Aortic stenosis (AS) is characterized by calcification and fibrosis. The ability to quantify these processes simultaneously has been limited with previous imaging methods. The purpose of this study was to evaluate the aortic valve fibrocalcific volume by computed tomography (CT) angiography in patients with AS, in particular, to assess its reproducibility, association with histology and disease severity, and ability to predict/track progression. In 136 patients with AS, fibrocalcific volume was calculated on CT angiograms at baseline and after 1 year. CT attenuation distributions were analyzed using Gaussian-mixture-modeling to derive thresholds for tissue types enabling the quantification of calcific, noncalcific, and fibrocalcific volumes. Scan-rescan reproducibility was assessed and validation provided against histology and in an external cohort. Fibrocalcific volume measurements took 5.8 ± 1.0 min/scan, demonstrating good correlation with ex vivo valve weight (r = 0.51; P < 0.001) and excellent scan-rescan reproducibility (mean difference −1%, limits of agreement −4.5% to 2.8%). Baseline fibrocalcific volumes correlated with mean gradient on echocardiography in both male and female participants (rho = 0.64 and 0.69, respectively; both P < 0.001) and in the external validation cohort (n = 66, rho = 0.58; P < 0.001). The relationship was driven principally by calcific volume in men and fibrotic volume in women. After 1 year, fibrocalcific volume increased by 17% and correlated with progression in mean gradient (rho = 0.32; P = 0.003). Baseline fibrocalcific volume was the strongest predictor of subsequent mean gradient progression, with a particularly strong association in female patients (rho = 0.75; P < 0.001). The aortic valve fibrocalcific volume provides an anatomic assessment of AS severity that can track disease progression precisely. BACKGROUND: Hypertension is one of the main risk factors for dementia and cognitive impairment. METHODS: We used the model of transverse aortic constriction to induce chronic pressure overload in mice. We characterized brain injury by advanced translational applications of magnetic resonance imaging. In parallel, we analyzed peripheral target organ damage induced by chronic pressure overload by ultrasonography. Microscopical characterization of brain vasculature was performed as well, together with the analysis of immune and inflammatory markers. RESULTS: We identified a specific structural, microstructural, and functional brain injury. In particular, we highlighted a regional enlargement of the hypothalamus, microstructural damage in the white matter of the fimbria, and a reduction of the cerebral blood flow. A parallel analysis performed by confocal microscopy revealed a correspondent tissue damage evidenced by a reduction of cerebral capillary density, paired with loss of pericyte coverage. We assessed cognitive impairment and cardiac damage induced by hypertension to perform correlation analyses with the brain injury severity. At the mechanistic level, we found that CD8+T cells, producing interferon-γ, infiltrated the brain of hypertensive mice. By neutralizing this proinflammatory cytokine, we obtained a rescue of the phenotype, demonstrating their crucial role in establishing the microvascular damage. CONCLUSIONS: Overall, we have used translational tools to comprehensively characterize brain injury in a mouse model of hypertension induced by chronic pressure overload. We have identified early cerebrovascular damage in hypertensive mice, sustained by CD8+IFN-γ+T lymphocytes, which fuel neuroinflammation to establish the injury of brain capillaries. [Display omitted] • We describe a Cretaceous endemic Neotropical freshwater catfish from Morocco. • Overlooked evolutionary events took place at the outset of the break-up of Gondwana. • Iconic Neotropical freshwater lineages diversified after surviving the K/Pg extinction. Siluriformes are considered as primarily freshwater and have frequently been a model for the study of historical biogeography. Among catfishes, the most diverse clade is the Loricarioidei, a Neotropical group for which the fossil record extends back to the Palaeocene of Argentina. Here we describe a fossil from the early Late Cretaceous of Morocco, exhibiting typical morphological traits of the Loricariidae. A phylogenetic analysis integrating morphological characters with a multigene database for the main loricarioid lineages and outgroups highly supports inclusion of the fossil within the Loricariidae. A time-calibrated analysis corroborates the origin of loricarioids at about 112 MYA. The presence of this loricariid in Africa provides evidence that loricarioids have diversified before the separation of Africa and South America. The Moroccan loricariid shows an ancient evolutionary history that, in Africa, ended in the Late Cretaceous but persisted in South America, later surviving the K/Pg extinction. The oropharynx represents one of the most challenging areas to reconstruct for the head and neck surgeon. The buccinator myomucosal island flaps pedicled on the facial artery [tunnelized facial artery myomucosal island flap (t-FAMMIF)] or the buccal artery [buccal artery myomucosal island flap (BAMMIF)] are an ideal reconstructive option for moderate size defects measuring up to 8–9 cm. Two fresh specimens have been used to show the step-by-step surgical technique of both island flaps. Design and flap extension, dissection plane, identification of the vascular pedicle, flap rotation and insetting are described. Reconstructive indications and the pros and cons of each one are discussed in this article. Myomucosal island flaps represent a very useful and versatile option for the functional reconstruction of the oropharynx. A detailed knowledge of the vascular anatomy of the cheek is key to obtain a large flap while minimizing the risk of complications. [ABSTRACT FROM AUTHOR]

To evaluate the durability, effectiveness, and safety of transperineal laser ablation (TPLA) of the prostate. Patients with symptomatic benign prostatic hyperplasia (BPH) underwent TPLA with a 1,064-nm continuous-wave diode laser. International Prostate Symptom Score (IPSS), quality of life (QoL), postvoid residual (PVR), and prostate volume were evaluated at baseline and successive timepoints. Forty prospectively enrolled patients had follow-up of ≥36 months; median duration of follow-up was 57 months (range, 36–76 months). Compared with baseline, the median reduction in IPSS at 12-month follow-up was 74% (interquartile range [IQR], 60%–81%) (P <.001). Median QoL score at 12 months was improved from 5 (IQR, 4–5) at baseline to 1 (IQR, 0–1) (P <.001). Median PVR at 12 months decreased from 108 mL (IQR, 38–178 mL) to 13.5 mL (IQR, 0–40.5 mL) (P <.001), a median reduction of 88% (IQR, 61%–100%). At 12 months, median prostate volume was significantly reduced from 66 mL (IQR, 48.5–86.5 mL) to 46 mL (IQR, 36–65 mL) (P <.001), a median reduction of 32% (IQR, 21%–45%). For all of these parameters, the benefit of TPLA persisted at last follow-up, and all changes were statistically significant compared with baseline. There were no intraprocedural adverse events; periprocedural adverse events consisted of 1 case of prostatitis and 1 case of urinary tract infection (both Society of Interventional Radiology [SIR] Grade I). Social circus refers to programs that use circus arts to facilitate social intervention with people experiencing marginalization. Although some programs focus specifically on youth, little is known about how they are impacted by their participation. We examined the experiences of youth participating in a four-day social circus event. Four themes were identified that characterized participants' experiences: (a) creating a safe social space; (b) enriching your self-understanding; (c) bolstering your expressive capacities; and (d) experiencing the world around you. This research highlights how social circus activities can create safe and enriching social spaces that are adapted to the experiences of youth. [ABSTRACT FROM AUTHOR]

Objective: The Wheel of Cardiovascular Health Diet was recently elaborated based on current nutritional recommendations for healthy eating and was made for person-centered nutritional counseling, thus it may be an interesting tool to improve food awareness. However, its validity is yet to be tested. We hypothesized that the self-perception of patients diagnosed with chronic diseases, evaluated by the Wheel of Cardiovascular Health Diet, is satisfactory in assessing the quality of their diets, compared with the Healthy Eating Index-2020. Methods: This is a cross-sectional analysis of adults with uncontrolled hypertension or type 2 diabetes. The patients answered the Wheel of Cardiovascular Health Diet, and then, based on the food frequency questionnaire, we analyzed the Healthy Eating Index-2020. Results: A total of 330 patients were included in the study: 91.5% had hypertension, 58.5% had type 2 diabetes, and the median age was 58 (50-65) years. The mean difference observed between the percentage of the graphic area assessed by the patients’ self-perception from the Wheel of Cardiovascular Health Diet and Healthy Eating Index-2020 was-10.0% (95%CI-35.3 to 15.3), and a moderate correlation was observed. Linear Regression models showed that a 10-point increase in patients’ self-perception in the Wheel of Cardiovascular Health Diet is associated with a 2.9% increase (95%CI 2.08 to 3.70) in the diet quality by the Healthy Eating Index-2020 and is associated with lower BMI values: β =-0.42kg/m² (95%CI-0.83 to-0.01). Conclusion: The Wheel of Cardiovascular Health Diet performed satisfactorily regarding validity and reliability by BMI and was associated with higher overall dietary quality, with the Healthy Eating Index-2020 as a relative reference. Data on P homeostasis in insulin resistance (IR) are still conflicting. We investigated calcium-phosphate homeostasis parameters in men with/without IR. 177 volunteers (aged 61.62 ± 12.11), whose body mass index (BMI) was 29.97 ± 6.35, were studied. On fasting blood and spot urine samples, we measured serum creatinine, sodium (sNa), potassium (sK), chloride (sCl), calcium (sCa), phosphate (sP), alkaline phosphatase total activity (ALP), glucose, insulin, parathyroid hormone (PTH), 25-hydroxy-vitamin D [25(OH)D], and urinary electrolytes corrected for creatinine (uNa/Cr, uK/Cr, uCl/Cr, uCa/Cr, and uP/Cr). Through the QUICKI index, we separated subjects with (IR+, n = 68) or without (IR-, n = 109) IR, and their parameters were compared. Associations were assessed by age-adjusted partial correlation, whose coefficients were compared by Fisher's transform. IR + had higher sP (3.54 ± 0.65 vs. 3.35 ± 0.47, p = 0.044) and lower uCa/Cr levels (0.073 ± 0.056 vs. 0.095 ± 0.072, p = 0.047) than IR-. BMI correlated with sP (r = 0.21, p < 0.05) and PTH (r = 0.29, p < 0.01). QUICKI negatively correlated with sCa (r = −0.22, p < 0.05) and positively with uCa/Cr (r = 0.21, p < 0.05), in turn correlating with uNa/Cr (r = 0.45, p < 0.001). In both groups, uCa/Cr correlated with eGFR and uNa/Cr (p < 0.05 to p < 0.001). In IR + only, sP correlated with BMI, PTH with insulin, and uP/Cr (p < 0.05 for all). IR+ and IR-coefficients differed (p < 0.05 to p < 0.001) for the correlation of sP with BMI and of PTH with insulin and uP/Cr. The higher sP and lower uCa/Cr levels found in men with IR + suggest that IR could modulate calcium-phosphate homeostasis, likely by affecting their renal handling. • Men with insulin resistance (IR+) had higher sP and lower uCa/Cr than IR- ones. • In IR+ only, sP correlated with BMI, and PTH with insulin and uP/Cr. • QUICKI (index of insulin sensitivity) positively correlated with uCa/Cr. • The renal clearances of Ca and P significantly differed between IR+ and IR-. Community-acquired pneumonia (CAP) is associated with low-grade endotoxemia but its relationship with cardiovascular events (CVE) has not been investigated. We evaluated the incidence of CVE including myocardial infarction, stroke, and cardiovascular death in 523 adult patients hospitalized for CAP. Serum lipopolysaccharide (LPS) and zonulin, a marker of gut permeability, were analyzed in the cohort, that was followed-up during hospitalization and up to 43 months thereafter. During the hospital-stay, 55 patients experienced CVE with a progressive increase from the lowest (0.6%) to highest LPS tertile (23.6%, p < 0.001). Logistic regression analyses showed that higher LPS tertile was independently associated with CVE; LPS significantly correlated with age, hs-CRP and zonulin. In a sub-group of 23 CAP patients, blood E. coli DNA was higher in patients compared to 24 controls and correlated with LPS. During the long-term follow-up, 102 new CVE were registered; the highest tertile of LPS levels was associated with incident CVE; Cox regression analysis showed that LPS tertiles, age, history of CHD, and diabetes independently predicted CVE. In CAP low-grade endotoxemia is associated to short- and long-term risk of CVE. Further study is necessary to assess if lowering LPS by non-absorbable antibiotics may result in improved outcomes. The manufacturing landscape is rapidly evolving, being characterized by the emergence of Industry 5.0, leading to a growing recognition of the importance of a human-centered, sustainable and resilient approach. In response to these needs, collaborative robotics has emerged as a promising solution to enhance flexibility in industrial operations, while ensuring a higher level of safety. Cobots enable human-robot collaboration without the need for physical barriers, reduce the footprint of the robotic station, and foster a closer working relationship between humans and machines. This article focuses on an application of collaborative robotics in end-of-line industrial operations, specifically in the unloading of products from an automatic machine. By managing repetitive tasks, such as placing the trays near the end of the line of the automatic machine, waiting for them to be filled, and placing them on a trolley, which will then be moved by the human operator, the robot significantly improves efficiency. The proposed solution involves alternating the placement of trays in two rack trolleys, allowing the worker to replace the full trolleys with empty ones at his own timing. The paper examines the potential advantages of using a human-centric approach that emphasizes operator empowerment and human oversight: the collaborative robotic cell and the automation implemented free the workers from the low value-added operations and allow them to oversee and manage multiple machines simultaneously, thus enhancing productivity. The approach behind this proposal is to keep robotic tasks simple, easily reprogrammable and modular, while empowering the human operator to engage in supervision, decision-making, and problem-solving tasks. This collaboration allows the operator to assume a supervisory role, organizing production based on identified needs, and elevating their involvement to production line management tasks. In recent years, there has been a growing recognition of the importance of balance and coordination in maintaining overall health, particularly among the elderly population. As societies around the world experience demographic shifts with an increasing number of elderly individuals, age-related declines in balance and coordination can lead to an elevated risk of falls and fall-related injuries, making it a significant public health concern. The need for effective balance rehabilitation and fall prevention strategies has become paramount. This paper presents a motion platform designed for balance rehabilitation exercises using exergames. The system consists of a 3 degrees of freedom (3DOF) platform actuated by four servomotors controlled by Omron-DeltaTau real-time motion controller. As a novelty with respect to other State of Art (SoA) actuated platforms, in the present work the movements of the user's body, which determine the platform motion, are tracked by exploiting a markerless motion capture system with an RGB camera and pose-estimation algorithm MediaPipe. The human displacement is used to generate the input for the roll rotation of the platform according to a one d.o.f. transfer function, whose parameters can be set to get different levels of difficulties in the exercise. The device offers therefore the flexibility to set different parameters to modify the level of the users' training. The proposed device has the potential for a larger variety of rehabilitation activities and exergames. This paper describes the elements adopted for the implementation of this human-in-the-loop system, presents some preliminary tests to explore the influence of transfer function parameters on human and platform movements, and concludes with future development possibilities. To evaluate safety and effectiveness of prostatic artery embolization (PAE) using polyethylene glycol (PEG) microspheres in patients with moderate-to-severe benign prostatic hyperplasia (BPH). A single-center, prospective study of 30 patients who underwent PAE from August 2020 to December 2021 using PEG 400-μm microspheres was conducted. Patient evaluation data using International Prostate Symptom Score (IPSS), quality of life (QoL) score, prostate-specific antigen (PSA), peak urinary flow rate (Qmax), postvoid residual volume (PVR), and prostate volume (PV) at baseline and 3 and 12 months after PAE were obtained. Bilateral PAE was performed in all patients. One patient had early clinical failure (3.3%) and another presented with lower urinary tract symptoms (LUTSs) recurrence (3.3%) at the 12-month follow-up. Twenty-eight patients (93.3%) experienced significant and durable LUTS improvement. Mean absolute (and relative) improvement at 3 and 12 months were: IPSS, 14.6 points (−69%) for both; QoL, 3.3 points (−70%) and 3.5 points (−74%); Qmax, 6.3 mL/s (+78%) and 8.6 mL/s (+100%); PSA reduction, 1.2 ng/mL (−22%) and 1.0 ng/mL (−15%); PVR reduction, 48 mL (−56%) and 58.2 mL (−49%); PV reduction, 23.4 cm3 (−29%) and 19.6cm3 (−25%); (P <.05 for all). No major adverse events were observed. Minor adverse events included urinary tract infection (4/30, 13.3%), prostatic tissue elimination (3/30, 10%), penile punctiform ulcer (1/30, 3.3%), and urinary retention (1/30, 3.3%). PAE using PEG microspheres was observed to be effective with sustained LUTS improvement at the 12-month follow-up. The ability to change direction rapidly is a key fitness quality especially in invasive sports where young players perform approximately 300 changes of direction in a game. There is currently limited understanding of how anthropometric characteristics and maturation status influence change of direction ability in adolescent. Therefore, the purpose of this investigation is to assess the influence of anthropometrics and maturation status on change of direction ability in young people. The study involved 706 adolescents (367 girls) aged 14-19-yearold attending the same high school in Northern Italy. Stature, body mass, seated height and leg length were measured to determine the anthropometrics and maturation status of the participants. Repeated change of direction ability (10 x 5 m shuttle run test), lower limb power and muscle strength were evaluated using field tests from the Eurofit test battery. Maturity offset was calculated separately for boys and girls, in accord with the equation proposed by Mirwald. Preliminary analysis with 10 x 5 m as a dependent variable and sex and PHV as a fixed factor, suggests a significant difference between sex (p < 0.001; d = 0.35) but not with PHV (p = 0.986; d = 0.000) and interaction PHV x sex (p = 0.836; d = 0.000). Our results suggested that repeated change of direction performance was influenced by anthropometrics, maturation and muscle qualities in adolescent boys and girls. [ABSTRACT FROM AUTHOR]

Metastatic breast cancer (BC) remains an incurable disease. Besides endocrine and targeted agents, chemotherapy is still a relevant therapeutic option for this disease. Recently, antibody-drug conjugates (ADCs) have shown to overcome the lack of tumor specificity and systemic toxicity typically associated with traditional chemotherapies, thus improving the therapeutic index. To effectively exploit this technological breakthrough, identification of optimal target antigens (Ags) is of utmost importance. To make the ideal target, differential expression of target Ags between healthy and cancer tissues, as well as specific mechanisms of ADC internalization after Ag-antibody interaction are required. Therefore, several in silico strategies to identify and characterize new promising candidate Ags have been developed. If initial in vitro and in vivo positive data are documented, thus providing a biological rationale for further Ag investigation, early phase clinical trials are designed. In BC, these strategies have already led to the development of effective ADCs, namely trastuzumab emtansine (T-DM1), trastuzumab deruxtecan (T-DXd) and sacituzumab govitecan (SG), primarily targeting HER2 and TROP-2. However, promising new Ags are currently under investigation, with encouraging results especially coming from targeting HER3, FRα, Tissue Factor, LIV-1, ROR1-2, and B7–H4. In this review, we describe the landscape of emergent and future potential targets (i.e., other than HER2 and TROP-2) investigated in BC for ADC development. Predominant target expression, function, preclinical rationale, potential clinical implication, as well as preliminary clinical trial results are provided. • Antibody-drug conjugates (ADCs) show improved efficacy and therapeutic index (80 /85). • To exploit ADCs, identifying optimal target antigens (Ags) is fundamental (77 /85). • In BC, ADC targeting HER2 (i.e., T-DM, T-DXd) and TROP-2 (i.e., SG) are approved (84 /85). • HER3, FRα, Tissue Factor, LIV-1, ROR1-2, and B7–H4 are promising target candidates. This paper offers a collection of found poems offering insights into representative worlds of children living with dis/abilities, chronic hospitalizations, and the ethical dilemmas surrounding their perioperative care. The collection represents the sum tota poetic crystallization of a multiyear ethnography focused on the ethical experiences of the childhood osteogenesis imperfecta community as encountered in a specialized North American pediatric OI clinic. The collection deliberately bridges multiple separate (yet deeply connected) worlds of children, clinicians, and family members to reveal their interdependence and yet preserve each individual's distinct viewpoints. Moving forward, we are discussing ways in which we can reach more applicants such as the following: expanding the number of early career editors both nationally and internationally; implementing a team-based approach where each editor mentors >1 trainee simultaneously; and providing webinar-based training and allowing early career reviewers not selected for the program to enter the I Hypertension i reviewer database to gain some initial experience. Anyone who has served as an editor or associate editor knows that reviewers who agree to review articles as a service to our field are difficult to find - good reviewers are even more difficult.
