A 54-year-old woman presented with extremely fluctuating and symptomatic blood glucose levels. Very high levels of somatostatin and low levels of insulin, C-peptide, gastric inhibitory peptide (GIP), and glucagon-like peptide-1 (GLP-1) in peripheral blood were constantly present. A benign somatostatinoma was localized by meta-iodobenzyl guanidine iodine 123 (MIBG-I123) scintigraphy and succesfully removed encapsulated in an ovarian teratoma. The patient made a complete recovery. The case described is unique with regard to clinical symptomatology and anatomic localization of the tumor.

The isotopic dilution technique was used for determining the content of myo-inositol in human urine, plasma and haemolysed erythrocyte samples. A deuterated myo-inositol, synthesized from inosose-2 by base-catalysed exchange of hydrogens by deuterium, followed by reduction of the inosose with 2H2, was added as internal standard to the samples at an early stage in the analytical procedure. After separation and derivatization to the hexa-acetate, the gas chromatographic mass spectrometric analysis was carried out. A 25 m fused silica capillary column coated with methyl silicone was used, and the ions selected for monitoring were m/z210 and m/z214, which are characteristic and abundant fragment ions from unlabelled and hexadeuterated myo-inositolhexa-acetate, respectively. Calibration curves from water, urine, plasma and haemolysed erythrocytes show parallel, linear responses in the ratio between analyte and internal standard in the area of interest (0.2–2.0).

Moderate glucose intolerance with concomitant hyperresponse of insulin production and absence of the normal suppression of growth hormone level in blood was found in two siblings with Wilson's disease. The abnormalities disappeared after one year's treatment with penicillamine at the time when liver biopsy showed normalization of the liver structure.

Muscle biopsies from eight cases of acute juvenile diabetes (a few weeks after the beginning of the disease) revealed well marked degenerative changes combined with vigorous regeneration of the terminal neuromuscular apparatus. This observation is in accordance with previous demonstrations of neurophysiological disturbances at this early stage of diabetes. Dans huit cas de diabete juvénile aigu (quelques semaines après le début de la maladie) des biopsies musculaires ont révélé des modifications dégénératives bien marquées, associées á une puissante régénération de l'appareil neuro-musculaire terminal. Cette observation est en accord avec les démonstrations précédentes de troubles neurophysiologiques à ce stade précoce du diabète. Muskelbiopsien von 8 Patienten mit akutem juvenilem Diabetes (einige Wochen nach Krankheitsausbruch) zeigten ausgeprägte degenerative Veränderungen mit kräftigen Regenerationserscheinungen an der neuromuskulären Endplatte. Diese Befunde stimmen mit früher mitgeteilten Beobachtungen neurophysiologischer Störungen dieses Frühstadiums des Diabetes überein.

Muscle biopsies were studied from sixteen juvenile, long-term diabetics. Most of the patients had severe retinopathy, but clinical signs of other forms of angiopathy and neuropathy were either absent or mild. The biopsies revealed severe degeneration, as well as ineffective regenerative changes of the terminal nerve fibres and endorgans. Des biopsies musculaires ont été étudiées chez seize sujets jeunes diabétiques depuis longtemps. La plupart des patients étaient atteints de rétinopathie grave, mais les signes cliniques d'autres formes d'angio-pathie et de neuropathie étaient soit absents, soit légers. Les biopsies ont révélé une dégénérescence grave ainsi que des modifications régénératives inefficaces des fibres nerveuses terminales et des organes terminaux. Die meisten Patienten litten an einer schweren Rétinopathie, doch wiesen sie nur schwache oder keine klinischen Anzeichen eines anderen Gefäßleidens oder einer Neuropathie auf. Die Biopsien zeigten schwere degenerative Erscheinungen und außerdem frustrane regenerative Veränderungen der terminalen Nervenfasern und Endorgane.
