Vertaisarvioitu. Näin hoidan. Silmäluomien useiden toiminnallisesti tärkeiden rakenteiden moitteeton toiminta on terävän näön edellytys. Monet sairaudet, vammat ja ikääntyminen muuttavat silmäluomien anatomiaa ja toimintaa, joita voidaan usein parantaa korjaavalla kirurgialla. Tämä edellyttää alueen anatomian ja eri rekonstruktiotekniikoiden hyvää tuntemusta. Kasvainleikkausten jälkeen silmäluomien eri kerrokset voidaan korjata siirteiden ja kielekkeiden avulla. Luomien virheasennot ja raskaat luomet voidaan korjata kiristämällä löystyneitä rakenteita. Silmän sulkeutumista ja luomien vetäytymistä voidaan korjata kirurgisesti. Julkisen terveydenhuollon piiriin kuuluvat vain toiminnallista haittaa aiheuttavat muutokset.

Primary cilia (PC) are solitary, post-mitotic, microtubule-based, and membrane-covered protrusions that are found on almost every mammalian cell. PC are specialized cellular sensory organelles that transmit environmental information to the cell. Signaling through PC is involved in the regulation of a variety of cellular processes, including proliferation, differentiation, and migration. Conversely, defective, or abnormal PC signaling can contribute to the development of various pathological conditions. Our knowledge of the role of PC in organ development and function is largely based on ciliopathies, a family of genetic disorders with mutations affecting the structure and function of PC. In this review, we focus on the role of PC in their major signaling pathways active in skin cells, and their contribution to wound healing and scarring. To provide comprehensive insights into the current understanding of PC functions, we have collected data available in the literature, including evidence across cell types, tissues, and animal species. We conclude that PC are underappreciated subcellular organelles that significantly contribute to both physiological and pathological processes of the skin development and wound healing. Thus, PC assembly and disassembly and PC signaling may serve as attractive targets for antifibrotic and antiscarring therapies.

Ääriolosuhteissa saunavammoja voi aiheutua kontaktin ja kuuman ilman lisäksi myös vesihöyryn syrjäyttäessä hapen. Saunomisen MM-kisoissa vuonna 2010 oletettavasti nimenomaan hapenpuute esti vaarallisten merkkien havaitsemisen ja saunojat jäivät saunaan kriittisiksi viimeisiksi minuuteiksi.

Outcome studies after burn injury have become increasingly important as overall mortality from injury has decreased. The present study, concerning physical and psychosocial aspects, is the second part of our larger survey of outcome after burn. The basis of our study instrument was the Abbreviated Burn Specific Health Scale. A postal survey was conducted among 315 patients aged 15-65 years, treated at Töölö Hospital Burns Unit between 11 November 1988 and 31 December 1994. The study series analysed consisted of 124 patients, whose mean total body surface area (TBSA) burned was 13.0%, mean full-thickness (FT) burns 6.2%, mean age 37 years and mean time of hospital treatment 16 days. Statistical analyses were performed with non-parametric tests and a probability level of 0.05 as the limit of statistical significance. Physical function was affected by the severity of the burn and by hand burns. Body image was influenced by FT burns but not by hand or facial injuries. Sexual life was affected by FT burns larger than 20%. Patients older than 25 years, those with TBSA greater than 30%, FT greater than 20% or time of hospitalization over one month were concerned about their general health. The effect of burns on patients' physical and psychosocial life was, however, slight, even with respect to severe injuries.

Although the use of split thickness skin grafts (STSGs) is the gold standard to cover excised deep burns, relatively little is known about how regenerating dermal connective tissue underneath the STSGs affects wound healing and ultimately the outcome from grafting. We compared three ways to treat a wound bed of a deep burn after excision onto fascia. The recipient sites before transplanting the STSGs were: a plain excised wound as a control, a site treated with permanent artificial dermal template (Integra®), and a site with granulation tissue promoted by cellulose sponge (Cellonex™). In study I ten adult burn patients with deep burns covering 22-45 % of total body surface-area were included. Test areas were divided into three equal sections (5 x 10 cm each) that were covered in random order with one of the aforementioned materials. After two weeks, the artificial dermal template and the wound bed under the viscose cellulose sponge were also covered with STSGs. At each site the wound healing process was followed-up for up to one year. Histological analysis from punch biopsy samples and clinical scar assessments were performed using the Vancouver Scar Scale at 3 and 12 months. Study II concentrated on the healing process of STSGs one week and 3 months after grafting from the material retrieved from study I. Healing was assessed using immunohistochemistry of late inflammation, angiogenesis/vascularity, and proliferation . In study III, samples from study I were analysed using proteomics. The differentially expressed proteins (DEPs) were further analysed using Ingenuity Pathway Analysis. Based on the results of pathway analysis the potential role and function of DNAH10 (a motor protein usually found in cilia and flagella) was assessed in a group of patients with psoriasis as well as healthy controls. This thesis has five main findings: 1) A valuable experimental model was designed for objective comparative studies of different treatment modalities with-in a patient-controlled study protocol. 2) In study I, inflammation was more distinctive in the granulation tissue promoted by cellulose sponge. In contrast to expectations, more pronounced inflammation combined with granulation tissue formation did not jeopardize the outcome in terms of scarring problems when the wound bed was covered in a timely fashion. 3) After one year, the clinical outcome and the histological findings of the artificial template site did not differ from the two other test sites. 4) In study III, at one-year follow-up, approximately 12% of all proteins detected demonstrated differential expression across comparisons between the different wound bed treatments, and 5) the biggest differences were found in the expression of the axonemal heavy chain dynein DNAH10. STSGs on pre-treated wound beds demonstrated higher expression of DNAH10 than STSGs of control sites. The expression of DNAH10 was also higher in intra-lesional psoriasis skin samples than non-lesional or healthy control skin samples. The dermal composition of the wound bed has an impact on cellular behaviour during wound healing, and it affects functions such as proliferation and angiogenesis. Surprisingly by the end of the follow-up period, healthy granulation tissue resulted in as little scarring as dermal substitute in the STSGs. Despite the lack of clinical and histological differences with those assessment methods used, clear differences were found in protein levels at the long-term follow-up. Terveeltä iholta otettu ihonsiirre on ollut vuosikymmeniä vaikeiden palovammojen hoidon kulmakivi. Siitä, miten haavapohjan koostumus vaikuttaa ihonsiirteeseen ja hoidon lopputulokseen tiedetään kuitenkin edelleen vain vähän. Tähän väitöskirjatutkimukseen värvättiin kymmenen aikuispotilasta, joiden syvien palovammojen laajuus oli 22-45% koko ihon pinta-alasta. Palovamman haavapohjasta jaettiin 15cm x 10cm kokoinen alue kolmeen yhtä suureen osaan. Yksi osa peitettiin osaihonsiirteellä heti palovamman poistoleikkauksen yhteydessä, toinen osa hoidettiin granulaatiokudoksen muodostumista edistävällä väliaikaisella selluloosapohjaisella haavasidoksella (Cellonex™) ja kolmas osa hoidettiin ihoon jäävällä keinotekoisella dermiksellä (Integra®). Kaksi viikkoa leikkauksen jälkeen myös granulaatiokudoksen ja keinotekoisen dermiksen alueet peitettiin osaihonsiirteillä. Keinotekoisen dermiksen avulla oletettiin parantuneen ihon laadun, ulkonäön ja toiminnallisuuden paranevan muihin osa-alueisiin verrattuna. Tutkimuksessa havaittiin selluloosa-haavasidoksella hoidetuilla osa-alueilla muita voimakkaampi tulehdusvaste. Vuoden seurannan jälkeen tehdyissä ihon laadun mittauksissa ja histologisissa arvioissa ei löytynyt eroja. Sen sijaan hoitojen todettiin vaikuttavan eri tavoin DNAH10-proteiinin ilmentymiseen ihon pintasolukossa, keratinosyyteissä. Alueilla, joiden haavapohjan uudiskudoksen muodostusta edistettiin eri materiaaleilla, todettiin suurempi DNAH10 proteiinin ilmentyminen kuin ihonsiirteellä välittömästi peitetyillä alueilla. Materiaalein hoidetuilla alueilla oli myös enemmän uudisverisuonitusta. Havaitsemamme erot tulehdusvasteessa ja eri proteiinien ilmentymisessä hoitokohtien välillä viittasivat erityisesti muutoksiin solujen jakaantumisessa ja erilaistumisessa. Psoriasis-iholle on tyypillistä korostunut tulehdusvaste, sekä häiriöt keratinosyyttien jakaantumisessa ja erilaistumisessa. Vertasimme tämän vuoksi DNAH10 ilmentymistä psoriasis-potilaiden tulehduksesta kärsivästä ja terveestä ihosta sekä verrokkipotilaiden ihosta otetuissa näytteissä. Tulehduksellisissa psoriasis-näytteissä DNAH10 proteiinin ilmentyminen oli erittäin voimakasta muihin näytteisiin verrattuna. Väitöstyön perusteella voidaan todeta, että haavapohjan soluväliaineen koostumus vaikuttaa ihonsiirteeseen haavan paranemisen aikana, ja että vaikutus heijastuu parantuneen ihon proteiinikoostumukseen pitkänkin ajan jälkeen. Jos saadut tulokset pystytään vahvistamaan muissa tutkimuksissa, DNAH10 saattaa tulevaisuudessa tarjota diagnostisen tai lääkehoidon kohteen ihon toimintaan vaikuttamiseksi esimerkiksi tulehduksellisissa ihosairauksissa, ehkä jopa ihosyövissä.

The skin is the largest organ of the human body. It maintains body homeostasis and provides an essential barrier that protects us from the environment. After injury to the skin, a complex series of tightly controlled responses are initiated to restore its integrity. In large-scale wounds, such as burns, the intrinsic capacity for healing is compromised due to lack of dermal support. Dermal fibroblasts guide crucial re-epithelialization events such as keratinocyte migration and proliferation. Fibroblast-keratinocyte interaction is directed by paracrine factors as well as cell-cell and cell-extracellular matrix contacts. Fibroblasts as well as other mesenchyme-derived cells such as bone marrow stromal cells can produce a vast array of growth factors in vitro. When activated by deprivation of extracellular matrix cell adhesion sites, the cells prefer cell-to-cell contacts, form clusters, and release growth factors. This present work focused on characterizing the use of a combination of growth factors released by mesenchymal cell aggregates, a process designated as nemosis, for treatment of skin wound healing. We used inhibitors of several intracellular signaling pathways to define the migration-inducing mechanisms of nemosis using a keratinocyte wound-healing assay. Model human keratinocytes HaCaT-cells responded to the nemosis-derived conditioned medium with enhanced migration, proliferation, and viability. A c-Met-specific small molecule tyrosine kinase inhibitor as well as an anti-hepatocyte growth factor antibody inhibited these responses. The study aimed to characterize means of improving wound healing by combining biological materials, such as fibrin or recombinant human collagen III with the growth factors to construct a transplantable cell- and growth factor-containing matrix. When we treated porcine partial-thickness wounds with the growth factor-containing fibrin matrix, we observed an increase in the proliferation and migration of hair-follicle and lateral wound-edge keratinocytes. In addition, granulation tissue formation was enhanced when compared with formation on saline-treated wounds. Keratinocyte transplantation in porcine full thickness wounds was feasible with a recombinant human collagen type III matrix. The results presented here indicate that cell-cell contact-activated fibroblasts or bone marrow stromal cells improve keratinocyte transplantation with paracrine factors. Fibrin or recombinant human collagen III can serve as vehicles for delivery of these signals to the target site in acute wounds. Since these factors are also crucial for healing of chronic non-healing wounds, it is likely that our therapeutic approach will be of benefit in their treatment, too. Ihon ulkopinta, orvaskesi, muodostaa kemiallisen ja fysikaalisen suojan elimistön ja ulkomaailman välille. Vaikeissa ihovaurioissa, kuten laaja-alaisissa palovammoissa ja kroonisissa haavoissa, ihon paranemiskyky on heikentynyt kasvutekijöiden puutteesta. Tiedetään, että kasvutekijät ovat välttämättömiä monitahoisessa paranemisprosessissa. Keskeinen rooli on verinahassa olevilla sidekudossoluilla, fibroblasteilla, jotka tuottavat kasvutekijöitä haavaympäristöön. Ihon sarveissolut reagoivat kasvutekijöihin lisääntymällä ja vaeltamalla haavapintaa pitkin pyrkien palauttamaan orvaskeden suojarakenteen. Vaikeiden haavojen hoitona käytetään tavallisesti iho- tai solusiirtoja toimenpiteen aiheuttaessa potilaalle ihonottohaavan. Sekä ihosiirteen, että ihonottohaavan hidas paraneminen on haitallista ja heikentää vaikeasti palaneiden potilaiden ennustetta ja pidentää merkittävästi sairaalassaoloaikaa. Väitöskirjatutkimuksessa pyrittiin selvittämään fibroblastien ja sarveissolujen kasvutekijävälitteisiä vuorovaikutuksia haavan paranemisessa. Väitöskirjatutkimuksessa osoitettiin, että sidekudossoluja, fibroblasteja ja luuytimen kantasoluja, voidaan soluviljelmissä aktivoida tuottamaan tarkoin määrättyä kasvutekijäyhdistelmää. Soluaktivaatiossa, nemoosissa, soluilta estetään niiden kiinnittyminen viljelymaljaan ja toisiinsa kiinnittyessään ne muodostavat solurykelmän ja vapauttavat muiden tulehdusvälittäjä-aineiden lisäksi suuren määrän HGF-kasvutekijää. Väitöstutkimuksessa havaittiin, että nemoosissa tuotetut aineet tehostivat merkittävästi sarveissolujen jakautumista ja vaeltamista soluviljelymalleissa. HGF-kasvutekijän osuus haavan paranemiseen osoitettiin estämällä sen c-Met reseptorin aktivoituminen lääkeaineilla. Lisäksi selvitettiin HGF-kasvutekijän solusisäistä viestitystä. Porsaan ihohaavamallissa havaitsimme, että kasvutekijäterapia nopeutti haavan paranemista aktivoimalla sarveissoluja ja niiden kantasoluja hiusfollikkeleista. Väitöskirjatyössä pyrittiin edelleen kehittämään käytössä olevia solusiirtomenetelmiä. Nykyisissä menetelmissä käytetään eläinperäisiä soluja ja soluväliaineita, kuten kollageenia, kasvatusalustana ja tukirakenteena. Sovelsimme rekombinanttiteknologian avulla tuotettua ihmisperäistä kollageenia, sekä fibriiniä yhdessä kasvutekijöiden kanssa tehostamaan sarveissolujen siirtoa haavaan. Tämä kokeelliseen kirurgiaan kuuluva väitöskirjatyö tarkentaa käsitystä kasvutekijöiden merkityksestä vaikeiden haavojen hoidossa ja antaa uusia ratkaisuja kliinisten solusiirtoterapioiden tehostamiseksi. Väitöskirjatyön tuloksia hyödynnetään jatkotutkimuksissa, joissa tavoitteena on syventää tietämystä fibroblasti-aktivaation osallisuudesta haavan paranemisen muihin osa-alueisiin.
