Background: Ultra-high-resolution computed tomography (U-HRCT) utilizes a 1024 × 1024 matrix with 0.25-mm section thickness, offering better spatial resolution than conventional multi-detector row CT to detect anatomic data for otologic surgery., Aims: We examined stapes footplate thickness using U-HRCT in relation to stapedotomy to predict the difficulty of the surgical procedure., Materials and Methods: Subjects were 12 otosclerosis patients and 25 controls who underwent diagnostic U-HRCT. A profile curve (Hounsfield units) was used to measure stapes footplate thickness along a perpendicular line across the stapes footplate in a plane parallel to the lateral semicircular canal., Results: Footplate thickness was smaller at the midpoint than just before the anterior crus and just after the posterior crus. Interobserver variability was lowest at the midpoint, where foot plate thickness was significantly greater in the affected ear in otosclerosis patients compared with controls (0.60 ± 0.09 mm vs 0.46 ± 0.04 mm; p  < .001). Otosclerosis patients were detected using U-HRCT with a high area under the curve. Difficulty in the stapes opening procedure correlated with stapes footplate thickness., Conclusions: Footplate thickness on U-HRCT correlated with temporal bone anatomy and corresponded to surgical difficulty. Significance: U-HRCT-derived anatomic data is useful for evaluating the stapes.

Conclusions: This prosthesis has the advantage of rapid adjustment at the time of insertion in order to achieve optimal tension and, as a result, optimal sound transmission., Objective: To test the acoustic performance of a new, adjustable incus replacement prosthesis in a human temporal bone model., Material and Methods: Experiments were performed in seven human temporal bones, before and after removal of the incus and insertion of the prosthesis. The input comprised 406 pure tones ranging in frequency between 0.1 and 10 kHz at an intensity of 80 dB SPL at the tympanic membrane. The output measurement was stapes footplate displacement, determined by means of a laser Doppler vibrometer. Three lengths of the prosthesis were investigated: optimal, optimal +0.2 mm and optimal +0.4 mm., Results: The optimal-length prosthesis produced similar results to those of an intact middle ear. The slightly longer prostheses decreased middle ear sound transmission at all test frequencies, except those near 1.5 kHz.

The amount and distribution of the post-natal bone deposition in the auditory ossicles and in the left tibia of dogs of varying ages were studied by means of alizarin labelling. The relative amount of fluorescent new-formed bone was expressed as a percentage ratio NB/(NB+PB) of new bone (NB) on the pre-existing bone (PB). The result was that the post-natal bone deposition (1) was larger in the tibia than in the incus, malleus and stapes; (2) significantly decreased with age both in the ossicles and in the tibia; (3) in the stapes it stopped at 3 months, while it was present in the tibia, incus and malleus even at 12 months. In the ossicles the post-natal bone deposition takes place both on the periosteal surfaces of the ossicles and on the internal surfaces of the haversian systems. The first process produces an appositional growth that stops in all three ossicles within the 1st month of post-natal life, the second one produces an internal growth that continues until the age of three months in the stapes, while in the incus and malleus it occurs in small amounts, even in the 12th month of life. In the ossicles all the new-formed bone tissue, periosteal and osteonic, is built up by primary bone (addition bone). In the tibia from 50 days of age the primary bone is gradually replaced by secondary haversian systems as a consequence of remodelling processes.
