The proportion of open pelvic fractures in the paediatric population is relatively high. While operative fixation is the primary approach for managing open pelvic fractures in adults, there is limited literature on treatment outcomes in Children, particularly regarding long-term musculoskeletal, neurological, and urogenital function.

The aim of this study was to assess long-term outcomes of surgically treated Bennett fractures, while comparing open reduction and internal fixation (ORIF) with closed reduction and percutaneous fixation (CRPF). Patients treated between 1994 and 2010 were assessed retrospectively during an outpatient visit using a validated questionnaire (i.e. DASH, pain assessed through VAS), sensory testing, grip- and pinch-strength and radiographic analysis for post-traumatic arthritis. Fifty patients were included. Mean follow-up was 10 years. Mean age at trauma was 34 years. ORIF was used in 35 patients. CRPF was used in 15 patients. No differences in grip- and pinch-strength were found. Re-operations were needed in five ORIF-treated patients. Higher Pain Scores (VAS) were seen in the ORIF-treated patients. No correlation was found between surgical technique and functional outcomes. A persistent step-off or gap larger than 2 mm after surgical fixation was significantly correlated with post-traumatic arthritis at 10 years’ follow-up. The need to perform ORIF for anatomical reduction seems to be less important in preventing post-traumatic arthritis as a persistent step-off or gap of more than 2 mm was found to be significantly correlated with the development of post-traumatic arthritis. Secondly, both techniques lead to good functional outcomes, although persistent pain was seen in the ORIF-treated patients. Bennett fractures can therefore be safely treated with CRPF when the persistent step-off and gap after fixation do not exceed 2 mm.

Conventionele randomised controlled trials (RCT’s) voor trauma- en orthopedisch-chirurgisch onderzoek zijn moeilijk uitvoerbaar. Dit heeft zowel chirurg- als patiëntgerelateerde redenen. Grote regionale cohortstudies en (quasi-)experimentele designs met vooraf gedefinieerde uitkomstparameters en een fulltimeonderzoeker bieden een oplossing. De conventionele RCT kan dan worden ingezet voor specifiek gedefinieerde problemen die voortkomen uit de resultaten van deze studies. This article describes the limitations of conventional randomised controlled trials (RCT’s) for orthopaedic and trauma surgeons. Major challenges are recruitment of patients, participation of surgeons, the learning curve of surgical procedures, sufficient follow-up rates and treatment standardization. Prospective observational studies and (quasi-)experimental designs may provide a solution. The conventional RCT can be used for specifically defined research questions evolving out of prospective observational and (quasi-)experimental studies.

Een patella bipartita komt regelmatig voor en is meestal een toevalsbevinding op een röntgenfoto. Een symptomatische patella bipartita is echter relatief zeldzaam. In dat geval kan gekozen worden voor een conservatieve, maar ook voor een chirurgische behandeling (bijv. excisie van het losse patellafragment). De literatuur is daar niet eenduidig over. Wij presenteren een casus van een 26-jarige jongeman met pijnklachten van de knie die waren gebaseerd op een patella bipartita. Excisie van het superolaterale fragment vond plaats, waarna de m. vastus lateralis aan de resterende patella werd gehecht. Acht weken postoperatief waren de pijnklachten verdwenen en was de kniefunctie volledig hersteld. A bipartite patella is relatively common. However, a symptomatic bipartite patella is a rare condition. There can be chosen for a conservative or surgical treatment (for example excision of the patellar fragment). Current literature is not uniformly about the most successful treatment. We present a case report of a 26-year old patient with a painful knee, based on a bipartite patella. The superolateral patellar fragment was removed. After that, the vastus lateral muscle was sutured to the remaining patella. At follow-up at the outpatient department after eight weeks, the pain complaints disappeared and functional recovery was complete.

Procesevaluatie van de traumaregistratie binnen een regionaal traumanetwerk in het kader van de landelijke traumaregistratie. Inventariserend, beschrijvend. Evaluatie van het hele proces van traumaregistratie, vanaf binnenkomst van de patiënt tot ontslag of overlijden, en vanaf de invoer van de gegevens tot aan het doorsturen naar de Landelijke Traumaregistratie (LTR). De studie bestond uit een schriftelijke vragenlijst en een mondeling interview. Iedereen die betrokken was bij de traumaregistratie in een regionaal traumanetwerk werd uitgenodigd voor deelname aan dit onderzoek. Alle benaderde personen (23) uit negen ziekenhuizen participeerden in dit onderzoek. De meerderheid van de ziekenhuizen (78%) had werkafspraken vastgelegd om de traumaregistratie te organiseren. Van de 19 inclusiecriteria voor de traumaregistratie waren er minimaal twaalf bekend bij de meerderheid van de geïnterviewden (80%). Twee derde (6) van de ziekenhuizen voerde een controle uit op de inclusie en ‘gemiste’ patiënten. Van de vitale parameters werd ademhalingsfrequentie het slechtst geregistreerd, gevolgd door de Glasgow Coma Scale (GCS). De mate waarin de traumaregistratie was geautomatiseerd en de wijze waarop deze automatisering was geïntegreerd in een bepaald ziekenhuisinformatiesysteem (ZIS) verschilden sterk per ziekenhuis. Er bestaat een ruime mate van variatie in de uitvoering per ziekenhuis van de regionale traumaregistratie binnen een traumazorgnetwerk. De voornaamste beperkingen schuilen in het ontbreken van controlemechanismen op inclusie en ontbrekende data, uniforme definities en werkafspraken, beperkende maatregelen op inter- en intra-observer variabiliteit in letselcodering, en ICT-oplossingen. The study consisted of a written questionnaire and an interview. All those involved in trauma registry in a regional trauma network were invited to participate in this investigation. All people approached (23) from nine hospitals participated in the study. The majority of the hospitals (78%) had registered work agreements to organize trauma registry. A minimum of twelve out of nineteen inclusion criteria for trauma registry was known to the majority of the interviewed persons (80%). Two- thirds (6) of the hospitals supervised and controlled the inclusion and checked for ‘missed’ patients. Respiratory rate was the vital parameter that was most frequently missing, followed by the Glasgow Coma Scale (GCS). The degree of automation of trauma registry and the way it is integrated in the hospital information system strongly differed per hospital. There is a considerable degree of variation in the implementation of regional trauma registry within a regional trauma network. Principle limitations are found in the absence of control mechanisms for inclusion and missing data, uniform definitions and work agreements, limitative measures for inter- and intra-observer variability in coding injuries, and ICT solutions.
