Peer reviewed: True, INTRODUCTION: Rapid evaluations can offer evidence on innovations in health and social care that can be used to inform fast-moving policy and practise, and support their scale-up according to previous research. However, there are few comprehensive accounts of how to plan and conduct large-scale rapid evaluations, ensure scientific rigour, and achieve stakeholder engagement within compressed timeframes. METHODS: Using a case study of a national mixed-methods rapid evaluation of COVID-19 remote home monitoring services in England, conducted during the COVID-19 pandemic, this manuscript examines the process of conducting a large-scale rapid evaluation from design to dissemination and impact, and reflects on the key lessons for conducting future large-scale rapid evaluations. In this manuscript, we describe each stage of the rapid evaluation: convening the team (study team and external collaborators), design and planning (scoping, designing protocols, study set up), data collection and analysis, and dissemination. RESULTS: We reflect on why certain decisions were made and highlight facilitators and challenges. The manuscript concludes with 12 key lessons for conducting large-scale mixed-methods rapid evaluations of healthcare services. We propose that rapid study teams need to: (1) find ways of quickly building trust with external stakeholders, including evidence-users; (2) consider the needs of the rapid evaluation and resources needed; (3) use scoping to ensure the study is highly focused; (4) carefully consider what cannot be completed within a designated timeframe; (5) use structured processes to ensure consistency and rigour; (6) be flexible and responsive to changing needs and circumstances; (7) consider the risks associated with new data collection approaches of quantitative data (and their usability); (8) consider whether it is possible to use aggregated quantitative data, and what that would mean when presenting results, (9) consider using structured processes & layered analysis approaches to rapidly synthesise qualitative findings, (10) consider the balance between speed and the size and skills of the team, (11) ensure all team members know roles and responsibilities and can communicate quickly and clearly; and (12) consider how best to share findings, in discussion with evidence-users, for rapid understanding and use. CONCLUSION: These 12 lessons can be used to inform the development and conduct of future rapid evaluations in a range of contexts and settings.

Scoping review of rapid evaluations in health and social care interventions

We have measured the changes in the production of volatile organic compounds (VOCs) by the oral pathogen Porphyromonasgingivalis, when treated in vitro with the antibiotic amoxicillin. We have also measured the VOC production of P.gingivalis grown in the presence and absence of supplemental hemin. Planktonic bacterial cultures were treated with different amounts of amoxicillin in the lag phase of the bacterial growth. Planktonic bacteria were also cultured with and without supplemental hemin in the culture medium. Concentrations of VOCs were measured with proton-transfer-reaction time-of-flight mass spectrometry (PTR-ToF–MS) and further molecular identification was done with gas chromatography–mass spectrometry (GC–MS) using solid phase microextraction (SPME) for sampling. The cell growth of P.gingivalis in the cultures was estimated with optical density measurements at the wavelength of 600 nm (OD600). We found that the production of methanethiol, hydrogen sulfide and several short- to medium-chain fatty acids was decreased with antibiotic treatment using amoxicillin. Compounds found to increase with the antibiotic treatment were butyric acid and indole. In cultures without supplemental hemin, indole and short- to medium-chain fatty acid production was significantly reduced. Acetic acid production was found to increase when supplemental hemin was not available. Our results suggest that the metabolic effects of both antibiotic treatment and supplemental hemin availability are reflected in the VOCs produced by P.gingivalis and could be used as markers for bacterial cell growth and response to threat. Analysis of these volatiles from human samples, such as the exhaled breath, could be used in the future to rapidly monitor response to antibacterial treatment.

As a vitamin C–rich fruit, choosing the eating time for kiwifruit with the best quality during the shelf period is still a problem for consumers. This paper mainly focuses on the correlation between cold storage time, quality indexes, volatile flavor compounds of postharvest kiwifruit and RGB value readouts from photos taken by mobile phone. Results indicated that the R to B ratio values (Central R/B) and B to G ratio values (Central B/G) of the central site of kiwifruit were strongly associated with storage time and all quality indicators. The central R/B was negatively correlated with titratable acidity, vitamin C and 2,6-Nonadienal contents and firmness and positively correlated with storage time, weight loss, soluble solids content, total soluble sugars, total plate counts and 1,3-Cyclooctadiene. We provide a novel and smart strategy to predict the shelf life and quality parameters of kiwifruit by capturing and calculating RGB values using a smartphone.

This national study investigated the positives reported by residents experiencing the large-scale public health measures instituted in Australia to manage the first wave of the COVID-19 pandemic in 2020. Most Australians had not previously experienced the traditional public health measures used (social distancing, hand hygiene and restriction of movement) and which could potentially impact negatively on mental well-being. The research design included qualitative semi-structured phone interviews where participants described their early pandemic experiences. Data analysis used a rapid identification of themes technique, well-suited to large-scale qualitative research. The ninety participants (mean age 48 years, 70 women) were distributed nationally. Analysis revealed five themes linked with mental well-being and the concept of silver linings: safety and security, gratitude and appreciation, social cohesion and connections, and opportunities to reset priorities and resilience. Participants demonstrated support for the public health measures and evidence of individual and community resilience. They were cognisant of positives despite personal curtailment and negative impacts of public health directives. Stories of hope, strength, and acceptance, innovative connections with others and focusing on priorities and opportunities within the hardship were important strategies that others could use in managing adversity.

Wetlands are one of the world’s three major ecosystems. They not only maintain regional ecological balance but also provide an important guarantee for human survival. Wetland ecosystem health assessment serves as the foundation for wetland protection, management, and restoration. In this study, the method for wetland ecosystem health assessment proposed by the United States Environmental Protection Agency (US EPA) was selected and improved to systematically evaluate the health status of the Cuihu wetlands’ ecosystem at three levels. The results revealed that the Cuihu wetlands’ landscape development intensity index was 1.55, the total landscape pattern value was 10 points, and the total score for rapid evaluation was 0.79. Levels I and II indicated that the Cuihu wetlands’ ecosystem was in a good near-natural state. Additionally, level III revealed that ecosystem health is higher in area B than in area A. The Cuihu wetlands were characterized by low species diversity and low distribution of benthic animals and aquatic plants. The comprehensive evaluation results revealed that the Cuihu wetlands’ ecosystem is in a good health. In the future, the health status of the wetland ecosystem should be monitored regularly, the cultivation and propagation of aquatic plants should be strengthened, and effective methods to improve water quality and reduce soil salinity should be used to achieve the best health status of the Cuihu wetlands.

Rapid evaluation of aerodynamic forces is a key technology in conceptual and preliminary performance assessment and design optimization for supersonic/hypersonic vehicles, which is expected to achieve a balance between accuracy and time. In this paper, the preliminary evaluation platform of aerodynamic forces is built based on the physical theories and engineering methods. This platform computes the local velocity vector by using the streamline tracing technology, and extracts the pressure and friction coefficients by calling methods in method database. Furthermore, an example based on HL-20 is brought out. A good agreement is obtained by comparing the calculated results via CFD with the experimental. Also, a good angle of attack compatibility of streamline tracing technology is concluded. This platform has a computation time less than that via CFD and an flight range greater than that via wind tunnel. Finally, the simple flight dynamics simulation is taken out and the synergetic process is verified.

This study aimed to develop a short version of the POFRAS - Preterm Oral Feeding Readiness Scale by removing items found by previous studies to be less discriminative. We also evaluated the accuracy of the reduced instrument in the assessment of oral feeding readiness in premature neonates. An observational, retrospective database study was conducted on a sample of neonates evaluated using the POFRAS. The overall accuracy, sensitivity, specificity, and cutoffs for the short version of the POFRAS were determined using receiver operating characteristic curves. Data analysis involved descriptive methods, normality tests (Shapiro-Wilk tests), and comparisons (Student’s t-test or Mann-Whitney U tests) of individuals classified according to oral feeding readiness. Results were considered significant at p < 0.05. The cutoff for the POFRAS was set to 30 while that of the POFRASreduced was 19. The overall accuracy rates for the POFRAS and POFRAS-reduced were 69.23 and 69.89%, respectively. Both instruments classified a similar percentage of participants as “Not ready” (36.62%) and “Ready” (63.38%) for oral feeding. Preterm neonates classified as ready for oral feeding according to the POFRAS and POFRAS-reduced transitioned to full oral feeding approximately 5 days before the remainder of the sample. This difference was statistically significant. Neonates classified as ready for oral feeding were also discharged from the hospital approximately 4 days earlier than their peers, although this difference was not statistically significant. In conclusion, the removal of less discriminative items did not influence the accuracy of the instrument at assessing oral feeding readiness. The shorter duration of infant manipulation required by the POFRAS-reduced can also be considered an advantage over the original POFRAS. Objetivou-se verificar a acurácia de uma modificação do instrumento POFRAS - Preterm Oral Feeding Readiness Scale para avaliação da presença de prontidão do prematuro para iniciar a alimentação por via oral. Foram retirados itens considerados de menor poder discriminatório, segundo estudos anteriores, resultando em uma versão reduzida do instrumento. Trata-se de um estudo observacional, retrospectivo, por meio de coleta em banco de dados de recém-nascidos avaliados pelo POFRAS, em estudo prévio. A obtenção da acurácia global, da sensibilidade, especificidade e do melhor ponto de corte para o POFRAS - reduzido foi realizada através da construção da curva ROC (Receiver Operating Characteristic Curves). RESUMO Objetivo Desenvolver e validar o conteúdo de instrumento de avaliação de linguagem pautado na Comunicação Suplementar e Alternativa (CSA). Métodos O instrumento foi elaborado a partir de revisão bibliográfica, em conjunto com a experiência clínica da pesquisadora. Em seguida, foi avaliado por juízes especialistas na área e uma nova versão foi construída, incorporando as colaborações dos juízes. Resultados O instrumento foi elaborado em 4 eixos de conteúdo, com instruções e sugestão de contextos e símbolos gráficos. A partir da avaliação dos juízes, o instrumento foi aprimorado e proposto, em sua versão final. Conclusão O instrumento denominado CSA_Linguagem se mostrou operacional, de fácil e rápida aplicação e baixo custo. Contudo, esta pesquisa tem limitações, na medida em que não há instrumentos de avaliação de linguagem padronizados, que considerem a utilização de símbolos gráficos como possibilidade de comunicação. ABSTRACT Purpose Content development and validation of a language assessment instrument lined on Augmentative and Alternative Communication (AAC). Methods The instrument was elaborated from a bibliographic review, along with the clinical experience of the researcher. Then, It was evaluated by expert judges in the area and a new final version was constructed, with computed collaboration. Results The Instrument was developed in four content axes, with instructions and suggestion of contexts and graphic symbols. Based on the evaluation of judges, the instrument was improved and the final version was proposed. Conclusion The instrument, named CSA_Linguagem, was proved to be operational, easy, of rapid application and low cost. However, this research has limitations, there are no standardized language assessment tools that consider the use of graphic symbols as communication possibilities.

Background Periodic assessment is one of the recommendations for improving health-care waste management worldwide. This study aimed at translating and adapting the Health-Care Waste Management - Rapid Assessment Tool (HCWM-RAT), proposed by the World Health Organization, to a Brazilian Portuguese version, and resolving its cultural and legal issues. The work focused on the evaluation of the concepts, items and semantic equivalence between the original tool and the Brazilian Portuguese version. Methods A cross-cultural adaptation methodology was used, including: initial translation to Brazilian Portuguese; back translation to English; syntheses of these translation versions; formation of an expert committee to achieve consensus about the preliminary version; and evaluation of the target audience’s comprehension. Results Both the translated and the original versions’ concepts, items and semantic equivalence are presented. The constructs in the original instrument were considered relevant and applicable to the Brazilian context. The Brazilian version of the tool has the potential to generate indicators, develop official database, feedback and subsidize political decisions at many geographical and organizational levels strengthening the Monitoring and evaluation (M&E) mechanism. Moreover, the cross-cultural translation expands the usefulness of the instrument to Portuguese-speaking countries in developing regions. Using OCA analyst can rapidly detect the project highlights along its critical path of the technology transfer project. At the end of the evaluation process, qualification of the project performance is performed using a coarse scale to measure the technology transfer capacity of the participant organizations. To prove the methodology proposed, we explore the economic and social impact results of some technology transfer projects developed by academic groups of the National Autonomous University of Mexico (UNAM, by its Spanish acronym). The conclusion is that in order to have a successful TTP, at least three conditions are necessary: (a) the project must be handled by an efficient technology transfer team (T3) ; (b) the technology transferred must be mature; and (c) the technology transferred must have economic and social impacts for the final organization or other users inside the receiver country.

In this study, seismic performance of one story hinged precast buildings, which represents the majority of existing lightweight industrial building stock of Turkey, was assessed. A lot of precast buildings, constructed in one of the important seismic zones of western Turkey, were investigated and building inventories were prepared. By this method, structural properties of inventory buildings and damaged precast buildings in recent earthquakes were compared. Damage estimations based on nonlinear analysis methods have shown that estimated damage levels of inventory buildings and observed damage levels in recent earthquakes are similar. Accuracy of damage estimation study and the simplicity of the one story precast building models implied that rapid seismic performance assessment method for these buildings can be developed. In this assessment method, capacity curves and vibration periods of precast buildings were calculated by using structural properties of precast buildings. The proposed assessment method was applied to inventory buildings by using two different seismic demand scenarios which reflect moderate and soft soil conditions. Comparison of detailed analysis and rapid assessment methods have indicated that reliable seismic performance estimations can be performed by using proposed method. It is also observed that distribution of damage estimations is compatible in both scenarios. © 2013 Techno-Press, Ltd.

Acoustic rhinometry, rhinomanometry and Inspiratory Peak Nasal Flow (IPNF) are used in order to objectively assess nasal patency. These are expensive not very practical tests, except for IPNF, which is a fast, simple and low cost method. OBJECTIVE: To assess IPNF in healthy individuals complaining of nose obstruction caused by allergic rhinitis. METHOD: IPNF use in 78 individuals with and without rhinitis symptoms. STUDY DESIGN: Contemporary cross-sectional cohort. RESULTS: IPNF showed significant results for nasal obstruction, rhinorrhea, pruritus, sneezes and tearing (p < 0. 001). There was no correlation between the presence of nasal septum deviation and IPNF (p = 0. 561). We found a positive correlation between IPNF and the Visual Analogue Scale (VAS) for nasal obstruction (p = 0. 002). In the multiple linear regression model, there was a statistical significance between the values found in IPNF with allergic rhinitis and age (p = 0. 005 and p = 0. 023 respectively). O objetivo deste estudo foi verificar a ocorrência de disfagia após acidente vascular cerebral (AVC) isquêmico agudo durante as primeiras 48 horas de aparecimento dos sintomas para o estabelecimento de uma possível relação entre o nível de comprometimento neurológico e o grau de severidade da disfagia. Após a admissão hospitalar de emergência, três pacientes passaram por avaliação clínica neurológica, composta por exame físico geral, exame neurológico e aplicação da NationalInstitute of Health Stroke Scale (NIHSS); e avaliação clínica da deglutição por meio do Protocolo Fonoaudiológico de Avaliação do Risco para Disfagia (PARD). Dos pacientes avaliados, um apresentou deglutição funcional, com NIHSS 11, e dois apresentaram disfagia orofaríngea leve e moderada, sendo o NIHSS 15 e 19, respectivamente. O fluxo do serviço e a procura tardia dos pacientes por auxílio médico determinaram o baixo número de amostra. Os resultados obtidos confirmam os dados da literatura em relação à gravidade do paciente neurológico e à manifestação de disfagia. The aim of this case study was to verify the occurrence of dysphagia in acute ischemic stroke within 48 hours after the onset of the first symptoms, in order to establish a possible relationship between the level of neurologic impairment and the severity degree of dysphagia. After emergency hospital admission, three patients underwent neurological clinical evaluation (general physical examination, neurological examination, and application of the National Institute of Health Stroke Scale - NIHSS), and clinical assessment of swallowing using the Protocolo Fonoaudiológico de Avaliação do Risco para Disfagia (PARD - Speech-Language Pathology Protocol for Risk Evaluation for Dysphagia). One of the patients presented functional swallowing (NIHSS score 11), while the other two had mild and moderate oropharyngeal dysphagia (NIHSS scores 15 and 19, respectively). The service flow and the delay on the patients' search for medical care determined the small sample. The findings corroborate literature data regarding the severity of the neurological condition and the manifestation of dysphagia.

Para a avaliacao objetiva da patencia nasal sao utilizados a rinometria acustica, rinomanometria e o peak flow nasal inspiratorio (PFNI). Estes exames, com excecao do PFNI, sao dispendiosos e complexos. Para utilizacao na Medicina do Trabalho e mesmo na pratica clinica diaria, faz-se necessario um metodo de mensuracao simples, rapido, de baixo custo e confiavel. Atualmente, os estudos tendem a correlacionar metodos que reunem estas caracteristicas, com aqueles metodos ja bem documentados. Objetivos: comparar os resultados do PFNI com achados da Escala Analogica Visual (EVA), antes e apos o uso de vasoconstritor nasal em individuos assintomaticos. Em um segundo momento, avaliar o PFNI entre individuos saudaveis e com queixas nasais obstrutivas. Forma de estudo: Artigo 1: Estudo de coorte em que a variavel em questao e a ralacao entre o PFNI e a EVA para obstrucao nasal antes e apos vasoconstricao nasal em individuos saudaveis; Artigo 2: Estudo transversal, em que a variavel estudada e o comportamento do PFNI em uma populacao de individuos sem e com queixas nasais obstrutivas. Metodos: Dissertacao realizada em formato alternativo composta de dois artigos. No primeiro foi feito uma correlacao entre o resultado do PFNI e EVA antes e apos vasoconstricao nasal de uma amostra de 60 sujeitos saudaveis. No segundo artigo foram avaliados 78 sujeitos com e sem sintomas nasais obstrutivos. Ambos estudos tiveram alocacao de sujeitos de forma nao randomizada. Analizira se razvoj modela za brzu procjenu vremena građenja, te istražuje mogućnost razvoja i primjene izvornog modela pogodnog za primjenu u Hrvatskoj. Prikazane su glavne karakteristike najznačajnijih modela, te njihova usporedna analiza s ocjenom praktične uporabljivosti. Opisani su rezultati istraživanja provedenog u Hrvatskoj tijekom 2003/05., kojima su predloženi izrazi za proračune održivog roka građenja za nekoliko skupina građevina. Dan je primjer iz visokogradnje., Development of models for rapid evaluation of construction time is analyzed, and possibilities are studied for the development and use of the initial model suitable for use in Croatia. Main properties of the most significant models are presented, including comparative analyses and evaluation of practical usability. The authors present results obtained by the study conducted in Croatia in 2003/05, in which expressions are proposed for calculating sustainable construction times for several groups of buildings. An example from the sphere of building construction is given.

ÖZET Ülkemizin büyük bir bölümü deprem riski altodadır. 1998 Adana ve 1999 Marmara depremleri sadece konutların bulunduğu yerleşim bölgelerinde değil, ülkemizin önemli yatırımlarının bulunduğu sanayi bölgelerinde de ciddi hasara sebep olmuştur. Ülkemizde sanayi yapılarının büyük çoğunluğu prefabrik, tek katlı, birleşimi mafsallı yapılardan oluşmaktadır. Bu yapıların ülke ekonomisi açısından önemi büyüktür. Yaşanan deprem felaketleri sadece binaların değil, binaların içine yatırılan çok büyük miktardaki sermaye birikiminin de kaybolmasına sebep olmaktadır. Bu durum muhtemel bir deprem felaketinden önce prefabrik sanayi yapılarının deprem davranışlarının daha ayrıntılı olarak incelenmesi, takviye gerektiren yapıların belirlenmesi için hızlı değerlendirme kriterlerinin saptanması gerekliliğini ortaya koymaktadır. Bu çalışmada, tek katlı, birleşimi mafsallı sanayi yapılarının deprem davranışlarının ve performanslarının belirlenebilmesi için Kapasite Spektrumu Yöntemi kullanılmıştır. Elde edilen veriler yardımı ile bu tarz yapılar için basitleştirilmiş bir hızlı değerlendirme yöntemi geliştirilmiştir. Ali Haydar KAYHAN VI ABSTRACT 1998 Adana and 1999 Marmara earthquakes caused many precast industrial buildings to be damaged. In this study, seismic behaviour of one-storey, pin connected precast industrial buildings and the effect of some structural and nonstructural parameters on this behaviour are investigated by using Capacity Spectrum Method. These parameters are building height, column dimensions, reinforcement ratio, roof girder length, concrete class and soil class. Then, the results obtained by using Capacity Spectrum Method and seismic design provisions of current Turkish earthquake code are compared. Two types of failure criteria were used for this aim: flexural hinges at the base of the columns and pounding of the precast elements at the roof level. Finally, a statistical study is carried out to find an equation for rapid evaluation of performance levels of one-storey, pin connected precast buildings.
