A 26-year-old female with undiagnosed multiple endocrine neoplasia (MEN) 2A syndrome presented with a large haemorrhagic phaeochromocytoma of the left adrenal gland, leading to acute catecholamine cardiomyopathy. The patient underwent angioembolisation and alpha- and beta-blockade treatment, resulting in the resolution of cardiac dysfunction. Following the excision of the tumour, the patient is awaiting surgery for medullary thyroid carcinoma and primary hyperparathyroidism. This case highlights the importance of patience and multidisciplinary management in the acute care of phaeochromocytoma, emphasizing the need for specialized care in such complex cases.
