El virus del Nilo Occidental es un arbovirus que puede infectar al ser humano y causar una enfermedad neuroinvasiva grave. Tomando como referencia el brote que tuvo lugar en España en 2020, se ha realizado una revisión clínica y de neuroimagen de dicha patología. Para ello, se han recogido datos demográficos, clínicos, analíticos y pruebas de imagen (tomografía computarizada y resonancia magnética) de 30 pacientes diagnosticados de infección por virus del Nilo Occidental en nuestro centro. Las principales manifestaciones clínicas fueron fiebre, cefalea y alteración del nivel de conciencia. Los estudios de neuroimagen, especialmente la resonancia, son de gran importancia para el diagnóstico y seguimiento. Los hallazgos más frecuentes fueron focos de aumento de señal en T2 en el tálamo y tronco del encéfalo, que ilustramos en esta revisión con casos de nuestro centro hospitalario.

A fundamental challenge regarding disordered solids is predicting macroscopic yield—the point at which elastic behaviour changes to plastic behaviour—from the microscopic arrangements of constituent particles. Yield is accompanied by a sudden and large increase in energy dissipation due to the onset of plastic rearrangements. This suggests that one path to understanding bulk rheology is to map particle configurations to their mode of deformation. Here, we subject two-dimensional dense colloidal systems to oscillatory shear, measure the particle trajectories and bulk rheology, and quantify particle microstructure using excess entropy. Our results reveal a direct relation between excess entropy and energy dissipation that is insensitive to the nature of interactions amongst particles. We use this relation to build a physically informed model that connects rheology to microstructure. Our findings suggest a framework for tailoring the rheological response of disordered materials by tuning microstructural properties.

Analizar los datos clínicos y tratar de identificar los factores asociados que pudieran haber influido en la mortalidad postoperatoria de los pacientes de cirugía cardiaca durante su permanencia en la Unidad de Cuidados Críticos de los hospitales de la sanidad pública de la Comunidad Valenciana durante el 2007.

Linking peacebuilding and development is an emerging area of specialisation. Changes in the political, social, and economic contexts, the intangible dimensions of attitudinal and relational change, and the need to take a long-term perspective in order to capture the effects of programming all pose substantial challenges to peacebuilding programming for development agencies. This article provides a series of guiding questions for evaluation which can also be used in the planning and monitoring stages of a peacebuilding or conflictsensitive development programming. 1Drawing upon the work of scholars and practitioners working in the fields of development and peacebuilding, the article presents a process to generate strategic building blocks for a comprehensive approach to evaluating peacebuilding programming.

It is known that a large class of smooth solutions ofCPn models can be constructed starting from holomorphic maps of an algebraic curve into complex projective spaces. Here we apply results from algebraic geometry to describe the energy spectrum and the parameter spaces for such models.

The problem of axisymmetric solutions of the Einstein equations depending on a single real function is re-examined. The solutions are geometrically represented as geodesics, of the hyperbolic plane. Here intuitive geometry can be applied to show that the gravitational fields found by Papapetrou are generalizations of the Weyl ones, under the action of the isometry group of the hyperbolic plane itself.

AimTo examine tumour samples immunohistochemically for MUC1 (episialin), epidermal growth factor receptor (EGFR), and c-erbB-2, since the disruption of the cell-cell adhesion system by MUC1 and the c-erbB oncoprotein family is known to be important in the development of metastasis in human cancers.Methods93 tumour samples from patients with early stage non-small cell lung cancer treated with surgery alone were examined for episialin, EGFR, and c-erbB-2.ResultsEpisialin depolarised expression did not correlate with any of the histo-pathological variables examined (T,N stage, grade, histology, Ki67 proliferation index). No correlation was observed between episialin and EGFR or c-erbB-2 expression. Survival analysis showed that episialin depolarised expression correlated with poor prognosis (p = 0.003), especially in squamous cell cases (p = 0.0003). Episialin expression defined a group of patients with poor prognosis in the node positive category (p = 0.003). In multivariate analysis episialin was the most significant independent prognostic factor (p = 0.007), followed by N stage (p = 0.04).ConclusionsDepolarised expression of episialin is associated with poor outcome in early stage non-small cell lung cancer. Despite the similar activity on the cadherin cell-cell adhesion system, the expression of episialin and c-erbB oncoproteins is likely to be activated within different pathogenic pathways.

In this review paper Einstein equations for axisymmetric vacuum fields are introduced in the form given by Lewis. Following Ernst they are reduced to one complex potential equation. Weyl-type, Schwarzschild, Kerr, Tomimatsu-Sato solutions, and their NUT-like generalizations are then discussed.

Using the null bivector approach, Petrov classification is studied for axisymmetric vacuum space-times with orthogonally transitive Killing vectors. It is shown that the equation on which the classification is based is biquadratic. This excludes that any such space-time can be type III. The only type-N manifolds are the radiative solutions considered by Hoffman. Van Stockum solutions are the most general type-II solutions. Degenerate Weyl solutions and Kinnersley solutions cover typeD. Stationary solutions with functional dependence of the potential are then examined. It is found that, except for special cases, Papapetrou and Lewis solutions are algebraically general.

Among the three basic variational approaches to general relativity, the metric-affine variational principle, according to which the metric and the affine connection are varied independently, is commonly known as the “Palatini method.” In this paper we revisit the history of the “golden age” of general relativity, through a discussion of the papers involving a variational formulation of the field problem. In particular we find that the original Palatini paper of 1919 was rather far from what is usually meant by “Palatini's method,” which was instead formulated, to our knowledge, by Einstein in 1925.

Compact complex manifolds solutions of the Euclidean Einstein equations with a Λ term are examined, through a survey of the literature on the classification of complex surfaces.

We report a case of late-onset postpartum sepsis from endometritis due to group A streptococci (GAS) in a 37-year-old white woman. The patient developed septic shock, with mitral regurgitation and cardiac dysfunction. Early treatment with broad-spectrum antibiotics and hemodynamic support was essential for a favorable outcome. Because of the resurgence of virulent strains of GAS that can cause fatal infections, these pathogens should be included in the differential diagnosis of postpartum infections in the mother. Although cardiac dysfunction is rare in association with GAS infection, it should be ruled out by echocardiography when the condition of a patient with sepsis does not improve.

Coherence therapy, popularly known by the actonym ADFR (Activate, Depress, Free, Repeat), was designed to increase bone mass in osteopenic patients. Accordingly, we report a case of a hypogonadal male with histologically proven low bone turnover osteoporosis and a progressive vertebral fracture syndrome, who responded favorably to ADFR treatment with the use of salmon calcitonin and inorganic phosphate. Dramatic increments in bone mass were observed during a 68-month period of therapy. Serial quantitative computerized tomography demonstrated a 146% increase from baseline in bone mineral density for the first 30 months of treatment, and dual energy radiography yielded a 36.5% increase for the subsequent 31–68 months. Furthermore, no episode of fracture occurred since coherence therapy was initiated.

Lewis solutions of Einstein equations and their generalizations are discussed using the complex-potential formalism. Le soluzioni delle equazioni di Einstein ottenute da Lewis sono discusse, insieme alle loro generalizzazioni, facendo uso del formalismo del potenziale complesso. Обсуждаются решения уравнений Эйнштейна, полученные Льюисом. Рассматриваются обобщения этих уравнений с использованием формализма комплексного потенциала.

We studied 114 girls with Turner syndrome aged from newborn to 30 years old. We observed spontaneous pubertal development and correlation with karyotype, statural growth and bone mineralization. 15% of the patients presented spontaneous telarche, 5% (6 girls) had spontaneous menarche at a mean age of 12 years. One of these patients had stopped menses after 7 years, another one presented oligomenorrhoea. 76% of these patients were above 50% percentile for age following Ranke chart. 23,5% of the patients with spontaneous pubertal development had 45 XO karyotype. Bone mineralization was lower than normal values in all patients with spontaneous pubertal development. There were no correlation between pelvic ultrasonography and clinical signs of pubertal development.We conclude that clinical features are actually the best criteria to start substitute therapy.

59 patients (49 females and 10 males; aged 7 months - 9 years) were studied in the last 13 years for precocious isolated pubarche. We evaluated clinical signs, bone age (B.A.), hormonal findings (ACTH-test, LHRH-test), ultrasound examination of adrenal and gonads. We devided ipatients in four groups:A) 81.3% Idiopathic precocious pubarche (HPP): with normal steroids plasma levels, high DHEAs plasma levels (47.3%), advanced B.A. (29%), advanced statural age (S.A.)(22.9%), normal pelvic ultrasonography.B) 10.1% Late Onset CAH (LOCAH): elevated basal and after ACTH stimulus 17 OH Progesteron plasma levels in all patients, advanced B.A. (83%), advanced S.A. (50%), normal pelvic ultrasonography.C) 6.9% Central Precocious Puberty (CPP): elevated LH plasma levels, advanced B.A. and advanced S.A. in 100%. In all patients large size of gonads were found with pelvic ultrasonography.D) 1 female (1.7%) had adrenal adenomas demonstred by hormonal data and adrenal ultrasound and tomography.In conclusion: HPP is the most frequent cause of premature pubarche, a differential ethiological diagnosis can be easily performed by hormonal and ultrasound investigations.
