of the solution and its applicability to a wide range of operational cases.

Abstract

Background Systemic lupus erythematosus (SLE) is regarded as a prototype autoimmune disease because it can serve as a means for studying differences between ethnic minorities and sex. Traditionally, all Hispanics have been bracketed within the same ethnic group, but there are differences between Hispanics from Spain and those from Latin America, not to mention other Spanish-speaking populations. Objectives This study aimed to determine the demographic and clinical characteristics, severity, activity, damage, mortality and co-morbidity of SLE in Hispanics belonging to the two ethnic groups resident in Spain, and to identify any differences. Methods This was an observational, multi-centre, retrospective study. The demographic and clinical variables of patients with SLE from 45 rheumatology units were collected. The study was conducted in accordance with Good Clinical Practice guidelines. Hispanic patients from the registry were divided into two groups: Spaniards or European Caucasians (EC) and Latin American mestizos (LAM). Comparative univariate and multivariate statistical analyses were carried out. Results A total of 3490 SLE patients were included, 90% of whom were female; 3305 (92%) EC and 185 (5%) LAM. LAM patients experienced their first lupus symptoms four years earlier than EC patients and were diagnosed and included in the registry younger, and their SLE was of a shorter duration. The time in months from the first SLE symptoms to diagnosis was longer in EC patients, as were the follow-up periods. LAM patients exhibited higher prevalence rates of myositis, haemolytic anaemia and nephritis, but there were no differences in histological type or serositis. Anti-Sm, anti-Ro and anti-RNP antibodies were more frequently found in LAM patients. LAM patients also had higher levels of disease activity, severity and hospital admissions. However, there were no differences in damage index, mortality or co-morbidity index. In the multivariate analysis, after adjusting for confounders, in several models the odds ratio (95% confidence interval) for a Katz severity index >3 in LAM patients was 1.45 (1.038–2.026; p = 0.02). This difference did not extend to activity levels (i.e. SLEDAI >3; 0.98 (0.30–1.66)). Conclusion SLE in Hispanic EC patients showed clinical differences compared to Hispanic LAM patients. The latter more frequently suffered nephritis and higher severity indices. This study shows that where lupus is concerned, not all Hispanics are equal.

7. Health-related quality of life in patients with systemic lupus erythematosus: a Spanish study based on patient reports

Abstract

INTRODUCTION AND OBJECTIVES: Systemic lupus erythematosus (SLE) is a disease that significantly affects the quality of life and welfare of patients. SLE patients can be classified into multimorbidity levels using Clinical Risk Groups (CRGs) to help to incorporate predictive models of health needs. The goal of this study was to correlate CRGs with health-related quality of life (HR-QoL) and costs in SLE patients. METHODS: A questionnaire was administered to SLE patients in four hospital centers of the Valencian Community (Spain) between October 2015 and March 2016. The factors studied included HR-QoL (EQ-5D-5L and VAS), disease activity (SLAI/SELENA), damage (SLICC/ACR), and severity (IGK). RESULTS: The patients (N = 190, 92.06% female, age (mean ± SD) 47.23 ± 13.43 years) were sorted according to health status in nine CRGs. We found that most SLE patients (> 70%) were in CRGs 5 and 6. The main HR-QoL issues in these patients were related to mobility, ability to perform usual activities, and pain/discomfort. The scores (mean ± SD) for EQ-5D-5L and VAS were 0.74 ± 0.25 and 65.67 ± 23.52, respectively. We found that the age of the patients negatively affected their HR-QoL (r = - 0.266). SLE direct costs per patient increased with each CRG group, representing 71.92% of the total costs, while indirect costs were highly variable. The average cost per patient with SLE amounted to €8432.85 (year 2014). CONCLUSIONS: Patients' quality of life is related with age, disease activity, damage, and severity. Age was the parameter which most affects HR-QoL. Most costs of SLE are concentrated in two CRGs in which the HR-QoL deteriorates sharply.

Abstract

Background Although chronic kidney disease (CKD) and age are major risk factors for cardiovascular disease (CVD), little is known about the relative proportions of atheromatous and non-atheromatous CVD by age in CKD patients. Methods We used baseline data from the French Chronic Kidney Disease-Renal Epidemiology and Information Network (CKD-REIN) cohort of 3033 patients (65% men) with CKD Stages 3–4 to study crude and adjusted associations between age, the estimated glomerular filtration rate (eGFR), atheromatous CVD (coronary artery disease, peripheral artery disease and stroke) and non-atheromatous CVD (heart failure, cardiac arrhythmia and valvular heart disease). Results Mean age was 66.8 and mean Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) eGFR was 32.9 mL/min/1.73 m2. In the Conclusions In this large cohort of CKD patients, both atheromatous and non-atheromatous CVD were highly prevalent and more frequent in older patients. In a given age group, the prevalence of atheromatous and non-atheromatous CVD was similar (except for a greater prevalence of non-atheromatous CVD after 85).

Abstract

Introduction: Evidence has shown that utilization of antiosteoporotic medications does not correspond with risk, and studies on other therapies have shown that adequacy of pharmaceutical prescribing might vary between regions. Nevertheless, very few studies have addressed the variability in osteoporotic drug consumption. We aimed to describe variations in pharmaceutical utilization and spending on osteoporotic drugs between Health Areas (HA) in Spain. Methods: Population-based cross-sectional ecological study of expenditure and utilization of the five therapeutic groups marketed for osteoporosis treatment in Spain in 2009. Small area variation analysis (SAVA) methods were used. The units of analysis were the 168 HA of 13 Spanish regions, including 7.2 million women aged 50 years and older. The main outcomes were the defined daily dose (DDD) per 1000 inhabitants and day (DDD/1000/Day) dispensed according to the pharmaceutical claims reimbursed, and the expenditure on antiosteoporotics at retail price per woman =50 years old and per year. Results: The average osteoporosis drug consumption was 116.8 DDD/1000W/Day, ranging from 78.5 to 158.7 DDD/1000W/Day between the HAs in the 5th and 95th percentiles. Seventy-five percent of the antiosteoporotics consumed was bisphosphonates, followed by raloxifene, strontium ranelate, calcitonins, and parathyroid hormones including teriparatide. Regarding variability by therapeutic groups, biphosphonates showed the lowest variation, while calcitonins and parathyroid hormones showed the highest variation. The annual expenditure on antiosteoporotics was €426.5 million, translating into an expenditure of €59.2 for each woman =50 years old and varying between €38.1 and €83.3 between HAs in the 5th and 95th percentiles. Biphosphonates, despite accounting for 79% of utilization, only represented 63% of total expenditure, while parathyroid hormones with only 1.6% of utilization accounted for 15% of the pharmaceutical spending. Conclusion: This study highlights a marked geographical variation in the prescription of antiosteoporotics, being more pronounced in the case of costly drugs such as parathyroid hormones. The differences in rates of prescribing explained almost all of the variance in drug spending, suggesting that the difference in prescription volume between territories, and not the price of the drugs, is the main source of variation in this setting. Data on geographical variation of prescription can help guide policy proposals for targeting areas with inadequate antiosteoporotic drug use.

Abstract

Objective: Interstitial lung disease (ILD) is one of the most serious complications of rheumatoid arthritis (RA). In the present study, we aimed to assess the efficacy of abatacept (ABA) in patients with ILD associated to RA. Methods: National multicenter, non-controlled, open-label registry study of RA patients with ILD treated with ABA. Results: 63 patients (36 women) with RA-associated ILD undergoing ABA therapy were studied. The mean +/- standard deviation age at the time of the study was 63.2 +/- 9.8 years. The median duration of RA and ILD from diagnosis were 6.8 and 1 year, respectively. RA was seropositive in 55 patients (87.3%). In 15 (23.8%) of 63 patients the development of ILD was closely related to the administration of synthetic or biologic disease modifying anti-rheumatic drugs. After a follow-up of 9.4 +/- 3.2 months, two-thirds of patients remained stable whereas one-quarter experienced improvement in the Modified Medical Research Council scale. At that time forced vital capacity remained stable in almost two-thirds of patents and improved in one out of five patients assessed. Also, diffusing capacity of the lung for carbon monoxide remained stable in almost two-thirds and showed improvement in a quarter of the patients assessed. At 12 months, 50% of the 22 patients in whom chest HRCT scan was performed due persistence of respiratory symptoms showed stabilization, 8 (36.4%) improvement and 3 worsening of the HRCT scan pattern. Eleven of 63 patients had to discontinue ABA, mainly due to adverse events. Conclusion: ABA appears to be an effective in RA-associated ILD. (C) 2018 Elsevier Inc. All rights reserved.

Abstract

Disorders of mineral metabolism, including secondary hyperparathyroidism, are thought to contribute to extraskeletal (including vascular) calcification among patients with chronic kidney disease. It has been hypothesized that treatment with the calcimimetic agent cinacalcet might reduce the risk of death or nonfatal cardiovascular events in such patients.In this clinical trial, we randomly assigned 3883 patients with moderate-to-severe secondary hyperparathyroidism (median level of intact parathyroid hormone, 693 pg per milliliter [10th to 90th percentile, 363 to 1694]) who were undergoing hemodialysis to receive either cinacalcet or placebo. All patients were eligible to receive conventional therapy, including phosphate binders, vitamin D sterols, or both. The patients were followed for up to 64 months. The primary composite end point was the time until death, myocardial infarction, hospitalization for unstable angina, heart failure, or a peripheral vascular event. The primary analysis was performed on the basis of the intention-to-treat principle.The median duration of study-drug exposure was 21.2 months in the cinacalcet group, versus 17.5 months in the placebo group. The primary composite end point was reached in 938 of 1948 patients (48.2%) in the cinacalcet group and 952 of 1935 patients (49.2%) in the placebo group (relative hazard in the cinacalcet group vs. the placebo group, 0.93; 95% confidence interval, 0.85 to 1.02; P=0.11). Hypocalcemia and gastrointestinal adverse events were significantly more frequent in patients receiving cinacalcet.In an unadjusted intention-to-treat analysis, cinacalcet did not significantly reduce the risk of death or major cardiovascular events in patients with moderate-to-severe secondary hyperparathyroidism who were undergoing dialysis. Abstract

Background— Patients with kidney disease have disordered bone and mineral metabolism, including elevated serum concentrations of fibroblast growth factor-23 (FGF23). These elevated concentrations are associated with cardiovascular and all-cause mortality. The objective was to determine the effects of the calcimimetic cinacalcet (versus placebo) on reducing serum FGF23 and whether changes in FGF23 are associated with death and cardiovascular events. Methods and Results— This was a secondary analysis of a randomized clinical trial comparing cinacalcet to placebo in addition to conventional therapy (phosphate binders/vitamin D) in patients receiving hemodialysis with secondary hyperparathyroidism (intact parathyroid hormone ≥300 pg/mL). The primary study end point was time to death or a first nonfatal cardiovascular event (myocardial infarction, hospitalization for angina, heart failure, or a peripheral vascular event). This analysis included 2985 patients (77% of randomized) with serum samples at baseline and 2602 patients (67%) with samples at both baseline and week 20. The results demonstrated that a significantly larger proportion of patients randomized to cinacalcet had ≥30% (68% versus 28%) reductions in FGF23. Among patients randomized to cinacalcet, a ≥30% reduction in FGF23 between baseline and week 20 was associated with a nominally significant reduction in the primary composite end point (relative hazard, 0.82; 95% confidence interval, 0.69–0.98), cardiovascular mortality (relative hazard, 0.66; 95% confidence interval, 0.50–0.87), sudden cardiac death (relative hazard, 0.57; 95% confidence interval, 0.37–0.86), and heart failure (relative hazard, 0.69; 95% confidence interval, 0.48–0.99). Conclusions— Treatment with cinacalcet significantly lowers serum FGF23. Treatment-induced reductions in serum FGF23 are associated with lower rates of cardiovascular death and major cardiovascular events. Clinical Trial Registration— URL: http://www.clinicaltrials.gov . 16. Centre characteristics determine ambulatory care and referrals in patients with spondyloarthritis

Abstract

To describe the variability in rheumatology visits and referrals to other medical specialties of patients with spondyloarthritis (SpA) and to explore factors that may influence such variability. Nation-wide cross-sectional study performed in 2009-2010. Randomly selected records of patients with a diagnosis of SpA and at least one visit to a rheumatology unit within the previous 2 years were audited. The rates of rheumatology visits and of referrals to other medical specialties were estimated-total and between centres-in the study period. Multilevel regression was used to analyse factors associated with variability and to adjust for clinical and patient characteristics. 1168 patients' records (45 centres) were reviewed, mainly ankylosing spondylitis (55.2 %) and psoriatic arthritis (22.2 %). The patients had incurred in 5908 visits to rheumatology clinics (rate 254 per 100 patient-years), 4307 visits to other medical specialties (19.6 % were referrals from rheumatology), and 775 visits to specialised nurse clinics. An adjusted variability in frequenting rheumatology clinics of 15.7 % between centres was observed. This was partially explained by the number of faculties and trainees. The adjusted intercentre variability for referrals to other specialties was 12.3 %, and it was associated with urban settings, number of procedures, and existence of SpA dedicated clinics; the probability of a patient with SpA of being referred to other specialist may increase up to 25 % depending on the treating centre. Frequenting rheumatology clinics and referrals to other specialists significantly varies between centres, after adjustment by patient characteristics.

The Z₂s-additive and Z₂Z₄-additive codes are subgroups of Z₂s^n and Z₂^α × Z₄^β, respectively. Both families can be seen as generalizations of linear codes over Z₂ and Z₄. A Z₂s-linear (resp. Z₂Z₄-linear) Hadamard code is a binary Hadamard code which is the Gray map image of a Z₂s-additive (resp. Z₂Z₄-additive) code. It is known that there are exactly ⌊(t−1)/2⌋ and ⌊t/2⌋ nonequivalent Z₂Z₄-linear Hadamard codes of length 2ᵗ, with α=0 and α≠0, respectively, for all t≥3. In this paper, new Z₂s-linear Hadamard codes are constructed for s>2, which are not equivalent to any Z₂Z₄-linear Hadamard code. Moreover, for each s>2, it is claimed that the new constructed nonlinear Z₂-linear Hadamard codes of length 2ᵗ are pairwise nonequivalent.

18. Centre-related variability in hospital admissions of patients with spondyloarthritis

Abstract

The aim of this study was to explore the variability in hospital admissions of patients with spondyloarthritis (SpA) in Spain, and the centre factors that may influence that variability. Descriptive cross-sectional study, part of the emAR II study, performed in Spain (2009-2010). Health records of patients with a diagnosis of SpA and at least one visit to the rheumatology units within the previous 2 years were reviewed. Variables related to hospital admissions, to the SpA, and to the patient and centre were collected. A multilevel logistic regression analysis of random intercept with non-random slopes was performed to assess variability between centres. From 45 centres, 1168 patients' health records were reviewed. Main SpA forms were ankylosing spondylitis (55.2 %) and psoriatic arthritis (22.2 %). A total of 248 admissions were registered for 196 patients (19.2 %, n = 1020). An adjusted variability of 17.6 % in hospitalizations between centres was noted. The following hospital-related factors showed a significant association with admissions: the total number of admissions of the centre, the existence of electronic admission, and the availability of ultrasound in rheumatology. However, these factors only explained 42.9 % of the inter-centre variability. The risk of a patient with SpA of being admitted could double (median OR 2.09), depending on the hospital where the patient was being managed. Hospital admissions of patients with SpA varied between hospitals due to centre characteristics. Further studies are needed to ascertain which specific factors may be causing the variation, as studied variables explained less than half of the variability.

Abstract

Metastatic arthropathy has been rarely reported as a manifestation of solid tumors. It is characterized by a monoarticular involvement and is frequently the first feature of the malignant process; therefore, it may pose a diagnostic challenge. We report two new cases of metastatic carcinomatous arthritis: a patient with right-knee monoarthritis as a presenting feature of pancreatic carcinoma and another one with right-elbow monoarthritis, preceded by constitutional and pulmonary symptoms, secondary to metastasis of undifferentiated carcinoma. We also review the literature for well documented cases of metastatic arthropathy secondary to solid tumors. Metastatic arthropathy indicates a poor prognosis because of a wide spread of the tumor when it is diagnosed. We emphasize the need for increasing awareness of cancer as a cause of monoarthritis.

Se examina la importancia que puede tener la contaminación del pasto con suelo en algunas zonas de Andalucía Occidental, especialmente en una pradera halófita de la Marisma del Guadalquivir, donde el pasto puede alcanzar con facilidad niveles de Al, Fe y Ti superiores a 3.000 ppm., 1.500 ppm. y 100 ppm., respectivamente. Se trata de un aspecto muy interesante, puesto que en algunas zonas de la Marisma pueden acumularse cantidades apreciables de metales pesados y, además, éstos pueden retener enérgicamente cierto tipo de pesticidas. Por consiguiente, en zonas contaminadas, un animal puede consumir cantidades notables de estas substancias a través del suelo que se adhiere a las plantas.

In this work it is proposed the usage of montmorillonile for zinc and copper ion removal from electrochemicai effluents. The initial concentrations of zinc etfluents analyzed in this work, varied between 1O and 94 ppm in Zn2+, being the pH between values 6-7. The obtained percentage of removal with calcic and sodic montmorillonite surpassed in all the cases 98%. The initial concenlrations of copper effluents oscillated between 16 and 160 ppm. in Cu2•. In the treatment of these effluents with calcic montmorillonite an elimination between 98.5 and 85.5% was reached al pH values between 4-5. In both cases it was used a relation of 5 grams ot clay 1200 milliliters of effluent for one hour agitation ., En este trabajo se propone la aplicación de la montmorillonita para la remoción de iones cinc y cobre presentes en efluentes electroquimicos. Las concentraciones iniciales de los efluentes de cinc con los cuales se trabajó, variaron entre 1O y 94 ppm. en Zn2- , encontrándose los valores de pH entre 6-7. Los porcentajes de remoción obtenidos con montmorillonita cálcica y sódica superaron en todos los casos el 98%. Las concentraciones iniciales de los efluentes de cobre oscilaron entre 16 y 160 ppm. en Cu2•. En el tratamiento de estos efluentes con montmorillonita de tipo cálcica se logró una eliminación entre el 98,5 y 85,5%, a valores de pH regulados entre 4-5. En ambos casos se utilizó una relación de 5 gramos de arcilla /200 mL de efluente con una agitación de una hora.

Abstract

We report a 58 years old male, presenting with malaise, weight loss and jaundice. An abdominal ultrasound showed multiple lymphadenopathies in the hepatic hilus and around the pancreas. Fine needle aspiration of these nodes demonstrated an undifferentiated carcinoma. Prostate specific antigen was over 100 ng/ml and a prostate biopsy demonstrated a high grade carcinoma. The patient was subjected to an orchiectomy and hormone therapy (flutamide). Jaundice subsided and he is well after 3 years of follow up and maintained hormone therapy.

El diagnóstico de la parvovirosis porcina sólo puede realizarse de una manera fiable con la ayuda del laboratorio, dadas sus escasas manifestaciones clínicas que incluso pueden ser análogas a las causadas por otras infecciones, como por ejemplo el síndrome porcino respiratorio y reproductor (PRRS). Existen numerosos procedimientos, tanto para la detección de virus como de anticuerpos. Para detectar virus se ha utilizado clásicamente la hemaglutinación (HA), un procedimiento sencillo y rápido pero con problemas a la hora de reproducir los resultados. Para obviar estos problemas y otros que presenta la inmunofluorescencia (IF) (cultivo de tejidos, equipo, subjetividad, etc.) se han desarrollado inmunoensayos específicos (ELISA) basados en la utilización de anticuerpos monoclonales (AM) que permiten el ensayo de un elevado número de muestras con altos índices en cuanto a la sensibilidad, especificidad y reproducibilidad de los resultados. De modo análogo la determinación de anticuerpos ha evolucionado desde la inhibición de la hemaglutinación hasta inmunoensayos enzimáticos fiables y sensibles. Aunque la determinación de anticuerpos no es criterio diagnóstico de la enfermedad debido a las vacunaciones, es de sumo interés la determinación de los niveles séricos de inmunoglobulinas anti-PPV, para evaluar el estado inmunitario de la cabaña y tomar decisiones sobre manejo, incluyendo: revacunaciones, separación/aislamiento, etc.

Abstract

From April 1988 to August 1993, 57 elderly recipients more than 60 years old (mean age 64 +/- 3 years, 35 males, 22 females) underwent cadaveric renal transplantation. The pretransplant screening included immunological and viral status and urological examination; cardiovascular risk factors were systematically estimated by medical history, physical examination, echocardiography, femoral arterial doppler, and myocardial stress thallium imaging. A coronarography was performed if myocardial ischemia was evidenced. Patients free of cardiovascular diseases or after correction of vascular and/or coronary lesions were included in the waiting list. A sequential immunosuppression regimen including azathioprine, prednisolone anad antilymphocyte globulins was given in all patients. Oral cyclosporin A (5-8 mg/kg/day) was started when serum creatinine level decreased to 200 mumol/l; antilymphocyte globulins were stopped when whole trough blood cyclosporin level reached 150-200 ng/ml. After 24 months of follow-up, the patient survival rate of elderly recipients was significantly lower than the survival rate observed in patients less than 60 years old (90% vs 97%; p0.005); the deaths were related to cardiovascular complications in 3 cases and to infectious diseases in 3 cases. No abdominal complications were observed in our series. The graft survival was identical in both groups (81% vs 82% at 2 years), and we observed a low incidence of acute rejection (23%) in the elderly group. The graft function, as determined by serum creatinine level, is significantly correlated with the donor age (p0.05). We conclude that the patients more than 60 years old, free of ischemic coronary disease or after correction of such a lesion could be safely transplanted.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract

Sixteen fatal cases of adenovirus infection in infants (n:10 aged 1 to less than 12 months) and children (n:6 aged 1 to 3, 8/12 years) are reported. Diagnosis were confirmed by direct viral isolation, viral immunofluorescence or both in 15 patients, and by autopsy in 12 of them, including one without positive virological workup. Evidence of multisystemic compromise, particularly that of severe lower respiratory tract infection was observed and lead to death by acute respiratory failure in all cases. White blood cell counts and erythrocyte sedimentation rate did not help to label initially the etiology as either viral or bacterial. Chest roentgenograms showed pulmonary over-inflation and evidence of pneumonitis as well as extensive and rapidly progressive lung opacifications. Most prominent pathologic findings were necrotizing bronchitis, bronchiolitis and broncopneumonia. Several cells containing typical intranuclear inclusion bodies were documented in ten cases and pneumonitis was the only finding in two. Hepatic fat infiltration and lymphocitic depletion of thymus, spleen and lymphatic nodes occurred in all cases. The need of rapid diagnostic tools to avoid nosocomial spread of this kind of infections with such serious consequences is stressed.

Abstract

A robotic system able to reach, capture and, if necessary, detumble a space target is a key technology required to carry out future in-orbit operations, such as On-Orbit Servicing (OOS) and Active Debris Removal (ADR). These mission scenarios involve complex technical challenges which can be addressed improving robustness and autonomy of Guidance, Navigation and Control functions. In this framework, this paper presents a research activity, conducted by a consortium of three Italian Universities under contract with the European Space Agency (ESA), aimed at development of algorithmic solutions for (i) combined control of a chaser equipped with a highly dexterous robotic arm, and (ii) target-chaser relative navigation exploiting Electro-Optical sensors. The performance of the proposed navigation and control techniques are tested both at unit level and in closed loop within a non-linear functional engineering simulator including realistic models for orbital, rotational and multi-body dynamics, as well as for chaser sensors and actuators. Data produced by passive cameras and active LIDAR systems are also realistically reproduced using the ESA Planet and Asteroid Natural Scene Generation Utility Tool. Preliminary design solutions and results are presented in this paper considering three OOS/ADR relevant scenarios, differing in terms of degree of target collaboration and cooperation.

Abstract

Fifteen patients with seasonal allergic pollenosis and five controls were investigated to elucidate the role of the T gamma-cell population in the in vitro IgE response by peripheral blood lymphocytes (PBL). In vitro IgE production by PBL of atopic patients after antigenic stimulation was measured in culture supernatants. The optimal dose for antigenic stimulation was found to be 0.16 micrograms/20 X 10(6) cells of purified antigen. No difference was found when comparing the percentages of T cells (E rosettes) between the two groups: mean per cent for controls was 69.2 +/- 5.76 versus 69.54 +/- 4.42 for the allergic group. With regard to the T gamma-cell population, the values obtained by rosetting with ox erythrocytes sensitized with IgG antibody were 12 +/- 0.71% in normals and 9.8 +/- 1.32% in those with allergic pollenosis. This difference, although significant, may not be enough to explain the different pattern when the in vitro IgE production of both groups investigated was compared. In order to detect the role of T gamma cells in this system, lymphocyte cultures, depleted of T gamma cells, were performed and compared with unfractionated cultures from the same donors. Our results show no differences in the in vitro IgE production when T gamma cells were depleted as compared with the unfractionated cultures.

Abstract

We have studied the WB reliability for the Lyme disease (LD) serological diagnosis. Twenty five samples (20 sera and 5 CSF) from 5 patients with LD, according to CDC criteria were analyzed. The syphilis was excluded in every patient. Borrelia burgdorferi antibodies were assessed by an indirect ELISA (IgG + IgM, B31 strain). Intrathecal antibodies in CSF were studied in patients with neurological diseases. Positive samples by ELISA were reanalyzed by WB according to the manufacturer, Tilton and Dressler criteria and presence either 93, 39, 34 or 23 kDa band for IgG. Four patients were controlled and one of them (25%) became negative the antibodies (serorreversion). Three of them, who had neurological symptoms, displayed intrathecal antibodies. WB sensibility for LD according to the manufacturer, Tilton, Dressler and any of the four IgG bands was 26.31%, 42.1%, 21% and 84% for IgG and 52.6%, 47.3% and 42.1% for IgM respectively. Tilton and Dressler criteria may not be as sensible as 93, 39, 34 or 23 kDa IgG bands for the diagnosis of LD in our environment.

Abstract

OBJECTIVES: A potential point of concern among clinicians is whether results derived from the clinical trials can be reasonably applied or generalised to a definable group of patients seen in real world. It can be the case of the GiACTA study that is a phase III randomised controlled trial of tocilizumab (TCZ) in giant cell arteritis (GCA). To address this question, we compared the clinical features and the responses to TCZ from the GiACTA trial patients with those from a series of GCA seen in the daily clinical practice. METHODS: Comparative study of clinical features between patients from the GiACTA trial (overall n=251) and those from a multicentre series of real-world GCA patients undergoing TCZ therapy (n=134). The diagnosis of GCA in the GiACTA trial was established by the ACR modified criteria whereas in the series of real-world patients it was made by using the ACR criteria, a positive biopsy of temporal artery or the presence of imaging techniques consistent with large-vessel vasculitis in individuals who presented cranial symptoms of GCA. GiACTA trial patients received subcutaneous TCZ (162 mg every 1 or 2 weeks) whereas those from the clinical practice series were treated using standard IV dose (8 mg/kg/month) or subcutaneous (162 mg/week). RESULTS: Real-life patients undergoing TCZ were older with longer disease duration and higher values of ESR and had received conventional immunosuppressive therapy (mainly methotrexate) more commonly than those included in the GiACTA trial. Despite clinical differences, TCZ was equally effective in both GiACTA trial and clinical practice patients. However, serious infections were more commonly observed in GCA patients recruited from the clinical practice. CONCLUSIONS: Despite clinical differences with patients recruited in clinical trials, data from real-life patients confirm the efficacy of TCZ in GCA.

44. Rethinking the PPP strategy concept : Towards consistency in discourse and management

Abstract

This research explores the phenomenon of inter-organisational collaboration between public and private organisations in public-private partnerships (PPPs). The state's need for cost savings, capital, and greater efficiency has been accentuated by the recent global economic instability and, forecasts suggest that governments, particularly in Europe, will increasingly enter into partnerships with the private sector in the future.In contracting out public services investment through the collaboration with private sector the British experience is considered an international reference, the National Health Service (NHS) being a relevant example and also one of the most controversial contexts for the intervention of the private sector. However, few previous studies have analysed collaborative provision of ancillary services in this context. To conduct such a study, the concept of "partnership" required further clarification in both theoretical and management terms. To this end, the research focused on the leading drivers and governing influences and players, and on the working climate generated, including the socio relational dimension and involved dynamics. The study was conducted by means of a multi case study methodology based on semi-structured interviews, focus groups and additional documentary information. Participants' values and expectations in terms of working in partnership were examined as well as implementation and governance practices of the concept of PPP, in order to identify the connection between theory and practice. Finally, a model was developed for enhancing partnership between public and private organisations, grounded in the principles of relational management theories and in accordance with the particularities of this research context.The main issue identified is that the drivers behind the formation of partnering agreements are not in tune with the concept of partnership. The explored PPPs cases manifested a highly transactional nature, representing contractual exchange relationships characterised by formalised methods of task control and assessment rather than by trust based collaboration. There was a lack of a supportive governance structure in terms of the principles of partnership that the state had avowedly adopted. This research maintains that managing the relational dimension serves as a catalyst for the success of PPPs and that these partnerships are not embedded within organisations; rather, their development depends on having the right people in place. The PPP site manager's aptitude and attitude are key factors.This study highlights the need for consistency in the practice of partnership in terms of providing a balance between the level of formalisation and attention to the socio-relational dimension. PPPs are not benefiting in full from collaboration due to the neglect of the public/private relationship as a source of added value. This research urges the government to turn the concept of partnership into a reality instead of fostering anti-collaborative relationships. A model is suggested providing guidance on the strategic management of PPPs beyond limiting partnership to mere fulfilment of outcomes.

45. Improved local search for job shop scheduling with uncertain durations

Abstract

This paper is concerned with local search methods to solve job shop scheduling problems with uncertain durations modelled as fuzzy numbers. Based on a neighbourhood structure from the literature, a reduced set of moves and the consequent structure are defined. Theoretical results show that the proposed neighbourhood contains all the improving solutions from the original neighbourhood and provide a sufficient condition for optimality. Additionally, a makespan lower bound is proposed which can be used to discard neighbours. Experimental results illustrate the good performance of both proposals, which considerably reduce the computational load of the local search, as well as a synergy effect when they are simultaneously used.

Abstract

Oxytocin (OT) is a peptide involved in several physiological functions in the central nervous system including central cardiovascular regulation. To clarify the role of endogenous OT in cardiovascular control, one group of anesthetized rats received unilateral microinjections of the OT receptor antagonist [d(CH(2))(5),Tyr(Me)(2),Orn(8)]-vasotocin (OTA) in the nucleus tractus solitarii (NTS) and a second group was injected with specific OT antiserum (Anti-OT). Moreover, the modulation of the cardiovascular effect of L-glutamate (GLU) by OT was also evaluated by cardiovascular analysis using effective and threshold doses of GLU. Mean arterial pressure (MAP) and heart rate (HR) were measured from a femoral catheter. OTA significantly (p0.01) decreased the vasopressor and tachycardiac long-lasting response elicited by an effective dose of OT. Microinjections of Anti-OT antibody did not modify the values of MAP and HR compared with the control group. With regard to the OT/GLU coinjections, a subthreshold dose of OT significantly (p0.001) counteracted the vasodepressor and bradycardiac responses induced by GLU. The coinjection of subthreshold doses of OT and GLU did not produce a change in MAP or in HR. These findings seem to exclude an endogenous tonic action of OT on central regulation of MAP and HR, although they confirm the significant role of OT on central cardiovascular control within the NTS. In fact, the modulation of GLU responses by OT supports the importance of OT on the central cardiovascular adjustments likely acting on the baroreceptor reflex sensitivity.

Supplemental Digital Content is available in the text, The aim of the study was to profile those patients included in the RELESSER registry with histologically proven renal involvement in order to better understand the current state of lupus nephritis (LN) in Spain. RELESSER-TRANS is a multicenter cross-sectional registry with an analytical component. Information was collected from the medical records of patients with systemic lupus erythematosus who were followed at participating rheumatology units. A total of 359 variables including demographic data, clinical manifestations, disease activity, severity, comorbidities, LN outcome, treatments, and mortality were recorded. Only patients with a histological confirmation of LN were included. We performed a descriptive analysis, chi-square or Student's t tests according to the type of variable and its relationship with LN. Odds ratio and confidence intervals were calculated by using simple logistic regression. LN was histologically confirmed in 1092/3575 patients (30.5%). Most patients were female (85.7%), Caucasian (90.2%), and the mean age at LN diagnosis was 28.4 ± 12.7 years. The risk for LN development was higher in men (M/F:47.85/30.91%, P
