2. Activity limitations related to cold hypersensitivity in patients with hand injuries.

- Abstract
Knowledge of specific activity limitations due to cold hypersensitivity is sparse. To explore activity limitations caused by cold hypersensitivity and the associated environmental factors in patients with hand injuries. Descriptive study. Seventy-seven patients with multiple tissue hand injuries described activity limitations related to cold hypersensitivity in questionnaires six and/or 12 months after surgery. To characterize the activity limitations, we performed a quantitative content analysis applying the International classification of functioning and health (ICF). The respondents described on average 3.6 cold related activity limitations in leisure and/or work. The activities were most frequently (n) assigned to the ICF domains d920 Recreation and leisure (95), d440 Fine hand use (77), and d850 Remunerative employment (39). The most common related environmental factors were Products and technology for e140 Culture, recreation and sport (79) and for e135 Employment (71), respectively, and e225 Climate (71). Gloves were both a facilitator and a barrier for activity performance. The impact of cold hypersensitivity on activity was related to both the activity and the environment. The results call for clinical interventions and product development, for example of gloves, to help patients solve their cold related activity limitations. • Several activity limitations due to cold hypersensitivity were reported. • Cold hypersensitivity affected both fine and gross motor activities. • The ICF-domains Fine hand use and Recreation and leisure were typically affected. • Climate was the environmental factor most commonly related to activity limitations. • Gloves or mittens could both facilitate and hinder activity performance. 3. Low adherence to prescribed time under tension in elastic band resistance training in older adults. An observational study.

- Abstract
Time under tension (TUT) is an important variable in resistance training (RT). A stretch sensor (Bandcizer) attached to an elastic band during RT can quantify TUT reliably. To explore adherence to prescribed TUT for elastic band RT with the Bandcizer during supervised and unsupervised RT at different time points. 22 men and women (66.2 ± 2.1 years) performed one supervised and two unsupervised sessions weekly of elastic band RT for the upper and lower extremities with the Bandcizer during a 25-week period. Prescribed TUT was 4 s/repetition. Bandcizer data were extracted from week: 1, 8, and median 24. The effects of time point and supervision on adherence (%) to TUT and number of repetitions were analyzed by Two-way ANOVA. During supervised sessions, adherence to prescribed TUT was 79 ± 4%, 60 ± 4%, and 53 ± 5% in week: 1, 8, and median 24, respectively (p < 0.001). There was no difference in adherence between supervised and unsupervised sessions at any time points. Adherence to the prescribed number of repetitions in each training session was close to 100%. Adherence to prescribed TUT was low during both supervised and unsupervised, home-based elastic band RT and decreased over time despite weekly supervision. Our results highlight the need to focus on TUT when planning and implementing elastic band resistance training in retirement age individuals. • Adherence to prescribed time under tension in elastic band resistance training in older adults was low and declined over time despite supervision. • More focus on time under tension is mandatory in elastic band resistance training interventions in order to reach a satisfactory adherence. • Group-based weekly supervision during elastic band resistance training in older adults may not be sufficient to uphold the prescribed time under tension. 4. Five-Year Results of the Salto XT Revision Ankle Arthroplasty.

Background: The treatment of failed ankle replacements is debated, and little is published about the medium- and long-term results of revision implants. We wanted to examine prosthesis survival and physical function at least 5 years after insertion of the Salto XT revision prosthesis. Methods: All consecutive patients operated with a Salto XT revision prosthesis underwent clinical and radiologic examinations preoperatively and after 3, 12, 24, and 60 months. Complications and reoperations are described, and changes in patient-reported outcome measures and clinical scores are reported. Results: Thirty patients were operated with a Salto XT revision prosthesis between March 2014 and March 2017. Three of these were revised (1 to a fusion and 2 to a new prosthesis), and 3 patients were reoperated with screw removal. A concurrent subtalar fusion was performed on 13 patients, and there was 1 case of likely nonunion after these procedures, but no reoperations. The mean AOFAS score increased from 39.2 (95% CI 30.8-47.5) preoperatively to 75.1 (95% CI 67.3-82.9) after 5 years, and the mean improvement was 34.2 points (95% CI 23.8-44.6). Mean EQ-5D increased from 0.36 (95% CI 0.30-0.42) preoperatively to 0.74 (95% CI 0.64-0.85) after 5 years, an improvement of 0.34 (95% CI 0.19-0.49). Radiolucent lines were present in all but 3 patients. Five-year prosthesis survival was 93% (83.6-100). Conclusion: This is the first study to present medium-term results of this implant. 9. Hand hygiene compliance in nursing home wards: The effects of feedback with lights on alcohol-based hand rub dispensers.

- Abstract
Hand hygiene (HH) among health care workers (HCWs) is crucial in preventing infections in nursing homes. However, HH compliance (HHC) among HCWs remains low. This study aimed to investigate the effect of feedback lights on HCWs' HHC. A 5-month interventional study was conducted in 3 wards in a nursing home in Denmark. During the intervention period, a green light with a smiley appeared on the alcohol-based hand rub (ABHR) dispensers when HCWs used the ABHR, acknowledging HCWs for using the ABHR. HHC was monitored using an automatic HH monitoring system (AHHMS). A total of 64 HCWs were enrolled. The AHHMS collected 23,696 HH opportunities in apartments and dirty utility rooms. Overall, HHC in the apartments increased from 50% at baseline (95% CI: 48, 53) to 56% (95% CI: 54, 58) during the intervention. However, the increased HHC level was not sustained during follow-up. The AHHMS enabled the assessment of the intervention. We found a significant effect of light-guided feedback in the apartments. However, the increased HHC was not sustained after the light was switched off. • Feedback with light on alcohol-based hand rub was investigated in nursing home wards. • Hand hygiene compliance (HHC) was measured using an automatic hand hygiene system. • The light had an immediate, significant effect on health care workers' (HCWs') HHC. • The increased HHC level was not sustained over time. • The light did not change HCWs' HHC in dirty utility rooms. Percutaneous active mechanical circulatory support (MCS) devices are being increasingly used in the treatment of acute myocardial infarction-related cardiogenic shock (AMICS) despite conflicting evidence regarding their effect on mortality. We aimed to ascertain the effect of early routine active percutaneous MCS versus control treatment on 6-month all-cause mortality in patients with AMICS.

- Abstract
Deep needle insertion to a target often poses a huge challenge, requiring a combination of specialized skills, assistive technology, and extensive training. One of the frequently encountered medical scenarios demanding such expertise includes the needle insertion into a femoral vessel in the groin. Conventionally, deep needle insertion is guided by ultrasound (US) imaging. However, utilizing US for needle tracking demands specialized training and skill to manipulate the probe effectively and interpret the imaging accurately. To address this challenge, this article presents an innovative technology for needle tip real-time tracking. This advancement will be instrumental in facilitating robotic-guided needle insertion toward the identified target. Specifically, our approach revolves around the creation of scattering imaging using an optical fiber-equipped needle and uses convolutional neural network (CNN)-based algorithms to enable real-time estimation of the needle tip’s position and orientation during insertion procedures. The efficacy of the proposed technology was rigorously evaluated through three experiments. The first two experiments involved rubber and bacon phantoms to simulate groin anatomy. - Abstract
Portal hypertension has cerebral consequences via its causes and complications, namely hepatic encephalopathy (HE), a common and devastating brain disturbance caused by liver insufficiency and portosystemic shunting. The pathogenesis involves hyperammonemia and systemic inflammation. Symptoms are disturbed personality and reduced attention. HE is minimal or grades I to IV (coma). Bouts of HE are episodic and often recurrent. Initial treatment is of events that precipitated the episode and exclusion of nonhepatic causes. Specific anti-HE treatment is lactulose. By recurrence, rifaximin is add-on. Anti-HE treatment is efficacious also for prophylaxis, but emergence of HE marks advanced liver disease and a dismal prognosis.

16. Real time organ hypoperfusion detection using Indocyanine Green in a piglet model

Background: Preserving sufficient oxygen supply to the tissue is fundamental for maintaining organ function. However, our ability to identify those at risk and promptly recognize tissue hypoperfusion during abdominal surgery is limited. To address this problem, we aimed to develop a new method of perfusion monitoring that can be used during surgical procedures and aid surgeons’ decision-making. Methods: In this experimental porcine study, thirteen subjects were randomly assigned one organ of interest [stomach (n = 3), ascending colon (n = 3), rectum (n = 3), and spleen (n = 3)]. After baseline perfusion recordings, using high-frequency, low-dose bolus injections with weight-adjusted (0.008 mg/kg) ICG, organ-supplying arteries were manually and completely occluded leading to hypoperfusion of the target organ. Continuous organ perfusion monitoring was performed throughout the experimental conditions. Results: After manual occlusion of pre-selected organ-supplying arteries, occlusion of the peripheral arterial supply translated in an immediate decrease in oscillation signal in most organs (3/3 ventricle, 3/3 ascending colon, 3/3 rectum, 2/3 spleen). Occlusion of the central arterial supply resulted in a further decrease or complete disappearance of the oscillation curves in the ventricle (3/3), ascending colon (3/3), rectum (3/3), and spleen (1/3). Conclusion: Continuous organ-perfusion monitoring using a high-frequency, low-dose ICG bolus regimen can detect organ hypoperfusion in real-time.

19. Hand hygiene compliance in nursing home wards: the effect of increased accessibility of alcohol-based hand rub.

- Abstract
Elderly nursing home residents are vulnerable to infection from micro-organisms. Hand hygiene is considered one of the most important measures to prevent transmission. To determine the effect of increased accessibility to alcohol-based hand rub (ABHR) in nursing home wards by monitoring hand hygiene compliance (HHC) among healthcare workers (HCWs). An 11-month intervention study was conducted in a Danish six-ward nursing home. Data were collected using an automatic hand hygiene monitoring system (AHHMS). After a baseline period, one extra ABHR dispenser was placed in each of the 150 apartments. Baseline HHC was compared with the HHC during an immediate intervention period and a long-term intervention period. A total of 159 HCWs were included. The AHHMS registered 341,078 hand hygiene opportunities. Overall baseline HHC was 31% (95% confidence interval: 30–32). A significant +18% absolute immediate effect (first five months) (95% CI: 17–19; P < 0.0001) and +13 percentage points (95% CI: 11–14; P < 0.0001) long-term effect (another four months) were recorded. HCWs working day shifts and short-term employees had a higher baseline HHC than HCWs working evening/night shifts. However, HCWs working night shifts achieved the greatest long-term effect with a mean +27 percentage point difference (P < 0.0001). Placing an additional ABHR dispenser strategically within staff workflow significantly increased HHC among HCWs, demonstrating a noteworthy effect. The study is the first to report the effect on nursing home dispenser accessibility as a single intervention and to show a significant unmet potential. [ABSTRACT FROM AUTHOR]

Referent unit is foundational knowledge for rational numbers and quantity. This exploratory study examined how sixteen practicing teachers reasoned about a mathematical situation during an interview requiring the use of referent unit. Half of the teachers correctly reasoned with referent unit and a quarter made a referent unit error. This error may have been connected to a previous interview question as opposed to indicating a misunderstanding about referent unit. 24. Nonlocal effects in plasmon-emitter interactions

- Abstract
Nonlocal and quantum mechanical phenomena in noble metal nanostructures become increasingly crucial when the relevant length scales in hybrid nanostructures reach the few-nanometer regime. In practice, such mesoscopic effects at metal–dielectric interfaces can be described using exemplary surface-response functions (SRFs) embodied by the Feibelman d-parameters. Here we show that SRFs dramatically influence quantum electrodynamic phenomena – such as the Purcell enhancement and Lamb shift – for quantum light emitters close to a diverse range of noble metal nanostructures interfacing different homogeneous media. Dielectric environments with higher permittivities are shown to increase the magnitude of SRFs calculated within the specular-reflection model. In parallel, the role of SRFs is enhanced in noble metal nanostructures characterized by large surface-to-volume ratios, such as thin planar metallic films or shells of core–shell nanoparticles, for which the spill-in of electron wave functions enhances plasmon hybridization. By investigating emitter quantum dynamics close to such plasmonic architectures, we show that decreasing the width of the metal region, or increasing the permittivity of the interfacing dielectric, leads to a significant change in the Purcell enhancement, Lamb shift, and visible far-field spontaneous emission spectrum, as an immediate consequence of SRFs. We anticipate that fitting the theoretically modelled spectra to experiments could allow for experimental determination of the d-parameters.

25. Techno-economic analysis of the effect of a novel price-based control system on the hydrogen production for an offshore 1.5 GW wind-hydrogen system

- Abstract
The cost of green hydrogen production is very dependent on the price of electricity. A control system that can schedule hydrogen production based on forecast wind speed and electricity price should therefore be advantageous for large-scale wind-hydrogen systems. This work presents a novel price-based control system integrated in a techno-economic analysis of hydrogen production from offshore wind. A polynomial regression model that predicts wind power production from wind speed input was developed and tested with real-world datasets from a 2.3 MW floating offshore wind turbine. This was combined with a mathematical model of a PEM electrolyzer and used to simulate hydrogen production. A novel price-based control system was developed to decide when the system should produce hydrogen and when it should sell electricity to the grid. The model and control system can be used in real-world wind-hydrogen systems and require only the forecast wind speed, electricity price and selling price of hydrogen as inputs. 11 test scenarios based on 10 years of real-world wind speed and electricity price data are proposed and used to evaluate the effect the price-based control system has on the levelized cost of hydrogen (LCOH). Both current and future (2050) costs and technologies are used, and the results show that the novel control system lowered the LCOH in all scenarios by 10–46%. The lowest LCOH achieved with current technology and costs was 6.04 $/kg H2. Using the most optimistic forecasts for technology improvements and cost reductions in 2050, the model estimated a LCOH of 0.96 $/kg H2for a grid-connected offshore wind farm and onshore hydrogen production, 0.82 $/kg H2using grid electricity (onshore) and 4.96 $/kg H2with an off-grid offshore wind-hydrogen system. When the electricity price from the period 2013–2022 was used on the 2050 scenarios, the resulting LCOH was approximately twice as high.

- Abstract
Clinical reasoning is considered one of the most important competencies but is not included in most healthcare curricula. The number and diversity of patient encounters are the decisive factors in the development of clinical reasoning competence. Physical real patient encounters are considered optimal, but virtual patient cases also promote clinical reasoning. A high-volume, low-fidelity virtual patient library thus can support clinical reasoning training in a safe environment and can be tailored to the needs of learners from different health care professions. It may also stimulate interprofessional understanding and team shared decisions. Implementation will be challenged by tradition, the lack of educator competence and prior experience as well as the high-density curricula at medical and veterinary schools and will need explicit address from curriculum managers and education leads.

Globally, more than 1 billion people with disabilities are disproportionately and differentially at risk from the climate crisis. Yet there is a notable absence of climate policy, programming, and research at the intersection of disability and climate change. Advancing climate justice urgently requires accelerated disability-inclusive climate action. We present pivotal research recommendations and guidance to advance disability-inclusive climate research and responses identified by a global interdisciplinary group of experts in disability, climate change, sustainable development, public health, environmental justice, humanitarianism, gender, Indigeneity, mental health, law, and planetary health. Climate-resilient development is a framework for enabling universal sustainable development. Advancing inclusive climate-resilient development requires a disability human rights approach that deepens understanding of how societal choices and actions—characterised by meaningful participation, inclusion, knowledge diversity in decision making, and co-design by and with people with disabilities and their representative organisations—build collective climate resilience benefiting disability communities and society at large while advancing planetary health.

29. Synthetic modelling of railway trackbed for improved understanding of ground penetrating radar responses due to varying conditions

Ground penetrating radar (GPR) is a commonly used tool for railway trackbed inspection due to its ability to collect information about subsurface materials at high resolution and high speed. Although GPR recording systems allow for the collection of vast quantities of data (hundreds of kilometres per day), accurate ground truth information is difficult to obtain. Models of trackbed can be used to generate synthetic radargrams to provide a better understanding and predictability of GPR responses to a wide range of trackbed conditions. In this research, we produced models of ballast using randomly shaped 3D particles, with a range of particle size distributions to represent various stages of ballast breakdown. Additionally, void spaces are partially filled with a constant dielectric material to represent ballast contamination. We used gprMax to simulate the GPR response for a 2 GHz horn antenna over the trackbed models. These simulations resulted in radargrams that are visually indistinct from real recorded data in known conditions. These radargrams, along with their formative models, have provided valuable insights into how variations in trackbed conditions can impact GPR data.

33. Combined endoscopic and laparoscopic surgery (CELS) for early colon cancer in high-risk patients

Background: Local excision of early colon cancers could be an option in selected patients with high risk of complications and no sign of lymph node metastasis (LNM). The primary aim was to assess feasibility in high-risk patients with early colon cancer treated with Combined Endoscopic and Laparoscopic Surgery (CELS). Methods: A non-randomized prospective feasibility study including 25 patients with Performance Status score ≥ 1 and/or American Society of Anesthesiologists score ≥ 3, and clinical Union of International Cancer Control stage-1 colon cancer suitable for CELS resection. The primary outcome was failure of CELS resection, defined as either: Incomplete resection (R1/R2), local recurrence within 3 months, complication related to CELS within 30 days (Clavien–Dindo grade ≥ 3), death within 30 days or death within 90 days due to complications to surgery. Results: Fifteen patients with clinical T1 (cT1) and ten with clinical T2 (cT2) colon cancer and without suspicion of metastases were included. Failure occurred in two patients due to incomplete resections. Histopathological examination classified seven patients as having pT1, nine as pT2, six as pT3 adenocarcinomas, and three as non-invasive tumors. In three patients, the surgical strategy was changed intraoperatively to conventional colectomy due to tumor location or size. Median length of stay was 1 day. Seven patients had completion colectomy performed due to histological high-risk factors. None had LNM. Conclusions: In selected patients, CELS resection was feasible, and could spare some patients large bowel resection.

Endotheliopathy of trauma has poor outcome. We conducted a RCT investigating 72-hour prostacyclin vs. placebo infusion in 229 shocked trauma patientt. No difference in the mean number of ICU-free days alive < 28 days was found. A significant reduction in hospital stay was found.

- Abstract
Inflammatory bowel diseases (IBD), including Crohn's disease (CD) and ulcerative colitis (UC), affect millions of people worldwide with increasing incidence. Several studies have shown a link between gut microbiota composition and IBD, but results are often limited by small sample sizes. We aimed to re‐analyze publicly available fecal microbiota data from IBD patients. We extracted original fecal 16S rRNA amplicon sequencing data from 45 cohorts of IBD patients and healthy individuals using the BioProject database at the National Center for Biotechnology Information. Unlike previous meta‐analyses, we merged all study cohorts into a single dataset, including sex, age, geography, and disease information, based on which microbiota signatures were analyzed, while accounting for varying technical platforms. Among 2518 individuals in the combined dataset, we discovered a hitherto unseen number of genera associated with IBD. A total of 77 genera associated with CD, of which 38 were novel associations, and a total of 64 genera associated with UC, of which 28 represented novel associations. Signatures were robust across different technical platforms and geographic locations. Reduced alpha diversity in IBD compared to healthy individuals, in CD compared to UC, and altered microbiota composition (beta diversity) in UC and especially in CD as compared to healthy individuals were found. Combining original microbiota data from 45 cohorts, we identified a hitherto unseen large number of genera associated with IBD. Identification of microbiota features robustly associated with CD and UC may pave the way for the identification of new treatment targets.

Anaerobic digestion (AD) has the potential to enhance the availability of nitrogen (N) from green manures; however, part of the sulfur (S) is lost with the biogas. In this study, the N and S availabilities from untreated and digested cover crop (CC) and straw mixtures were measured after application to soil in the laboratory. In addition, a microplot experiment was carried out to assess the N availability in a spring barley crop. A CC consisting of red clover (Trifolium pratense L.) and chicory (Cichorium intybus L.) was harvested and ensiled with barley straw (Hordeum vulgare L.) at different ratios (1:0, 10:1 and 3:1). The silages were either digested alone or co-digested with cattle manure (CM) in continuously fed anaerobic reactors. Six months after soil application, the net inorganic N release was − 7%, 15% and 25% of total N for untreated CC:straw ratios of 3:1, 10:1 and 1:0, respectively. AD significantly increased the net inorganic N release of these plant materials to 5%, 26% and 38% (mono-digestion) and 40%, 41% and 43% (co-digestion with CM). Results from the laboratory experiment corroborated the field experiment in which AD significantly increased the nitrogen fertiliser replacement value (NFRV) of the CC:straw from 19 to 41% of total N applied. Gaseous loss of S up to 30% was observed during AD while the total N content remained unchanged. Sulfate from untreated CC:straw and digestates was immobilised soon after application to soil, resulting in low availability of inorganic S. Digestion of CC and straw significantly reduced N immobilisation but clearly did not contribute to inorganic S supply.

38. The media's coverage and framing of hospital reforms: The case of Germany

- Abstract
Aim: In Germany, a long-lasting debate about the need for a hospital reform, including centralisation, now resulted in reform plans. As cutbacks to essential services such as hospitals are an emotional topic, the public should be properly informed, which is a central role of the media. Based on a media analysis of a large-scale hospital reform in Denmark, showing that the media’s reporting might have had an agenda-setting effect, we analysed the media’s coverage and framing of restructuring the hospital landscape in German newspapers. Subject and methods: A systematic search identified relevant articles in national and regional newspapers. The coverage between January 2018 and January 2023 was analysed regarding quantity and main theme (agenda-setting). Each article’s title and main text were analysed for tone and whether it focused on episodic (single events) or thematic (broader context) framing. Results: A total of 705 newspaper articles (592 regional, 113 national) were included. Both regional and national newspapers mainly used episodic framing. Regional newspapers mostly reported on single closures of hospitals or departments. The tone of the title and main text in national newspapers was mainly negatively depicted, which applied to less than half of the articles in regional newspapers. These differences were, however, not statistically significant. Conclusion: Results indicate higher agenda-setting effects in regional compared to national newspapers. However, the overall high amount of episodic framing can result in less attribution of responsibility to policy-makers, thereby conserving the status quo. To succeed, a media strategy might help to raise problem awareness among the public.

ObjectivePatients with Crohn’s disease (CD) exhibit great heterogeneity in disease presentation and treatment responses, where distinct gut bacteria and immune interactions may play part in the yet unresolved disease aetiology. Given the role of antibodies in the barrier defence against microbes, we hypothesised that gut bacterial antibody-coating patterns may influence underlying disease-mediated processes.DesignAbsolute and relative single and multicoating of gut bacteria with IgA, IgG1, IgG2, IgG3 and IgG4 in patients with CD and healthy controls were characterised and compared with disease activity. IgG2-coated and non-coated taxa from patients with severe CD were identified, profiled for pathogenic characteristics and monitored for enrichment during active disease across cohorts.ResultsPatients with severe CD exhibited higher gut bacterial IgG2-coating. Supervised clustering identified 25 bacteria to be enriched in CD patients with high IgG2-coating. Sorting, sequencing and in silico-based assessments of the virulent potential of IgG2-coated and bulk stool bacteria were performed to evaluate the nature and pathogenicity of IgG2-coated and non-coated bacteria. The analyses demonstrated IgG2-coating of both known pathogenic and non-pathogenic bacteria that co-occurred with two non-coated pathobionts, Campylobacterand Mannheimia. The two non-coated pathobionts exhibited low prevalence, rarely coincided and were strongly enriched during disease flares in patients with CD across independent and geographically distant cohorts.ConclusionDistinct gut bacterial IgG2-coating was demonstrated in patients with severe CD and during disease flares. Co-occurrence of non-coated pathobionts with IgG2-coated bacteria points to an uncontrolled inflammatory condition in severe CD mediated via escape from antibody coating by two gut pathobionts.

41. Effects of light-guided nudges on health care workers' hand hygiene behavior.

- Abstract
Hospital-acquired infections are the most frequent adverse events in health care and can be reduced by improving the hand hygiene compliance (HHC) of health care workers (HCWs). We aimed to investigate the effect of nudging with sensor lights on HCWs' HHC. An 11-month intervention study was conducted in 2 inpatient departments at a university hospital. An automated monitoring system (Sani Nudge
TM ) measured the HHC. Reminder and feedback nudges with lights were displayed on alcohol-based hand rub dispensers. We compared the baseline HHC with HHC during periods of nudging and used the follow-up data to establish if a sustained effect had been achieved. A total of 91 physicians, 135 nurses, and 15 cleaning staff were enrolled in the study. The system registered 274,085 hand hygiene opportunities in patient rooms, staff restrooms, clean rooms, and unclean rooms. Overall, a significant, sustained effect was achieved by nudging with lights in relation to contact with patients and patient-near surroundings for both nurses and physicians. Furthermore, a significant effect was observed on nurses' HHC in restrooms and clean rooms. No significant effect was found for the cleaning staff. Reminder or feedback nudges with light improved and sustained physicians' and nurses' HHC, and constitute a new way of changing HCWs' hand hygiene behavior. • Nudges with lights improved hand hygiene (HH) compliance. • We found an effect of light nudges on nurses' and physicians' HH in patient rooms. • We found an effect of light nudges on nurses' HH in restrooms and clean rooms. • Light nudges did not significantly change cleaning staff's behavior. • Single nudges were as effective in changing HH behavior as combined nudges. Continuous technologic development and updated recommendations for image acquisitions creates a need to update the current normal reference ranges for echocardiography. The best method of indexing cardiac volumes is unknown. The authors used 2- and 3-dimensional echocardiographic data from a large cohort of healthy individuals to provide updated normal reference data for dimensions and volumes of the cardiac chambers as well as central Doppler measurements. In the fourth wave of the HUNT (Trøndelag Health) study in Norway 2,462 individuals underwent comprehensive echocardiography. Of these, 1,412 (55.8% women) were classified as normal and formed the basis for updated normal reference ranges. Volumetric measures were indexed to body surface area and height in powers of 1 to 3. Normal reference data for echocardiographic dimensions, volumes, and Doppler measurements were presented according to sex and age. Left ventricular ejection fraction had lower normal limits of 50.8% for women and 49.6% for men. According to sex-specific age groups, the upper normal limits for left atrial end-systolic volume indexed to body surface area ranged from 44 mL/m
2 to 53 mL/m2 , and the corresponding upper normal limit for right ventricular basal dimension ranged from 43 mm to 53 mm. Indexing to height raised to the power of 3 accounted for more of the variation between sexes than indexing to body surface area. The authors present updated normal reference values for a wide range of echocardiographic measures of both left- and right-side ventricular and atrial size and function from a large healthy population with a wide age-span. The higher upper normal limits for left atrial volume and right ventricular dimension highlight the importance of updating reference ranges accordingly following refinement of echocardiographic methods. - Abstract
The simulation of intrinsic contributions to molecular properties holds the potential to allow for chemistry to be directly inferred from changes to electronic structures at the atomic level. In the present study, we demonstrate how such local properties can be readily derived from suitable molecular orbitals to yield effective fingerprints of various types of atoms in organic molecules. In contrast, corresponding inferences from schemes that instead make use of individual atomic orbitals for this purpose are generally found to fail in expressing much uniqueness in atomic environments. By further studying the extent to which entire chemical reactions may be decomposed into meaningful and continuously evolving atomic contributions, schemes based on molecular rather than atomic orbitals are once again found to be the more consistent, even allowing for intricate differences between seemingly uniform nucleophilic substitutions to be probed.

ABSTRACTBone points were one of the major hunting implements in northern European hunter-gatherer societies. They differ in shapes, types, and manufacturing techniques. In this paper, we investigate 22 bone points from the territory of Lithuania, by studying their morpho-technological characteristics, direct dates, and adhesive residues. The majority are isolated finds, but four points were selected from excavated archaeological sites dated between the 5th and 3rd millennia cal BC. Most of the points belong to the barbed points category, but six slotted points were also studied. Of the 22, 16 previously undated points were sampled for accelerator mass spectrometry radiocarbon (AMS 14C) dating. The results of 10 successfully dated samples are discussed together with previously published 14C dates of bone points from the same region. ATR-FTIR analysis of adhesive residues from six points suggest that birch bark tar was used to haft barbed points and lithic inserts. The results reveal the diversity of types of Early Holocene bone points in the territory of Lithuania, while the slotted and Kunda-type bone points fall into narrow timeframes.

47. Clinical Presentation and Outcome of Sinonasal Extraosseous Plasmacytoma in Denmark: A Nationwide Cohort From 1980 to 2017

Objectives: Extraosseous plasmacytoma (EOP) is a rare plasma cell neoplasm that tends to convert to plasma cell myeloma (PCM) in about 11% to 35% of cases. It has a predilection for the upper respiratory tract, prototypically affecting the nasal cavity and paranasal sinuses. Contemporary first-line treatment is radiotherapy, with more recent studies showing an added benefit of combining radiation with surgery. In this cohort study, we aimed to examine clinical presentation, treatment, and prognosis for all patients nationwide from 1980 through 2017. Furthermore, we determined the size and extension of tumors, investigating the rate at which minimally invasive surgery would have been possible.Methods: Patients were found in the national pathology registry, and all biopsies were collected for pathology review by a hematopathologist. We performed survival statistics for overall survival (OS), progression-free survival (PFS), and the cumulative incidence of conversion to PCM.Results: Twenty-three patients were included. The median age was 65, and patients were primarily men (78%). Tumors were located in either the nasal cavity (57%), maxillary sinus (39%), or sphenoid sinus (4%). In most cases, the tumor was <5 cm (65%) without extension to adjacent structures (60%). The national incidence was 0.02/100,000 person-years, the median symptom duration until diagnosis was 5 months, and none of the patients presented with contiguous spread to regional lymph nodes. Stand-alone radiotherapy was the predominant treatment (61%). In the entire cohort, one patient died from the initial disease, and six patients died from either relapse of EOP or PCM. The 5-year OS, PFS, and conversion rate to PCM were 78%, 56%, and 23%, respectively.Conclusion: SN-EOP responds well to radiotherapy, but relapse and conversion to PCM were not uncommon and entailed a poor prognosis. Most tumors were endoscopically resectable and non-invasive, making the majority of tumors suitable for surgery as an addition to radiation.

48. Antineuronal Autoantibodies in the Cerebrospinal Fluid and Serum From 106 Patients With Recent-Onset Depression Compared With 106 Individually Matched Healthy Control Subjects

No large studies have investigated the prevalence of cerebrospinal fluid antineuronal autoantibodies in isolated depression. In this case-control study comparing 106 patients with isolated depression (ICD-10 code F32) with 106 healthy control subjects, cerebrospinal fluid and serum samples were tested for 7 immunoglobulin G autoantibodies using commercial fixed cell-based assays. To explore validity of methods, positive samples were retested twice by cell-based assays and once by tissue-based assays (monkey cerebellum). The prevalence of any of the antineuronal autoantibodies in cerebrospinal fluid was 0.0% in both groups and the seroprevalence was 0.9% in both groups, based on consistent findings in cell-based assays. However, all samples were negative by the tissue-based assay. Evaluation of antineuronal autoantibodies in cerebrospinal fluid cannot be recommended routinely for patients with isolated depression of moderate severity. Future studies of isolated depression should consider much larger sample sizes and evaluation of antineuronal autoantibodies using modalities other than commercial kits.

49. Measurements of airborne asbestos fibres during refurbishing

- Abstract
Although the use of asbestos fibres in building materials has been prohibited in Norway since 1985, asbestos-containing materials (ACMs) are still found in many buildings. Lack of knowledge and awareness of these materials may lead to exposure during refurbishing. The aim of this study was to investigate the airborne fibre concentration and classify fibres found during the abatement of various ACMs. The release of fibres during short-term work tasks, such as drilling and sawing, was also investigated. Parallel air samples were collected during asbestos abetment of different building materials and analysed with scanning electron microscope (SEM) and phase-contrast microscope (PCM), respectively. Material samples were analysed with SEM. A real-time fibre monitor was used to measure asbestos during short-term work. The highest fibre concentrations were measured for samples collected during the removal of asbestos insulating boards (1.5–4.5 fibres/cm3[f/cm3]), and the numbers were relatively similar for SEM and PCM. A large difference in asbestos concentrations was found between SEM and PCM when analysing floor materials, which were probably caused by a high number of gypsum fibres that the PCM operator counted. Thin fibres (<0.2 µm in width) were included in the SEM count and constituted up to 50% of the total fibre concentration for the asbestos cement materials. The presence of other inorganic and organic fibres on these samples probably led to similar results between SEM and PCM. Short-term work led to peak concentrations above 30 f/cm3.

Teachers' attention and flexibility on referent unit is important to better understand fractions and fractions operations while it is documented that teachers struggle with it. In this study, we explored teachers' different levels of identifying referent unit in a fraction multiplication problem involving a drawn representation. By analyzing data from five pilot interviews with five middle school mathematics teachers, we found out that teachers attended referent unit differently. Moreover, their different levels of mastering referent unit related to what they view as a whole through their thinking. [ABSTRACT FROM AUTHOR]
