Carpal fractures of bones other than the scaphoid occur at a much lower rate than scaphoid fractures. The close relationship between the carpus, intrinsic and extrinsic wrist ligaments, and wrist kinematics makes a thorough history, clinical examination, and interpretation of imaging for carpal malalignment essential. Carpal malalignment should be addressed with reduction and fixation. Nondisplaced fractures are often treated nonoperatively and displaced intraarticular fractures are almost always treatment operatively. The physician should keep in mind the athlete's specific goals and needs. Treatment must be individualized. Options for early return to play should be discussed when possible. [ABSTRACT FROM AUTHOR]

Objective: The present quantitative study aimed to assess the three-dimensional (3-D) cartilage wear patterns of the first metacarpal and trapezium in the advanced stage of osteoarthritis (OA) and compare cartilage measurements with radiographic severity.Design: Using 19 cadaveric trapeziometacarpal (TMC) joints, 3-D cartilage surface models of the first metacarpal and trapezium were created with a laser scanner, and 3-D bone surface model counterparts were similarly created after dissolving the cartilage. These two models were superimposed, and the interval distance on the articular surface as the cartilage thickness was measured. All measurements were obtained in categorized anatomic regions on the articular surface of the respective bone, and we analyzed the 3-D wear patterns on the entire cartilage surface. Furthermore, we compared measurements of cartilage thickness with radiographic OA severity according to the Eaton grading system using Pearson correlation coefficients (r).Results: In the first metacarpal, the cartilage thickness declined volarly (the mean cartilage thickness of the volar region was 0.32 ± 0.16 mm, whereas that of the dorsal region was 0.53 ± 0.18 mm). Conversely, the cartilage evenly degenerated throughout the articular surface of the trapezium. Measurements of the categorized regions where cartilage thinning was remarkable exhibited statistical correlations with radiographic staging (r = -0.48 to -0.72).Conclusions: Our findings indicate that cartilage wear patterns differ between the first metacarpal and trapezium in the late stage of OA. Carpal fractures are exceedingly rare clinical entities and are often associated with concomitant injuries. In this review, we focus on fractures of the carpus, excluding the scaphoid, and provide an update on the current consensus as to mechanism, diagnosis, management, outcomes, and complications after such injuries. Purpose: In thumb carpometacarpal osteoarthritis, current evidence suggests that degenerative, bony remodeling primarily occurs within the trapezium. Nevertheless, the pathomechanics involved and the most common sites of wear remain controversial. Quantifying structural bone morphology characteristics with high-resolution computed tomography CT (micro-CT) infer regions of load transmission. Using micro-CT, we investigated whether predominant trabecular patterns exist in arthritic versus normal trapeziums. Methods: We performed micro-CT analysis on 13 normal cadaveric trapeziums and 16 Eaton stage III to IV trapeziums. We computationally divided each specimen into 4 quadrants: volar-ulnar, volar-radial, dorsal-radial, and dorsal-ulnar. Measurements of trabecular bone morphologic parameters included bone volume ratio, connectivity, trabecular number, and trabecular thickness. Using analysis of variance with post hoc Bonferroni/Dunn correction, we compared osteoarthritic and normal specimen quadrant measurements. Results: No significant difference existed in bone volume fraction between the osteoarthritic and normal specimens. Osteoarthritic trapeziums, however, demonstrated significantly higher trabecular number and connectivity than nonosteoarthritic trapeziums. Comparing the volar-ulnar quadrant of osteoarthritis and normal specimens collectively, this quadrant in both consistently possessed significantly higher bone volume fraction, trabecular number, and connectivity than the dorsal-radial and volar-radial quadrants. Conclusions: The significantly greater trabecular bone volume, thickness, and connectivity in the volar-ulnar quadrant compared with the dorsal-radial and dorsal-ulnar quadrants provides evidence that the greatest compressive loads at the first carpometacarpal joint occur at the volar-ulnar quadrant of the trapezium, representing a consistently affected region of wear in both normal and arthritic states. Clinical relevance: These findings suggest that trapezial trabecular morphology undergoes pathologic alteration. This provides indirect evidence that changes in load transmission occur with thumb carpometacarpal joint arthritis development. [Copyright &y& Elsevier]

Purpose: Radial subluxation and cartilage thinning have been associated with initiation and accelerated development of osteoarthritis of the trapeziometacarpal joint. Few investigators have reported on the benefits of opening wedge trapezial osteotomy for altering the contact mechanics of the trapeziometacarpal joint as a possible deterrent to the initiation or progression of osteoarthritis. We used cadaveric specimens to determine whether opening wedge osteotomy of the trapezium was successful in reducing radial subluxation of the metacarpal base and to quantify the contact area and pressure on the trapezial surface during simulated lateral pinch. Methods: We used 8 fresh-frozen specimens in this study. The flexor pollicis longus, abductor pollicis longus, adductor pollicis, abductor pollicis brevis, and flexor pollicis brevis/opponens pollicis tendons were each loaded to simulate the thumb in lateral pinch position. We measured radial subluxation from anteroposterior radiographs before and after placement of a 15° wedge. We used real-time sensors to analyze contact pressure and contact area distribution on the trapezium. Results: Center of force in the normal joint under lateral pinch loading was primarily located in the dorsal region of the trapezium. After wedge placement, contact pressure increased in the ulnar-dorsal region by 76%. Mean contact area increased in the ulnar-dorsal region from 0.05 to 0.07 cm2, and in the ulnar-volar region from 0.003 to 0.024 cm2. The average reduction in joint subluxation was 64%. Conclusions: The 15° opening wedge osteotomy of the trapezium reduced radial subluxation of the metacarpal on the trapezium and increased contact pressure and contact area away from the diseased compartments of the trapezial surface. Trapezial osteotomy addresses the 2 preeminent theories about the initiation and progression of osteoarthritis. Clinical relevance: By reducing radial subluxation and altering contact pressure and contact area, trapezial osteotomy may prove an alternative to first metacarpal extension osteotomy or ligament reconstruction in early stages of degenerative arthritis of the trapeziometacarpal joint. Purpose: To investigate whether palmaris longus interposition or flexor carpi radialis ligament reconstruction and tendon interposition improve the outcome of trapezial excision for the treatment of basal joint arthritis after a minimum follow-up of 5 years. Methods: We randomized 174 thumbs with trapeziometacarpal osteoarthritis into 3 groups to undergo simple trapeziectomy, trapeziectomy with palmaris longus interposition, or trapeziectomy with ligament reconstruction and tendon interposition using 50% of the flexor carpi radialis tendon. A K-wire was passed across the trapezial void and retained for 4 weeks, and a thumb spica was used for 6 weeks in all 3 groups. We reviewed 153 thumbs after a minimum of 5 years (median, 6 y; range, 5–18 y) after surgery with subjective and objective assessments of thumb pain, function, and strength. Results: There was no difference in the pain relief achieved in the 3 treatment groups, with good results in 120 (78%) patients. Grip strength and key and tip pinch strengths did not differ among the 3 groups and range of movement of the thumb was similar. Few complications persisted after 5 years, and these were distributed evenly among the 3 groups. Compared with the results at 1 year in the same group of patients, the good pain relief achieved was maintained in the longer term, irrespective of the type of surgery. While improvements in grip strength achieved at 1 year after surgery were preserved, the key and tip pinch strengths deteriorated with time, but the type of surgery did not influence this. Conclusions: The outcomes of these 3 variations of trapeziectomy were similar after a minimum follow-up of 5 years. Osteochondroma of the carpal is rare. We found only 1 case of osteochondroma of the trapezium in the literature. We present a case of a 52-year-old woman with an osteochondroma of the left trapezium and trapeziometacarpal arthritis. [ABSTRACT FROM AUTHOR]

Medical staff hardly recognize fractures of trapezium due to the small size and the complex anatomy of the wrist and the hand. Seventy percent of all carpal fractures are scaphoid's fractures. Only 0.4% of all carpal injuries are trapezial fractures. In this paper, we presented a case of a high speed trauma where there were only trapezial fractures. Purpose: The Weilby procedure is one of several accepted methods to treat primary thumb carpometacarpal osteoarthritis. We found no previous studies that included preoperative and postoperative subjective outcomes using validated questionnaires or preoperative and postoperative objective outcomes such as specific strength and range-of-motion measurements. Therefore, we performed a prospective cohort study in which we analyzed preoperative and postoperative objective and subjective outcomes after Weilby interposition tendoplasty. Methods: Nineteen patients (20 thumbs) with primary thumb carpometacarpal osteoarthritis were treated with Weilby interposition tendoplasty. For subjective assessment, the Disabilities of the Arm, Shoulder, and Hand (DASH) outcome data collection instrument was used to evaluate preoperative and postoperative outcomes at 0, 3, 6, and 12 months. Furthermore, patients completed a specific personal questionnaire at 12 months of follow-up. Objective assessments included interphalangeal joint flexion/extension; metacarpophalangeal joint flexion/extension; and carpometacarpal joint palmar abduction, opposition, and extension. Tip pinch, key pinch, 3-point pinch, and overall grip strengths were also measured. The measurements were performed preoperatively and at 3, 6, and 12 months after surgery. All complications were registered. Results: The DASH score was significantly improved, and 17 of 19 patients were satisfied with the procedure. The interphalangeal joint flexion/extension, metacarpophalangeal joint flexion/extension, and carpometacarpal joint extension did not significantly change. Carpometacarpal joint palmar abduction and opposition were significantly improved at 12 months. The tip pinch and key pinch strengths were increased but not significantly. The 3-point pinch and overall grip strengths were significantly improved at 12 months. Conclusions: The Weilby procedure is a reliable alternative to treat primary thumb carpometacarpal osteoarthritis without requiring bone tunnel creation. It achieves pain relief, stability, mobility, and strength. The objective and subjective outcomes of this study compare favorably with those of earlier reports of the Weilby procedure and are similar to the published results of the more commonly performed Burton-Pellegrini technique. Purpose: Although hematoma and distraction arthroplasty has been found to be successful in the treatment of thumb basal joint arthritis in the short term, questions about its efficacy in the long term have been raised. The goal of the present study was to evaluate the results of this procedure in patients at least 6.5 years after surgery. Methods: Twenty-two thumbs from 22 patients from a single surgeon’s practice were entered into a prospective single-arm study for surgical treatment of basal thumb arthritis. Treatment consisted of piecemeal excision of the entire trapezium and 5 weeks of K-wire immobilization of the thumb metacarpal in opposition and slight distraction. No ligament reconstruction or tendon interposition was used. Motion, strength, standardized dexterity tests, stress radiographs, and outcome questionnaires including the Arthritis Impact Measurement Scales 2 were evaluated before surgery and at 6, 24, and at least 79 months after surgery. Results: At 6 months after surgery, 17 of 22 patients reported complete pain relief, and at the most recent follow-up evaluation (average, 88 months after surgery) 18 patients were entirely pain free. Range-of-motion evaluation at the most recent follow-up evaluation showed 21 of 22 thumbs adducted fully into the plane of the palm, and 21 of 22 opposed to the small finger metacarpal head. Comparisons between preoperative and the recent postoperative strength measurements showed an average of 21% increase in grip strength and tip pinch strength, and an 11% increase in key pinch strength over preoperative values. Although the radiographically determined scaphoid–thumb metacarpal distance decreased with time from surgery, no correlation with strength or functional outcome measurements was found. The Arthritis Impact Measurement Scales 2 data showed postoperative improvement in the hand and finger function and arthritis pain scales. Conclusions: After trapezial excision, K-wire immobilization in a slightly overcorrected position without formal interposition or ligament reconstruction allows for restoration and maintenance of a stable, pain-free thumb that has comparable strength and motion with published reports of more complicated interventions over comparable time periods. Purpose: To examine the effects of partial or complete trapezoidectomy in conjunction with trapezium resection on proximal migration of the index metacarpal during axial loading. Methods: Five specimens from below the elbow were obtained from fresh-frozen cadavers. A customized vise was used to position the wrist in 20° extension and 5° ulnar deviation, simulating a power grip. The index metacarpal was instrumented to allow axial loading at 125 N and 250 N. Specimens were tested under 4 experimental conditions: (1) in the intact state, (2) after trapezial resection, (3) after partial trapezoid resection, and (4) after complete trapezoid resection. Specimens were tested 5 times under each experimental condition, displacement was measured with a linear variable differential transformer, and stiffness was calculated. Results: There was no difference in stiffness between the different treatments at either 125 N or 250 N. Displacement was not significantly different between treatments for loads of 125 N. For loads of 250 N the displacement was approximately 1 mm greater with complete trapezoid resection than with any of the other treatment conditions. Displacement did not differ significantly between the other 3 treatments. Conclusions: The addition of a partial trapezoid resection performed at the time of trapezial resection does not affect proximal migration of the second metacarpal. When a complete trapezoidectomy is performed at the time of trapezial excision, however, the loading characteristics of the index metacarpal do change statistically significantly, although the magnitude of that change is only approximately 1 mm. Further testing will be necessary to elucidate the biomechanical effects of a partial trapezoid resection in conjunction with a trapezial resection on the second metacarpal in other planes. Osteogenic sarcoma is a malignant tumor that rarely affects the hand. When it does, it most often involves the phalanges or metacarpal heads. We present the case of a 51-year-old woman with a low-grade osteosarcoma affecting the trapezium bone of her left hand. A total trapeziectomy with partial removal of the first metatarsal, scaphoid, trapezoid, and capitate bones was performed, and no adjuvant therapy was administered. Six years after the intervention, the patient is disease-free, with excellent functionality and yearly imaging tests showing no signs of recurrence. [ABSTRACT FROM AUTHOR]

Osteoid osteoma is a benign bone tumor representing approximately 10% of all benign bone tumors. Although osteoid osteoma of the long bones and carpus is frequent, the location in the trapezium is extremely rare. Abstract: We report a 42-year-old man with a rare carpal tunnel syndrome (CTS) secondary to an osteophyte of the trapezium. The patient presented with a 3-year history of CTS, consisting of progressive pain and paresthesias in his right hand, positive Tinel and Phalen signs, and an electrodiagnostic study demonstrating median nerve compression at the wrist. The procedure was an open carpal tunnel release. Intraoperatively, a bony protuberance was found beneath the transverse carpal ligament (TCL), resulting in compression of the median nerve. The median nerve was decompressed and the patient’s symptoms resolved postoperatively. Surgical pathology revealed bony fragments, and a postoperative CT scan was supportive of an osteophytic remnant protruding from the trapezium. Abstract: Fractures of the trapezium are rare and are usually treated by open reduction and internal fixation. The trapeziometacarpal joint is difficult to visualize with standard radiographic techniques; proper visualization of the joint surface of the scaphometacarpal and trapeziometacarpal joints is paramount. A 36-year-old male patient complained of isolated wrist pain and had limitation of wrist function after a motor vehicle accident. The injury was closed without obvious deformity. Radiographs showed a longitudinal shear fracture of the trapezium. Five days later, the patient was taken to surgery, and with standard wrist arthroscopy, a 1.9-mm arthroscope was inserted via the radial carpometacarpal (CMC) portal into the CMC joint. A probe was inserted via the ulnar CMC to manipulate and reduce the fragment. A bone clamp achieved reduction, and 0.035-inch K-wires provisionally stabilized the fragments. Compressive fixation was accomplished with a percutaneous mini Acutrak headless screw (Acumed, Hillsboro, OR). A thumb spica splint was placed for 3 weeks, and, subsequently, progressive mobilization and strengthening was provided. At 6 weeks, union, normal range of motion, and full strength were present, and at 18 months, the patient returned to his previous level of activity with an excellent outcome.
