Venous thromboembolic event in pregnancy is a rare but dreaded complication. When it occurs in labour, it presents with sudden severe fetal distress and maternal haemodynamic compromise. We present to you a case where in patient was taken up for emergency caesarean section for severe fetal distress. Intraoperative ECG showed right heart strain. Hence, an immediate bedside echocardiography was done in medical intensive care unit and it picked up a swirling thrombus in right atrium which immediately got dislodged to pulmonary vessels. Postcaesarean, we faced challenge of controlling active bleeding from atonic uterus with non-surgical techniques (Bakri balloon tamponade and uterine artery embolisation) before considering anticoagulation therapy for pulmonary embolism. The patient recovered well after anticoagulation treatment.

Background: Ayurveda is an ancient medical science. The body is governed by three humors in Ayurveda called dosha: Vata, Pitta, and Kapha. Aim: Variation in facial wrinkles among different deha prakriti with the help of the wrinkles assessment scale. Material and Method: This analytical cross-sectional study was conducted at the institute and its periphery. Initially, 500 female volunteers were screened based on inclusion and exclusion criteria. Out of which, 310 were assessed for their prakriti with standard prakriti proforma of MUHS. Facial wrinkles were evaluated with the help of a wrinkles assessment scale of 180 female volunteers. Result: The average number of wrinkles was significantly higher in pitta pradhan prakriti compared to kapha pradhan and Vata pradhan prakriti. Premature ageing signs (wrinkles) were higher in Pitta pradhan prakriti.

Background: There is ongoing debate regarding platelet-rich plasma’s (PRP) ability to eff ectively heal soft tissue injuries. Lyophilized platelet-rich plasma (L-PRP) was created and kept using the blood bank’s standard separation procedures. The objective of study was to evaluate eff ects of lyophilization on PRP and its properties such as level of growth factors. Also, the eff ect of storage conditions on growth factors. Methods: The PRPs were created through a two-step centrifugation process and lyophilized. After freeze-thawing to allow platelet growth factors to be released, levels of hepatocyte growth factor (HGF), transforming growth factor (TGF), fi broblast growth factor (FGF)-basic, platelet-derived growth factor (PDGF)-AB, insulin-like growth factor (IGF)-1, and vascular endothelial growth factor (VEGF) were measured each day during storage. Results: After L-PRP preparation, levels of FGF-basic, VEGF, PDGF-AB, EGF and TGF- beta1 signifi cantly raised and levels of HGF and IGF-1 in L-PRPs had light rise. Conclusions: In comparison to F-PRP, platelet counts and seven growth factors increased during preservation in L-PRP. PRPs that have been stored may be injected several times using our method. L-PRP was effi cacious and stable for 90 days when stored at 8oC.

A 38-year-old man presented to the emergency room with a new generalised tonic-clonic seizure. He also complained of headaches, and brain MRI/magnetic resonance venography (MRV) showed an anterior left temporal encephalocoele with gliosis and brain parenchyma herniating into the left foramen ovale. Ophthalmic examination revealed bilateral optic disc oedema and his lumbar puncture confirmed an elevated opening pressure of 48 cm of water. He was diagnosed with idiopathic intracranial hypertension (IIH) and his papilloedema resolved with weight loss and acetazolamide. Raised intracranial pressure (ICP) can be associated with encephalocoeles and lead to seizures. It is important to screen for papilloedema in these patients as they are at risk for permanent vision loss. This was a unique case in which IIH was diagnosed only after a seizure due to an encephalocoele, which was likely related to chronically undetected raised ICP.

Advances in computer-aided design and computer-aided manufacturing software have improved translational applications of virtual surgical planning (VSP) in craniomaxillofacial surgery, allowing for precise and accurate fabrication of cutting guides, stereolithographic models, and custom implants. High-resolution computed tomography (CT) imaging has traditionally been the gold standard imaging modality for VSP in craniomaxillofacial surgery but delivers ionizing radiation. Black bone magnetic resonance imaging (MRI) reduces the risks related to radiation exposure and has comparable functionality when compared with CT for VSP. Our group has studied the accuracy of utilizing black bone MRI in planning and executing several types of craniofacial surgeries, including cranial vault remodeling, maxillary advancement, and mandibular reconstruction using fibular bone. Here, we review clinical applications of black bone MRI pertaining to VSP and three-dimensional (3D)-printed guide creation for craniomaxillofacial surgery. Herein, we review the existing literature and our institutional experience comparing black bone MRI and CT in VSP-generated 3D model creation in cadaveric craniofacial surgeries including cranial vault reconstruction, maxillary advancement, and mandibular reconstruction with fibular free flap. Cadaver studies have demonstrated the ability to perform VSP and execute the procedure based on black bone MRI data and achieve outcomes similar to CT when performed for cranial vault reshaping, maxillary advancement, and mandibular reconstruction with free fibula. Limitations of the technology include increased time and costs of the MRI compared with CT and the possible need for general anesthesia or sedation in the pediatric population. VSP and 3D surgical guide creation can be performed using black bone MRI with comparable accuracy to high-resolution CT scans in a wide variety of craniofacial reconstructions. Successful segmentation, VSP, and 3D printing of accurate guides from black bone MRI demonstrate potential to change the preoperative planning standard of care. Black bone MRI also reduces exposure to ionizing radiation, which is of particular concern for the pediatric population or patients undergoing multiple scans.

The complex three-dimensional (3D) anatomy in facial allotransplantation creates a unique challenge for surgical reconstruction. Evolution of virtual surgical planning (VSP) through computer-aided design and computer-aided manufacturing has advanced reconstructive outcomes for many craniomaxillofacial indications. Surgeons use VSP, 3D models, and surgical guides to analyze and to trial surgical approaches even prior to entering the operating room. This workflow allows the surgeon to plan osteotomies and to anticipate challenges, which improves surgical precision and accuracy, optimizes outcomes, and should reduce operating room time. We present the development, evolution, and utilization of VSP and 3D-printed guides in facial allotransplantation at our institution, from guide conception to first clinical case.

Maintaining a strong, dependable, and exceptionally smart generation, distribution, and transmission infrastructure is crucial. In order to preserve consistent quality & flow of electricity with its stability, this paper presents many power quality difficulties & tasks in the penetration of renewable energy resources with solutions. The article outlines the methods put in place on the power system to reduce power quality issues.

In this project, we are using plastic as a fiberin road construction. We have used plastic milk bags as a fiber because global plastic production is growing with the population growth and many environmental issues can arise as a result of the significant increase in the use of plastics like polyethylene (PE) and polypropylene (PP). Therefore, it's critical to find strategies for getting rid of these waste products without endangering the environment. The disposal of plastic becomes complicated because they are not biodegradable. It is therefore better to recycle than to dispose of. One of the trends in plastic recycling is its use in road construction. Road transport is undoubtedly the lifeline of the nation and its development is a vital concern. Therefore, we decided to use the concept of using plastic as a fiber in road construction. This type of recycling can also help protect the environment from greenhouse gases that are released into the atmosphere during disposal. This will also exhibit high strength, better bindingproperties, stability and increased resistance to wear, and better durability and tear of pavements.

Gujarat, being a coastal state, is likely to be impacted by global warming and climate change not only due to sea level rise and salinity ingress but also due to an increase in the frequency of cyclonic storms and other extreme weather events, causing uncertainty in crop production. An attempt has therefore been made to understand the climate of Gujarat in the past, present, and future based on the works done at the Department of Agricultural Meteorology, Anand. Analysis carried out on climatic trends and climatic extremes using past available data from different stations in Gujarat has been highlighted. Crop simulation models validated with experimental data collected for different crops across Gujarat state were used to understand the response of crops to climatic variability. The climate change impact studies and adaptation strategies carried out under the NPCC project have also been highlighted. And lastly, the work done by the author as an emeritus scientist on climate projections under RCP 4.5 and RCP 8.5 for all the districts of Gujarat and their likely impact on selected crops is presented. The results revealed that in the past, temperatures have shown increasing trends but not reached significant levels except at certain locations at night. Rainfall has also increased, but marginally. Future temperatures have been projected to increase in different parts of Gujarat under RCP4.5 and RCP8.5 with varying magnitudes. Similarly, the rainfall has also been projected to increase, while the sunshine hours are projected to decrease. The ultimate impact would be a drastic reduction in yields in spite of the increase in CO2 level, suggesting that the present-day crop varieties would not be able to sustain crop production levels under a changing climatic scenario.

Domestic sewage-based constructed wetland (CW) showed that the effluent from CW-system contain enough plant nutrients and enhanced the growth of microalgae. Hence, a pilot CW system employing Typha latifolia in domestic sewage, integrated with the cultivation wild type Chlorella sp. was investigated. Phytoremediation at 48 hours of detention time caused significant changes in its physicochemical properties and the generated effluent was notably attractive for the cultivation of microalgae. The microalga was grown in 6 treatments: treated-mixotrophic (T1), treated-heterotrophic (T2), treated-autotrophic (T3), control-mixotrophic (T4), control-heterotrophic (T5) and control-autotrophic (T6) conditions for 8 days inside an incubator. The results suggested that phytoremediation effluents integrated with mixotrophic cultivation of microalgae, utilising both light and carbon sources could be the most efficient, environmentally safe, sustainable and novel technique for synergistic resource generation and bioremediation.

The major objective is to build a platform for developers that can compile, test and run the application with the least specifications and configurations and also conceptualize the piece of deployment with a more relaxed architecture and a small learning period so that developers can take full advantage of multiple platforms available as a cloud service without any inconvenience.

PURPOSE Magrolimab is a monoclonal antibody that blocks cluster of differentiation 47, a don't-eat-me signal overexpressed on cancer cells. Cluster of differentiation 47 blockade by magrolimab promotes macrophage-mediated phagocytosis of tumor cells and is synergistic with azacitidine, which increases expression of eat-me signals. We report final phase Ib data in patients with untreated higher-risk myelodysplastic syndromes (MDS) treated with magrolimab and azacitidine (ClinicalTrials.gov identifier: NCT03248479 ). PATIENTS AND METHODS Patients with previously untreated Revised International Prognostic Scoring System intermediate-/high-/very high-risk MDS received magrolimab intravenously as a priming dose (1 mg/kg) followed by ramp-up to a 30 mg/kg once‐weekly or once‐every-2-week maintenance dose. Azacitidine 75 mg/m2 was administered intravenously/subcutaneously once daily on days 1-7 of each 28-day cycle. Primary end points were safety/tolerability and complete remission (CR) rate. RESULTS Ninety-five patients were treated. Revised International Prognostic Scoring System risk was intermediate/high/very high in 27%, 52%, and 21%, respectively. Fifty-nine (62%) had poor-risk cytogenetics and 25 (26%) had TP53 mutation. The most common treatment-emergent adverse effects included constipation (68%), thrombocytopenia (55%), and anemia (52%). Median hemoglobin change from baseline to first postdose assessment was −0.7 g/dL (range, −3.1 to +2.4). CR rate and overall response rate were 33% and 75%, respectively. Median time to response, duration of CR, duration of overall response, and progression-free survival were 1.9, 11.1, 9.8, and 11.6 months, respectively. Median overall survival (OS) was not reached with 17.1-month follow-up. In TP53-mutant patients, 40% achieved CR with median OS of 16.3 months. Thirty-four patients (36%) had allogeneic stem-cell transplant with 77% 2-year OS. CONCLUSION Magrolimab + azacitidine was well tolerated with promising efficacy in patients with untreated higher-risk MDS, including those with TP53 mutations. A phase III trial of magrolimab/placebo + azacitidine is ongoing (ClinicalTrials.gov identifier: NCT04313881 [ENHANCE]).

A study was conducted with an objective to assess the soil quality of command areas of Kaleshwaram Project in Karimnagar, Nizamabad and Warangal districts. The assessment was made by developing a soil quality index (SQI) based on relationships among pairs of soil parameters, followed by principal component analysis. The study indicated about superiority of soil pH, available Cu, Mn, P, K and Fe that have significantly contributed to the SQI. Four leading principal components were significant based on eigenvalue of ‘more than 1’ and explained 62.11% of variance in soil parameters. All Kaleshwaram project command areas in Karimnagar, Nizamabad and Warangal were found to have SQIs in the medium range (0.35-0.55) when soil quality was evaluated by looking at the variability present in soil parameters.

Background: Associations between epigenetic aging with cognitive aging and neuropsychiatric measures are not well-understood. Objective: 1) To assess cross-sectional correlations between second-generation DNA methylation (DNAm)-based clocks of healthspan and lifespan (i.e., GrimAge, PhenoAge, and DNAm-based estimator of telomere length [DNAmTL]) and cognitive and neuropsychiatric measures; 2) To examine longitudinal associations between change in DNAm markers and change in cognition over 2 years. Methods: Participants were members of VITAL-DEP (VITamin D and OmegA-3 TriaL- Depression Endpoint Prevention) study. From previously ascertained cognitive groups (i.e., cognitively normal and mild cognitive impairment), we randomly selected 45 participants, aged≥60 years, who completed in-person neuropsychiatric assessments at baseline and 2 years. The primary outcome was global cognitive score (averaging z-scores of 9 tests). Neuropsychiatric Inventory severity scores were mapped from neuropsychiatric symptoms (NPS) from psychological scales and structured diagnostic interviews. DNAm was assayed using Illumina MethylationEPIC 850K BeadChip at baseline and 2 years. We calculated baseline partial Spearman\hfilneg correlations between DNAm markers and cognitive and NPS measures. We constructed multivariable linear regression models to examine longitudinal relations between DNAm markers and cognition. Results: At baseline, we observed a suggestive negative correlation between GrimAge clock markers and global cognition but no signal between DNAm markers and NPS measures. Over 2 years: each 1-year increase in DNAmGrimAge was significantly associated with faster declines in global cognition; each 100-base pair increase in DNAmTL was significantly associated with better global cognition. Conclusion: We found preliminary evidence of cross-sectional and longitudinal associations between DNAm markers and global cognition.

Predicting the accurate amount of the real estate is very much a matter of concern as people are investing too much now a days in property where a property dealer could easily charge more amount if the buyer is not knowing the market price of the property. The major focus of this research paper is to predict the accurate price of the property without a hassle. Apart from that it also focuses on increasing the accuracy of the already existing system. There is various machine learning algorithm available for prediction such as Naive Bayes, Logistic Regression, Classification, Random Forest KNN, Support Vector Machine, Lasso, Linear Regression etc. The aim of this research is to predict the market value of real estate properties based on geological location. By analyzing previous market patterns, value ranges, and upcoming developments, we can determine a starting price for a property based on geological variables. Our study is applicable to any location, and we have utilized three machine learning algorithms to make predictions. Our findings indicate that linear regression provides the most accurate predictions, with an accuracy rate of 85%. This system eliminates the need for clients to rely on brokers and provides them with the confidence to invest in real estate. The accuracy of our system surpasses that of previous methods used in the industry.

Pathya–Apathya plays important role in the management of various disorders. Prameha is one of such disorder in which Pathya-Apathya affect its morbidity. Etiology, etiopathogenesis with clinical manifestation of Prameha are very well described in ancient textbook like Charaka Samhita, Sushruta Samhita, Astanga Hridyam and Astanga Samgraha etc. In all treatise of Ayurveda, Prameha is considered as Santarpanajanya Vikara that is of three types i.e., Vataj, Pittaj and Kaphaja. Madhumeha mentioned under the heading of Vataj Prameha that is closely correlated to diabetes mellitus. Diabetes Mellitus is of two type viz., Type-1 Diabetes & Type-2 Diabetes. Among that, Type-2 diabetes is non-insulin dependent diabetes mellitus that occur in middle age. In this clinical entity insulin resistance was developed. In its management, Pathya–Apathya contributes a major role in controlling blood sugar level. Therefore, author attempted to highlight role of Pathya–Apathya (do’s & don’t) in the management of Type-2 Diabetes Mellitus.

Clean, sufficient water is becoming increasingly scarce worldwide, particularly in African metropolitan areas. Today's urban water systems face several challenges, including, among other things, migration from rural to urban areas, climate change, natural disasters, and a lack of institutional capacity, expertise, and financial and human resources. As a result, governments across Africa are struggling to meet constitutional requirements to provide clean and sufficient water and sanitation. This article aims to investigate the value of public-private partnerships (PPPs) in improving Africa's urban water infrastructure. The research in this article was qualitative. Data were gathered through a literature review, and conceptual analysis was used. The findings show that a variety of factors are at work.

Gender disparity is a significant issue that is clearly present in South African municipalities. Municipalities play an important role in providing services to individuals based on their needs and expectations because they are the level of government closest to the people. Gender inequality poses a barrier within municipalities due to discrimination, a lack of gender mainstreaming, a lack of gender policies, and a lack of women's empowerment, limiting women's capacity to contribute to civic life and municipal duties that guide the nation's growth. Inequality between the sexes makes it more difficult for women to support themselves, creating socioeconomic problems in their communities and additional issues for the government to address to ensure effective, efficient, economical, fair, and inclusive governance. This article aims to highlight areas of concern that the government could address through policies by focusing on gender disparities in South African local government from a policy standpoint. Using a qualitative desktop analysis, the article collects information under various themes. The goal is to advise local governments on ensuring gender equality in all decision-making structures and procedures. It is impossible to overstate the importance of involving local governments in the fight against gender inequality. As a result, a gap in research on gender equality in municipal government must be filled. This article seeks to contribute to this direction by examining several aspects of gender inequality at the local government level in South Africa.

Background and Objectives: This study aims to compare the outcomes of pneumatic retinopexy (PnR) versus pars plana vitrectomy (PPV) for rhegmatogenous retinal detachment (RRD). Materials and Methods: A systematic review and meta-analysis were performed following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) Guidelines. An electronic search was conducted identifying six comparative studies of PnR versus PPV for RRD enrolling 1,061 patients. The primary outcome was visual acuity (VA). Anatomical success and complications were the secondary outcomes. Results: No statistically significant difference was observed in VA between the groups. There was a statistically significant difference in the odds of re-attachment favoring PPV over PnR (odds ratio [OR] = 0.29, P < 0.00001). No statistically significant difference was found in final anatomical success (OR = 1.00, P = 1.00) and the development of cataracts (OR = 0.34, P = 0.61). Other complications, including retinal tears and postoperative proliferative vitreoretinopathy, were more frequently reported in the PnR group. Conclusion: PPV has a higher rate of primary reattachment compared to PnR for treating RRD with comparable final anatomical success, complications, and VA outcomes. In recent years, the introduction of steel pre-engineered building (PEB) designs has helped streamline design concepts and reduce dead loads in structures. PEB construction, replacing the traditional steel structure (CSB) construction concept, has brought many advantages as the element is the design. According to the bending moment distribution diagram over the full span of the beam and column he dimensions the PEB structure and reduces the steel requirements with the help of conical IS profiles. In this study, his PEB frames and conventional steel frames of industrial structures were subjected to Seismic analysis and designed according to Indian Standards IS 800-1984, IS 800-2007 and IS1893:2016. I was. Traditional steel and prefabricated buildings require aesthetic appearance and high quality construction to be built very quickly. Traditional steel structures and prefabricated buildings can be widely used in the construction of industrial and residential buildings. This paper was prepared to study various research work related to the analysis of PEB and conventional steel structures.

Background: An important complication of thalassemia is iron deposition in the cardiac tissue resulting in degeneration, fibrosis and dysfunction. Cardiac disease is the primary cause of death. Aims and objectives were to detect early cardiac involvement in patients with transfusion dependent thalassemia by conventional doppler echocardiography and tissue doppler imaging while they are still asymptomatic. Methods: 150 patients in the age group of 2-18 years who were receiving regular and frequent blood transfusions were enrolled in this study. Institutional ethical clearance was taken. After informed consent, 2D echocardiography was performed on all the patients. Previous blood reports reviewed. Various parameters like mitral flow pattern, LVEF and Tricuspid regurgitant jets were taken into account to diagnose cardiac dysfunction. Results analysed by standard statistical tests. Results: Out of 150 patients, 58% were males and 42% were female patients. The 13 (8.6%) patients were diagnosed as having diastolic dysfunction. Out of 150, majority (86.6%) were below the age of 15 years, 81.33% children were diagnosed as having thalassemia before the age of 2 years, 97% patients were on regular iron chelation therapy, 32% patients had pre-transfusion Hb 2500 ng/ml and more number of males (7.33%) have undergone splenectomy than females. Conclusions: Diastolic dysfunction on Doppler echocardiography is an early sign of myocardial dysfunction in patients with beta thalassemia.

Pragyaparadha are main causes of all noncommunicable diseases (NCDs) such as diabetes, cancer, hypertension etc.

The Internet of Things, an emerging global Internet-based technical architecture facilitating the exchange of goods and services in global supply chain networks has an impact on the security and privacy of the involved stakeholders. Measures ensuring the architecture's resilience to attacks, data authentication, access control and client privacy need to be established. An adequate legal framework must take the underlying technology into account and would best be established by an international legislator, which is supplemented by the private sector according to specific needs and thereby becomes easily adjustable. The contents of the respective legislation must encompass the right to information, provisions prohibiting or restricting the use of mechanisms of the Internet of Things, rules on IT-security-legislation, provisions supporting the use of mechanisms of the Internet of Things and the establishment of a task force doing research on the legal challenges of the IoT

In this paper, we focus on makerspace practices that can hinder or enable access for under-resourced women of colour who are novices to making, predominantly migrants and refugee women. We report on a 21 month long ethnographic fieldwork at nine makerspaces across two metropolitan Australian cities. The findings present barriers and opportunities around access of under-resourced women of colour into makerspaces. We present these findings through four themes: first impressions and visual representations; dimensions of the enabling environment; role of community partnerships; and intersectional identities of women. We conclude by discussing three contributions this paper makes to the HCI and CSCW literature, with supporting lessons and recommendations so that makerspaces no longer encode the involvement of only certain types of users (predominantly well-educated white males or white females), to the exclusion of women at the intersectional margins. If makerspaces can facilitate an enabling environment appropriate to be inclusive of women who face intersecting oppressions, the benefits of the maker movement can expand substantially.

SummaryThis study aimed at the performance evaluation of a closed flock of Marwari sheep and also to study the effect of accumulated inbreeding on the growth traits using a linear mixed model methodology. The data generated for 39 years (1981 to 2020) on Marwari sheep maintained at ICAR-Central Sheep & Wool Research Institute, Arid Region Campus (CSWRI, ARC), Bikaner, Rajasthan, India were used for analysis on growth traits. The overall least-squares means (LSM) of live weights at birth (BWT), weaning (3MWT), 6 months (6MWT), 9 months (9MWT) and 12 months (12MWT) were observed to be 3.02 ± 0.01, 14.30 ± 0.04, 20.12 ± 0.05, 23.68 ± 0.06 and 26.39 ± 0.07 kg, respectively. Overall LSM for average daily gain from birth to 3 months (ADG1), 3 to 6 months (ADG2) and 6 to 12 months (ADG3) were 125.44 ± 0.40, 67.37 ± 0.40 and 35.83 ± 0.29 g/day, respectively. Kleiber ratio (KR) from birth to 3 months (KR1), 3 to 6 months (KR2), and 6 to 12 months (KR3) were 16.78 ± 0.02, 6.58 ± 0.04 and 3.05 ± 0.02, respectively. Results revealed a 4.36, 25.83, 36.33, 31.50 and 28.99% improvement in the live weights since the inception of the improvement programme. This is also reflected by a significant effect of sire on all the growth traits. The estimate of inbreeding in the flock was 1.55%. Highly inbred animals were 5.13% (>5% Fi). The study revealed the non-significant effect of inbreeding level on all growth traits except for BWT and KR3. For BWT, inbreeding classes had variation; however, a negative effect was not seen. The inbreeding class (>5% Fi) was reduced by 0.05 units for KR3 as against its preceding class. Dam’s age at lambing and weight influenced the birth weight and subsequent weights. The study concluded that the selection programme of Marwari sheep is in the right direction; however, regular monitoring of inbreeding is necessary and factors affecting growth must be monitored to attain better growth rates in the nucleus.

The mixing of literary theory or criticism with many different literary texts is one of the most interesting changes in the field of research in the modern era. In modern times, literary theory's critical lens is used more and more to explain how a wide range of texts should be read. This trend has become increasingly widespread. The conduct of research in the modern era necessitates innovation; specifically, this refers to the procedure by which a specific theory infiltrates the territory of something that is referred to as a "text." The turbulent times of the post-war period made the 1960s an ideal time for the emergence of the theory of psychoanalysis, along with two of its father figures, Sigmund Freud and Jacques Lacan. This period was perfect for its emergence when taking the 1960s into account. This theory vividly depicted the complexities of the mind and the repercussions of thoughts, and most importantly, it captured what would have otherwise remained hidden: the complexities of human nature. The problems that are associated with the human mind are, without a doubt, the most important ones in psychoanalysis, despite the fact that numerous other issues have established themselves as important components of the theory of psychoanalysis. The novel The Remains of the Day, written by the renowned Japanese author Kazuo Ishiguro, who was born in Japan and won the Nobel Prize, exemplifies, without a doubt, the complexities that are associated with the main character of the book, Stevens, the butler. This book has traditionally been classified as either post-imperial or post-colonial, which has resulted in the inherent presence of the theory of psychoanalysis being relegated to a secondary role in critical discourse. There are a lot of events that take place in the book that seem to point in the direction of this theory. With the help of psychoanalytical theory, the purpose of this paper is to make an attempt to bring to light the inner thoughts that were going through the mind of Butler Stevens. This paper will adhere to the hypothesis that this novel can, in fact, be read in a manner that is comparable to that of a psychoanalytic novel

Despite the exponential increase in research toward better treatment options for breast cancer patients, developing an effective drug with fewer side effects continues to remain a challenge. Natural compounds have emerged as a viable option and several drugs have been derived or inspired from them. In this study, we screened a library of natural compounds with diverse chemical structures against selected kinase proteins using in silico methods such as molecular docking and dynamics simulation. The best results were obtained between β tetralone and MDM2 E3 ubiquitin ligase protein. In vitro experiments such as cytotoxicity, scratch assays and flow cytometry analysis using an MCF7 cell line were performed to determine the anti-cancer potential of the compound. As the treatment resulted in cell death and apoptosis, β tetralone was screened in silico against anti-apoptotic targets where the best results were obtained between Bcl-w and β tetralone. This comprehensive study suggests that the anti-cancer activity of β tetralone is probably through the dual targeting of MDM2 E3 ubiquitin kinase and Bcl-w anti-apoptotic protein. Controversy exists regarding the timing of intervention for patients with critical coronary artery disease (CAD) awaiting coronary artery bypass and severe carotid artery stenosis (CAS). Transcarotid artery revascularization (TCAR) is a minimally invasive revascularization alternative through direct transcervical carotid access that minimizes the chance of arch manipulation and consequent antegrade embolic stroke rate. While the TCAR procedure can be performed under local anesthesia (monitored anesthesia care [MAC]) versus general anesthesia, the hemodynamic benefits of local anesthesia in patients with severe CAD are significant. Patients receiving staged TCAR-coronary artery bypass grafting (CABG) have high-risk cardiovascular disease and require accurate perioperative neurological and hemodynamic evaluation that can be safely provided with local anesthesia.In this retrospective single-center study, 14 patients were systematically identified to have undergone staged TCAR prior to CABG surgery from December 2018 to October 2021. All patients underwent TCAR with local anesthesia and minimal sedation. Relevant patient demographics, medical and surgical history, preoperative covariates, and type of anesthesia administered were obtained from patients' charts. CAD was confirmed by either carotid duplex imaging or computed tomography angiography (CTA) of the head/neck.Staged TCAR-CABG interventions were performed on 14 patients (64% male; mean age 65.0 years). No major adverse cardiac events were reported including transient ischemic attack (TIA), stroke, myocardial infarction (MI), or TCAR-related death in the interval between their TCAR and CABG as well as in a 12-month follow-up period. One patient required to return to the operating room (OR) for evacuation of a neck hematoma.This study demonstrated high success rate of TCAR under local anesthesia prior to CABG (100%) with no incidence of perioperative stroke, MI, or death at 1-month, 6-month, and 12-month follow-up intervals. The authors support the use of staged TCAR-CABG with local anesthesia as a safe and promising treatment option for patients with high-grade cardiac disease, high risk of stroke, or multiple comorbidities that preclude a carotid endarterectomy (CEA).

To determine the future stock value of a company is the main purpose of stock price prediction there is a continuous change in the price of stocks which is affected by different industries and market conditions. The high dimensionality of data is a challenge for machine learning models because highly correlated dimensions/attributes may exert influence on precision of the model. PCA is used to reduce dimensionality to fit linear regression algorithm for future stock price prediction. Linear regression algorithm is used prior to and subsequent to implementation of Principal Component Analysis on the Tesla stock price data. Results manifest that production of machine learning models can be boosted by PCA, reducing the correlation and appropriate selection of principal components for high redundancy of data.
