Using 2 global postmarketing surveillance databases, Goldman and colleagues report that progressive multifocal leukoencephalopathy (PML), a viral disease associated with profound immunosuppression, occurs in approximately 0.9 cases per 1000 recipients of CD19-directed CAR T-cell therapy. The risk of PML appears higher with CAR T-cell therapy than other cancer therapies, but its precise role cannot be distinguished from antecedent therapies that these patients receive.

INTRODUCTION:CAR-T therapy is an effective treatment for hematological malignancies. However, in recent years there is a growing body of literature suggesting adverse cardiovascular (CV) events among patients treated with CAR-T therapy. Whether cardiovascular risk factors serve as determinants of CV complications and CAR-T outcomes is unclear. The aim of this study is to investigate the landscape and prognostic role of CV comorbidities in patients with non-Hodgkin lymphoma (NHL) treated with autologous CD19- CAR-T.

It has been reported that Atlantic tomcod (Microgadus tomcod) from the Hudson River exhibit an extremely high incidence of liver tumors. More than 90% of spawning 2-year-old fish display hepatocellular carcinomas. In contrast, representatives of this species from a relatively pristine environment show a much lower incidence of tumors. Genomic DNA and mitochondrial DNA (mtDNA) were isolated from tomcod from the Hudson River, New York, and the Saco River and Royal River, Maine. We found a statistically significant difference in the frequency ofPstI-generated restriction fragment length polymorphisms in theabl cellular oncogene between Hudson and Maine tomcod. Allelic variation was observed at two of the threeabl domains scored. A single composite genotype seen in approximately 40% of Hudson River fish was seen in only one Maine fish. This polymorphism enabled us to differentiate a Hudson River population from that encountered in the Maine rivers. This is the first demonstration of a population-specific polymorphism at a cellular oncogene locus in any species. In contrast, no restriction site polymorphisms were seen in mtDNA between the populations. The lack of mtDNA diversity in these fish is consistent with the geological history of the area. In combination, these results suggest that the genetic diversity observed at the c-abl oncogene locus must have been a fairly recent event and that oncogene loci may be particularly sensitive to mutational change.

The effect of a behaviorally-oriented program to reduce calories and increase physical activity on HDL cholesterol level was evaluated in overweight older adult subjects (n = 31). HDL cholesterol increased from 45.8 to 52.1 mg/dl (p < .0003), and total mean cholesterol increased from 238.0 to 249.5 mg/dl (p < .05). Body weight decreased from 175.0 lbs to 172.6 lbs (p < .03). Analysis of baseline and 40 week food frequency questionnaires indicated that the mean caloric intake had decreased from 1535 to 1303 kcal/day (p < .02), and total fat intake decreased by 9.8 gm/day (p < .01). Weight loss accounted for 13% of the variance in HDL cholesterol change (p < .05). Decreasing fat intake resulted in a smaller increase in HDL cholesterol (F, (l,29) = 5.91, p < .03). Our findings provide further support that a prudent approach to weight reduction can be beneficial in an older adult population.

Purpose. To report a case of a 34-year-old male with recurrent herpes zoster ophthalmicus (HZO) preceded by a 6-week cycle of anabolic steroids and high-dose amino acid supplementation. Case Presentation. A 34-year-old man presented to our institution for left eye pain for one week associated with a vesicular rash in the V1 dermatome, respecting the midline. The patient had no significant past medical or past ocular history, including systemic immunosuppressive agents or HIV. However, prior to the onset of his symptoms the patient had completed a 6-week course of anabolic steroids including trenbolone, deca-durabolin, and testosterone as well as high-dose arginine supplementation averaging more than 40 grams a day. The best-corrected vision was 20/25 OS with slit-lamp examination remarkable for punctate staining and pseudodendrites at 6 o’clock, outside the visual axis. The patient was treated with oral acyclovir 800 mg five times a day for seven days along with prednisolone QID and moxifloxacin QID which was tapered over a month. Four months after resolution, the patient developed a recurrent HZO keratitis preceded by another cycle of anabolic steroids and amino acid supplementation. Conclusion. In vitro L-arginine supplementation has been associated with the proliferation and virulence of a variety of herpes viruses. Anabolic steroids have also been demonstrated by various studies to negatively affect cell-mediated immunity necessary to prevent viral infection. Thus, it is possible that anabolic steroids in conjunction with increased L-arginine intake may have precipitated a recurrent HZO in a previously healthy, immunocompetent individual.

Dihydropyridine and nondihydropyridine calcium channel blockers (CCB) differ in pharmacologic characteristics. Few clinical studies distinguish effects of CCB as monotherapy. We conducted a comprehensive comparison of two CCB on patients with moderate to severe hypertension. Thirty patients with pretreatment diastolic blood pressures =100 mm Hg were randomly assigned to either nifedipine–GITS or verapamil-SR. Dose titration achieved a diastolic blood pressure of =95 mm Hg or a decrease of =15 mm Hg over 4 weeks. Clinic blood pressure (BP), 24-h ambulatory BP, exercise BP, left ventricular mass, systolic and diastolic function by echocardiography, and coronary flow reserve by split-dose thallium-201 imaging with adenosine were assessed at baseline, end of titration, 3 months and 6 months of treatment. Plasma renin activity, atrial natriuretic peptide, norepinephrine, and epinephrine were assayed. Both drugs caused similar reductions in clinic and 24-h ambulatory BP and similar reductions in left ventricular mass index. Compared to nifedipine–GITS, verapamil-SR produced a significantly lower resting and peak exercise heart rate. Nifedipine–GITS elicited a lower peak exercise systolic BP. At end titration nifedipine–GITS produced lower plasma atrial natriuretic peptide levels, no longer apparent by 6 months. Plasma norepinephrine was lower with verapamil-SR, also at end titration and at 3 months, but not at 6 months. Plasma epinephrine and plasma renin activity were unchanged by either drug. There was no difference for systolic or diastolic left ventricular function or coronary flow reserve between the two treatments. Once daily nifedipine–GITS and verapamil-SR are equally effective for reduction of arterial pressure in moderate to severe hypertension. Differences in their hemodynamic profiles and neurohormonal responses are consistent with preclinical pharmacologic characteristics.
