A 36-year-old unmarried man presented to our colorectal surgery clinic with symptoms of anal pain for 2 months and anal itching for 1 month. Secondary syphilis presents with systemic symptoms, including skin rash, mucous lesions, lymphadenopathy, and fever, resulting from systemic spreading of spirochetes.[5] The skin manifestations of secondary syphilis vary widely from macular, papular and pustular to follicular lesions, and itching may occur.[5] The symptoms of primary syphilis may be ignored by patients and the disease is then diagnosed at a later stage (so-called Syphilis D' emblée).[6] Our patient did not have chancre and came directly to our colorectal surgery clinic for perianal skin symptoms. Based on the exposure history and clinical data, the reasoned diagnosis was secondary syphilis after undergoing skin tattooing. [Extracted from the article]
