2. Reliability of countermovement jump estimation using the Chronojump jump mat: intra-session and within-session.

- Abstract
Vertical jump is a fundamental metric for monitoring and regulating lower body capacities, especially in assessing sports performance through the countermovement jump (CMJ). In recent years, various instruments aimed at estimating vertical jump heights have emerged. However, ensuring effective performance monitoring requires that jump mats prove consistency in measuring jumps across repeated tests, i.e., they must prove reliability. This study focuses on evaluating the intra-session and within-session test-retest reliability of the Chronojump jump mat in highly trained female volleyball players. Ten athletes from the Spanish Superliga 2 league participated in 100 CMJs over two sessions spaced a week apart. A repeated measures design collected jump height data using Chronojump jump mat. The protocol included a 10-min warm-up, a 5-min rest, and the execution of 5 CMJs with 2 min of rest between trials. Intra-session test-retest consistency was assessed by analyzing consecutive pairings of the first five trials. The study reveals moderate noise for SEM (1.56 cm) and standardized SEM (0.37), accompanied high SDC (4.33 cm) and SWC (0.44 cm). Correlation analysis indicated very high reliability (ICC =0.89), high concordance (CCC = 0.82) and a moderate CV (5.97%). Regarding within-session reliability, no significant differences were observed (Paired t-test p = 0.08; Hedges effect size g = 0.09). Additionally, very high correlations between both sessions were observed (r = 0.86). Absolute reliability analysis revealed a noise of 1.65 cm (SEM), resulting in high SDC (4.59 cm) and SWC (0.47 cm). Relative reliability, assessed through correlation coefficients, displayed very high values (ICC = 0.89 and CCC =0.89), although a moderate standardized SEM of 0.44 was observed. The Bland-Altman plot indicated systematic errors of the mean of 0.41 cm without substantial dispersion. Linear regression analyses between sessions showed a high correlation (r = 0.86), with a systematic error of 6.65 cm (intercept) and a random error of 2.01 cm (SEE). To sum up, the Chronojump jump mat proves reliability in measuring CMJ in female volleyball players across both intra-session and within-session contexts. 4. Within-session reliability and criterion validity of ADR Jumping photocell in the estimation of CMJ.

- Abstract
Vertical jump height is a key measure of lower limb power and performance in many sports and athletic activities. Accurate measurement of vertical jump height is essential to monitor progress, set goals, and design training programs. Besides, new more economical and manageable instruments are emerging. However, it is necessary to know if these devices can measure with the required precision and accuracy. This study examines the concordance (criterion validity) between a new portable photocell ADR jumping and the jump mat Chronojump. Additionally, the within-session test-retest reliability (internal consistency) of CMJ was studied. Twelve highly trained female volleyball players (age: 21.7 ± 2.9 years; height: 1.73 ± 0.06 m; body mass 64.3 ± 7.6 kg; body mass index (BMI): 16.6 ± 2.5 kg/cm2: training experience 8.5 ± 1.8 y). Following a standardized warm-up and a resting period of 5 minutes, 10 CMJs were executed, resting for 2 minutes for each attempt. All records were collected simultaneously by the ADR jumping photocell and the Chronojump Jump mat to compare data from both devices. The within-session test-retest reliability was evaluated using the first three jumps of all participants. The agreement between devices was near-perfect (r = 0.95; CCC = 0.94; SEM = 1.47 cm; Regression Slope=0.99), However, significant differences were detected between instruments (0.81 cm p < 0.05, ES = 0.2). Within-session reliability was slightly higher in ADR jumping (ICC = 0.93; CCC = 0.90; SEM = 1.3 cm; CV = 4.89%) than Chronojump (ICC = 0.91; CCC = 0.87; SEM = 1.6 cm; CV = 6.0%). Furthermore, sensitivity was higher for ADR jumping (SWC = 0.37 cm) than for the Chronojump (SWC = 0.45 cm). The noise of the measurement (SEM) was greater than the worthwhile change (SWC), indicating a lack of capacity to detect meaningful changes. In conclusion, the ADR jumping photocell is a valid instrument when compared to the Chronojump jump mat in the measurement of CMJ. In addition, ADR jumping shows high internal consistency values, therefore, can be considered a reliable device for the within-session test-retest in the measurement of CMJ in highly trained female athletes. Purpose: Amelogenesis imperfecta (AI) is a hereditary condition which affects the composition and structure of enamel in terms of hypoplasia and/or hypomineralization. The condition severely affects patients facing such difficulties as hypersensibility, loss of tooth substance and poor aesthetics. The objective is to perform a systematic review of patient-reported outcome measures (PROMs) in patients with amelogenesis imperfecta. Methods: Inclusion criteria were articles written in English, including PROMs from patients with amelogenesis imperfecta. The databases PubMed, Scopus and Web of Science were searched on April 27, 2022, and eligible articles were screened. Exclusion criteria were articles based on proxy reports and single case reports. Results: 405 studies were screened in terms of title and abstract, with 31 articles eligible for full-text screening, resulting in a total of 11 articles eligible for inclusion, (articles including 4–82 patients). The content was analyzed, resulting in the outcome divided into seven domains: Oral Health-Related Quality of Life (OHRQoL), Dental fear, Esthetics, Psychosocial factors, Function, Dental hypersensitivity, and Treatment outcome. Conclusion: The limited quantity of research on PROMS from patients with AI indicates a significant impact of OHRQoL and daily life. A large variety of approaches have been presented in the articles. Patients report concerns of esthetics, hypersensitivity, function, and a general impact on well-being and social interaction. This highlights the importance for the need of early dental treatment. Starting from biographical evidence about the role of disability within the Austen family, this article investigates how Jane Austen's narrative is pervaded by an ongoing tension between the healthy and active body and the ill and sick one. Specifically, it studies how this tension reaches its climax in her last works, Persuasion ( IBIS ) and Sanditon ( IBI7, published in I925), and results in a polemical rejection of hypochondria. In these two novels, Austen is extremely critical of imaginary patients who use their illness or disability as a justification for their own indolence, while also exalting the authentically sick and fragile who face their illness with pride and dignity. This attitude is particularly explicit in Sanditon, the highly satirical unfinished novel written during the last months of her life, in which satire and irony seem to function as a reaction against the illness that is killing the author, an extreme attempt to ridicule her fatal disease and affirm her agency against it. The binary opposition between the "selfindulgent" and the "genteel" invalid seems to exclude the representation of those who, like Austen's own brother George, are affected by a condition that makes them dependent upon the care of others. 8. The efficacy of sensory nerve coaptation in DIEP flap breast reconstruction – Preliminary results of a double-blind randomized controlled trial.

Sensory nerve coaptation has great potential to restore sensation after autologous breast reconstruction. However, blinded and randomized studies are lacking. We therefore present the preliminary results of our ongoing double-blinded randomized controlled trial that compares sensory recovery of innervated versus non-innervated DIEP flaps. Patients who underwent DIEP flap breast reconstruction between July 2019 and February 2022 were included and randomized. The anterior cutaneous branch of the second or third intercostal nerve was coapted. Pre- and postoperative sensory testing was performed with Semmes-Weinstein Monofilaments, Pressure Specified Sensory Device, and a thermostimulator, for tactile and temperature thresholds. This interim analysis comprised 41 patients contributing 29 innervated and 38 non-innervated breasts. At 24 months of follow-up, the mean monofilament value of the flap skin was lower in innervated than in non-innervated flaps (4.48 vs. 5.20, p = 0.003). Touch thresholds were lower the center of the innervated flaps (47.8 vs. 71.2 g/mm
2 , p = 0.036), and heat pain was more often imperceptible in non-innervated flaps (42.1% vs. 10.3%, p = 0.004). No adverse events were associated with sensory nerve coaptation. These preliminary results indicate superior sensibility and recovery of protective sensation in innervated compared with non-innervated DIEP flaps. Although the results of the completed trial must be awaited to establish the full clinical impact, including highly anticipated quality of life outcomes, we encourage continuation of scientific and clinical efforts in this promising technique. • Sensibility is an important outcome after autologous breast reconstruction. • DIEP flaps can be innervated by coaptation of intercostal nerve branches. • Innervated DIEP flaps have improved postoperative sensibility. • Protective sensation more often returns in innervated DIEP flaps. • Nerve coaptation is a meaningful and relevant addition to breast reconstruction. If we are to move beyond the instrumentalization of nature-as-resource, we must develop ways of being in the world that challenge anthropocentric perspectives that tend to reduce nature to its utilitarian value. This essay is an exploration of the imagination as one such means by which we can move beyond a vision of nature as something that is to be catalogued, classified or otherwise “improved” – a perspective that tends to frame the environmental crisis as primarily a set of problems requiring solutions that can be fully articulated through empirically verifiable modalities of calculative rationality. The imagination is herein articulated as the “organ of perception” (Goethe) capable of giving expression to otherwise insensible phenomena and expanding the realm of relationality to include the more-than-human. A uniquely “telluric” imagination will be articulated as an ethical response to the current ecological moment that moves beyond models of the imagination as being merely mimetic, productive or parodic (Kearney). It differentiates itself from these subjective modalities by emphasizing the processrelational aspect of the imagination as emergent from the space between subject and object. The essay concludes with a brief examination of Adam Dickinson’s “Metabolic Poetics” as an expression of the telluric imagination that responds to the “invitation” at the heart of the environmental crisis not by providing solutions, but by gesturing beyond sensible nature towards the latent possibilities therein. Due to the expansion of the human issues to the text, various manifestations of emotionality are structured or hidden, unconscious movements and subjective reactions to the world are revealed in the text. The individual becomes the basis for the development of the typology of the emotional images. Such images become public, turn to the carriers of genre, and form narrative matrices. The text becomes a particularly sensitive field, a tuning fork, which allows the readers to correlate their own emotions with previous, already experienced, and tested literature. Endowed with powerful cultural and historical meanings, built up and enriched in the perspective of epochs, emotions become a fluid basis for comparisons of human images at the transcultural and transnational levels. They form the tertium comparationis, within which there are a selection and birth of new images of emotion, and thus, the properties of artistic writing, measurements of the sensibility of the text to extraneous trends and inclusions, modern approaches to the interpretation of the tradition. As a result of this longevity, the world of emotions is internalized into history as a memory of the past, which lives in the present and has its internal temporality and cultural stage. However, there is always a constant "theoretical" excess in the variety of manifestations of sensibility, which serves as the basis for their comparisons, assimilations, and distributions. It has no historical layers; it is the criterion and measure of the meeting of even distant images and models on the border, in the field of tertium comparationis. The study of the emotional world of the work involves correlation with the circulation of ideas, worldviews, and aesthetic forms in the melting pot of culture. In this semiotic device, the theory of cultural transfer is formed. It allows going beyond local comparative studies to the level of globalization expansion of objects of interpretation. The founder of transfer analysis M. Espan' formulated this methodology as the output of literary criticism on the other side of comparative. P. Kulish's prose is a very interesting phenomenon in the context of this theory. The author resorted to a special technique of intertextual interaction, palimpsest as "embedding" and too sensitive attitude to tradition, rethinking at a cultural distance in fundamentally different historical and literary conditions. 13. Taste and art in Elias' Theory of the Civilizing Process.

The aim of this paper is to systematize the main ideas on "taste" and "art" in the writings of German sociologist Norbert Elias. Even though Elias's work is mainly known for his theory on "The civilizing process," he developed substantial ideas on problems that were later associated mainly with Bourdieusian sociology: distinction, taste, art, status. Following Elias's insights, the paper argues that "taste" should be understood as the expression of sensibilities that arise from networks of interdependence (or, as he called them, "figurations") that are situated in particular historical contexts. These sensibilities, thus, are historically contingent and sociologists should study them observing their variations over long periods of time. The paper analyses the key texts were Elias developed these ideas, above all his books "The Court Society" and "Mozart: Portrait of a Genius", and his essays on kitsch, German lyric poetry, and African art. 16. FREEDOM IN THE ARCHIVE: On Doing Philosophy through Historiography.

It is argued in this article that Foucault's most distinctive contribution to philosophical practice is to be found in his distinctive mode of taking up historiography, exploring critically the conditions and limits of knowledge through archival work. The focus on knowledge would seem to place him in the critical lineage of Kant; however, his appeal to history and archival explorations reconfigure the relation between sensibility and the understanding in a way that suggests a different concern with the conditions of "a possible knowledge." After discussing how Foucault's archival work engages a distinct sensibility, I suggest that his concern with knowledge can best be understood not within a transcendental project, however historicized, but within the logical space of Hegel's Doctrine of the Concept, specifically the discussion of the Idea of cognition where it is a question of the twin processes of unifying subjectivity and objectivity as expressive of cognition's "urge to truth." Understood within this conceptual context, Foucault's distinctive archival approach to what he calls "subjectivation" and "objectivation" allows us to appreciate the distinction between knowledge and truth in a way that leaves open the possibility of transformation. Amit Chaudhuri is a multi-talented writer of contemporary Indian writings in English. He is a literary critic, essayist, novelist, poet and also a great singer. He is celebrated as an important star of contemporary Indian writers in English. His writings are discernible, observable which strikes and vibrate the heart and mind of the readers. He has given a noted importance for the Indian values in his writings. His works reflect the Indian values coated in Bengali sensibility. Chaudhuri works rebound the Bengali culture, ethnicity and regional sensibility of Calcutta. Amit Chaudhuri has also explored the vitality of Indian culture through his novels and embellished the tradition of Indian English novels with his creative innovations. Chaudhuri's novels have provided a new perspective on the theoretical models of the Indian domesticity. The domestic settings of his novels illustrate the material history of the Indian home, associated with religious, social and cultural practices, he also exhibits how these are adopted and reworked by the protagonists. His artistic perception and innovative vision assist him to change the conventional concepts of Indian English fiction. The region is a mere setting or background of his works. It plays an important role in the development of the story and characterization. Objective We developed a Tag Array chip for detecting first- and second-line anti tuberculosis drug resistance in pulmonary tuberculosis and compared the analytical performance of the gene chip to that of phenotypic drug susceptibility testing (DST). Methods From November 2011 to April 2016.234 consecutive culture–confirmed, clinically and imaging diagnosed patients with pulmonary tuberculosis from Southwest Hospital, Chongqing were enrolled into the study. Specimens collected during sputum or bronchoalveolar lavage fluid from the pulmonary tuberculosis patients were subjected to M. tuberculosis species identification and drug-resistance detection by the Tag Array gene chip, and evaluate the sensitivity and specificity of chip. Results A total of 186 patients was diagnosed drug-resistant tuberculosis. The detection of rifampicin (RFP), isoniazid (INH), fluoroquinolones (FQS), streptomycin (SM) resistance genes was highly sensitive and specific: however, for detection of amikacin (AMK), capreomycin (CPM), Kanamycin (KM), specificity was higher, but sensitivity was lower. Sensitivity for the detection of a mutation in the eis promoter region could be improved. The detection sensitivity of the EMB resistance gene was low, therefore it is easy to miss a diagnosis of EMB drug resistance, but its specificity was high. Conclusion Tag Array chip can achieve rapid, accurate and high-throughput detection of tuberculosis resistance in pulmonary tuberculosis, which has important clinical significance and feasibility. In addition, it aims to prove that Wordsworth, under the tremendous influence of 'Pantheism', 'French Revolution', "Rousseau's philosophy' and 'Godwin's outlook', took his tenets from the deep-rooted convictions of the day and gave them the authenticity of personal experience and vitality of the poetic expression. As a matter of fact, Wordsworth regarded himself with Coleridge as a philosophical poet and his philosophy according to his confessions, was hewn out of his own experiences and entitled him to the position of teacher of society which he was anxious to achieve and maintain. 25. Long-term sensory disturbances after orbitozygomatic fractures.

Summary Background Orbitozygomatic fractures often lead to infraorbital nerve (ION) injury, and affected sensibility is a common long-term complaint within this patient group. We present a long-term follow-up study where the validated von Frey filament system was used for testing ION sensibility. Furthermore, we examined the incidence of persistent nerve injury and whether more complex fractures led to more pronounced ION sensibility disturbances. Methods Patients treated for facial fractures involving the orbitozygomatic complex were included and the follow-up time was 3 years or more. Depending on the location and severity of the fractures, the patients were divided into 4 groups. The patients answered a questionnaire before ION sensibility testing with von Frey filaments. Results Eighty-one patients were examined: 65 males (80%) and 16 females (20%). Examinations were conducted between 3.0 and 7.6 years (mean 4.9 years) after injury. Sixteen patients (20%) had affected and 6 patients (7.4%) had severely affected ION sensibility according to von Frey testing. No statistically significant differences were found in terms of questionnaire score between the groups. There was also no statistically significant correlation between questionnaire results and log von Frey values. Although the effect of groups could not be statistically verified using the log von Frey values, a larger proportion of patients with complex fractures had higher log von Frey values than the other groups. Conclusions Patients with complex fractures report more permanent sensory disturbance of the ION after surgery than those with isolated orbitozygomatic fractures, although this could not be verified statistically with von Frey filament testing at several locations. Hence, a validated method for testing facial sensibility such as von Frey filaments, although sensitive, is inadequate to determine all aspects of sensory malfunction after orbitozygomatic fractures. This suggests that the patient's experience of long-term sensation after trauma may not be correlated with objective measures. [ABSTRACT FROM AUTHOR]

In order to analyze the sealing performance of a new designed premium threaded connection, this paper studies the sealing reliability and sensitivity of the premium threaded connection at its most dangerous load condition based on Kriging model. The load condition of premium threaded connection in deep well is so complex that the mos dangerous load point of premium threaded connection under the typical series loads is studied by finite element analysis (FEA) method firstly. Since the FEA of the premium threaded connection is too time-consuming, Kriging model which is accurate and efficient for multidimensional and highly nonlinear problems is built to calculate the sealing reHability of the premium threaded connection at the mos dangerous load point. Finally, the sealing sensitivity analysis of the premium threaded connedton is performed by the method of variance-based global sensitivity analysis. 31. The Pittsburgh sleep quality index as a screening tool for sleep dysfunction in clinical and non-clinical samples: A systematic review and meta-analysis.

This review appraises the process of development and the measurement properties of the Pittsburgh sleep quality index (PSQI), gauging its potential as a screening tool for sleep dysfunction in non-clinical and clinical samples; it also compares non-clinical and clinical populations in terms of PSQI scores. MEDLINE, Embase, PsycINFO, and HAPI databases were searched. Critical appraisal of studies of measurement properties was performed using COSMIN. Of 37 reviewed studies, 22 examined construct validity, 19 - known-group validity, 15 - internal consistency, and three - test-retest reliability. Study quality ranged from poor to excellent, with the majority designated fair. Internal consistency, based on Cronbach's alpha, was good. Discrepancies were observed in factor analytic studies. In non-clinical and clinical samples with known differences in sleep quality, the PSQI global scores and all subscale scores, with the exception of sleep disturbance, differed significantly. The best evidence synthesis for the PSQI showed strong reliability and validity, and moderate structural validity in a variety of samples, suggesting the tool fulfills its intended utility. A taxonometric analysis can contribute to better understanding of sleep dysfunction as either a dichotomous or continuous construct. [ABSTRACT FROM AUTHOR]

33. Tears and the manly sailor in England, c . 1760–1860.

This article explores the apparent paradoxes in representations of the British Jack Tar in order to reconsider the ways in which his figure came to embody idealised attributes of manliness in the period 1760–1860. Naval historians have revealed that images of sailors were closely related to ideas about class, sex, and nation, identifying distinctive types of Jack Tar such as the drunken carouser, the fatalistic, fearless but simple Tar, and the domesticated family man. Notions of heroism changed over the period too. The officer was the hero in the eighteenth and early nineteenth centuries and notions of valour only extended to the lower ranks by the mid-nineteenth century. Thus the picture is of the ordinary tar emerging as manly hero once his character was scrubbed clean of its less salubrious aspects. This article argues that there were not diverse and distinct Jack Tars successively climbing the ladder of morality over time. The tar was not sanitised and civilised, but was given feelings. In particular, tears identified the sailor as a man of feeling as well as a fearless fighter, stressing his tenderness, fidelity, duty, and self-sacrificing love for his brother sailor, his ‘Poll’, and his nation. But he still loved grog, dance, and girls. The Jack Tar was thus the everyman, which aided his popularity and democratised the manly hero. [ABSTRACT FROM PUBLISHER]

Résumé La prise en charge d’une intoxication repose avant tout sur l’anamnèse et sur l’identification d’un toxidrome à partir de l’évaluation clinique et biologique. Les analyses toxicologiques peuvent compléter l’évaluation clinico-biologique. La Société de toxicologie clinique (STC) a proposé une liste restreinte de molécules pouvant être dosées en urgence dans le sang, pour lesquelles une relation effet-concentration a été décrite en lien avec le pronostic. Les techniques de dépistage sont réalisées en première intention par immunochimie et permettent de détecter la présence de molécules ou famille de molécules. Ces techniques ont l’avantage d’être rapides mais leurs performances analytiques doivent être considérées en fonction de leur sensibilité et de leur spécificité (possibles réactions croisées). Les résultats obtenus en dépistage sont généralement vérifiés par des techniques plus spécifiques comme la chromatographie couplée à la spectrométrie de masse qui permet d’identifier et de quantifier précisément les molécules. Aujourd’hui, de nouvelles molécules apparaissent quotidiennement sur le « marché », avec une accessibilité simplifiée sur Internet. L’identification et la quantification de ces nouvelles molécules constituent un nouveau défi pour les laboratoires. Summary Acute care of intoxication is based on a clinical and biological evaluations and toxidrome identification. The prescription of toxicological analyses can complete clinical examination and standard biological analyses. The STC (Clinical toxicology society) recommend quantifying the drugs for which a relation between concentration and effect/prognosis is established. The STC published a limited list of drugs following these criteria. First lines of analyses could be performed by screening techniques based on immunochemistry methods. Screening methods are fast but lack of sensibility and specificity. Results are mostly checked by methods with high level of precision such as chromatography method associated with mass spectrometry detection which allows identification and quantification. 36. Sensibility of a new instrument to assess clinical reasoning in post-graduate orthopaedic manual physical therapy education.

Sound application of clinical reasoning (CR) by the physical therapist is critical to achieving optimal patient outcomes. As such, it is important for institutions granting certification in orthopaedic manual physical therapy (OMPT) to ensure that the assessment of CR is sufficiently robust. At present, the dearth of validated instruments to assess CR in OMPT presents a serious challenge to certifying institutions. Moreover, the lack of documentation of the development process for instruments that measure CR pose additional challenges. The purpose of this study is to evaluate the sensibility of a newly developed instrument for assessing written responses to a test of CR in OMPT; a 'pilot' phase that examines instrument feasibility and acceptability. Using a sequential mixed-methods approach, Canadian OMPT examiners were recruited to first review and use the instrument. Participants completed a sensibility questionnaire followed by semi-structured interviews, the latter of which were used to elaborate on questionnaire responses regarding feasibility and acceptability. Eleven examiners completed the questionnaire and interviews. Questionnaire results met previously established sensibility criteria, while interview data revealed participants' (dis)comfort with exerting their own judgment and with the rating scale. Quantitative and qualitative data provided valuable insight regarding content validity and issues related to efficiency in assessing CR competence; all of which will ultimately inform further psychometric testing. While results suggest that the new instrument for assessing clinical reasoning in the Canadian certification context is sensible, future research should explore how rater judgment can be utilized effectively and the mental workload associated with appraising clinical reasoning. 37. Writing Pain: Sensibility and Suffering in the Late Letters of Anna Seward and Mary Robinson.

'Writing Pain' argues that Anna Seward's Letters (1811) and Mary Robinson's letters (1800) create alternative models of sensibility from the suffering poet of Charlotte Smith's Elegiac Sonnets. Immensely popular, Smith's sonnets made feminine suffering a source of poetic agency by aestheticizing and privatizing it. However, despite their sincerity, her sonnets effaced the physical, nervous body of sensibility on which Seward's and Robinson's early poetic reputations had depended and for which they had been mocked. The popularity of Smith's model made it an important model for women poets, but, by the end of the eighteenth century, sensibility was also associated with sickness and artifice. For Seward and Robinson, who wanted to build their literary reputations but were living with disabled bodies, Smith's example needed to be reimagined to account for the reciprocity of body and mind as they struggled to write through pain. 40. Breast sensibility after bilateral risk-reducing mastectomy and immediate breast reconstruction: A prospective study.

Summary: The demand for risk-reducing mastectomy (RRM) to avoid breast cancer has increased over the last several years, and knowledge of the outcome after this prophylactic surgical procedure is important. The primary aim of this study was to prospectively compare breast sensibility before and after RRM in a consecutive series of women. The study also investigated whether the nipples were less numb if the nipple areola complexes (NACs) were spared compared with regrafted nipple tips. Forty-six women who selected bilateral RRM with immediate reconstruction using implants at the Karolinska University Hospital, Solna, Stockholm, Sweden, were included in the study. The median patient age at the time of surgery was 39 years (range 26–58). All patients were evaluated preoperatively and at least 2 years postoperatively (median 29 months). Tactile, thermal and nociceptive cutaneous sensibilities were studied with quantitative techniques. The patients at the postoperative evaluation completed a questionnaire about subjective feelings in both breasts. The results showed that breast sensibility is significantly impaired after RRM. Additionally, the ability to experience sexual sensations in the breast is often lost. An NAC-sparing surgery did not result in better nipple sensibility. [Copyright &y& Elsevier]

The article defines indie sensibility as the combination of aesthetic, stylistic and emotional characteristics employed by filmmakers who have transitioned from the independent genre to mainstream film producing. The author suggests how to study American independent cinema in the post-2008 era, by evaluating the impact of neoliberalism on independent filmmaking. Also discussed is an overview of the rise of independent cinema, its convergence with mainstream filmmaking and its decline.

45. Xavier Bichat's Medical Thought in the Historical Context of French Vitalism.

The French vitalism is different from vitalism in general. It is a position of some physiologists who worked from the end of the 18th century to the early days of the 19th century, defending the peculiarity of life phenomena in contrast to the Cartesian theory of the animal-machine. Its main representatives are Paul-Joseph Barthez and Théophile de Bordeu, who belonged to the vitalist school of Montpellier. They argue, in contrast to mechanism, that life involves a special principle and cannot be explained in terms of physical and chemical properties alone. Marie François Xavier Bichat (1771-1802), inheriting this position, endeavored to establish physiology as a science which cannot be reduced to the physical sciences. He was also the first to introduce the notion of tissues as distinct entities. The aim of his concept of physiology is to explain the whole of life phenomena through the ultimate properties of tissues, that is, through sensibility and contractility. After Bichat, François Magendie inherited his experimental concepts, but critiqued his vitalism. Claude Bernard, known as the founder of experimental physiology, was influenced considerably by Bichat's idea of physiology. Through the notion of tissues, he unites zoology, botany and medicine in the domain of general physiology. Other topics mentioned include the championing of the senses over reason in this period by French philosophers such as Jean-Jacques Rousseau; modern views on the pain and suffering of others, and calling into question all pretensions to privilege and power in the atmosphere of the French revolution.

Summary: Background: The free anterolateral thigh flap (ALT) is now used as a ‘workhorse flap’. The donor site morbidity is thought to be minimal, although most evidence derives from questionnaire-based studies rather than rigorous objective clinical assessment. In particular, robust quantitative data on thigh sensibility and quadriceps function is lacking. The aim of this study was to provide a comprehensive clinical assessment of donor site morbidity. Methods: We performed a prospective study of consecutive free ALT perforator flaps performed at Salisbury Foundation Trust between March 2008 and April 2010. The donor site was assessed at six months including a questionnaire (symptoms, function), scar analysis (Vancouver Scar Scale, VSS), and evaluation of quadriceps power and lateral thigh sensibility (compared with the contralateral unoperated thigh). Results: One hundred ALT flaps were performed on 97 patients (mean age 46.8 years). The donor site was closed directly in 88 cases and using a split skin graft in 12 cases. At follow-up (n = 68), tingling was the most common reported symptom (59%), whereas pain, itching and muscle herniation were reported infrequently. Donor site scars were mostly flat, pale and soft but widened. Pathological scarring was rare. Sensibility was reduced in donor thighs (p < 0.001) and correlated with flap width but peak quadriceps contraction was similar between donor and unoperated thighs. Conclusions: There was a high throughput and diverse application of ALT flap reconstructions during the study period. Reduced sensibility was common around the scar but rarely troublesome. Quadriceps function was not affected even when dissection of intramuscular perforators was required. The ALT is a versatile flap that can deliver a large skin paddle with minimal impact on thigh function. [ABSTRACT FROM AUTHOR]

Introduction: to do a review of the bibliography published in statistical methodology specialized journals about the procedures developed to estimate the prevalence and performance of clinical diagnostic tests. Methods: this review includes 52 articles about procedures developed to estimate the prevalence and performance of clinical diagnostic test parameters found in journals specialized in statistical methodology applied to health areas. Results: three important statistical problems were found: the presence of a number of parameters higher than the pieces of information available to do the estimation procedure, known as lack of the ability to identify, the presence of individuals with negative outcome in the screening tests for comparison, whose actual health conditions are unknown (verification bias) and the dependence between test results. Conclusions: a wide range of possibilities to address the three problems that characterized the estimation problem was found. To address the verification bias problem many alternatives consider the use of the latent variables and others use different parameters. Many strategies to address the lack of the ability to identify and the dependence between test outcomes were found too. Most of the proposed alternatives are characterized by their complexity and difficult practical applicability. [ABSTRACT FROM AUTHOR]
