A road visibility information system (RVIS) was developed that uses images transmitted by multiple closed-circuit television (CCTV) cameras as road visibility information. Feasibility studies in the daytime during the winters of 2005 to 2006 and 2006 to 2007 were performed on national routes 230 and 231. During those two winters, the RVIS calculated visibility scale and road visibility index (RVI) automatically, accurately, continuously, and in a timely manner from road images recorded by multiple CCTV cameras. The RVIS processed 90% of the images transmitted by 14 CCTV cameras. The RVIS provided areawide visibility information in many formats and through many kinds of media and devices. On the basis of stored road visibility data, the finding was a high correspondence between the RVI and the subjective visibility evaluation for the same road images. In addition, this study demonstrates that the RVI monitoring website and e-mail alerts can speedily and reliably inform road administrators of snowstorm-induced poor visibility. The RVIS has the potential to collect data on road visibility reliably during daytime using images transmitted by multiple CCTV cameras. However, it is necessary to improve data collection at night and to capture still images accurately and quickly.

This study was undertaken to identify characteristics of residents who left their training program before the end of the program. A survey was sent to 248 Program Directors in the United States, after institutional review. Anonymous responses were received from 27 (11%) programs. Data was received on 166 residents, including 111 males and 55 females. The group included 146 categorical residents and 20 preliminary residents. Of these, 60 residents left in the middle of the year and 105 left at the end of the year. Of the 164 residents for whom data was reported, 30 (21%) used counseling services. Of 110 residents who left before 2004, 54 (49%) left to choose another specialty. In 2004, of 25 residents who left, 13 (52%, P> 0.05) pursued training in another specialty, and in 2005 of 31 residents who left, 23 (74%, P< 0.025) chose another specialty. Significantly more residents who started a career in surgery after 2004 left to train in another specialty. This may be due to implementation of work hour restrictions in 2003 leading residents to enter surgery who would not have done so previously. Strategies for better retention of matched residents are necessary to reverse this worrisome trend.

However, when road condition information is given as an image, the provision media are limited. Japan's Hokkaido Regional Development Bureau has sought ways to quantify road image information. If it can be proved that the estimation of road visibility from the same image does not differ among people and if drivers' assessments of visibility correlate with assessments given by a road image-based visibility scale (VS), then the VS can be used to quantify road visibility. VS is calculated from a weighted intensity of power spectrum developed by Hagiwara and colleagues. Subjective assessments of road images taken by CCTV cameras were surveyed. Assessments of daytime images also had little variation among subjects and correlated closely with the VS; assessments of nighttime images had little variation among subjects but did not correlate closely with the VS. To determine the best way to provide VS information, subjects' understanding of information given was surveyed in response to three provision media (text, numerical rank, and symbol); the differences were small.

This study proposes a method for identifying poor visibility under adverse weather conditions by processing closed-circuit television (CCTV) digital images. The weighted intensity of the power spectrum (WIPS) was examined to determine its applicability as a value for the identification of poor visibility. The magnitude of WIPS represents the difference in spatial frequencies within the image on the basis of the human contrast sensitivity function. WIPS was calculated by the following image-processing procedure: the spatial frequency of the cutout image was calculated with a two-dimensional Fourier transform, and the power spectrum of the cutout image was calculated; WIPS was totaled at spatial frequencies that ranged from 1.5 to 18 cycles per degree. Two kinds of experiments were performed to determine whether WIPS represented the subjective visibility assessment values (SVAVs) given by the test subjects. Clear linear relationships between WIPS and SVAVs were found in both experiments. In addition, the two correlation lines overlapped within the whole range of WIPS. These results suggest that WIPS may be appropriate for identifying poor visibility by the use of digital images.

A mouse HER2-derived peptide, HER2p63 (A) (TYLPANASL), can induce K^d-restricted mouse cytotoxic T lymphocytes (CTL) and also function as a tumor rejection antigen in an in vivo assay. Since the anchor motif of mouse K^d for peptide binding has much similarity to that of human HLA-A2402, we asked if human HER2p63 (T) (TYLPTNASL) could induce HER2-specific CTL in HLA-A2402-positive individuals. Peripheral blood mononuclear cells (PBMC) of HLA-A2402-positive individuals were sensitized in vitro with HER2p63-pulsed autologous dendritic cells prepared from PBMC. CTL clone derived from these specifically lysed HER2-expressing cell lines bearing HLA-A2402. Cytotoxic activity of the CTL clone against the HER2-expressing cell line bearing HLA-A2402 was blocked by antibodies against CD3, CD8, HLA-A24 or MHC class I, and was also inhibited by the addition of excess HER2p63-pulsed C1R bearing HLA-A2402. Killer cells were generated from PBMC of seven healthy individuals and five ovarian cancer patients, all of HLA-A2402 type, by in vitro sensitization with HER2p63-pulsed autologous antigen presenting cells. These killer cells selectively lysed HER2-expressing SKOV3 transfected with HLA-A2402 cDNA, indicating high immunogenicity of HER2p63 in all 12 individuals examined.

We have identified an H-2K^d-binding peptide, HER2p780 (PYVSRLLGI), derived from murine HER2/neu (HER2), that can induce HER2-specific murine cytotoxic T lymphocytes (CTL). Weekly vaccination of BALB/c mice by syngeneic dendritic cells pulsed with HER2p780 peptide, entirely common to murine and human HER2, suppressed growth of pretransplanted HER2-expressing syngeneic tumors. A HER2-expressing human cancer cell line SKOV3 transfected with murine H-2K^d cDNA could also be lysed by HER2p780-specific murine CTLs, indicating that human HER2-expressing cancer cells can process and present the cognate peptide in the context of H-2K^d. Since H-2K^d and HLA-A2402 molecules have similar anchor motifs, the possibility of inducing HER2-specific CTL activity with HER2p780 in HLA-A2402 individuals was examined. CD8⁺ CTL clones specific for HER2-expressing cancer cell lines were established from peripheral blood lymphocytes with HLA-A2402 by repeatedly sensitizing with peptide-pulsed autologous dendritic cells as well as peripheral blood mononuclear cells. Detailed analysis of their specificity revealed that the cytotoxicity of CTL clones is specific for the cognate peptide with HLA-A2402 restriction. The results suggest that HER2p780 is a unique peptide that may function as a tumor rejection antigen peptide in HLA-A2402 individuals, as it was directly proven here to function in a murine tumor system. NY-ESO-1 is a member of the cancer-testis family of tumor antigens that elicits strong humoral and cellular immune responses in patients with NY-ESO-1–expressing cancers. Since CD4+ T lymphocytes play a critical role in generating antigen-specific cytotoxic T lymphocyte and antibody responses, we searched for NY-ESO-1 epitopes presented by histocompatibility leukocyte antigen (HLA) class II molecules. Autologous monocyte-derived dendritic cells of cancer patients were incubated with recombinant NY-ESO-1 protein and used in enzyme-linked immunospot (ELISPOT) assays to detect NY-ESO-1–specific CD4+ T lymphocyte responses. To identify possible epitopes presented by distinct HLA class II alleles, overlapping 18-mer peptides derived from NY-ESO-1 were synthetized and tested for recognition by CD4+ T lymphocytes in autologous settings. We identified three NY-ESO-1–derived peptides presented by DRB4*0101–0103 and recognized by CD4+ T lymphocytes of two melanoma patients sharing these HLA class II alleles. Specificity of recognition was confirmed by proliferation assays. The characterization of HLA class II–restricted epitopes will be useful for the assessment of spontaneous and vaccine-induced immune responses of cancer patients against defined tumor antigens. Further, the therapeutic efficacy of active immunization using antigenic HLA class I–restricted peptides may be improved by adding HLA class II–presented epitopes.

The myocardial clearance rate of C-11 palmitate as an index of fatty acid oxidation was assessed by means of position emission tomography (PET) at rest and during dobutamine infusion in seven normal subjects and 10 patients with coronary artery disease. In the normal subjects the clearance half time was homogeneous in the left ventricle at rest and uniformly shortened during dobutamine infusion. In the myocardium at risk, clearance half time tends to be longer in the segments with an abnormal Q wave on ECG, exhibiting regional wall motion abnormality, and supplied by severely stenosed coronary arteries, particularly during dobutamine infusion. These data indicate that fatty acid oxidation may be decreased in infarcted myocardium and associated with regional asynergy. Such an abnormality was most striking in those with severe coronary stenosis during dobutamine infusion. We conclude that PET with the use of C-11 palmitate at rest and during dobutamine is a useful means of identifying impaired fatty acid oxidation and decreased metabolic reserve in patients with coronary artery disease.

Imatinibmesylate (imatinib) is a small molecule tyrosine kinase inhibitor administered to patients with chronic myelogenous leukemia and gastrointestinal stromal tumor. Although imatinib-associated interstitial lung disease is uncommon, a few cases have been reported so far. However, in all these cases interstitial lung disease developed during the use of imatinib. The present case is the first report of imatinib-induced interstitial lung disease developing after discontinuation of the drug.

An optical method using the Kerr effect is useful to measure electric fields in dielectric liquids without disturbance. The computed tomography (CT) method has been applied to the measurement of two-dimensional field distributions by us. However, three-dimensional electric field distributions, in which field vectors are not always perpendicular to the transmitted light, have not been measured as yet. A three-dimensional field vector can be reduced to a two-dimensional one. This leads to the application of the CT method to the measurement of a more general distribution. This paper reports on a theory and a simulation of nonuniform three-dimensional electric field measurements.
