BACKGROUND: The identification of aberrant phenotypes in acute leukemia is crucial for treatment monitoring and minimal residual disease analysis. Flowcytometric immunophenotyping in acute leukemia is an important tool for the detection of these aberrancies. There is wide variation in the incidence of aberrant antigens in acute leukemia in different studies and its correlation with prognostic markers. AIMS AND OBJECTIVES: The aim and objective of this study is to assess the frequency of aberrant markers in acute leukemia and determine any association with hematological parameters. MATERIALS AND METHODS: A total of 150 newly diagnosed cases of acute leukemia during the period January 2018–December 2020 were included. The flowcytometric immunophenotyping of all the above cases was done using BD FACS Canto II. RESULTS: Out of the total 150 cases of acute leukemia, 56.7% expressed aberrant phenotype. The proportional frequency of aberrant antigen expression in B-acute lymphoid leukemia (ALL), T-ALL and acute myeloid leukemia (AML) was 56.6%, 50%, and 58.5%, respectively. CD13, CD13, and CD7 were the most common aberrancies in B-ALL, T-ALL, and AML, respectively. Aberrant myeloid phenotype in B-ALL was associated with lower mean total leukocyte count (TLC) and blast percentage in peripheral blood as compared to the B-ALL with conventional phenotypes. Aberrant lymphoid phenotype in AML was associated with a higher TLC and greater blast percentage in peripheral blood than the AML with conventional phenotypes. CONCLUSION: Aberrant lymphoid phenotype in AML is associated with unfavorable hematological features. However, aberrant myeloid phenotype in B-ALL is not associated with adverse features.

Cette thèse de doctorat intitulée Lu Yang : une esthétique aberrante cherche à étudier dans une perspective à la fois philosophique et conceptuelle la création médiatique de l’artiste chinois Lu Yang (1984-) ( 陆扬) entre les années 2013 et 2020. À travers l’animation digitale, des installations et des performances, l’artiste conceptualise de nouvelles configurations des sexes, des genres, du corps et de la conscience qui échappent aux paramètres de la normativité et qui contribuent à déconstruire et à repenser autrement l’anthropomorphisme. Ces nouvelles configurations des sexes, du corps et de la conscience possèdent la particularité d’être aberrantes. Elles remettent aussi en cause la distinction entre le rationnel et l’irrationnel mais aussi entre ce qui est considéré comme « normal » ou « anormal ». L’hypothèse de la recherche est alors la suivante : l’artiste crée et développe une esthétique aberrante caractérisée par des logiques irrationnelles mais aussi par un imaginaire singulier qui relève de l’étrange, de l’anormal, du monstrueux, et du grotesque. L’esthétique aberrante comme il le sera démontré tout au long de cette thèse, contribue simultanément à faire éclater les normes sociales, à remettre en question et à transgresser les limites et les frontières sexuelles et corporelles et à proposer de nouvelles conceptions et configurations de la subjectivité. Ainsi, les questions fondamentales posées par cette thèse sont les suivantes : comment l’oeuvre médiatique de Lu Yang développe-t- elle une esthétique aberrante ? Quelles sont les fonctions et composantes à la fois visuelles et sonores de cette esthétique ? Que nous révèle cette esthétique du point de vue philosophique et conceptuel mais aussi d’un point de vue politique ? Comment propose-t- elle de nouvelles perspectives critiques sur l’art contemporain chinois ? Mon analyse des films et des performances définit trois modalités distinctes de l’esthétique aberrante qui caractérise ses oeuvres. La première modalité développée dans le premier chapitre, s’associe à la question du sexe et de la reproductivité, à travers ce que je vais nommer le sexe-aberrant. La deuxième modalité repose plutôt sur le corps et explore ses nombreuses déformations à travers ce que je définis comme une esthétique du grotesque. Finalement, la troisième et dernière modalité, repose sur l’esprit et la conscience et cet aspect demeure étroitement lié à l’intérêt de l’artiste pour la neuroscience et le bouddhisme. À partir de ces éléments, je démontrerai alors comment l’artiste crée ce que j’appelle une cosmo- technique aberrante. Le corpus de cette thèse se constitue ainsi des cinq oeuvres suivantes : Uterus Man (2013), Delusional Mandala (2015), Electromagnetic Brainology (2017), Electromagnetic Brainology Brain Control Messenger (2018) et, Delusional World (2020)., This doctoral thesis entitled Lu Yang: An Aberrant Aesthetic analyze through a philosophical and conceptual approach the artwork of Chinese new media artist Lu Yang ( 陆扬) (1984-). The artist creates vibrant and sophisticated live-action films, 3D animation, video games installations, and motion capture performance. In these films and performances, the artist visualizes alternative configurations of sexes, bodies, and minds that challenges normativity while deconstructing anthropocentrism and the boundaries between normality and abnormality. These new configurations of sex, body, and mind are aberrant. The main hypothesis of this research is the following: the artist conceptualizes and create what I call an aberrant aesthetic that is characterized by an irrational logic but also a deviant imagination featuring the grotesque, the bizarre, and the monstrous. Then, the following questions can be asked: how does Lu Yang’s media art develop an aberrant aesthetic? What are the visual and sonic functions and components of this aesthetic? What does this aesthetic reveal to us from a philosophical and conceptual point of view, but also from a political point of view? How does this aesthetic offer new critical perspectives on contemporary Chinese art? My analysis of his films and performances distinguishes three different but interrelated modalities of the aberrant aesthetic. The first modality, developed in the first chapter, is related to sex and reproduction through what I define as the aberrant- sex. The second modality is related to the body and his deformations with what I define as the aesthetic of the grotesque. Finally, the third and last modality, represents the mind. It is associated with the preoccupation of the artist for neurosciences and Buddhism and his obsession with the question of control. Then, the third modality designates what I call an aberrant cosmo-technic. The corpus is constituted of the five following artworks: Uterus Man (2013), Delusional Mandala (2015), Electromagnetic Brainology (2017), Electromagnetic Brainology Brain Control Messenger (2018), and Delusional World (2020).

La separación filosófica entre el ser humano y los seres no humanos en Occidente ha resultado en la cosificación de «la naturaleza» y la destrucción del medio ambiente. Este estudio hace dialogar aspectos de la epistemología de «la naturaleza» y su representación artística con el concepto «dark ecology» [ecología oscura] de Timothy Morton (2018), tras un breve paso por la historia contemporánea de las orquídeas y una lectura de varios textos literarios y, en particular, de la novela (traducida al inglés) Aberrant (2017) del escritor checo Marek Šindelka.

Background Dual left anterior descending (LAD) coronary artery is a rare congenital anomaly. To date, eleven variants of dual LAD have been described with three published reports of type X dual LAD. Here, we describe a new variant of type X dual LAD with a short LAD artery masquerading as type 1 LAD. Case presentation A 42-year hypertensive female presented with recent onset angina with a treadmill test positive for inducible ischemia. Coronary angiography showed a normal right coronary artery (RCA). The left main coronary artery (LMCA) originated from the left sinus of Valsalva (SOV), giving rise to a LAD and the left circumflex artery (LCX). Appearing a normal angiogram with type 1 LAD based on its length, the presence of a large bare area in LAD territory (especially at the apex) and lack of septal branches prompted a search for an additional vessel. Right SOV injection showed a vessel originating separately from RCA, which was confirmed to be a long LAD on selective injection, with a pre-pulmonic course and giving rise to septal branches exclusively before wrapping around the apex. Computed tomography coronary angiography (CTCA) confirmed the pre-pulmonic course of long LAD, defined its entry to the distal interventricular septum to the right of short LAD, and ruled out other coronary artery anomalies. In the absence of a stenotic lesion in the epicardial coronaries, angina in our case was presumed to be due to microvascular dysfunction. She was discharged on beta-blocker therapy for co-existing hypertension and is asymptomatic on follow-up at one year. Conclusions A short LAD artery of type X Dual LAD could be potentially misdiagnosed as type 1 LAD based on its length. However, an active search for a long LAD could properly diagnose the case as a variant of type X dual LAD, which has important clinical implications. Its awareness is critical for cardiologists and cardiac surgeons to correctly interpret the coronary angiogram and plan proper management.

Lateral medullary syndrome is a common brainstem stroke associated with a classical triad of Horner’s Syndrome, ipsilateral ataxia and hypalgesia and thermoanasthesia of ipsilateral face. We report a case of a 49-year-old diabetic, non-hypertensive, postmenopausal female who presented with symptoms involving the left dorsal medulla along with right sided hemiparesis and left UMN-type facial palsy. Contralateral hemiparesis was explained by caudal extension of infarct involving the pyramids before decussation at the medulla, known as Babinski–Nageotte Syndrome. UMN-type facial palsy was attributed to involvement of hypothetical supranuclear aberrant corticobulbar fibres of facial nerve which descend down in the contralateral ventromedial medulla, decussate at level of upper medulla and then ascend in the dorsolateral medulla to reach the facial nerve nucleus. Association of these two entities with Wallenberg’s Syndrome have been reported separately in literature, but not together as in this case.

The substantial clinical burden and disability after stroke injury urges the need to explore therapeutic solutions. Recent compelling evidence supports that neurogenesis persists in the adult mammalian brain and is amenable to regulation in both physiological and pathological situations. Its ability to generate new neurons implies a potential to contribute to recovery after brain injury. However, post-stroke neurogenic response may have different functional consequences. On the one hand, the capacity of newborn neurons to replenish the damaged tissue may be limited. In addition, aberrant forms of neurogenesis have been identified in several insult settings. All these data suggest that adult neurogenesis is at a crossroads between the physiological and the pathological regulation of the neurological function in the injured central nervous system (CNS). Given the complexity of the CNS together with its interaction with the periphery, we ultimately lack in-depth understanding of the key cell types, cell-cell interactions, and molecular pathways involved in the neurogenic response after brain damage and their positive or otherwise deleterious impact. Here we will review the evidence on the stroke-induced neurogenic response and on its potential repercussions on functional outcome. First, we will briefly describe subventricular zone (SVZ) neurogenesis after stroke beside the main evidence supporting its positive role on functional restoration after stroke. Then, we will focus on hippocampal subgranular zone (SGZ) neurogenesis due to the relevance of hippocampus in cognitive functions; we will outline compelling evidence that supports that, after stroke, SGZ neurogenesis may adopt a maladaptive plasticity response further contributing to the development of post-stroke cognitive impairment and dementia. Finally, we will discuss the therapeutic potential of specific steps in the neurogenic cascade that might ameliorate brain malfunctioning and the development of post-stroke cognitive impairment in the chronic phase. BACKGROUND In the classic descriptions of the human liver, the common hepatic duct forms at the confluence of left and right hepatic ducts. Many authors have documented variations in the intra-hepatic ductal system, but to the best of our knowledge there has been no report on bile duct variations in Caribbean populations. AIM To evaluate the variations in bile duct anatomy using magnetic resonance cholangiography (MRC) in unselected patients at a major hepatobiliary referral centre in the Eastern Caribbean. Knowledge of the intra-hepatic biliary anatomy is important to optimize service delivery for any physician treating liver and biliary disorders. METHODS This study was carried out at a tertiary referral hospital for hepatobiliary diseases in the Eastern Caribbean. We retrospectively evaluated magnetic resonance cholangiograms in 152 consecutive patients at this facility over a two-year period from April 1, 2017 to March 31, 2019. Two consultant radiologists experienced in MRC interpretation reviewed all scans and described biliary anatomy according to the Huang's classification. A systematic review of published studies was performed and relevant data were extracted in order to calculate the global prevalence of each biliary variant. The variants in our population were compared to the global population. RESULTS There were 152 MRCs evaluated in this study in 86 males and 66 females. There were 109 (71.7%) persons with "classic" biliary anatomy (type A1) and variants were present in 43 (28.3%) persons. There was no statistical relationship between the presence of anatomic variants and gender or ethnicity. We encountered the following variants: 29 (19.1%) type A2, 7 (4.6%) type A3, 6 (3.95%) type A4, 0 type A5 and a single variant (quadrification) that did not fit the classification system. Compared to the global prevalence, our population had a significantly greater occurrence of A1 anatomy (71.7% vs 62.6%; P = 0.0227) and A2 trifurcations (19.1% vs 11.5%; P = 0.0069), but a significantly lower incidence of A3 variants (4.61% vs 11.5%; P = 0.0047). CONCLUSION There are significant differences in intra-hepatic biliary anatomy in this unselected Eastern Caribbean population compared to global statistics. Specifically, persons of Caribbean descent have a greater incidence of Huang A2 trifurcations and a lower incidence of Huang A3 variants.

Background: CD34 antigen is expressed by early hematopoietic progenitor cells and acute leukemia cells. Its expression is associated with good prognosis in acute myeloid leukemia. Literature is sparse on its prognostic significance in B- acute lymphoblastic leukemia (B-ALL) especially from India. Hence the present study was undertaken to analyse the frequency of CD34 expression in B-ALL in Indian patients and determine its prognostic significance by associating with other prognostic markers and aberrant antigen expression. Methods: Seventy-five B-ALL patients diagnosed by flow cytometry over a period of 3½ year were studied. Correlation of CD34 expression was studied with gender, age, total leucocyte count (TLC), French-American-British (FAB) morphological type, immuno-phenotypic markers, cytogenetics and minimal residual disease. Aberrant female and male medusae of Turritopsis rubra and T. sp. (most frequently those with three oral lips and three gonads, but with normal four radial canals) were found (c 13% incidence in female of both species, n = 107, 85) in November, 2016 from Iwaki City, Fukushima Prefecture, Japan. In both species, parasitic trematoda was found; in T. sp. maximally eight individuals/normal medusa and five ones/aberrant medusa. Moreover, in the same place in Fukushima Prefecture and in the other place in Ibaragi Prefecture aberrant Turritopsis rubra mature medusae without oral lips or with 1, 2, 3, 5 oral lips, were also found (18.3 %, n = 142 in Otsu harbour in Ibaragi Prefecture) in October, 2017, 放射能の影響下にある福島県いわき市で2016年11月に同時採集したベニクラゲとニホンベニクラゲの成熟クラゲの雌雄において, 放射管は4本で正常だが口唇や生殖巣が三つの奇形などが出現した(2種の雌で各々約13%の奇形率, n= 107, 85). 2種とも吸虫類の寄生があり, ニホンベニクラゲの正常個体に最多で8個体. 奇形個体には5個体が見られた. 加えて, 福島県の同地点と茨城県の別地点で2017年10月に採集したベニクラゲの成熟クラゲで, 口唇が無いもの, 一つ, 二つ, 三つ. 五つ持つ個体等の奇形個体が出現したので(茨城県大津港で18.3%の奇形率, n= 142), それらの形態も記載した.

Development anomalies of the aortic arch and its major branches are rare congenital cardiovascular disorders. In this article, we present aberrant left subclavian artery associated with right aortic arch.

Background An aberrant right subclavian artery which arises from the proximal descending aorta may result in aortic dissection. The dissection may occur at either the site of the primary intimal tear or from an aortic branch. These conditions may lead to blood flow limitation and possible aneurysmal degeneration in the future. Case presentation We described the clinical presentation and management of a 54-year old patient diagnosed with a rare case of an aberrant right subclavian artery with Stanford Type B aortic dissection. A hybrid surgical approach was successfully performed and the patient had an uneventful recovery. Conclusion Even though aortic dissection is often an incidental finding, this case highlighted that in rare situations, it can be associated with an aberrant right subclavian artery. It is important to disseminate this association as it has profound diagnostic and therapeutic implications in safeguarding the clinical outcomes of patients with such condition.

Propomos com essa dissertação uma análise de como a ficção veiguiana reconstrói figurações do processo de modernização do Brasil na segunda metade do século XX. Objetivamos identificar e examinar como os narradores dos contos “A usina atrás do morro” (1959), “A estranha máquina extraviada” (1967) e do romance A hora dos ruminantes (1966) observam e concebem a modernidade: não como processo material, mas como entidade indiferente e misteriosa. Defendemos a hipótese de que o insólito, em Veiga, é elemento mimético, que representa a forma como populações de vilarejos e cidades isoladas do Brasil conceberam a modernidade: ora como insígnia do progresso e da eliminação da miséria, ora como morte dos antigos costumes e tradições locais. Para alcançar esses objetivos, propomos uma discussão inicial a respeito do insólito em Veiga, a partir dos estudos de Tzvetan Todorov (1992) e Gregório Foganholi Dantas (2002), seguida de uma problematização da relação da literatura de Veiga com questões regionais, com o suporte de Antonio Candido (1989). A aproximação de Veiga, por vias indiretas e ficcionais, com a realidade brasileira do século XX é discutida a partir de Antonio Candido (2000). As diferentes figurações da modernidade que aparecem ao longo da história do Brasil, e que ressoam na voz dos narradores selecionados, são identificadas e discutidas pela perspectiva de Caio Prado Júnior (2011), Raimundo Faoro (1977) e Lúcio Flávio Rodrigues Almeida (2006). We propose with this dissertation an analysis of how ficcion of Veiga rebuilds the figuration of the processo f modernization of Brazil in the second half of the twntieth century. We aim at identifying and examing how the narrators of the short stories “A usina atrás do morro” (1959), “A estranha máquina extraviada” (1967) and the novel A hora dos ruminantes (1966) observe and conceive modernity: not as a material process, bu as na indiferente and misterious entity. We defend the hypothesis that aberrant, in Veiga, is a mimetic elemento, which representsthe way populations of villages and isolated cities in Brazil conceived modernity: sometimes as a sign of progress and elimination of misery, sometimes as the destruction of old habbits and local traditions. To reach these purposes, we propose na inicial discussion about the aberrant in Veiga, based on the studies of Tzvetan Todorov (1992) and Gregório Foganholi Dantas (2002), followed by a problematization of the relation between Veiga’s literature with regional questions, with the support of Antonio Candido (1989). The nearness of Veiga, through indirect and fictional means, and Brazilian’s reality of twentieth century is discussed through Antonio Candido (2000). The diferente figurations of modernity which appear along Brazilian history, and that resound in the voices of the selected narrators, are identified and discussed through the perspective of Caio Prado Júnior (2011), Raimundo Faoro (1977) and Lúcio Flávio Rodrigues Almeida (2006).

BACKGROUND: Aberrant antigen expression is an unexpected antigen that is detected on lineage in question without alteration of lineage definition. Immunophenotypic aberrancies are thought to predict treatment outcome in acute myeloid leukemia (AML). Some studies reported an adverse prognostic association, while others failed to show any association. AIM: To determine percentage of lymphoid antigens expression in adult AML patients and correlate them with hematological parameters and response to induction chemotherapy protocol. SUBJECTS AND METHODS: Peripheral blood and bone marrow fresh samples from 81 cases of de novo AML were included in this study from November 2018 to October 2019. Hematological parameters were obtained from patients' medical records. Aberrant antigens expression was detected using flow cytometry at the diagnosis. Patients were evaluated after one cycle of chemotherapy regarding remission status. RESULTS: Out of 81 patients, 60 patients had completed data for evaluation. CD7, CD19 expression, and co-expression of CD7 and CD2 were found in 60%, 10.7%, and 10.7% of patients, respectively. Statistically significant associations were found between initial hemoglobin level and aberrant expression as patients with aberrant expression have higher hemoglobin level than patients without aberrant expression. Complete remission was achieved in 29 out of 60 patients (48.3%) with standard 3 and 7 protocol, whereas 31 patients did not achieve complete remission. There is no statistically significant association between aberrant expression and treatment response to the first cycle of chemotherapy protocol (P > 0.05). CONCLUSIONS: CD7 is the most frequent aberrant antigen expressed in this Iraqi AML patient group. Hemoglobin level was higher in patients with aberrant expression. No significant association between aberrant expression and response to the first cycle of induction.

Background: Ectopic breast tissue (EBT) develops along the mammary line owing to incomplete embryologic regression of the mammary ridges. This includes supernumerary breasts and aberrant breast tissue, in which malignancy is rarely reported. The commonest site of ectopic breasts is at the axilla, and the subcutaneous axillary mass may pose a diagnostic challenge to the clinician. Case Report: We report the case of a 31-year-old multiparous Nigerian woman who presented with a painless left axillary mass of two months' duration. The anatomical breasts were grossly and radiologically normal. A diagnosis of aberrant breast carcinoma in her left axilla was confirmed by tissue biopsy. She had wide local excision and left axillary dissection, followed by one course of cytotoxic chemotherapy before she began radiotherapy. Conclusion: Malignancy of aberrant breast tissue is a rare entity. A high index of suspicion and a low threshold for biopsy of subcutaneous lesions in the periphery of the breast allows for early intervention and a better prognosis. Triple assessment with clinical, radiological and pathological assessment of lesions in the axilla or along the embryonic milk line can not be overemphasized

Biliary anatomy and its common and uncommon variations are of considerable clinical significance when performing living donor transplantation, radiological interventions in hepatobiliary system, laparoscopic cholecystectomy, and liver resection (hepatectomy, segmentectomy). Because of increasing trend found in the number of liver transplant surgeries being performed, magnetic resonance cholangiopancreatography (MRCP) has become the modality of choice for noninvasive evaluation of abnormalities of the biliary tract. The purpose of this study is to describe the anatomic variations of the intrahepatic and extrahepatic biliary tree.

Lacrimal fistula can occur due to trauma, iatrogenic or congenital. The most common presentation in patients with congenital lacrimal fistula is epiphora or purulent discharge due to cystic dilatation from secondary infection.

Background The presentation of patients with knee injuries which can have a significant impact on their ability to work and perform activities of daily living is constantly rising. The posterolateral corner of the knee has a complex anatomy of muscles, tendons, and ligaments, with huge variation in the population. The fabella is one such structure, found in the posterolateral corner of the knee, which can serve as a common origin point of various ligaments. Case report We present a case report of a 53-year-old white man who presented with atraumatic, posterior knee pain and was found to have a congenital, anomalous band originating from the fabella, causing semimembranosus impingement. This was diagnosed with magnetic resonance imaging; he underwent division of the anomalous band, which resulted in complete resolution of his symptoms. Conclusion We propose that patients who present with posterior knee pain, without any history of trauma, and have no abnormalities on plain radiographs, should undergo magnetic resonance imaging of their knees. This will help in assessing the ligament complex in the posterior compartment of the knee, and exclude impingement of the semimembranosus as an, albeit rare, cause of posteromedial knee pain.

SUMMARY For the past decade, cancer genomic studies have focused on mutations leading to splice-site disruption, overlooking those having splice-creating potential. Here, we applied a bioinformatic tool, MiSplice, for the large-scale discovery of splice-site-creating mutations (SCMs) across 8,656 TCGA tumors. We report 1,964 originally mis-annotated mutations having clear evidence of creating alternative splice junctions. TP53 and GATA3 have 26 and 18 SCMs, respectively, and ATRX has 5 from lower-grade gliomas. Mutations in 11 genes, including PARP1, BRCA1, and BAP1, were experimentally validated for splice-site-creating function. Notably, we found that neoantigens induced by SCMs are likely several folds more immunogenic compared to missense mutations, exemplified by the recurrent GATA3 SCM. Further, high expression of PD-1 and PD-L1 was observed in tumors with SCMs, suggesting candidates for immune blockade therapy. Our work highlights the importance of integrating DNA and RNA data for understanding the functional and the clinical implications of mutations in human diseases., In Brief Jayasinghe et al. identify nearly 2,000 splice-site-creating mutations (SCMs) from over 8,000 tumor samples across 33 cancer types. They provide a more accurate interpretation of previously misannotated mutations, highlighting the importance of integrating data types to understand the functional and the clinical implications of splicing mutations in human disease.

Somatic cell nuclear transfer (SCNT) is a promising technique to produce mammalian transgenic clones. Only a small proportion of manipulated embryos, however, can develop into viable offspring. The abnormal growth and development of cloned animals, furthermore, are accompanied by aberrant lung development. Our objective was to investigate molecular background of lung developmental problems in transgenic (random insertion of exogenous DNA) cloned goats. We examined expression of 15 genes involved in growth regulation, imprinting, and epigenetic transcription in lung tissue of deceased transgenic cloned and normal goats of various ages. Compared with normal goats of the same age from conventional reproduction, expression of 13 genes (BMP4, FGF10, GHR, HGFR, PDGFR, RABP, VEGF, H19, CDKNIC, PCAF, MeCP2, HDAC1, and Dnmt3b) decreased in transgenic cloned goats that died at or shortly after birth; Expression of eight genes (FGF10, PDGFR, RABP, VEGF, PCAF, HDAC1, MeCP2, and Dnmt3b) decreased in fetal death of transgenic cloned goats. Expression of two epigenetic transcription genes (PCAF and Dnmt3b) decreased in disease death of transgenic cloned goats (1-4 months old). Disruptions in gene expression might be associated with the high neonatal mortality in transgenic cloned animals. Compression of the esophagus by a retroesophageal aberrant right subclavian artery (ARSA) is a rare cause of dysphagia. We present the case of a 47-year-old female with symptoms of progressive dysphagia diagnosed with dysphagia lusoria using barium swallow and contrast computed tomography and successfully treated with a hybrid procedure: right carotid to subclavian bypass and endovascular insertion of an Amplatzer II Vascular Plug through the right superficial femoral artery. We consider this approach safer, less invasive and more complete to avoid recurrent dysphagia.

Aberrant glycosylation is often cited as a ‘hallmark of cancer’ but is notably absent from both the original hallmarks of cancer and from the next generation of emerging hallmarks. This review discusses how glycosylation is clearly an enabling characteristic that is causally associated with the acquisition of all the hallmark capabilities. Rather than aberrant glycosylation being itself a hallmark of cancer, another perspective is that glycans play a role in every recognised cancer hallmark.

Introduction Variations in the Right Hepatic Artery (RHA) are exceedingly common and these variations are of great importance to surgeons and interventional radiologists. A thorough knowledge of the right hepatic arterial anatomy is mandatory while performing hepatic surgery and hepatic arteriography. Aim To find out the variations in the origin and course of the RHA and its surgical significance. Materials and methods This study was conducted in 60 embalmed adult cadavers of both sexes of age group between 50 to 80 years. Abdomen was opened by a linear midline incision extending from the xiphoid process to the pubic symphysis. The origin of RHA from the proper hepatic artery was traced. The presence of aberrant right hepatic arteries -replaced and accessory were noted. The source of origin of accessory or replaced RHA arising from the superior mesenteric artery (SMA), right renal artery and inferior mesenteric artery was traced. The course of normal and aberrant RHA from its origin to the entrance into the right lobe of the liver was followed and structures related to it were noted. Results The RHA mostly originated from the main trunk of the proper hepatic artery in 52 specimens (86.6%). Presence of aberrant RHA was found in 8 specimens (13.3%). Among 8 specimens of aberrant RHA, replaced RHA arising from SMA was found in 5 specimens (8.3%) and accessory RHA arising from SMA was observed in 3 specimens (5%). In 86.6% of specimens, the RHA coursed dorsal to the duct system to enter the Calot's triangle. In 8.3% of specimens, the RHA coursed ventral to duct system to reach the Calot's triangle. The caterpillar like loop of the RHA was found in two specimens (3%). The replaced and accessory RHA (13.3%) arising from the SMA had a course dorsal to duct system to reach the Calot's triangle. Conclusion Right hepatic artery is subject to anatomical variation in its origin and course. Our study identified the variations in the origin and course of right hepatic artery. Adequate knowledge of these variations would be of incredible help to the hepatobiliary surgeon and interventional radiologist.

Adenocarcinoma arising from jejunal ectopic pancreas is very rare. We report a case of a 69-year-old female with adenocarcinoma arising from jejunal ectopic pancreas after resection of advanced colon cancer. She underwent right hemicolectomy for advanced ascending colon cancer (ypT3N0M0, stage IIA) after chemotherapy. Two and half years after colectomy, her tumor markers were elevated, and computed tomography revealed a mass measuring 20 × 20 mm in the small intestine, having an abnormal uptake of (18)F-fluorodeoxyglucose on (18)F-fluorodeoxyglucose-positron emission tomography ((18)FDG-PET). Double-balloon enteroscopy revealed a submucosal tumor in the jejunum, and histopathology of biopsy specimens from that lesion showed ectopic pancreas without malignancy. Therefore, peritoneal metastasis from colon cancer concomitant with ectopic pancreas or adenocarcinoma arising from ectopic pancreas was considered as a differential diagnosis. She underwent laparoscopic jejunectomy. Pathological examination revealed a moderately differentiated adenocarcinoma arising from jejunal ectopic pancreas, not peritoneal metastasis from colon cancer. Even if histopathology of the biopsy specimen shows ectopic pancreas without malignancy, adenocarcinoma arising from ectopic pancreas should be considered when the tumor markers are elevated or the lesion has an abnormal uptake of (18)FDG.

Farenks kitlelerinde kitle üzerinde pulsasyon mutlaka değerlendirilmeli ve eğer varsa, aberan internal karotis arter akılda bulundurulmalıdır. Bilateral pharyngeal internal carotid artery aberration is a rarely seen variation which poses a risk during the surgical interventions of this area. A 74-yearold male patient was admitted to our clinic with the complaints of a sensation of fullness in his throat and dysphagia. Oropharyngeal examination revealed bilateral smooth-surfaced masses in the posterior pharyngeal wall. Magnetic resonance imaging showed an aberrant internal carotid artery. The patient was followed up with necessary warnings. The pulsation on the pharyngeal masses should be definitely evaluated and aberrant internal carotid artery should be kept in mind, if present

Pre-eruptive migration of a tooth across the midline is termed as transmigration. It is believed that transmigration is rare and unique to the mandibular permanent canines, and even more rarely reported for others. Transmigration is a phenomenon of yet unknown etiology. It follows the direction of its long axis, with the crown leading the migration. The tendency of a canine to cross the barrier of mandibular midline suture is a more important consideration than the distance of migration after crossing the midline. Here we present one new case of aberrantly positioned right mandibular canine which has undergone migration and was accidently found on radiological examination before orthodontic treatment. Once diagnosed an aberrantly positioned impacted canine requires surgical removal.

Background: Leukemic blasts in acute lymphoblastic leukemia (ALL) may have immunological features of both lymphoid and myeloid lineages known as aberrant myeloid antigens expression in ALL, which are explained as abnormal genetic program of leukemic cells that lead to lineage infidelity. In this study, CD13, CD14, and CD33 (which are the most frequent myeloid antigens associated with aberrant antigens expression in ALL) were investigated. Objectives: To evaluate the occurrence of aberrant myeloid antigens expression in childhood ALL and its effects on complete remission and other parameters. Materials and Methods: This study was conducted on 31 pediatric patients with newly diagnosed de novo ALL (27 B-ALL, 4 T-ALL) in Children Welfare Teaching Hospital/Medical City of Baghdad; diagnosis of ALL based on morphology and cytochemistry, CD13, CD14, CD33 were investigated as myeloid antigens using four-color flow cytometry. Results: Five cases of ALL (16.13%) out of 31 cases were confirmed to have aberrant myeloid antigens expression (CD13 was expressed in all five cases, CD33 was expressed in three cases, and CD14 was not expressed in any of these five cases). Complete remission was achieved in 90.30% (28 patients) and all cases with aberrant myeloid antigens expression achieved complete remission; however, despite this, there was no significant difference in complete remission between myeloid-positive (MY+) and myeloid-negative (MY−) cases, P > 0.05. Regarding other parameters, there were significant statistical differences in lactate dehydrogenase (LDH), hemoglobin value, and bone marrow (B.M.) blasts percent at diagnosis between MY+ and MY− cases, P< 0.05; however, there was no significant differences in leukocytes count, platelets count, peripheral blood blast percent at diagnosis, age, and gender, P > 0.05. Conclusions: The most frequent aberrant myeloid antigens expression in childhood ALL is CD13 and less frequently is CD33 while CD14 showed no expression; myeloid antigens expression in ALL may have better prognosis as they have lower B.M. blast percent and LDH value.

We have described a rare group of prostate adenocarcinomas that show aberrant expression of p63, a protein strongly expressed in prostatic basal cells and absent from usual-type acinar prostate cancers. The partial basal-like immunophenotype of these tumors is intriguing in light of the persistent debate surrounding the cell-of-origin for prostate cancer; however, their molecular phenotype is unknown. We collected 37 of these tumors on radical prostatectomy and biopsy and assessed subsets for a diverse panel of molecular markers. The majority of p63-expressing tumors were positive for the ΔNp63 isoform (6/7) by immunofluorescence and p63 mRNA (7/8) by chromogenic in situ hybridization. Despite p63 positivity, these tumors uniformly expressed luminal-type cytokeratin proteins such as CK18 (13/13), CK8 (8/8), and markers of androgen axis signaling commonly seen in luminal cells, including androgen receptor (10/11), NKX3.1 (8/8), and prostein (12/13). Conversely, basal cytokeratins such as CK14 and CK15 were negative in all cases (0/8) and CK5/6 was weakly and focally positive in 36% (4/11) of cases. Pluripotency markers including β-catenin, Oct4, and c-kit were negative in p63-expressing tumors (0/11). Despite nearly universal expression of androgen receptor and downstream androgen signaling targets, p63-expressing tumors lacked ERG rearrangements by fluorescence in situ hybridization (0/14) and ERG protein expression (0/37). No tumors expressed SPINK1 or showed PTEN protein loss (0/19). Surprisingly, 74% (14/19) of p63-expressing tumors expressed GSTP1 protein at least focally, and 33% (2/6) entirely lacked GSTP1 CpG island hypermethylation by bisulfite sequencing. In contrast to usual prostatic adenocarcinomas, prostate tumors with p63 expression show a mixed luminal/basal immunophenotype, uniformly lack ERG gene rearrangement, and frequently express GSTP1. These data strongly suggest that p63-expressing prostate tumors represent a molecularly distinct subclass and further study of this rare tumor type may yield important insights into the role of p63 in prostatic biology and the prostate cancer cell-of-origin.

The patient was a 71-year-old who had received laparoscopic cholecystectomy for cholecystitis due to cholecystolithiasis on November, 1993. He had spent with no complaint after operation. But he consulted our department with epigastralgia and jaundice on February 1997. Abdominal CT and DIC revealed one calculus-like shadowy defect in the common bile duct and demonstrated metallic density at the defect in the lower part of the common bile duct. We diagnosed postoperative bile duct stone by aberrant surgical clip after laparoscopic cholecystectomy. We performed endoscopic balloon dilation of duodenal papilla and removed the common bile duct stone using basket forceps. In Japan, eight cases of migration of clip after laparoscopic cholecystectomy into bile duct have been reported, including our case. In accordance with the prevalence of laparoscopic cholecystectomy, such cases will increase and it was suggested that we must observe the course over a long period of time.

We present the case of a patient who exhibited continued flow in the distal internal carotid artery (ICA) despite complete occlusion at the carotid bifurcation. Patency was maintained by the presence of an aberrant branch distal to the occlusion.

A morphological peculiarity was observed in the form of an aberrant lateral bony attachment of the orbital part of the Orbicularis oculi muscle on the zygomatic bone, during routine dissection of a cadaver of an adult male of Indian origin. Fibers of this part of the muscle are not known to show any lateral attachment on bone. This paper discusses the presentation, probable embryological cause and clinical implications of this unusual finding.

Recombinant glycoproteins can be produced at high levels in permanently transfected mammalian cells using expression vectors with strong viral promoters. CHO-K1 cell lines developed to produce the recombinant complement activator blocking protein, CAB-2 (a fusion of membrane co-factor protein, MCP, and decay accelerating factor, DAF), showed unexpectedly low expression. Northern blot analysis revealed that in addition to the expected 2300 base CAB-2 mRNA species, these cell lines expressed 790 and 1500 base mRNA species accounting for ∼50% and ∼10% of the total CAB-2 mRNA, respectively. RT-PCR studies established that the 1500 base species resulted from aberrant splicing from within the DAF region of the CAB-2 coding sequence to a site within the 3′ untranslated region. 3′ RACE analysis confirmed that the 790 base species resulted from premature polyadenylation at an AATAAA site within the MCP coding region of CAB-2. Another prematurely polyadenylated species, not observed on Northern blots, was observed in the DAF region by 3′ RACE. Analysis of human tissues and cell lines revealed that these internal polyadenylation signals in native MCP and DAF coding regions also generated prematurely polyadenylated mRNAs. Genetic modification of these functional RNA processing elements within the CAB-2 gene eliminated the aberrant mRNA species and significantly increased recombinant CAB-2 expression. These results illustrate that protein expression can be limited by aberrant mRNA processing and demonstrate the importance of identifying and eliminating these mRNA processing signals from within coding DNA to maximize recombinant protein expression.

A middle-aged woman with the chief complaints of dysnea and chest pain was referred to our center. Chest X-rays and computed tomoram revealed a mass in the posteriof mediastinum, which was excised in toto. Pathologicat report of the specimen labeled it as normal thyroid tissue (aberrant thyroid).
