Paraovarian cysts are common intrapelvic neoplasms, but the magnetic resonance (MR) findings of paraovarian cyst have never been reported. We investigated the spectrum of MR imaging features of paraovarian cyst.MR images of 18 paraovarian cysts in 16 patients were reviewed retrospectively. MR images were evaluated for the size and location of paraovarian cysts, single or multicystic, signal intensity on T1- and T2-weighted images, and visualization of the normal ovary on the affected side.The normal ovary of the affected side was recognized in 13 lesions. Four of these 13 cysts were separated from the ipsilateral ovary. In seven cysts, the normal ovary was abutted by cysts but maintained its shape. In two cysts, the beak sign was recognized at the interface between the cyst and the ovary. Most other MR features were nonspecific.Most paraovarian cysts were homogeneous cystic masses near the ipsilateral round ligament and the uterus. Demonstration of a normal ipsilateral ovary close to, but separated from, the adnexal cyst may be an important MR finding for the diagnosis of paraovarian cysts.

To evaluate the changes of liver parenchymal enhancement in the cirrhotic liver by means of triple-phase dynamic magnetic resonance (MR) imaging.Triple-phase multisection dynamic MR imaging was performed in 32 patients with liver cirrhosis. The control group consisted of 19 patients without liver cirrhosis. After precontrast images were obtained, arterial phase images were acquired 20 s after the start of intravenous bolus administration of 0.10 mmol/kg of gadopentetate dimeglumine. Portal and delayed phase images were then acquired 1 and 3 min, respectively, after the injection of contrast material. On each phase image, the signal-to-noise ratio (S/N) from the liver parenchyma was measured by operator-defined regions of interest (ROIs). The contrast-enhanced ratio (CER) on each phase was then obtained according to the following formula: [S/N(arterial or portal or delayed phase image) - S/N(precontrast image)] / S/N(precontrast image). The portal perfusion index (PPI) also was obtained according to the following formula: [S/N(portal phase image - S/N(arterial phase image)] / S/N(arterial phase image). The results were expressed as mean +/- SD.The CERs of arterial, portal, and delayed phase images in patients with and without liver cirrhosis were 0.256 +/- 0.211, 0.640 +/- 0.384, and 0.554 +/- 0.318 and 0.132 +/- 0.094, 0.404 +/- 0.204, and 0.324 +/- 0.144, respectively. The CERs were highest in the portal phase and lowest in the arterial phase in patients with and without liver cirrhosis. The CER of the cirrhotic liver was significantly higher than that of the normal liver in every phase (p0.05). PPIs with and without liver cirrhosis were 2.90 +/- 4.03 and 3.86 +/- 3.89, respectively. The PPI with liver cirrhosis was significantly lower than that without liver cirrhosis (p0.05).The enhancement of cirrhotic liver parenchyma is greater than that of the normal liver parenchyma at every phase of triple-phase dynamic MR imaging.

We present two cases of primary splenic malignant lymphoma associated with chronic hepatitis C virus infection detected early by routine follow-up imaging studies. Septumlike structures were seen on postcontrast computed tomography or magnetic resonance imaging, which presented characteristic gross findings. These findings may suggest primary splenic malignant lymphoma during the course of chronic hepatitis C.

Background: To compare fat-suppressed three-dimensional gradient echo (3D-GRE) and two-dimensional gradient echo (2D-GRE) techniques for hepatic magnetic resonance imaging. Methods: For 107 patients with suspected focal hepatic lesions, unenhanced 2D-GRE multislice in-phase and opposed-phase images and 3D-GRE images were obtained. Two radiologists independently and in random order reviewed the images. Quantitative analysis compared contrast-to-noise ratio (C/N) of liver masses and of the liver to spleen and signal-to-noise ratio (S/N) of the liver. Subjective grading of lesion conspicuity, confidence for lesion detection, artifacts, and overall image quality were also evaluated. Results: Conspicuity and confidence for focal lesions was statistically significantly higher on 3D-GRE than on 2D-GRE images. Liver-to-spleen C/N was higher on 3D-GRE images than on in-phase and opposed-phase 2D-GRE images (p < 0.01). Liver S/N was higher on in-phase 2D-GRE images than on 3D-GRE and opposed-phase 2D-GRE images (p < 0.001). Lesion-to-liver C/N was higher on 3D-GRE images than on 2D-GRE images for nonsolid lesions (p < 0.05), but there was no significant difference for solid lesions. Motion and pulsation artifacts were significantly fewer on 3D-GRE images, but graininess and shading were significantly higher. Overall image quality was not significantly different for the three sequences. Conclusion: Fat-suppressed 3D-GRE sequences can afford comparable or better image quality than 2D-GRE images and may have the potential to detect more focal hepatic lesions.

To evaluate the relationship between magnetic resonance (MR) imaging grading of iron deposition and serial serum ferritin concentration in patients with chronic viral liver diseases.In 80 patients with viral hepatitis and cirrhosis, MR images including T2*-weighted gradient echo images (echo timeor = 6.5 ms) were reviewed. The grades of parenchymal iron deposition and iron-containing nodules in the liver and spleen and the liver-to-muscle and spleen-to-muscle signal intensity ratios were compared with the most recent, the mean, the lowest, and the highest values from all available serum ferritin levels.The serum ferritin concentration was significantly correlated with the grades of iron deposition in liver and spleen and with the grades of iron-containing nodules seen on MR images (p0.05). Liver-to-muscle signal intensity ratio was weakly correlated with the ferritin concentrations. Among categories of ferritin concentration, correlation with MR grades was highest for mean ferritin concentration (r = 0.487, p0.001).MR imaging grades of hepatic iron and siderotic nodules correlate with serum ferritin, especially with the mean levels.

Immature teratoma, which contains variable quantities of immature tissues that resemble those of the embryo, is one of the primitive germ cell tumors. It occurs most frequently in young women but it is rarely reported in association with pregnancy. We report a case of immature teratoma associated with pregnancy exhibiting unique MR findings with pathologic correlation.

Dynamic contrast-enhanced whole-abdomen axial images were obtained by using a fat-suppressed, three-dimensional, breath-hold enhanced fast spoiled gradient-echo technique, configured for optimal evaluation for detection and characterization of liver lesions. We then evaluated the feasibility of using these images to reconstruct “free” abdominal magnetic resonance angiography, without additional cost or acquisition time, in 32 randomly chosen patients. The aorta, celiac trunk, superior mesenteric, hepatic, splenic, and renal arteries were clearly depicted.

Background: T1- and T2-weighted magnetic resonance (MR) images frequently show fan-shaped areas of hypo- or hyperintensity in the hepatic parenchyma adjacent to a treated hepatocellular carcinoma after percutaneous ethanol injection (PEI) therapy. These areas correspond to abnormal contrast enhancement on serial dynamic MR images. The purpose of the present study was to describe the location, appearance, and frequency of these abnormalities because it is important to understand these entities for the correct assessment of therapeutic efficacy. Methods: MR imaging including a multisection dynamic study was performed in 20 consecutive patients with hepatocellular carcinoma treated with PEI therapy. We retrospectively evaluated the presence of fan-shaped hypointensities adjacent to treated tumors in the liver parenchyma on T1-weighted images and hyperintensities on T2-weighted images and corresponding fan-shaped contrast enhancement on both arterial-dominant and delayed-phase dynamic MR images. We review the location, appearance, and frequency of these findings, and we discuss the possible causes on the basis of pathologic examinations. Results: Seven (35%) of the 20 patients showed fan-shaped hyperintense areas adjacent to the treated tumors on T2-weighted images. These areas showed isointensity in five patients and hypointensity in two patients on T1-weighted images. Of these seven patients, one (14%) underwent the MR imaging within 1 month after the completion of PEI therapy, and six (86%) had it 2–9 months after the completion of PEI therapy (mean = 6 months). In all seven patients, fan-shaped hyperperfusion abnormalities corresponding to these areas of hyperintensity on T2-weighted images were seen on both arterial-dominant and delayed-phase dynamic MR images. Pathologically, the coagulative necrosis of the hepatocytes with sinusoidal dilatation and the restoration by the development of fibrous tissue were seen in these fan-shaped areas. Conclusion: The fan-shaped areas of abnormal intensity on T1- and T2-weighted images and contrast enhancement on dynamic MR images seem to be attributable to pathologic changes in the normal liver parenchyma induced by the toxic reaction of ethanol. Awareness of the occurrence of such abnormalities in the peripheral liver parenchyma adjacent to the treated tumor is important for the correct assessment of therapeutic efficacy. The goal of this study was to investigate the frequency of inhomogeneous parenchymal enhancement of the liver in magnetic resonance imaging during arterial portography (MRAP) versus computed tomography during arterial portography (CTAP).CTAP and MRAP were performed in 29 and in 21 patients, respectively, who had suspected primary or secondary liver tumors on clinical or biological grounds. We evaluated the frequency of inhomogeneous hepatic parenchymal enhancement not related to a decrease of portal blood supply due to compression or obstruction by the tumor and physiologic variation in portal perfusion. Inhomogeneous parenchymal enhancement of the liver was classified as segmental or subsegmental and as nonsegmental.Segmental or subsegmental inhomogeneous parenchymal enhancement was seen in six of 29 patients (20.1%) on CTAP and in one of 21 patients (4.8%) on MRAP. Nonsegmental inhomogeneous parenchymal enhancement was seen in five of 29 patients (17.2%) on CTAP images and in none of the patients (0%) on MRAP images. The incidence of nonsegmental inhomogeneous parenchymal enhancement was significantly lower on MRAP than on CTAP.MRAP was superior to CTAP in achieving homogeneous parenchymal enhancement of the liver.
