Background: Nursing care delivery models influence nursing workload. Problem: Poorly designed nursing workforce models may lead to lack of satisfaction in practice, inability to recruit staff, and nursing turnover. Approach: This paper describes a process that our team undertook to evaluate one nursing unit's nursing care delivery model and provides a recommendation for a nursing workforce model that ensures practice to top of scope while maintaining a balanced workload. Team brainstorming and affinity diagramming tools were used. Outcomes: The team findings resulted in a recommendation for an innovative care-based nursing model pilot. Patients in the intensive care unit (ICU) increasingly are expected to eventually return home after acute hospital care. Yet transitional care for ICU patients and their families is often delayed until the patient is about to be transferred to another location or level of care. Transitions theory is a middle-range nursing theory that aims to provide guidance for safe and effective nursing care and research while an individual experiences a transition. Intensive care unit nurses are well positioned to provide ICU transitional care planning early. This article applies the transitions theory as a theoretical model to guide the study of the transition to home after acute hospital care for ICU patients and their families. This theory application can help ICU nurses provide holistic patient- and family-centered transitional care to achieve optimal outcomes by addressing the predischarge and postdischarge needs of patients and families. Before Magnet designation, nurse scientists functioned primarily in academia. The Magnet model's emphasis on new knowledge required that health care organizations demonstrate knowledge generation to achieve and sustain designation. The nurse scientist role definition and function within health care organizations continues to evolve, which contributes to a lack of clarity about who and how nurses generate new knowledge. The purpose of this scoping review was to (1) identify nurse scientist role components in the context of 2 theoretical models (Thompson's Knowledge Brokering Model and Edward's Research Appreciation, Accessibility, and Application Model), (2) explore the strengths and barriers associated with existing nurse scientist practice models in US health care organizations, and (3) describe a unique, expanded practice model applied within Stanford Health Care's Office of Research and its implications for building new knowledge and innovation capacity with recommendations for ongoing role development. Study Aim: To investigate whether use of admission lanyards improves nurse confidence, care coordination, and infant health outcomes during neonatal emergency admissions. Methods: Admission lanyards that defined team roles, tasks, and responsibilities were evaluated in a mixed-methods, historically controlled, and nonrandomized intervention study. Methods included (i) 81 pre- and postintervention surveys to explore nurse confidence, (ii) 8 postintervention semistructured interviews to elicit nurse perceptions of care coordination and nurse confidence, and (iii) a quantitative comparison of infant care coordination and health outcomes for 71 infant admissions before and 72 during the intervention. Results: Nurse participants reported that using lanyards during neonatal admissions improved clarity of roles and responsibilities, communication, and task delegation, contributing to better admission flow, team leadership, accountability, and improved nurse confidence. Care coordination outcomes showed significantly improved time to stabilization for intervention infants. Radiographies for line placement were performed 14.4 minutes faster, and infants commenced intravenous nutrition 27.7 minutes faster from time of admission. Infant health outcomes remained similar between groups. Conclusion: Admission lanyards were associated with improved nurse confidence and care coordination during neonatal emergency admissions, significantly reducing time to stabilization for infants, shifting outcomes closer to the Golden Hour. • For the 75 items for which there is consensus on the role of bedside nurses ("nurses" below) in antimicrobial stewardship (AS), between 18.8% and 98.8% of infection control nurses (ICNs) in Japan responded that this was the role of nurses, with implementation rates ranging from 1.3%–92.8%. • We proposed adding 5 types of care for preventing and resolving infections as roles for nurses. These 5 types of care have not been indicated as AS roles for nurses in guidelines or previous studies. • Between 89.5% and 98.0% of the ICNs in Japan who responded to the survey indicated that these 5 types of care are roles for nurses in AS. • The rates of implementation of these 5 types of care ranged from 67.5%-93.0%. • An understanding by AS team members that nurses contribute to AS will lead to improved team performance. In the United States and the United Kingdom, the roles of nurses in antimicrobial stewardship (AS) have been described in guidelines. However, in Japan, no previous studies have clarified nurses' recognition of the role of AS. Moreover, how the AS roles were implemented among nurses in Japan has not been fully clarified. The objectives of this study were to determine the perceptions of infection control nurses (ICNs) in Japan regarding the AS role of nurses and the extent of nurses' practice. A questionnaire survey of ICNs was conducted. Four hundred responses (response rate, 30.8%) were analyzed. Some of the items that have already gained consensus as the AS role of nurses were not recognized as the AS role of nurses by ICNs or had low implementation rates in Japan. Meanwhile, both recognition and implementation rates were high for the 5 types of care proposed. The reason the ICNs agreed that these 5 types of care are AS roles for nurses is that they know that such care can prevent infection and thereby obviate the need for antimicrobial administration. However, whether nurses themselves understand that these are roles for nurses in AS is unclear. Objective: to know and understand the perceptions and meanings attributed to the biographical and relational identity of being a nurse by the social group that acted as director of the Department and Division of Nursing of the University Hospital of the University of São Paulo. Method: social-historical study with a qualitative approach. Data collection occurred between March and November 2017. The theory of Claude Dubar was used as a theoretical reference. Eleven interviews were conducted, which were analyzed by the technique of Discourse Analysis. Results: two categories emerged. The first highlighted the articulation process, the practice of care, and the Teaching-Assistance Integration program, and addressed aspects related to power and the model-reference. The second listed the role of the teacher/manager and emphasized issues related to recognition and respect. Objective: to analyze the production of knowledge in the scientific literature on the constitution of professional identity in nurses working in the field of mental health. Method: integrative literature review through the Virtual Health Library portal in four databases: Lilacs, Scielo, PubMed and BDENF. The eligibility criteria were full original articles published from 2017-2022 in English, Portuguese, or Spanish. Results: the sample was composed of 18 studies grouped into two thematic categories. The results are about the (in)definitions of the work process of nurses working in the mental health field, and about the fragmentation of care and its implications for the constitution of this professional identity. Conclusion: nurses play diversified roles in their daily work, which contributes to an insufficient understanding of their scope of attributions and directly impacts the perception of their professional identity. High attrition rates in new graduate nurses maybe attributed to unrealistic perceptions of being a nurse. It is therefore important to identify nursing students' perceptions and the factors that influence them. The present study was conducted to identify and describe the literature relating to nursing students' perceptions of being a nurse. Research questions guiding the review were: (1) What are nursing students' perceptions of nursing? (2) What factors influence nursing students' perceptions of nursing? A scoping literature review was conducted between the years 2008 and March 2022 to capture nursing student perceptions from multiple countries, and year levels of study to note the changes in perceptions overtime. The data sources consisted of five electronic data bases; CINAHL, Medline, PsychINFO, Scopus, and Web of Science and search engine Google Scholar revealed 39 relevant sources. A scoping review methodology informed by the Joanna Briggs Institute and a published Scoping Review Checklist guided this review. An inductive thematic analysis identified five key themes. Key themes: Attributes and characteristics of nurses; Nursing as a profession; Skills required for nursing; Gender perceptions; and Influence of time and previous health work experience. Findings provide insight for future research to ensure the adequacy of curricula experiences in preparing new graduates for having realistic perceptions for practice. For nursing students to be prepared for the role of a nurse they must have realistic perceptions of what it means to be a nurse. To analyze and define the concept of nurses' media competency. Concept analysis. We conducted a literature search in PubMed, CINAHL, PsycInfo, and RISS International, as well as a hand-search, for relevant articles published between January 1990 and April 2023. A total of 57 articles related to nurses' media competency, published in English or Korean, were included in this study. We used Walker and Avant's method of concept analysis to identify the defining attributes, antecedents, and consequences of the concept. The seven defining attributes of nurses' media competency were networking with members of the media, selecting the appropriate media platform, considering that the target audience is the public, creating one's own media products, delivering intended messages through the media, monitoring and responding to the media, and maintaining professional dignity. Antecedents of the concept included cultivation of desirable nursing professionalism, critical evaluation of policies and issues, recognition of media influence, establishment of education and guidelines for media use, and having facilities and availability of technologies for media use. Consequences of the concept were improved influence of individual nurses, enhanced public awareness of the nursing profession, stronger networks and collaboration among professionals, and contributions to the promotion of public health. Cystic fibrosis is a life-shortening genetic disease. It affects both patient and family with the nurse playing a key role in the monitoring process. This study sought to contribute to enhanced targeted health care to adolescents with cystic fibrosis and their parents by understanding the experiences of living with cystic fibrosis. Based on Afaf Meleis' Transitions Theory (1986) nurses identify the transition experienced by the study participants, thus contributing to the quality of nursing interventions. Two qualitative research studies using data collected through semi-structured interviews were conducted. The Straussian Grounded Theory was applied. The snowball technique was used for recruitment, under the inclusion criteria: adolescents aged between 10 and 21 years; diagnosis of cystic fibrosis for more than one year; and parents of these adolescents. A final sample of 16 adolescents and 14 parents was obtained. Nursing therapeutic interventions acted as a facilitator of the health/illness transition process. Nurses' intervention areas were identified to empower adolescents and their parents with targeted knowledge and abilities to cope with problems. After diagnosis, parents assumed a new role-playing. Adolescents with cystic fibrosis and their parents experience various transition phases. Nurses can better help identifying the onset, persistence and ending of harmful periods. Millions of people globally have been affected by the Covid-19 pandemic. It's impact on pain management nurses roles' remains unknown. To explore role changes among pain management nurses performing patient care during the Covid-19 pandemic. Qualitative descriptive research study. The American Society for Pain Management Nursing's listserv, E-News Brief postings, and snowball sampling. English-speaking registered nurses or advanced practice registered nurses who provided direct patient care since 2020 were eligible. Data were collected through individual, semi-structured telephone interviews. An interview guide was used and included questions about participants' characteristics and the effect of the Covid-19 pandemic on their roles in clinical work. Data were analyzed using qualitative content analysis. A homogenous sample of eighteen nurses from the United States was interviewed. Their normal roles, roles during the pandemic, and surges in patients with Covid-19 as the condition for role changes emerged from their descriptions. Most participants did not experience significant changes in their normal roles, but all described how their normal functions were impacted by the pandemic. As the infectious variants of this disease evolve or other disastrous conditions occur, further changes to roles may occur. The skill sets of pain management nurses, including understanding assessment of pain across the lifespan, administration of opioids and multimodal analgesia, monitoring of patients, and communicating by educating and consultations, reinforce the significant contribution pain management nurses have as valued team members in times of crisis. • The more that nurses cared for patients the higher the nurses' depression and anxiety. • Both home-work and work- home conflict were associated with higher levels of depression and anxiety. • Perceived mastery had the strongest negative correlation with depression and anxiety. • When asked what has helped the nurses to carry out their care of patients the most common responses were co-worker support, training in proper PPE, and support from family/friends. • Fewer than one quarter of the nurses reported that their profession nursing education was helpful in caring for the COVID population. Infectious disease pandemics, such as COVID-19, have dramatically increased in the last several decades. To investigate the personal and contextual factors associated with the psychological functioning of nurses responding to COVID in the New York City area. Cross sectional data collected via a 95-item internet-based survey sent to an email list of the 7,219 nurses employed at four hospitals. 2,495 nurses responded (RR 35%). The more that nurses cared for COVID patients as well as experienced home-work conflict and work-home conflict the higher the nurses' depression and anxiety. When asked what has helped the nurses to carry out their care of patients the most common responses were support from and to co-workers, training in proper PPE, and support from family/friends. Understanding the potential triggers and vulnerability factors can inform the development of institutional resources that would help minimize their impact, reducing the risk of psychological morbidity. There is a growing recognition of significant, unmet palliative care needs in nursing facilities, yet limitations in the workforce limit access to palliative care services. Attention to palliation is particularly important when there are efforts to reduce hospitalizations to help ensure there are no unintended harms associated with treating residents in place. A specialized palliative care registered nurse (PCRN) role was developed as part of the OPTIMISTIC (Optimizing Patient Transfers, Impacting Medical quality, and Improving Symptoms: Transforming Institutional Care) program, a federally funded project to reduce potentially avoidable hospitalizations. Working in collaboration with existing clinical staff and medical providers, the PCRN focuses on managing symptoms, advance care planning, achieving goal concordant care, and promoting quality of life. The PCRN serves as a resource for families through education and support. The PCRN also provides education and mentorship to staff to increase their comfort, knowledge, and skills with end-of-life care. The goals of this article are to provide an overview of the PCRN role and its implementation in nursing facilities and describe core functions that are transferrable to other contexts. Purpose: The role of registered nurses is expanding in scope as the healthcare paradigm shifts from acute, hospital-based care to community and population-based care. Given this paradigm shift, this study explores the legal aspects of the role of a registered nurse. Methods: We used document analysis for extracting laws and legal orders related to nursing from the entirety of Korean law. Using textualism approach, we examined the contents utilizing a framework that was developed based on the role classification of community nurses by Clark in this study. Results: A total of 119 items related to nursing were derived from 64 laws. Of these, 71.4 % can be performed by people in multiple types of occupations including nurses. As a result of analyzing required qualifications, 45.4% of 119 items required additional qualifications besides registered nurse license. Analysis of workplace and activity type demonstrated that 26.1% of the 119 items were related to medical institutions, with nurses performing mostly "Client-oriented role." More than half (68.9%) were non-medical institutions, with nurses performing mostly "Delivery-oriented role." Some, however, did not stipulate the nurse's roles clearly. Conclusion: Therefore, to match the enhanced scope and responsibilities of registered nurses and to appropriately recognize, guide, and hold these nurses accountable, laws and policy must reflect these changes. Merged healthcare settings, particularly those with Magnet designated sites, present distinct opportunities for PhD nurse scientists developing nursing research infrastructure. This article aims to assist nurse scientists and healthcare leaders in defining nurse scientist roles, and in developing research infrastructure for conducting multi-site research in merged settings. Practical strategies and a framework are provided to assist in building and navigating nurse scientist roles and research infrastructure development. Emphasizing the necessity of organizational support, the article underscores the importance of clear role delineation and leadership support. RN4CAST-Chile builds upon two decades of global nursing outcomes research and the successful implementation of the RN4CAST study in 30 countries around the world to replicate this research for the first time in South America. Over 70 scientific papers in leading nursing and interdisciplinary journals have resulted just from the RN4CAST-EU study, making it the most productive study ever funded by the European Commission (www. rn4cast.eu). The results of this program of research have been influential in changing clinical practice, managerial policies, and governmental policies in many countries. Based upon the establishment of a link between lower patient to nurse ratios and better patient outcomes, a number of jurisdictions and countries have adopted safe nurse staffing standards including California in the U.S., Victoria and Queensland in Australia, Ireland, and Wales. Documentation that a nurse workforce qualified at least at the bachelor's level is associated with better patient outcomes including lower hospital mortality has been the catalyst for U.S. hospitals working toward a target date of 2020 to achieve at least a 80% bachelor's educated nurse workforce. The European Parliament for the first time approved a bachelor's pathway for nurse education. Additionally research from RN4CAST and its predecessor studies has been influential in creating the evidence base in support of redesigning clinical work environments to provide greater involvement of clinicians in institutional policy decisions and better managerial support to reduce operational errors in clinical settings that detract from nurses, doctors, and others providing safe clinical care. Background Nurses in acute medical units are uniquely positioned to support goals of care communication. Further understanding of nurse and physician perceptions about hospital nurses' actual and possible roles was required to improve goals of care communication. Objective To critically examine nurse and physician perceptions of the nurse's role in communication with seriously ill patients and their families. Design We focus on the qualitative component of a mixed method study. We employed an interpretive descriptive approach informed by Flanagan's critical incident technique. Settings Participants were recruited from the acute medical units at three tertiary care hospitals in three Canadian provinces. Participants Thirty participants provided interviews (10 from each site): 12 nurses, 9 staff physicians and 9 medical resident physicians. Methods Participants' described “critical incidents” they considered as “excellent” or “poor” or “usual” practice. Interviews, were audiotaped and transcribed. Team-based analysis used constant comparison and triangulation to identify healthcare team members' roles in goals of care communication. Results We identified two major themes from 120 critical incidents: 1) the ambiguous nature of the nurse's role in formal, physician-led, decision-making communication, and 2) embedded in care serious illness communication. Physicians understood nurses' supportive role in relation to their own communication practices that culminated in decisions about care; nurses' reported their roles were determined by unit routines, physician practices and preferences, and their self-confidence in supporting decision-making. Nurses described their unique role in facilitating informal and spontaneous communication with patients and families that was critical background work to physician-led goals of care communication. Conclusions Nurses and physicians had different understandings, practices and beliefs about goals of care communication The value of nurses embedded in care work is key to supporting the interprofessional team's work during formal goals of care communication. To explore final-year bachelor nursing students' view on their future role and skills regarding euthanasia due to unbearable mental suffering (UMS-euthanasia) and to discover possible education needs. Since UMS-euthanasia is legal in Belgium, nursing students will be confronted with this issue during their future internship, or professional career. Graduated nurses believe to have an important role in the process of euthanasia where good communication skills and sufficient legal knowledge are essential. Mixed-methods sequential explanatory design. First, a questionnaire was administered to all final-year bachelor nursing students at eight university colleges in Flanders (October 2020 and March 2021). Second, qualitative information was collected by organising focus groups with these students to gain more insights into the quantitative results (October 2021). The final-year nursing students from the survey (n = 249) and from the three focus groups (n = 21) see a clear future role for themselves in the decision-making process, as well as in supporting the patient and family before, during and after UMS-euthanasia, but not in assisting in administering UMS-euthanasia. However, they also indicate they lack knowledge and skills in dealing adequately with UMS-euthanasia. Psychiatric internship alumni perceive a diminished role for themselves in the preparatory acts (p < 0.05). Ever involved in euthanasia shows a statistically significant difference in the demand for simulation. Nursing students see an important role for nurses in UMS-euthanasia but feel inadequately prepared to take on this role. After childbirth, most American women are not scheduled for follow-up care for 6 weeks, and this visit is poorly attended. Many new mothers feel unprepared for the common health issues they encounter and are uncertain of whom to contact. To improve care, the 4th Trimester Project is bringing together mothers, health care providers, and other stakeholders to explore what families need most from birth to 12 weeks postpartum. Eighty-seven individuals convened in March 2016 in Chapel Hill, NC. Four major topic areas emerged: (1) the intense focus on women's health prenatally is unbalanced by infrequent and late postpartum care; (2) medical practice guidelines often do not align with women's experiences and constraints; (3) validation of women as experts of their infants and elevating their strengths as mothers is necessary to achieve health goals; and (4) mothers need comprehensive care, which is difficult to provide because of numerous system constraints. Considerations for improving postpartum services include enabling more convenient care for families that is holistic, culturally appropriate, conversation based, and equitable. Maternal health issues in the fourth trimester intersect and can compound one another. Enhanced collaboration among health care providers may improve the focus of clinical interactions to address the interrelated health issues most important to women. [ABSTRACT FROM AUTHOR]

Aim: the aim of this paper is to investigate, from the perspective of nurse managers, the means/factors that facilitate the implementation process of patient safety strategies. Method: seventy-two nurse managers from four university hospitals of the state of Paraná participated, through an individual interview, which was recorded and guided by the question "Tell me about aspects that facilitate the implementation of safety strategies in this hospital." The statements were transcribed in full and were submitted for content analysis in the thematic modality. Results: two categories emerged from the speeches: "Management tools as facilitators in the implementation of patient safety strategies" and "Educational processes: means that facilitate the implementation of patient safety strategies". Conclusion: it was found that the support of the top management contributed greatly to the implementation of security strategies, as well as the militant leadership for this benefit and the improvement of human capital, which was strictly conveyed to the institutional teaching characteristic. Introduction Autism spectrum disorder (ASD) is a lifelong condition characterized by very mild symptoms to severe impairments in communication skills, social interactions, and unusual behaviors that interfere with independent functioning and interactions with others. ASD may cause financial, social, and emotional difficulties that negatively affect the entire family. The main focus of current research in caregivers of children with ASD focuses on the mothers rather than fathers or both parents, and no studies have used the fathers' words to describe their experience with ASD. Fathers have an important role to improve outcomes for their children receiving therapy. With more fathers taking on the primary caregiver role, it is important to understand their experience with ASD and their needs to help them actively engage in their role as fathers. The purpose of this study was to describe fathers' experience with ASD using their own words and identify any resources needed to help them actively engage in their role as a father of a child with ASD. Methods A single-case, multiple-participant, phenomenological approach case study was conducted to describe the experience of fathers of children with ASD. The case was defined as White non-Hispanic (the population identified as having the highest incidence of ASD) fathers who are living in the same home with their child(ren) who has/have a formal diagnosis of ASD. Fathers answered open-ended questions about their experience with ASD, including the challenges they face, their advice for other fathers of children with ASD, and what they need to overcome the challenges of ASD. The theoretical model for the study was the Resiliency Model of Family Stress, Adjustment, and Adaptation. The model was designed to help health care professionals identify a family's strengths and weaknesses as they face existing and new stressors related to a long-term medical condition and then provide interventions to help the family become resilient so they can meet the challenges. Results Fathers in this study had experiences similar to those described in previous studies with regard to mothers of children with ASD. The similarity of the findings in the small sample size of fathers and those in studies with larger sample sizes of mothers supports development of clinical interventions that will benefit both mothers and fathers of children with ASD. Fathers described the adjustments made by each member of the family to face the challenges of ASD. Fathers also explained their needs, including money, teamwork, honesty, information, and time. The fathers experienced stages of grief and loss that are similar to those that occur when experiencing a death. Discussion Nurses are the eyes and ears of the health care team and the voice for the parents, thus creating a critical connection between the parents and health care team. Fathers in the current study and mothers in previous studies described their need for someone to listen to them, educate them and others about ASD, acknowledge their experience and emotions, and direct them to appropriate resources. Nursing implications include taking action to lead the parents through the challenges and helping them develop resiliency and facilitating improved outcomes for the children and the entire family. A resource book or computer file including information about available resources and information about ASD and treatment can be a valuable tool to help overcome challenges related to ASD. Admission to an emergency department can be considered a stressful event for both the child and the family. Due to the nature of traumas, illnesses and fatalities it is a chaotic forum in which good communication between child and staff can be difficult to establish. Purpose The purpose of the study was to describe nurses' methods when interacting with children aged three to six at a pediatric emergency department and to identify aspects in need of further investigation. Methods The study included seven nurses who work with children. The data were collected through semi-structured interviews and analyzed using qualitative content analysis. Results The analysis resulted in three main themes; fundamentals for being able to create a good encounter, nurse's adaptations when encountering children and limitations associated with providing child and family-centered care in the pediatric emergency department. Conclusions Healthcare organizations must create time to allow important communication to take place between staff and pediatric patients so that children and families feel safe when being treated. The implementation of effective measures to train staff in communication with pediatric patients is important. Practice implications The child should participate in his/her care and in procedures as much as possible. By listening to children and their parents proposals, especially before invasive procedures, effective ways to handle pain and discomfort may be developed. [ABSTRACT FROM AUTHOR]

The nurse-physician relationship is undergoing profound changes after the introduction of the degree in nursing sciences. We conducted a survey among 18 Internal Medicine Units in Liguria region to investigate the real picture of this collaboration, using the Jefferson collaboration scale (JCS) and the nurse-physician collaboration scale (NPCS). We sent anonymous questionnaires to 580 nurses and 180 physicians of whom over 50% responded. Of 15 items of JCS and 27 of NPCS, 9 (60%) and 13 (48.1%) did not show different opinions between nurses and physicians, respectively. However, significant differences were observed on several items, including the caring process, the nurse autonomy, the physician authority and the human relationship. On the question: The primary function of the nurse is to carry out the physician’s orders, 23% of the nurses and 33% of physicians agreed, whereas only 46% of the nurses and 22% of the physicians strongly disagreed (P<0.001). Our study demonstrates the desire of nurses and physicians to cooperate, although some differences are still evident and require plans of improvement. [ABSTRACT FROM AUTHOR]

The aim was to study if health outcome and secondary prevention were satisfactory 1 year after stroke and if nurse-led interventions 3 months after stroke could have impact. Design was a randomized controlled open trial in a 1-year population. Primary outcome was health status 1 year after stroke. One month after stroke, survivors were randomized into intervention group (IG) with follow-up by a specialist nurse (SN) after 3 months (n = 232), and control group (CG) with standard care (n = 227), all to be followed up 1 year after stroke. At the first follow-up, patients graded their health, replied to the EuroQol-5 Dimensions (EQ-5D) health outcome questions, health problems were assessed, and supportive counseling was provided in the IG. Health problems requiring medical interventions were primarily referred to a general practitioner (GP). One year after stroke, 391 survivors were followed up. Systolic blood pressure (BP) had decreased in IG (n = 194) from median 140 to 135 ( P = .05), but about half were above the limit 139 in both groups. A larger proportion (22%) had systolic BP >155 in the CG (n = 197) than in the IG (14%; P = .05). In the IG, 62% needed referrals compared with the 75% in the CG ( P = .009). Forty percent in the IG and 52.5% in the CG ( P = .04) reported anxiety/depression. In the IG, 75% and 67% in the CG rated their general health as fairly good or very good ( P = .05). Although nurse-led interventions could have some effect, the results were not optimal. A more powerful strategy could be closer collaboration between the SN and a stroke clinician, before referring to primary care. [ABSTRACT FROM AUTHOR]

Background: Implantable cardioverter defibrillator (ICD) implantation rates are increasing as advances in heart failure and arrhythmia management progress. Consequently, the number of ICD generator replacements is rising and ICD replacement is an opportune time for shared decision-making (SDM). Nurses should have distinct roles and responsibilities in SDM processes. Objectives: To use a relational lens to localize the role of the nurse in SDM, and recommend ways in which nurses can be involved in SDM. Methods: An integrative review of 17 articles was conducted to determine the role of nurses in SDM. Results: Our analysis revealed four themes that helped us articulate nurse involvement in SDM; knowledge as a basis for SDM, sharing power in the nurse-patient relationship, utilization of decisional support strategies, and communication. Objectives: To evaluate the level of agreement and applicability of the EULAR recommendations for the role of the nurse in the management of chronic inflammatory arthritis in Portugal. Methods: Nurses from all Portuguese rheumatology centers were invited to fill-in a questionnaire addressing the level of agreement (1=strongly disagree to 5=strongly agree) and the level of confidence (1=no confidence to 5=complete confidence) in the applicability of each of the recommendations. Comparisons were established between subgroups of nurses. Results: A total of 75 nurses (85% female; mean (SD) of 3.9 (4.1) years of practice, 32% with some type of training in rheumatology) participated in the study. The mean level of agreement with the recommendations was 4.8 (SD 0.3). The level of confidence in their full applicability was 3.6 (SD 0.8). Significantly more nurses with rheumatology training totally agreed with recommendations 7 (88% vs 73%, p=0.03) and 3 (96% vs 65% p=0.04) and were fully confident in the applicability of recommendations 2, 3, 7, 9 and 10, comparing with those without specific training. Conclusions: The overall level of agreement with EULAR recommendations among Portuguese nurses is high, although rheumatology does not exist as a nursing specialty. Agreement and confidence in the applicability of these recommendations is higher among nurses with specific training, underlining the importance of continuous education for future commitment. Summary: Background: Scribe nurses within metropolitan emergency departments are traditionally Registered Nurses who document the resuscitation event to provide a true and timely representation of what occurred. Enrolled Nurses undertake the scribe role in some Australian emergency department resuscitations, particularly in rural and remote health services. There is no Australian research evidence pertaining to the role of the scribe nurse within a resuscitation team. This study explored the scribe role and the nursing work involved within it to appraise whether it is appropriate to delegate the responsibility away from Registered Nurses. Method: Using a qualitative descriptive design, semi-structured interviews were conducted with eight Registered Nurses who had emergency department scribe nurse experience. Thematic analysis was adopted to identify common threads within the interview data. Results: The four themes identified were the role, scribe effectiveness, expertise and ‘scribe by default’. Participants were generally positive regarding the potential for Enrolled Nurses to scribe in metropolitan emergency department resuscitation teams. Conclusion: The characteristics of an effective scribe; well developed communication skills, confidence and assertiveness and resuscitation ‘know how’, may be the measurement of readiness for the position of scribe nurse within the resuscitation team, rather than number of years of clinical experience or designation. Summary: Aim: This research explores the roles of nurses that participated in the Black Saturday and Victorian bushfires in February 2009, including aspects that influence nurses’ roles, such as prior education, training and availability of resources. Background: It is acknowledged that nurses play an important role in disaster response and recovery. However, our understanding of nurses’ roles is superficial and commonly based on descriptions of events in which specifics relating to the nurses’ roles are embedded within other topics or issues. Similarly, aspects that support nurses in the disaster environment, including previous experience, education and the provision of resources, are not well understood. Method: Single, semi-structured telephone interviews were conducted with 11 volunteer nursing members of St John Ambulance Australia. These interviews were electronically recorded, transcribed verbatim and thematically analysed using a well-recognised human science approach. Findings: The thematic analysis identified two broad themes: being prepared and having an expansive role. Participants indicated that they were educationally prepared and had adequate clinical experience. They outlined that they took many resources with them; however, they were used very little, as their role consisted of minimal clinical care. Additionally, nurses performed roles including a psychosocial supporter, a coordinator of care and resources, and problem solvers. Conclusions: The nurses’ role in providing health care during and/or following a disaster is more than a clinical care role. This understanding should be applied to the development of education programs, competencies and policies, with a particular focus on contextualising the education to the realities of possible disastrous scenarios that incorporates elements of coordination, problem solving and psychosocial care within a national framework. Summary: Introduction: Traumatic injury is a leading cause of premature mortality and preventable morbidity worldwide. It is considered that nurses perform an integral role in the assessment and management of major trauma, and any in hospital resuscitation. Aim: This systematic search and integrative review of the literature aims to determine the state of knowledge of the impact of nursing roles in trauma and other hospital patient resuscitation. Results: Following a comprehensive stringent search and screening process, 18 articles were included in the review. Ten articles focused on trauma resuscitation and 8 described other resuscitation nursing roles in other contexts such as part of a Rapid Response Team (RRT). Nine papers were original research papers, with a mixture of methods, the remaining 9 articles were literature reviews. Quantitative research, with the exception of one paper, was restricted to the impact of the team rather that the individual nursing role. Analysis found that nurses contribute significantly to verbal and non-verbal communication, leadership and effective team work in resuscitation. The use of expert nurses has been demonstrated to facilitate effective treatment of patients at high risk of mortality. Conclusion: Nurses contribute significantly to communication, leadership and effective team work, which ultimately impact patient outcomes. Although there is literature to support the nurse adopting a leadership role during resuscitation it is limited and demonstrates the need for further dedicated research. [Copyright &y& Elsevier]

Purpose: The purpose of this study was to explain the role transition process to nurse case managers (NCMs) for Medical-aid beneficiaries in Korea. Methods: Fourteen NCMs were interviewed regarding their experiences of becoming proficient in the new role of case manger. Data were analyzed through the application of grounded theory. Results: 'Taking root in a barren land' was the core category explaining the role transition process of NCMs. They engaged in four stages: launching, trial and error, proficiency, and wait-and-see stages. NCMs showed not only fear but also passion for case management practice. Despite their passion and effort, NCMs went through a period of trial and error. After becoming skilled, NCMs went through a stage of wait-and-see often because of job insecurity related to temporary position or few opportunities for promotion. Factors influencing NCMs' role transition process included their understanding of client characteristics, belief in case management, and support from their colleagues and families. Conclusion: NCMs experience many challenges in the process of becoming proficient NCMs. To help with their role transition, there is a need for education programs, preceptorship programs, research on their roles and functions, and regulation for securing NCMs' employment and career stability. Abstract: Purpose: To assess the role of the Clinical Trials Nurse (CTN) and to evaluate the quality of the job performed by Clinical Trials Nurses (CTNs) in Italy. Methods: The study design was descriptive. The sample included 30 CTNs in Italy who were involved in conducting clinical trials in the last years. Respondents completed the Italian Clinical Trials Nursing Questionnaire (CTNQ) developed to measure frequency and importance of clinical trials nursing activities. Data analyses included descriptive statistics, Student’s t-test and Chi–Square test. Results: Thirty out of 34 CTNs consented to participate. Respondents were more involved in the experimental drug management, in the protocol implementation and, partially, in the informed consent process. CTNs have a marginal position with respect to the protocol assessment and planning, subject recruitment, data management. CTNs reported high rates for the importance evaluation. Number of years in the nursing role was significantly associated with data management related activities (p = 0.016). Items with minor response rate differences between frequency and importance were not statistically significant (p values ranging from 0.087 to 0.911). The CTNs reported to be autonomous and competent; however, they lack and/or do not perform some nursing-related responsibilities and/or activities. Conclusions: Although CTNs are not involved in all of the activities listed on the CTNQ, most of them are fully aware to be a key member of research teams. Overall, the Italian CTN role is mostly practical task-oriented and focuses little on data management and organizational activities. [Copyright &y& Elsevier]
