RESUMO Objetivo A terapia cognitivo-comportamental (TCC) tem eficácia bem-documentada na literatura científica para transtornos relacionados aos sintomas da ansiedade. No entanto, há uma parcela de pacientes que não responde ao tratamento psicoterápico. Por isso, os estudos sobre as alterações no córtex cingulado anterior (CCA) como preditoras neurais do tratamento têm contribuído para encontrar respostas sobre as diferenças nas respostas ao tratamento. O objetivo do presente estudo é descrever, por meio de revisão sistemática, os estudos encontrados até o ano de 2018 sobre o papel do CCA na predição de resposta à terapia. Métodos Foram realizadas buscas nas bases PsycInfo, Web of Science e PubMed com termos referentes ao tema “córtex cingulado anterior”, “terapia cognitivo-comportamental” e “predição de respostas”, incluindo estudos com neuroimagem estrutural e funcional. Resultados As buscas apresentaram 14 artigos sobre “transtorno de estresse pós-traumático (TEPT)”, “transtorno obsessivo-compulsivo (TOC)” e “transtorno de ansiedade social (TAS)”. Os estudos com neuroimagem estrutural apresentaram resultados promissores. A maior espessura do CCA foi preditora de melhor resposta ao tratamento para TEPT e TOC. Os resultados de neuroimagem funcional foram promissores para maior ativação como preditora de melhor resposta para TAS. Por outro lado, os resultados para TEPT apontaram que a menor ativação pode ser preditora de melhores respostas. Conclusão As alterações nos estudos de neuroimagem sugerem que o CCA tenha um papel de predição de resposta ao tratamento com TCC. Estudos posteriores com amostras maiores podem contribuir para a ampliação da eficácia nos tratamentos de tais transtornos. ABSTRACT Objective The efficacy of cognitive-behavioral therapy (CBT) on the treatment of anxiety-related disorders has been well documented. However, a number of patients do not respond to psychotherapeutic treatment. Therefore, changes in the anterior cingulate cortex (ACC) as a neural predictor of treatment response have contributed to understanding the differences in treatment outcome. The aim of this study is to describe, through a systematic review, studies published until 2018 that investigate the role of the anterior cingulate cortex on the prediction of response to therapy. Methods Searches have been conducted in the PsycInfo, Web of Science and PubMed databases for articles related to the terms “anterior cingulate cortex”, “cognitive-behavioral therapy” and “prediction of response”, including studies with structural and functional neuroimaging. Results We selected 14 articles on “post-traumatic stress disorder (PTSD)”, “obsessive-compulsive disorder (OCD)” and “social anxiety disorder (SAD)”. Overall, Structural neuroimaging studies functional neuroimaging results were promising. A greater thickness on the ACC was associated with a better response to treatment for PTSD and OCD. Greater activation of the ACC was positively associated with a greater response to treatment for patients with SAD. On the other hand, for those with PTSD, lower activation may be a better predictor of improvement. Conclusion The structural and functional alterations observed in neuroimage studies suggest that the ACC has a role in predicting treatment response to CBT. Future studies with larger samples may contribute to the improvement of treatment efficacy in such disorders.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES Fundação de Apoio a Pesquisa e à Inovação Tecnológica do Estado de Sergipe - FAPITEC/SE The high prevalence of anxiety as a disorder and its important socioeconomic impact in the world has been highlighted. Furthermore, its neurobiology is still a challenge. In recent decades, the neural circuitry involved in the state anxiety has been identified. State anxiety is the anxiety a subject experiences when faced with a threatening stimulus at a particular moment in time. However, little is known about trait anxiety, a relatively stable personality trait and a predisposing factor for anxiety disorders. The brain structure that has been related to the anxious trait is the medial prefrontal cortex. However, few studies show differentiation of roles of their subnuclei, so that the anterior cingulate cortex (ACC) would be involved with the trait. Thus, knowing that neurotransmitters such as serotonin, gamma aminobutyric acid (GABA) and glutamate are involved in anxiety, the present study investigated the participation of these neurotransmitter systems in ACC in the trait anxiety of adult Wistar rats. As, by definition, trait anxiety modulates state anxiety, this was also evaluated. For the behavioral assessment, three animal models were used: 1) Free Exploratory Paradigm (FEP), animal model of trait anxiety; 2) Hole board (HB); and 3) Elevated plus maze (EPM), state anxiety models. Initially, the animals were categorized according to their trait anxiety levels (high, low and medium) (FEP1) and allocated to experiments. Exp I used high; Exp II used low; Exp III used medium trait anxiety animals. The animals were again submitted to FEP (FEP2) and HB, and before each behavioral assessment were administered in ACC drugs that increased or decreased serotonergic (Fluoxetine or WAY-100635), GABAergic (Muscimol or Bicuculline) and glutamatergic (NMDA or Ketamine) and their respective controls. Moreover, in Exp IV, all animals from previous experiments were submitted to EPM, and evaluated without taking into account trait anxiety levels. At the end of the experiments, the histological analysis of the drug administration site was performed. All data were analyzed using analysis of variance and a posteriori Tukey test. The results of the present study showed that, in Exp I, the modulation of the serotonergic, GABAergic and glutamatergic systems in the ACC decreased trait anxiety in highly anxious rats, while by submitting the animals to HB, the administration of Fluoxetine increased state anxiety. In Exp II, the modulation of all systems increased trait anxiety in rats with low trait anxiety, whereas, in HB, state anxiety levels were increased with the administration of NMDA. In Exp III, only the modulation of the glutamatergic system, with NMDA, increased both trait and state anxiety levels. However, none of the evaluated neurotransmitter systems altered the state anxiety modeled in the EPM. Thus, the present study showed, for the first time, the participation of the serotonergic, GABAergic and glutamatergic systems in the ACC in the trait anxiety of Wistar rats with different levels of anxiety. Furthermore, we showed that ACC is also involved with the modulation of the state anxiety. A ansiedade, como transtorno, se destaca por sua alta prevalência e importante impacto sócio-econômico no mundo. Além disso, sua neurobiologia ainda é um desafio. Nas últimas décadas, tem-se identificado a circuitaria neural envolvida no estado ansioso (ansiedade-estado), estado transitório quando confrontado com um estímulo ameaçador. Porém, pouco se sabe a respeito da ansiedade-traço, traço de personalidade relativamente estável e fator predisponente para os transtornos ansiosos. Uma estrutura encefálica que tem sido relacionada ao traço ansioso é o córtex pré-frontal medial. Entretanto, poucos estudos mostram diferenciação de papéis de seus subnúcleos, e, dentre eles, há mais evidências sobre o envolvimento do córtex cingulado anterior (CCA) com o traço. Desse modo, sabendo-se que neurotransmissores como serotonina, ácido gama-aminobutírico (GABA) e glutamato são conhecidamente envolvidos na ansiedade, o presente estudo investigou a participação desses sistemas de neurotransmissores no CCA na ansiedade-traço de ratos Wistar adultos. Como, por definição, a ansiedade-traço modula a ansiedade-estado, esta também foi avaliada. Para a avaliação comportamental foram utilizados três modelos animais: 1) Paradigma da Exploração Livre (PEL), modelo animal de ansiedade-traço; 2) Placa Perfurada (PP); e 3) Labirinto em Cruz Elevado (LCE), modelos de ansiedade-estado. Inicialmente, os animais foram categorizados de acordo com seus níveis de ansiedade-traço (alta, baixa e média) (PEL1) e alocados em experimentos, a saber: Exp I que utilizou animais de alta; Exp II, animais de baixa; Exp III, animais de média-ansiedade-traço. Os animais foram submetidos novamente ao PEL (PEL2) e à PP, sendo que antes de cada avaliação comportamental foram administradas drogas (no CCA) que aumentavam ou diminuíam a neurotransmissão serotonérgica (Fluoxetina ou WAY-100635), GABAérgica (Muscimol ou Bicuculina) e glutamatérgica (NMDA ou Ketamina) e seus respectivos controles. E no Exp IV, todos os animais dos experimentos anteriores foram submetidos ao LCE e avaliados sem levar em consideração os níveis de ansiedade-traço. Ao final dos experimentos, foi realizada a análise histológica do sítio de administração da droga. Todos os dados foram analisados por meio de análise de variância e teste a posteriori de Tukey. Os resultados do presente estudo mostraram que, no Exp I, a modulação dos sistemas serotoninérgico, GABAérgico e glutamatérgico no CCA diminuiu a ansiedade-traço de ratos altamente ansiosos, enquanto ao submeter os animais à PP, a administração da Fluoxetina aumentou a ansiedade-estado dos mesmos. No Exp II, a modulação de todos os sistemas aumentou a ansiedade-traço de ratos com baixo traço ansioso, ao passo que, na PP, os níveis de ansiedade-estado foram aumentados com a administração do NMDA. Já no Exp III, apenas a modulação do sistema glutamatérgico, com o NMDA, aumentou tanto os níveis de ansiedade traço quanto os níveis de ansiedade-estado. No entanto, nenhum sistema de neurotransmissores avaliados alterou o estado ansioso modelado no LCE. Sendo assim, o presente estudo mostrou, pela primeira vez, a participação dos sistemas serotoninérgico, GABAérgico e glutamatérgico no CCA na ansiedade-traço de ratos Wistar com diferentes níveis de ansiedade, além de evidenciar que CCA também está envolvido com a modulação do estado ansioso. Não recebi financiamento Empathy is the ability to recognize the other's emotional state, being fundamental to social interaction and survival of the species. The study of empathy in rodents can be carried out using models that assess anxiety responses promoted by cohabitation with a chronic stress partner. Evidence has shown the amygdala and anterior cingulate cortex (ACC) involvement in anxiety-like behaviors and empathy. The present study investigated the role of the amygdala and the ACC in anxiety induced by living with a conspecific subjected to chronic restraint stress. Male Swiss mice were housed in pairs and lived for 28 days. On the 15th day, they were divided into two groups: stress and control. The stress group consisted of the stress and stress observer subjects (they shared the same housing box), being that the stress subject underwent stress sessions in the tube for 14 days for 1 hour in the presence of their partner living. The control group was presented by the control and observer control subjects (housed in the same housing box); none of the animals experienced stress. In the 29th day, in experiment 1, the stress observer and control observer subjects were submitted to the immunofluorescence protocol to verify neuronal activation by quantifying of FosB protein in the amygdala and ACC. In experiment 2, on the 24th day, the stress observer and control observer subjects underwent stereotaxic surgery to implant guide cannulas in the amygdala and ACC. On the 29th day, the animals received intra-amygdala or intra-ACC injection of saline or CoCl2 and, after 5 minutes, were evaluated in the elevated plus maze (EPM). The results revealed that living with conspecific in restraint stress did not alter the expression of positive cells for the Fos B protein in the amygdala; however, it promoted a decrease in FosB quantification in the ACC compared to animals that lived with control. Living with a pair in chronic stress led to a reduction in the percentage of entries (%OAE) and time spent in the open arms (% OAT), without changing the entries in the closed arms (CAE) of the EPM, in addition to the increase in% SAP (stretched attend posture) protected and the percentage of protected head-dipping. Amygdala inactivation with CoCl2 did not change %OAE, %OAT, and CAE. In contrast, the inactivation of ACC with CoCl2 displayed an increase in %OAE and %OAT, without altering CAE. Also, there was a decrease in protected head-dipping in animals that lived with a pair in chronic stress. Our results suggest that living with a pair submitted to chronic restraint stress raised increased responses related to anxiety in mice exposed to EPM. Amygdala inactivation was not able to mitigate the anxiety induced by living together; however, the inactivation of the ACC attenuated the anxiety like-behavior. These results are suggestive that ACC plays a significant role in anxiety induced by living with a pair subjected to chronic stress. O estudo da empatia em roedores pode ser realizado por meio de modelos que avaliam respostas de ansiedade promovidas pela coabitação com um parceiro em estresse crônico. Evidências demonstraram o envolvimento da amígdala e do córtex cingulado anterior (CCA) nos comportamentos relacionados a ansiedade e na empatia. O presente trabalho investigou o papel da amígdala e do CCA na ansiedade induzida pelo convívio com parceiro submetido ao estresse crônico de contenção. Camundongos machos da linhagem Suíço-albinos foram alojados aos pares e conviveram por 28 dias. No 15 dia, foram divididos em dois grupos: estresse e controle. O grupo estresse foi constituído pelos sujeitos estresse e observador estresse (alojados na mesma caixa-moradia), sendo que o sujeito estresse passou por sessões de estresse no tubo por 14 dias durante 1 hora, na presença de seu parceiro de caixa. O grupo controle foi composto pelos sujeitos controle e observador controle (alojados na mesma caixa-moradia), sendo que nenhum dos animais passou por estresse. No 29 dia, no experimento 1, os sujeitos observador estresse e observador controle foram submetidos ao protocolo de imunofluorescência para verificar a ativação neuronal, através da quantificação de proteína FosB na amígdala e no CCA. No experimento 2, no 24º dia, os sujeitos observador estresse e observador controle foram submetidos à cirurgia estereotáxica para implantação de cânulas-guia na amígdala e CCA. No 29º dia, os animais receberam injeção intra-amígdala ou intra-CCA de salina ou CoCl2 e, após 5 minutos, foram avaliados no labirinto em cruz elevado (LCE). Os resultados demonstraram que convívio com coespecífico em estresse de contenção não alterou a expressão de células positivas para a proteína FosB na amígdala; entretanto promoveu diminuição na quantificação de FosB no CCA comparados aos animais que conviveram com controle. O convívio com parceiro submetido ao estresse crônico levou a diminuição da porcentagem de entradas (%EBA) e de tempo gasto nos braços abertos (%TBA), sem alterar as entradas nos braços fechados (EBF) do LCE, além do aumento de %SAP (stretched attend posture) protegido e da porcentagem de mergulho protegido. A inativação da amígdala com CoCl2 não alterou a %EBA, a %TBA e as EBF, enquanto a inativação do CCA com CoCl2 produziu aumento da %EBA e %TBA, sem alterar as EBF, além disso houve diminuição da porcentagem de mergulho protegido nos animais que conviveram com parceiro em estresse crônico. Nossos resultados sugerem que o convívio com parceiro submetido ao estresse crônico de contenção promoveu aumento das respostas relacionadas à ansiedade em camundongos expostos ao LCE. A inativação da amígdala não foi capaz de atenuar a ansiedade promovida pelo convívio, entretanto a inativação do CCA atenuou a ansiedade dos animais. Os resultados sugerem que o CCA desempenha um importante papel na ansiedade induzida pelo convívio com parceiro submetido ao estresse crônico.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) Empathy is characterized by the ability to recognize and respond to the emotional signals of others, being a crucial factor in the emotional experience and social interaction, allowing humans, as well as other species, to detect affective states of others. In this sense, recent studies have shown that cohabitation with an animal with chronic pain induces increased nociception and anxiety in the partner, that is, in the observer. These results reinforce the assumption that responses related to anxiety make it possible to identify the existence of emotional responses, which may be related to empathy. Evidence has suggested the involvement of the anterior cingulate cortex (ACC) and insula in the modulation of anxiety. However, no study has evaluated whether these structures participate in the increased anxiety factor observed in mice living with their partner with chronic pain. Thus, the aim of this study was to investigate the effects of reversible inactivation of ACC and insula on the modulation of anxiety induced by the empathy model in mice exposed to the elevated plus-maze (EPM). For this, male Swiss mice (n = 10-12/group), 21 days after birth, were housed in pairs to establish familiarity. On the 14th day of cohabitation, one of the animals of the pair underwent surgery for sciatic nerve constriction (NC) or surgery without nerve constriction (Sham). On the 23rd day, cagemates were submitted to stereotactic surgery for implantation of guide cannulas directed to the CCA (experiment 1) or to the insula (experiment 2). On the 28th day, cagemates (CNC and CS) that lived with their respective NC and Sham groups received bilateral intra-CCA or intra-insular saline or cobalt chloride (CoCl2) (reversible and non-selective synaptic blocker) injections, and after five minutes they were exposed to the EPM. The t test for independent samples showed that in Experiments 1 and 2, the NC mice submitted to the hot plate test showed decreased paw withdrawal latency when compared to the Sham mice. The two-way analysis of variance (ANOVA) followed by the Duncan test revealed that cohabitation with nociception mice produced an increased anxiety response in observers, confirming previous results in the literature. The chemical inactivation of the CCA and the insula produced an increased percentage of entries and time spent in the open arms, without altering the entries in the closed arms of the LCE. Furthermore, it also decreased the display of risk assessment behaviors, such as percentage of time in the center, percentage of head-dipping and percentage of protected stretched attend postures and total of stretched attend postures in experiment 1; and in experiment 2, decreased the percentage of head-dipping and protected stretched attend postures, and the total of head-dipping and stretched attend postures, which suggests a decrease in anxiety-like behaviors. The results suggest that the inactivation of the CCA and the insula reverses the display of anxiety-like behaviors in mice submitted to the empathy model evaluated in the LCE. In addition, they indicate that these structures are essential for the display of anxious behaviors induced by emotional contagion, assessed through cohabitation with animals subjected to neuropathic pain. A empatia é expressa pela capacidade em captar sinais emocionais em outros indivíduos, sendo um fator crucial na vivência emocional e interação social, permitindo que humanos, assim como outras espécies, possam detectar os estados afetivos alheios. Neste sentido, estudos recentes têm demostrado que o convívio com o animal com dor crônica produz aumento da nocicepção e ansiedade no parceiro, ou seja, no observador. Esses resultados fortalecem o pressuposto de que respostas referentes à ansiedade possibilitam identificar a existência de respostas emocionais, o que pode estar relacionado à empatia. Evidências apontam para a participação do córtex cingulado anterior (CCA) e da ínsula na modulação da ansiedade. Entretanto, nenhum estudo avaliou se estas estruturas participam do aumento de ansiedade observado em camundongos que convivem com o parceiro com dor crônica. Diante do exposto, o objetivo deste estudo foi investigar os efeitos da inativação reversível do CCA e da ínsula na modulação da ansiedade induzida pelo modelo de empatia em camundongos expostos ao labirinto em cruz elevado (LCE). Para isso, camundongos Suíços machos (n=10-12/grupo), no 21º dia após o nascimento, foram alojados em pares para estabelecer familiaridade. No 14º dia de convívio, um dos animais da dupla foi submetido à cirurgia de constrição do nervo ciático (NC) ou à cirurgia sem constrição do nervo (Sham). No 23º dia, os animais observadores foram submetidos à cirurgia estereotáxica para implante bilateral de cânulas-guia direcionadas ao CCA (Experimento 1) ou na ínsula (Experimento 2). No 28º dia, os coespecíficos observadores (CNC e CS) que conviveram com respectivos grupos NC e Sham, receberam injeções bilaterais intra-CCA ou intra-ínsula de solução salina ou cloreto de cobalto (CoCl2), bloqueador sináptico reversível e não seletivo, e após cinco minutos foram submetidos ao LCE. O teste t para amostras independentes mostrou que nos Experimentos 1 e 2, os camundongos NC submetidos ao teste de placa quente apresentaram diminuição da latência de retirada da pata quando comparados aos camundongos Sham. A análise de variância (ANOVA) de dois fatores seguida pelo teste de Duncan revelou que o convívio com camundongos com nocicepção produziu aumento da resposta de ansiedade avaliada no LCE, confirmando resultados anteriores da literatura. A inativação química reversível do CCA e da ínsula produziu aumento da porcentagem de entradas e de tempo gasto nos braços abertos, sem alterar as entradas nos braços fechados do labirinto. Corroborando com estes resultados, os comportamentos de avaliação de risco, tais como porcentagens de tempo gasto no centro, mergulho protegido, esticar protegido e total de esticar no experimento 1; e porcentagens de mergulho e esticar protegido e total de mergulho e de esticar, no experimento 2, foram diminuídos. Os resultados sugerem que a inativação do CCA e da ínsula reverte o aumento da ansiedade induzida em camundongos pelo modelo de empatia para a nocicepção. Além disso, indicam que essas estruturas são essenciais para a exibição de comportamentos ansiosos induzidos por contágio emocional, avaliados por meio da convivência com animais submetidos à dor neuropática. Apathy is considered the most frequent neuropsychiatric disturbance in dementia and its outcome is generally deleterious. Apathy can be related to a dysfunction of the anatomical-system that supports the generation of voluntary actions, namely the prefrontal cortex and/or the prefrontal-subcortical circuits. In Alzheimer's disease, pathological and neuroimaging data indicate that apathy is likely due to a dysfunction of the medial prefrontal cortex. Accordingly, in this review article, we propose a pathophysiological model to explain apathetic behavior in Alzheimer's disease, combining data from neuroimaging, neuropathology and experimental research on the role of orbito-frontal cortex, anterior cingulate cortex, basal ganglia and dopamine in decision-making neurobiology.Apatia é considerada a alteração neuropsiquiátrica mais freqüente nas demências e suas conseqüências são habitualmente deletérias. Apatia pode ser relacionada à disfunção do sistema anatômico responsável pela geração de ações voluntárias, conhecido com córtex pré-frontal e/ou circuitos pré-frontais-subcorticais. Na doença de Alzheimer, evidências neuropatológicas e de neuroimagem funcional indicam que a apatia é provavelmente decorrente da disfunção do córtex pré-frontal medial. Assim, neste artigo de revisão, apresentamos uma proposta de um modelo fisiopatológico para explicar o comportamento apático na doença de Alzheimer, combinando dados de neuropatologia, neuroimagem e experimentação animal sobre o papel do córtex órbito-frontal, cíngulo anterior, núcleos da base e dopamina na neurobiologia da tomada de decisão.

Universidade Federal de Minas Gerais Empathy can be defined as the capacity for perceive emotional signals from others. Among these signals, the ability to perceive pain has clear adaptive and evolutionary value. Pain can be defined as a subjective experience that includes sensorial, emotional and cognitive components. Evidence has emphasized the role of amygdala, anterior cingulate cortex (ACC) and insula in modulation of pain and empathy. Research indicates the capacity of rodents to express empathy to a conspecific in pain or suffering. Works from literature and finds from our laboratory demonstrated that living together with a cagemate is able to alter the nociceptive behavior in mice. However, there are no works evidencing if occur alterations in nociception by living together with a cagemate with chronic pain and which encephalic structures would be involved in this modulation. To overcome this, male Swiss-albino mice were housed in groups or in pairs. The role of amygdala, ACC and insula are accessed by non-selective inactivation with cobalt chloride (CoCl2). Mice housed in groups (Experiment 1), aging 6-8 weeks, underwent a stereotaxic surgery. 4 to 5 days after surgery, these animals received saline or CoCl2 microinjection, and, after 10 minutes, they were submitted to the writhing test during 5 minutes (acetic acid 0.6%, i.p., nociceptive stimulus). On the dyads (Experiment 2), animals lived together for 28 days since weaning. On the 14th day, one animal of each pair were submitted to a sciatic nerve constriction (SNC animal) or not (sham animal). On the 24th day, the cagemate underwent a stereotaxic surgery, and, on the 28th day, they were submitted to the writhing test after microinjection of saline or CoCl2, like the procedure described to Experiment 1. To Experiment 1 were utilized Student s t test to independent samples; to Experiment 2 were utilized two-way analysis of variance (ANOVA; living together x treatment). Duncan s multiple range tests were utilized as post hoc. A p value of 0.05 or less was required for significance in both experiments. In Experiment 1, inactivation of the amygdala increased the number of writhing, while inactivation of ACC and insula did not alter this measure, suggesting a distinct modulatory role of these structures on the sensorial compound of pain. Our results demonstrated that for the mice that lived in groups, while inactivation of the ACC and insula did not change writhing, inactivation of amygdala increased it, suggesting a distinct modulatory role of these structures on sensory component of pain in the writhing test. In Experiment 2, living together with a SNC-cagemate increased writhing on the pair, suggesting that this experience activates the circuitry of neural representation of pain on the observer mouse (state of priming ). Thus, when this animal experienced nociception, its response was exacerbated. In this condition, inactivation of insula and amygdala produces opposite results, i.e., decreased and increased in contortions in those animals that lived together with a SNC animal, respectively. ACC inactivation did not alter writhing behavior. In this sense, our results suggest a different modulatory role of these structures on cognitive, affective-emotional and sensorial components of pain, and on empathy for pain. Sob uma perspectiva evolucionista, a empatia é expressa pela capacidade de captar sinais emocionais nos outros. Neste sentido, a habilidade em perceber a dor também possui valor claramente adaptativo e evolutivo. A dor pode ser definida como uma experiência subjetiva que inclui componentes sensoriais, afetivo-emocionais e cognitivos. Evidencias apontam para o papel da amídala, córtex cingulado anterior (CCA) e insula na modulação da dor e da empatia. Estudos indicam para a capacidade de roedores em apresentarem empatia frente à dor ou ao sofrimento de seus coespecíficos. Trabalhos da literatura e do nosso grupo demonstram que a convivência em pares é capaz de alterar bidirecionalmente a resposta nociceptiva em camundongos. Entretanto, nenhum estudo havia ainda evidenciado se ocorrem alterações nociceptivas devido à convivência com um coespecífico em quadro de dor crônica, e quais estruturas encefálicas estariam envolvidas nessa modulação. Neste sentido, camundongos machos Suiço-albinos foram alojados em grupos ou em duplas para avaliação do papel da amídala, insula e córtex cingulado anterior por meio de inativação com cloreto de cobalto (CoCl2). Os animais alojados em grupo (Experimento 1), ao atingirem idade entre 6-8 semanas, passaram por cirurgia estereotáxica. De 4 à 5 dias após a cirurgia, esses animais receberam microinjeção de salina ou CoCl2 e, após 10 minutos, foram submetidos ao teste de contorções abdominais (ácido acético 0,6%, i.p., estímulo nociceptivo) durante 5 minutos. Nas duplas (Experimento 2), os animais conviveram por um período de 28 dias após o desmame. No 14º dia, um animal de cada par foi submetido à cirurgia de constrição do nervo ciático (animal CNC) ou não (animal sham). No 24º dia, o camundongo que conviveu com o animal CNC ou animal sham passou por uma cirurgia estereotáxica, e, no 28º dia, foi submetido ao teste de contorções abdominais, após microinjeção de salina ou CoCl2, conforme Experimento 1. Para o Experimento 1 foi utilizado o teste t de Student para amostras independentes; no Experimento 2 foi utilizada a análise de variância (ANOVA) de dois fatores (convívio x tratamento). O post hoc utilizado foi o teste de comparações múltiplas de Duncan. Os valores de p menores ou iguais a 0,05 foram considerados como significativos nos dois experimentos. No Experimento 1, a inativação da amídala aumentou o número de contorções, enquanto a inativação do CCA e da insula não alterou esse parâmetro, sugerindo um papel modulatório distinto dessas estruturas no componente sensorial da dor para o teste de contorções. No experimento 2, o convívio com um animal CNC aumentou o número de contorções no parceiro, sugerindo que essa convivência causou ativação dos circuitos de representatividade neural da dor no camundongo observador (state of priming ). Dessa forma, quando esse animal experiencia nocicepção, sua resposta é exacerbada. Nessa condição, a inativação da insula e amídala produziu resultados opostos, ou seja, diminuição e aumento das contorções naqueles animais que conviveram com o animal CNC, respectivamente. A inativação do CCA não alterou o número de contorções. Nesse sentido, nossos resultados sugerem um papel modulatório distinto dessas estruturas nos componentes cognitivo, afetivo-emocional e sensorial da dor, e na empatia para a dor.

La lesión, que puede ser vascular, infecciosa, tóxica o degenerativa, incide sobre dos circuitos que podrían estar funcionalmente relacionados: el cortex frontal medial con afectación del cíngulo anterior, o el daño en regiones talámicas y mesencefálicas relacionadas con la formación reticular activadora ascendente. El diagnóstico de mutismo acinético resulta a veces difícil de establecer, y suele confundirse con algunos tipos de afasias o estados de alteración de la conciencia, que tienen un abordaje clínico y un pronóstico notablemente diferentes. Con el objetivo de llevar a cabo un buen diagnóstico diferencial proponemos un protocolo clínico con una evaluación neuropsicológica breve de las funciones que pudieran estar diferencialmente afectadas en todos esos síndromes, como atención, praxias y lenguaje. Exponemos un caso de mutismo evaluado con este protocolo para demostrar su especificidad a la hora de distinguirlo de otras afecciones o síndromes. Akinetic mutism is considered as a disturbance of a person´s motivational state, therefore the patient is unable to initiate verbal or motor responses, although they continue to maintain sensorimotor and vigi-lant functions. The lesion can be vascular or it can be caused by infections, cerebral degeneration and toxicity. Lesions can influence the functions of two cerebral circuits: the Frontal medial cortex affecting to cingulated cortex, or it can damage the thalamic and mesencephalic regions related to the ascending reticular activating system. It is difficult to diagnose Akinetic Mutism since it can be confused with some types of aphasia and altered states of consciousness, which require different interventions. In order to complete a clinical protocol we recommend a brief neuropsychological evaluation of functions such as attention, praxis and language, all of which could be affected by the syndromes previously named. We expound a case of akinetic mutism evaluated with this protocol in order to show its specificity distinguishing akinetic mustism from other syndromes.

In this case report we discuss the lack of emotional reactivity and evasive motor motivation to nociceptive stimuli presented by a patient with frontotemporal degenerative disease and apathy as a predominant behavioral symptom. Resumo Neste relato de caso, nós discutimos a falta de reatividade emocional e motora frente a um forte estímulo doloroso apresentada por uma paciente com doença degenerativa fronto-temporal cujo principal sintoma era a apatia.

Apathy is considered the most frequent neuropsychiatric disturbance in dementia and its outcome is generally deleterious. Apathy can be related to a dysfunction of the anatomical-system that supports the generation of voluntary actions, namely the prefrontal cortex and/or the prefrontal-subcortical circuits. In Alzheimer's disease, pathological and neuroimaging data indicate that apathy is likely due to a dysfunction of the medial prefrontal cortex. Accordingly, in this review article, we propose a pathophysiological model to explain apathetic behavior in Alzheimer's disease, combining data from neuroimaging, neuropathology and experimental research on the role of orbito-frontal cortex, anterior cingulate cortex, basal ganglia and dopamine in decision-making neurobiology. Apatia é considerada a alteração neuropsiquiátrica mais freqüente nas demências e suas conseqüências são habitualmente deletérias. Apatia pode ser relacionada à disfunção do sistema anatômico responsável pela geração de ações voluntárias, conhecido com córtex pré-frontal e/ou circuitos pré-frontais-subcorticais. Na doença de Alzheimer, evidências neuropatológicas e de neuroimagem funcional indicam que a apatia é provavelmente decorrente da disfunção do córtex pré-frontal medial. Assim, neste artigo de revisão, apresentamos uma proposta de um modelo fisiopatológico para explicar o comportamento apático na doença de Alzheimer, combinando dados de neuropatologia, neuroimagem e experimentação animal sobre o papel do córtex órbito-frontal, cíngulo anterior, núcleos da base e dopamina na neurobiologia da tomada de decisão.
