Objective: To evaluate the association between provider religion and religiosity and consensus about end-of-life care and explore if geographical and institutional factors contribute to variability in practice.Methods: Using a modified Delphi method 22 end-of-life issues consisting of 35 definitions and 46 statements were evaluated in 32 countries in North America, South America, Eastern Europe, Western Europe, Asia, Australia and South Africa. A multidisciplinary, expert group from specialties treating patients at the end-of-life within each participating institution assessed the association between 7 key statements and geography, religion, religiosity and institutional factors likely influencing the development of consensus.Results: Of 3049 participants, 1366 (45%) responded. Mean age of respondents was 45 ± 9 years and 55% were females. Following 2 Delphi rounds, consensus was obtained for 77 (95%) of 81 definitions and statements. There was a significant difference in responses across geographical regions. South African and North American respondents were more likely to encourage patients to write advance directives. Fewer Eastern European and Asian respondents agreed with withdrawing life-sustaining treatments without consent of patients or surrogates. While respondent’s religion, years in practice or institution did not affect their agreement, religiosity, physician specialty and responsibility for end-of-life decisions did.Conclusions: Variability in agreement with key consensus statements about end-of-life care is related primarily to differences among providers, with provider-level variations related to differences in religiosity and specialty. Geography also plays a role in influencing some end-of-life practices. This information may help understanding ethical dilemmas and developing culturally sensitive end-of-life care strategies.

Caloric restriction and intermittent fasting prolong the lifespan and healthspan of model organisms and improve human health. The natural polyamine spermidine has been similarly linked to autophagy enhancement, geroprotection and reduced incidence of cardiovascular and neurodegenerative diseases across species borders. Here, we asked whether the cellular and physiological consequences of caloric restriction and fasting depend on polyamine metabolism. We report that spermidine levels increased upon distinct regimens of fasting or caloric restriction in yeast, flies, mice and human volunteers. Genetic or pharmacological blockade of endogenous spermidine synthesis reduced fasting-induced autophagy in yeast, nematodes and human cells. Furthermore, perturbing the polyamine pathway in vivo abrogated the lifespan- and healthspan-extending effects, as well as the cardioprotective and anti-arthritic consequences of fasting. Mechanistically, spermidine mediated these effects via autophagy induction and hypusination of the translation regulator eIF5A. In summary, the polyamine–hypusination axis emerges as a phylogenetically conserved metabolic control hub for fasting-mediated autophagy enhancement and longevity.

Die Zunahme der intensivmedizinischen Behandlungsverfahren ist mit einer Steigerung der Komplexität der Entscheidungen über Auswahl, Anwendung und Dauer der Therapiemaßnahmen verbunden. Ob im individuellen Fall ein Behandlungsziel erstrebenswert und der dafür notwendige Behandlungsweg akzeptabel ist, hängt von den persönlichen Wünschen, Werthaltungen und Lebensentwürfen der Patientinnen und Patienten ab. Häufig bestehen Unsicherheiten darüber, ob ein patientenzentriertes Behandlungsziel erreicht werden kann. Der Einsatz eines zeitlich begrenzten Therapieversuches (TLT) als eine verbindliche Übereinkunft zwischen dem Behandlungsteam und der Patientin/dem Patienten bzw. dem juristischen Stellvertretenden zu einem Behandlungskonzept über einen definierten Zeitraum auf der Intensivstation kann hilfreich sein, um Unsicherheiten zu reduzieren und die Fortführung intensivmedizinischer Maßnahmen im Sinne des/der Patient/in besser einordnen und rechtfertigen zu können.

Background: End-of-life care is common in German intensive care units (ICUs) but little is known about daily practice. Objectives: To study the practice of end-of-life care. Methods: Prospectively planned, secondary analysis comprising the German subset of the worldwide Ethicus‑2 Study (2015–2016) including consecutive ICU patients with limitation of life-sustaining therapy or who died. Results: Among 1092 (13.7%) of 7966 patients from 11 multidisciplinary ICUs, 967 (88.6%) had treatment limitations, 92 (8.4%) died with failed CPR, and 33 (3%) with brain death. Among patients with treatment limitations, 22.3% (216/967) patients were discharged alive from the ICU. More patients had treatments withdrawn than withheld (556 [57.5%] vs. 411 [42.5%], p< 0.001). Patients with treatment limitations were older (median 73 years [interquartile range (IQR) 61–80] vs. 68 years [IQR 54–77]) and more had mental decision-making capacity (12.9 vs. 0.8%), advance directives (28.6 vs. 11.2%), and information about treatment wishes (82.7 vs 33.3%, all p< 0.001). Physicians reported discussing treatment limitations with patients with mental decision-making capacity and families (91.3 and 82.6%, respectively). Patient wishes were unknown in 41.3% of patients. The major reason for decision-making was unresponsiveness to maximal therapy (34.6%). Conclusions: Treatment limitations are common, based on information about patients’ wishes and discussion between stakeholders, patients and families. However, our findings suggest that treatment preferences of nearly half the patients remain unknown which affects guidance for treatment decisions.

Hintergrund: Akupunktur hat sich bei saisonaler allergischer Rhinitis (SAR) als wirksam erwiesen, ist jedoch zeit- und kostenintensiv. Wir untersuchten die Durchführbarkeit und die Auswirkungen der Körperakupressur als Selbstbehandlungstechnik, bei der Akupunkturpunkte durch manuellen Druck bei SAR-Patient*innen stimuliert werden. Methoden: Wir führten eine zweiarmige, randomisierte, kontrollierte klinische Studie durch, um die Wirkung von selbst applizierter Akupressur bei SAR-Patient*innen über vier Wochen an fünf Akupunkturpunkten plus Bedarfsmedikation (RM) mit Cetirizin versus RM allein zu vergleichen. Neben anderen Zielparametern bewerteten wir die krankheitsbezogene Lebensqualität (Rhinitis Quality of Life Questionnaire [RQLQ]), die allgemeinen SAR-Symptome anhand einer visuellen Analogskala (VAS) und die Inanspruchnahme antiallergischer Medikation (Rescue-Medication-Score (RMS)) nach vier und acht Wochen. Ergebnisse: Einundvierzig SAR-Patient*innen (Durchschnittsalter 38,5 ± 10,0 Jahre, n= 21, 51,2 % Frauen) wurden randomisiert. Im Vergleich zu RM allein (n= 21) war Akupressur plus RM (n= 20) nach vier Wochen mit relevanten Verbesserungen verbunden, wie der Unterschied zwischen den Gruppen im adjustierten Mittelwert des RQLQ: −0,9 Punkte (95 % CI, −1,6 bis −0,2; p= 0,011) und der VAS-Gesamtsymptome der SAR zeigte: −21,6 mm (95 % CI, −36,3 bis −6,8; p= 0,005). Der RMS-Wert war in der Akupressurgruppe niedriger als in der Kontrollgruppe: 1,9 Punkte (95 % CI, −3,8 bis −0,1; p= 0,120). Die Gruppenunterschiede nahmen bis zur achten Woche leicht ab. Die Akupressur erwies sich als gut durchführbar und sicher. Schlussfolgerung: Die Ergebnisse dieser explorativen Studie deuten darauf hin, dass selbst angewendete Akupressur gut durchführbar ist, die krankheitsspezifische Lebensqualität verbessern und krankheitsbedingte Symptome sowie die Einnahme antiallergischer Medikamente bei SAR-Patient*innen verringern kann. Qualitativ hochwertige Studien, die auch eine Scheinkontrollgruppe umfassen, sind in Zukunft erforderlich, um diese Ergebnisse zu bestätigen.

Die Behandlungssituation in der Intensivmedizin ist durch eine starke Asymmetrie in der Begegnung gekennzeichnet: Patient:innen sind in besonderer Weise auf ihr Umfeld angewiesen, weisen häufig Beeinträchtigungen des Bewusstseins und der Willensbildung auf. Dies erleichtert die Anwendung von Zwang bzw. ermöglicht diesen und/oder ruft ihn hervor. Ziel dieser Handlungsempfehlung ist es, Wege aufzuzeigen, um Patient:innen mit ihren Wünschen und Bedürfnissen wahrzunehmen und sie in Behandlungskonzepte auf der Intensivstation einzubinden, um Zwang zu reduzieren und wann immer möglich zu vermeiden. Der Beitrag zeigt die Vielfalt möglicher Formen von Zwang auf und diskutiert die im ethischen Abwägungsprozess zu berücksichtigenden moralischen Maßstäben sowie rechtliche Bedingungen zur Rechtfertigung seiner Anwendung. Es wird deutlich, dass Behandlungsmaßnahmen, die die Anwendung von Zwang beinhalten können, immer einer sorgfältigen und auch selbstkritischen Überprüfung der Maßnahmen im Verhältnis zur Indikation und zum Therapieziel bedürfen. Die Handlungsempfehlung möchte daher nicht prinzipiell die Anwendung von Zwang durch die interprofessionellen Teams missbilligen, sondern sie soll vielmehr zu einer sensiblen Wahrnehmung von Zwang und zu einem kritischen und fürsorglichen Umgang mit formellem und insbesondere informellem (indirektem) Zwang beitragen.

In Notaufnahmen sind tagtäglich Entscheidungen mit enormer medizinischer und ethischer Tragweite erforderlich. Diese müssen trotz Zeitdruck und hoher Arbeitsbelastung in allen sie betreffenden Dimensionen fachkundig getroffen werden. Eine Strukturierung anhand standardisierter Vorgehensweisen kann bei der ethischen Ad-hoc-Entscheidungsfindung hilfreich sein, vorausgesetzt, dass sie interdisziplinär und interprofessionell geschult und (vor)gelebt werden. Die vorliegende Ad-hoc-Entscheidungshilfe empfiehlt zur Evaluierung der Therapieentscheidung ein „ethical team time out“, in dessen Rahmen Patientenwille und medizinische Indikation strukturiert überprüft werden. Weitere Expertise (im Idealfall im Sinne einer Ad-hoc-Ethikberatung) sollte hinzugezogen werden, wenn Maßnahmen medizinisch nur fraglich sinnvoll sind und/oder die Zustimmung des Patienten fraglich ist.

End-of-life practices vary among intensive care units (ICUs) worldwide. Differences can result in variable use of disproportionate or non-beneficial life-sustaining interventions across diverse world regions. This study investigated global disparities in end-of-life practices.

Ungeachtet der sozialgesetzlichen Vorgaben existieren im deutschen Gesundheitssystem in der Patientenversorgung nebeneinander Unter‑, Fehl- und Überversorgung. Überversorgung bezeichnet diagnostische und therapeutische Maßnahmen, die nicht angemessen sind, da sie die Lebensdauer oder Lebensqualität der Patienten nicht verbessern, mehr Schaden als Nutzen verursachen und/oder von den Patienten nicht gewollt werden. Daraus können hohe Belastungen für die Patienten, deren Familien, die Behandlungsteams und die Gesellschaft resultieren. Dieses Positionspapier erläutert Ursachen von Überversorgung in der Intensivmedizin und gibt differenzierte Empfehlungen zu ihrer Erkennung und Vermeidung. Zur Erkennung und Vermeidung von Überversorgung in der Intensivmedizin erfordert es Maßnahmen auf der Mikro‑, Meso- und Makroebene, insbesondere die folgenden: 1) regelmäßige Evaluierung des Therapieziels im Behandlungsteam unter Berücksichtigung des Patientenwillens und unter Begleitung von Patienten und Angehörigen; 2) Förderung einer patientenzentrierten Unternehmenskultur im Krankenhaus mit Vorrang einer qualitativ hochwertigen Patientenversorgung; 3) Minimierung von Fehlanreizen im Krankenhausfinanzierungssystem gestützt auf die notwendige Reformierung des fallpauschalbasierten Vergütungssystems; 4) Stärkung der interdisziplinären/interprofessionellen Zusammenarbeit in Aus‑, Fort- und Weiterbildung; 5) Initiierung und Begleitung eines gesellschaftlichen Diskurses zur Überversorgung.

AbstractIntroduction:Prehospital blood component therapy poses a possible treatment option among patients with severe bleeding. The aim of this paper was to characterize patients receiving prehospital blood component therapy by a paramedic-doctor-staffed, ground-based prehospital critical care (PHCC) service.Methods:Bleeding patients with a clinical need for prehospital blood transfusion were included prospectively. The following data were collected: indication for transfusion, mechanism of injury, vital parameters, units of red blood cells (RBCs)/plasma transfused, degree of shock, demographics, and mortality.Results:Twenty-one patients received blood products: 12 (57%) traumatic injuries and nine (43%) non-traumatic bleeds, with a median of 1.5 (range 1.0-2.0) units of RBCs and 1.0 (range 0.0-2.0) unit of plasma. The most frequent trigger to initiate transfusion was on-going excessive bleeding and hypotension. Improved systolic blood pressure (SBP) and milder degrees of shock were observed after transfusion. Mean time from initiation of transfusion to hospital arrival was 24 minutes. In-hospital, 11 patients (61%) received further transfusion and 13 (72%) had urgent surgery within 24 hours. Overall, 28-day mortality was 29% at 24-hours and 33% at 28-days.Conclusion:Prehospital blood component therapy is feasible in a ground-based prehospital service in a medium-sized Scandinavian city. Following transfusion, patient physiology and degree of shock were significantly improved.

In view of the globally evolving coronavirus disease (COVID-19) pandemic, German hospitals rapidly expanded their intensive care capacities. However, it is possible that even with an optimal use of the increased resources, these will not suffice for all patients in need. Therefore, recommendations for the allocation of intensive care resources in the context of the COVID-19 pandemic have been developed by a multidisciplinary group of authors with the support of eight scientific medical societies. The recommendations for procedures and criteria for prioritisations in case of resource scarcity are based on scientific evidence, ethicolegal considerations and practical experience. Medical decisions must always be based on the need and the treatment preferences of the individual patient. In addition to this patient-centred approach, prioritisations in case of resource scarcity require a supraindividual perspective. In such situations, prioritisations should be based on the criterion of clinical prospect of success in order to minimize the number of preventable deaths due to resource scarcity and to avoid discrimination based on age, disabilities or social factors. The assessment of the clinical prospect of success should take into account the severity of the current illness, severe comorbidities and the patient’s general health status prior to the current illness.

Angesichts der beginnenden COVID-19-Pandemie haben die deutschen Krankenhäuser ihre intensivmedizinischen Kapazitäten erhöht. Trotz optimaler Nutzung erscheint es möglich, dass die intensivmedizinischen Ressourcen nicht mehr für alle Patienten ausreichen, die ihrer bedürfen. Vor diesem Hintergrund wurden von einer multidisziplinären Autorengruppe und mit Unterstützung von acht medizinisch-wissenschaftlichen Fachgesellschaften Empfehlungen zur Verteilung intensivmedizinischer Ressourcen im Kontext der COVID-19-Pandemie erarbeitet. Die Empfehlungen zu Verfahren und Kriterien für Priorisierungsentscheidungen bei Ressourcenknappheit wurden auf Grundlage der verfügbaren wissenschaftlichen Evidenz, ethischer und rechtlicher Überlegungen sowie praktischer Erfahrungen erstellt. Medizinische Entscheidungen müssen sich immer am Bedarf und den Behandlungspräferenzen des einzelnen Patienten orientieren. Ergänzend zu dieser patientenzentrierten Betrachtung kommt mit der Priorisierung bei absoluter Mittelknappheit eine überindividuelle Perspektive hinzu. In dieser Situation sollte die Priorisierung anhand des Kriteriums der Erfolgsaussicht einer Behandlung erfolgen, um die Anzahl der vermeidbaren Todesfälle zu minimieren und eine Diskriminierung von Patienten aufgrund des Alters, von Behinderungen oder des sozialen Status zu vermeiden. Die Einschätzung der klinischen Erfolgsaussicht berücksichtigt den Schweregrad der aktuellen Erkrankung, das Vorliegen schwerer Komorbiditäten und den allgemeinen prämorbiden Gesundheitsstatus des Patienten.

Background/Objectives: Metabolism is controlled by macro- and micronutrients. Protein-rich diets should lead to latent acidosis at tissue level with further negative implications. Food supplements with alkaline salts are available and popular pretending to prevent these changes. Subjects/Methods: Within a randomised double-blind placebo-controlled trial we tested the hypotheses that (1) a 4-week protein-rich diet induces a latent tissue acidosis and (2) an alkaline supplement can compensate this. Acid–base balance and important metabolic parameters were determined before and after 4 weeks of supplementation by peripheral blood samples, indirect calorimetry and muscle microdialysis before and after a protein-rich test meal. Results: Fourty volunteers were randomised 1:1 to either verum or placebo supplements. Protein-rich diet by itself did not significantly affect acid–base balance. Alkaline supplementation increased plasma bicarbonate concentration without changing pH. Postprandial increases in serum glucose and insulin tended to be lower for verum vs. placebo. Resting and postprandial energy metabolism, and carbohydrate and fat oxidation did not differ significantly before and after supplementation in both groups. In muscle, postprandial glucose uptake and aerobic glucose oxidation were significantly higher for verum. In addition, verum significantly increased serum magnesium concentrations. Conclusions: Four weeks of protein-rich diet did not significantly influence acid–base balance. However, alkaline supplementation improved systemic and tissue acid–base parameters and oxidative glucose metabolism.

Background/aim: Medically supervised fasting has long tradition and broad acceptance in some European countries. The exact amount to which ketone bodies are produced as well as their possible contribution to beneficial effects in this procedure are open. Methods: Open-label observational trial with in-patients undergoing medically supervised fasting with supplementation of approximately 40 g/d carbohydrates as part of an established routine care. Daily finger-stick blood samples for measurement of glucose and ß-hydroxybutyrate. Descriptive analysis for all data. Results: Complete sets of data of 17 patients (5 m, mean age 63.1) could be evaluated. Mean concentrations of ß-hydroxybutyrate rose continuously to a mean maximum of 3.6 mmol/L with an individual maximum of 5.1 mmol/L, while glucose decreased within normal range. Two patients with type 2 diabetes produced significantly less ß-hydroxybutyrate. Courses for ß-hydroxybutyrate and glucose showed a weak inverse correlation, while no serious adverse effects could be observed. Conclusion: Medically supervised fasting with definite small amounts of carbohydrates by fluid intake may lead to limited increases of ketone bodies in their biopositive range, as known e.g. from epileptology or sports medicine. Clinical consequences deserve further research.

Verletzungen, insbesondere Schädel-Hirn-Traumata, oder bestimmte Erkrankungen und ihre jeweiligen Folgen können zur Folge haben, dass die davon betroffenen Patienten trotz aller intensivmedizinischen Bemühungen daran versterben. Sollte eine Organspende nach deren Tod prinzipiell in Betracht kommen, sind regelhaft intensivmedizinische Maßnahmen zur Aufrechterhaltung der Organe erforderlich, die aber nicht mehr dem betreffenden Patienten zunutze sein können. Dies kann zu einem ethischen Zielkonflikt zwischen einer würdevollen Begleitung des Patienten am Lebensende und einem erweiterten Behandlungsbedarf zur bestmöglichen Organprotektion führen – besonders dann, wenn der Patient vorab eine Therapiebegrenzung bestimmt hat. Die Sektion Ethik und die Sektion Organspende und -transplantation der Deutschen Interdisziplinären Vereinigung für Intensiv- und Notfallmedizin (DIVI) haben in mehreren Sitzungen und einer Telefonkonferenz eine Entscheidungshilfe erarbeitet, die den Entscheidungsprozess hinsichtlich des Behandlungsumfangs bei potenziellen Organspendern zunächst auf die Bewertung 5 einzelner Dimensionen fokussiert, nämlich die Gewissheit des irreversiblen Hirnfunktionsausfalls, den Organspendewunsch des Patienten, seinen Willen zur Therapiebegrenzung, die Eingriffsintensität der zur Organprotektion erforderlichen erweiterten Behandlungsmaßnahmen und die Wahrscheinlichkeit einer erfolgreichen Organprotektion. Aus der Zusammenschau der Einzelbewertungen, die anhand eines dafür entwickelten Netzdiagramms verdeutlicht werden kann, lässt sich bei aller Komplexität der Fragestellung doch erkennen, ob eine Fortführung oder ggf. Erweiterung der intensivmedizinischen Maßnahmen ethisch gerechtfertigt, fragwürdig oder gar ausgeschlossen erscheint. Die beschriebene Vorgehensweise kann ethische Zielkonflikte hinsichtlich des Therapieumfangs bei dem todgeweihten Patienten entschärfen helfen, bei denen eine Organspende medizinisch prinzipiell möglich erscheint. Injuries, especially traumatic brain injury, or specific illnesses and their respective sequelae can result in the demise of the patients afflicted despite all efforts of modern intensive care medicine. If in principle organ donation is an option after a patient’s death, intensive therapeutic measures are regularly required in order to maintain the homeostasis of the organs. These measures, however, cannot benefit the patient afflicted anymore—which in turn might lead to an ethical conflict between dignified palliative care for him/her and expanded intensive treatment to facilitate organ donation for others, especially if the patient has opted for the limitation of life-sustaining therapies in an advance directive. The Ethics Section and the Organ Donation and Transplantation Section of the German Interdisciplinary Association of Critical Care and Emergency Medicine (DIVI) have convened several meetings and a telephone conference and have arrived at a decision-making aid as to the extent of treatment for potential organ donors. This instrument focusses first on the assessment of five individual dimensions regarding organ donation, namely the certitude of a complete and irreversible loss of all brain function, the patient’s wishes as to organ donation, his or her wishes as to limiting life-sustaining therapies, the intensity of expanded intensive treatment for organ protection and the odds of its successful attainment. Then, the combination of the individual assessments, as graphically shown in a {Netzdiagramm}, will allow for a judgement as to whether a continuation or possibly an expansion of intensive care measures is ethically justified, questionable or even inappropriate. The aid described can help mitigate ethical conflicts as to the extent of intensive care treatment for moribund patients, when organ donation is a medically sound option. Background: Treatment of female infertility has been growing globally in recent years. In spite of improvements in medical strategies and the improved outcomes for infertile couples, treatment attempts remain largely unsuccessful. A growing number of patients pursue complementary and alternative medicine treatment options like Ayurveda that offers a variety of inpatient and outpatient treatments for infertility. Case Report: A case of a 38 year-old woman with infertility of unknown origin is presented. She received 18 conventional fertility treatments in 5 different fertility centers and 3 different countries. After several complications, the patient quitted conventional treatment and admitted to an Ayurvedic outpatient clinic where she received a complex Ayurvedic treatment, which included botanicals, dietary and lifestyle advice, manual therapy, yoga, and spiritual elements. The patient then became pregnant and gave birth to a healthy boy in 2012. Conclusions: Ayurveda may be a useful complementary option in the case of futile conventional treatment attempts in female infertility. Nevertheless, the evidence base for Ayurvedic interventions remains weak and requires well-designed clinical trials. This case raises some questions, such as whether the exposure to a large number of assisted reproduction procedures can lead to more health problems than health benefits. The Ayurvedic approach to fertility strives first to improve the health of the patient leading to a higher likelihood of pregnancy. As this is a case report, we are not able to exclude temporal factors stimulating the pregnancy. However, the chronology suggests that this approach might have been an important factor in the eventual pregnancy. © 2015 S. Karger GmbH, Freiburg

The aim of this cross-sectional analysis was to identify sociodemographic, and health related predictors for fasting use among patients of a large integrative internal medicine ward.

The objective of this study was to assess the effectiveness of a specific composition of a traditional herbal preparation (DurrDerma) in adult patients with moderate to severe skin psoriasis. The preparation is a newly developed topical combination containing plant-based extracts traditionally used in skin disease as black cumin, olive oil, tea tree oil, cocoa butter completed by vitamin A and vitamin B12. We documented the effectiveness of the preparation in a first case series. A total of 12 patients (8 males and 4 females, 21–86 y) with manifest and treatment-resistant psoriasis were included and treated for 12 weeks. All patients were assigned to twice-daily treatment with the DurrDerma preparation. Treatment success as determined by the Psoriasis Area and Severity Index (PASI) score, the body surface area, and the dermatology life index was achieved (PASI reduction of >75%) in 10 of the 12 treated patients (83%). The remaining two patients showed a PASI reduction of ≤50%. In 5 of the patients PASI reduction was achieved <12 weeks (between week 3–11). The beneficial effect in responder patients might be explained by a synergistic anti-oxidative and anti inflammatory activity of all components present in DurrDerma. We conclude that the new preparation using a traditional approach seems to be a promising complementary treatment for psoriasis.

End-of-life care in the intensive care unit (ICU) was identified as an objective in a series of Task Forces developed by the World Federation of Societies of Intensive and Critical Care Medicine Council in 2014.

Dietary interventions have not been effective in the treatment of multiple sclerosis (MS). Here, we show that periodic 3-day cycles of a fasting mimicking diet (FMD) are effective in ameliorating demyelination and symptoms in a murine experimental autoimmune encephalomyelitis (EAE) model. The FMD reduced clinical severity in all mice and completely reversed symptoms in 20% of animals. These improvements were associated with increased corticosterone levels and regulatory T (Treg) cell numbers and reduced levels of pro-inflammatory cytokines, TH1 and TH17 cells, and antigen-presenting cells (APCs). Moreover, the FMD promoted oligodendrocyte precursor cell regeneration and remyelination in axons in both EAE and cuprizone MS models, supporting its effects on both suppression of autoimmunity and remyelination. We also report preliminary data suggesting that an FMD or a chronic ketogenic diet are safe, feasible, and potentially effective in the treatment of relapsing-remitting multiple sclerosis (RRMS) patients (NCT01538355).

Die Sektion Ethik der Deutschen Interdisziplinären Vereinigung für Intensiv- und Notfallmedizin (DIVI) veröffentlichte vor kurzem einen Dokumentationsbogen „Therapiebegrenzung“, der die Entscheidungen abbildet, welche medizinischen Maßnahmen unterlassen werden sollen, weil sie nicht (mehr) indiziert sind und/oder vom Patienten nicht gewünscht werden. Der individuelle Fall eines Organspendewunsches wurde hierbei nicht explizit berücksichtig. Daher wurde von der Sektion Ethik und der Sektion Organspende und Organtransplantation der DIVI unter Mitarbeit der Sektion Ethik der Deutschen Gesellschaft für Internistische Intensivmedizin und Notfallmedizin eine Ergänzung vorgenommen. In der Fußnote wurde für den Fall eines Organspendewunsches ein entsprechender Hinweis in der aktuellen Version der „Dokumentation Therapiebegrenzung“ aufgenommen. The Ethics Section of the German Interdisciplinary Association of Critical Care and Emergency Medicine (DIVI) recently published a documentation for decisions to withhold or withdraw life-sustaining therapies. The wish to donate organs was not considered explicitly. Therefore the Ethics Section and the Organ Donation and Transplantation Section of the DIVI together with the Ethics Section of the German Society of Medical Intensive Care Medicine and Emergency Medicine worked out a supplementary footnote for the documentation form to address the individual case of a patient’s wish to donate organs.

Chlorinated volatile organic compound (cVOC) degradation rate constants are crucial information for site management. Conventional approaches generate rate estimates from the monitoring and modeling of cVOC concentrations. This requires time series data collected along the flow path of the plume. The estimates of rate constants are often plagued by confounding issues, making predictions cumbersome and unreliable. Laboratory data suggest that targeted quantitative analysis of Dehalococcoides mccartyi(Dhc) biomarker genes (qPCR) and proteins (qProt) can be directly correlated with reductive dechlorination activity. To assess the potential of qPCR and qProt measurements to predict rates, we collected data from cVOC-contaminated aquifers. At the benchmark study site, the rate constant for degradation of cis-dichloroethane (cDCE) extracted from monitoring data was 11.0 ± 3.4 yr–1, and the rate constant predicted from the abundance of TceA peptides was 6.9 yr–1. The rate constant for degradation of vinyl chloride (VC) from monitoring data was 8.4 ± 5.7 yr–1, and the rate constant predicted from the abundance of TceA peptides was 5.2 yr–1. At the other study sites, the rate constants for cDCE degradation predicted from qPCR and qProt measurements agreed within a factor of 4. Under the right circumstances, qPCR and qProt measurements can be useful to rapidly predict rates of cDCE and VC biodegradation, providing a major advance in effective site management.
