Long bone fractures are a common injury in the pediatric population. Differentiation between abusive, or non-accidental trauma, and accidental trauma in children remains challenging for forensic practitioners. A recent clinical-based study was able to separate pediatric abusive from accidental trauma based on femoral fracture pattern using the ratio of fracture length over bone diameter (fracture ratio), as determined from radiographic analysis of this fractured bone. The forensic literature indicates more cases of abuse in younger pediatric victims than accidental cases. While this was the case in the clinical study, the effect was not shown to be statistically significant. Furthermore, while speed of trauma was not considered in the clinical study, a laboratory study with an immature bovine model indicates rotational speed influences fracture pattern, but specimen age was not varied in that study. Therefore, the objective of the current study was to use immature porcine femora to investigate the effects of age and rate of twist on a modified version of this fracture ratio parameter. Fifteen pairs of porcine femora with various ages were twisted until observable failure using a custom-built torsional fixture. The left femur of each pair was twisted to failure at a rate of 3 deg/s, while the right femur was twisted at a rate of 90 deg/s. The torque and angle of rotation were recorded at a sampling rate of 10,000 Hz. Fracture ratio was defined as total fracture length divided by bone diameter. Fracture ratio increased with specimen age, with specimens under the low rate of twist yielding a consistently lower fracture ratio than those from specimens under the high rate of twist. The results showed that both specimen age and rate of twist were significant factors influencing fracture ratio. The determination of abusive from accidental trauma in criminal cases, based on the pattern of long bone fracture alone, may need to include additional data on the specific age of the pediatric victim and the potential speed of the traumatic event. Nonunion of long bones is a major complication following fractures that requires a substantial amount of investment both monetary and time from patients and surgeons. The need to comprehensively understand the complications, outcomes and distraction capability is profound, which necessitates a review of current evidence on special fixators used for distraction. The current systematic review aims to study the available literature on distraction osteogenesis using special fixators (Ilizarov and Limb Reconstruction System) in the management of nonunion (infected and non-infected). Cochrane Library, PubMed, Scopus were searched until January 2022. The review included all original studies which used Ilizarov or Monorail Fixators/LRS in the treatment of nonunion of long bones. The quality of studies was assessed using the Modified Coleman Methodology Score. A total of 35 original studies with Ilizarov (n = 29) and LRS (n = 8) were selected including two comparative studies. The pooled data meta-analysis and the subgroup analysis of these studies found both Ilizarov and LRS fixator yield similar functional outcomes for the treatment of long bones nonunion. The review was conducted to understand the scenario of nonunion in long bones. Pin tract infection is found to be the most common complication followed by adjacent joint stiffness and deformity. In our review we observed that both external fixator time and index were lower in LRS compared to Ilizarov group. Further RCT are required comparing Ilizarov and LRS fixators to effectively comment upon the superiority of the implants. [ABSTRACT FROM AUTHOR]

Determining the status of bone healing after osteotomy can be challenging and has implications ranging from clinical decision-making to standardization of research outcomes without the use of computed tomography. To date, no method has been validated for determining osseous healing of an osteotomy site of the long bones of the foot. The purpose of the present study was to develop a radiographic union scoring system that would enhance the diagnostic healing assessment. We adapted existing orthopedic scales that had been validated for healing in the leg for application in the long bones of the foot. One hundred cases were evaluated by 6 blinded assessors to test the inter- and intrarater reliability of the subjective healing assessment compared with the proposed scoring system. The radiographs were classified by postoperative period: <4, 5 to 12, and >12 weeks. The proposed scale had a high interrater reliability but was burdensome. Using a priori item reduction protocols, the scale was limited to the 5 items with the best internal consistency, which significantly reduced the burden. The result was excellent interrater reliability (a = 0.87) among all assessors compared with acceptable reliability (a = 0.66) for the subjective osteotomy healing assessment. The intrarater reliability during the subsequent retest phase demonstrated similar relationships, with low agreement (r = 0.38) for subjective healing. Each of the items included in the final scoring scale had moderate to good agreement across all assessors (r = 0.51 to 0.63). The reliability of this system appeared superior to the subjective assessment of osseous healing alone, even in the absence of clinical correlates after an osteotomy in the foot. Objective: To explore suitable scaffold material for big segmental long bone defect by studying the properties of the prepared deproteinized bone. Methods: Cancellated bone were made as 30 mm × 3 mm × 3 mm bone blocks from inferior extremity of pig femur along bone trabecula. The deproteinized bone was prepared with an improved method. Their morphological features, components, cell compatibility, mechanical and immunological properties were investigated respectively. Results: Deproteinized bone maintained natural reticular pore system. The main organic material is collagen I and inorganic composition is hydroxyapatite. It has good mechanical properties, cell adhesion rate and histocompatibility. Conlusion: This deproteinized bone can be applicable as scaffold for reparation of big segmental defect in long bone.
