La 18F-Fludarabine a montré des résultats prometteurs dans l’évaluation des pathologies lymphoprolifératives chez l’homme et dans l’étude du myélome multiple chez les modèles murins. Afin d’évaluer son intérêt dans le myélome multiple chez l’être humain, nous menons une étude prospective multicentrique exploratoire évaluant la TEP à la 18F-Fludarabine (FludaTEP) en comparaison à la TEP au 18F-FDG (TEP-FDG) pour le bilan initial ou de rechute et l’évaluation de la réponse thérapeutique des patients porteurs de myélomes multiples (MM) symptomatiques.

The therapeutic landscape of multiple myeloma (MM) has evolved spectacularly over the past decade with the discovery and validation of proteasome inhibitors and immunomodulatory agents as highly active agents, both in front-line therapy as well as in the relapse and maintenance settings. Although previous attempts to apply available monoclonal antibodies (Mabs) to the treatment of patients with MM has until recently been disappointing, novel targets specifically explored in the context of MM have recently lead to the first approvals of Mabs for the treatment of patients with MM. We have performed a literature search to identify preclinical targeting of MM, including in vitroand in vivomodels using monoclonal antibodies, as well as clinical trials of monoclonal antibodies in patients with MM. Sources used were peer-reviewed publications, congress abstracts and on-line clinical trials data (such as clinicaltrials.gov). Several targets have been evaluated in preclinical models and a growing number of agents are being evaluated in clinical trials, as single agents or in combination and under various antibody formats. Two agents, targeting for the first time CD38 and SLAMF7, respectively, have recently been approved for the treatment of patients with MM. The recent approval of these two antibodies is expected to have a strong impact on treatment modalities and outcome in patients with MM, including both transplant eligible and elderly patients.

La TEP-FDG, examen de référence pour le bilan initial et l’évaluation thérapeutique du MM, présente néanmoins certaines limites laissant place au développement de nouveaux radiopharmaceutiques. Le récepteur CXCR4 étant surexprimé dans le MM, nous conduisons une étude clinique prospective de phase II évaluant la pertinence de la TEP au 68GaPentixaFor (TEP-PentixaFor) en comparaison avec la TEP-FDG pour le bilan initial et l’évaluation thérapeutique des patients MM symptomatiques en première ligne ou en première rechute (NCT05321862).

High-dose therapy with autologous stem cell transplantation (HDT–ASCT) has been considered to be the standard frontline treatment for younger, fit patients with multiple myeloma (MM) since the 1990s. Efforts continue to optimize the use of HDT–ASCT with the aim of improving outcomes. One strategy has been the incorporation of novel agents (thalidomide, lenalidomide and bortezomib) in the pre-transplantation setting as an induction therapy or in the post-transplantation setting as a consolidation or maintenance therapy. Given their high response rates, three-drug induction therapy regimens (for example, bortezomib–thalidomide–dexamethasone, lenalidomide–bortezomib–dexamethasone and cyclophosphamide–bortezomib–dexamethasone) are now the standard of care. Thalidomide and bortezomib are well suited for consolidation therapy, and regimens using these agents can improve the depth of response following HDT–ASCT. Lenalidomide is particularly well suited for long-term maintenance therapy following HDT–ASCT, and initial results are promising and have shown improvements in disease outcomes such as progression-free survival and overall survival in some cases, although a low incidence of second primary malignancies have been observed. Further studies are needed to determine the optimal regimen and duration of induction therapy, the impact of maintenance on overall survival and the safety of long-term treatment. Many of the studies currently underway in MM will help address these aspects.

Le myélome multiple (MM) est un cancer de la moelle osseuse caractérisé par une survie à 5 ans de 54 %. Très peu d’études s’intéressent à la prédiction de la survie des patients atteints de MM et encore moins à partir de biomarqueurs par imagerie. Les méthodes de Cox et de Kaplan–Meier sont les plus utilisées dans le contexte de la survie. Cependant, les Random Survival Forest(RSF), une méthode d’ensemble d’arbres, peut fournir un procédé plus robuste pour l’analyse de la survie et n’est pour le moment pas étudié dans le cadre du MM et de l’imagerie TEP. Nous proposons une méthode pour la prédiction de la progression dans le contexte du MM en utilisant des RSF avec la méthode de sélection par « variable importance » (VIMP) (Ishwaran et al., Annals of Applied Statistics, 2008).

Deux études prospectives (Bartel, Blood 2009 ; Zamagni, Blood 2011) ont montré la valeur pronostique de plus de 3 lésions focales (FL), de la SUVmax et des lésions extra-médullaires (EMD) sur la TEP-FDG réalisée au diagnostic chez des patients atteints de MM. L’intérêt potentiel du volume métabolique total (tMTV) et de la glycolyse totale (tTLG) a également été rapporté (McDonald, Clin Cancer Res 2016). Le but de cette étude est d’évaluer la valeur pronostique de métriques d’hétérogénéité (TF) déterminés sur la TEP-FDG au diagnostic chez 134 patients inclus dans l’étude prospective multicentrique IMAJEM (Moreau, ASH 2015).
