This study aims to develop a new conceptual model of how lifestyle, physical environment, and e-payments affect consumers' millennial purchasing decisions and how they affect their satisfaction with social factors as a moderating. This study can fill the limitations of previous research as well as fill in the gaps in research on millennial consumers in making purchasing decisions. In this quantitative study, data was collected through questionnaires that were distributed to millennial consumers who made purchases at coffee shops at Riau Province. The sample calculation was carried out using a formula that produced 392 respondents using a purposive random sampling approach. A Likert-scaled questionnaire was used to collect the data, and SmartPLS 4.0 was used to process it. The study's findings include that the fit and quality index models have validated their value, lifestyle has a positive and significant effect on purchasing decisions; physical environment has a positive and significant effect on purchasing decisions; e-payment has a positive but not significant effect on purchasing decisions; factor social can significantly strengthen the influence of lifestyle on purchasing decisions; factor social can significantly strengthen the influence of physical environment on purchasing decisions; but factor social weakens the effect of e-payment on purchasing decisions but not significantly and purchasing decisions has a positive and significant effect on consumer satisfaction. Diagnosis of extraintestinal microsporidiosis is always hampered due to non-specific symptoms and difficulty in diagnosis. This study aimed to compare the diagnostic utility of blood and faecal-based polymerase chain reaction (PCR) to detect microsporidiosis in immunocompromised patients. A total of 42 immunocompromised patients consisting of HIV-infected and chemotherapy-treated patients were enrolled. Paired faecal and blood samples were collected and subjected to PCR to detect Enterocytozoon bieneusi and Encephalitozoon spp. Faecal samples were microscopically screened for microsporidia spores. Overall, 42.9% (18/42) of patients were positive for microsporidiosis. Of this, 19.0% (8/42) and 4.8% (2/42) were positive by blood and stool PCR respectively. Meanwhile, 33.3% (14/42) of the faecal specimens were microscopically positive. Among the positive patients, 22.2% (4/18) had microsporidia confirmed by blood PCR and stool microscopy, suggestive of dissemination. Interestingly, the stool specimen in which microsporidia spores were detected via microscopy is not positive via PCR method. This highlights the limitation of the faecal-based detection method and the important use of blood samples for diagnosing extraintestinal microsporidiosis. Only E. bieneusi species were detected in all PCR-positive samples. This study highlights the diagnostic value of blood PCR in diagnosing extraintestinal microsporidiosis infections. Atrial fibrillation (AF) patients at high risk for stroke and for bleeding may be unsuitable for either oral anticoagulation or endocardial left atrial appendage (LAA) occlusion. However, minimally invasive, epicardial left atrial appendage exclusion (LAAE) may be an option. The purpose of this study was to evaluate outcomes of LAAE in high-risk AF patients not receiving oral anticoagulation. A retrospective analysis of Medicare claims data was conducted to evaluate thromboembolic events in AF patients who underwent LAAE compared to a 1:4 propensity score–matched group of patients who did not receive LAAE (control). Neither group was receiving any oral anticoagulation at baseline or follow-up. Fine-Gray models estimated hazard ratios and evaluated between-group differences. Bootstrapping was applied to generate 95% confidence intervals (CIs). The LAAE group (n = 243) was 61% male (mean age 75 years). AF was nonparoxysmal in 70% (mean CHA 2 DS 2 -VASc score 5.4; mean HAS-BLED score 4.2). The matched control group (n = 972) had statistically similar characteristics. One-year adjusted estimates of thromboembolic events were 7.3% (95% CI 4.3%–11.1%) in the LAAE group and 12.1% (95% CI 9.5%–14.8%) in the control group. Absolute risk reduction was 4.8% (95% CI 0.6%–8.9%; P =.028). Adjusted hazard ratio for thromboembolic events for LAAE vs non-LAAE was 0.672 (95% CI 0.394–1.146). In AF patients not taking oral anticoagulation who are at high risk for stroke and for bleeding, minimally invasive, thoracoscopic, epicardial LAAE was associated with a lower rate of thromboembolic events. Traumatic brain injury (TBI) is a global health concern with significant economic impact. Optimal fluid therapy aims to restore intravascular volume, maintain cerebral perfusion pressure and blood flow, thus preventing secondary brain injury. While 0.9% saline (NS) is commonly used, concerns about acid-base and electrolyte imbalance and development of acute kidney injury (AKI) lead to consideration of balanced fluids as an alternative. This study aimed to compare the outcomes of patients with moderate to severe TBI treated with Sterofundin (SF) versus NS. A double-blinded randomised controlled trial of patients aged 18 to 65 years with TBI was conducted at the University Malaya Medical Centre from February 2017 to November 2019. Patients were randomly assigned to receive either NS or SF. The study fluids were administered for 72 h as continuous infusions or boluses. Participants, investigators, and staff were blinded to the fluid type. The primary outcome was in-hospital mortality. Relative risk (RR) with 95% confidence interval (CI) was calculated. A total of 70 patients were included in the analysis, with 38 in the NS group and 32 in the SF group. The in-hospital mortality rate were 3 (7.9%) in the NS group vs. 4 (12.5%) in the SF group, RR = 1.29 (95% CI, 0.64 to 2.59; p = 0.695). No patients developed AKI and required renal replacement therapy. ICP on day 3 was significantly higher in the SF group (18.60 ± 9.26) compared to 12.77 ± 3.63 in the NS group, (95% CI, −11.46 to 0.20; p = 0.037). There were no significant differences in 3-day biochemical parameters and cerebral perfusion pressure, ventilator-free days, length of ICU stay, or Glasgow Outcome Scale-Extended (GOS-E) score at 6 months. In patients with moderate to severe TBI, the use of SF was not associated with reduced in-hospital mortality, development of AKI, or improved 6-month GOS-E when compared to NS. This research focused on the history learning concerns among 11th-grade Social Science students at MAN Kota Pasuruan, a senior high school. To support research activities, the instruments included a questionnaire to measure the effectiveness of the product. The questionnaire was distributed to validators and users, specifically the 11th-grade students of social science at MAN Kota Pasuruan. The participants in this study were selected using purposive sampling for validation and simple random sampling for testing. During the implementation, this research used the ADDIE research and development model. The product effectiveness analysis technique developed by Nunuk Suryani, Achmad Setiawan, and Aditin Putria was used for analysis of the collected data. Based on the comparison results, it is evident that the developed product is truly valuable, appealing, and aligned with the current advancements in learning technology. The oil palm plantation industry has been competitive in the face of the global economy. Competitive advantage can be created through employees and organizations. Work engagement is the primary antecedent of a successful organization. This study is to investigate work-family conflict, and personality traits that determine work engagement among oil palm plantation officers. Participants were 782 executive officers from 36 oil palm plantation estates. The Hierarchical regression analysis showed that work-family conflict and personality traits are predictors of work engagement. Work-family conflict and neuroticism traits are associated with a low level of work engagement. Meanwhile, extraversion trait, agreeableness trait, openness to experience trait, and conscientiousness trait are associated with a high level of work engagement. The demands of work, job resources, and personal resources contribute to work engagement. This study examined the effect of Zn2+ ion substitution on ZnxMn(1-x) Fe2O4 nanoparticles (x = 0.2, 0.4, 0.6, 0.8) towards signal Surface Plasmon Resonance (SPR). The co-precipitation method synthesised the nanoparticles at a temperature of 90°C. The X-Ray Diffraction (XRD) results showed a polycrystalline structure with a crystallite size varied from 19.05 nm to 19.77 nm. In addition, as the Zn element increased in the ZnxMn(1-x)Fe2O4 nanoparticles, the degree of crystallinity increased by 21.72%, respectively. The optical properties of the nanoparticles ZnxMn(1-x) Fe2O4 indicate absorbance at wavelengths of 330.36–331.74 nm. The absorbance level increased as the Zn element decreased, resulting a direct optical gap of 3.47 eV and an indirect optical gap of 3.23 eV. The Attenuated Total Reflection (ATR) curve in the SPR biosensor indicated a shift in the reflective angle of the Prism/Au/Air configuration, which is 47.78° on Prisma/Au/nanoparticle of Zn0.8Mn0.2Fe2O4/air by 50.75°, and the prism/Au/nanoparticle of Zn0.2Mn0.8Fe2 O4/air by 51.04°. The substitution of Zn ions in the compound Zn x Mn(1-x)Fe2O4 affects the resulting ATR curve. The shift in the SPR angle is caused by changes in the refractive index in each layer which is influenced by the concentration of each material which has an impact on the absorbance ability and photon reflectance. This research has the potential to continue with the testing of dielectric and magnetic properties, as well as special testing for biological analytes compatible with ZnxMn(1-x)Fe2O4 nanoparticles. This study aims to obtain evidence on the ability of economic growth to moderate the effect of oil palm plantation development (land area, production, farmers, infrastructure, productivity, household expenditure) on the welfare (human development index) of districts in North Sumatra Province. This study covers 8 districts in North Sumatra Province in the 2011-2020 observation period, with secondary data obtained from the ministry of agriculture and the Central Statistics Agency. Data analysis techniques include: classical assumption test, Moderated Regression Analysis, coefficient of determination test, F test, and t test. The result of this study is that economic growth moderates the effect of oil palm plantation development (land area, productivity).
