De Garengeot's hernia, the presence of an incarcerated vermiform appendix within a femoral hernia, is a rare general surgery emergency that predominantly affects elderly female patients. Due to its rarity, there is significant variation in surgical technique; however, most case reports favor an open approach. Here we present a case of a De Garengeot's hernia with a unique hybrid open and laparoscopic repair, utilizing the hernia defect as a port site. We will also review the relevant literature. The artic presents the case study of 63-year-old white female presented to the ED with one month of malaise, chills, and ten kilogram weight loss. Topics include the CT angiogram has foreign body resembling a fish bone eroding through the third portion of the duodenum into the portal vein, the patient underwent an exploratory laparotomy with Kocher maneuver to mobilize the duodenal stump, and the penetration site has inspected by repeat duodenoscopy before closing the duodenotomy transversely.

The article presents a case study of a 56-year-old man with Class II obesity, Type II diabetes mellitus, tobacco use and a complex surgical history of multiple abdominal operations. Topics include synthetic mesh being preferred to biologic mesh because of its relative durability, lower cost and superior outcomes in complex abdominal reconstruction; and patients risking separation or necrosis of the overlying subcutaneous tissue, potentially causing autolysis of the mesh.
