1. Evaluation of dental demands and needs of people experiencing homelessness in the city of Rome, Italy.

Abstract: OBJECTIVE: Poor oral health and oral diseases are common among people experiencing homelessness. The aim of this study was to evaluate the dental demands and needs of a population of homeless persons in the city of Rome, Italy. PATIENTS AND METHODS: The clinical records of 165 homeless patients admitted between October 2020 and October 2021 to the dental service of the Primary Care Services of the Eleemosynaria Apostolica, Vatican City, were retrospectively reviewed. The service employed dentists to evaluate dental needs and oral conditions in patients experiencing homelessness. The main dental and oral pathological conditions were noted. RESULTS: One hundred and sixty-five records of homeless patients were included in the study. The sample consisted in 138 males (76.97%) and 27 females (23.03%) with a mean age of 46.9 years (range 7-85 years). Acute tooth pain was reported by 132 (80%) patients, 42 (25.45%) had edentulism or missing teeth and 18 (10.91%) patients had oral lesions. Both dental and oral pathologies were intercepted and managed in secondary healthcare facilities. CONCLUSIONS: Given the specific peculiarities of this vulnerable population, it is important to implement strategies that facilitate the access of persons experiencing homelessness to dental evaluation with a preventive and curative perspective. 2. Exome sequencing identifies rare damaging variants in the ATB8B4 and ABCA1 genes as novel risk factors for Alzheimer's disease.

Abstract: Background: Damaging rare variants in the TREM2, SORL1 and ABCA7 genes have been associated with an increased risk of developing Alzheimer's Disease (AD) with odds ratios that were not observed since the identification of the main AD genetic risk factor, the APOE‐ε4 allele. Here, we aimed to identify additional AD‐associated genes by investigating the burden of rare damaging variants in the exomes of AD cases and controls. Method: On a single server, we analyzed in two stages, the data from 52,270 exome sequences from several independent datasets from Europe and the United States. After comprehensive QC, Stage‐1 and Stage‐2 datasets comprised in total 16,396 AD cases (5,672 EOAD) and 18,107 controls with European ancestry. All detected non‐synonymous and loss‐of‐function rare variants were prioritized by REVEL and LOFTEE, and analyzed in a per‐gene burden analysis. After a Stage‐1 discovery analysis, we replicated findings in an independent dataset (Stage‐2). We combined the Stage‐1 and Stage‐2 datasets and determined, for each gene, the features of the variants that drive the burden‐associations. Results: We confirmed the AD‐association of rare damaging variants SORL1, TREM2, ABCA7, and newly identified a significant AD‐association of rare damaging variants in the ATP8B4 and ABCA1 genes. In addition, we find a strong indication for the AD‐association of ADAM10 and SRC genes (Stage‐2 p<0.05). For most genes, we observed a larger effect size for LOF variants compared to missense variants (Figure‐A). High‐impact variants in these genes are mostly extremely rare and enriched in AD patients with early ages at onset (Figure‐B). Conclusion: We identified, for the first time, the AD‐association of rare damaging variants in two genes: (i) microglial ATP8B4 which is involved in phospholipid transport, and (ii) ABCA1 which plays a critical role in lipidation of apoE thereby supporting Aβ processing. Further, we found strong evidence for the AD‐association of damaging variants in ADAM10 and SRC genes. ADAM10 is involved in the proteolytic processing of APP, while SRC is a Non‐Receptor Tyrosine Kinase which binds PTK2B/Pyk2, a known AD risk factor. Abstract: Characterization of the genetic landscape of Alzheimer’s disease (AD) and related dementias (ADD) provides a unique opportunity for a better understanding of the associated pathophysiological processes. We performed a two-stage genome-wide association study totaling 111,326 clinically diagnosed/‘proxy’ AD cases and 677,663 controls. We found 75 risk loci, of which 42 were new at the time of analysis. Pathway enrichment analyses confirmed the involvement of amyloid/tau pathways and highlighted microglia implication. Gene prioritization in the new loci identified 31 genes that were suggestive of new genetically associated processes, including the tumor necrosis factor alpha pathway through the linear ubiquitin chain assembly complex. We also built a new genetic risk score associated with the risk of future AD/dementia or progression from mild cognitive impairment to AD/dementia. The improvement in prediction led to a 1.6- to 1.9-fold increase in AD risk from the lowest to the highest decile, in addition to effects of age and the APOEε4 allele.

Abstract: Alzheimer’s disease (AD), the leading cause of dementia, has an estimated heritability of approximately 70%1. The genetic component of AD has been mainly assessed using genome-wide association studies, which do not capture the risk contributed by rare variants2. Here, we compared the gene-based burden of rare damaging variants in exome sequencing data from 32,558 individuals—16,036 AD cases and 16,522 controls. Next to variants in TREM2, SORL1and ABCA7, we observed a significant association of rare, predicted damaging variants in ATP8B4and ABCA1with AD risk, and a suggestive signal in ADAM10. Additionally, the rare-variant burden in RIN3, CLU, ZCWPW1and ACEhighlighted these genes as potential drivers of respective AD-genome-wide association study loci. Variants associated with the strongest effect on AD risk, in particular loss-of-function variants, are enriched in early-onset AD cases. Our results provide additional evidence for a major role for amyloid-β precursor protein processing, amyloid-β aggregation, lipid metabolism and microglial function in AD.

6. Flexural strength and elastic modulus evaluation of structures made by conventional PMMA and PMMA reinforced with graphene.

Abstract: OBJECTIVE: The aim of this study was to compare both the elastic modulus (EM) and the flexural strength (FS) of two materials used in dental prosthesis, namely polymethylmethacrylate (PMMA) and polymethylmethacrylate reinforced with graphene (G-PMMA). MATERIALS AND METHODS: Twenty rectangular samples were manufactured by a milling machine and divided into two groups (n= 10/group): Group 1, PMMA; Group 2, G-PMMA. The specimens were subjected to a three-point bending test conducted in the elastic range to evaluate EM. A similar test was protracted until fracture to evaluate FS. Data on EM and FS were statistically analyzed with independent-samples t-test in order to compare the two groups. A scanning electron microscope (SEM) (5.00 kx and 1.00 kx magnification) was used to evaluate the morphology of sample's fracture. RESULTS: Compared to PMMA samples, each G-PMMA sample showed significantly higher values of FS (p <0.001) and EM (p <0.001). SEM images analysis showed an inhomogeneous fracture morphology in G-PMMA samples. CONCLUSIONS: The results show that G-PMMA is a promising material to be used for prosthetic purposes. This is demonstrated by a significant increase in both peak load and bending stiffness, resulting from the bending test performed on G-PMMA samples. Furthermore, the latter exhibit greater homogeneity in their mechanical behavior, supporting the potential value of this material in dental prosthesis. [ABSTRACT FROM AUTHOR]

Abstract: Measuring the spin structure of protons and neutrons tests our understanding of how they arise from quarks and gluons, the fundamental building blocks of nuclear matter. At long distances, the coupling constant of the strong interaction becomes large, requiring non-perturbative methods to calculate quantum chromodynamics processes, such as lattice gauge theory or effective field theories. Here we report proton spin structure measurements from scattering a polarized electron beam off polarized protons. The spin-dependent cross-sections were measured at large distances, corresponding to the region of low momentum transfer squared between 0.012 and 1.0 GeV2. This kinematic range provides unique tests of chiral effective field theory predictions. Our results show that a complete description of the nucleon spin remains elusive, and call for further theoretical works, for example, in lattice quantum chromodynamics. Finally, our data extrapolated to the photon point agree with the Gerasimov–Drell–Hearn sum rule, a fundamental prediction of quantum field theory that relates the anomalous magnetic moment of the proton to its integrated spin-dependent cross-sections.

Abstract: Sialic acid-binding immunoglobulin-type lectins, which are predominantly expressed in immune cells, represent a family of immunomodulatory receptors with inhibitory and activating signals, in both healthy and disease states. Genetic factors are important in all forms of dementia, especially in early onset dementia. CD33 was recently recognized as a genetic risk factor for Alzheimer disease (AD). Here, we present a 2-generation family with 4 members, the father and the 3 siblings, characterized by an early form of unusual dementia exhibiting a behavioral variant close to behavioral variant frontotemporal dementia phenotype and severe forms of memory loss suggestive of AD. We analyzed the CD33 gene in this family and identified 10 single nucleotide polymorphisms (SNPs) in a linkage disequilibrium block associated with the disease. We also identified a tag SNP, rs2455069-A>G, in CD33 exon 2 that could be involved with dementia risk. Additionally, we excluded the presence of C9orf72 expansion mutations and other mutations previously associated with sporadic FTD and AD. The tag SNP association was also analyzed in selected sporadic AD patients from the same Southern Italy region. We demonstrate that CD33 and SIGLECL1 have a significantly increased level of expression in these patients.

9. Neurovascular Dysfunction in Alzheimer Disease: Assessment of Cerebral Vasoreactivity by Ultrasound Techniques and Evaluation of Circulating Progenitor Cells and Inflammatory Markers.

Abstract: Aims: The aims of this study were to assess vascular dysfunction in patients with Alzheimer disease (AD) by investigating cerebral vasomotor reactivity using transcranial Doppler ultrasound (TCD) and to evaluate any correlations between cerebral vasoreactivity and endothelium dysfunction. Moreover, the frequency of circulating progenitor cells (CPCs) and the blood concentration of vascular/inflammatory markers were evaluated.Materials and Methods: We recruited 35 AD subjects and 17 age-matched, sex-matched, and education-matched healthy control subjects. Cerebral vasomotor reactivity was assessed by means of the TCD-based breath-holding index test (BHI). The level of CPCs was evaluated by means of flow cytometry from venous blood samples, while blood vascular/inflammatory markers were measured by means of enzyme-linked immunosorbent assay.Results: Both cerebral assay blood flow velocity in the middle cerebral artery (MCAFV) and BHI values were significantly lower in AD subjects than in healthy controls (P<0.05). A positive trend was found between MCAFV and BHI values and Mini-Mental State Evaluation (MMSE) scores. Moreover, the hematopoietic progenitor cells' count was found to be lower in patients with AD than in controls (P<0.05). Finally, a significantly higher expression of the plasma chemokine CCL-2 was observed in AD patients than in healthy controls.Conclusions: Our results confirm that cerebral hemodynamic deterioration may be a critical marker of cognitive decline. This article, after a short introduction on the ossification processes involved, describes the main modifications of the craniofacial complex induced by acromegaly on both hard and soft tissues. Abstract: Risk for late-onset Alzheimer’s disease (LOAD), the most prevalent dementia, is partially driven by genetics. To identify LOAD risk loci, we performed a large genome-wide association meta-analysis of clinically diagnosed LOAD (94,437 individuals). We confirm 20 previous LOAD risk loci and identify five new genome-wide loci (IQCK, ACE, ADAM10, ADAMTS1,and WWOX), two of which (ADAM10, ACE) were identified in a recent genome-wide association (GWAS)-by-familial-proxy of Alzheimer’s or dementia. Fine-mapping of the human leukocyte antigen (HLA) region confirms the neurological and immune-mediated disease haplotype HLA-DR15 as a risk factor for LOAD. Pathway analysis implicates immunity, lipid metabolism, tau binding proteins, and amyloid precursor protein (APP) metabolism, showing that genetic variants affecting APP and Aβ processing are associated not only with early-onset autosomal dominant Alzheimer’s disease but also with LOAD. Analyses of risk genes and pathways show enrichment for rare variants (P= 1.32 × 10−7), indicating that additional rare variants remain to be identified. We also identify important genetic correlations between LOAD and traits such as family history of dementia and education.

Abstract: Objectives. To define the possible complications of oral surgery in childhood in patients affected by type 1 Osteogenesis imperfecta (OI) and treated with bisphosphonates (BP). Methods. The study was conducted among 20 patients in childhood with an age range 8-14 (12 ♂ e 8 ♀) affected by OI. Patients were initially evaluated at the Policlinico Umberto I, University Hospital of Rome, Rare Disease Center Skeletal Dysplasia-Bone Metabolic Pathologies and after at the Policlinico Umberto I, University Hospital of Rome, Head and Neck Department, UOC Pediatric Dentistry. Results. From this experience, we showed that a proper patient management from the medical and dental point of view can protect these patients from the risk of post-operative problems, such as onj, soft tissue flogos, intraoral and extraoral fistulas, failure to heal the post-extractive alveolus, infections, post-operative pain and pathological fractures. The follow- up, ranging from a minimum of 2 years to a maximum of 5 years, have not demonstrated the presence of particular complications or healing defects. Conclusions. The clinical experiences observed in these patients are encouraging because no postoperative complications have been observed compared to patients non-affected by OI. [ABSTRACT FROM AUTHOR]

Abstract: We identified rare coding variants associated with Alzheimer's disease in a three-stage case–control study of 85,133 subjects. In stage 1, we genotyped 34,174 samples using a whole-exome microarray. In stage 2, we tested associated variants (P < 1 × 10−4) in 35,962 independent samples using de novo genotyping and imputed genotypes. In stage 3, we used an additional 14,997 samples to test the most significant stage 2 associations (P < 5 × 10−8) using imputed genotypes. We observed three new genome-wide significant nonsynonymous variants associated with Alzheimer's disease: a protective variant in PLCG2 (rs72824905: p.Pro522Arg, P = 5.38 × 10−10, odds ratio (OR) = 0.68, minor allele frequency (MAF)cases= 0.0059, MAFcontrols= 0.0093), a risk variant in ABI3 (rs616338: p.Ser209Phe, P = 4.56 × 10−10, OR = 1.43, MAFcases= 0.011, MAFcontrols= 0.008), and a new genome-wide significant variant in TREM2 (rs143332484: p.Arg62His, P = 1.55 × 10−14, OR = 1.67, MAFcases= 0.0143, MAFcontrols= 0.0089), a known susceptibility gene for Alzheimer's disease. These protein-altering changes are in genes highly expressed in microglia and highlight an immune-related protein–protein interaction network enriched for previously identified risk genes in Alzheimer's disease. These genetic findings provide additional evidence that the microglia-mediated innate immune response contributes directly to the development of Alzheimer's disease.

16. Listeria monocytogenesin Smoked Salmon and Other Smoked Fish at Retail in Italy: Frequency of Contamination and Strain Characterization in Products from Different Manufacturers

Abstract: Seven hundred seventy-eight samples of packaged smoked fish (774 smoked salmon and 4 smoked swordfish) on sale in Italy, from 50 different manufacturers located in 12 European Union countries, were purchased from the Italian market between May and December 2011. The surface temperatures of the samples on sale ranged from 0 to 13°C (3.4 ± 1.5°C, mean ± SD). Six hundred eighty (87.4%) of 778 samples were stored at ≤4°C. One hundred fifty-seven samples (20.2%, 95% confidence interval 17.5 to 23.1%) were contaminated by Listeria monocytogenes, with 26 samples (3.3%, 95% confidence interval 2.3 to 4.8%) at levels >100 CFU/g. The maximum level of contamination was 1.3 ×106CFU/g. The differences in the level of contamination of smoked fish between countries (χ2= 91.54, P< 0.05) and manufacturers (χ2= 193.22, P< 0.05) were significant. The frequency of detection for products from different manufacturing premises ranged from 0 to 76.9%. Serotyping by serological agglutination revealed that the main serotypes detected were 1/2a (65.3%) and 1/2b (22.4%). Pulsed-field gel electrophoresis typing with restriction enzymes AscI and ApaI yielded 36 pulsotypes from 144 isolates, clustering into 17 groups. Eight main pulsotypes accounted for 70.8% of the isolates. Three of the main pulsotypes were exclusively from products of a single manufacturer. In general, products from the same manufacturer showed genetic homogeneity, with one strongly prevalent pulsotype. Different manufacturers usually showed very different levels of contamination of the final product, confirming the importance of the management of process hygiene for controlling L. monocytogenescontamination.

Abstract: Abstract: Introduction: The pathogenesis of temporomandibular joint intracapsular disease (TMJI) is multifactorial and its diagnosis is not easy. In this work authors show two types of clinical analysis: the Condylar Position Indicator (CPI) and T-Scan 2 system. Material and methods: Twenty patients (mean age of 24.5years) with TMJI problem and 10 healthy matched subjects (mean age: 25.4years) were selected. Analysis of TMJI was performed on each patient by means of Condylar Position Indicator (CPI) and T-Scan System II tests. Results: Eight patients presented vertical symmetrical condylar distraction greater than healthy subjects (P-value<0.001). T-Scan showed a difference of Percentage of Force (POF) not greater than 5%. Seven patients showed sagittal shift greater than healthy subject (P-value<0.001). T-Scan records showed a difference of POF greater than 5%. Five non-healthy subjects presented sagittal, vertical, transverse shift greater than healthy subjects (P-value<0.001). T-Scan records show a difference of POF greater than 5%. Conclusion: In this work authors present a new method of analysis. CPI indicates discrepancy of the condyle position in CO from CR and T-Scan allows the operator to study all teeth contacts and occlusal forces taking place during dynamic jaw movement. Abstract: Purpose: To evaluate the fluoride concentration in enamel after the use of different fluoride products and 48 hours after the cessation of fluoride use. Methods: 16 enamel slabs were divided and treated for 15 days as following: Group A, with an amine fluoride toothpaste and mouthrinse (1400 and 250 mg/L-1, respectively); Group B, with an amine fluoride toothpaste (1400 mg/L-1) and mouthrinse without fluoride; Group C, with a sodium fluoride toothpaste (1400 mg/L-1) and mouthrinse (250 mg/L -1 Chemical biopsy and multiple means were used to determine fluoride concentration. The data were statistically analyzed using the two-way ANOVA and Tukey HSD tests (P< 0.05). Results: After 15 days of fluoride product use, Group A samples had the highest fluoride uptake (0.19 μg mm2) (P< 0.05). 48 hours after the interruption of fluoride product usage, a higher fluoride concentration was found in Group A samples. 21. Remifentanil for cesarean section under general anesthesia: effects on maternal stress hormone secretion and neonatal well-being: a randomized trial.

Abstract: Abstract: Background: Remifentanil may attenuate maternal hemodynamic response during cesarean section under general anesthesia, but could cause transient but significant neonatal depression. We investigated the effect of low-dose remifentanil on maternal neuroendocrine response and fetal wellbeing. Methods: Forty-two ASA I-II parturients undergoing cesarean section at term under general anesthesia were randomized to receive either fentanyl after delivery (n=21, group C) or remifentanil bolus 0.5 μg/kg before induction followed by a continuous infusion at 0.15 μg·kg−1min−1 until peritoneal incision, then restarted after delivery (n=21, group R). Maternal heart rate and blood pressure, and epinephrine, norepinephrine, adrenocorticotropic hormone (ACTH), and growth hormone levels were measured at baseline, uterine incision, and the end of surgery. Remifentanil was measured in maternal and umbilical arterial and venous blood. One- and 5-minute Apgar scores and umbilical arterial and venous pH were recorded. Results: ACTH was significantly higher in group C at uterine incision (P <0.01). No significant differences were observed in hemodynamics, catecholamines or growth hormone. Apgar scores at 1 (P <0.05) and 5 min (P <0.01) were significantly higher in group C. Mean umbilical pH values were within normal range but significantly higher in group C. Three neonates in group R required intubation but recovered at 5 min without naloxone. Mean±SD maternal remifentanil concentration was 1.67±1.04 ng/mL. Conclusions: Remifentanil administration before peritoneal incision partially reduced the hormonal stress response. Maternal benefits must be weighed against transitory but significant neonatal respiratory depression. Neonatal resuscitation facilities are mandatory when remifentanil is used. Abstract: The article examines the effect of glutathione S-transferase (GST) P1-M1-T1 gene polymorphisms and their interactions with the apolipoprotein E e4 allelic variant during a three-year longitudinal course of 99 late-onset Alzheimer's disease (LOAD) patients. The study showed that GSTP1 was associated with a faster decline in cognitive functioning and basic activities of daily living. GSTs may show that oxidative stress plays a major role in Alzheimer's disease neurodegeneration.

Material and Methods: Dental caries was recorded in 2,040 schoolchildren (42.5% boys, 57.5% girls) using decayed/missed/filled surface index (d level) in primary dentition. Parents filled in a standardised questionnaire regarding nationality, level of education, frequency of dental cheek-up and perception of child's oral health and child's oral hygiene habits. Results: At the individual-level of SES, mothers' educational level was associated with their children's caries severity (y=\A1.51 p<0.01): as educational level rose the proportion of children with high numbers of carious lesions fell. The two income indicators (area-level SES) were not associated. A multinomial logistic regression model was run for caries risk factors. Caries severity was used as dependent variable and the model was stratified by mothers' educational level. Mothers' perception of child's oral health was the only covariate that was always associated in every caries severity strata and for each level of mothers' education. Conclusions: The present study shows that mothers' educational level is a useful individual SES indicator for caries in Italian children living in a low-income population. [ABSTRACT FROM AUTHOR]

25. Presence of Anisakid Larvae in the European Anchovy, Engraulis encrasicolus, Fished Off the Tyrrhenian Coast of Central Italy

Abstract: A survey was carried out to determine the prevalence of anisakid nematode larvae in European anchovy (Engraulis encrasicolus) fished off the Tyrrhenian coast of central Italy. From February through July 2012, 1,490 specimens of E. encrasicoluscaught in three different fishing areas (off Civitavecchia, Anzio, and Gaeta in the northern, central, and southern Lazio region of Italy, respectively) were tested for the presence of anisakid larvae, both by visual microscopic inspection and enzymatic digestion. In each of the three fishing areas, each of two sampling times produced 250 fish (with the exception of one sampling time in Gaeta that produced 240 fish). Larvae of the family Anisakidae were detected with an overall estimated prevalence of 2.3%, and each positive fish harbored a single larva. No anisakid larvae were detected in fish caught off Gaeta. Fish with larvae were significantly longer (standard length) than fish without larvae. Twenty-six larvae (74.3%) were detected by visual inspection of the viscera, eight larvae (22.8%) were detected by visual inspection of the fillets, and one larva (2.8%) was detected after digestion of pooled fillets. Molecular analysis to fully characterize the 35 detected larvae revealed 15 specimens of Anisakis pegreffii, 10 specimens of Hysterothylacium aduncum, and one hybrid genotype of A. pegreffii× Anisakis simplex. For nine specimens, no visible product was obtained after PCR amplification. The overall prevalence for A. pegreffiiand H. aduncumwas 1.0 and 0.7%, respectively. A comparison between fishes harboring A. pegreffiilarvae and those harboring H. aduncumrevealed that those with A. pegreffiiwere significantly heavier. The prevalence of anisakid larvae found in the present study is lower then that reported previously in E. encrasicoluscollected in the Mediterranean Sea.

26. Effects of Mechanical versus Manual Non Surgical Periodontal Therapy on Patient Comfort and Periodontal Healing: A Randomized Controlled Clinical Trial

Abstract: Few studies have focused on the problem of pain and discomfort experienced during and after periodontal debridment. The aim of this study was to evaluate the effect of manual versus hand driven non surgical periodontal instrumentation on the patient's comfort, perception of pain and dental hypersensitivity during and after the instrumentation. Moreover, the influence of the treatment modality on the healing of slight to moderate periodontitis was assessed with careful attention to indices of periodontal inflammation. 22 subjects with a minimum of 4=6mm periodontal pockets with 3 to 4mm attachment loss in different quadrants, the presence of =20 teeth with a minimum of four molars were enrolled. PD (probing depth, 6 sites per tooth), number of sites with PD> 6mm, buccal and lingual recessions were collected. Two quadrants Mouth Bleeding and Plaque Scores (T.M.P.S, T.M.B.S.) were assessed as the presence or absence of bleeding on probing and plaque following disclosing in quadrants 1–4 and 2–3. Non surgical periodontal treatment was delivered in two appointments performed within one week. In the first appointment the first and the fourth quadrants (patient's right side) were completely treated by mechanical or manual devices according to the randomization codes. In the second appointment the remaining two quadrants (patient's left side) were instrumented with the other therapeutical approach. The duration of each session, need for local anesthesia and additional information were recorded during the instrumentation appointments. All the patients were requested to fill in a form regarding pain, hypersensitivity, and need for painkillers following the two debridment appointments. Two drop outs were observed. Mean pain scores after treatment were higher in manual than in machine driven side (3.11±1.40 vs 2.33±1.41), whereas mean dentine hypersensitivity scores were slightly higher in mechanical side (4.40±1.56 vs 3.77±1.11). The need for painkillers after both treatment approaches was minimal. 6 out of 20 patients asked for local anesthesia and clustering regarding the request for both treated sides was observed. The mean instrumentation time was significantly lower for mechanical versus manual instrumentation (84.57±12.92 vs 119.25±13.50, p<0.001). Periodontal healing was similar in both the hand and machine driven instrumented sides. TMPS and TMBS were significantly lower at baseline compared to re-evaluation visits and the within group changes were significant lower at re-evaluation. Most of patients well tolerated non surgical periodontal treatment despite the type of instruments that are chosen by the clinician. Pain is infrequently reported and is more common after manual instrumentation compared to machine driven one. The need for local anesthesia is quite uncommon and is surely subject-dependent. Temporary, slight dentine hypersensitivity is a common adverse effect reported by most of patients after subgingival debridment. Machine driven debridment shows a dramatic advantage compared to manual treatment due to the lower amount of time needed to complete the instrumentation.

Conclusioni. I pazienti che presentano difetti dello smalto sistematici devono essere sottoposti a uno screening per la malattia celiaca anche in assenza di sintomi gastrointestinali.

Abstract: OBJECTIVE To develop a blood-based test for screening populations at risk for Alzheimer disease. DESIGN Case-control study. SUBJECTS A total of 180 patients with mild cognitive impairment (MCI) and 105 age-matched, cognitively normal controls. INTERVENTIONS The titer of β-amyloid 1-42 autoantibodies in the plasma was obtained at the time of diagnosis and evaluated by enzyme-linked immunosorbent assay before and after dissociation of the antigen-antibody complexes. A total of 107 patients with MCI were followed up for 36 months; 70 of the 107 cases progressed to Alzheimer disease. RESULTS The average level of β-amyloid 1-42 plasma autoantibodies in patients with MCI that progressed to Alzheimer disease, but not that of the stable cases, was significantly higher than in cognitively normal controls (P < .001). CONCLUSIONS The results suggest that the plasma β-amyloid 1-42 autoantibodies parallel β-amyloid 42 deposition in the brain, which is known to precede by several years the clinical onset of Alzheimer disease. The evaluation of β-amyloid 1-42 autoantibodies after dissociation of the complexes is a simple and inexpensive method that can be used to predict the occurrence of Alzheimer disease.Arch Neurol. 2010;67(7):867-872 --

Abstract: Interleukin (IL)‐2 plays an important role in the control of the immune responses, and it is released in a variety of tissues in response to inflammatory stimuli. As monocytes and mature dendritic cells (DCs) express CD25, the high‐affinity subunit of IL‐2 receptor, we examined the effect of exogenous IL‐2 on the in vitro generation and maturation of DCs from monocytes. Human monocyte‐derived DCs (MDDCs) were generated by culturing monocytes with granulocyte macrophage‐colony stimulating factor (GM‐CSF) and IL‐4 in the presence or absence of IL‐2. The cytokine was added at the beginning and after 5 days of culture. Our findings indicate that IL‐2 does induce monocytes to differentiate into DCs with the same morphology and phenotype of that obtained in the presence of GM‐CSF and IL‐4 alone, but with some distinctive functional properties. DCs differentiated in the presence of IL‐2 secreted significantly more IL‐1β, TNF‐α, and IL‐12 p70 in response to lipopolysaccharide stimulation and induced allogeneic, naïve T cells to release a significantly higher amount of interferon‐γ if compared with DCs obtained by culturing monocytes with GM‐CSF and IL‐4. These results indicate unrecognized effects of IL‐2 on human MDDCs and suggest that an IL‐2‐rich environment during differentiation and maturation of DCs can modify their T helper cell‐inducing properties.

30. Alzheimer's disease and immune activation: A translational perspective

Abstract: Alzheimer's Disease (AD) is a neurodegenerative disorder characterised by abnormal fibrillar deposits of amyloid beta peptides (Aβ), which are suggested to play a pivotal role in driving disease pathogenesis. Although the aetiology of AD is still unknown, a perturbation of the immune system has been widely demonstrated in both animal experimental models and patients. The most studied aspect of immune reaction in AD is inflammation, which has been described to be implied in its pathogenic processes. Moreover, a growing body of evidence, indicating that the triggering of the specific Aβ immune response can be effective in reducing AD sympotoms, suggest that a dysregulated network of innate and adaptive immune mechanismsm could be critical in AD pathogenic process. The present short review, aimed to summarise some of the principal research studies on immune activation in AD, has the intent to provide suggestions useful to formulate new immune‐therapeutic strategies for pathology treatment.

31. Chiral investigation of midodrine, a long‐acting α‐adrenergic stimulating agent

Abstract: Midodrine hydrochloride is a peripheral α1‐adrenoreceptor agonist that induces venous and arterial vasoconstriction. Midodrine, after oral or intravenous administration, undergoes enzymatic hydrolysis and releases deglymidodrine, a pharmacologically active metabolite. Midodrine and deglymidodrine have a chiral carbon in the 2‐position. To investigate the bioactivity of racemates and enantiomers of the drug and metabolite, three chromatographic chiral stationary phases, Chiralcel OD‐H, Chiralcel OD‐R, and α1‐AGP, were evaluated for enantiomeric resolution. Good enantioseparation of midodrine racemate was obtained using the Chiralcel OD‐H column. This stationary phase was then used to collect separately the midodrine enantiomers. By alkaline hydrolysis of rac‐midodrine and each separated enantiomer, rac‐deglymidodrine and its enantiomers were prepared. The control of the enantiomeric purity was carried out by α1‐AGP stationary phase, while the hydrolysis of rac‐midodrine and its enantiomers was controlled by capillary electrophoresis using trimethyl‐β‐cyclodextrin as chiral selector. The pharmacological activity of the two racemates and the two enantiomeric pairs was tested in vitro on a strip of rabbit descending thoracic aorta. 32. Cycle-Dependent Expression of Interleukin-1 Receptor Type II in the Human Endometrium1

Abstract: Cytokines such as interleukin-1 (IL-1) play a major role in the reparative and inflammatory-like processes that occur in human endometrium during every menstrual cycle, but they also seem to be implicated in critical reproductive events such as ovulation and implantation. Interleukin-1 is tightly regulated in the body by a complex network of control systems. In the present study, we examined the expression of IL-1RII, a natural specific inhibitor of IL-1, in the human endometrium and found an interesting distribution and temporal pattern of expression throughout the menstrual cycle. Immunoreactive IL-1RII was found in stromal as well as epithelial cells, but it was predominant within the lumen of the glands and the apical side of surface epithelium. In situ hybridization and reverse transcription-polymerase chain reaction (RT-PCR) analyses showed higher levels of mRNA in epithelial than in stromal cells. The IL-1RII cellular and luminal secretion followed a regulated cycle phase-dependent pattern of expression. Although elevated in the late proliferative/early secretory phase of the menstrual cycle, IL-1RII luminal secretion significantly decreased in the midsecretory phase, reaching its lowest levels at Day 21, before augmenting markedly again during the late secretory phase. This pattern of expression was less obvious at the level of cellular staining, as examined by immunohistochemistry, but it was corroborated by Western blot analysis of IL-1RII protein and semiquantitative RT-PCR of IL-1RII mRNA in the whole endometrial tissue and separated glandular epithelial cells. The reduced expression of IL-1RII within the implantation window suggests the existence of accurate regulatory mechanisms that, by down-regulating IL-1RII expression, alleviate IL-1 inhibition during this crucial period and facilitate IL-1 proimplantation actions. The elevated expression of IL-1RII observed during the late secretory phase suggests an involvement of IL-1RII in control of the proinflammatory state that takes place in the endometrium during the premenstrual and menstrual periods.

34. Determination of fenticonazole and its impurities by capillary electrophoresis and high performance liquid chromatography

Abstract: Fenticonazole, a topical antifungal agent containing a stereogenic centre, is used in therapy as a racemic mixture. Five related compounds can be found as impurities in the drug. We propose a HPLC method using as stationary phase a RP‐8 column eluted with different gradients of acetonitrile/phosphate buffer (pH 6) for simultaneous determination of the drug and these impurities. We also studied a high performance capillary electrophoresis (HPCE) method for quality control of fenticonazole. The separation of fenticonazole from all its impurities by HPCE was obtained in a relative short capillary (40 cm, effective length 34 cm, 50 μm ID) with a running buffer of 30 mM phosphate (pH 3) containing 8 mM trimethyl‐β‐cyclodextrin. Under these experimental conditions very good separation of fenticonazole from each individual impurity is obtained in less than 20 minutes. The choice of cyclodextrin added to the running buffer was dictated by the chiral nature of fenticonazole; trimethyl‐β‐cyclodextrin is mainly used as chiral selector. The optimised HPLC and HPCE methods are compared.

36. Improved Assay for Serum Amphotericin-B by Fast High Performance Liquid Chromatography

Abstract: A selective and sensitive method for the determination of amphotericin B in dog serum, after deproteinization, using high performance liquid chromatography HPLC with UV detection at 382 nm was developed. After protein precipitation with methanol, 5 μl of the supernatant was injected onto a reversed phase C18 column (1.5μm, 33 × 4.6 mm ID), with a mobile phase composed of 0.05 M sodium acetate - acetonitrile - methanol (40:30:30, v/v). The analytical recovery of amphotericin B in serum was 92.30%. The linearity of the assay method was verified up to 15 μg/mL of amphotericin B. The assay was reproducible with satisfactory intra-day and inter-day coefficients of variations (CV < 3%). The sensitivity of the method was ≤0.625 ng of amphotericin B for serum. The method was successfully employed for the pharmacokinetic analysis of amphotericin B in dogs following intravenous administration of liposomal amphotericin B (AmBisome).

Abstract: Interleukin-1 (IL-1) is a family of polypeptides that is thought to play an important role in the regulation of the systemic response to invasive stimuli such as infection, trauma or antigenic challenge, exerting both immunopotentiating and inflammatory actions on a wide variety of cellular targets. Binding studies with radiolabeled IL-Is showed that structurally distinct receptors are present on different cell types, suggesting that the different biological activities might be the consequence of IL-1 binding to distinct receptor subtypes.The highly detrimental activities of IL-1 represent a major drawback for its therapeutic application. Studies with IL-1 fragments have shown that the nonapeptide corresponding to the 163–171 sequence of human IL-1β is able to mimic the immunopotentiating activities of IL-1 while being devoid of its pro-inflammatory potentials, thus it might find clinical application as an immunostimulating agent. Since IL-1 is involved in some pathological conditions, strategies for anti-IL-1 therapy should also be developed. In this regard a recently characterized naturally occurring IL-1 receptor antagonist might play an important role and its structural relationships with IL-1β are hereafter discussed.

Abstract: Human interleukin 1 receptor antagonist (IL-1ra) and IL-1ra mutants were constitutively expressed in recombinantBacillus subtilisin endocellular and active form. In order to optimize the purification of the recombinant proteins, a new method has been developed. After bacterial growth in fermenter, release of recombinant protein was achieved by starvation-induced sporulation. The sporulation supernatant was recovered by centrifugation, filtered, and subjected sequentially to cation- and anion-exchange chromatography. Alternatively, the fermenter's contents were directly subjected to expanded bed adsorption on a Streamline cation-exchange column, thus avoiding the centrifugation and filtration steps. Up to 88 mg of biologically active purified recombinant protein per liter of culture was obtained, with a 72–79% recovery and 98% purity, depending on the molecule. By using the method described here, it is possible to achieve a spontaneous release of recombinant proteins expressed endocellularly at high levels inB. subtiliswithout need of a cell breakage step. Thus, this method could allow purification of the endocellular recombinant protein as if it were secreted. Furthermore, when using the expanded bed adsorption, highly purified protein was obtained in only two steps after sporulation. Among the advantages of the method, one of the most relevant is the possibility of keeping the system closed up to completion of the first purification step.

Abstract: High performance capillary electrophoresis was applied to analyse FCE 23762, a new antracycline analogue (doxorubicin derivative) with anti-tumor activity. It has been shown earlier that a FCE 23762 sample may contain two main impurities, one of the optical isomers of FCE 23762 and the most probable degradation product, adriamycinone. The pharmacologically active compound and its diastereomer were resolved in free zone electrophoresis experiments. Adriamycinone, a neutral compound, is baseline separated from the other two (cationic) compounds by micellar electrokinetic chromatography. The use of two different separation methods was advantageous for an efficient and sensitive analysis for the synthetic product of FCE 23762.

Abstract: The synthetic nonapeptide VQGEESNDK, corresponding to the fragment 163-171 of human IL-1 beta, showed in vivo immunomodulatory capacities qualitatively and quantitatively comparable to those of the mature human IL-1 beta protein. In fact, both IL-1 beta and the 163-171 fragment stimulated the immune response of normal mice and restored immune reactivities of immunocompromised animals. In addition, the synthetic IL-1 peptide was as efficient as the entire protein in inducing tumor rejection and radioprotection. On the other hand, the 163-171 fragment did not cause any of several inflammation-associated metabolic changes inducible by the whole IL-1 beta molecule in vivo: hypoferremia, hypoglycemia, hyperinsulinemia, increase in circulating corticosterone, SAA and fibrinogen, decrease in hepatic drug-metabolizing enzymes. Furthermore, at variance with IL-1 beta, the 163-171 peptide did not show the toxic effects causing shock and death in adrenalectomized mice. Thus, these results confirm our previous in vitro observations that functional domains are identifiable within the multipotent cytokine IL-1 beta, and demonstrate the biological relevance of this finding in a variety of in vivo systems. The identification of a selectively active fragment of a cytokine may thus represent a significant step towards a better directed and more rational immunotherapeutic approach.

Abstract: PT-9K/129G, a nontoxic mutant of pertussis toxin (PT) obtained by genetic manipulation, has been shown in animal models to be a promising candidate for new vaccines against whooping cough. To assess the safety and the immunogenicity of PT-9K/129G in humans, a pilot study has been performed in adult volunteers. The protein was found to be safe, capable of inducing high titers of toxin-neutralizing antibodies, and capable of generating immunological memory. In fact, vaccination caused an increase of cell-mediated response to PT, PT-9K/129G, S1 subunit, and B oligomer, indicating that memory T cells are induced by the vaccine. Since PT-9K/129G is mitogenic for T lymphocytes in vitro, it was investigated whether this activity is also present in vivo. No variation was observed in the proportion of T cells (CD3+), T helper cells (CD4+), and cytotoxic T cells (CD8+), as well as in that of other lymphoid populations, by FACS analysis. Interestingly, no thorough correlation was found between humoral and cellular responses. In one case, a very high cellular response was present in absence of detectable antibodies, suggesting that the antibody response, which is the only parameter measured in most clinical trials, may not give a complete picture of the response induced by a vaccine.

44. Insights into the role of intestinal microbiota in hematopoietic stem-cell transplantation

Abstract: The gut microbiota (GM) is able to modulate the human immune system. The development of novel investigation methods has provided better characterization of the GM, increasing our knowledge of the role of GM in the context of hematopoietic stem-cell transplantation (HSCT). In particular, the GM influences the development of the major complications seen after HSCT, having an impact on overall survival.In fact, this evidence highlights the possible therapeutic implications of modulation of the GM during HSCT. Insights into the complex mechanisms and functions of the GM are essential for the rational design of these therapeutics. To date, preemptive and curative approaches have been tested. The current state of understanding of the impact of the GM on HSCT, and therapies targeting the GM balance is reviewed herein.
