Purpose: To evaluate the outcome of our patients with central retinal artery occlusion after local fibrinolysis and to compare these data with results reported in the literature., Material and Methods: Over a period of 7 years, 22 patients (11 male, 11 female, mean age 64.6 +/- 12.1 years) were treated with super-selective local fibrinolysis. In 1 case, treatment was carried out via the maxillary-ophthalmic anastomoses due to preexisting occlusion of the ipsilateral internal carotid artery. The latency period from the onset of symptoms to the beginning of therapy was 7.6 +/- 1.8 h. Urokinase was used in 7 cases (300,000-1.1 million units) and recombinant tissue plasminogen activator (20-40 mg) was applied in 15 patients. Visual acuity and fundus were examined before and after treatment., Results: One patient (1/22 = 4.6%) recovered completely and regained a visual acuity of 20/20. Six patients (6/22 = 27.3%) showed a marked improvement with a range of visual outcome from 20/800 to 20/320. In 2 cases (2/22 = 9.1%) only a slight improvement was observed, with a visual outcome allowing detection of hand movements. In 13 cases (13/22 = 59.1%) no change in visual acuity as a result of treatment was observed. In 2/22 cases (9.2%) reversible neurological side-effects occurred, in 1 case suffered a stroke, and in another case intracerebral bleeding was observed., Conclusion: In our study, the recovery of visual acuity and the complication rate were not as positive as reported in the literature.

Purpose: To find a suitable high-resolution MR protocol for the visualization of lesions of all 12 cranial nerves., Material and Methods: Thirty-eight pathologically changed cranial nerves (17 patients) were studied with MR imaging at 1.5 T using 3D T2*-weighted CISS, T1-weighted 3D MP-RAGE (without and with i.v. contrast medium), T2-weighted 3D TSE, T2-weighted 2D TSE and T1-weighted fat saturation 2D TSE sequences. Visibility of the 38 lesions of the 12 cranial nerves in each sequence was evaluated by consensus of two radiologists using an evaluation scale from 1 (excellently visible) to 4 (not visible)., Results: The 3D CISS sequence provided the best resolution of the cranial nerves and their lesions when surrounded by CSF. In nerves which were not surrounded by CSF, the 2D T1-weighted contrast-enhanced fat suppression technique was the best sequence., Conclusions: A combination of 3D CISS, the 2D T1-weighted fat suppressed sequence and a 3D contrast-enhanced MP-RAGE proved to be the most useful sequence to visualize all lesions of the cranial nerves. For the determination of enhancement, an additional 3D MP-RAGE sequence without contrast medium is required. This sequence is also very sensitive for the detection of hemorrhage.

Objective: To determine sensitivity and specificity of high-resolution MR imaging and of high-resolution axial CT (HRCT) and to compare the two modalities in predicting the surgical and functional success of cochlear implantation., Material and Methods: The presurgical MR images (2D T2W TSE, 3D T2*W CISS, plain and contrast-enhanced 3D T1W MP-RAGE) and axial HRCT findings of 26 patients were evaluated with regard to the predictive value concerning the success of cochlear implantation., Results: We found a high correlation between MR and HRCT and the success of cochlear implantation. In all 26 patients, the MR-based predictions concerning the success of cochlear implantation were correct. In 10 patients, MR gave additional information to HRCT. In all patients, MR gave sufficient information about the status of the inner ear, inner auditory canal and cochlear nerve to aid the surgeon during the operation., Conclusion: A high-resolution MR protocol consisting of coronal 2D T2W TSE, 3D T2*W axial CISS, plain and contrast-enhanced sagittal T1W 3D MP-RAGE is recommended for the evaluation of candidates scheduled for cochlear implantation. It provides information which cannot be obtained by HRCT.

Purpose: To evaluate the frequency of benign focal hepatic lesions incidentally detected at contrast-enhanced thin-section portal venous phase spiral CT., Material and Methods: Between January 1998 and February 1999, contrast-enhanced hepatic spiral CT examinations were performed in 1,892 patients. Out of these, only 100 patients fulfilled the following inclusion criteria: No underlying malignant disease, no liver cirrhosis, no suspected or known focal liver lesions. Standardized spiral CT parameters were applied. All CT studies were reviewed retrospectively by one radiologist. Any focal lesion was recorded and classified. Lesion size and number were noted., Results: A total of 108 hepatic lesions were reported in 33 out of 100 patients (80 cysts; 18 hemangiomas; 3 focal fatty infiltrations; 2 focal non-tumorous perfusion defects; 1 calcification; and 4 non-classified lesions). The average lesion size was 9.4 mm (< or =5 mm: n=40; 6-10 mm: n=30; 11-15 mm: n=28; >15 mm: n=10)., Conclusion: Benign liver lesions are probably a frequent incidental finding at abdominal spiral CT.

Purpose: To obtain morphologic and functional information in patients with dissection of the descending aorta using contrast-enhanced MR angiography (MRA) and MR blood flow quantification of the true and false lumina., Material and Methods: Fourteen patients were studied prospectively using a 1.5 T unit. MRA was performed with a 3D FISP sequence (TR/TE/flip angle 4.7/1.9 ms/30 degrees) after injection of 0.2 mmol Gd-DTPA per kg b.w. Flow quantification with phase velocity mapping was done at the level of the diaphragm using a 2D FLASH technique (TR/TE/flip angle 28/6.5 ms/30 degrees) with an average temporal resolution of 23 frames per cardiac cycle (34 ms). A spectral broadening index was applied to quantify the amount of flow irregularity within both channels of the aorta. Extension of the dissection and involvement of the major branch vessels were analyzed., Results: The mean flow volume per minute was 1,982 ml (SD 1,083 ml) in the true and 1,052 ml (SD 763 ml) in the false lumen. Average peak-velocities were 98 cm/s (SD 33 cm/s) in the true channel and 47 cm/s (SD 26 cm/s) in the false channel. Ten patients had bidirectional flow in the false lumen with a reflux volume ranging between 6.8% and 98%. Only 1 patient presented with bidirectional flow in the true lumen (reflux volume 15%). A significantly higher degree of flow irregularity was found in the false lumen compared with the true channel., Conclusion: Different hemodynamic patterns were found in aortic dissection. Their prognostic value and the impact on therapy, specifically percutaneous interventional procedures, have to be further studied.

Purpose: To evaluate the availability and completeness of previous radiological examinations., Material and Methods: Seven different types of examinations were analysed separately: 1) chest examinations, 2) bedside chest examinations, 3) skeletal examinations, 4) angiographic examinations, 5) mammographic examinations, 6) CT examinations, and 7) MR examinations. A retrospective part of the study covered the calendar year 1997, while a prospective part referred to 1998. The sample size in each modality and each part of the study was 100 observations, resulting in a total number of 1,400 examinations., Results: The average availability and completeness of radiographs in the retrospective analysis was 96.9% in 1997. The average availability and completeness of radiographs in the prospective analysis was 85.5% for the year 1998. This applied to necessary previous examinations of the same or different modality irrespective whether the examination was conducted internally or externally., Conclusion: The real loss rate of radiographs derived from the retrospective investigation was 3.1%. The prospective part of the study showed that 14.5% of the required previous internal and external examinations of the same or different modality were not available completely and in time to the examining radiologist.

Purpose: To evaluate the usefulness of a dedicated head-neck coil in preoperative imaging of the supra-aortic arteries., Material and Methods: Forty consecutive patients with suspected carotid artery stenosis underwent MR angiography (MRA). Using a dedicated head-neck coil, we made a complete evaluation of the supra-aortic arteries and graded the internal carotid artery (ICA) stenoses. MRA was performed at 1.5 T with: coronal 3D FISP from the aortic arch to the circle of Willis; transverse 2D FLASH and 3D TONE of the carotid bifurcation; transverse 3D TONE of the carotid siphon and the circle of Willis; and transverse 3D FISP of the aortic arch. I.a. digital subtraction angiography (DSA) was used as the reference. ICA stenoses of 70% and more at DSA (NASCET methodology) were regarded as severe., Results: Severe ICA stenoses were detected with high sensitivity and specificity: 93% and 92% respectively for coronal 3D FISP; 90% and 85% respectively for transverse 2D FLASH; and 97% and 94% respectively for transverse 3D TONE. The carotid siphon and the intracranial ICA were best depicted by 3D TONE. None of the applied sequences gave a satisfactory visualization of the aortic arch or of the origins of the vertebral arteries., Conclusion: With the head-neck coil, the supra-aortic arteries (including the intracranial vessels) were visualized without the need to reposition the patient, but depiction of the aortic arch was not acceptable. The quantification of ICA stenoses was reliable.

Purpose: The purpose of this study was to determine whether MR with and without SPIO (AMI-25) could replace spiral-CTAP in the staging of colorectal adenocarcinoma., Material and Methods: Thirty-five patients were studied prospectively by means of i.v. contrast-enhanced spiral-CT, spiral-CTAP, and MR of the liver. MR imaging was performed before and after infusion of AMI-25. Diagnoses were compared to intraoperative findings (n = 35) which included intraoperative ultrasound (n = 21), and follow-up CT (n = 18)., Results and Conclusion: Fifteen patients were found to have a total number of 53 liver metastases and 43 benign lesions were detected. Evaluation was performed in four different ways: 1) i.v. contrast-enhanced spiral-CT; 2) i.v. contrast-enhanced spiral-CT + spiral-CTAP; 3) plain MR; 4) plain MR + SPIO-enhanced MR. I.v. contrast-enhanced spiral-CT, spiral-CTAP and SPIO-enhanced MR identified patients with liver metastases with equal sensitivity. However, owing to its significantly higher sensitivity, based on a lesion-by-lesion analysis, spiral-CTAP cannot be replaced by SPIO-enhanced MR in patients who are to undergo liver resection. A limitation in spiral-CTAP is its relatively low specificity.

The purpose of this study was to evaluate MR angiography (MRA) and color Doppler sonography as noninvasive screening methods in suspected renovascular hypertension. Fifty-five consecutive patients with arterial hypertension were examined prospectively using high resolution 3-D TOF MRA and color Doppler sonography. Intraarterial angiography was the standard of reference. Stenoses of 60% or more were regarded as significant. MR angiograms were evaluated by 3 independent observers who studied 110 main renal arteries. All 8 significant stenoses and 2 occlusions were correctly classified with MRA while one 60% stenosis was underestimated by color Doppler sonography. Mild stenoses were overestimated by MRA in 4 and by color Doppler sonography in 6 cases. A drawback of both methods was the large number of not evaluable arteries (6 in MRA, 11 in color Doppler sonography). These arteries were regarded as pathologic because stenosis could not be excluded. Due to this fact specificities of MRA and color Doppler sonography were 90% and 85% respectively. Accessory vessels were detected in 47% (8/17) by MRA and in 0% (0/17) by color Doppler sonography.
