Purpose: To report branch retinal artery occlusion caused by diathermy of a persistent hyaloid artery., Methods: Case report. A 21-year-old woman with unilateral tractional retinal detachment caused by contraction of the persistent primary vitreous underwent vitrectomy with endocoagulation of the primary hyaloid artery., Results: Postoperatively, the patient had opacification of the neurosensory retina in the area of the inferotemporal branch retinal artery accompanied by a corresponding visual field defect., Conclusions: Diathermy of a persistent hyaloid artery may be complicated by the occlusion of a branch retinal artery. In similar cases, atypical branches of retinal arteries must be considered. Dissection of the anomaly from the posterior lens capsule should be performed without coagulation or perhaps endoscopically if visualization is obscured by a Mittendorf dot.

Two young patients (27 and 30 years old, respectively) with long-standing diabetes mellitus underwent vitreoretinal surgery for traction retinal detachment and vitreous hemorrhage. The postoperative course in each was characterized by early recurrence of the hemorrhage into the vitreous cavity, hypotony, and severe visual loss. Histopathologic examination of both operated on eyes showed anterior extraretinal fibrovascular proliferation extending along the anterior hyaloid to the posterior lens surface, causing traction detachments of the peripheral retina and ciliary body. The vessels originated from the anterior retina. There was no evidence of excess fibrous tissue at the well-healed sclerotomy wounds.

Vitrectomy was performed to treat 74 consecutive eyes for complications of diabetic retinopathy. Eight (13%) of 61 eyes followed up for an average of 12 months developed anterior hyaloidal fibrovascular proliferation. This was the most common postoperative complication, whose features included recurrent hemorrhages into the vitreous cavity or anterior vitreous, or both; vessels or fibrovascular tissue on the posterior lens capsule; anterior extraretinal vascularization extending toward the lens on the anterior hyaloid; traction detachment of the peripheral retina or ciliary body; and hypotony. Patients who developed this complication tended to be young males with severe retinal neovascularization and extensive retinal ischemia; traction retinal detachment as an indication for surgery; placement of a scleral buckle; postoperative rubeosis iridis, recurrent vitreous hemorrhages, and retinal detachment; and multiple surgeries. Four eyes progressed to atrophia bulbi. Early recognition followed by additional surgery in two patients and extensive additional photocoagulation in two other patients was successful in preserving good visual function.

We reviewed 27 consecutive eyes with peripheral uveitis and vitreous base neovascularization that had been treated with cryopexy and followed up for a median of 4.5 years. During the follow-up period, 21 eyes (78%) remained quiescent, whereas five eyes (18%) demonstrated intermittent inflammation, although only one of these eyes progressed to a traction retinal detachment. One eye (4%) eventually atrophied; however, this was believed to be a result of the ongoing uveitis rather than the cryopexy. The treated eyes had an average improvement of three lines in Snellen visual acuity. We found that corticosteroid therapy remains the primary treatment modality for active inflammation, and vitreous base cryopexy should be reserved for those cases which are resistant to corticosteroids, and which demonstrate active neovascularization.

A 25-year-old man with systemic sarcoidosis had neovascularization of the optic nerve heads and associated bilateral vitreous hemorrhages. Oral administration of prednisone produced a prompt resolution of these unusual neovascular fronds and cleared the vitreous hemorrhages.
