To examine the relationship between axial length (AL) and choriocapillaris (CC) flow deficits percentage (FD%) in non-pathological highly myopic eyes.Prospective cross-sectional study.This study included Chinese patients with non-pathological high myopia, which was defined by an AL of26 mm and a META-PM classification grade of2. Swept-source optical coherence tomography angiography was used to obtain 6 × 6 mm images of the macular CC. The CC FD% was measured in the fovea, parafovea, and perifovea subfields.A total of 1017 individuals (1017 eyes) with a mean age of 35.95 ± 14.11 years were included. After adjusting for age, sex, intraocular pressure, body mass index, systolic blood pressure, and image quality score, the overall CC FD% increased by 0.27% (95% CI 0.02, 0.52; P = .034) for each mm increase in AL. Among subfields, longer AL was associated with a higher CC FD% in the perifovea (β = 0.53, 95% CI 0.30, 0.77; P.001), and was not associated with a higher CC FD% in the parafovea (β = 0.08, 95% CI -0.26, 0.42; P = .652) and fovea (β = 0.001, 95% CI -0.50, 0.50; P = .999).The CC FD% increased with a longer AL in high myopia in the perifovea region but not in the fovea and parafovea fields. These findings may be of interest in elucidating the etiology of myopic axial elongation.

Myopia is a major public health issue and occurs at young ages. Apart from its high prevalence, myopia results in high costs and irreversible blinding diseases. Accurate prediction of the risk of myopia onset is crucial for its precise prevention. We aimed to develop and validate an effective nomogram for predicting myopia onset in schoolchildren.School-based prospective cohort study.A total of 1073 schoolchildren were enrolled from November 2014 to May 2019 in China, and were divided into the training and validation cohorts. Myopia was defined as a spherical equivalent refraction (SER) ≤-0.5 diopters. Predictors of myopia were determined through the least absolute shrinkage and selection operator regression and multivariable Cox proportional hazard model based on the training cohort. The predictive performance of the nomogram was validated internally through time-dependent receiver operating characteristic (ROC) curves, calibration plot, decision curve analysis, and Kaplan-Meier curves.Independent predictors at baseline including gender, SER, axial length, corneal refractive power, and positive relative accommodation were included in the nomogram prediction model. This nomogram demonstrated excellent calibration, clinical net benefit, and discrimination, with all the area under the ROC curves (AUCs) between 0.74 and 0.86 in the training and validation cohorts. The Kaplan-Meier curves showed that 3 distinct risk groups stratified through X-tile analysis were well discriminated and robust among subgroups. The Harrell's C-index and net reclassification improvement demonstrated that the nomogram substantially improved compared with previous models. An online myopia risk calculator was generated for better individual prediction.The nomogram provides accurate and individual prediction of myopia onset in schoolchildren. External validation is needed to verify the generalizability of this nomogram.

To evaluate the diagnostic accuracy of a novel optical coherence tomography texture-based en face image analysis (SALSA-Texture) that requires segmentation of only 1 retinal layer for glaucoma detection in eyes with axial high myopia, and to compare SALSA-Texture with standard macular ganglion cell-inner plexiform layer (GCIPL) thickness, macular retinal nerve fiber layer (mRNFL) thickness, and ganglion cell complex (GCC) thickness maps.Comparison of diagnostic approaches.Cross-sectional data were collected from 92 eyes with primary open-angle glaucoma (POAG) and 44 healthy control eyes with axial high myopia (axial length26 mm). Optical coherence tomography texture en face images, developed using SALSA-Texture to model the spatial arrangement patterns of the pixel intensities in a region, were generated from 70-μm slabs just below the vitreal border of the inner limiting membrane. Areas under the receiver operating characteristic curves (AUROCs) and areas under the precision recall curves (AUPRCs) adjusted for both eyes, axial length, age, disc area, and image quality were used to compare different approaches.The best parameter-adjusted AUROCs (95% confidence intervals) for differentiating between healthy and glaucoma high myopic eyes were 0.92 (0.88-0.94) for texture en face images, 0.88 (0.86-0.91) for macular RNFL thickness, 0.87 (0.83-0.89) for macula GCIPL thickness, and 0.87 (0.84-0.89) for GCC thickness. A subset analysis of highly advanced myopic eyes (axial length ≥27 mm; 38 glaucomatous eyes and 22 healthy eyes) showed the best AUROC was 0.92 (0.89-0.94) for texture en face images compared with 0.86 (0.84-0.88) for macular GCIPL, 0.86 (0.84-0.88) for GCC, and 0.84 (0.81-0.87) for RNFL thickness (P ≤ .02 compared with texture for all comparisons).The current results suggest that our novel en face texture-based analysis method can improve on most investigated macular tissue thickness measurements for discriminating between highly myopic glaucomatous and highly myopic healthy eyes. While further investigation is needed, texture en face images show promise for improving the detection of glaucoma in eyes with high myopia where traditional retinal layer segmentation often is challenging.

To compare progression of myopia and refractive error in former premature infants with retinopathy of prematurity (ROP) treated using intravitreal bevacizumab (IVB) or laser.Retrospective clinical cohort study.We identified premature infants with ROP treated using IVB from 2011 to 2020 and compared their longitudinal cycloplegic refraction data to that of infants with ROP treated using laser during the same timeframe. A subset of infants treated using IVB also underwent additional treatment using laser. We included cycloplegic refractions from 789 cumulative visits over a median 3.2 years. We used a linear mixed-effects model with a log decay function to evaluate how refraction changed with age after treatment.In aggregate, the model estimated a significant (P.001) trend in refraction-from slight hyperopia to relatively more myopic states. However, progression in laser-treated eyes was significantly (P.001) more rapid, regardless of treatment with IVB. The number of laser spots resulted in increased myopic progression by approximately 0.16 diopters per 100 laser spots. Both ROP stage and zone had a significant effect on myopic progression, with more severe disease resulting in faster myopic progression. Random effects, including individual subject variation with nested variance for left and right eye, accounted for 86.4% of the remaining variance not explained by age and treatment.Laser treatment for severe ROP increases the trend to severe myopia. In our sample, IVB did not affect myopic progression but did substantially reduce the amount of consequent laser required to treat ROP. The effect of laser persists after accounting for differences in ROP stage and zone.

To determine the impact of corneal epithelial thickness maps on screening for refractive surgery candidacy in a single refractive surgical practice.Comparison of screening methods.A total of 100 consecutive patients who presented for refractive surgery screening were evaluated. For each patient, screening based on Scheimpflug tomography, clinical data, and patient history was performed and a decision on eligibility for laser in situ keratomileusis (LASIK), photorefractive keratectomy (PRK), and small incision lenticule extraction (SMILE)was independently made by 2 masked examiners. Examiners were then shown patients' epithelial thickness maps derived from optical coherence tomography (OCT). The percentage of screenings that changed after evaluating the epithelial thickness maps, with regard to candidacy for surgery, and ranking of surgical procedures from most to least favorable was determined.Candidacy for corneal refractive surgery changed in 16% of patients after evaluation of the epithelial thickness maps, with 10% of patients screened in and 6% screened out. Surgery of choice changed for 16% of patients, and the ranking of surgical procedures from most to least favorable changed for 25% of patients. A total of 11% of patients gained eligibility for LASIK, whereas 8% lost eligibility for LASIK. No significant difference was found between the evaluations of the 2 examiners.Epithelial thickness mapping derived from optical coherence tomography imaging of the cornea altered candidacy for corneal refractive surgery, as well as choice of surgery, in a substantial percentage of patients in our practice, and was thus a valuable tool for screening evaluations. Overall, the use of epithelial thickness maps resulted in screening in a slightly larger percentage of patients for corneal refractive surgery.

To evaluate whether pediatric eyes that deviate from age-adjusted normative biometry parameters predict variation in myopic shift after cataract surgery.This is a single institution longitudinal cohort study combining prospectively collected biometry data from normal eyes of children10 years old with biometry data from eyes undergoing cataract surgery. Refractive data from patients with a minimum of 5 visits over ≥5 years of follow-up were used to calculate myopic shift and rate of refractive growth. Cataractous eyes that deviated from the middle quartiles of the age-adjusted normative values for axial length and keratometry were studied for variation in myopic shift and rate of refractive growth to 5 years and last follow-up visit. Multivariable analysis was performed to determine the association between myopic shift and rate of refractive growth and factors of age, sex, laterality, keratometry, axial length, intraocular lens power, and follow-up length.Normative values were derived from 100 eyes; there were 162 eyes in the cataract group with a median follow-up of 9.6 years (interquartile range: 7.3-12.2 years). The mean myopic shift ranged from 5.5 D (interquartile range: 6.3-3.5 D) for 0- to 2-year-olds to 1.0 D (interquartile range: 1.5-0.6 D) for 8- to 10-year-olds. Multivariable analysis showed that more myopic shift was associated with younger age (P.001), lower keratometry (P = .01), and male gender (P = .027); greater rate of refractive growth was only associated with lower keratometry measures (P = .001).Age-based tables for intraocular lens power selection are useful, and modest adjustments can be considered for eyes with lower keratometry values than expected for age.

To evaluate longitudinal changes in subfoveal choroidal thickness (SFChT) among children receiving atropine 0.05%, 0.025%, or 0.01% over 2 years and their associations with treatment outcomes in myopia control.Double-blinded randomized controlled trial.SFChT was measured at 4-month intervals using spectral domain optical coherence tomography. Cycloplegic spherical equivalent (SE), axial length (AL), best-corrected visual acuity, parental SE, outdoor time, near work diopter hours, and treatment compliance were also measured.314 children were included with qualified choroidal data. The 2-year changes in SFChT from baseline were 21.15 ± 32.99 µm, 3.34 ± 25.30 µm, and -0.30 ± 27.15 µm for the atropine 0.05%, 0.025%, and 0.01% groups, respectively (P.001). A concentration-dependent response was observed, with thicker choroids at higher atropine concentrations (β = 0.89, P.001). Mean SFChT thickness significantly increased at 4 months in the atropine 0.025% (P = .001) and 0.05% groups (P.001) and then remained stable until the end of the second year (P.05 for all groups). Over 2 years, an increase in SFChT was associated with slower SE progression (β = 0.074, P.001) and reduced AL elongation (β = -0.045, P.001). In the mediation analysis, 18.45% of the effect on SE progression from atropine 0.05% was mediated via its choroidal thickening.Low concentration atropine induced a choroidal thickening effect along a concentration-dependent response throughout the treatment period. The choroidal thickening was associated with a slower SE progression and AL elongation among all the treatment groups. Choroidal response can be used for assessment of long-term treatment outcomes and as a guide for concentration titrations of atropine.

Purpose To verify the dose-response relation between degree of myopia and open-angle glaucoma (OAG) risk Design Dose-response meta-analysis Methods We searched the PubMed, EMBASE, and Cochrane databases for population-based studies published until 30 November 2020 and reporting on both myopia and OAG. Random-effect models generated pooled odds ratios (OR) and 95% confidence intervals (CIs). Results robustness was confirmed by influence and subgroup analyses. A two-stage dose-response meta-analysis (DRMA) calculated the OAG risk per unit dose of myopia (spherical equivalent [SE] decrease of 1 diopter [D]) and examined the relationship pattern. Results Twenty-four (24) studies in 11 countries (514,265 individuals) made up the meta-analyses. The pooled OR of any myopia degree's association with OAG was 1.88 (95% CI, 1.66–2.13; I2=53%). The OR differences based on ethnicity (Asians vs. Westerners) or 5 geographic areas were not statistically significant (P=0.80 and 0.06, respectively). The pooled ORs of the associations between low, moderate, moderate-to-high, high myopia, and OAG were 1.50 (95% CI, 1.29–1.76), 1.69 (1.33–2.15), 2.27 (1.74–2.96), and 4.14 (2.57–6.69), respectively. According to the DRMA, the pooled OR (per SE 1 D change) was 1.21 (95% CI, 1.15–1.28); the OAG risk accelerated at around –6 D, and further accelerated from –8 D, showing a non-linear concave upward slope (P=0.03). Conclusions We found that for each unit (1 D) increase in myopia, the risk of glaucoma increases by approximately 20%. The risk more steeply increases in high-degree myopia, representing a significant non-linear relationship.

Purpose To compare the effect of duration of fluorometholone 0.1% treatment on corneal haze after photorefractive keratectomy (PRK) with mitomycin C (MMC) 0.02%. Design Prospective, randomized, double-blind, clinical trial. Methods Setting: Single center clinical trial. Study Population: A total of 252 myopic PRK candidates (252 eyes) aged 21 to 40 years with a mean spherical equivalent (SE) of ≤ 6 diopters (D) were randomized to receive one of the three corticosteroid regimens after PRK (group A = 1 month followed by 2-month placebo, group B = 2 months followed by 1-month placebo and group C = 3 months). Main Outcome Measures: Corneal haze incidence, subjective SE, uncorrected distance visual acuity (UDVA), and corneal densitometry. Results The corneal haze incidence (grade ≥ 1) at 12 months was 1.35% (1/74 eyes) in group A and 0% in the other two groups. The mean anterior corneal densitometry (grayscale unit) was 21.19 ± 2.07, 21.09 ± 2.19, and 21.31 ± 2.21 in group A, B and C, respectively. The mean SE was 0 ± 0.09, 0 ± 0.11, and 0 ± 0.10 and UDVA (decimal) was 1 ± 0, 1 ± 0.01, and 1 ± 0 in group A, B and C, respectively. During 1-year follow-up, no statistically significant difference was observed in mean SE (p = 0.158), UDVA (p = 0.343) and anterior corneal densitometry (p = 0.109) at any stage between the study groups. Conclusions Long-term topical corticosteroids are not necessary following PRK with MMC for moderate myopia.

Myopic vitreopathy features precocious fibrous vitreous liquefaction and early posterior vitreous detachment (PVD). It is unclear whether visual function is affected by myopic vitreopathy and PVD. This study assessed the relationships among axial length, structural vitreous density, PVD, and visual function.Retrospective case-control study.Ultrasonography measurements were made of axial length, logMAR VA, contrast sensitivity function (CSF [Freiburg acuity contrast test]), and quantitative B-scan ultrasonography.Seventy-nine subjects (45 men and 34 women; mean age: 49 ± 14 years) were analyzed. Axial lengths ranged from 22 to 29.2 mm (mean: 24.9 ± 1.8 mm; myopic eyes: 26.35 ± 1.35 mm; and nonmyopic eyes: 23.45 ± 0.75 mm; P.001). With increasing axial length there was greater vitreous echodensity (R: 0.573; P.01) and degradation in CSF (R: 0.611; P.01). Subgroup analyses found that myopic eyes (- 3 diopters) had 37% more vitreous echodensity than nonmyopic eyes (762 ± 198 arbitrary units [AU] vs. 557 ± 171 AU, respectively; P.001) and that CSF was 53% worse in myopic eyes (3.30 ± 1.24 Weber index [%W]) than in nonmyopic eyes (2.16 ± .59 %W; P.001). Myopic eyes with PVD had 33% greater vitreous echodensity (815 ± 217 AU; P.001) and 62% degradation in CSF (3.63 ± 2.99 %W) compared to nonmyopic eyes with PVD (613 ± 159 AU; 2.24 ± 0.69 %W; P.001, each). Limited vitrectomy was performed in 11 of 40 cases (27.5%), normalizing vitreous echodensity and CSF in each case.Axial myopia is associated with increased fibrous vitreous liquefaction and echodensity, as well as profound degradation of CSF. PVD in myopic eyes is associated with even more structural and functional abnormalities, normalized by limited vitrectomy. These findings may explain some common complaints of myopic patients with respect to vision and quality of life.

Purpose The aim of this study was to determine the sociodemographic and risk factors for keratoconus (KC) patients with a nationwide Asian database. Design Population-based matched case-control study. Methods We performed a secondary data analysis of the Taiwan National Health Insurance Research Database (NHIRD). Cases were patients with newly diagnosed KC in 1998-2015. Controls were patients without KC and matched 4:1 with the KC cases by age, sex, and index date. Comorbidities diagnosed before KC included diabetes mellitus (DM), asthma, allergic rhinitis, mitral valve prolapse, collagen vascular disease, aortic aneurysm, Down syndrome, sleep apnea, depression, hyperlipidemia, astigmatism, and myopia. Conditional logistic regression with forward selection were used to obtain risk factors for KC. Results A total of 5,055 patients with KC were matched with 20,220 controls. The average age at KC first diagnosis was 29.76 years. Individuals who lived in suburban and rural area had lower odds ratio of KC (adjusted odds ratio [OR] 0.86, 95% confidence interval [CI] 0.80-0.94; and 0.73, 95% CI 0.67-0.79; respectively) when comparing with those who lived in urban area. Multivariate analysis revealed that Down syndrome, astigmatism, myopia, allergic rhinitis, and asthma were positively associated with KC with adjusted odds ratios (adjusted OR 8.69, 95% CI 3.74-20.19; 6.23, 95% CI 5.35-7.24; 2.99, 95% CI 2.70-3.32; 1.22, 95% CI 1.14-1.32; and 1.18, 95% CI 1.07-1.30, respectively). On the other hand, hyperlipidemia, depression, and DM (uncomplicated and complicated) were negatively associated with KC (adjusted OR 0.67, 95% CI 0.59-0.77; 0.58, 95% CI 0.48-0.71; 0.77, 95% CI 0.64-0.93; and 0.61, 95% CI 0.44-0.86, respectively). Conclusions Our study found that patients with hyperlipidemia, depression, or DM were less likely to have KC, and patients with asthma, allergic rhinitis, astigmatism, myopia, or Down syndrome had higher odds ratio of KC.

Purpose To review the impact of increased digital device usage arising from lockdown measures instituted during the COVID-19 pandemic on myopia and to make recommendations for mitigating potential detrimental effects on myopia control. Design Perspective. Methods We reviewed studies focused on digital device usage, near work, and outdoor time in relation to myopia onset and progression. Public health policies on myopia control, recommendations on screen time, and information pertaining to the impact of COVID-19 on increased digital device use were presented. Recommendations to minimize the impact of the pandemic on myopia onset and progression in children were made. Results Increased digital screen time, near work, and limited outdoor activities were found to be associated with the onset and progression of myopia, and could potentially be aggravated during and beyond the COVID-19 pandemic outbreak period. While school closures may be short-lived, increased access to, adoption of, and dependence on digital devices could have a long-term negative impact on childhood development. Raising awareness among parents, children, and government agencies is key to mitigating myopigenic behaviors that may become entrenched during this period. Conclusion While it is important to adopt critical measures to slow or halt the spread of COVID-19, close collaboration between parents, schools, and ministries is necessary to assess and mitigate the long-term collateral impact of COVID-19 on myopia control policies.

To compare the accuracy of glaucomatous defects and diagnostic power for primary open-angle glaucoma (POAG) between swept-source optical coherence tomography (SS-OCT) and spectral-domain OCT (SD-OCT) in myopic eyes.Prospective, case-control study.A total of 150 myopic POAG eyes and 100 healthy myopic eyes underwent SD-OCT and SS-OCT in random order on the same day. The locations of glaucomatous defects on SD-OCT thickness and deviation maps and SS-OCT wide-field thickness (thickness surfaces) and SuperPixel maps were rated, and the maps' accuracies were compared. The area under receiver operating characteristic (AUROC) of the peripapillary retinal nerve fiber layer (RNFL) and the macular parameters (GCL++: equivalent to ganglion cell-inner plexiform layer [GCIPL] + RNFL; GCL+: equivalent to GCIPL) from each of the devices for myopic POAG were calculated and compared.The wide-field RNFL thickness (thickness surface) map showed the best accuracy for glaucomatous defects in the inferotemporal (96.4%) and superotemporal (92.4%) regions. The RNFL/GCL++/GCL+ wide-field thickness (thickness surface) map showed better accuracy for glaucomatous defects in both the superotemporal and inferotemporal regions than with the SD-OCT thickness map (all P values.05). The average GCL++ (87.6%) and GCL+ (87.5%) thicknesses showed significantly greater AUROC for myopic POAG than GCIPL thickness from SD-OCT (83.8%, all P values.05).In myopic eyes, the SS-OCT wide-field map exhibited better accuracy for glaucomatous defect and greater diagnostic power for POAG than SD-OCT outcomes. This result might have been due to SS-OCT's wider scan and measurement area.

Purpose To determine the effects on childhood myopia of parental myopia, parental education, children's outdoor time, and children's near work. Design Population-based cross-sectional study. Methods A total of 6,155 subjects in 2,055 family trios (1 child and both parents). Cycloplegic autorefraction was measured for children and noncycloplegic autorefraction for parents. Parental education, children's outdoor time, and near work were collected by questionnaires. Children were categorized into 10 groups based on parental myopia levels. Associations of the above factors with myopia were evaluated by regression analyses. The areas under the receiver operating characteristic curve (AUROCs) for myopia were evaluated. Results Mild parental myopia did not increase childhood myopia's risk, but the risk was 11.22-folds when both parents were highly myopic. Higher parental education (Father: OR 1.08, P = .046; Mother: OR 1.11, P = .001) and more reading time of children were risk factors (OR 1.21, P = .044). Reduced odds of myopia were associated with more time spent on outdoor activities (OR 0.78, P = .017). Notably, all these factors became insignificant after adjustment, except for parental myopia. Children with more severe parental myopia spent more time on reading, but less on electronic devices. Parental myopic status alone accounted for 11.82% of myopia variation in children. With age and parental myopia, the AUROC for myopia was 0.731. Conclusions Among parental and environmental factors, parental myopia confers, in a dose-related manner, the strongest independent effect on childhood myopia. Therefore children with high risk of myopia can be identified for early prevention, based on parental myopia data.

Purpose To investigate the structural characteristics of the posterior sclera around the optic nerve head (ONH) in patients with normal-tension glaucoma (NTG) and myopia with central visual field (VF) defect. Design Cross-sectional study. Methods Ninety-seven eyes of 97 NTG patients with myopia were included in this study. Swedish interactive thresholding algorithm (SITA) 24-2 and 10-2 were both performed to compare the central VF with peripheral VF. Optic disc torsion, tilt, and peripapillary atrophy area were calculated in 2-dimensional disc photographs. The most posterior point—that is, the deepest point of the eye (DPE)—was identified in 3-dimensional en face optical coherence tomography results and related measurements were calculated using built-in software. Results Forty-two eyes had worse SITA 10-2 mean deviation (MD) than SITA 24-2 MD and they were assigned to the central dominant VF defect group. The central dominant VF defect group had larger disc torsion and ONH tilt angle and shallower disc-DPE depth than the peripheral dominant VF defect group. According to logistic regression analysis, large ONH tilt angle and shallow disc-DPE depth were significant factors associated with central visual function impairment. Conclusions Subjects with worse central VF than peripheral showed greater peripapillary scleral deformation, such as in torsion and tilt. Their posterior scleral deformation appeared to be closer to the ONH. Scleral deformation around the ONH may be associated with papillomacular bundle and central VF defects.

To clarify the changing characteristics of choroidal thickness over time within different age ranges and among different refractive statuses of children aged 6 to 18 years.Prospective cohort study.Data of axial length, cycloplegic refraction, and choroidal thickness (using swept-source optical coherence tomography) were collected at baseline and at a 1-year follow-up for 756 participants. One-year change in choroidal thickness and its association with age and refraction were analyzed.Significantly greater attenuation of choroidal thickness was observed in younger children aged 6-9 years for all participants (-9 ± 25 μm) and for those with a myopic shift (-12 ± 25 μm), whereas there was a larger increase in adolescents aged 10-13 years for those without a myopic shift (9 ± 23 μm). There was a marked decrease in the choroidal thickness for newly developed myopic patients compared with persistent-nonmyopic patients and persistent-myopic patients (P.01). The association between changes in axial length and choroidal thickness was less strong in persistent-myopic patients (β = -15.4, P = .022) than that in persistent-nonmyopic patients (β = -30.4, P.001) and newly developed myopic patients (β = -33.7, P = .001), whereas among the persistent-myopic patients, the association was less strong in the baseline mild-myopic patients (β = -10.4, P = .193) than that in the baseline moderate-to high-myopic patients (β = -31.4, P = .026).Changes in choroidal thickness varied by age. There was an increased rapid thinning of the choroid among newly developed myopic patients, and a nonsignificant association between changes in choroidal thickness and axial length in the early stages of myopia.

Purpose We sought to determine the association of refractive error and its associated determinants (axial length [AL], anterior chamber depth, and corneal curvature) with the incidence and progression of diabetic retinopathy (DR). Design Population-based cohort study. Methods A total of 1562 eyes of 840 individuals with diabetes and gradable retinal photographs (mean age [SD], 57.0 [8.3] years, 48.2% female) from the Singapore Malay and Indian Eye Studies at baseline (2004–2009) and follow-up (2011–2015) examinations were included in the analyses. Refractive error was calculated as sphere plus half negative cylinder, while AL, anterior chamber depth, and corneal curvature were assessed using optical biometry. Incident DR was defined as having no baseline DR and any DR at follow-up; incident vision-threatening DR as no baseline vision-threatening DR but present at follow-up; and DR progression as an increase in severity at follow-up from at least minimal baseline DR. Eye-specific data and generalized estimating equation models were used to account for between-eye correlation to determine the relationships between the exposures and outcomes, adjusted for traditional DR risk factors. Results At follow-up, 164 of 1273 (12.9%) eyes had incident DR, 17 of 1542 (1.1%) eyes had incident vision-threatening DR, and 75 of 269 (27.9%) eyes with baseline DR experienced progression. A longer AL (per millimeter increase) (risk ratio 0.58 [95% confidence interval 0.38–0.88) was associated with a lower risk of incident DR. No other associations were found. Conclusion Our findings show that a longer AL is protective of incident DR.

Wavefront-guided laser in situ keratomileusis (WFG-LASIK) and small incision lenticule extraction (SMILE) are keratorefractive surgeries that can improve uncorrected visual acuity in myopic patients. Comparison of visual outcomes in myopic patients treated with LASIK and SMILE is needed.Prospective, randomized contralateral eye-controlled trial.We performed a single-center prospective, randomized contralateral eye comparison of WFG-LASIK and SMILE (NCT03067077). Myopic patients with low levels of astigmatism were treated with WFG-LASIK in one eye and SMILE in the fellow eye from March 2017 to March 2021. Treatments were randomized by ocular dominance. WFG-LASIK and SMILE were performed. Postoperative evaluation at 1 day consisted of uncorrected distance visual acuity (UDVA), wavefront aberrometry, and a slit-lamp examination. On subsequent postoperative visits at 1 month, 3 months, 6 months, and 12 months, UDVA, manifest refraction, 5% and 25% low-contrast visual acuity, wavefront aberrometry, and slit-lamp examination were performed.The primary outcome measure was UDVA at 12 months.Eighty-eight eyes of 44 patients with myopia were enrolled in the study. Seventy-four eyes of 37 patients had successful treatments and completed 12 months of follow-up. At postoperative month 12, there were a significantly higher proportion of WFG-LASIK eyes that had ≥20/20 UDVA compared with SMILE eyes (94% vs 83%, P.05). There was no difference between spherical equivalent between WFG-LASIK eyes and SMILE eyes (-0.17 ± 0.25 vs -0.29 ± 0.38, P.05); there was no difference in higher order aberrations, including coma, trefoil, and spherical aberrations (P0.05); and there were a significantly higher proportion of WFG-LASIK eyes that had improved 5% and 25% low-contrast visual acuity compared with SMILE eyes (P0.05).WFG-LASIK and SMILE both offered marked improvements in corrected distance visual acuity and excellent predictability in both eyes. Compared with SMILE, WFG-LASIK resulted in faster visual recovery, better low-contrast visual acuity, and greater gains in uncorrected visual acuity.

To characterize visual outcomes in children screened for retinopathy of prematurity (ROP).Retrospective, interventional case series.Patients who received ROP screening examinations at UCLA Medical Centers and were followed with outpatient eye examinations at Stein Eye Institute and/or Doheny Eye Institute (Los Angeles, California) were included. Data were collected on birth characteristics, worst type of ROP, and ROP treatment. Adverse visual outcomes included myopia, strabismus, amblyopia, macular dragging, and optic atrophy. Snellen visual acuity was reported for children 4 years and older.A total of 175 infants (350 eyes) were included for analysis (mean gestational age = 28.2 weeks and birth weight = 1059 g) from a screening population of 539 infants (1078 eyes, 32.4% follow-up) over a 9-year period. Fifteen eyes received primary anti-vascular endothelial growth factor (anti-VEGF) therapy, whereas 59 eyes received primary laser therapy. Primary anti-VEGF therapy, as compared with primary laser treatment, was associated with a decreased incidence of amblyopia (adjusted odds ratio [aOR] = 0.6-0.86, P.0001) after controlling for gestational age and birth weight. The rates of optic atrophy (P = .79), strabismus (P = .98), and myopia (P = .93) were not different between anti-VEGF and laser treatment groups. Infants receiving anti-VEGF therapy had more posterior disease than laser-treated infants (P = .041). Infants receiving laser therapy were more likely to have severe myopia (aOR = 1.02-1.3, P = .023), amblyopia (aOR = 1.12-1.61, P = .002), and optic atrophy (aOR = 1.01-1.32, P = .045) than infants not treated.These findings add to the advantages of anti-VEGF treatment compared with primary laser treatment, particularly in posterior ROP.

To evaluate the influence of corneal epithelial thickness (CET) on the deviation from the targeted refraction (refractive error [RE]) post corneal refractive surgery.Retrospective, single-center, cross-sectional study.This study involved 211 eyes of 211 patients who previously underwent laser-assisted in situ keratomileusis (LASIK) or epipolis LASIK for the correction of myopia and myopic astigmatism from August 2000 to May 2014 at the Baptist Eye Institute, Kyoto, Japan, and who subsequently underwent examination of CET via optical coherence tomography imaging of 17 zones (central: n = 1 zone, paracentral: n = 8 zones, and midperipheral: n = 8 zones) within a 6.0-mm-diameter area of the central cornea from April 2014 to February 2015. The relationship between CET and RE was evaluated using Spearman coefficient in 5 parameters: CET in the (1) central, (2) paracentral, and (3) midperipheral zones, and the difference between the (4) central and paracentral zones and the (5) central and midperipheral zones. The influence of CET on RE was evaluated in the CET parameter that showed a significant correlation with RE using multiple liner regression analysis.The correlation coefficient with RE was (1) -0.238 (P.01), (2) -0.172 (P.05), (3) -0.002 (P = .98), (4) -0.186 (P.01), and (5) -0.266 (P .01), respectively. Multiple liner regression analysis revealed that the difference of mean CET between the central and midperipheral zones had a significant influence on RE (β coefficient = -0.028, P.01).The difference between CET at the central and midperipheral zones may play a role in the final RE post corneal refractive surgery.

To compare outcomes in wavefront-optimized (WFO) laser in situ keratomileusis (LASIK) to high-resolution wavefront-guided (WFG) LASIK.Randomized, fellow eye controlled, clinical trial.A total of 200 eyes of 100 patients with myopia or compound myopic astigmatism undergoing bilateral LASIK between October 2015 and February 2017 underwent wavefront-optimized (WFO) LASIK in 1 eye and wavefront-guided (WFG) LASIK in the fellow eye. Each eye was evaluated for uncorrected distance visual acuity (UDVA), corrected distance visual acuity (CDVA), contrast sensitivity, refractive error, and wavefront aberrometry. Outcome measurement primarily UDVA at 12 months. Secondary outcomes were safety, predictability, stability, refractive error, CDVA, contrast sensitivity, and higher-order aberrations at 12 months.WFG LASIK had higher levels of supervision and better contrast sensitivity compared to WFO. Both types had excellent postoperative UDVA results, without a significant difference in either mean UDVA or frequency of achieving a specified UDVA, for example, 20/16 or better (all P0.05). The mean gain in lines of CDVA was significantly higher in the WFG than in the WFO group (0.80 ± 0.75 vs 0.62 ± 0.72, respectively; P = 0.04). The WFG cohort had significantly better contrast sensitivity for mean and frequency of achieving20/40 for5% low-contrast sensitivity (37.1%; 95% confidence interval [CI], 27.5-46.7 vs 24.7%; 95% CI, 16.2-33.3, respectively; P = 0.01) and20/32 for25% low-contrast sensitivity (10.3%; 95% CI, 4.3-16.4 vs 4%, respectively; 95% CI, 0.0-8.1, respectively; P = 0.04). The WFO group had a significantly higher frequency of0.25 diopters (D) of refractive astigmatism at 12 months (82.5%; 95% CI, 75.0-90.0; vs 72.1%, respectively; 95% CI, 63.2-81.2; P = 0.02) but not0.50 D (95.6%; 95% CI, 91.9-99.8; vs 96.9, respectively; 95% CI, 93.5-100; P = 0.61). The WFG group was significantly closer to emmetropia for both sphere and spherical equivalents at all time points. Trefoil was significantly lower in the WFG group and was also significantly lower than preoperative measurements.Wavefront-optimized and high-resolution wavefront-guided LASIK achieve excellent visual outcomes. Nonetheless, treatments based on a high-resolution aberrometer appear to offer superior results in some regards.

Purpose To compare the accuracy and precision of the new Hill-RBF version 2.0 (Hill-RBF 2) formula with other formulas (Barrett Universal II, Haigis, Hoffer Q, Holladay 1, and SRK/T) in predicting residual refractive error after phacoemulsification in high axial myopic eyes. Design Retrospective case series. Methods 127 eyes of 127 patients with axial length (AL) ≥26 mm were included. The refractive prediction error (PE) was calculated as the difference between the postoperative refraction and the refraction predicted by each formula for the intraocular lens (IOL) power actually implanted. Standard deviation (SD) of PE, median absolute PE (MedAE), proportion of eyes within ±0.25, ±0.50, and ±1.00 diopter (D) of PE were compared. A generalized linear model was used to model the mean function and variance function of the PE (indicative of the accuracy and precision) with respect to biometric variables. Results The MedAE and SD of Hill-RBF 2 were lower than that of Hoffer Q, Holladay 1, and SRK/T (P ≤ .036) and were comparable to Barrett Universal II and Haigis (P ≥ .077). Hill-RBF 2 had more eyes within ±0.25 D of the intended refraction (76 out of 127 eyes [59.84%]) compared to other formulas (P ≤ .034) except Barrett Universal II (P = .472). AL was associated with the mean function or variance function of the PE for all formulas except Hill-RBF 2. Conclusions In this study, the precision of Hill-RBF 2 is comparable to Barret Universal II and Haigis. Unlike the other 5 formulas, its dispersion and the accuracy of the refractive prediction is independent of the AL.

To characterize the morphologic features of corneal parameters and their correlation with axial length (AL) elongation in patients with high axial myopia.Cross-sectional comparative study.A total of 299 eyes with high myopia (299 patients) and 266 eyes of age- and sex-matched control subjects (266 patients) were examined by anterior segment swept-source optical coherence tomography (SS-OCT) and an IOL Master. Central corneal thickness (CCT), thinnest corneal thickness (TCT), corneal volume (CV), and anterior corneal curvature (CC), posterior CC, and total CC were obtained, and their correlation with AL was investigated using multiple linear regression model.The CCT, TCT, CV, and anterior, posterior, and total CC of patients with high myopia were smaller than those of the control subjects. The CCT, TCT, and CV had a continuous downward trend with AL. However, CC decreased rapidly with AL when AL was26 mm and the slope decreased when AL was 26 to 28 mm. This linear association was absent when AL was28 mm. Multiple linear regression showed that the CCT (β = -1.98, P = .007), TCT (β = -1.63, P = .019), and CV (β = -0.13, P = .001) were associated with AL. Anterior, posterior, and total CCs were negatively associated with AL when AL was26 mm (all P.001) and when AL was between 26 and 28 mm (all P.05).CC decreases may serve as a refractive compensation on myopia when AL is28 mm. However, this effect does not exist when AL is28 mm. Consideration should be given to the special distribution of cornea curvature for IOL calculations in subjects with high myopia.

To investigate the occurrence and risk factors of clinically significant intraocular lens (IOL) decentration and tilt in highly myopic eyes using swept-source anterior segment optical coherence tomography (SS-AS-OCT).Cross-sectional study.This study included 334 participants (334 eyes) with high myopia, defined as axial length (AL) ≥26 mm, who underwent phacoemulsification with IOL implantation. Decentration and tilt of IOL were assessed by SS-AS-OCT. Clinically significant IOL decentration and tilt was defined as decentration ≥0.4 mm and tilt ≥7°. Routine preoperative and postoperative examinations included visual acuity, refraction, biometric measurement using IOLMaster 700 (Carl Zeiss Meditec), and objective visual quality evaluated by OPD-Scan III (Nidek Technologies).Among the 334 highly myopic participants, 71 (21.3%) had clinically significant IOL decentration, and 26 (7.78%) had clinically significant IOL tilt. The proportion of clinically significant IOL decentration (37.1% vs 14.0%, P.001) and tilt (16.2% vs 3.90%, P.001) in those with AL ≥30 mm was significantly higher than in those with AL30 mm. The multivariable logistic regression model showed only AL ≥30 mm was associated with clinically significant IOL decentration (odds ratio, 1.65; P = .002). AL ≥30 mm (odds ratio, 2.09; P = .001) was an independent risk factor for clinically significant IOL tilt after adjusting for confounders. AL ≥30.3 mm could effectively predict IOL decentration ≥0.6 mm (area under the curve, 0.802).Participants with AL30 mm have a higher risk of clinically significant IOL decentration and tilt, thus caution should be taken to implant multifocal or toric IOL for these patients.

PURPOSE The purpose of this study was to investigate how mild-to-moderate myopia and aging affect visual field sensitivity (VF-S) in normal eyes, correcting for effects of each. DESIGN Combined cross-sectional and cohort study. METHODS Two normal groups, a cross-sectional group (n = 703; 1,051 eyes; mean age, 52.6 years) and a longitudinal group (n = 44; 83 eyes; mean age, 52.3 years; follow-up, 4.2 years; VF tests, 12) were included. In the cross-sectional group, the mean VF-S of the entire field and 3 disc portion-oriented subfields of the Humphrey Field Analyzer 24-2 program were correlated with subjects' age, axial length (AL), disc, rim and β-peripapillary area, and disc ovality and torsion, using linear mixed-regression models. Their time changes in the longitudinal group were correlated with time, subjects' ages, and AL using linear mixed-regression models. RESULTS In the cross-sectional group, the VF-S correlated negatively with age (-0.081 decibel [dB]/year; P < .001), which was more negative (P = .020) in the midperipheral than the central subfield, and with AL (P = .049) without intersubfield differences. In the longitudinal group, no changes in the ocular media were significant, and the VF-S declined by 0.074 dB/year (P = .007), which accelerated with higher age (P < .002) and baseline VF-S (P < .001) without intersubfield differences. The AL showed little effects on the VF-S longitudinal changes. CONCLUSIONS In normal eyes with mild-to-moderate myopia, the VF-S was lower subfield-independently with longer AL, whereas the AL had little effect on the aging-associated VF-S reduction. The VF-S decreased with aging with intersubfield differences. The aging-associated VF-S reduction accelerated with higher age, to which the ocular media changes were unrelated.
