PurposeThis study investigated reading teachers' (RTs) views of speech-language pathologists' (SLPs) abilities to provide services for clients who have written language disorders. Previous research has shown SLPs often do not provide such services due to time constraints, lack of training, and their perception that other school personnel are more qualified. However, little is known about RTs' views of SLPs' abilities to provide reading/writing intervention.MethodThree hundred twenty-three RTs responded to a survey designed to determine their perceptions and experiences of SLPs providing services to children with written language difficulties. Respondents were from all regions of the United States.ResultsApproximately 80% of the respondents supported the idea of SLPs providing written language intervention services, with 69% recognizing that there was an evidence base for such intervention. However, many RTs registered some disagreement that SLPs had sufficient training to provide written language instruction and may lack knowledge of the curriculum. These results also identified factors that SLPs wishing to be involved in literacy instruction may need to address including administrative support, involvement in school-wide literacy learning decisions, and participation in professional development opportunities on literacy instruction.ConclusionThese results indicated that RTs may be supportive of SLPs addressing the written language needs of the students they serve. Although written language intervention is within SLPs' scope of practice, SLPs currently practicing in school settings may need to advocate for themselves to actively be involved in written language instruction.Supplemental Materialhttps://doi.org/10.23641/asha.14312783

Purpose: There is much anecdotal evidence that birds and their droppings are a major problem for the heritage profession. The purpose of this paper is to examine how serious heritage practitioners consider the bird impact to be. Design/methodology/approach: An online survey was conducted of 59 Australian heritage professionals of between one and >20 year’s experience in the field. Findings: Bird impacts were not considered of major concern to buildings. The longer experience a practitioner had, the less likely the impacts were considered an issue. Feral pigeons were deemed the most problematic, followed by cockatoos, starlings, swallows, seagulls, mynas, sparrows, cormorants, ibis, ducks and birds of prey. The professionals ranked common deterrent methods. The highest-ranking deterrents were bird netting and bird spikes, but they were only considered moderately effective. The costs of installation and maintenance, as well the ease of installation, were all deemed significantly less important than the physical impact, the aesthetic sympathy and the effectiveness of a deterrent method. Practical implications: This study indicates that the impact of birds on buildings in Australia may be of less concern than previously thought, and may be driven by other factors (i.e. aesthetics, commercial companies) rather than actual effects. Originality/value: This is first study of its kind that surveyed the experiences of a wide range of heritage practitioners.

Purpose: Birds are implicated in spoiling and decay of buildings, especially through their droppings. Pigeons are considered the main culprits, and several studies have examined the effects and chemistry of accumulations of droppings without evidence to the exact origins of the source of the excreta. The paper aims to discuss these issues. Design/methodology/approach: This study reviews and summarises the state of knowledge with regard to the impact of bird excreta on buildings. It experimentally assesses the acidity of fresh pigeon excreta with different diets and examines the development of the acidity of the excreta after voiding. Findings: Feral pigeons in urban settings are known to be fed by a range of foods. Urban food scraps-derived diets produce more acidic excreta than more natural diets such as seeds. This is a first study of its kind to examine the impact of a bird’s diet on the pH and thus the resulting (potential) decay of masonry. Research limitations/implications: This study showed that from a management’s perspective, pigeons that subsist entirely on human provided foods will be depositing more initially acidic faeces. If faecal accumulation occurs; then, mould and other bacteria quickly alter the chemistry from acidic towards basic, but the damage may already be done. Originality/value: This paper is the first study of its kind to examine the effects of fresh pigeon droppings of known origin and age once voided from the intestine. This allows the authors to assess the impact during the first few days.

ABSTRACT: Nonspeech oral motor exercises (NSOME) are used often by speech-language pathologists to help children improve their speech sound productions. However, the phonology, articulation, and motor speech development and disorders literature does not support their use. This article presents five reasons (four theoretical, one empirical) why NSOME are not an appropriate therapeutic technique for treating children's speech sound production problems.

The authors assessed the risk for psychosocial maladjustment in a population-based sample of dependent children of parents with serious physical illness. In the context of The Hamburg Health Survey, a wide range of data on current life situation and health status was collected from a representative sample of families with children and adolescents between 4 and 18 years old (N = 1,950). For 4- to 18-year-old children and adolescents in Germany, the prevalence of a serious physical illness in a parent was 4.1%. The adjusted risk of psychosocial maladjustment in this target group, depending on the case definition, is elevated, with internalizing problems being more prevalent than externalizing problems. Although problems for affected boys decline with puberty, they increase for girls. In this age-group, girls and boys appear to be under particular strain when the respective same-sex parent has a serious physical illness. Exposure to serious parental physical illness is an epidemiologically relevant risk factor for psychosocial maladjustment in children and adolescents that needs to be better recognized by medical professionals.

Data relating to the impact of bone marrow transplantation (BMT) on sexual functioning are equivocal; some studies have shown no major impact whereas others demonstrate a significant adverse effect on sexual health in patients treated with BMT. Further clarification is required to facilitate treatment choices and follow-up management of patients. A cross-sectional study of sexual health and infertility was conducted in 479 patients with acute myeloid leukemia (AML) in first complete remission (CR) who were entered into the UK MRC AML 10 trial comparing allogeneic BMT (Allo-BMT), autologous BMT (A-BMT), and intensive consolidation chemotherapy (CCT). Assessment was made by patient questionnaire via the treating centers for completion and returned directly to the coordinating center. Both Allo-BMT and A-BMT were observed to have severe, highly significant adverse effects on the patients' sexual health. Significantly more BMT patients than CCT patients reported a decrease in interest in sex (48% vs. 24%), sexual activity (53% vs. 35%), pleasure from sex (36% vs. 18%), and ability to have sex (38% vs. 18%) (P < 0.001 in each case). Hormonal disorders and infertility also were more common in BMT patients than in CCT patients. These differences were more apparent in women and remained after adjustment for age. These results indicate a need to consider quality of life parameters when reviewing treatment options and to instigate effective proactive management strategies for dealing with sexual health problems in leukemia survivors. Our objective was to develop a health-related quality of life measure for use in pediatric oncology. The development process followed the EORTC Quality of Life Study Group (QLSG) guidelines but utilized a parental proxy rating methodology developed within the framework of the EORTC QLSG. Data are reported on the preliminary stages of development, which include interviews in the target population, specialist review of questionnaire content and initial results on the psychometric structure of the measure. The questionnaire has been translated from English to Swedish and Dutch and is available for international field testing. Suggestions for further development of the new measure are described, including the need for parallel forms for use with children and adolescents as well as the parental proxy rating form described here. Anticipatory nausea and vomiting (ANV), as a side-effect of cancer chemotherapy, is a well recognized phenomenon. The literature is reviewed for data on: prevalence rates, factors contributing to onset and psychological treatments used to attempt a remission in the symptoms. From the available evidence it appears that ANV is a substantial problem with one major review indicating an average combined prevalence of 32%. Although explicable using a conditioning model it has a complex aetiology with emotional and cognitive elements, as well as specific pharmacological factors, playing a role in onset. ANV appears to be responsive to psychologically-based treatment methods, with a good outcome overall. However, more process evaluation is needed in order to understand aetiologic and treatment mechanisms and to determine how best to treat cases of ANV with different aetiologies. It is considered that procedures for identifying and referring patients with ANV should be routinely incorporated into the care of cancer patients receiving repeated cycles of chemotherapy.

Development of self-report measures of coping remains important to thc process of monitoring the mental state of patients suffering from cancer. The present study extended and refined the scope of the Mental Adjustment to Cancer (MAC) scale, a self-report measure designed to give a rapid assessment of coping style. A heterogeneous group of 573 cancer patients completed a questionnaire containing the MAC scale and an additional 26-item research scale. A rigorous factor analysis procedure was used to determine the items that would be retained for a new scale: the Mini-MAC. The scope of the original MAC scale was widened to include items relating to avoidance (an important psychological response), and the original response items were clarified and refined. The Mini-MAC should be a useful tool for obtaining a rapid, reliable, and economical assessment of coping style.

The evaluation of a specialist nurse-counselling service is described. Forty newly diagnosed breast cancer patients, treated by mastectomy, were randomised to receive either routine care or routine care plus counselling by a nurse. Psychological morbidity was assessed at one week, 3 months and 12 months postoperatively to determine the efficacy of this service. Comparisons between the groups indicated that counselled patients were significantly less depressed at 3 months postoperatively and reported more beliefs in personal control over health. At 12 months postoperatively there were no significant differences between the groups. Although both groups continued to adjust throughout the year following surgery, this adjustment occurred more rapidly if patients were counselled. The results suggest that a nurse-counselling service can be of value in helping to reduce the amount of distress experienced as a result of the diagnosis and treatment of breast cancer.

The English form of the Cancer Locus of Control Scale was administered to an heterogeneous sample of 68 cancer patients. A principal components analysis confirmed the three subscales described in the original Dutch form, however, with five items excluded. Correlations with other measures indicated that high internal control over the courseof the illness was associated with a tendency to adopt an attitude toward cancer of ‘fighting spirit’ whereas high internal control over the causeof the illness was associated with an ‘anxious preoccupation’ about cancer. No specific relationship was found between scale scores and either depression and anxiety or the tendency to be emotionally controlled. It was concluded that the scale provides a valid measure of perceptions of control of cancer patients and is a useful method of assessing these important psychological responses.

Although explicable using a conditioning model, it may have a complex aetiology with affective and cognitive elements as well as specific pharmacological factors playing a role in its onset and maintenance. It is amenable to treatment using psychological techniques but to treat successfully it is important to understand the aetiology and formulate treatment plans according to those factors that make a significant contribution to the cause. It is suggested here that a behavioural model provides an over-simplistic conceptualisation and that cognitive factors play a central role in onset. As such, treatment plans need to be broader in scope by focusing on a range of strategies to enhance coping and particularly cognitive coping techniques. More recently there has also been some indication that psychological factors may contribute to symptoms of nausea and comiting occurring during or after chemotherapeutic infusion. Should this effect be replicated, the scope for psychological treatments within the care of patients receiving chemotherapy would be widened.

The idea of a cancer-prone personality type has been raised in numerous studies. Although no clear personality type has emerged from this research, the inability to express emotion has been reported consistently. Evidence for this trait is based solely on self-report measures or clinical observations; no objective behavioral data are available to validate its existence. The present study was designed to obtain objective ratings of behavior under conditions of experimentally manipulated stress for a group of breast cancer patients and a group of healthy women. The data supported the hypothesis that the breast cancer patients would be more emotionally inhibited. Self-report measures indicated that although the patients tended to control or repress their emotional reactions, they were more anxious and disturbed as a result of the stress manipulation than were the controls. The results are discussed in relation to current theories of emotion and to psychobiological models.

High-dose therapy (HDT) and ASCT is the standard of care in a variety of hematologic malignancies. Whereas for some indications a survival advantage for HDT and ASCT has been demonstrated, a benefit only in terms of better progression-free survival has been shown for CLL. Because of this the quality of life (QoL) deserves particular attention. QoL assessment was a major focus of a randomized controlled EBMT-Intergroup trial on the value of HDT compared to observation in first or second remission of CLL (Michallet, Blood, 2011).222 patients were enrolled into the study and allocated to either ASCT or observation. In the transplant arm, 72% received HDT and ASCT (for those median time from randomization to transplant was 3.01 months); in the observation arm 9% received ASCT. QoL was assessed with the EORTC QLQ C30 version 3.0, a questionnaire that has to be filled in by the patients. The answers to the questions yielded 15 scores, each on a scale from 0 to 100. The scores represent 15 domains: global health status/QoL, 5 functional scales (100 representing perfect health) and 9 symptom scales (0 representing no complaints). QoL forms had to be completed at randomization and at months 4, 8, 12, and 24. Data on 56%, 53%, 54%, 61%, and 50% of the baseline patients are available for the respective periods. Missing forms were not systematically related to baseline variables or relapse. The numbers of drop out due to death at 2 years were 5 patients in the HDT arm and 4 patients in the control arm.All QoL outcomes were analyzed with mixed models according to the intent to treat principle. Time (as factor), age, gender, treatment arm and the interaction of time and treatment arm were modelled as fixed effects, whereas individual random effects were added for the intercept.The mean values for global health status/QoL, physical functioning, role functioning and social functioning over time for the transplant and the observation group are shown in Figure 1. Global health status/QoL at 4 months (estimated effect from the multivariate model −7.15, p=0.034) was significantly inferior in the transplant cohort compared to the control group. At 8 months the estimated effect of HDT on global health status/QoL was −3.06 (p=0.36). This difference further diminished over the first year (estimate at 1 year −0.53, p=0.87). QoL did not decrease independently from the treatment during the first 2 years. The same global pattern of change over time was observed for physical functioning, role functioning and social functioning; however, the treatment impact was still significant at 8 months for physical functioning (-6.58; p=0.025) and social functioning (-11.18; p=0.014). No significant covariate effects could be delineated for either of these scales apart from age having a beneficial effect on social functioning.Quality of life is affected multi-dimensionally in the first year after high-dose therapy and autologous stem cell support. The negative impact of HDT on QoL has disappeared after two years. Patients should be informed that HDT followed by ASCT impairs quality of life in the first year after transplantation.No relevant conflicts of interest to declare.
