Abstract Question In people who have undergone surgical carpal tunnel release, do sensorimotor-based exercises performed on the touchscreen of a tablet device improve outcomes more than a conventional home exercise program prescribed on paper? Design Randomised, parallel-group trial with concealed allocation, assessor blinding, and intention-to-treat analysis. Participants Fifty participants within 10 days of surgical carpal tunnel release. Intervention Each participant was prescribed a 4-week home exercise program. Participants in the experimental group received the ReHand tablet application, which administered and monitored exercises via the touchscreen. The control group was prescribed a home exercise program on paper, as is usual practice in the public hospital system. Outcome measures The primary outcome was functional ability of the hand, reported using the shortened form of the Disabilities of the Arm, Shoulder and Hand (QuickDASH) questionnaire. Secondary outcomes were grip strength, pain intensity measured on a 10-cm visual analogue scale, and dexterity measured with the Nine-Hole Peg Test. Outcomes were measured by a blinded assessor at baseline and at the end of the 4-week intervention period. Results At Week 4, functional ability improved significantly more in the experimental group than the control group (MD –21, 95% CI –33 to –9) on the QuickDASH score (0 to 100). Although the mean estimates of effect on the secondary outcome also all favoured the experimental group, none reached statistical significance: grip strength (MD 5.6 kg, 95% CI –0.5 to 11.7), pain (MD –1.4 cm, 95% CI –2.9 to 0.1), and dexterity (MD –1.3 seconds, 95% CI –3.7 to 1.1). Conclusion Use of the ReHand tablet application for early rehabilitation after carpal tunnel release is more effective in the recovery of functional ability than a conventional home exercise program. Background: Among the various components of disaster management during unexpected accidents, health care systems, especially hospitals, play the most important role in providing primary care services in the initial phase of the disaster. Health services in natural and humanmade disasters are the main cause of human survival. Accidents and disasters always have a significant impact on the safety, general health, and welfare of the affected population. Materials and Methods: In this paper, by collecting relevant data and their analyses in SPSS, we reviewed the response of the headquarters of the operation command of Alborz University of Medical Sciences to the Kermanshah earthquake in November 2017. Results: According to the obtained results, 25-27 years old and 80 years old groups had the highest number of casualties to receive relief services. Based on the results of the analysis in SPSS, the number of injured men who received health care services was higher than women (38 men vs. 37 women). Madani Hospital admitted the highest number of victims by accepting 54 (72%) injured people. Based on our analysis, multiple trauma had the highest prevalence (24%) among injured people, then trauma to the foot (9%), followed by the trauma of the lower back and hand fractures (6.8%). Of the total 75 injured people transferred to the hospital, 39 (52%) underwent the operation. One case was sent to a hospital in Tehran Province, and 7 (9%) casualties were settled temporarily. There was a case of amputation. One patient was re-hospitalized. Finally, 4 (5%) of the injured people left the hospital. Conclusion: Regarding the services provided, it can be acknowledged that the headquarters of the Crisis Operation of the Alborz University of Medical Sciences (in spite of its new establishment) had an excellent performance regarding the type and amount of the services provided to the injured people in the field of risk management of accidents/disasters in Kermanshah earthquake in November 2017. Some of the positive and essential measures of the headquarters of crisis operation command were planning operations; triage at the airport; transfer of the injured people; coordination with the hospitals; declaration of yellow status to all medical centers; use of the full capacities of the Medical Sciences University; assignment of beds and the issue of surge capacity of health centers; communication with organizations, centers, and hospitals affiliated to Alborz University of Medical Sciences. The article presents a synopsis on comprehensive non-surgical treatment which leads to improved walking ability in people with lumbar spinal stenosis. Topics discussed include dominant symptom of people with lumbar spinal stenosis; marginal differences between treatment groups in other more traditional outcomes; and training program offered to the comprehensive training and motivated patients.

Background and objective Lung cancer is the leading cause of cancer-related death, of which non- small cell lung cancer (NSCLC) is the most common type. Immune checkpoint inhibitors (ICIs) have now become one of the main treatments for advanced NSCLC. This paper retrospectively investigated the effect of peripheral blood inflammatory indexes on the efficacy of immunotherapy and survival of patients with advanced non-small cell lung cancer, in order to find strategies to guide immunotherapy in NSCLC. Methods Patients with advanced non-small cell lung cancer who were hospitalized in The Affiliated Cancer Hospital of Nanjing Medical University from October 2018 to August 2019 were selected to receive anti-PD-1 (pembrolizumab, sintilimab or toripalimab) monotherapy or combination regimens. And were followed up until 10 December 2020, and the efficacy was evaluated according to RECIST1.1 criteria. Progression-free survival (PFS) and overall survival (OS) were followed up for survival analysis. A clinical prediction model was constructed to analyze the predictive value of neutrophil-to-lymphocyte ratio (NLR) based on NLR data at three different time points: before treatment, 6 weeks after treatment and 12 weeks after treatment (0w, 6w and 12w), and the accuracy of the model was verified. Results 173 patients were finally included, all of whom received the above treat-ment regimen, were followed up for a median of 19.7 months. The objective response rate (ORR) was 27.7% (48/173), the disease control rate (DCR) was 89.6% (155/173), the median PFS was 8.3 months (7.491-9.109) and the median OS was 15.5 months (14.087-16.913). The chi-square test and logistic multi-factor analysis showed that NLR6w was associated with ORR and NLR12w was associated with ORR and DCR. Further Cox regression analysis showed that NLR6w and NLR12w affected PFS and NLR0w, NLR6w and NLR12w were associated with OS. Conclusion In patients with advanced non-small cell lung cancer, NLR values at different time points are valid predictors of response to immunotherapy, and NLR <3 is often associated with a good prognosis. This paper describes psychocardiology studies conducted from 2002 through 2018 in the Cardiology Department of the Dr Gustavo Aldereguía Lima University General Hospital in Cienfuegos Province, Cuba. Given the strong association between cardiovascular diseases and lifestyle, negative emotions and personality traits, psychology and medicine are equally necessary components of prevention and patient care, indispensable to primary and secondary prevention and to rehabilitation. When its therapeutic principles are appropriately applied, psychology can have a positive impact on the course of the disease and on patients' adaptation to new habits and lifestyles. The psychocardiologist's job is fundamental in achieving conscious participation by patients in their rehabilitation. Theoretical and practical contributions include a gender-based approach, addressing patients' sexual needs, neuropsychological assessment of damage associated with cardiovascular diseases, the particularities of pediatric patient care, family involvement in rehabilitation, and services for families at risk for cardiovascular events due to genetic factors. Background: Visitors to hospitalized patients during an epidemic might themselves be carriers and are therefore likely to spread the disease in wards. Although measures were taken to restrict hospital visits, traditional paper-based registration is insufficient to screen and monitor the numbers of visitors to a large hospital. Methods: Throughout March 2020, during the coronavirus disease 2019 crisis, a computer system was deployed in the 2800-bed Taipei Veterans General Hospital (Taipei, Taiwan) to register, screen, and monitor inpatient visitors. This system comprised three parts: online registration form, entrance check-in interface, and registration database. The early utilization of this newly deployed system was then analyzed. Results: A total of 22,336 visits were recorded between March 11, 2020, and March 31, 2020, with 1064 a day on average. Out of these visits, 18.1% (n = 4049) had made online reservations within 48 hours. On the other hand, of all 4941 online reservations, 18.1% (n = 892) were no-shows. In the last 12 days of the study period, eight prospective visitors were identified as ineligible by the computer system, and so their visits were denied. Conclusion: Using a computer system, the hospital was able to enforce restrictions on hospital visits. Although the online registration system had not been fully used yet in the early phase of adoption, its superiority from the standpoint of disease control should enable hospital managers to consider abolishing on-site visitor registration. Malignant melanoma is a kind of tumor produced by human melanocytes. It has a high degree of malignancy, early metastasis and high mortality. Most melanomas are caused by malignant skin nevus and can also be seen in the digestive tract such as rectum and anus. Primary malignant melanoma of pleura is rare, rarely seen in case reports. This paper reports the clinical data of a case of malignant melanoma with cough, expectoration and pleural effusion as the first symptoms diagnosed by thoracoscopy combined with pathology in Beijing Friendship Hospital Affiliated to Capital Medical University, and analyzes and summarizes the literature data. Background and objective The patients with advanced lung adenocarcinoma should select targeted drugs based on the type of tumor epidermal growth factor receptor (EGFR) gene mutation. However, it is difficult to collect tumor tissue of advanced lung adenocarcinoma, and some experts agree that peripheral blood can be used as a substitute for tumor tissue as a test specimen. This paper aimed to investigate the clinical value of ddPCR and super-amplification refractory mutation system (ARMS) in detecting EGFR gene mutation in peripheral blood of patients with advanced lung adenocarcinoma. Methods A total of 119 patients diagnosed in Beijing Chest Hospital Affiliated to Capital Medical University from February 2016 to February 2019 were collected, and the sensitivity and specificity of plasma ctDNA EGFR gene mutation detected by ddPCR and super-arms were compared. Some patients with positive EGFR gene mutations received oral treatment with first-line EGFR tyrosine kinase inhibitors (EGFR-TKI). The patients were divided into subgroups according to the test results. In group 1, both ddPCR and super-arms showed positive EGFR gene mutation results, with 21 cases. In group 2, ddPCR and super-arms detection of EGFR gene mutation were all negative, with 16 cases. In group 3, the ddPCR test was positive and the super-arms test was negative, with 5 cases. In group 4, the ddPCR test result was negative while the super-arms test result was positive. Since the number of patients in group 4 was 0, no statistics were included. Objective response rate (ORR) and disease control rate (DCR) were used to evaluate the short-term outcome, and progression-free survival (PFS) was compared with survival analysis to evaluate the long-term outcome. Results EGFR mutations were detected in 58 (48.7%) of 119 patients with advanced lung adenocarcinoma. The coincidence rate between ddPCR and EGFR gene mutation in tumor tissues was 82.4% (Kappa=0.647, P<0.001), the sensitivity was 74.1%, and the specificity was 90.2%. However, the coincidence degree of superarms test and tissue test was 71.4%, the sensitivity was only 58.6%, and the specificity was 83.6%. The ORR and DCR values in group 3 were lower than those in group 1 and 2, but there was no significant difference in ORR between groups (P>0.05). Survival analysis showed that the PFS of the three groups was compared. The difference was not statistically significant (^2=2.221, P=0.329). Conclusion ddPCR, as a high sensitivity and specificity liquid gene detection method, can be used as a reliable method to detect the mutation of plasma ctDNA EGFR gene in patients with advanced lung adenocarcinoma. The results of plasma genetic testing can also be used as the basis for predicting the efficacy of EGFR-TKIs in patients. [ABSTRACT FROM AUTHOR]

Statement of the Problem: Swelling of the surrounding tissue of surgical site is one of the most common, devastating effects after maxillofacial surgery and can have an effect on quality of life and the patients' satisfaction. Purpose: The purpose of this study was to investigate the effect of a custom-made appliance by the method of Computer-aided design/Computer-aided manufacturing (CAD/CAM) compared with conventional dressing for reducing facial swelling after maxillofacial surgery. Materials and Method: This clinical trial accomplished in 14 patients voluntarily referred to the Department of Oral and Maxillofacial Surgery at Bahonar hospital in Kerman. These patients were the candidate for surgical treatment of similar bilateral facial fractures (10 patients) and orthognathic surgery (4 patients). Facial computed tomography (CT) scans of the patients were taken to build up a custom-made appliance. Based on three-dimensional reconstruction from facial CT scan data, custom-made appliance was designed and built. Swelling on one side with custom-made appliance and on the opposite side with conventional dressing measured with paper tape graded before surgery, immediately after surgery, 24 hours, 48 hours, and 7 days later, we divided the patients into two groups of trauma and orthognathic surgical procedures Results: Based on the defined landmarks, all of the measurements were performed on both sides. Statistical analyses do not show any asymmetry before surgery (p= 0.48). In addition, there was no significant difference between two methods of dressing on the day of surgery and a day later (p= 0.084, p= 0.09), but there was a significant difference between two methods of dressing on second and seventh days. The swelling has decreased faster and further in our new method. Conclusion: Regarding the exact adaptation to the surgical site on facial skin, using custommade appliance by the method of CAD/CAM could be an effective tool that reduces swelling after maxillofacial surgery. The objective of this study was to analyze the recording of the nursing process, supported by information and communication technologies in both printed and electronic media in the neonatal intensive care scenario. This case study was exclusive, integrated, and conducted between January and April 2014. The study counted on the participation of seven nurses who worked at a neonatal ICU before and after the deployment of new information and communication technologies, which combined electronic and physical (paper) support. Data were collected from medical records and a questionnaire answered by the nurses. Simple and percentage frequency in the levels of the nursing process application were used for analysis, as well as the set of intervening factors related to the work organization structure and process. Positive and negative results were seen, as well as intervening factors. The study concluded that the new information and communication technologies delivered in physical materials accounted for the registration of the higher number of records in the data collection and that the registration of the other stages did not show any substantial improvement. Background: Treacher Collins syndrome (TCS) is a rare disorder with different levels of severity due to congenital head and face abnormalities which affect language, orofacial musculature, voice and breadth, suction, chewing and swallowing functions. Aims: This paper objectifies to report a Treacher Collins syndrome patient, describing phonoaudiological changes that can be found in individuals suffering from this syndrome. Methods & Procedures: The study was carried out with a male child at a teaching Speech Language Pathology clinic in Southern Brazil. The patient underwent audiological assessment, as well as orofacial motricity, swallowing, voice and speech assessments. Outcomes & Results: By means of the assessments, it was possible to perceive that the patient presents moderate hearing loss, inadequacy of phonoarticulatory organs, base of the tongue going backwards and epiglottis collapse over the larynx. In spite of evidencing significant difficulty in producing speech sounds, due to musculoskeletal changes, he effectively interacts in communicative situations, besides understanding sentences and narrated stories. Conclusions & Implications: The screened case report evidences that an interdisciplinary follow-up is indispensable for this patient suffering from Treacher Collins syndrome, which implies not only speech language therapeutic follow-up, but also in ear, nose and throat and orthodontics areas. Background: A large proportion of surgical patient harm is preventable; yet, our ability to systematically learn from these incidents and improve clinical practice remains limited. The Operating Room Black Box was developed to address the need for comprehensive assessments of clinical performance in the operating room. It captures synchronized audio, video, patient, and environmental clinical data in real time, which are subsequently analyzed by a combination of expert raters and software-based algorithms. Despite its significant potential to facilitate research and practice improvement, there are many potential implementation challenges at the institutional, clinician, and patient level. This paper summarizes our approach to implementation of the Operating Room Black Box at a large academic Canadian center.Objective: We aimed to contribute to the development of evidence-based best practices for implementing innovative technology in the operating room for direct observation of the clinical performance by using the case of the Operating Room Black Box. Specifically, we outline the systematic approach to the Operating Room Black Box implementation undertaken at our center.Methods: Our implementation approach included seeking support from hospital leadership; building frontline support and a team of champions among patients, nurses, anesthesiologists, and surgeons; accounting for stakeholder perceptions using theory-informed qualitative interviews; engaging patients; and documenting the implementation process, including barriers and facilitators, using the consolidated framework for implementation research.Results: During the 12-month implementation period, we conducted 23 stakeholder engagement activities with over 200 participants. We recruited 10 clinician champions representing nursing, anesthesia, and surgery. We formally interviewed 15 patients and 17 perioperative clinicians and identified key themes to include in an information campaign run as part of the implementation process. Two patient partners were engaged and advised on communications as well as grant and protocol development. Many anticipated and unanticipated challenges were encountered at all levels. Implementation was ultimately successful, with the Operating Room Black Box installed in August 2018, and data collection beginning shortly thereafter.Conclusions: This paper represents the first step toward evidence-guided implementation of technologies for direct observation of performance for research and quality improvement in surgery. With technology increasingly being used in health care settings, the health care community should aim to optimize implementation processes in the best interest of health care professionals and patients. Introduction: Revising medical curricula & establishing reformed programs result in refined educational methods which are accompanied by development of active & student-based teaching methods such as Team Based Learning (TBL). This paper describes an experience of implementation and evaluation findings of TBL according to medical students' viewpoints. Methods: This action research was planned for a census sample (n=187) basic science students of Tehran University of Medical Sciences with the new curriculum. After briefing the students about TBL and forming teams we did TBL in the following steps: Individual Readiness Assurance Test (IRAT), Team Readiness Assurance Test(TRAT), appeal, Team Application(TAP), and peer assessment in 15% of each program block. Data gathered by observation recording sheets, and a researcher-made questionnaire, and analyzed using descriptive statistics. Results: A major part of the students (63.8% ) have admitted TBL as a deep learning approach. Students believed that TRATs were useful (46.2%), team application was a learning facilitator in TBL classes (51.7%) and the class management was suitable (60.5%). The observational data showed that the students changed their seats in order to take part in group discussions. They were listening carefully, asking questions and giving reference to support their claims. Some were against this method because it increased their study workload. Conclusion: It seems TBL has a positive effect on deep learning and facilitates learning. The results indicate that TBL results in cooperative & active learning, and it improves class activity and may be, because of the developed interaction between students, it can promote communication skills. Adequate patient flow is an absolute requisite for fiscal health in any clinical environment. In an academic setting, patient visits are also a cornerstone of the curriculum. In order for clinical education to be effective, students and residents must have a reasonable volume of patient encounters. For these reasons, substantial efforts are aimed at maintaining the patient base in a school or college of optometry. This paper presents historical patient volume data of the University of Alabama at Birmingham School of Optometry and describes recent strategic initiatives, which have resulted in growth of patient volume by 23 percent in a single year. A rapid increase in patient visits is achievable, even in an academic health center. Lessons learned have wide applicability, inside and outside the educational arena. [ABSTRACT FROM AUTHOR]

Over the last ten years, faculty at the John A. Burns School of Medicine (JABSOM) and the University of Hawaii (UH) have been actively engaged in ongoing efforts to increase the quantity and improve the quality of biomedical research in the State of Hawaii. JABSOM's Clinical Research Center (CRC), funded in 1995 by the National Center for Research Resources (NCRR) and the Research Centers in Minority Institutions (RCMI) of the National Institutes of Health, has provided research infrastructure that has been essential to these efforts. The CRC and other JABSOM departments and affiliated programs have collaborated with public and private entities within the community, particularly in the area of health, related to diverse racial and ethnic populations. This paper sets forth a number of the significant indicators of research progress, as illustrated primarily through CRC support for various research activities conducted at JABSOM. Medication errors are common in the Intensive Care Unit (ICU) and can result in adverse events. Computer-based systems may reduce errors. Nurses' experiences with medication systems are important to understand. We surveyed ICU nurses in a large, academic, urban hospital before and after introducing an e-system using the Medication Administration System - N urses' Assessment of Satisfaction scale. Each item was scored on six-point scale (strongly agree - strongly disagree) with higher scores indicating greater satisfaction. Overall satisfaction was reported on a 0 - 10 scale (0 = completely dissatisfied; 10 = completely satisfied). Three hundred and twenty-eight surveys were distributed. Response rate was 37% (n=120) before and 35% (n = l 15) after. Overall satisfaction was significant with the new system (6.2 versus 7.0, p <0.01). There were also significantly higher scores on all items related to safety (p<0.05) and two of six items related to access [access to systems that support medication administration (4.4 versus 4.9, p<0.01) and availability of information to manage bad reactions (3.1 versus 3.8, p<0.01)]. Nurses reported the system was effective in reducing and preventing medication errors (3.2 versus 4.5, pcO.OOl). However, there was no difference in perceptions of system efficiency (4.2 versus 4.4, p=0.25) or the proportion who agreed the system was user-friendly (68 versus 71%, p=0.70). At both time points, a large proportion of nurses (45 versus 49%, p=0.25) agreed they stashed medication for patient care. The introduction of an e-medication system was associated with ICU nurses' perceptions of greater safety and increased overall satisfaction with medication processes.
