We describe a case of a man in his 70s who presented with tachycardia and several weeks of right lower quadrant pain 3 months after partial right hepatectomy for hepatocellular carcinoma. The objective of this study was to determine the minimum number of cultures needed to detect Cutibacterium acnes in primary reverse shoulder arthroplasties (RSAs).It is a prospective study including 160 primary RSAs. Exclusion criteria included an active infection, an invasive shoulder treatment in the last 6 months before surgery, an Arthro-SCAN or Arthro-MRI in the last 6 months before surgery, previous shoulder surgeries and revision cases. In 90 cases, 11 cultures were obtained. Another 10 cultures were obtained in the other 70 cases (culture 10 was a sterile sponge to detect false positives). To determine the minimum number of cultures needed to detect C acnes, the prevalence of C acnes contamination of the 160 patients included was determined.There were 128 females and 32 males, with a mean age of 74 years. There were 1690 cultures obtained from the 160 primary RSA surgeries, and 132 of them turned out to be positive for C acnes. There were 42 patients with positive cultures. Twenty of them were males and 22 females. When considering the skin and the deep tissue cultures altogether, the prevalence of positive cultures for C acnes was of 26.25%. If only deep tissues cultures were considered, the prevalence of positive cultures for C acnes was of 23.13%. When considering the skin and the deep tissue cultures together, the sensitivity to detect the C acnes is 19% if only 1 culture is obtained, 31% if 2 cultures are obtained, 50% if 3 cultures are obtained, 59.5% if 4 cultures are obtained, 66.7% if 5 cultures are obtained, 73.8% if 6 cultures are obtained, 85.7% if 7 cultures are obtained, and 92.9% if 8 cultures are obtained. When considering only the deep tissue cultures, if only 1 culture is obtained, the sensitivity to detect the C acnes is 24.3%, 40.5% if 2 cultures are obtained, 54.1% if 3 cultures are obtained, 73% if 4 cultures are obtained, 89.2% if 5 cultures are obtained, and 97.3% if 6 or 7 cultures are obtained.A minimum number of 8 cultures are needed to detect C acnes in skin. Moreover, a minimum of 6 cultures are needed to detect it in deep tissues when performing an RSA.

Unexpected positive cultures (UPCs) are very commonly found during shoulder arthroplasty when surgeons send intraoperative cultures to rule out periprosthetic joint infection (PJI) without clinical or radiographic signs of infection. Cutibacterium acnes is thought to be the most common bacteria cultured in this setting; however, the implications of an unexpected positive result are neither well defined nor agreed upon within the literature. The current review evaluates the incidence of UPCs and C acnes in reverse total arthroplasty; the clinical significance, if any, of these cultures; and various prognostic factors that may affect UPC incidence or recovery following PJI.A systematic review was performed with PRISMA guidelines using PubMed, CINAHL, and Scopus databases. Inclusion criteria included studies published from January 1, 2000, to May 20, 2021, that specifically reported on UPCs, native or revision shoulder surgery, and any study that directly addressed one of our 6 proposed clinical questions. Two independent investigators initially screened 267 articles for further evaluation. Data on study design, UPC rate/speciation, UPC risk factors, and UPC outcomes were analyzed and described.A total of 22 studies met the inclusion criteria for this study. There was a pooled rate of 27.5% (653/2373) deep UPC specimen positivity, and C acnes represented 76.4% (499/653) of these positive specimens. Inanimate specimen positivity was reported at a pooled rate of 20.1% (29/144) across 3 studies. Male patients were more likely to have a UPC; however, the significance of prior surgery, surgical approach, and type of surgery conflicted across multiple articles. Patient-reported outcomes and reoperation rates did not differ between positive-UPC and negative-UPC patients. The utilization of antibiotics and treatment regimen varied across studies; however, the reinfection rates following surgery did not statistically differ based on the inclusion of antibiotics.UPCs are a frequent finding during shoulder surgery and C acnes represents the highest percentage of cultured bacteria. Various preoperative risk factors, surgical techniques, and postoperative treatment regimens did not significantly affect the incidence of UPCs as well as the clinical outcomes for UPC vs. non-UPC patients. A standardized protocol for treatment and follow-up would decrease physician uncertainty when faced with a UPC from shoulder surgery. Given the results of this review, shoulder surgeons can consider not drastically altering the postoperative clinical course in the setting of UPC with no other evidence of PJI.

Although evidence supports that the acne microbiome harbors a diverse range of microbes that play a vital role in the progression of acne vulgaris, the culturable microbes in the acne microbiome have not yet been largely identified. Here, we grew microbe colonies from entire acne lesions on agar plates and identified abundant Staphylococcus, Acinetobacter, and Pseudomonas species from forty selected single colonies. Staphylococcus species, including Staphylococcus epidermidis (S. epidermidis), Staphylococcus hominis (S. hominis), and Staphylococcus aureus (S. aureus), were isolated from tryptic soy broth (TSB) agar plates. However, Cutibacterium acnes (C. acnes) was predominately isolated from furazolidone-supplemented TSB agar plates. Results from gas chromatography-mass spectrometry (GC-MS) analysis revealed that, besides acetate, propionate and butyrate were the main short-chain fatty acids (SCFAs) in fermentation metabolites of C. acnes and S. epidermidis isolates, respectively. The culturable bacteria and SCFA profiles presented in this study provide a reservoir for selecting acne probiotics and developing SCFA-associated therapies against acne vulgaris.

The use of personal-protection surgical helmet/hood systems is now a part of the standard surgical attire during arthroplasty in North America. There are no protocols for the disinfection of these helmets.This is a prospective, single-center, observational study. Helmets worn by 44 members of the surgical team and foreheads of 44 corresponding surgical personnel were swabbed at three distinct time points. In addition, 16 helmets were treated with hypochlorite spray to determine if pathogens could be eliminated. Swabs obtained were processed for culture and next-generation sequencing (NGS).Of the 132 helmet samples, 97 (73%) yielded bacteria on culture and 94 (71%) had evidence of bacterial-deoxyribonucleic acid (DNA) on NGS. Of the swabs sent for bacterial identification at the three time points, at least one from each helmet was positive for a pathogen(s). Of the 132 forehead samples, 124 (93%) yielded bacteria on culture and 103 (78%) had evidence of bacterial-DNA on NGS. The most commonly identified organism from helmets was Cutibacterium acnes (86/132) on NGS and Staphylococcus epidermidis (47/132) on culture. The most commonly identified organism from the foreheads of surgical personnel was Cutibacterium acnes (100/132) on NGS and Staphylococcus epidermidis (70/132) on culture. Sanitization of helmets was totally effective; no swabs taken the following morning for culture and NGS identified any bacteria.This study demonstrates that surgical helmets worn during orthopedic procedures are contaminated with common pathogens that can potentially cause surgical site infections. The findings of this study should at the minimum compel us to develop protocols for the disinfection of these helmets.

Antimicrobial resistance has become increasingly common across the globe, claiming more than 33,000 lives annually in Europe and 35,000 lives in the United States alone. The problem lies in trying to find potential solutions capable of tackling resistance and being able to fight infections that may resist various antimicrobials. Since Alexander Fleming's discovery in 1928, every antimicrobial synthesized in the past 70 years has developed at least one or more strains of resistant bacteria. One particular alternative to antimicrobials has brought hope to many in the scientific community: the bacteriophage. Bacteriophages are viruses that can replicate within bacteria, triggering genetic alterations and changes in pathways of protein expression by encoding a few to hundreds of genes within their genomes. The bacteriophage can hijack the cell, using the cell's genetic apparatus to replicate within the bacterium until bacterial lysis occurs. This therapy has been used in the genodermatosis Netherton syndrome, which has been associated with the increased risk of Staphylococcus aureus infections. Emerging data support the potential role of bacteriophage therapy for Cutibacterium acnes in acne vulgaris, with a potential role in genetic disorders with severe acne vulgaris, including Apert syndrome. Bacteriophages hold benefits for genodermatoses associated with recurrent cutaneous infections, that is, the immunodeficiencies with distinctive cutaneous features as well as conditions such as atopic dermatitis, in which bacterial colonization plays a strong role.

Acne is a frequently presented dermatological condition brought about by an interplay among inflammation, increased sebum production, hyperkeratinisation, and predominantly Propionibacterium acnes (renamed as Cutibacterium acnes) proliferation, leading to debilitating psychological scars. However, it has been shown that it is the loss of microbial diversity in the skin and the imbalance among C. acnes phylotypes that brings about acne rather than the C. acnes species as a whole. Interestingly, recent evidence suggests that other microorganisms may be implicated, such as the fungi Malassezia and the bacteria Cutibacterium granulosum. A plethora of scientific evidence suggests that the gut microbiome is implicated in the overall health and physiology of the host; studies show that the gut microbiome of acne patients is distinct and depicts less microbial diversity compared to individuals without acne. Herein, using the key terms: acne, C. acnes, IGF-1, sebum, and gut microbiome, we carried out a review of the literature, using Google Scholar and PubMed, and discussed the role of the gut and skin microbiome in relation to acne, as a narrative review. The role of hormones, diet, sebum, and stress in relation to the gut microbiome was also investigated. Therapeutic implications and the use of pre-/postbiotics are also deliberated upon. In this light, future research should investigate the relationship between the gut microbiome and the agreed upon factors of acne pathology, potentially leading to the discovery of novel acne treatments with milder side effects.

Cutibacterium acnes, formerly known as Propionibacterium acnes, is increasingly recognized as a cause of surgical site infection and implant failure despite the use of prophylactic antibiotics and antiseptic surgical preparations. The aim of this study was to investigate whether C. acnes persists in the dermal layer of the skin after standard perioperative antibiotics and skin prepping with alcoholic betadine solution in consecutive patients undergoing a craniotomy.A single centre prospective observational study was performed at Flinders Medical Centre. Adult patients undergoing a cranial neurosurgical intervention between October 2019 to March 2021 were eligible for inclusion. After administration of standard preoperative antibiotics (Cefazolin), three swabs were taken for each patient: one before prepping the skin with alcoholic betadine, one after prepping the skin and a dermal swab once the skin was incised.73 patients were included. Cutibacterium acnes cultures were positive in 61 patients of the "pre-prep" group (83.6%), 12 (16.4%) in the "post-prep" group, and 53 (72.6%) were from dermal swabs There was a significant reduction of positive cultures of the skin after surgical preparation was applied (p 0.00001). There was a non-significant reduction of positive cultures in the dermal swabs after skin preparation (p = 0.068) CONCLUSIONS: Cutibacterium acnes persists within the dermis of the scalp despite standard prophylactic measures using alcoholic betadine solution and cefazolin.

The close balance between Cutibacterium acnes and the skin flora, particularly between C. acnes phylotypes, has been suggested to play an important role in the onset of acne. C. acnes has been classified into ribotypes (RTs) based on polymorphisms in its 16S rRNA sequence, with RT4 and RT5 being associated with the onset of acne and RT6 with healthy skin.The present study investigated the impact of erythritol on the growth of C. acnes strains classified into different RTs and attempted to elucidate the molecular mechanisms underlying its effects.Culturing tests were performed on several RTs of C. acnes with or without erythritol. A transcriptional analysis of HM554 (RT6) and HM514 (RT5) was also conducted.The growth of RT2 and RT6, RTs associated with healthy skin, was significantly promoted in a medium containing 10% (W/W) erythritol, whereas that of RT1, RT3, RT4, RT5, and RT8, RTs associated with the development of acne, was inhibited. A RNA-seq analysis of HM554 showed that the expression of six genes (EIGs) potentially involved in carbohydrate metabolism was strongly induced by the presence of 10% erythritol (LogErythritol inhibited the growth of RTs associated with acne and promoted that of RTs associated with healthy skin. The enzyme encoded by g3pD may play an important role in the metabolism of erythritol and the dissolution of its growth inhibitory effects on C. acnes.

SUMMARY The role of C. acnes biofilm production in the pathogenesis of breast implants infections has not been deeply assessed. We analyze biofilm production (in terms of biomass and metabolic activity) between 40 Cutibacterium acnes strains isolated from breast implants and 32 from other sites. C. acnes strains isolated from breast implants showed higher metabolic activity than those isolated from other-locations and, especially, those from patients with capsular contracture.

O mel de abelha jataí (Tetragonisca angustula) é saboroso e procurado por suas propriedades medicinais. Sua composição química está relacionada a ação antimicrobiana, incluindo bactérias e leveduras. Acne é a doença de pele humana mais comum que pode resultar em alterações psicossociais comprometendo a qualidade de vida do indivíduo, principalmente adolescentes. A bactéria Propionibacterium acnes é descrita como principal causadora de acne vulgar, associada a bactéria Staphylococcus epidermidis e a levedura Malassezia furfur. A atual medicina alternativa, baseada na medicina popular, indica o uso de mel como aliado no combate e prevenção de acne. Na busca por método natural e de fácil acesso contra bactérias que causam acne vulgar, testou-se a ação antimicrobiana do mel de abelha sem ferrão jataí. Para tanto, isolou-se colônias provenientes de acne facial e testou-se a ação bactericida do mel in vitro. A concentração inibitória mínima (CIM) de mel de abelha sem ferrão jataí contra microrganismos causadores de acne, apresentou proporção de 0,6:1, numa relação de 300 µl (6 gotas) de mel para 500 µl de cultura de bactérias. Porém, redução no número de bactérias foi observada na presença de 200 µl (4 gotas) de mel. Os efeitos antibacterianos do mel de abelha jataí foram anulados quando este permaneceu em temperatura ambiente. Sendo assim, o mel de abelha jataí apresenta potencial atividade antibacteriana como tratamento alternativo de acne vulgar, quando o produto é mantido em geladeira. A presença de fungos fermentadores no mel, associados as abelhas, resistentes à baixas temperaturas e as condições ácidas e osmofílicas do mel, podem ter papel significante na ação contra P. acnes.

Propionibacterium acnes is bacteria that is commonly found on healthy human skin. However, unbalanced hormonal conditions can lead to excessive growth of P. acnes, a bacteria causing acne. Previous research reported that Ocimum basilicum L. essential oil (BEO) has activity against P. acnes. Therefore, this study aims to make BEO into cream, ointment, and emulgel forms to determine its properties and antibacterial activity on those topical drug delivery systems. The preparations consisted of variations in BEO concentrations of 5%; 10%; and 15%. The three preparations were evaluated based on organoleptic, homogeneity, pH value, adhesion, and spreadability. The antibacterial activity test of the three preparations against P. acnes was carried out by the good diffusion method with the observation of the diameter of the inhibition zone (in mm). The produced cream, ointment, and cream had a typical BEO aroma, yellowish white color, and homogeneous. Based on organoleptic consistency, the texture of the thicker and longer adhesion was ointment > emulgel > cream, respectively. The pH value of the topical preparation was 5-6 and easily leveled. Antibacterial activity against P. acnes from the strongest was ointment > cream > emulgel, with a value of 14,30 + 0,30 mm; 10,54 + 0,01 mm; 3,90 + 0,04 mm, respectively.

With a view to reducing the impact of Cutibacterium acnes (C. acnes) on acne vulgaris, it now appears interesting to modify the balance between acneic and non-acneic strains of C. acnes using moderate approach. In the present study, we identified that a G2 dendrigraft of lysine dendrimer (G2 dendrimer) was able to modify membrane fluidity and biofilm formation of a C. acnes acneic strain (RT5), whereas it appeared no or less active on a C. acnes non-acneic strain (RT6). Moreover, skin ex vivo data indicated that the G2 is able to decrease inflammation (IL1α and TLR-2) and improve skin desquamation after of C. acnes acneic strains colonization. Then, in vivo data confirmed, after C. acnes quantification by metagenomic analysis that the G2 cream after 28 days of treatment was able to increase the diversity of C. acnes strains versus placebo cream. The data also showed a modification of the balance expression between C. acnes phylotype IA1 and phylotype II abundances. Taken together, the results confirm the interest of using soft compounds in cosmetic product for modifying phylotype abundances and diversity of C. acnes strains could be a new strategy for prevent acne vulgaris outbreak.

Cutibacterium acnes (C. acnes) is an organism implicated in the pathogenesis of acne. Despite regular immersion in antimicrobial chlorine, adolescent swimmers suffer from acne and tend to be resistant to standard therapies. Given the presence of Pseudomonas within swimming facilities, we hypothesized that "swimmer acne" is potentially driven by a different microbial mechanism. In this study, we aimed to examine the microbial dynamics of C. acnes and Pseudomonadaceae, a family of gram-negative bacteria (includes Pseudomonas aeruginosa), in swimmers and its potential contribution to the pathogenesis of acne in this population. Using fluorescence photography that measures the Coproporphyrin III (CPIII), we quantitated an absolute abundance of C. acnes present on the face of each participant pre- and post-swimming. In addition, 16S rRNA gene sequencing was utilized to assess relative abundance of the skin microbiota on each participant pre- and post-swimming. 16 swimmers (8 girls and 8 boys) completed the study. Seven had acne on the face. The CPIII fluorescence levels decreased for all swimmers after 1 h of swimming (p-value0.001). In contrast, the relative abundance of C. acnes remained unchanged, while that of Pseudomonadaceae increased after swimming (p-value =0.027). Comparing the relative abundances of Pseudomonadaceae before swimming, there was a significant increase in variance from the mean in acne group as compared to no acne group (p-value0.001). Taken together, we conclude that the skin dysbiosis resulting from repeated decolonization and colonization of C. acnes and Pseudomonadaceae, respectively, can potentially be associated with the pathogenesis of acne in swimmers.

Acne is a problem among teenagers generally caused by bacterial infections such as Propionibacterium acnes, Staphylococcus aureus, and Staphylococcus epidermidis. Mugwort (Artemisia vulgaris L.) and Gotu Kola (Centella asiatica L.) extracts have antibacterial activity formulated in anti-acne gel preparations. The purpose of this study was to test and obtain a gel formula of mugwort leaf extract and gotu kola leaf extract which effectively kills and controls the growth of acne-causing bacteria. The methods used in this study include simplicity making, leaf extraction, making anti-acne gel preparations, testing pH, homogeneity, inhibition, and data analysis. The extract obtained by maceration was made in three concentrations, namely 2.50; 5.00; and 10.00%. The results showed that the pH value of the gel preparations obtained was in the range of 8.08-9.96. Mugwort leaf extract, gotu kola leaf, and mugwort-gotu kola have a distinctive smells with a thick texture at a concentration of 2.50% and a liquid texture at a concentration of 10.00%. Inhibition tests showed that the mugwort extract sample had a value of 16-18 mm, while the gotu kola extract and mugwort-gotu kola had a value of 14 mm. Mugwort leaf extract is able to inhibit the growth of acne-causing bacteria. This success was obtained because the extracts of mugwort and gotu kola leaves contain flavonoids that can inhibit the growth of bacteria that cause acne. Mugwort leaf extract gel with a concentration of 2.50 and 5.00% had the largest zone of inhibition in inhibiting P. acnes bacteria compared to other concentrations.

In this study, hydrogels containing azelaic acid were developed using chitosan or HPMC (1-7%) for local treatment of acne vulgaris. Physicochemical properties such as viscosity, pH and mechanical properties were evaluated. In vitro release and ex vivo permeability studies were performed using the Franz diffusion cell system. The pH of the hydrogels was highly compatible with the skin pH and varied between 4.38 and 5.84. The cumulative release percentages of the hydrogels at the end of 6 hours were 65-78%, whereas the marketed product yielded 50% drug release. According to the ex vivo permeability results, azelaic acid accumulated in the skin was found to be 9.38 +/- 0.65% (marketed cream), 19.53 +/- 1.06% (K3), 10.96 +/- 1.91% (H6). The antiacne studies with Cutibacterium acnes revealed that K3 (29.45 +/- 0.95) and H6 (32.35 +/- 0.15) had higher inhibition zones compared to the marketed cream (24.50 +/- 0.90). Additionally, the gels were found to be highly stable as a result of the stability studies for 6 months. Among the hydrogels that were prepared based on experimental findings, K3 (3% Chitosan) and H6 (6% HPMC) represented elevated in vitro release profile, higher permeability and increased antiacne activity. The findings of this research suggest that the developed hydrogels might be an alternative to the marketed product.

Objectives Cutibacterium acnes is one of the common multifactorial causes that play an important role in the pathophysiology of acne vulgaris. We aimed to develop novel antimicrobial peptides for reduction of the hypercolonization. Methods Six cationic peptides were derived by de novo designation. The antimicrobial and cytotoxic activities of peptides were investigated. The peptide conformation was determined by circular dichroism spectrometry. The antimicrobial effects of peptides were evaluated using scanning electron microscopy (SEM), transmission electron microscopy (TEM) and DNA-binding ability assay. Results Among designed peptides, WSKK11 and WSRR11 were effective antimicrobials against C. acnes at MICs of 128 and 64 mg/L, respectively. The MICs of WSKK11 against Staphylococcus epidermidis, Staphylococcus aureus and Candida albicans were 8, 8 and 32 mg/L, while those of WSRR11 were 64, 32 and 32 mg/L, respectively. WSKK11 and WSRR11 were less toxic to human erythrocytes ( Conclusions The two novel antimicrobial peptides WSKK11 and WSRR11 are members of a new class of antimicrobial agents that could deal with acne problems. Therefore, the antimicrobial peptides may be promising novel active agents for dermatological, beauty and cosmeceutical applications.

Background Intra-incisional deposition of vancomycin powder is a strategy to limit Cutibacterium acnes infection after shoulder surgery. Unfortunately, limited research exists examining the effectiveness of vancomycin in a clinically relevant joint infection model. This basic science study investigated the efficacy of vancomycin administration as prophylaxis for C acnes growth in vitro using a mimetic shoulder arthroplasty. Methods A new bioartificial shoulder joint mimetic implant (S-JIM) was used to investigate the effect of vancomycin powder on C acnes growth within the first 48 hours after surgery. The impact of vancomycin was assessed on a skin-derived (ATCC 11827) C acnes strain and a periprosthetic joint infection–derived strain. C acnes strains were applied to titanium alloy foil and embedded beneath multiple layers of collagen-impregnated cellulose scaffold strips containing human shoulder joint capsular fibroblasts, facilitating the development of an oxygen gradient with an anaerobic environment around the foil and inner layers. Ten milligrams of vancomycin powder was applied between the C acnes layer and the human cell–containing scaffold strips to model direct antibiotic application, and intravenous vancomycin prophylaxis was modeled by adding vancomycin in media at 5 or 20 μg/mL. After 48 hours, the C acnes inoculum layer was subcultured from each S-JIM onto agar plates to assess the formation of viable C acnes colonies. Primary human shoulder capsule cells were assessed microscopically to detect any detrimental effects of vancomycin on cellular integrity. Results Agar plates inoculated with extracts from untreated S-JIMs consistently resulted in the growth of large numbers of C acnes colonies, whereas treatments with vancomycin powder or vancomycin in media at 20-μg/mL dilution effectively prevented the recovery of any C acnes colonies. The lowest vancomycin dilution tested (5 μg/mL) was insufficient to prevent the recovery of C acnes colonies. Vancomycin powder had no discernible short-term impact on shoulder capsule cell morphology, and the presence of these cells had no discernible impact on vancomycin degradation over time. Conclusions Vancomycin administration effectively prevented C acnes growth in a bioartificial S-JIM. These results support the hypothesis that intra-incisional vancomycin application may limit C acnes prosthetic joint infections.

The early diagnosis of low-grade Cutibacterium acnes prosthetic shoulder joint infection is challenging due to the lack of clinical and laboratory signs. Patients present with atypical symptoms such as stiffness or failure to improve shoulder function. The diagnosis is often delayed with impact on long-term outcomes. We present the case of an 82-year-old man with a surgical site erythema occurring 7 weeks after reverse shoulder arthroplasty associated with a light raise of C reactive protein (20 mg/L). At 9 weeks, radiographs revealed a ‘periosteal spur sign’ (humeral calcar) and localised adjacent osteolysis. Open surgery showed morphological signs of infection confirmed by growth of C. acnes in eight cultures. A ‘periosteal spur sign’ is a useful early radiographic indicator of low-grade prosthetic joint infection usually occurring with some delay after clinical symptoms. A high clinical index of suspicion is needed to proceed with biopsies and to initiate combined operative and antibacterial treatment.

Aims To isolate and characterise phage which could lyse P. acnes and to formulate the phage into a delivery form for potential application in topical treatment of acne infection. Methods and Results Using standard phage isolation techniques, ten phage capable of lysing P. acnes were isolated from human skin microflora. Their genomes showed high homology to previously reported P. acnes phage. These phage were formulated into cetomacrogol cream aqueous at a concentration of 2.5x108 PFU per gram, and shown to lyse underlying P. acnes cells grown as lawn cultures. These phage formulations remained active for at least 90 days when stored at four degrees Celsius in a light protected container. Conclusions P. acnes phage formulated into cetomacrogol cream aqueous will lyse surrounding and underlying P. acnes bacteria, and are effective for at least 90 days if stored appropriately. Significance and Impact of the Study There are few reports of phage formulation into semi solid preparations for application as phage therapy. The formulation method described here could potentially be applied topically to treat human acne infections. The potential exists for this model to be extended to other phage applied to treat other bacterial skin infections.

We report a case of a pulmonary necrotizing sarcoid granulomatosis (NSG)-like lesion possibly associated with coinfection of Mycobacterium avium and Propionibacterium acnes. A solitary nodule in the right middle lobe of the lung was notable for coagulative necrosis with aggregates of sarcoid-like epithelioid granulomas. Small arteries were damaged by granulomas. Both M. avium and P. acnes were detected in the lesion. Furthermore, more P. acnes genomes were detected in the granulomas than in the non-lesion lung. These findings blur the pathophysiologic boundaries among NSG, sarcoidosis, and mycobacteriosis, and suggest that NSG needs to be recognized as continuous spectra of sarcoidosis/mycobcteriosis.

Acne is a multifactorial inflammatory dermatose that affects all age categories from teenagers to adults, resulting in important psychological impacts. Multiple hypotheses currently attempt to decrypt the physiopathology of this disease, and four main actors were identified as highly implicated in it: hyperkeratinization of the pilosebaceous follicle, hyperseborrheae, host factors (innate immunity) and skin microbiota. In this letter, we present results illustrating the impact of skin microbiota on inflammatory skin response, and how far the proper balance between each bacterial community, especially C. acnes and S. epidermidis, is crucial to maintain an appropriate inflammatory response on the skin. The data presented in this study demonstrate that within the skin microbiota, an imbalance between Cutibacterium acnes and Staphylococcus epidermidis, is able to induce the activation of inflammation-related markers such as IL-1ra, IL-6, IL-8, G-CSF and the molecules C5/C5a, soluble CD14 MIP-3beta, Serpin E1, VCAM-1 and beta-defensin-2. Moreover, S. epidermidis appears to have a more important role than C. acnes on the induction of inflammation-related markers, particularly on IL-6. This work is the basis of future in vitro studies to further understand acne physiopathology, inspiring the development of future innovative therapies based on skin microbiota modulation.

Azelaic acid (AZA) is a white crystalline dicarboxylic acid naturally found in grains, rye, and barley. AZA has substantial biological and therapeutic abilities (viz a viz) its anti-inflammatory, anti-oxidant, anti-keratinizing, anti-microbial properties, etc., which contribute to its applicability in the management of mild to harsh dermatological complications (acne, rosacea, dermatitis, hyper-pigmentation, carcinomas, etc.). AZA has shown its effectiveness against varied non-inflammatory and inflammatory lesions by normalizing hyper-keratinization state and attenuating the increased levels of microbial content. Topically AZA, either alone or in conjunction with other active moieties, has proved to effectively prevent acne and several other hyper-pigmentary conditions. Chronic applicability of AZA has been evidenced with the effects like itching, burning, stinging, redness, etc. To deal with the former issues, research is being conducted to substitute the conventional formulations with novel preparations (liposome’s, niosomes, micro sponges, lipid nanocarriers, etc.), which could enhance the overall pharmaceutical and pharmacological profile of the drug. This article is an attempt to highlight the basic physiochemical properties of AZA, its physiological role (especially in dermatology), various commercial preparations and recent novel approaches that are in research with an aim to augment the therapeutic and safety profile of AZA.

Background: Acne is one of the most infectious diseases related to human skin. This disease of acne is of higher prevalence in adults. The main causative organism involved in the acne vulgaris is Propionibacterium acne, which leads to sebum production and follicular hyperkeratinization. Methods: The scientific data shows inflammatory mediators of genetic factors, stress, physiological factor, androgens and hormonal changes; all these factors play a major part in the pathophysiology of acne. Several drugs are currently available for the treatment of Acne like bactericidal, antibiotic and several others which are directly or indirectly involved in the eradication of acne. Results: Nanocarrier systems including liposomes, niosomes, microsponges, nano-emulsion and micro-emulsion, microspheres and solid lipid nanoparticles have emerged as a successful treatment for acne. In this article, the authors have laid special emphasis on these nanocarriers and the study of various patents based on novel technologies in this field. The entrapment of the anti-acne drug molecule into a particular nanocarrier system has been shown to enhance patient compliance and reduction in side-effects. Conclusion: The present review article provides an overview of the mechanism, advantages, drawbacks and various patents associated with the nanocarrier systems involved in acne treatment. Some state of the art patented and novel technologies such as stem cell secretion technology, sol-gel technology incorporating microcapsules and ultrasound delivery of nanoparticles for acne treatment, are also briefly discussed. The patent study on these nanocarriers was done through the worldwide database of the United States patent office, European patent office and several other official patent information websites.

Daruju leaves (Acanthus Ilicifolius L) is a type of plant that is widely grown and developed in Indonesia. Daruju leaves (Acanthus Ilicifolius L) found the presence of many secondary metabolite compounds. Secondary metabolite compounds are non polar, semipolar and polar so that to maximize the extraction of secondary metabolites, fractionation is carried out. The fractionation process in this study used n-hexane, ethyl acetate and methanol as solvent, while the test bacteria used were Propionibacterium acnes and Staphylococcus epidermidis. Antibacterial activity testing was carried out using the agar diffusion method. The antibacterial activity test showed that the ethyl aceat fraction of Daruju leaves (Acanthus Ilicifolius L) was able to inhibit the growth of Propionibacterium acnes and Staphylococcus epidermidis bacteria. The activity test of the ethyl acetate fraction of Daruju leaves (Acanthus Ilicifolius L) on Propionibacterium acnes bacteria was obtained at a concentration of 20% with an average diameter of the inhibition zone of 20.99 mm where the inhibition zone category was included in the strong category, while the Stаphylococcus epidermidis bacteria were obtained in concentration of 15% with an average diameter of the inhibition zone of 20.21 mm where the inhibition zone category is included in the strong category.

Topical and oral antibiotic therapy is also a popular method of treatment. The effectiveness of this method is limited by the increasing resistance of bacteria to antibiotics. Over the decades since the introduction of antibiotics to treat acne, the resistance levels of bacteria have changed. This defense mechanism is developed evolutionarily. Modifications of antibiotic receptor sites, alteration of drug influx/efflux, or enzymatic degradation are common mechanisms used by bacteria to initiate and strengthen internal antibiotic resistance. The basic chromophores used in light therapy are hemoglobin, melanin, water bound to proteins, and porphyrins. Hemoglobin absorbs light mainly at 580 nm, while melanin absorbs the entire visible spectral range (400-750 nm). Porphyrins are aromatic compounds, classified as photosensitizing substances, intensively absorbing blue light, and to a lesser extent in long visible bands, such as orange and red light. Using IPL makes it possible to cover the maximum light absorption of porphyrins and hemoglobin, therefore it can be an effective tool in the treatment of inflammatory lesions in acne vulgaris. In view of the effectiveness of light therapy and its effect even on antibiotic-resistant bacteria, it is worth considering the possibility of using light therapy instead of antibiotic therapy. Due to the increasing resistance of bacteria to antimicrobials, they should be used with caution and as a last resort. The high-energy light treatment act only locally (unlike with antibiotics taken orally) and on a chromophore, such as melanin, hemoglobin, or porphyrins.

Propionibacterium acnes plays a major role in acne vulgaris. In the pre-experiment, the growth of P. acnes was inhibited effectively using surfactin; however, the antibacterial mechanism has not been described. Therefore, the aim of this study was to evaluate antibacterial activity and analyse the mechanism of surfactin against P. acnes. Minimum inhibitory concentration, time-killing kinetics and scanning electron microscopy were used to evaluate the activity of surfactin against P. acnes, which showed that 128 μg ml-1 effectively inhibited growth. Cell wall permeability was evaluated by detecting the extracellular alkaline phosphatase activity, which increased to 1·83- and 2·32-fold after incubating with 128 and 256 μg ml-1 of surfactin for 10 h, respectively. Propidium iodide fluorescence, leakage of nucleic acid, protein, K+ , and Ca2+ , membrane potential and the leakage of calcein from small unilamellar vesicles all increased after incubation with surfactin, indicating that its strong biological activities act mainly by altering membrane integrity. In a mouse model of acne, surfactin significantly reduced P. acnes-induced epidermal swelling and erythema. These results indicate that surfactin effectively inhibited the growth of P. acnes by destroying the cell wall and membrane, and is a potential candidate for acne treatment.

The excessive colonization of Propionibacterium acnes (P. acnes) is responsible for the genesis of acne vulgaris, a common inflammatory disease of skin. However, the conventional anti-acne therapies are always limited by various side effects, drug resistance, and poor skin permeability. Microneedles (MNs) are emerging topical drug delivery systems capable of noninvasively breaking through the skin stratum corneum barrier to efficiently enhance the transdermal drug penetration. Herein, MNs loaded with intelligent pH-sensitive nanoplatforms were constructed for amplified chemo-photodynamic therapy against acne vulgaris, jointly exerting antimicrobial and anti-inflammatory effects. The photosensitizer indocyanine green (ICG) was loaded into the zeolitic imidazolate framework-8 (ZIF-8) to improve its photostability, which would be triggered by 808 nm laser irradiation to generate cytotoxic reactive oxygen species (ROS) to result in oxidative damage and disturbed metabolic activities of P. acnes. In addition to the efficient drug delivery, the ZIF-8 carrier could selectively degrade in response to the acidic microenvironment of acne lesions, and the released Zn2+ also exhibited a potent antimicrobial activity. The fabricated ZIF-8-ICG@MNs presented an outstanding synergistic anti-acne efficiency both in vitro and in vivo. This bioresponsive microneedle patch is expected to be readily adapted as a generalized, modular strategy for noninvasive therapeutics delivery against superficial skin diseases.

The principal actors in acne are the sebaceous gland, keratinocytes of the follicle and skin microbiome/innate immunity. Current acne treatments are frequently accompanied by side effects or may cause antibacterial resistance. New formulations and emerging treatments with novel mechanisms of action and improved formulations have recently been made available or are currently under development. This review provides an update on the most recent advances in topical or systemic acne therapy related to recent data on the pathophysiology of acne. A review of the most recent literature about new and emerging acne treatments since 2016 listed in the PubMed and Clinicaltrials.gov database was performed by a group of dermatologists interested in acne (GEA). Several novel treatments have been made available or are currently under development, including Clascosterone, Trifarotene and Sarecy-cline, as well as more effective and better tolerated formulations of existing compounds, such as Minocycline, Tretinoin, Tazarotene and Lidose-isotretinoin, and emerging acne therapies (including hyaluronic acid, cannabidiol, modulators of the skin microbiota, insulin-like growth factor, vaccines, bacteriophages, probiotics and antimicrobial peptides), targeting the sebaceous gland and its activity, inflammation or keratinocytes of the follicle and skin microbiome including Cutibacterium acnes. Recently, in addition to other fixed combinations, a fixed combination of adapalene and benzoyl peroxide that targets acne scars has been made available for the first time. The newly available products and other potentially emerging treatment options will increase the armamentarium of acne therapies and potentially reduce its prevalence worldwide.

Currently, bioactive compounds derived from nature have been thought to be promising anti-acne substances owing to the variety of potential biological effects. This study aimed to evaluate the ameliorative effect of Bouea macrophylla Griffth seed extract against bacteria-induced acne inflammation for the first time in terms of antibacterial effects against acne-inducing bacteria, anti-inflammatory, and antioxidant properties. Initially, extracting procedures were optimized and five different extracts were obtained. Considering their antibacterial activities against Cutibacterium acnes, Staphylococcus aureus, and Staphylococcus epidermidis, ethanolic and ethyl acetate fractions exerted a notable effect which were highly superior above those of polyphenol standards. Additionally, these two extracts presented outstanding antioxidant capacities in terms of DPPH and ABTS radicals scavenging effects, reducing power, and inhibitory effect on lipid peroxidation which also play a role in the exacerbation of acne inflammation. Besides, inhibition on lipid peroxidation and reducing power of ethanolic fraction were significantly (p0.05) better than those of ethyl acetate fraction which was corresponding to their phenolic and ellagic acid contents. However, flavonoids found in ethyl acetate fraction might play an important role in its potentials. After that, the anti-inflammatory effects of the extracts were elucidated by means of inhibition on nitric oxide production from LPS-induced RAW 264.7 cell lines at which the effects of both extracts were dosedependency. Taken together, our findings have apparently proven that B. macrophylla seed extracts exerted a variety of potential properties including antioxidation, anti-acne-inducing bacteria, and anti-inflammatory effects which could serve as a promising anti-acne agent for cosmeceutical applications.

The purpose of this study was to investigate if the C. acnes present at the end of a primary shoulder arthroplasty could be responsible for shoulder arthroplasty infection. Prospective study includes patients undergoing primary shoulder arthroplasty from January 2015 until December 2018. From all the patients included, 5 to 12 tissue samples were obtained and were specifically cultured to detect the presence of C. acnes. DNA was extracted from the C acnes isolated colonies and Whole Genome Sequencing (WGS) analysis was done. A cohort of 156 patients was finally included. In twenty-seven patients, the C. acnes was present at the end of the primary surgery. Two of these patients developed a C. acnes periprosthetic shoulder infection at 6 and 4 months after the primary surgery. WGS of C. acnes isolated colonies showed that all the revision-surgery isolates clustered near to the corresponding primary-surgery isolates compared to the other independent bacterial colonies. (99.89% of similarity). C. acnes present at the end of the primary surgery can be the cause of early or delayed periprosthetic joint infections in shoulder arthroplasty.

The research had conducted the antibacterial activity of ethanol extract of Chromolaena odorata (L.) leaves by TLC Bioautography. It aimed to evaluate the antibacterial activity of the ethanol extract.The ethanol extract of leaves Chromolaena odorata (L.) was done by screening at the concentration of 0.1% on Staphylococcus epidermidis, Staphylococcus aureus, Propionibacterium acnes, then test bioautigrafi TLC and identification of chemical components. The results of screening showed that the ethanol extract of 0,1% concentration had an antibacterial activity. Meanwhile, TLC-Bioatography method resulted in 5 active spots with different Rf values 0,1, 0,3, 0,6, 0,8, and 0,9 (Staphylococcus epidermidis), and (Propionibacterium acnes). 4 active spots with Rf values 0,1, 0,3, 0,6, and 0,8 (Staphylococcus aureus) Therefore, the identification of chemical components of ethanol extract contained phenol group compounds.

Propionibacterium acnes (P. acnes) is a gram-positive, anaerobic, commensal organism found on nonglabrous skin, including the face, scalp, chest, and axilla. Recently, P. acnes is one of the more frequently recognized organisms causing postoperative infections in implant surgery of the shoulder, second to only Staphylococcus aureus (S. aureus), yet it is a rare postoperative complication of the wrist and hand. Multiple factors, including slow growth, multiorganism involvement, and selective growth media, complicate attributing a primary infection to P. acnes. We present a case of primary, acute P. acnes infection after carpal tunnel decompression, demonstrating the need for considering P. acnes for persistent postoperative hand and wrist infections.

Background Cutibacterium acnes is a common pathogen associated with surgical site infection after shoulder surgery; current standard of care products are largely ineffective at reducing C acnes bacterial burden before surgery. The purpose of this systematic meta-analysis was to assess the efficacy of peroxide-containing solutions (PCS) in decreasing the C acnes burden on the shoulder. Methods This was a systematic review of all level I and II studies investigating the effect of peroxidase-containing products for skin preparation. We extracted data regarding demographics, treatment details and timing, study methodology, and culture positivity. Forest plots were used to determine the pooled efficacy of peroxide solutions versus control. Results Seven studies with 412 patients were eligible for inclusion. Notable heterogeneity was observed in the manner and timing of peroxide application. Two studies applied PCS at the time of surgery; four studies applied PCS in the 24- to 72-hour period leading up to culture acquisition. Compared with the placebo, peroxide significantly diminished C acnes culture positivity (Hazard Ratio 0.174, P = 0.009). When considering using peroxide-containing products in the period leading up to surgery or at the time of surgery, in addition to standard preparation, the addition of peroxide significantly diminished C acnes culture positivity (HR 0.467, P = 0.004). Owing to study heterogeneity, we could not make notable comparisons based on the timing or duration of benzoyl peroxides application. Conclusions Despite heterogeneity in study design, pooled results of high-quality data suggest that the addition of PCS can markedly reduce C acnes bioburden. This review was not able to identify the ideal regimen for the utilization of PCS for reduction of C acnes burden. Level of evidence Level II.

Purpose: The purpose of the study is to culture uncultured oral bacteria with helper strains using the coculture method from the subgingival plaque samples of chronic periodontitis patients. Materials and Methods: The samples were processed and inoculated on a blood agar medium enriched with hemin and Vitamin K. A helper strain Propionibacterium acnes (ATCC 6919) was cross-streaked across the inoculums to facilitate coculture. The plates were then incubated for 7 days with subsequent subculturing and further incubation. Results: Satellite colonies around helper strain showed one colony type of Porphyromonas gingivalis, one was of nonpigmented Prevotella, three were of Fusobacterium nucleatum and five isolates remained unidentified. Conclusions: Coculture could be used effectively as one of the methods in the isolation and in vitro cultivation of oral bacteria. Incubation using the anaerobic jar technique was found to be economical and efficient for the growth of anaerobic oral bacteria.

Bidara memiliki kandungan fenolat dan flavonoid yang bermanfaat sebagai anti inflamasi, antioksidan, antimikroba, dan mencegah timbulnya tumor. Kandungan kimia lain Bidara yang berperan pada pengobatan yakni alkaloid, fenol, flavonoid, dan terpenoid. Penelitian ini bertujuan untuk mengetahui pengaruh konsentrasi ekstrak etanol daun Bidara (Ziziphus-Spina Christi L.) terhadap sifat fisik krim dan aktivitas antibakteri krim daun Bidara terhadap bakteri Propionibacterium Acnes ATCC 6919 dibuat dalam tipe M/A (Minyak dalam Air) menggunakan vanishing cream dengan 3 macam konsentrasi yaitu 10%, 12,5%, dan 15%. Sifat fisik (organoleptis, daya sebar, daya lekat, dan daya proteksi) dan aktivitas antibakteri terhadap Propionibacterium acnes yang diukur menggunakan metode difusi sumuran. Hasil uji sifat fisik dan aktivitas antibakteri dianalisis menggunakan uji statistik dengan taraf kepercayaan 95%. Hasil uji sifat fisik menunjukkan bahwa krim ekstrak etanol daun Bidara mempunyai aroma daun Bidara, warna kehijauan, bentuk semipadat, dan tekstur homogen. Krim ekstrak etanol daun Bidara konsentrasi 15 % menunjukkan daya sebar dan daya lekat paling besar yakni 6,9 cm dan 6 detik. Kemampuan proteksi krim ekstrak etanol bidara pada keseluruhan konsentrasi ditunjukkan sampai dengan menit ke-5. Selain itu, krim ekstrak etanol daun Bidara memiliki aktivitas antibakteri yang paling baik terhadap Propionibacterium acnes pada konsentrasi 15 % yang ditunjukkan dengan terbentuknya diameter zona hambat yang paling besar yakni 7,73 mm.

Background Cutibacterium acnes ( C. acnes) is the most common pathogen responsible for post-operative shoulder infections. The purpose of this study was to evaluate the effectiveness of skin preparation methods against C. acnes in shoulder surgery. Methods A systematic review was conducted evaluating the effectiveness of skin preparation methods in the reduction of C. acnes in patients undergoing shoulder surgery. Outcomes were assessed based on the effectiveness of the method used; side effects and cost were also analysed. Results Of the 19 included studies, 9 evaluated pre-surgical home treatments: 8 assessed benzoyl peroxide (BPO) and 6 concluded it is effective in reducing C. acnes. Nine studies assessed surgical skin preparation and concluded that Chlorhexidine gluconate (CHG) was not effective; in contrast hydrogen peroxide reduced C. acnes. Finally, one study evaluated an aseptic protocol using CHG and concluded that it was not effective. Conclusions It was demonstrated that BPO as home treatment is effective in reducing C. acnes load on skin ; it rarely causes side effects and is also cost-effective. This study highlights non-effectiveness of CHG. There was some evidence that the addition of hydrogen peroxide could have a positive effect in the reduction of C. acnes skin load; however, more studies are required.

Objectives Intra-amniotic infection, defined by the presence of microorganisms in the amniotic cavity, is often accompanied by intra-amniotic inflammation. Occasionally, laboratories report the growth of bacteria or the presence of microbial nucleic acids in amniotic fluid in the absence of intra-amniotic inflammation. This study was conducted to determine the clinical significance of the presence of bacteria in amniotic fluid samples in the absence of intra-amniotic inflammation. Methods A retrospective cross-sectional study included 360 patients with preterm labor and intact membranes who underwent transabdominal amniocentesis for evaluation of the microbial state of the amniotic cavity as well as intra-amniotic inflammation. Cultivation techniques were used to isolate microorganisms, and broad-range polymerase chain reaction coupled with electrospray ionization mass spectrometry (PCR/ESI-MS) was utilized to detect the nucleic acids of bacteria, viruses, and fungi. Results Patients whose amniotic fluid samples evinced microorganisms but did not indicate inflammation had a similar perinatal outcome to those without microorganisms or inflammation [amniocentesis-to-delivery interval (p=0.31), spontaneous preterm birth before 34 weeks (p=0.83), acute placental inflammatory lesions (p=1), and composite neonatal morbidity (p=0.8)]. Conclusions The isolation of microorganisms from a sample of amniotic fluid in the absence of intra-amniotic inflammation is indicative of a benign condition, which most likely represents contamination of the specimen during the collection procedure or laboratory processing rather than early colonization or infection.

BACKGROUND AND OBJECTIVES The skin microbiota partly determined by epidermal barrier plays an important role in acne vulgaris and intense pulsed light (IPL) has been verified as a safe and effective therapeutic option for this disease. Nevertheless, the exact role of the IPL treatment on the skin microbiota and epidermal barrier for patients with acne vulgaris remains unclear. This article was designed to solve this problem. STUDY DESIGN/MATERIALS AND METHODS Nineteen healthy controls and 20 patients with mild to moderate acne were enrolled in this study, who received IPL treatment for 12 weeks. The epidermal barrier and skin samples were collected at baseline and after treatment. BACKGROUND Acne vulgaris is a chronic inflammatory skin disease of the pilosebaceous units which can affect the individual's physiological and psychological health. Abnormal growth of lipophilic anaerobic bacteria such as Propionibacterium acnes is reported to be a major factor in the development of acne. However, the relationship between skin microorganisms and acne has not been fully elucidated. Our study aimed to explore the microbial differences between patients with acne and healthy controls (HCs). METHODS The study involved 16 participants diagnosed with acne vulgaris and 5 HCs. We collected skin microbe samples from the cheeks, brow, forehead, neck, chin, or chest of the participants with sterile cotton swabs depending on the location of the acne lesions. Cutaneous microbe samples from the participants were tested by 16s sequencing. RESULTS Patients with acne showed increased diversity of skin microbiota in their samples. OTU535601 (Lachnospiraceae), OTU4460604 (Clostridiales), OTU3217705 (Moraxellaceae), OTU1066814 (Prevotella), and OTU455671 (Lactococcus garvieae) were the top 5 most abundant species found in patients with acne but were not present in HCs. OTU423327 (Achromobacter), OTU4423360 (Stenotrophomonas), OTU993127 (Porphyromonas), OTU677680 (Prevotella), and OTU269901 (Pseudomonas) were the top 5 most abundant species in HCs but were not found in patients with acne. CONCLUSIONS The present study has analyzed and compared the diversity and abundance of microorganisms and the characteristics of the main pathogenic bacteria in patients with acne and HCs. Our findings indicate the importance of maintaining the skin's commensal microflora balance with the development of acne vulgaris.
