3. Altered ascending aortic wall shear stress in patients with corrected atrioventricular septal defect: a comprehensive cardiovascular magnetic resonance and 4D flow MRI evaluation

AbstractAim:In patients after atrioventricular septal defect correction, altered geometry leads to a changed position and subsequent flow over the left ventricular outflow tract. We hypothesised that this altered flow may influence haemodynamics in the ascending aorta.Methods:In total, 30 patients after atrioventricular septal defect correction (age 27.6 ± 12.8 years) and 28 healthy volunteers (age 24.8 ± 13.7 years) underwent 4D flow cardiovascular magnetic resonance. Left ventricular ejection fraction and mean and peak wall shear stress calculated at ascending aortic peak systole were obtained from cardiovascular magnetic resonance. Left ventricular outflow tract data including velocity and diameter were obtained from echocardiography.Results:Patients showed a higher mean (911 ± 173 versus 703 ± 154 mPa, p = 0.001) and peak ascending aortic wall shear stress (1264 ± 302 versus 1009 ± 240 mPa, p = 0.001) compared to healthy volunteers. Increased blood flow velocities over the left ventricular outflow tract (1.49 ± 0.30 m/s versus 1.22 ± 0.20 m/s, p < 0.001) correlated well with mean and peak ascending aortic wall shear stress (r = 0.67, p < 0.001 and r = 0.77, p < 0.001).Conclusion:After atrioventricular septal defect correction, increased wall shear stress was observed, which correlated to velocities over the left ventricular outflow tract. These findings imply that altered outflow tract geometry contributes to changed aortic haemodynamics.

To evaluate the inflow pattern and flow quantification in patients with functional univentricular heart after Fontan's operation using 4D flow magnetic resonance imaging (MRI) with streamline visualization when compared with the conventional 2D flow approach. Seven patients with functional univentricular heart after Fontan's operation and twenty‐three healthy controls underwent 4D flow MRI. In two orthogonal two‐chamber planes, streamline visualization was applied, and inflow angles with peak inflow velocity (PIV) were measured. Transatrioventricular flow quantification was assessed using conventional 2D multiplanar reformation (MPR) and 4D MPR tracking the annulus and perpendicular to the streamline inflow at PIV, and they were validated with net forward aortic flow. Inflow angles at PIV in the patient group demonstrated wide variation of angles and directions when compared with the control group (P< .01). The use of 4D flow MRI with streamlines visualization in quantification of the transatrioventricular flow had smaller limits of agreement (2.2 ± 4.1 mL; 95% limit of agreement −5.9–10.3 mL) when compared with the static plane assessment from 2DFlow MRI (−2.2 ± 18.5 mL; 95% limit of agreement agreement −38.5‐34.1 mL). Stronger correlation was present in the 4D flow between the aortic and trans‐atrioventricular flow (R2correlation in 4D flow: 0.893; in 2D flow: 0.786). Streamline visualization in 4D flow MRI confirmed variable atrioventricular inflow directions in patients with functional univentricular heart with previous Fontan's procedure. 4D flow aided generation of measurement planes according to the blood flood dynamics and has proven to be more accurate than the fixed plane 2D flow measurements when calculating flow quantifications.

Magnetic resonance imaging (MRI) of the vessel wall enables determination of luminal area, vessel wall thickness, and atherosclerotic plaque characteristics. For clinical application, high spatial resolution, derived from optimal signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR), is paramount. Vessel wall MRI is expected to benefit from higher magnetic field strength. Therefore, the purposes of the present study were to develop an ultrahigh-field 7-T MRI hardware and protocols for vessel wall imaging of the carotid artery and to compare quantitative parameters of vessel wall morphology and image quality between 3-T and 7-T MRI.

Magnetic resonance imaging (MRI) can be used to distinguish bone marrow (BM) from cartilage and may therefore be used to measure BM volume in intact bones. We used MRI to measure the total human fetal BM volume in intact fetuses during the second trimester of pregnancy and determined the contribution of the individual bones to the total compartment. The total BM volume ranged from 934 μL at 17 to 18 weeks to 4563 μL at 22 to 23 weeks of gestation. The largest contributor to the total BM volume was the spine, constituting 26.4% ± 2.7% of the total volume. By analyzing leukocyte content and percentages of CD34+ cells, lymphocytes, granulocytes, and monocytes of determined volumes, absolute numbers of these cell populations in BM could be measured. The cellular composition of the BM compartment did not significantly change throughout the second trimester of gestation. Absolute white blood cell counts per fetus increased from 111 × 106 at 16 to 17 weeks to 1229 × 106 at 21 to 22 weeks. The absolute numbers of CD34+ cells increased from 25 × 106 at 16 to 17 weeks to 256 × 106 at 21 to 22 weeks. Similar analysis of liver and spleen revealed comparable absolute numbers of CD34+ cells in BM and liver throughout the second trimester of gestation. In fetal liver, CD34+ cells differentiate into red cells, myeloid cells, and platelets, while lymphopoiesis mainly occurs in BM or spleen. Combining MRI and cell counts provides a method to quantify specific cell populations in fetal compartments. This study may enable better evaluation of fetal diagnostics and therapies.

10. The Role of Hydrolysis in the Detoxification of 1,2:3,4-Diepoxybutane by Human, Rat, and Mouse Liver and Lungin Vitro

1,2:3,4-Diepoxybutane (BDE) is probably the ultimate genotoxic metabolite of the rodent carcinogen 1,3-butadiene (BD). The formation of BDE from BD has been characterizedin vitrousing tissues from rats, mice, and humans. For assessment of human risk following exposure to BD, a quantitative understanding between the balance of formation and inactivation of BDE is essential. BDE can be removed by glutathione (GSH) conjugation and by hydrolysis. Recently, significant species differences were reported in GSH conjugation of BDEin vitro,with rats being more efficient than humans and mice being much more efficient than either rats or humans (Boogaardet al., Toxicol. Appl. Pharmacol.136,307, 1996). In the present study the microsomal hydrolysis of BDE was quantified using tissues of rats, mice, and humans. Hydrolysis of BDE was well described by Michaelis–Menten kinetics. Two metabolites, erythritol and anhydroerythritol, were identified following incubation of BDE with human microsomes, but these metabolites did not fully account for the disappearance of BDE, suggesting that there may be other as yet unidentified routes of metabolism. In contrast to GSH conjugation, which was most efficient in mice compared with rats or humans, the efficiency of hydrolysis as expressed byVmax/Kmwas much lower in mouse (3.93 μl/min/mg protein) than in rat (19.2) or human (32.5) liver. Pulmonary hydrolysis was also most efficient in humans, with averageVmax/Kmvalues of 7.7, 6.7, and 2.7 μl/min/mg protein for humans, mice, and rats, respectively. However, the interindividual variation among the human samples was considerable with individualVmax/Kmvalues varying from 17.9 to 49.5 μl/min/mg protein for liver and from 4.57 to 16.2 μl/min/mg protein for lung tissue. This means that the heterogeneity among humans in the formation as well as in the removal of BDE will be an important factor in human risk assessment. The present data, coupled with earlier studies on formation and removal of BDE and the observation that GSH conjugation of BDE is a potentially mutagenic pathway, explain the high susceptibility of mice to BD-induced carcinogenesis.

11. Urinary excretion of N-acetyl-S-allyl-L-cysteine upon garlic consumption by human volunteers

- Abstract
Abstract: N-Acetyl-S-allyl-L-cysteine (allylmercapturic acid, ALMA) was previously detected in urine from humans consuming garlic. Exposure of rats to allyl halides is also known to lead to excretion of ALMA in urine. ALMA is a potential biomarker for exposure assessment of workers exposed to allyl halides. It is not known whether garlic consumption can lead to urinary concentrations of ALMA which may interfere with biological monitoring of exposure to allyl halides by determination of urinary ALMA. Therefore, this study was undertaken to determine the cumulative excretion and the excretion kinetics of ALMA in urine of humans consuming garlic. Six human volunteers were given orally two garlic tablets, each containing 100 mg garlic extract (each representing 300 mg fresh garlic). Three of the volunteers consumed additional garlic after the garlic tablet intake. Urine samples were collected up to 24 h after the intake of the garlic tablets. ALMA was identified in the urine using gas chromatography-mass spectrometry (GC-MS) and determined quantitatively with a limit of detection of 0.10 μg/ml with gas chromatography with sulphur selective detection. The total amount of ALMA found in urine of volunteers who consumed two garlic tablets was 0.43�0.14 mg (n=3). In the urine of the three volunteers who consumed not only two garlic tablets but also additional fresh garlic, a significantly higher amount of ALMA was excreted in the urine, 1.4�0.2 mg (n=3). The elimination half-life of ALMA, estimated from urinary excretion rate versus time curves, was 6.0�1.3 h (n=5). One volunteer, who ate additional garlic, showed an irregular elimination profile and was excluded from this estimation. The highest urinary concentration of ALMA found in this study was 2.2 μg/ml. In a preliminary biological monitoring study of exposure in workers with potential exposure to allyl chloride (AC) up to the occupational exposure limit of 1 ppm (8-h TWA), we recently found urinary ALMA concentrations up to 4 μg/ml. Based on the results presented here, we conclude that garlic consumption is a potential confounder when monitoring human exposure to allylhalides and other chemicals leading to ALMA excretion when ALMA is used as a biomarker of exposure.

12. Glutathione Conjugation of 1,2:3,4-Diepoxybutane in Human Liver and Rat and Mouse Liver and Lungin Vitro

1,3-Butadiene (BD) has been classified as a probable human carcinogen based on sufficient evidence of a carcinogenic response in B6C3F1mice and Sprague–Dawley rats and limited human evidence of carcinogenicity. Mice are much more susceptible to BD-induced carcinogenicity than rats. Previousin vitrostudies revealed that mouse liver microsomes formed 1,2-epoxy-3-butene (BMO) from BD and 1,2:3,4-diepoxybutane (BDE) from BMO at much higher rates than rat or human microsomes. BDE was also readily quantitated in blood and tissues of mice exposed to BD but could not be detected in rats similarly exposed. These findings suggest a key role for BDE in BD-induced carcinogenicity. The purpose of this study was to characterize the glutathione (GSH) conjugation of BDE by liver and lung cytosol from B6C3F1mice and Sprague–Dawley rats and human liver cytosol from six different individualsin vitro.BDE and glycine-[2-3H]GSH were incubated, at pH 7.4, with cytosol.13C NMR and mass spectral analysis indicated formation of two isomeric conjugates,S-(1-(hydroxymethyl)-2,3-epoxypropyl)glutathione andS-(2-hydroxy-3,4-epoxybutyl)glutathione, which were rapidly hydrolyzed in cytosol to the corresponding trihydroxy conjugates. Total conjugates were quantitated by HPLC. Conjugation of BDE with GSH followed Michaelis–Menten kinetics in human as well as rat and mouse cytosolic fractions. The conjugation rates in mouse and rat liver cytosol were similar (Vmax162 ± 16 and 186 ± 37 nmol/mg protein/min, respectively) and an order of magnitude higher than in human liver cytosol (Vmax6.4 ± 1.9 nmol/mg protein/min). The apparentKMvalues were lower in human (2.1 ± 1.4 mM) than mouse (6.4 ± 1.6 mM) or rat (24 ± 6 mM) liver. Mouse lung cytosol (Vmax38.5 ± 2.5 nmol/mg protein/min,KM1.70 ± 0.37 mM) is also more efficient in GSH conjugation than rat lung cytosol (Vmax17.1 ± 3.0 nmol/mg protein/min,KM4.2 ± 1.7 mM). These results suggest that the higher BDE blood concentrations in mice compared with rats following inhalation exposure to BD are not due to differences in hepatic or pulmonary GSH conjugation of BDE. Also, considering the low oxidation rates of BD to BMO and of BMO to BDE in humans as compared to mice, the relatively low capacity of GSH conjugation of BDE in human liver will not necessarily lead to increased BDE blood levels in humans potentially exposed to BD.

13. 026 Dimensional flow cardiovascular magnetic resonance: two-centre, 1.5t, phantom and in-vivo validation study

BackgroundValidation of four-dimensional (4D) flow CMR accelerated acquisition methods is needed to make them more robust for clinical applications.1Our aim was to compare three widely-used acceleration methods in 4D flow CMR: 4D segmented fast-gradient-echo (4D- turbo-field-echo, 4D-TFE), 4D non-segmented gradient-echo with echo-planar imaging (4D- EPI) and 4D-k-tBroad-use Linear Acquisition Speed-up Technique accelerated TFE (4D-k-tBLAST).MethodsCMR was performed in two institutions on identical 1.5T systems. Acceleration methods were compared in static/pulsatile phantoms (Figure 1) and 25 volunteers. In volunteers, the CMR protocol included: cines, 2D phase contrast (PC) at the aortic valve (AV) and mitral valve (MV) and three whole-heart free-breathing (no respiratory motion correction) 4D flow CMR pulse sequences. Field-of-view, slices, phases (30), voxel size and VENC were the same for each subject. In volunteers, net acquisition time for each 4D flow sequence was recorded, as well as a visual grading of image quality on a four-point scale: 0, no artefacts to 3, non-evaluable.[Figure]ResultsFor the pulsatile phantom experiments, the mean error against the reference flow by time beaker measurements for 4D-TFE was 4.9%±1.3%, for 4D-EPI 7.6%±1.3% and for 4D-k-tBLAST 4.4%±1.9%. In vivo, acquisition time was shortest for 4D-EPI at 7 min59s±2 min30s. 4D- EPI and 4D-k-tBLAST had minimal artefacts, while for 4D-TFE, 40% of AV and MV assessments were non-evaluable because of phase dispersion artefacts. Peak velocity assessment using 4D-EPI demonstrated best correlation to 2D PC (AV: r=0.78, p<0.001; MV: r=0.71, p<0.001). - Abstract
Objectives: Myocardial fibrosis in noninfarcted myocardium is emerging as a principal phenotype of vulnerability to adverse events such as mortality and hospitalization for heart failure (HHF), but its optimal noninvasive measurement remains uncertain despite consistently robust histologic validation data for extracellular volume fraction (ECV). We therefore compared ECV, native T1, post contrast T1, the gadolinium contrast partition coefficient (lambda), and the presence of nonischemic scar in their associations with mortality and HHF outcomes. Method: To quantify of myocardial fibrosis, we performed T1 mapping (MOLLI) in basal and mid short axis slices with cardiovascular magnetic resonance (CMR) before contrast and 12-30 minutes post contrast bolus in 1185 consecutive patients without amyloidosis, hypertrophic or stress cardiomyopathy. We assessed associations with outcomes using Kaplan-Meier plots and chi square values from univariable Cox regression models. All standard T1 mapping parameters were obtained: native and post contrast myocardial T1, the partition coefficient lambda, and ECV. ECV = (1-hematocrit) · [ΔR1myocardium]/[ΔR1bloodpool], where R1 = 1/T1 Late gadolinium enhancement imaging with phase sensitive reconstruction identified nonischemic scar. Results: Over a median of 1.7 years, 111 individuals experienced events after CMR: 55 HHF events and 74 deaths. ECV yielded better separation of Kaplan-Meier curves in a dose dependent fashion (Figure) and also stronger associations with the combined endpoint of death or HHF. The ECV chi square (77.3, p < 0.001) was at least twice as large as the Native T1 chi square (37.5, p < 0.001), the lambda chi square (34.8, p < 0.001) and nonischemic scar (chi square = 20.5, p<0.001). Post-contrast T1 was not associated with outcomes, even when adjusting further for time after contrast bolus, renal function, and patient weight (chi square <3, p >0.10). Conclusion: Analogous to histologic previously published validation data, quantitative ECV myocardial fibrosis measures associated with outcomes far stronger than other surrogate measures outcome measures such as native T1, post contrast T1 and nonischemic scar on LGE images.
