Urinary schistosomiasis, caused by Schistosoma haematobium, is an insidious disease of public health concern worldwide, particularly in African countries where Nigeria carries the highest burden of infection. Hence, this study was carried out to determine the prevalence and distribution of Schistosoma haematobium infection among residents of Aye-Oba and Aye-Amodo rural communities of Osun State, Nigeria, using appropriate standard parasitological methods including a total of 427 volunteer individuals. The result showed that out the total 427 participants, 139 (32.6%) had urinary schistosomiasis though no significant difference (p>0.05). Males significantly had higher prevalence of 38.8% compared to females with 25.6% (p<0.05). The age pattern of infection revealed that the highest prevalence of infection (53.1%) was observed among age group 11-20 years while no prevalence was observed among age group 51-60 years. The findings of this study showed that urinary schistosomiasis is prevalent in the two communities, and as such concerted efforts are needed to curb the menace of this neglected tropical disease. Background: Acute phase proteins (APPs), including haptoglobin (Hp), are a large and varied group of plasma proteins that can be used as biomarkers for disease diagnosis/detection/severity. Objective: The main objective was to assess the levels of haptoglobin (Hp) in serum and detect Hp phenotypes using polyacrylamide gel electrophoresis in 100 school-aged children infected with Schistosoma haematobium compared with 60 healthy control. Methods: We conducted a case-control study on 160 schoolchildren (ages 9-15 years) recruited from Tayba Eltejania village, Sinar state, Sudan. Unrelated children with Schistosoma haematobium (case group 100) and unrelated healthy children (control group 60) were included, while those with both Schistosoma types were excluded. The enrolled subjects were evaluated for the levels of Hp and its phenotypes as early markers for disease severity. ELISA quantified biochemical analysis for the serum Hp level. Hp phenotypes were determined, and their frequency was compared between cases and controls. Results: The Hp 2-1 was the highest frequency among cases and controls 72/143 (50.3%), followed by Hp 2-2 (28%), while Hp 1-1 phenotype was 22%. The Hp 2-1 and Hp 2-2 frequency did not differ significantly between cases and controls, considering the Hp 1-1 as the reference group. Multiple comparisons were executed between Hp phenotypes; the differences between these groups were not statistically different. The disease severity was set according to the egg count (Group I: moderate infection ≤ 35/10 ml, Group II: severe infection ≥ 36/10 ml), the Hp 2-2 was four times more frequent in cases with severe infection considering Hp 1-1 as the reference phenotype (OR=3.85, 95% CI: 1.044-14.24). Confirming the result, Hp 2-2 was significantly associated with disease severity than Hp 1-1 and Hp 2-1 (OR= 3.77 95% CI: 1.39-10.20). Background: There is a scarcity of accurate data on Schistosoma haematobium infection on country-specific prevalence despite mass drug administration over the years. Prevalence estimates for the number of people infected and the number at risk of infection must still be made based on calculations of limited prevalence survey data at the country level. This study aimed to fill in the gap of data on prevalence in an urban setting that has been receiving mass drug administration with praziquantel and determine the risk factors associated with S. haematobium infection among school pupils. Method: A cross-sectional, stage-wise random sampling survey of S. haematobium infection, factors influencing its transmission and mass drug administration with praziquantel were studied among primary school pupils in Morogoro Municipality. A semi-structured questionnaire was used to collect data on risk factors, and urine samples were collected from pupils and examined for S. haematobium eggs and macro and microhematuria. Results were analyzed using SPSS version 12.0. Result: The overall prevalence rate of S. haematobium infection was found to be 32.5% (95% CI, - 3.1- 5.6%) in the ten schools that were sampled. It was observed that 228/884 (25.8%) of the pupils had low infection intensity and 82/884 (9.3%) had high infection intensity. The total number of pupils that had S. haematobium infection was 287, where 116 (40.42%) of them had microhematuria. The proportion of students that did receive praziquantel in the last general distribution was found to be 14.3% while 25.8% of the students had low infection intensity and 9.3% had high infection intensity across all age groups. Whereby 3.96% of pupils that received praziquantel in the last general administration also had S. haematobium infection (OR 0.77, 95% CI 0.5-1.2) The risk factors associated with S. haematobium infection were playing, bathing, fishing in rivers and helping parents work in rice fields (p –value<0.001). Conclusion: The prevalence and intensity are high enough to cause re-infection. Still, more effort is needed to enforce mass praziquantel administration among primary school pupils, alternative water sources for recreational activities, provision of proper latrines and further studies needed to explore the risk factors. These intrepid investigators voluntarily exposed themselves to infectious agents for reasons of expediency as well as ethics and watched themselves while disease developed. Barlow, a missionary surgeon and parasitologist of the early 20th century, used this experimental technique on many occasions during a long career in helminthology. In several schistosomiasis-endemic countries, the prevalence has remained high in some areas owing to reinfection despite repeated mass drug administration (MDA) interventions; these areas are referred to as persistent hot spots. Identifying hotspots is critical for interrupting transmission. This study aimed to determine an effective means of identifying persistent hot spots. First, we investigated the differences between Schistosoma haematobium and Schistosoma mansoni prevalence among school-aged children (SAC) estimated by a community-based survey, for which local key informants purposively selected communities, and a randomly sampled school-based survey. A total of 6,225 individuals residing in 60 villages in 8 districts of North Kordofan, Blue Nile, or Sennar States, Sudan participated in a community-based survey in March 2018. Additionally, the data of 3,959 students attending 71 schools in the same 8 districts were extracted from a nationwide school-based survey conducted in January 2017. The communitybased survey identified 3 districts wherein the prevalence of S. haematobium or S. mansoni infection among SAC was significantly higher than that determined by the randomly sampled school survey (e.g., S. haematobium in the Sennar district: 10.8% vs. 1.1%, P<0.001). At the state level, the prevalence of schistosomiasis among SAC, as determined by the community-based survey, was consistently significantly higher than that determined by the school-based survey. Purposeful selection of villages or schools based on a history of MDA, latrine coverage, open defecation, and the prevalence of bloody urine improved the ability for identifying persistent hot spots. Background: The parasite Schistosoma haematobium causes urogenital schistosomiasis, a chronic infectious disease that occurs mainly among school-age children. Objective: The prevalence of S. haematobium infection and level of intensity relative to age, gender and status of selected serum micronutrients among school-age children were investigated in suburban communities in Bekwarra, Nigeria. Methods: This cross-sectional school-based study randomly recruited 353 children aged between 4 and 16 years from five elementary schools between June 2019 and December 2019. We gathered socio-demographic data about each child using a semi-structured questionnaire. Blood samples were collected for micronutrient analysis and urine samples were collected for assessment of S. haematobium infection. Results: A total of 57 (16.15%) school-age children were infected with S. haematobium. Girls (n = 34; 9.63%) were more frequently infected than boys (n = 23; 6.52%). Infection was most frequent among children aged 8–11 years (n = 32; 23.19%) and was significantly associated with age (p = 0.022) and gender (p < 0.001). Serum levels of iron, calcium, copper and zinc among infected children were significantly lower than those of non-infected children. Intensity of infection was negatively associated with iron (r = −0.21), calcium (r = −0.24), copper (r = −0.61; p < 0.001) and zinc (r = −0.41; p < 0.002). Conclusion: This study showed that S. haematobium infection adversely impacted the micronutrient status of school-age children in suburban Nigeria. Measures to lower the prevalence of schistosomiasis among school-age children, including efficient drug distribution, education campaigns and community engagement, are necessary. Chronic schistosomiasis affects either the genitourinary or gastrointestinal tract. Rarely, schistosomes cause ectopic disease, such as in the case of a South African woman from a non-endemic province, who presented with suspected pericardial tamponade because of tuberculosis. However, histology and polymerase chain reaction from pericardial biopsy confirmed Schistosoma haematobium. A finding of mediastinal non-Hodgkin lymphoma came to light when our patient's clinical condition unexpectedly deteriorated. Background: Schistosomiasis or bilharzia is one of the neglected tropical diseases affecting over 230 million people globally. In Zambia, the infection poses as one of the major public health burdens among parasitic infections. The diagnosis of schistosomiasis in endemic countries is still a challenge. The aim of this study was to evaluate the diagnostic performance of the Schistosoma immunochromatographic (ICT) IgG–IgM rapid diagnostic test in the detection of schistosome infections from Schistosoma haematobium and mansoni in Siavonga district in Zambia. Results: The diagnostic performance of Schistosoma ICT IgG–IgM test was evaluated using 430 of the 440 participants' samples which had complete data. The prevalence of S. haematobium and S. mansoni was 4.4% and 6%, respectively, while seroprevalence was 71.4%. The sensitivity and specificity of the Schistosoma ICT IgG–IgM test was found to be 100% and 32.2%, respectively. The associated positive predictive value was 13.8% and 100% for negative predictive value. Diagnostic accuracy [area under the curve (AUC)] was 0.57 [95% confidence interval (CI) 0.52, 0.62]. Conclusions: The Schistosoma ICT IgG–IgM proved to have remarkably high sensitivity with a fairly good specificity and diagnostic accuracy for the detection of both urinary and intestinal schistosomiasis. Global efforts to identify groups at high risk for schistosomiasis have mainly concentrated on identifying their geographical distribution. Investigations on the socioeconomic characteristics of high-risk groups are relatively scarce. This study aimed to explore the associations between schistosomiasis among students and their parents' occupations. A nationwide cross-sectional survey was conducted targeting 105,167 students in 1,772 primary schools across Sudan in 2017. From these students, 100,726 urine and 96,634 stool samples were collected to test for Schistosoma haematobium and S. mansoni infection. A multi-level mixed effect analysis was used with age and sex as fixed factors, and school as a random factor. The odd ratios (ORs) of practicing open defecation among farmers' children were almost 5 times higher than their counterparts whose parents were government officials (OR=4.97, 95% confidence intervals (CIs): 4.57-5.42, P<0.001). The ORs of contacting water bodies for watering livestock among farmers' children were more than 4 times higher than those of children whose parents were government officials (OR=4.59, 95% CIs: 4.02-5.24, P<0.001). Background: Diagnosis of schistosomiasis depends mainly on stool or urine microscopy for Schistosoma egg detection as well as immunoassays. The low sensitivity of these conventional tests makes molecular detection the diagnostic method of choice. The study aimed to detect the molecular prevalence of urine schistosomiasis and evaluate microscopic examination vs. PCR technique for detection of Schistosoma haematobium (S. haematobium) in urine of patients with suggestive symptoms or previous history of urine schistosomiasis coming from endemic regions. Results: This cross-sectional study was performed on eighty patients attending the urology clinic of Sohag University Teaching Hospital from August 2016 to July 2018. Socio-demographic data and clinical data were collected. Urine samples from all study individuals were collected and examined microscopically for S. haematobium eggs as well as detection of S. haematobium DNA of using PCR assay. Microscopic examination and PCR were positive among (68.8%) and (87.5%) of cases, respectively. There was 60% agreement between microscopy and molecular assay. Microscopy was a good test to rule in cases of urine schistosomiasis, with 100% specificity and 100% PPV, but was of limited sensitivity (NPV = 40%) and missed 12.5% of positive cases. Among studied patient variables, only hematuria showed association with urine schistosomiasis with statistical significance. Conclusion: Urine schistosomiasis was highly prevalent in studied population. Considering the high sensitivity and specificity of PCR, it should be implemented as the test of choice, especially in chronic urinary schistosomiasis with low infection setting. Background: Knowledge gaps exist between host genetic factors and susceptibility to schistosomiasis. Objective: This study determined cytokine levels and single nucleotide polymorphisms of tumour necrosis factor (TNF)-α (rs1800629) and interleukin (IL)-10 (rs1800871) and their possible impact on susceptibility to schistosomiasis in preschool-age children in the Madziva area of Shamva district, Mashonaland Central province, Zimbabwe. Methods: Urogenital schistosomiasis was diagnosed using the urine filtration method, while a sandwich enzyme-linked immunosorbent assay was used for cytokine level determination. The survey was done in August 2015 and reinfection levels post treatment were assessed at 3, 6 and 12 months. Amplification refractory mutation system polymerase chain reaction with visualisation on 2% agarose gel electrophoresis was used for genotyping. Results: Schistosomiasis prevalence was found to be 10.5% (59/563). Reinfections were detected in only six children at 3 months and only one was reinfected at 12 months. There were no significant differences in TNF-α-308 G/A allele or genotype frequencies between the Schistosoma haematobium infected participants (p = 0.360) and uninfected participants (p = 0.279). However, no children with the IL-10-819 TT genotype had schistosomiasis. The TNF-α GG genotype corresponded with significantly lower TNF-α levels when compared with the GA or AA genotypes (p < 0.001), and TNF-α levels were significantly lower in infected children compared to uninfected children (p < 0.001). Conclusion: Higher TNF-α levels and lower IL-10 levels are potentially protective against schistosomiasis infection. This study aimed to investigate whether mass drug administration (MDA) intervention has an equivalent effect on reducing the prevalence and intensity of Schistosoma haematobium infection regardless of the baseline values. A repeated cross-sectional survey was performed targeting students of 12 primary schools in Al Jabalain and El Salam districts of White Nile State, Sudan, at both 1 week before and 8 months after the MDA. Prior to the baseline survey, schoolaged children in Al Jabalain had received MDA interventions twice in 4 years, while those in El Salam had not. The baseline prevalence was 9.1% in Al Jabalain and 35.2% in El Salam, which were reduced to 1.8% and 5.5% at 8 months after the MDA, respectively. The corresponding reduction rates were 80.3% and 84.4%, not significant difference between both districts. However, changes in the geometric mean intensity (GMI) of egg counts were significantly different between both districts. The baseline GMIs were 14.5 eggs per 10 ml of urine (EP10) in Al Jabalain and 18.5 EP10 in El Salam, which were reduced to 7.1 and 11.2 EP10 after treatment, respectively. The corresponding reduction rates were 51.0% and 39.5%. In conclusion, MDA interventions were found to bring about similar relative reduction in prevalence regardless of the baseline value; however, the relative reduction in infection intensity was more salient in the district with a low baseline value for both prevalence and intensity. Soil-transmitted helminths and Schistosoma haematobium affect more than 3 billion people globally and mainly occur in sub-Saharan Africa. The present study assessed the overall infection status of a 1716-student cohort of schoolchildren in Zanzibar and applied mass drug administration (MDA) to the cohort from 2007 to 2009. Schools in Pemba, Zanzibar, had a much higher prevalence of soil-transmitted helminth infections than those in Unguja, and the Chaani, Ghana, and Machui schools of Unguja exhibited high S. haematobium infection rates. The MDA program only partially controlled parasite infections, owing to high rates of re-infection. The infection rate of S. haematobium across all 10 schools, for example, was only reduced by 1.8%, and even this change not significant, even though the S. haematobium infection rates of the Chaani and Mzambarauni schools were significantly reduced from 64.4 and 23.4%, respectively, at the first screening, to 7.3 and 2.3% at the last screening. The overall infection rate of Ascaris lumbricoides was reduced from 36.0% at the first screening to 22.6% at the last screening. However, the infection rates for both Trichuris trichiura and hookworm were generally unaffected by MDA. In the future, parasite control programs should involve strategically designed MDA schedules and holistic intervention (e.g., sanitation improvement, hygiene behavior changes, and control of intermediated hosts). A 23-year-old Korean woman with a residence history in Kenya and Malawi for about 2 years presented with gross hematuria for 1 month. Blood tests were within normal range except eosinophilia. Asymmetrically diffuse wall thickening and calcification were observed at the urinary bladder on CT. Multiple erythematous nodular lesions were observed in the cystoscopy and transurethral resection was done. Numerous eggs of Schistosoma haematobium with granulomatous inflammation were observed in the submucosal layer of the bladder. The patient was diagnosed with schistosomiasis- related cystitis and treated with praziquantel (40 mg/kg/day) twice before and after transurethral resection. This case suggests that S. haematobium infection should be considered as a cause of hematuria in Korea when the patient had a history of traveling endemic areas of schistosomiasis. Background: Urinary schistosomiasis is a serious public health challenge in some communities of Ebonyi State, south-east Nigeria, partly resulting from a lack of adequate epidemiological data for the institution of effective control strategies. Objective: This study evaluated the prevalence and risk factors of urinary schistosomiasis in rural communities of Ebonyi State, south-east Nigeria. Methods: A total of 300 students, comprising 185 boys and 115 girls, were randomly selected for the study between July and December 2016. A questionnaire was administered to all participants to determine the risk factors for the disease in the area. Urine specimens collected from the participants were processed by sedimentation and examined microscopically for the eggs of Schistosoma haematobium. Results: The overall prevalence rate for urinary schistosomiasis was 8.0%. Students aged 6–10 years had the highest prevalence of infection (10.3%). The prevalence was significantly higher amongst male students (10.3%; p = 0.038) compared with female students (4.4%). Logistic regression analysis showed a significant association between schistosomiasis infection and freshwater contact activities (p = 0.007; odds ratio = 1.89; 95% confidence interval: 4.33–16.17). Contact with stream, pond, river and well water were associated with infection rates of 25%, 14%, 5.3%, and 4.4%, respectively. Conclusion: A relatively low prevalence of urinary schistosomiasis was found in the area. Participants' socio-economic status and dependence on contaminated water sources were core modifiable risk factors. Health education and development of potable water infrastructure, amongst other interventions, would likely reduce the burden and transmission of urinary schistosomiasis in this locality. Background: Host genetic factors can influence susceptibility, morbidity and mortality from schistosomiasis. The study explored the association between single nucleotide polymorphisms (SNPs) in interleukin-10 (IL-10) and tumour necrosis factor alpha (TNF-a) promoter regions and susceptibility to Schistosoma haematobium infection. Methods: Urine specimens were collected from 361 primary school children aged 5-15 years from schistosomiasis endemic areas of Manicaland and Mashonaland central provinces. Schistosoma haematobium was diagnosed using the urine filtration method. Only 272 participants provided adequate blood for genotyping. Genotyping was performed using the amplification refractory mutation system-polymerase chain reaction. The association between IL-10 and TNF-a SNPs and S. haematobium infection was analysed using the chi-square test. Results: Schistosoma haematobium infection was confirmed in 26.8% of the participants. No significant difference in S. haematobium prevalence between men (51.6% of those infected) and women (48.4%) (X² = 0.008, df = 1, p = 0.928) was observed. The total IL-10 -1082 G, IL-10 -819 C and TNF-a -308G allele distribution between S. haematobium infected and uninfected participants was 50.7% and 51.5% (X² = 0.025, df = 1, p = 0.87), 54.3% and 60.6% (X² = 1.187, df = 1, p = 0.187) and 82.1% and 80.9% (X² = 0.099, df = 1, p = 0.753), respectively, and the differences were not significant. Conclusion: Interleukin-10 -1082 G/A, IL-10 -819 C/T and TNF-a -308 G/A SNPs were not significantly associated with susceptibility to S. haematobium infection. Background: Inadequate water supply and sanitation adversely affects the health and socioeconomic development of communities and places them at risk of contracting schistosomiasis and soil-transmitted helminths (STHs). The aim of this study was to quantify the prevalence and intensity of schistosomiasis (bilharzia) and STHs amongst female school-going pupils in Ugu district. Methods: A descriptive cross-sectional study was conducted in Ugu district amongst primary school pupils from 18 randomly selected schools in 2010. A structured questionnaire was used to collect information on the history and knowledge of bilharzia of 1057 pupils. One stool and 3 consecutive days of urine samples were collected per participant and screened for helminth ova. Findings were compared with those reported by the parasite control programme, which collected data in the same area in 1998. Results: The prevalence of Ascaris lumbricoides and Trichuris trichiura was 25% and 26%, respectively, and their corresponding mean intensities of infection were 21 and 26 eggs per gram. The prevalence of Schistosoma haematobium was 32%, and its mean intensity of infection was 52 eggs per 10 mL urine. Of the pupils, 60% knew about schistosomiasis, 9% reported red urine in the past week and 22% had had dysuria before. Although the prevalence of ascariasis and trichuriasis had decreased since 1998 (62% and 59%, respectively), the prevalence of schistosomiasis had increased to 32% (p < 0.05). Conclusion: Female pupils in rural schools remain at risk. There have been some reports on schistosomiasis of school children in Sudan's Nile River basin area; however, information about the infection status of Schistosoma species and intestinal helminths among village residents of this area is very limited. Urine and stool samples were collected from the 1,138 residents of the Al Hidaib and Khour Ajwal villages of White Nile State, Sudan in 2014. The prevalence of overall schistosomiasis and intestinal helminthiasis was 36.3% and 7.7%, respectively. Egg positive rates were 35.6% for Schistosoma haematobium, 2.6% for S. mansoni, and 1.4% were mixed. The prevalence of schistosomiasis was significantly higher in men (45.6%) than in women (32.0%), in Khou Ajwal villagers (39.4%) than in Al Hidaib villagers (19.2%), and for age groups ≤15 years old (51.5%) than for age groups >15 years old (13.2%). The average number of eggs per 10 ml urine (EP10) of S. haematobium infections was 18.9, with 22.2 eggs in men vs 17.0 in women and 20.4 in Khou Ajwal villagers vs 8.1 in Al Hidaib villagers. In addition to S. mansoni eggs, 4 different species of intestinal helminths were found in the stool, including Hymenolepis nana (6.6%) and H. diminuta (1.0%). Collectively, urinary schistosomiasis is still prevalent among village residents in Sudan's White Nile River basin and was especially high in men, children ≤15 years, and in the village without a clean water system. Schistosomiasis is prevalent in Nigeria, and the foremost pathogen is Schistosoma haematobium, which affects about 29 million people. Single dose of the drug praziquantel is often recommended for treatment but the efficacy has not been documented in certain regions. Therefore, this study was designed to assess the impact of single dose praziquantel treatment on S. haematobium infection among school children in an endemic community of South-Western Nigeria. Urine samples were collected from 434 school children and 10 ml was filtered through Nucleopore filter paper before examination for egg outputs by microscopy. The prevalence was 24.9% at pre-treatment. There was no statistically significant difference for the prevalence of infection between males (14.7%) and females (10.2%), although the mean egg count for the females (9.87) was significantly more (P<0.05) than the males (6.06). At 6 and 12 months post-treatment there was 74.4% and 86.4% reduction in the mean egg count, respectively. Interestingly, an increased prevalence of infection from 2.1% at 6 months to 7.7% at 12 months post-treatment was observed, nonetheless the mean egg count was reduced to 0.27 at 12th month from 1.98 at 6 months post-treatment. Resurgence in the prevalence rate between 6 and 12 months post-treatment with praziquantel is herein reported and the need for a follow-up treatment in endemic areas for adequate impact on schistosomiasis control is discussed. Urinary schistosomiasis is a tropical infection with a high endemicity in the developing countries and is included in the list of “Neglected Tropical Diseases”. It is caused by a parasitic worm, Schistosoma haematobium, and it has come into the spotlight as a major cause of urogenital disease. Furthermore, it is linked to bladder cancer and it is a predisposing factor for HIV/AIDS. Schistosoma mansoni and Schistosoma haematobium are intravascular, parasitic flatworms that infect >250 million people in 70 developing countries, yet not all people of the same community and household are afflicted. Regarding laboratory rodents, mice but not rats are susceptible to infection with S. mansoni and hamsters but not mice are entirely permissive to infection with S. haematobium . A recent Brazilian publication has demonstrated that resistance of the water-rat, Nectomys squamipes to S. mansoni infection might be ascribed to stores of arachidonic acid (ARA)-rich lipids in liver. Several reports have previously shown that ARA is a safe and effective schistosomicide in vitro , and in vivo in mice, hamsters and in children . Schistosoma haematobium appeared more sensitive than S. mansoni to ARA in in vitro and in vivo experiments. Accordingly, it was proposed that ARA increased levels might be predominantly responsible for natural attrition of S. mansoni and S. haematobium in resistant experimental rodents. Therefore, the levels of ARA in serum, lung, and liver of rats (resistant) and mice (susceptible) at 1, 2, 3, 4 and 6 weeks after infection with S. mansoni cercariae and between mice (semi-permissive) and hamster (susceptible) at 1, 2, 3, 4, and 12 weeks after infection with S . haematobium cercariae were compared and contrasted. Neutral triglycerides and ARA levels were assessed in serum using commercially available assays and in liver and lung sections by transmission electron microscopy, Oil Red O staining, and specific anti-ARA antibody-based immunohistochemistry assays. Schistosoma haematobiuminfection is reported to facilitate the development of urogenital diseases. Its symptoms include haematuria, dysuria and tiredness, and it may cause cognitive decline in children. The prevalence ofS. haematobiuminfection needs to be known in endemic areas and a mass treatment programme against the disease implemented. The aim of this study was to investigate the prevalence and intensity ofS. haematobiuminfection in ILembe and uThungulu health districts, using the major symptom, haematuria, as an indicator. A total of 6 265 urine samples, from 96 rural schools, was collected for analysis using dipsticks. The prevalence of haematuria in the ILembe health district was 37% (95% CI, 35–39%) for boys and 39% (95% CI, 37–41%)for girls. The prevalence of haematuria in the uThungulu health district was 56% (95% CI, 53–59%) and 53% (95% CI, 50–56%) for girls and boys, respectively. Light-intensity infection was the most common infection level in both health districts. A negative relationship was observed between prevalence and altitude (r = −0.262,p = 0.009); whereas, we found a slight, though significant, positive association with mid-summer temperatures (r = 0.234,p = 0.021). Background:Schistosomiasis remains a public health burden in South Africa, particularly in KwaZulu-Natal. The study aimed to identify the risk factors for transmission ofSchistosoma haematobiumamong school-going children in the Ndumo area of uMkhanyakude district, KwaZulu-Natal. Methods:A cross-sectional study involving 320 school-going children, aged 10–15 years, was conducted in 10 local primary schools in the Ndumo area, from May to June 2015. Data were collected using a structured questionnaire based on socio-demographic information, sanitation and water access, recreational, occupational activities, and knowledge about bilharzia. A filtration technique was used to detectS. haematobiumeggs in 10 ml of urine. A Chi square test, bivariate and logistic regressions were performed to assess the association between variables. Odds ratios were used to determine the strength between significant predictors with 95% confidence interval andpvalue < 0.05. Results: From the 320 participants, 120 (37.5%) were positive forSchistosoma haematobiuminfection.The risk factors associated with schistosomiasis were age, household head, poor sanitation, access to water source and knowledge about schistosomiasis. Conclusion:The Ndumo area is considered a moderate zone for schistosomiasis endemicity according to the World Health Organisation (WHO) classification. The objectives of the study were to determine the prevalence of urinary schistosomiasis in the different communities, the intensity of infection, prevalence among gender and age groups and to relate infection with parent's occupation. A study on the prevalence of urinary schistosomiasis was conducted in four villages around Kiri Lake in Shelleng Local Government Area, Adamawa State, Nigeria. Two hundred and thirty two urine samples were collected from four primary school children randomly selected from within the four study communities. Overall, prevalence of urinary schistosomiasis was 48% (111/232), with males recording 49% (69/142) and females 47% (42/90). The total mean egg count (MEC) was 8.3. There was no significant difference in prevalence between males and females (P>0.05). Prevalence was higher among age groups, with the 13-15 year old age group having the highest 62.96% (17/27) and the 4-6 year old age group had the least 37% (19/52). There was no statiscally significant difference in prevalence among the different age groups (P>0.05). Infection was also high among children of fishermen 59.09% (13/22), followed by farmers' children 56.25% (45/80) and the least prevalence was among children of teachers 20% (3/15). Old Banjiram had the highest infection of 91% (21/23), while Kwadadai had the least 36.8% (21/57) (P<0.05). Old Banjiram and children in the 10-12 years age group had the highest mean egg count of 10.6 and 9.4 respectively. The study reveals a high prevalence of urinary schistosomiasis. And therefore portable water source should be made functional in all communities surrounding the lake. While rehabilitation and repair of the existing water borehole system in the community should be effected as well as drilling new additional boreholes to serve their water needs. Schistosoma haematobium is a biocarcinogen of human urinary bladder (UB). The present study investigated developing UB cancer mouse model by injecting S. haematobium eggs into the bladder wall and introduction of chemical carcinogens. Histopathological findings showed mild hyperplasia to epithelial vacuolar change, and high grade dysplasia. Squamous metaplasia was observed in the S. haematobium eggs+NDMA group at week 12 but not in other groups. Immunohistochemistry revealed significantly high expression of Ki-67 in urothelial epithelial cells of the S. haematobium eggs+BBN group at week 20. The qRT-PCR showed high expression of p53 gene in S. haematobium eggs group at week 4 and S. haematobium eggs+BBN group at week 20. E-cadherin and vimentin showed contrasting expression in S. haematobium eggs+BBN group. Such inverse expression of E-cadherin and vimentin may indicate epithelial mesenchymal transition in the UB tissue. In conclusion, S. haematobium eggs and nitrosamines may transform UB cells into squamous metaplasia and dysplasia in correlation with increased expression of Ki-67. Marked decrease in E-cadherin and increase in p53 and vimentin expressions may support the transformation. There is renewed interest in natural products as a starting point for discovery of drugs for schistosomiasis. Recent studies have shown that phytol reveals interesting in vivo and in vitro antischistosomal properties against Schistosoma mansoni adult worms. Here, we report the in vitro antischistosomal activity of phytol against Schistosoma haematobium juvenile and adult worms and alterations on the tegumental surface of the worms by means of scanning electron microscopy. The assay, which was carried out with 6 concentrations (25, 50, 75, 100, 125, and 150 μg/ml) of phytol, has shown a promising activity in a dose and time-dependent manner. There was a significant decline in the motility of the worms and a mortality rate of 100% was found at 48 hr after they had been exposed to phytol in the concentration of 150 μg/ml. Male worms were more susceptible. On the ultrastructural level, phytol also induced tegumental peeling, disintegration of tubercles and spines in addition to morphological disfiguring of the oral and ventral suckers. This report provides the first evidence that phytol is able to kill S. haematobium of different ages, and emphasizes that it is a promising natural product that could be used for development of a new schistosomicidal agent. Summary Schistosomiasis and malaria are two common parasitic diseases that are co-endemic in resource-poor communities of sub-Saharan Africa. This study aims to assess the effects of single and concomitant Plasmodium falciparum and Schistosoma haematobium infections on two indicators of renal injury in school children in a rural community of Nigeria. A cross-sectional epidemiological survey was carried out on a total of 173 schoolchildren between ages 6 and 18 years (mean age 11.4 ± 2.6 years). Urine and blood samples were collected by standard methods for concurrent microscopic diagnosis of S. haematobium and P. falciparum infections. Urinary blood (hematuria) and protein were determined using a urinalysis dipstick. The prevalence of single infections was 75.1% and 78.2% for S. haematobium and P. falciparum , respectively. A total of 57.1% individuals were infected with the two parasites. The prevalence of hematuria was significantly higher in the co-infection status (63.8%) than in single S. haematobium (52.2%) and P. falciparum (43.7%) infection statuses ( p = 0.04), while no significant variation was recorded in proteinuria in the three infection statuses ( p = 0.53). The proportion of children with renal injury associated with the co-infection of these parasites is very high, particularly in young children, who seem to have a higher prevalence of hematuria. Schistosomiasis is a trematodes infection that causes significant morbidity and mortality in the Middle East, particularly in Yemen. Despite its impact, testicular schistosomiasis is still viewed as an uncommon illness. We present a 15-year-old child with left testicular pain and fever for three days diagnosed as left epididymo-orchitis based on the clinical and ultrasonographical findings. The patient was treated empirically with intravenous antibiotics alongside supportive care without improvement after one week. Ultrasonography follow-up showed that the left testis was heterogeneously enlarged without intratesticular blood flow, suggesting left testicular torsion. Immediate scrotal exploration was performed, revealing a dark, congested, and 180° twisted left testicle. Thus, an orchiectomy was performed. The specimen's histology revealed calcified ova of Schistosoma haematobium. In conclusion, even though testicular schistosomiasis is rare, clinicians should provide a thorough clinical evaluation and histopathological analysis for resected specimen, particularly in high-risk patients. The objective of this study was to carry out a community survey on schistosomiais and soil-transmitted helminth (STH) infections in order to suggest feasible and effective intervention strategies in Lake Victoria basin, Tanzania. A total of 37 communities selected from 23 districts of the 4 regions in the Lake Victoria basin of Tanzania were involved in the study. From each of the selected locality, 50 adult community members, 25 males and 25 females, were recruited for the study. Each study participant was requested to submit stool and urine specimens. From each stool specimen, duplicate Kato-Katz thick smears were prepared and microscopically examined for Schistosoma mansoni and STH eggs. Urine specimens were processed by the filtration technique and microscopically examined for Schistosoma haematobium eggs. Ultrasound examination for morbidity due to schistosomiasis was performed. Mass treatment was done using praziquantel and albendazole for schistosome and STHs infections, respectively. Out of 1,606 adults who provided stool specimens, 199 (12.4%) were positive for S. mansoni, 349 (21.7%) for hookworms, 133 (8.3%) for Ascaris lumbricoides, and 33 (2.0%) for Trichuris trichiura. Out of 1,400 participants who provided urine specimens, 25 (1.8%) were positive for S. haematobium eggs. Because of the co-endemicity of these afflictions and their impact on vulnerable population groups, the helminthiasis could be simultaneously treated with 2 drugs, praziquantel for schistosomiasis and albendazole for STHs. The objectives of this study was to conduct a survey on schistosomiasis and soil-transmitted helminth (STH) infections in order to come up with feasible control strategies in Lake Victoria basin, Tanzania. Depending on the size of the school, 150-200 schoolchildren were recruited for the study. Duplicate Kato-Katz stool smears were prepared from each child and microscopically examined for Schistosoma mansoni and STHs. Urine specimens were examined for Schistosoma haematobium eggs using the filtration technique. After the survey, mass drug administration was done using praziquantel and albendazole for schistosomiasis and STHs infections, respectively. A total of 5,952 schoolchildren from 36 schools were recruited for the study and had their stool and urine specimens examined. Out of 5,952 schoolchildren, 898 (15.1%) were positive for S. mansoni, 754 (12.6%) for hookworms, 188 (3.2%) for Ascaris lumblicoides, and 5 (0.008%) for Trichuris trichiura. Out of 5,826 schoolchildren who provided urine samples, 519 (8.9%) were positive for S. haematobium eggs. The results revealed that intestinal schistosomiasis, urogenital schistosomiasis, and STH infections are highly prevalent throughought the lake basin. The high prevalence of intestinal and urogenital schistosomisiasis in the study area was a function of the distance from Lake Victoria, the former being more prevalent at localities close to the lake, whilst the latter is more so away from it. Control of schistosomiasis and STHs in the study area requires an integrated strategy that involves provision of health education to communities, regular treatments, and provision of adequate safe water supply and sanitation facilities. The genetic diversity of Schistosoma haematobium remains largely unstudied in comparison to that of Schistosoma mansoni. To characterize the extent of genetic diversity in S. haematobium among its definitive host (humans), we collected S. haematobium eggs from the urine of 73 infected schoolchildren at 5 primary schools in White Nile State, Sudan, and then performed a randomly amplified polymorphic DNA marker ITS2 by PCR-RFLP analysis. Among 73 S. haematobium egg-positive cases, 13 were selected based on the presence of the S. haematobium satellite markers A4 and B2 in their genomic DNA, and used for RFLP analysis. The 13 samples were subjected to an RFLP analysis of the S. haematobium ITS2 region; however, there was no variation in size among the fragments. Compared to the ITS2 sequences obtained for S. haematobium from Kenya, the nucleotide sequences of the ITS2 regions of S. haematobium from 4 areas in Sudan were consistent with those from Kenya (> 99%). In this study, we demonstrate for the first time that most of the S. haematobium population in Sudan consists of a pan-African S. haematobium genotype; however, we also report the discovery of Kenyan strain inflow into White Nile, Sudan. Objective To investigate the effect of pre-existing Schistosoma haematobium ( S. haematobium ) infection on malaria disease severity. Methods The study involved the use of twenty-five imprinting control region mice, fifteen of which were initially infected with S. haematobium . Five of the remaining ten schisto-uninfected mice together with five schisto-infected mice were infected with Plasmodium berghei ( P. berghei ) after four weeks (acute stage) of schistosoma infection. The remaining five schisto-uninfected mice together with five schisto-infected mice were also infected with P. berghei after seven weeks (chronic stage) of schistosoma infection. The last five schisto-infected mice were used as control group. They were then monitored for changes in P. berghei parasitaemia on Days 3, 5, 7, 9 and 11 post-infection. Records on their survivability were also taken. Results The co-infected mice had significantly higher malaria parasitaemia, compared with the mono-infected mice during acute S. haematobium infection. In contrast, the co-infected mice had significantly lower malaria parasitaemia during chronic S. haematobium infection and a higher survival rate. Conclusions Co-infection of mice with P. berghei during acute S. haematobium infection resulted in rapid P. berghei development and increased malaria parasitaemia. However, the co-infection resulted in slower P. berghei development and decreased malaria parasitaemia with enhanced survivability of the mice during chronic S. haematobium infection. Schistosoma haematobium is one of the most prevalent parasitic flatworms, infecting over 112 million people in Africa. However, little is known about the genetic diversity of natural S. haematobium populations from the human host because of the inaccessible location of adult worms in the host. We used 4 microsatellite loci to genotype individually pooled S. haematobium eggs directly from each patient sampled at 4 endemic locations in Africa. We found that the average allele number of individuals from Mali was significantly higher than that from Nigeria. In addition, no significant difference in allelic composition was detected among the populations within Nigeria; however, the allelic composition was significantly different between Mali and Nigeria populations. This study demonstrated a high level of genetic variability of S. haematobium in the populations from Mali and Nigeria, the 2 major African endemic countries, suggesting that geographical population differentiation may occur in the regions. Endogenous viral elements (EVEs) are widely distributed throughout eukaryotic genomes, and their evolution and potential function have attracted a lot of interest. Draft genome sequences for Schistosoma mansoni, Schistosoma japonicum and Schistosoma haematobium are now available; however, information about EVEs in blood flukes of the genus schistosoma is scanty. Here, genome-wide survey into the putative EVE sequences of the three key schistosome genomes were present. Totally 4, 117 gene sequences were identified, including retrovirus-like gypsy elements, RNA viruses and dsDNA viruses. Compared with S. japonicum and S. haematobium, S. mansoni appeared to greatly outnumbered by gypsy members. Phylogenetic analysis revealed one novel endogenous retrovirus element in S. mansoni. This initial characterization of schistosomes showed that schistosomes harbour distinct EVEs that may have played an important evolutionary role. Abstract: Objective: The study evaluates the prevalence of urogenital schistosomiasis and diagnostic performance of chemical reagent strips used for disease diagnosis in preschool-aged children (≤5 years) in Nigeria rural communities. Patients and methods: Urine samples from 419 children were observed microscopically for Schistosoma haematobium and screened for hematuria using standard urine chemical reagent strips. Results: Prevalence and intensity of infection were 9.8% and 14.4 eggs/10 ml of urine, respectively. Prevalence of infection was similar in girls (10%) and boys (9.6%) (p > 0.05). The intensity of infection was higher in boys (17.1 eggs/10 ml of urine) than in girls (12.8 eggs/10 ml of urine); however, this was not gender dependent (p > 0.05). The occurrence of hematuria was not associated with gender (p > 0.05), but was associated with prevalence of infection (p < 0.05). Conclusion: Infection with S. haematobium occurs early in life in the communities and although intensity of infection is low, it could have serious implications in disease transmission. Initial laboratory workup revealed hypereosinophilia (1910 cells/mm SP 3 sp ) and elevated levels of total serum IgE (332 IU/ml), with normal kidney function and levels of electrolytes and serum complement C3 and C4. Abstract: Schistosomiasis is a parasitic disease caused by blood flukes (Trematodes) of the genus Schistosoma (S.). It is well documented that schistosomiasis haematobium was endemic in Ancient Egypt. Infection was diagnosed in mummies 3000, 4000 and 5000years old. Scott was the first to describe the pattern of schistosomiasis infection in Egypt. Schistosomiasis haematobium was highly prevalent (60%) both in the Nile Delta and Nile Valley South of Cairo in districts of perennial irrigation while it was low (6%) in districts of basin irrigation. Schistosoma mansoni infected 60% of the population in the Northern and Eastern parts of the Nile Delta and only 6% in the Southern part. Neither S. mansoni cases nor its snail intermediate host were found in the Nile Valley South of Cairo. The building of the Aswan High Dam -which was completed in 1967 – did not cause any increase in schistosomiasis prevalence. In 1990, a study conducted in nine governorates of Egypt confirmed the change in the pattern of schistosomiasis transmission in the Delta. There was an overall reduction in S. mansoni prevalence while Schistosoma haematobium had continued to disappear. In Middle and Upper Egypt there was consistent reduction in the prevalence of S. haematobium except in Sohag, Qena, and Aswan governorates. However, foci of S. mansoni were detected in Giza, Fayoum, Menya and Assiut. All schistosomiasis control projects implemented in Egypt from 1953 to 1985 adopted the strategy of transmission control and were based mainly on snail control supplemented by anti-bilharzial chemotherapy. In 1997, the National Schistosomiasis Control Program (NSCP) was launched in the Nile Delta. It adopted morbidity control strategy with Praziquantel mass treatment as the main component. In 1996, before the NSCP, 168 villages had S. mansoni prevalence >30%, 324 villages 20–30% and 654 villages 10–20%. Abstract: Schistosomiasis is a debilitating disease affecting approximately 600 million people in 74 developing countries, with 800 million, mostly children at risk. To circumvent the threat of having praziquantel (PZQ) as the only drug used for treatment, several PZQ derivatives were synthesized, and drugs destined for other parasites were used with success. A plethora of plant-derived oils and extracts were found to effectively kill juvenile and adult schistosomes, yet none was progressed to pre- and clinical studies except an oleo-gum resin extracted from the stem of Commiphora molmol, myrrh, which action was challenged in several trials. We have proposed an essential fatty acid, a component of our diet and cells, the polyunsaturated fatty acid arachidonic acid (ARA) as a remedy for schistosomiasis, due to its ability to activate the parasite tegument-bound neutral sphingomyelinase, with subsequent hydrolysis of the apical lipid bilayer sphingomyelin molecules, allowing access of specific antibody molecules, and eventual worm attrition. This concept was convincingly supported using larval and adult Schistosoma mansoni and Schistosoma haematobium worms in in vitro experiments, and in vivo studies in inbred mice and outbred hamsters. Even if ARA proves to be an entirely effective and safe therapy for schistosomiasis, it will not prevent reinfection, and accordingly, the need for developing an effective vaccine remains an urgent priority. Our studies have supported the status of S. mansoni calpain, glutathione-S-transferase, aldolase, triose phosphate isomerase, glyceraldehyde 3-phosphate dehydrogenase, enolase, and 2-cys peroxiredoxin as vaccine candidates, as they are larval excreted-secreted products and, contrary to the surface membrane molecules, are entirely accessible to the host immune system effector elements. We have proposed that the use of these molecules, in conjunction with Th2 cytokines-inducing adjuvants for recruiting and activating eosinophils and basophils, will likely lead to development and implementation of a sterilizing vaccine in a near future. Abstract: Objective: To assess if microhaematuria and proteinuria as measured by reagent strips could estimate intensity of Schistosoma haematobium (S. haematobium) infection in endemic areas and evaluate their screening performance among children in Benue State, Nigeria. Methods: A total of 1 124 urine samples were collected, screened for microhaematuria and proteinuria using reagent strips (Combi 9) and results were compared to filtration technique, the gold standard method. Results: A significant correlation was observed between microhaematuria (rho= 0.66, P<0.01), proteinuria (rho = 0.71, P<0.01) and intensity of S. haematobium eggs. Proteinuria had sensitivity of 95.7% and specificity of 67.2%, while microhaematuria had sensitivity of 64.8% and specificity of 89.6%. The proportion of false positive diagnoses was higher in proteinuria (19.2%) than microhaematuria (6.0%). Conclusions: The findings suggest that use of urine reagent strips could potentially estimate intensity of S. haematobium infection and their performance to screen urinary schistosomiasis agreed with previous observations. Abstract: Objective: To determine secreted antischistosoma antibodies in urine and to discern the epidemiological situation of schistosomiasis in the agricultural field labourers''camps city in the Gezira State-central Sudan. Methods: Total of 66 urine and 66 serum paired samples were collected from those who confirmed parasitologically positive and negative with schistosomiasis from the two camps. Samples were tested using ELISA technique to measure and compare the immunoglobulin G (IgG) levels in serum and urine samples of schistosomiasis patients. Results: The overall prevalence of S. mansoni and S. haematobium was 53.8% and 15.4%, while the intensity were (2.04 GMEC) and (0.9 GMEC) respectively. The relative percentage of positive IgG individulas in urine was 92.40% where as 96.97% in serum. Statistically no significant difference between the IgG levels in serum and urine samples was observed. Conclusions: This study shows that the detection of secreted IgG antibodies in urine can substitute serum for diagnosis of schistosomiasis. [Copyright &y& Elsevier]

There is increasing evidence of an association between female genital Schistosoma haematobium infection and HIV. In KwaZulu-Natal, we aimed to explore girls' water contact practice and to determine whether a study exclusively on girls would be manageable and welcomed. Three primary schools that had participated in a parasite control programme eight years prior were approached. Subject to consent, girls aged 9 to 12 years were interviewed on water-body contact, symptoms and household composition. Urine samples were analysed for S. haematobium infection eggs. Good dialogue was achieved in all schools and 95% consented to participation; 43% had an S. haematobium infection, geometric mean intensity 10.5 ova per 10 ml urine. Only 12% had ever been treated for S. haematobium. Water-body contact was significantly associated with S. haematobium (OR 2.8, 95% CI 1.3-5.9, p= 0.008); however, S. haematobium was also found in 20% of girls who claimed to never have had water-body contact. Sixty-four percent thought they had no choice but to use unprotected water; 21% had no mother in the household, and being an orphan increased the risk of having S. haematobium. The community welcomed the study. Prevalence levels in South Africa are so high that some communities are eligible for WHO-recommended regular mass treatment. [ABSTRACT FROM AUTHOR]
