Background: The development and potential of artificial intelligence (AI) is remarkable. Its application in all medical disciplines, including rheumatology, is attracting attention. To what extent AI is already used in clinical routine in rheumatology is unknown. In addition, the perceived barriers, potentials, and expectations regarding AI by rheumatologists have not yet been studied.Objectives: To examine the current usage and perceived barriers and facilitators of AI, including large language models (LLMs), among rheumatologists.Design: National, observational, non-interventional, and cross-sectional web-based study.Methods: A web-based survey was developed by the Working Group Young Rheumatology (AGJR) of the German Society for Rheumatology. The survey was distributed at the Congress of the German Society for Rheumatology and via social media, QR code, and email from August 30 until November 4, 2023.Results: Responses from 172 rheumatologists (55% female; mean age 43?years) were analyzed. The majority stated that they did not previously use AI (73%) in their daily practice. Eighty-eight percent of rheumatologists rated their AI knowledge as low to intermediate and 84% would welcome dedicated training on LLMs. The majority of rheumatologists anticipated AI implementation to improve patient care (60%) and reduce daily workload (62%). Especially for diagnosis (73%), writing medical reports (70%), and data analysis (70%), rheumatologists reported a potential positive benefit of AI. Main AI concerns addressed the responsibility for medical decisions (64%) and data security (58%).Conclusion: Overall, the results indicate that rheumatologists currently have little AI knowledge and make very little use of AI in clinical routine. However, the majority of rheumatologists anticipate positive AI effects and would welcome increased AI implementation and dedicated training programs.

Einleitung: Um die Telemedizin erfolgreich in den rheumatologischen Alltag zu integrieren, sind sowohl die Patienten- als auch Arztperspektive wichtig. Hierzu wurde eine ausführliche Studie mittels webbasierter Umfrage durch die Arbeitsgemeinschaft Junge Rheumatologie der Deutschen Gesellschaft für Rheumatologie (DGRh) und die Deutsche Rheuma-Liga Bundesverband e. V. durchgeführt. Mittels Subgruppenanalyse der Daten im Hinblick auf die Videosprechstunde galt es nun herauszufinden, welche Anforderungen und Wünsche unter den Patienten/innen und Ärzte/innen an die Videosprechstunde gestellt wurden. Methoden: Die prospektive Umfrage wurde über die sozialen Medien, QR-Code und E‑Mail verteilt. Es wurden deskriptive Statistiken und Regressionsanalysen in Bezug auf die Videosprechstunde durchgeführt und Korrelationen aufgezeigt. Ergebnisse: Die Daten zeigen eine positive Einstellung zur Videosprechstunde sowohl aufseiten der Patienten/innen (n= 299) als auch der Rheumatologen/innen (n= 129) auf. Eine Korrelation zwischen Alter und positiver Meinung zur Videosprechstunde zeigte sich unter den Patienten/innen (r = 0,161, p= 0,006), insbesondere bei den weiblichen Patienten fand sich mit zunehmendem Alter ein positiver Zuspruch für die Videosprechstunde (r = 0,244, p< 0,001 zu männlichen Patienten: r = −0,190, p= 0,145). Bezüglich der Fahrtzeit bis zum behandelnden Rheumatologen tendierten die männlichen Patienten mit zunehmender Fahrzeit zur Videosprechstunde (r = 0,229, p= 0,078), dies war jedoch nur schwach positiv. Hinsichtlich der Wünsche von Patienten/innen und Ärzten/innen sollte die Videosprechstunde v. a. für Folge- oder Notfalltermine genutzt werden. Eine Videosprechstunde für die Erstvorstellungen war hingegen nur sehr selten gewünscht. Schlussfolgerung: Während der COVID-19-Pandemie hat die Videosprechstunde zunehmenden Zuspruch unter den rheumatologischen Patienten/innen als auch unter den behandelnden Rheumatologen/innen erhalten.

In der Therapie der rheumatoiden Arthritis sind seit ca. 20 Jahren Biologika ein fester Bestandteil. Da Arzneimittelpatente in der Regel nach 10 Jahren auslaufen, wurden in den letzten Jahren Biosimilars auf den Markt gebracht. In vielen Studien konnte gezeigt werden, dass sie bei vergleichbarer Sicherheit und Effektivität eine gleichwertige Alternative zum Referenzarzneimittel darstellen. In manchen Fällen zeigten sich sogar geringere Raten an unerwünschten Arzneimittelwirkungen im Vergleich zu den Referenzarzneimitteln. Weiterhin können durch Biosimilars erhebliche Kosten eingespart werden, die sich allein in Deutschland auf einen jährlichen dreistelligen Millionenbetrag belaufen. Dabei lassen sich große regionale Unterschiede bei der Verschreibungshäufigkeit von Biosimilars in Deutschland ausmachen, die sich auch im Einsparungspotenzial widerspiegeln. Eine Umstellung auf ein Biosimilar ist unter Einbezug des Patienten gut möglich und auch erwünscht. In diesem Sinne ist auch die Erstverschreibung eines Biosimilars statt des Referenzarzneimittels zu befürworten.

Background: Timely diagnosis and treatment are essential in the effective management of inflammatory rheumatic diseases (IRDs). Symptom checkers (SCs) promise to accelerate diagnosis, reduce misdiagnoses, and guide patients more effectively through the health care system. Although SCs are increasingly used, there exists little supporting evidence. Objective: To assess the diagnostic accuracy, patient-perceived usability, and acceptance of two SCs: (1) Ada and (2) Rheport. Methods: Patients newly presenting to a German secondary rheumatology outpatient clinic were randomly assigned in a 1:1 ratio to complete Ada or Rheport and consecutively the respective other SCs in a prospective non-blinded controlled randomized crossover trial. The primary outcome was the accuracy of the SCs regarding the diagnosis of an IRD compared to the physicians’ diagnosis as the gold standard. The secondary outcomes were patient-perceived usability, acceptance, and time to complete the SC. Results: In this interim analysis, the first 164 patients who completed the study were analyzed. 32.9% (54/164) of the study subjects were diagnosed with an IRD. Rheport showed a sensitivity of 53.7% and a specificity of 51.8% for IRDs. Ada’s top 1 (D1) and top 5 disease suggestions (D5) showed a sensitivity of 42.6% and 53.7% and a specificity of 63.6% and 54.5% concerning IRDs, respectively. The correct diagnosis of the IRD patients was within the Ada D1 and D5 suggestions in 16.7% (9/54) and 25.9% (14/54), respectively. The median System Usability Scale (SUS) score of Ada and Rheport was 75.0/100 and 77.5/100, respectively. The median completion time for both Ada and Rheport was 7.0 and 8.5 min, respectively. Sixty-four percent and 67.1% would recommend using Ada and Rheport to friends and other patients, respectively. Conclusions: While SCs are well accepted among patients, their diagnostic accuracy is limited to date. Trial registration: DRKS.de, DRKS00017642. Registered on 23 July 2019
