Raising a child with autism spectrum disorder (ASD) can be a very stressful experience, impacting parents and their ability to take care of their child. We examined the relationship between parenting stress and child, and parent and contextual factors in a sample of 61 families of children with ASD recruited in a centre for ASD assessment and treatment. Results showed that mothers had a higher level of stress related to their parental role when compared to fathers. Data also showed the importance of considering the specific role of parental emotion regulation, family functioning and educational level in influencing maternal stress and the specific role of couple adjustment in influencing fathers’ parental distress. This study pointed out the importance of employing a vision that embraces different domains of family life in cases of children with ASD and in parents’ intervention programmes because these factors may contribute to parental stress.

Purpose The assessment of Autism Spectrum Disorders (ASD) in childhood has two essential aspects: the identification of the risk (under 30 months of age) and the definition of a diagnosis that takes into account its core areas as well as further non-specific aspects. The purpose of this paper is to present an approach that considers the combination of clinical evaluation with the use of tools that analyse the various levels of the child’s functioning as fundamental. Design/methodology/approach The comprehensive assessment at the Institute of Ortofonologia in Rome provides the ADOS-2 and the Leiter-R for the evaluation of the symptomatology, the severity level, the non-verbal cognitive functioning and the fluid reasoning; the TCE and the UOI are used to identify, respectively, the child’s emotional skills and the ability to understand the intentions of others, as precursors of the theory of mind. Within this assessment, the Brief-P, the Short Sensory Profile and the RBS are also included for the evaluation of executive functions, sensory pattern and of restricted and repetitive behaviours, as observed by parents. Findings How to define a reliable development profile, which allows to plan a specific intervention calibrated on the potential of the child and on his development trajectory, is described. Two clinical cases are also presented. Originality/value The entire process is aimed both at a detailed assessment of the child’s functioning and at identifying a specific therapeutic project and predictive factors for achieving an optimal outcome.

Based on cross-sectional research linking poor reflective functionining (RF) to eating disorders, the current follow-up study tested whether maternal RF would explain the variance of mothers' and children's weight beyond the effects of maternal emotional dysregulation. During pregnancy (Time 1 [T1]), 51 women were administered the Difficulties in Emotion Regulation Scale (DERS) and interviewed using the Adult Attachment Interview (AAI). Seven months after delivery (Time 2 [T2]), mother-baby dyads who remained in the study (n = 44) were videotaped (Feeding Scale) during their feeding interaction. Last (Time 3 [T3]), the weight of the 34 children who were still in the study was collected at 3 years of age. Maternal AAI-RF at T1 did not correlate with the DERS at T1 nor with the quality of the feeding interacions at T2. However, it correlated, significantly, with maternal body mass index (BMI) at T1, r = -.298, P = .034, and marginally significantly with baby's BMI at T3, r = -.296, P = .089. Moreover, multiple regression models showed a trend indicating that maternal AAI-RF might explain the variance of mothers' and children's weight beyond the effects of maternal emotional dysregulation. These findings suggest that working on maternal mentalization might contribute to helping prevent childhood obesity from pregnancy.Objetivo: con base en la investigación de naturaleza transversal que conecta el pobre funcionamiento con reflexión (RF) con los trastornos de alimentación, el presente estudio de seguimiento examinó si el RF materno podría explicar la variación del peso de las madres y de los niños más allá de los efectos de la desregulación emocional materna. Método: durante el embarazo (T1), a 51 mujeres se les administró la Escala de Regulación de las Dificultades en la Emoción (DERS) y se les entrevistó usando la Entrevista de la Afectividad Adulta (AAI). Siete meses después del parto (T2), las díadas mamá-bebé que permanecían en el estudio (n = 44) fueron grabadas en video (Escala de Alimentación) durante su interacción de alimentación. Por último (T3), se determinó el peso de los 34 niños que aún estaban en el estudio a los tres años de edad. Resultados: la información materna de AAI-RF al T1 no se correlacionó con DERS al T1, ni tampoco con la calidad de las interacciones de alimentación al T2. Sin embargo, sí se correlacionó, significativamente, con BMI materno al T1 (r = −.298, p = .034), y marginalmente significativo, con el BMI del bebé al T3 (r = −.296, p = .089). Es más, los modelos de regresión múltiple mostraron una tendencia indicando que la AAI-RF materna pudiera explicar la variación del peso de las madres y de los niños más allá de los efectos de la desregulación emocional materna. Conclusiones: estos resultados sugieren que trabajar en la mentalización materna pudiera contribuir a ayudar a prevenir la obesidad en la niñez desde el embarazo.Objectif: Se basant sur des recherches transversales liant un faible fonctionnement de réflexion (RF) aux troubles du comportement alimentaire, cette étude de suivi a testé si le RF maternel pouvait ou non expliquer la variance des poids des mères et des enfants au-delà des effets de la dysrégulation émotionnelle maternelle. Méthode: Durant la grossesse (T1), 51 femmes ont reçu l'Echelle de Difficultés de Régulation de l'Emotion (en anglais Difficulties in Emotion Regulation Scale, soit la DERS) et ont passé un entretien utilisant l'Entretien d'Attachement Adulte (Adult Attachment Interview, AAI). Sept mois après l'accouchement (T2), les dyades mère-bébé encore dans l’étude (n = 44) ont été filmées (Echelle d'Alimentation) durant leur interaction alimentaires. Enfin (T3), le poids des 34 enfants étant encore dans l’étude a été pris à l’âge de trois ans. Résultats: Le RF-AAI maternel au T1 n’était pas en corrélation avec la DERS au T1, ni avec la qualité des interactions alimentaires au T2. Cependant, il était corrélé, et de manière importante, avec l'IMC maternel au T1 (r = −,298, p = ,034), et de manière marginalement importante, avec l'IMC du bébé au T3 (r = −,296, p = ,089). De plus plusieurs modèles de régression ont montré une tendance indiquant le le RF-AAI maternel pourrait expliquer la variance des poids des mères et des enfants au delà des effets de la dysrégulation émotionnelle maternelle. Methode: 51 Frauen füllten während der Schwangerschaft (T1) die Difficulties in Emotion Regulation Scale (DERS) aus und durchliefen das Adult Attachment Interview (AAI). Sieben Monate nach der Geburt (T2) wurden die in der Studie verbleibenden Mutter-Kind-Dyaden (n = 44) beim Füttern gefilmt (Feeding Scale). Zuletzt (T3) wurde das Gewicht der 34 in der Studie verbleibenden Kinder im Alter von drei Jahren erhoben. Ergebnisse: Die mütterliche AAI-RF zum Zeitpunkt T1 wies weder zum DERS zu T1 noch zur Qualität der Fütterungsinteraktionen zu T2 Zusammenhänge auf. Parents of children with autism spectrum disorder (ASD) have to address various challenges mainly due to their children’s atypia related to communication, emotion regulation and behaviors, arising also within the relationship with their caregivers. Several studies have pointed out that children with ASD can exhibit many difficulties regarding initiating and maintaining meaningful relationships with others. To date, little research has explored the interactions between children with ASD and their fathers, focusing more on mothers. In the context of parent–child interactions in the presence of autism, recent studies have highlighted the importance of parental attunement but there is little research considering solely the affective-bodily dimension. Therefore, the aim of the present study was to explore the parental attunement in fathers of children with ASD observed during play interactions and to investigate the relationship between paternal attunement and the perception of their psychological characteristics related to care relationships. The results highlight that fathers who describe themselves as better in affective care and sensitivity toward others more likely have an absence of paternal attunement during play interactions. The data presented are discussed in the light of intervention hypotheses, aimed at improving the relationship between fathers and children with autism.

Early parent-child relationships are an important factor influencing many domains of child development, even in the presence of autism. In this study, we investigated the associations between parent-child attunement during play, parental insightfulness, and parental acceptance of their child's diagnosis of an autism spectrum disorder. A sample of 50 parents (26 mothers and 24 fathers) of 26 children aged between 24 and 58 months were videotaped during parent-child play interactions and then interviewed about what they thought had gone through their child's head during the play interaction, and about their feelings and thoughts about their child's diagnosis. Play interactions were evaluated using a coding protocol to assess parental attunement. The results showed that parents who were more able to accept their child's diagnosis and to see things from their child's perspective were more likely to also be attuned during play interactions with their children. These findings highlight the importance of studying the parental ability of insightfulness and acceptance of their child diagnosis of ASD for the implementation of intervention programs for supporting parental attunement and improving the interactions between the parents and the children with autism spectrum disorders.

This paper reports on a long-term follow-up of a longitudinal study conducted in Italy that assessed attachment patterns of late-adopted children (placed between 4 and 8 years old) and their adoptive mothers, in three phases: T1, at placement; T2, in childhood (7 to 8 months after adoption); and T3, in adolescence (current study). The following hypotheses were tested: 1) children’ IWMs will shift from insecurity towards security in a long-term follow-up; and 2) there will be a significant association between adoptees’ and adoptive mothers’ IWMs in adolescence. Participants were 22 late-adopted adolescents (aged 11-16) and their adoptive mothers, all assessed in previous phases. Participants completed several measures of attachment, including the Separation-Reunion Procedure (T1, T2), Manchester Child Attachment Story Task (T2), Friends and Family Interview (T3), and Adult Attachment Interview (T1, T3). Late-adopted adolescents showed both an increase in attachment security and a decrease in disorganized attachment from childhood to adolescence. Adoptive mothers’ (T1 and/or T3?) secure states of mind were associated significantly(?) to their adopted children attachment security in adolescence. These findings reinforce the importance of taking attachment into account for adoptive families from the beginning of adoption.

This study compared an attachment-based coding system for family drawings with narrative and observational attachment measures in adopted children and those living with their natural parents (“community-based”) in Italy. Attachment patterns of 41 children (ages 5–8) were assessed by: a graphic measure, the Family Drawing (FD); a narrative measure, the Manchester Child Attachment Story Task (MCAST); and an observational measure, the Separation-Reunion Procedure (SRP). In the community sample, a significant association emerged between the FD and MCAST disorganized vs. organized classifications, and a number of expected correlations between the FD Global Scales and the MCAST Coherence and Mentalizing continuous scales were found. In the adopted group, some FD Global Scales linked with insecure and disorganized representations revealed negative correlations with the MCAST Mentalizing scale. No correlations were found between the children’s FD and SRP classifications and scales in both groups. The FD attachment-based coding system revealed a particular capacity to assess attachment representations during childhood capturing closely both insecure and disorganized patterns, both in adopted and community children. A special focus was put on the measurement of disorganization, as reflected in one drawing classified as disorganized.

A growing body of attachment literature has focused on bridging the gap between research and clinical applications, even in clinical work with adoptive families. A brief-term clinical intervention focused on a multi-method assessment of attachment relationships and representations was performed in the first year after placement. This single case study aimed at analysing the attachment outcomes, through a long-term follow-up, both for the adoptive mother and her late-adopted son. We assumed that this five-session attachmentoriented intervention could help the mother enhance her sensitivity skills and her ability to understand both her own past attachment experiences and her child’s insecure attachment as a consequence of the failures of his previous experiences of care, supporting her sixyear old child in acquiring a positive image of himself and of others and the security of the caregiver’s availability up to adolescence.

Introduction: Recent literature has shown that the good outcome of adoption would mostly depend on the quality of adoptive parenting, which is strongly associated with the security of parental internal working models (IWMs) of attachment. Specifically, attachment states-of-mind of adoptive mothers classified as free and autonomous and without lack of resolution of loss or trauma could represent a good protective factor for adopted children, previously maltreated and neglected. While most research on adoptive families focused on pre-school and school-aged children, the aim of this study was to assess the concordance of IWMs of attachment in adoptive dyads during adolescence. Method: Our pilot-study involved 76 participants: 30 adoptive mothers (mean age = 51.5 ± 4.3), and their 46 late-adopted adolescents (mean age = 13.9 ± 1.6), who were all aged 4–9 years old at time of adoption (mean age = 6.3 ± 1.5). Attachment representations of adopted adolescents were assessed by the Friend and Family Interview (FFI), while adoptive mothers’ state-of-mind with respect to attachment was classified by the Adult Attachment Interview (AAI). Adolescents’ verbal intelligence was controlled for. Results: Late-adopted adolescents were classified as follows: 67% secure, 26% dismissing, and 7% preoccupied in the FFI, while their adoptive mothers’ AAI classifications were 70% free-autonomous, 7% dismissing, and 23% unresolved. We found a significant concordance of 70% (32 dyads) between the secure–insecure FFI and AAI classifications. Specifically adoptive mothers with high coherence of transcript and low unresolved loss tend to have late-adopted children with high secure attachment, even if the adolescents’ verbal intelligence made a significant contribution to this prediction. Discussion: Our results provides an empirical contribution to the literature concerning the concordance of attachment in adoptive dyads, highlighting the beneficial impact of highly coherent states-of-mind of adoptive mothers on the attachment representations of their late-adopted adolescent children.

Transition to parenthood is a critical stage of life due to several changes the couple has to handle. A large body of studies described how transition to parenthood can be linked to the onset of depressive symptoms, as well as the perception of a low social support, and an increased stress, representing a risk for the early mother—baby relationship. Infant Massage (IM) emerged as an helpful tool to improve maternal skills in interacting with the baby, and leading toward a decreasing of post partum symptoms. However, a growing body of literature highlights that men also may experience post-partum diseases, representing an additional risk for the development of the baby. To date, no study observed the impact of the infant massage on both partners. The aim of the current qualitative research is to observe the impact of the IM on a single couple of parents at childbirth. Pre (Time 1) and post-intervention (Time 3) procedure has been established to observe the changes occurring over the time in the couple. In particular, each member of the couple filled out the EPDS, the BDI-II, the MSPSS and the PSI-SF both at Time 1 and at Time 3. The treatment (Time 2) was represented by the IM training, and lasted 4 weeks. Findings revealed a decrease in depressive symptoms in both partners, as well as an improvement of their perception of stress related to parental role. No changes has been detected with respect to the perception of social support. The IM seems to be a helpful approach to prevent the establishment of pathological conditions in new parents. Although no direct measures on the child were used, the current qualitative data seem to suggest that the IM may represent a valuable tool to prevent the onset of early negative outcomes of the baby. Further investigations and empirical data are needed to improve the knowledge in this field.

La presente rassegna ha avuto come obiettivo la disamina della letteratura relativa al tema del maltrattamento e dell’abuso subito da bambini affetti da Disturbo dello Spettro Autistico (DSA). Gli studi emersi hanno evidenziato, da una parte, l’elevato rischio di vittimizzazione a cui i bambini con Disturbo dello Spettro Autistico sono esposti e, dall’altra, la scarsita di indagini in grado di fornire informazioni specifiche, soprattutto rispetto all’intervento nelle famiglie. Gli studi esaminati indicano la necessita di stabilire programmi tesi alla prevenzione del maltrattamento e dell’abuso attraverso il miglioramento delle abilita sociali e di problem-solving dei minori e al trattamento delle disfunzioni della relazione genitore-bambino.

Introduction Adverse childhood experiences (ACE) have been shown to be associated with negative outcomes in adulthood and risk factors for psychopathology. Objectives To assess mental representations of children underwent to ACE administering a recently developed semi-projective method: the Coffy Test. Aims To compare mental functioning of children who underwent to one or several adverse life events with the one of a control sample of non referred children. Methods The sample, recruited at the Neuropsychiatry Unit after adverse life events occurred, was composed of 40 children (21 male, 19 female) ( M age = 108.10 months; SD = .26). The control sample was composed of 160 children (86 males, 74 females) ( M age = 108.80 months; SD = 21,10). Children were administered the Coffy Test, which is a new projective method focused on assessing possible elements of distortion in emotional processing within the relationship with parental figures in middle and late childhood (Cardi, Leonardi, D'Amico, Battista, 2012). Results Coffy Test scores of the sample underwent to ACE and the control sample did not differ significantly neither for gender nor for age. However, the differences of the Coffy Test scores between the two groups were statistically significant ( U = 1.26, p Conclusions The Coffy Test score suggested that children underwent to ACE had a worst mental functioning and thus may be exposed to the risk of developing psychopathology in adulthood.

Introduction Several caregiving activities appear to be compromised by postpartum depression including breastfeeding, sleep routines and well-child visits, such as vaccinations and safety practices. Previous studies highlighted the need for universal screening of maternal and paternal depression, as well as the establishment of early interventions during the postpartum period such as the infant massage for their infants. Aim The goal of the present pilot study is to purpose the neonatal massage as an intervention to decrease the depressive symptoms of the couple during the post partum period. Method Depressive symptoms were evaluated through the Edinburgh Postnatal Depressive Scale (EPDS; Cox, Holden & Sagovsky, 1987) and the Beck Depression Inventory-II (BDI-II; Beck, Steer, Brown, 1996) at 3 months of the baby. In the following 4 weeks, the couple was trained to touch the baby through the neonatal massage techniques. Their interaction was videotaped and assessed through the Assessment Tool for Observation of Mother/Father-infant Interaction (Dumas, Bystrova, Widstrom, 2005). At the end of the training, depressive symptoms were evaluated through the same measures. Results Preliminary findings have shown lower scores of depressive symptoms on the EPDS, and the BDI in both mother and father after the neonatal massage took place. Also parents reported lower complaints of the baby in several area. Conclusion Early intervention during the postnatal period of life of the family may lead toward a greater functioning of the couple, and to a greater sensitive approach with the baby.

Introduction A growing body of research has indicated that difficulties in emotion regulation (ER) are associated to eating disorders as well as to weight changes. However, it has not been clearly investigated the intergenerational transmission of ER and its role on the onset of early feeding and weight problems of the child. Aim The goals of the present study are: 1) to examine the relationship between maternal ER during pregnancy and the feeding practices at 8 months of the baby; 2) and to assess the predictive role of ER on the onset of overweight at 3 years of age. Method The sample is made up by 65 dyads distributed to 2 different groups based on their BMI. During pregnancy they were administered DERS. 8 months after the delivery they were videotaped during the meal time with baby through the Feeding Observational Scale (Ammaniti et al., 2002). Weight outcome of the baby was reported by the mother at 3 years of age. Results Findings has shown that overweight mothers emerged as more emotional dysregulated during pregnancy. Their feeding interaction with the baby was also featured by greater difficulties compared to the control group. ER during pregnancy was able to predict the affective state of the dyad and the interactional conflict during the mealtime at 8 months. The affective state of the dyad at 8 months predicted the weight gain of the baby at 3. Conclusion The maternal and the dyadic emotional functioning play a key role in shaping the risk for childhood obesity.

IntroductionLiterature on parents’ adjustment in families with autism spectrum disorder (ASD) children highlights on one hand that raising a child with ASD represent a higher stressful experience comparing to families of children with other disabilities and families of children with typical development. On the other hand, a recent systematic review on relationship satisfaction of these parents stressed the very lower levels of couple satisfaction in parents raising a child with ASD. Give that, the aim of this study is to investigate the association between relationship satisfaction and parental stress in a sample of parents of ASD children.Method70 parents were recruited (34 = M and 36 = F) to sign the following self-reports: Parenting stress index-short-form (PSI-SF), to assess stress relative to parental role, and dyadic adjustment scale (DAS), to assess couple satisfaction.ResultsThe analysis showed no differences between mothers and fathers respect to investigated variables. Negative correlations between almost all subscales of the PSI-SF and the subscales of DAS emerged. Moreover, from the regression analysis performed, it can be concluded that the values of the total score of the DAS predicts the PSI-SF total score.ConclusionsIn accordance with and building on the achievements of previous studies, these data illustrate a positive influence of couple adjustment on parental stress in parents of ASD children, supporting the hypothesis that relationship satisfaction emerge as a protective variables in the process of parental adaptation.Disclosure of interestThe authors have not supplied their declaration of competing interest.

IntroductionLiterature on parents of children affected from cleft lip and/or palate has described the risk of higher levels of stress and anxiety during the pre-surgery period. To the best of our knowledge, just one study has empirically investigated the differences in the psychosocial adjustment of both mothers and fathers, but information on the pre-surgery period were not given. Given that, the aim of the current study is to evaluate the psychological functioning of both parents waiting for the child operation.MethodData from 34 Italian parents (F = 18; M = 16; Mean age = 36.62, SD = 6.07) of children affected by cleft lip and/or palate (Mean age = 12 months; SD. = 13.75 months) were collected during the pre-hospitalization visits. The following questionnaires were administered, respectively to mothers and fathers: PSI-SF, MSPSS, PACQ, DAS and FACES-IV.ResultsData shows no significant differences between fathers and mothers on the total score of each variable taken into account. Differently, significant differences emerge on the “Self Blame” PACQ subscales.ConclusionsMothers and fathers seem to share the same psychological experience during their child pre-surgery period. To note, our preliminary data highlight the maternal perception as featured by a greater sense of guilty for the child's disease. Introduction Pregnancy represents a critical life stage for its physical and emotional changes. Maternal thoughts are often oriented to the worry to lose control on their body, and to their new physical shape. According to this, literature on eating disorder (ED) highlights how pregnancy is usually associated to a temporary interruption of the dysfunctional eating habits due to the move of concerns about the self and the body toward the caregiving of the baby. Aim The goal of the present study is to explore the impact of generalized difficulties in emotion regulation (as a stable trait) versus specific difficulties in emotion regulation (as pregnancy-related) in women with pre-pregnancy eating problems. Method The sample is made by 15 women previously overweight and recruited during their 3 rd trimester of pregnancy. The following measures were administered: Difficulties in Emotion Regulation Scale (DERS), Eating Attitudes Test (EAT-26), Emotional Difficulties Specific to Pregnancy Scale (EDS), and the Adult Attachment Interview (AAI). Results Preliminary findings have shown how higher scores on the DERS Impulse scale as well as higher difficulties in handling emotional states related to pregnancy (e.g. fear of the delivery) are associated to the existence of dysfunctional eating behaviors during the 3rd trimester. Conclusions Emotion regulation has emerged as a transversal skill during the life cycle. Difficulties in this area seem to represent a stable trait in the individual functioning, with the risk to affect the eating behaviors, as well as the health of the mother and the baby during pregnancy.
