Amyloidosis is characterized by systemic or local deposition of amyloid fibrils outside organs and tissues. Amyloidosis is rarely seen on cornea. A 30-year-old woman patient had had trichiasis in both eyes for 8 years. Trichiasis was observed, which touched the cornea. Slit lamp microscopy showed white gelatinous droplet-like eminences and trichiasis in the lower cornea of the right eye. Optical coherence tomography showed that the lesion involved most of the cornea. Hematoxylin and eosin staining showed that most of the stroma stained red, with scattered inflammatory cells. High expression of lactoferrin was detected by mass spectrometry, and the case was diagnosed as secondary corneal lactoferrin amyloidosis in the right eye. [ABSTRACT FROM AUTHOR]

To observe and analyze the clinical effect of nifedipine controlled-release tablets combined with valsartan in the treatment of essential hypertension, and to analyze the adverse reactions of patients. A total of 180 patients with primary hypertension treated in our hospital were enrolled as research objects in the study. According to different treatment regimens, they were divided into the control group treated with valsartan dispersible tablets and the research group treated with nifedipine controlled-release tablets combined with valsartan. The therapeutic effects of the two groups of patients were compared and analyzed. Compared with the control group (82.22%), the total treatment effective rate of the research group (95.56%) was higher, p<0.05. Comparing the blood pressure level before and after treatment of the two groups, the improvement effect of the research group after treatment was more obvious, p<0.05. The incidence of adverse reactions was 6.67% in the research group, which was significantly lower than that (20.00%) in the control group, p<0.05. The application of nifedipine controlled-release tablets combined with valsartan in the treatment of patients with primary hypertension can significantly improve the therapeutic effect of patients, and has good safety and reliability, which is a treatment mode worthy of promotion and practice. Background: Several antiplatelet medications used during and after percutaneous coronary intervention (PCI) are contraindicated for specific patient groups. A broad assessment of contraindicated medication use and associated clinical outcomes is not well described.Methods and Results: Using national Veterans Affairs Clinical Assessment, Reporting, and Tracking Program data for all PCI between 2007 and 2013, we evaluated patients with contraindications to commonly used antiplatelet medications during and after PCI, defined in accordance with package inserts. Adjusted association between contraindicated medication use and outcomes of periprocedural bleeding and 30-day mortality were assessed using Cox proportional hazards with inverse probability weighting. Among 64 294 patients undergoing PCI, 11 315(17.6%) had a contraindication to a common antiplatelet medication and 737 (6.5%) of these patients received a contraindicated medication. In unadjusted analyses, any contraindicated medication use was associated with both increased bleeding and 30-day mortality. In adjusted models, contraindicated abciximab use in patients with thrombocytopenia (hazard ratio, 2.23; 95% confidence interval, 1.58-3.16) and in patients with a previous stroke (hazard ratio, 1.93; 95% confidence interval, 1.37-2.71) remained significantly associated with increased bleeding. Contraindicated abciximab use was not significantly associated with 30-day mortality in adjusted models. Use of eptifibatide in dialysis patients was not significantly associated with an increased risk of bleeding or mortality.Conclusions: In this national cohort, ≈18% of patients undergoing PCI had contraindications to common antiplatelet medications. Approximately 6% of those patients received a contraindicated medication with attendant bleeding risk, although this did not translate into significantly higher risk of 30-day mortality. Abstract: A comprehensive study on the temporal and spatial distribution of H2S accumulations and the geochemical characteristics of sour gas fields (reservoirs) in the Sichuan Basin has been conducted. It has been followed by a series of carefully designed simulation experiments to elucidate an H2S formation mechanism which is able to predict the distribution of H2S. In some reservoirs, for example P2ch(T1f, have sufficient magnesic SO4 2( and abundant hydrocarbons (palaeo pools) present at sufficiently high temperatures to cause thermochemical sulfate reduction (TSR). This is the major origin of abundant H2S in large to medium scale gas fields. The formation mechanism of highly sour gas fields (reservoirs) is an in-situ reaction under conditions where sufficient source, enough energy supply, suitable reservoir volume and severely reducing environments occur. However, in some areas or strata, such as slope belts or on continental shelf deposits, or formations such as the Jialingjia and Leikoupo Formations, which are dominantly compact carbonatite and have contemporaneously different facies, it is difficult to form abundant H2S. These formations lack a source of sulphur and energy compared to the P2ch(T1f reef flat facies reservoirs. Even though highly sour natural gas migrating from the P2ch(T1f reservoirs is present, H2S is often consumed due to long distance migration and results in hydrocarbon-rich sweet natural gas. This study predicts that the Heichiliang reef flat facies sedimentary zone in northeastern Sichuan to have highly sour natural gas reservoirs. [ABSTRACT FROM AUTHOR]

Abstract: In this context, the bulk ratio of CH4/N2 is examined as a potential alternative geochemical parameter for the assessment of thermal maturity of natural gas and compared to other previously published data. Open-system non-isothermal pyrolysis of low-mature coal from the Manjiaer sag, Tarim basin, yielded generation curves for methane and nitrogen. Analysis of the change of vitrinite reflectance indicates a two-stage process of thermal maturation with increasing temperatures. The relationship between R o and pyrolysis temperature could be expressed by the following equations: Stage I: R o = 0.0014T + 0.109, r = 0.9931(R o < 0.6%); Stage II: R o = 0.0067T −1.5855, r = 0.9996 (R o > 0.6%). A kinetic interpretation of the characteristics of nitrogen and methane generation in humic coal during laboratory pyrolysis indicates that the bulk ratios of methane and nitrogen as thermal maturity parameters may be applied to assess the maturity of gas-sources. Thus, the maturity of the source rocks of natural gases in the Tarim basin was expressed in terms of CH4/N2 ratios and compared with other geochemical natural gas parameters, such as stable isotopes.
