•The Glasgow prognostic score (GPS) is a potent prognostic tool in malignancy.•In RRMM undergoing CAR-T, high-risk GPS had increased risk of grade 3 CRS and ICANS.•There was significant difference in day-100 OS between high-risk versus low-risk GPS.•GPS remained significant in multivariable models for day-100 overall survival.•GPS is a simple, powerful tool for identifying patients at higher risk from CART.

Abstract

Idecabtagene vicleucel (ide-cel) was FDA-approved in March 2021 for the treatment of relapsed/refractory multiple myeloma after 4 lines of therapy. On the KarMMa trial, grade ≥ 3 cytopenias and infections were common. We sought to characterize cytopenias and infections within 100 days after ide-cel in the standard-of-care (SOC) setting. This multi-center retrospective study included 52 patients who received SOC ide-cel; 47 reached day-90 follow-up. Data were censored at day 100. Grade ≥ 3 cytopenia was present among 65% of patients at day 30 and 40% of patients at day 90. Granulocyte colony stimulating factor (G-CSF) was administered to 88%, packed red blood cell transfusions to 63%, platelet transfusions to 42%, thrombopoietin (TPO) agonists to 21%, intravenous immunoglobulin to 13%, and CD34+ stem cell boosts to 8%. At day 100, 19% and 13% of patients had ongoing use of TPO agonists and G-CSF, respectively. Infections occurred in 54% of patients and were grade ≥ 3 in 23%. Earlier infections in the first 30 days were typically bacterial (68%) and severe (50%). Later infections between days 31 and 100 were 50% bacterial and 42% viral; only 13% were grade ≥ 3. On univariate analysis, high pre-CAR-T marrow myeloma burden (≥ 50%), circulating plasma cells at pre-lymphodepletion (LD), and grade ≥ 3 anemia at pre-LD were associated with grade ≥ 3 cytopenia at both days 30 and 90. Longer time from last bridging treatment to LD was the only significant risk factor for infection.

Minor histocompatibility antigen (mHA)-specific alloreactive donor T cells cause graft vs. host disease (GVHD) in matched related donor allogeneic hematopoietic cell transplantation (HCT). In a translational phase I trial, we expanded and infused mHA-specific donor regulatory T cells (Treg) in the context of sirolimus-based pharmacologic prophylaxis to examine safety and preliminary efficacy of this targeted GVHD prevention approach.

16. The knowns and unknowns of disparities, biology, and clinical outcomes in Hispanic and Latinx multiple myeloma patients in the U.S.

Abstract

Multiple myeloma (MM) is a plasma cell malignancy that accounts for approximately a tenth of all blood cancers. The last two decades, have witnessed considerable improvement in survival rates driven by a better understanding of the biology of MM and the development of novel therapies to treat MM. Despite these advancements, MM remains an incurable disease. Marked disparities exist by race and ethnicity for MM, particularly a higher incidence and an earlier age of onset of MM among Non-Hispanic Black and Hispanic and Latinx individuals compared to Non-Hispanic Whites. Inequities in receipt and time to utilization of novel therapies and stem cell transplantation as well as participation in clinical trials have also been observed for Non-Hispanic Black and Hispanic and Latinx patients with a diagnosis of MM compared to Non-Hispanic White patients, yet Non-Hispanic Black patients have slightly better survival rates compared to Non-Hispanic White and Hispanic patients with MM. Most of the ongoing work to characterize and elucidate causes of these racial and ethnic disparities in MM etiology, outcomes, and treatment response compares Black and Non-Hispanic White individuals, with the Hispanic and Latinx population vastly understudied. Here, we provide an overview of the current state of knowledge on racial and ethnic differences in the epidemiology, biology, and clinical outcomes of patients with MM, with a special emphasis on Hispanic and Latinx individuals. We additionally discuss potential opportunities to consider for disparities research that includes Hispanic and Latinx patients with MM. Despite the encouraging progress in the study of health disparities in MM thus far, our review highlights the critical gaps that still remain to better understand MM in diverse populations and to develop more effective tools and approaches to provide all patients diagnosed with MM meaningful and transformative care.

Abstract

Anti-B-cell maturation antigen (BCMA) chimeric antigen receptor T-cell (CAR-T) therapy is transforming the care of patients with relapsed/refractory multiple myeloma (MM). Unfortunately, despite being a disease of older adults these patients remain under-represented in most pivotal clinical trials. We performed a systematic review and proportion meta-analysis of prospective clinical trials and observational studies of anti-BCMA CAR-T therapy in patients with MM with the aim to determine the efficacy and safety of this therapy in older adults (≥65 years). We searched the Pubmed, Scopus, Web of Science (WOS), Ovid, Embase, CENTRAL, and CINAHL databases through September 9, 2022 and abstracts from the American Society of Hematology (ASH) Annual Meeting 2022. Primary outcome measures included overall response rate (ORR), rates of cytokine release syndrome (CRS), and immune cell-effector-associated neurotoxicity syndrome (ICANS). study was registered with PROSPERO (study number: CRD42022334287). After screening 2218 references, 14 studies were included for data extraction, with a total of 558 patients, 26.2% (n = 146) of whom were older adults. The pooled ORR amongst this population was 93%, which was comparable to the ORR of 86% amongst younger patients. In older adults, the rates of CRS (any grade) and grade ≥ 3 were 95% and 21%, respectively. For younger patients, the pooled rate of CRS (any grade) and grade ≥ 3 CRS was 91% and 20%, respectively. The rate of ICANS (any grade) in older adults was 15%, which was higher than that observed in those <65 years. Older adults experience comparable outcomes to younger patients with anti-BCMA CAR-T therapy, albeit with numerically higher rates of neurotoxicity. [ABSTRACT FROM AUTHOR]

Abstract

Ciltacabtagene autoleucel (cilta-cel) is a chimeric antigen receptor-T cell (CAR-T) immunotherapy approved for patients (pts) with relapsed/refractory multiple myeloma (RRMM). In the phase 3 trial CARTITUDE-4 (NCT04181827), cilta-cel demonstrated improved efficacy vs standard of care (SOC; daratumumab plus pomalidomide and dexamethasone [DPd] or pomalidomide plus bortezomib and dexamethasone [PVd]) with an overall response rate (ORR) of 84.6% vs 67.3% and ≥complete response (CR) of 73.1% vs 21.8%. A cost per responder model was developed to assess the value of cilta-cel and SOC within the CARTITUDE-4 trial from a US mixed payer perspective incorporating efficacy and total treatment costs.

B cell maturation antigen (BCMA)-targeted chimeric antigen receptor (CAR) T-cell therapy including ciltacabtagene autoleucel (cilta-cel: CarvyktiTM) has revolutionized the landscape of multiple myeloma (MM) treatment. Various practice styles exist for CAR T delivery in part due to difference in healthcare infrastructure and payer models. We established a dedicated cellular therapy service, Immune Cell Therapy (ICE T), at our center where CAR T patients receive specialized and focused care throughout their treatment. We evaluated the single center experience of our outpatient cilta-cel delivery model.

Abstract

Tacrolimus (TAC)/methotrexate (MTX) is an important regimen for graft versus host disease (GVHD) prophylaxis. Standard MTX dosing is 15 mg/m2 on day +1 and 10 mg/m2 on days +3, +6 and +11 (Ratanatharathorn 1998). Dose modifications or omissions may be warranted due to HCT- or MTX-related toxicities. Reducing or omitting MTX has shown to negatively impact GVHD incidence and no standard exists (Martinez-Cibrian 2021; Bensinger 2006). At Moffitt Cancer Center, standard dosing for TAC/MTX GVHD prophylaxis consists of full dose MTX (above). In renally impaired recipients, empiric dose adjustments are applied based on creatinine clearance (CrCL). We aimed to validate this renal protocol and its impact on GVHD development, relapse, or death from any cause post allogenic (allo) hematopoietic cell transplantation (HCT).

Human herpesvirus 6 (HHV-6) frequently reactivates following allogeneic hematopoietic stem cell transplant (alloHCT) with spontaneous resolution in most patients. HHV-6 related encephalitis may occur; however, other clinical syndromes are less well defined. Previous antiviral pre-emptive and prophylactic studies have not demonstrated clinical efficacy in preventing HHV-6 related encephalitis or improving transplant related outcomes and is not routinely recommended by clinical guidelines. Less is known about the clinical impact of HHV-6 reactivation and antiviral treatment following haploidentical alloHCT.

While the pivotal studies exclusively treated patients in an inpatient setting, robust infrastructure planning at some centers across US has fostered outpatient administration of the commercially available product. We thus conducted a qualitative study to evaluate clinical and administrative perspectives and trends in outpatient administration of commercialized cilta-cel with an aim to understand best practices from certified CAR-T centers in the US.

•We explored the impact of ALC on the survival outcomes of myeloma patients treated with commercial idecabtagene vicleucel.•We created a new ALC profile and included 3 groups: Normal pre-lymphodepletion (LD) ALC, low pre-LD ALC + percent reduction (%R) from pre-apheresis to pre-LD of <37.5 and low pre-LD ALC + %R ≥37.5.•Low pre-LD ALC with high %R from pre-apheresis to pre-LD was associated with inferior survival outcomes.
