The central Meseta is a high plateau located in the heart of the Iberian Peninsula. Abundant evidence of Lower and Middle Palaeolithic occupations of the region contrasts with scarce evidence of a human presence during the early Upper Palaeolithic. On this basis, it has been suggested that climatic downturns triggered the temporary abandonment, or near abandonment, of the central Meseta during the Last Glacial period. We conducted three archaeological surveys in Guadalajara province, located in the southern part of the region, in 2009, 2010, and 2017. Survey results, interpreted in the light of a habitat suitability model, support a hypothesis of climate-driven abandonment (or near-abandonment) of the central plateau during the Last Glacial Maximum and suggest that the Tagus River Valley, which links the Spanish interior to the Atlantic seaboard, was a focus for the Palaeolithic occupation of the region at other times. Continuous growth of mobile populations has influenced the global epidemiology of infectious diseases, including chronic and acute viral hepatitis. A prospective observational multicentre study was performed in a Spanish network of imported infections. Viral hepatitis cases from January 2009 to September 2017 were included. Of 14,546 records, 723 (4.97%) had imported viral hepatitis, including 48 (6.64%) acute cases and 675 (93.36%) chronic cases. Of the 48 acute cases, 31 were travellers and immigrants returning from visiting friends or relatives (VFR), while 19 (61%) were acute Hepatitis A or Hepatitis B. Only 18.2% of VFR immigrants and 35% of travellers received pre-travel advice. Acute hepatitis was more frequent in VFR immigrants (AOR 2.59, CI95% 1.20–5.60) and travellers (AOR 2.83, CI95% 1.46–5.50) than immigrants. Of the 675 Chronic cases, 570 were immigrants, and 439 (77%) had chronic Hepatitis B. Chronic hepatitis was more frequent in immigrants (AOR 20.22, CI95% 11.64–35.13) and VFR immigrants (AOR 11.12, CI95% 6.20–19.94) than travellers. Chronic viral hepatitis was typical of immigrants, acute viral hepatitis was common among travellers, and VFR immigrants had mixed risk. Improving pre-travel consultation and screening of immigrants may contribute to preventing new cases of viral hepatitis and avoiding community transmission. [ABSTRACT FROM AUTHOR]

Background Post-artesunate delayed haemolysis is described as hemolytic anemia presenting days after malaria treatment in hyperparasitemic patients. Physiopathological mechanisms and clinical manifestations have not been thoroughly characterised. Methods We conducted a retrospective study of hospitalised malaria patients who received artemisinin derivatives from January 1, 2010 to December 31, 2015. Results 21 patients were included in the study: 11 travellers, 8 travellers visiting friends and relatives and 2 immigrants. Median age was 35.5 years (IQR: 25.7–44.8) and 11 were men. Eight patients received oral and 13 received intravenous (IV) artemisinin-based drugs. Follow-up after the malaria episode was available for 15 patients (12 with IV treatment). Four patients presented with delayed haemolysis 9–14 days after artesunate treatment; all had been admitted with severe malaria, were treated IV and had hyperparasitaemia (17%–33%). Other than hyperparasitaemia, no other factors were associated with artesunate haemolysis. Patients' outcomes were favourable and the only additional therapeutic measure needed was a blood transfusion. Conclusions Delayed haemolysis is a frequent complication in hyperparasitemic malaria treated with IV artesunate. Follow-up is mandatory for at least 2 weeks after treatment initiation. Summary Background Leprosy remains infrequent in non-endemic areas. The objective of this study was to describe the cases of leprosy reviewed at a referral unit for imported diseases in Europe and to compare these findings with published data on imported leprosy. Methods Cases of leprosy evaluated at a referral centre are described and salient features of autochthonous and imported cases are compared. A review of the literature on imported leprosy was performed. Results During the study period, 25 patients with leprosy were followed-up (10 were autochthonous cases and 15 were considered to be imported). Regarding imported cases, the majority were diagnosed in Latin American immigrants (10/15, 67%), mean age was 42 years, there were no differences in gender distribution, estimated average time from arrival in Spain to first visit at the unit was 3 years and from symptom onset to diagnosis was 2 years. Over 80% of imported cases had multibacillary disease and over one third of patients had been previously diagnosed with leprosy. One third had received alternate incorrect diagnoses initially, <50% of patients with imported leprosy completed standard therapy and were considered cured and over one third were lost to follow-up. Conclusions Leprosy remains a complex disease for healthcare professionals unfamiliar with this infection. Manifestations are polymorphic so misdiagnoses and consequent delays in diagnosis are not infrequent and may lead to resulting disabilities. Early diagnosis and management are essential to prevent sequelae and possible transmission. Summary Background Brucellosis is one of the main neglected zoonotic diseases. Several factors may contribute to the epidemiology of brucellosis. Imported cases, mainly in travellers but also in recently arrived immigrants, and cases associated with imported products, appear to be infrequently reported. Methods Cases of brucellosis diagnosed at a referral unit for imported diseases in Europe were described and a review of the literature on imported cases and cases associated with contaminated imported products was performed. Results Most imported cases were associated with traditional risk factors such as travel/consumption of unpasteurized dairy products in endemic countries. Cases associated with importation of food products or infected animals also occurred. Although a lower disease incidence of brucellosis has been reported in developed countries, a higher incidence may still occur in specific populations, as illustrated by cases in Hispanic patients in the USA and in Turkish immigrants in Germany. Imported brucellosis appears to present with similar protean manifestations and both classical and infrequent modes of acquisition are described, leading on occasions to mis-diagnoses and diagnostic delays. Conclusions Importation of Brucella spp. especially into non-endemic areas, or areas which have achieved recent control of both animal and human brucellosis, may have public health repercussions and timely recognition is essential. Up to 40% of cases of imported malaria in Europe are diagnosed in recently arrived migrants, who generally exhibit asymptomatic or mild symptoms and show low parasitaemia (submicroscopic). The study describes the prevalence of malaria infection among asymptomatic Sub-Saharan African migrants (ASSAM) and compares asymptomatic malaria-infected (AMI) vs non-malaria infected patients. An observational, comparative, retrospective study was carried out in ASSAM who underwent a medical examination, between 2010 and 2019 at the National Reference Unit for Tropical Diseases (NRU-Trop) in Madrid, Spain. Medical examination and systematic screening protocol for infectious diseases, including screening for malaria infection b y Polymerase Chain Reaction (PCR) was performed. During the study period, 632 out of 1061 ASSAM were screened for malaria, median age: 24 years (IQR:1–5); median time from arrival to diagnosis: 2 months (IQR:1–5). P. falciparum was the most frequent species: 61 patients (67.8%). Compared to non-malaria infected, AMI subjects had: higher rate of co-infection with S. stercoralis (41.1%VS 22.9%; p < 0.001) and filariae (8.9% VS 2.4%; p = 0.006), lower erythrocyte corpuscular volume (83.6 VS 84.4; p = 0.008) and lower levels of cholesterol (151.0 VS 167.3; p < 0.001). We observed a high prevalence of AMI among ASSAM. This highlights the need to consider routing screening of migrants from endemic areas and to study if such screening could avoid the potential morbidities associated with chronic infection, reduce morbi-mortality of acute malaria and the risk of transmission in host communities. Introduction: Immigrants are increasingly traveling back to their countries of origin to visit friends and relatives (VFRs). They account for an important proportion of all international travelers and have a high risk for certain travel-related infectious diseases. Methods: We describe the spectrum of infectious diseases diagnosed in a cohort of 351 VFRs and compare them with two previously published cohorts: of immigrants and travelers attended at our centre. Results: The most frequent diagnoses observed among VFRs were typical travel-associated infections such as malaria (75 [21.4%]), traveler's diarrhea 17 [4.8%]), intestinal parasites (16 [4.6%]) and dengue (11 [3.1%]). Asymptomatic chronic infectious diseases, such as latent tuberculosis (56 [16%]), chronic viral hepatitis (18 [5.1%]) and filariasis (18 [5.1%]), probably acquired before migration, were also observed. Conclusions: VFRs should thus be approached from two perspectives as concerns imported infectious diseases: as travelers and as immigrants. Etiological studies focusing on the presenting complaint as well as systematic screening for other latent infectious diseases should be performed. [ABSTRACT FROM AUTHOR]

This paper reports on the fifth season of fieldwork for the Palaeoanthropology part of the Desert Migrations Project. Previous seasons established an extensive archaeological map based on a wide survey of the rich record along the edges of the Ubari Sand Sea, a small area of the Messak Settafet, the Wadi al-Ajal, the Wadi ash-Shati and the Wadi Barjuj. These surveys, complemented with small excavations, demonstrated a deep prehistory to the Fazzan, with Mode 1 technologies, Acheulean, Mode 3 (classic Levallois, large blade technologies, and Aterian), and Late Stone Age and Neolithic assemblages, some with pottery and grinding stones. The focus of the 2011 season was the Wadi Barjuj. This is an extensive wadi system to the South of the Messak, running along the northern edge of the Murzuq Sand Sea, and draining towards the east. Geomorphologically, it comprises old channel cut and fill gravels, usually finely graded, with areas of ancient lacustrine duricrusts, some of which are elevated significantly above the landscape. Surveys, complementing those of 2008, revealed a particularly rich and extensive Holocene series of sites, with microliths, pottery and grinding stones. These sites indicate a dense early Holocene occupation which seems associated with both hunter-gatherers and pastoralists. In addition, there was a low frequency of MSA lithics (compared to other survey areas), although one extremely rich site (BJJ4) had a very large number of water-worn, highly diagnostic small bifaces, characteristics of some Africa MSA traditions. Acheulean was relatively rare. One restricted locality (BJJ6) provided further evidence for a Mode 1 presence in the region, similar to that reported earlier from the Messak and Wadi ash-Shati (Mirazón Lahr et al. 2009, 2010). The context for the finds suggests that at various points (luring the Pleistocene the Wadi Barjuj was a low lying, swampy and/or lacustrine region, possibly riverine at times, and attractive to hominin populations because of its relatively rich resource base. This paper reports on the results of the work of the Palaeo team of the DMP in January 2010. The fieldwork was focused on two different areas - the Wadi ash-Shati and the southwestern margin of the Ubari Sand Sea. Work in the Wadi ash-Shati confirmed the existence of Oldowan sites in Fazzan (locality SHT11 and environs), which although undated, represent a major addition to the extent of prehistoric occupation of the area. Further work near the site originally studied by Petit-Maim in the 1980s confirmed the association of a shell layer dated to the last interglacial with archaeological artefacts in situ. These artefacts are of MSA affinity, thus placing Fazzan in the context of other northern African sites of that time. The work in the southwestern margin of the basin represented the first formal archaeological survey of that area. This revealed extensive Holocene and MSA occupations, and in a group of sites at the southern edge of the Hamada Zaqher, also Oldowan lithics. Little if any Acheulean remains were observed, while the latter predominate in the interdunes just north of the Wadi al-Ajal. Overall, the spatial and technological analyses of the sites and remains suggest that there were different resource constraints on hominins in the Middle and later Quaternary, shaping the distribution of industries and raw materials. Furthermore, the relative widespread distribution of Oldowan sites - Wadi ash-Shati, southern Messak, and Hamada Zaqher, shows that the Central Sahara played an important role in the evolutionary geography of Lower Pleistocene hominins in Africa. Differences in fine structure of a polyester tow stabilized at different conditions of temperature, treatment time and compactness have been studied. The effective temperature of thermal treatments, crystallinity, and total and superficial oligomers have been determined. The variation in fine structure due to stabilization treatment through iodine sorption and differential solubility tests has also been studied. It is observed that the temperature is the most important variable in the process as it produces important fine structure variations (crystallinity differences). Multifilaments of polyester stabilised at different temperatures (180-220 °C) in the plant of production were treated under similar thermal conditions to dyeing at high temperature or with carrier. Original substrates and treated substrates were characterized by differential solubility and iodine sorption techniques. Stabilised substrates at different temperatures and after treated present differences of fine structure, specially when this structure is evaluated by differential solubility technique. Patients with leprosy can present with systemic inflammatory complications called leprosy reactions (LR), which can be severe and cause a loss of nerve function. The treatment of choice is prolonged corticosteroid therapy, frequently associated with severe side effects. We have used methotrexate as a corticosteroid-sparing regimen with good results. To evaluate the role of methotrexate in managing LR, we performed a systematic review of the literature including our cases. We evaluated studies, prospective and retrospective, in both adults and children, which included any dose/regimen of methotrexate for the treatment of LR type 1 or 2. The systematic search revealed 261 records that yield 21 patients including our 3 cases (19 adults/two children), who were treated with methotrexate for LR type 1 and 2. There were 14 males. Median age was 35 years (P 25 –P 75 28 to 52). Patients showed lepromatous (7), borderline lepromatous (9) or borderline tuberculoid (3) leprosy, among the 19 cases in which the type of leprosy was specified. As for the type of LR, 15 patients showed erythema nodosum leprosum (ENL), five showed LR type 1 and one showed polyarthritis and previous ENL. Methotrexate at weekly doses ranging from 7.5 mg to 20 mg (median 15 mg per week), typically administered with low-dose corticosteroids, was effective and safe as a corticosteroid-sparing agent. Methotrexate could be a suitable ancillary treatment or alternative to corticosteroids, especially in populations who are more prone to its adverse events. However, this evidence is based only on case reports and short clinical series. • Leprosy reactions can be severe and cause a loss of nerve function. • Treatment of choice is prolonged corticosteroid therapy, which is associated with severe side effects. • Methotrexate is effective as a corticosteroid-sparing regimen for the treatment of complex leprosy reactions. • Nevertheless, quality evidence is still lacking. Tuberculosis (TB) is the leading cause of infectious disease mortality worldwide. We analysed active and latent TB infections (LTBI) from the Spanish Network for the Study of Imported Infectious Diseases by Travellers and Immigrants (+REDIVI). Observational, retrospective, multicentre study of TB and LTBI registered in the +REDIVI network from October 2009 to December 2016. Of 1008 cases of LTBI, 884 (87.7%) were immigrants; 93 (4.5%), immigrants visiting friends and relatives (VFR); 2 (0.9%), VFR-travellers; and 29 (1.1%), travellers. Absolute (N = 157 vs. N = 75) and relative (12.5% vs. 5.9%) frequency decreased over the study period (p = 0.003). Median time to diagnosis was 24.6 months (females 50.3 vs males 11.9; p < 0.001). Of 448 TB cases, 405 (90.4%) were in immigrants; 30 (6.7%), VFR-immigrants; 6 (1.3%), VFR-travellers; and 7 (1.6%), travellers. Median time to diagnosis was 62.5 months (females 86.6 vs males 70.1; p = 0.0075). There were 8 multidrug resistant TB cases and 1 extensively drug resistant case of TB, all in immigrants. TB was frequently diagnosed more than 5 years after arrival in Spain. Screening programmes for TB and LTBI in immigrants should be considered beyond this time point.
