Abstract:Este ensayo estudia una serie de encuentros eróticos entre Don Quijote y los personajes femeninos Maritornes, Doña Rodríguez y Altisidora para demostrar que un aspecto del humor en la novela deriva de la presentación de personajes y situaciones basadas en la exageración cómica del comportamiento y habla de lo que Aristóteles y los teóricos de la literatura española de la Edad Moderna identificaron como “hombres bajos o no heroicos.” El ensayo muestra la importancia fundamental de la comedia áurea de la época en la prosa cómica de Cervantes, quien, al no contar con una teoría de la comedia en prosa, forjó la suya propia en el camino, al adoptar y adaptar conceptos cómicos originariamente aplicados al teatro.

That the Lutheran World Federation and the Pontifical Council for Unity attempted a joint declaration on the doctrine of justification is worthy of commendation. The resulting Joint Declaration constitutes some of the best contemporary efforts at ecumenical dialogue in the spirit of Christian union. This essay outlines the development of both medieval Catholic and subsequent Protestant conceptions of justification that led to disunion in the Western Church, reviews the initial points of division on the doctrine during the era of the Reformation for the purpose of grasping more fully the ecumenical feat of the JDDJ, and seeks to clarify what issues appear to remain unclear or unresolved in the document. The article also outlines the history of how Baptists in America have understood the doctrine of justification in order to consider how such Baptists might perceive the promise and potential lingering challenges or questions regarding the joint declaration.

Gastrointestinal (GI) symptoms are common among patients with systemic lupus erythematosus (SLE), although only rarely are they caused by active organ system involvement from SLE itself. Rapid diagnosis and appropriate treatment of lupus enteritis and other GI manifestations of SLE are critical, because of the potential for organ and life-threatening complications. The 3 main variants of lupus enteritis are lupus mesenteric vasculitis, intestinal pseudo-obstruction, and protein-losing enteropathy. These GI manifestations and others in patients with SLE are reviewed here.

Problems related to the combination of an arterial injury and a blunt fracture in the lower extremity are well known—delayed diagnosis, damaged soft tissue, and high amputation rate. The actual incidence of this injury pattern is, however, unknown. The purposes of this study were to determine the current incidence of named arterial injuries in patients with blunt fractures in the lower extremities and assess potential associated risk factors. This was a 7-year (2007–2013) retrospective review of patients =18 years with blunt lower extremity fractures at a Level I trauma center. Fracture location and concomitant arterial injury were determined and patients stratified by age, gender, and injury velocity. Low injury velocity was defined as falls or assaults, whereas an injury secondary to a motorized vehicle was defined as high velocity. A total of 4413 patients (mean age 52.2 years, 54.3% male, mean Injury Severity Score 13.1) were identified. Forty-six patients (1.04%) had arterial injuries (20.4% common femoral, 8.2% superficial femoral, 44.9% popliteal, and 26.5% shank). After stratifying by age and injury velocity, younger age was associated with a significantly higher rate of vascular injury. For high-velocity injuries, there was no difference based on age. In conclusion, the prevalence of arterial injury after blunt lower extremity fractures is 1.04 per cent in our study. A significant paradoxical relationship exists between age and associated arterial injuries in patients with low-velocity injuries. If these data are confirmed in future studies, a low index of suspicion in patients >55 years after falls is appropriate.

Postoperative CT is a useful adjunct to emergent laparotomy in the trauma setting for identifying unexpected injuries, confirming suspected injuries that were not fully explored at initial surgery, and aiding management after both blunt and penetrating trauma.

Abstract:Este trabajo analiza la aventura del yelmo de Mambrino (Don Quijote1.21) como una sátira política en la que la parodia literaria da paso a una crítica generalizada del imperialismo español. Propone una lectura del episodio cervantino como un tratamiento satírico de los arbitrios contemporáneos que pretendían aumentar los ingresos de la Real Hacienda por medio del corso patrocinado por la Corona y la continuada expansión territorial en ultramar dentro del marco ideológico de la Reconquista. Cervantes contrasta este modelo de piratería y conquista militar con un nuevo imperio de contratos y relaciones económicas voluntarias basado en los principios escolásticos del derecho a la propiedad privada y la justicia. Estos conceptos, tal y como fueron formulados por los teólogos y filósofos morales de la Escuela de Salamanca durante el siglo XVI, se concebían como la expresión de la ley natural y el derecho de gentes, y por tanto eran igualmente aplicables a todo el mundo, aunque no fuera ni europeo ni cristiano. Además, los escolásticos salmantinos, y con ellos Cervantes, postulaban que el intercambio económico pacífico era congruente con la evolución socio-económica de los seres humanos y, por consiguiente, indicio de un alto grado de desarrollo social.

Traumatic transection of the thoracic aorta is a life-threatening complication that most commonly occurs after high-speed motor vehicle collisions. Although such injuries were previously treated with open surgical reconstruction, they are now more commonly being treated with endovascularly placed stent grafts. Unfortunately, most stent grafts are designed for treating aortic aneurysmal disease instead of traumatic injury. Further refinements in stent graft technology depend on a thorough anatomic understanding of the transection injury process.

Abstract:Felipo de Carrizales, protagonista epónimo de “El celoso extremeño,” es una de las más extravagantes creaciones cervantinas, un personaje que oscila entre los extremos conductuales de desordenada prodigalidad y arrepentimiento por sus fallos personales. La característica más notable de Carrizales son sus exagerados celos, que no se corresponden con conceptos modernos de verosimilitud psicológica sino con ideologías contemporáneas de ética comercial. En la época, el lucro y la lujuria se concebían como dos caras de la misma moneda, variantes anejas de la misma patología moral. Así, el desordenado apetito de riqueza está en consonancia con los irracionales celos: éstos vienen a ser la expresión erótica del incontrolado impulso hacia el enriquecimiento excesivo por medio de la usura. Con Carrizales, Cervantes trasciende la representación tradicional del marido celoso mediante la incorporación de un discurso económico específico, subordinando sus tópicos a un proceso de caracterización literaria para enriquecer la complejidad del personaje.

Although many states mandate that motorcyclists wear helmets, their laws do not indicate which type of helmet should be used. In addition, there are no prospective studies in the literature evaluating patterns of injuries as they relate to helmet type. The hypothesis in this study was that full-face helmets (FFHs) reduce craniofacial injuries associated with motorcycle collisions when compared with other helmet types.

To investigate reticulocyte hemglobin content (CHr) as a primary screening test to detect iron deficiency (ID) in healthy toddlers. Demographic and dietary risk factors associated with ID were studied for an association with low CHr.

Based on empirical data from a qualitative study, this paper explores the complexity of ‘real world’ management in Hong Kong’s public sector, as contrasted with various paradigmatic claims under ‘new public management’ (NPM). A plurality of sub-worlds within the broad public sector is identified, which makes the management roles and responsibilities much less ‘homogenised’ than depicted in NPM exhortations. The instrumental rationality underpinning NPM is identified as too restrictive in understanding the way in which public managers reach decisions. When the daily challenges of reconciling values and practices arising from the complexities of politics, policies and service delivery are considered it is necessary to incorporate ideas related to procedural and expressive rationality to fully appreciate the nature of management in public organisations.
