Objective: Papillary lesions of the breast are a heterogeneous group of lesions that are difficult to diagnose as benign or malignant. The purpose of this article is to review clinical presentation, imaging features, and pathologic correlation of papillary lesions of the breast and to discuss the prognosis and management of these lesions., Conclusion: Recognition of the variety of benign and malignant papillary lesions of the breast will facilitate diagnosis and proper management.

Objective: This study was undertaken to determine the efficacy of percutaneous vertebroplasty in treating patients with painful compression fractures of the thoracic and lumbar vertebrae that contain intraosseous vacuum phenomena., Materials and Methods: Nineteen cases of painful vertebral compression fractures with intraosseous phenomena occurring in 18 patients (six men, 12 women; age range, 59-88 years; mean age, 75.5 years) were identified from 393 percutaneous vertebroplasties performed in 199 patients during 32 and a half months. All patients had osteoporosis, with severe vertebral compression to less than one third of the vertebral body height in 13 of 19 cases. Affected levels were T6 (n = 1), T8 (n = 2), T9 (n = 1), T11 (n = 1), T12 (n = 4), L1 (n = 5), L2 (n = 2), L3 (n = 1), L4 (n = 1), and L5 (n = 1). All cases had the typical intravertebral body vacuum cleft appearance on radiographs. Imaging and clinical features were analyzed., Results: The mean volume of polymethyl methacrylate injected was 7.43 mL (range, 4.0-15.0 mL). Typically, the polymethyl methacrylate filled the intravertebral vacuum cleft. Complications during radiography consisted of minimal polymethyl methacrylate leakage into the adjacent disk (15/19 cases) and the paravertebral soft tissues (8/19 cases). No complications required surgical intervention. At clinical follow-up, pain relief was complete in eight patients (44.4%), partial in six patients (33.3%), and unchanged in four patients (22.2%)., Conclusion: Percutaneous vertebroplasty is effective in the treatment of patients with painful vertebral compression fractures with intraosseous vacuum phenomena.

Objective: Increasingly shorter TEs are being used for T1-weighted and proton density-weighted sequences in MR imaging of the knee. This study aims to evaluate the effect of a short TE on meniscal signal intensity., Materials and Methods: Thirty porcine knees were imaged with a 1.5-T MR scanner using spin-echo T1-weighted and proton density-weighted sequences. TR was kept constant at 700 msec for T1-weighted and 2200 msec for proton density-weighted sequences. For each set of sequences, sagittal images were obtained using these TE values: 9, 12, 16, 20, and 25 msec. Imaging parameters, such as slice thickness and interslice gap, number of excitations, matrix size, and field of view, were identical for each set of sequences. Using a fixed window level and width, we assessed the anterior and posterior horns of the medial meniscus for signal hyperintensity at each TE value. Signal intensity was also measured in eight knees. The menisci were then dissected and examined grossly and histopathologically., Results: Intrameniscal signal intensity increased progressively with shorter TEs. At a TE of 9 msec, signal hyperintensity was present in 100% of T1-weighted and 96.7% of proton density-weighted images. At a TE of 12 msec, signal hyperintensity was seen in 86.7% of T1-weighted and 80% of proton density-weighted images. At a TE of 16 msec, increased signal intensity was seen in only 3.3% of T1-weighted and 6.6% of proton density-weighted images. At TEs of 20 and 25 msec, increased signal intensity was seen in none of the T1-weighted and proton density-weighted images. Meniscal signal intensity increased exponentially at very short TE values. All menisci were found to be normal on gross and histopathological examination., Conclusion: Spurious signal hyperintensity appears in normal menisci at short TE values. Images acquired with short TEs should be interpreted with caution, and a TE of 16 msec or more is recommended.

Objective: Currently, the standard methods for therapeutic reduction of intussusception in children involve considerable ionizing radiation. This study tested the effectiveness of sonographically guided hydrostatic reduction of intussusception using Hartmann's solution, a fluid with near-physiologic composition., Subjects and Methods: Between March 1, 1994, and January 31, 1996, all children clinically suspected of having intussusception were evaluated by sonography. Those with positive findings on sonography were entered into the study and underwent confirmatory sonographically guided meglumine diatrizoate enema. During continuous sonographic monitoring, we used Hartmann's solution for attempted reduction of intussusception. Criteria for successful reduction were disappearance of the intussusceptum and passage of fluid through the ileocecal valve. Another sonographically guided meglumine diatrizoate enema was used to confirm successful reduction., Results: We detected 25 consecutive intussusceptions in 22 patients. The patients were 12 girls and 10 boys, with a mean age of 14 months (range, 1-72 months). Sonograms revealed in all patients doughnut or pseudokidney signs or both. The sites of intussusception were the transverse colon (17 of 25), hepatic flexure (4 of 25), ascending colon (2 of 25), splenic flexure (1 of 25), and descending colon (1 of 25). Other findings were dilated fluid-filled small bowel (11 of 25) and free intraperitoneal fluid (9 of 25). The success rate of our sonographically guided attempts at hydrostatic reduction was 76% (19 of 25). Success was proven by meglumine diatrizoate enema in all 19 patients. The mean time of the reduction procedure was 18 min (range, 2-45 min). No complications occurred. All six patients in whom hydrostatic reduction was unsuccessful underwent surgery. Five of these patients had ileoileocolic intussusceptions. On sonography, when surrounded by fluid, ileoileocolic intussusceptions had a typically complex, fronded appearance. The remaining patient in whom hydrostatic reduction was unsuccessful had ileocolic intussusception. Of six ileoileocolic intussusceptions, one was hydrostatically reduced and a second was converted into an ileoileal intussusception before requiring surgery. The other four intussusceptions were surgically treated., Conclusion: Our data suggest that sonographically guided hydrostatic reduction with Hartmann's solution can be used to treat ileocolic intussusception and to diagnose ileoileocolic intussusception.
