La autonomía ha sido vista como un indicador de la profesionalización de la disciplina. La autonomía de enfermería ha sido identificada como una variable importante que afecta la percepci ón de la enfermera en relación con su satisfacción en el trabajo, los ambientes positivos de su práctica profesional y la calidad del cuidado de enfermería. La suposición de que un aspecto clave de la autonom ía de enfermería es actuar con un juicio y práctica independiente, fue cuestionada en un estudio interpretativo que exploró la forma como las enfermeras entienden la autonomía en su práctica clínica y en su trabajo. Este artículo describe el proyecto de investigación, y explora el énfasis que hacen las enfermeras sobre los aspectos inherentes al ejercicio de la autonomía en su trabajo clínico. Las descripciones de sus relaciones con los médicos, colegas y directores de enfermería, ofrecen muchos ejemplos en los cuales la práctica autó- noma puede ser respaldada y fortalecida. Este artículo también discute estas posibilidades para que este concepto se pueda llevar a cabo en tres áreas claves: crear ambientes de práctica que enriquezcan la pericia clínica y la toma de decisiones; hacer posible que las enfermeras practiquen, aplicando todas sus capacidades educativas, sus experiencias y competencias; y fortalecer la práctica multidisciplinaria con colaboración. Autonomy has been viewed as a key indicator of disciplinary professionalization. Nursing autonomy has been identified as an important variable that affects nurses’ perceptions of job satisfaction, positive professional practice environments, and quality nursing care. The assumption that a key feature of nursing autonomy is the exercise of independent judgment and practice was challenged in an interpretive study that explored how nurses understand autonomy in their everyday clinical practice and worklife. This paper describes the research project, and explores nurses’ emphasis on the relational features inherent in the exercise of autonomy in their clinical work. Their descriptions of relationships with physicians, nursing peers, and nurse managers offered ample instances whereby autonomous practice could be supported and enhanced. This paper will also discuss these possibilities for enabling nursing autonomy in relation to three key areas: generating practice environments that nurture clinical expertise and decision-making; enabling nurses to practice to the full scope of their education, experience, and competence; and fostering collaborative multidisciplinary practice.

Effective medical treatment with uninterrupted engagement in care is critical to improving the survival and the quality of life of patients infected with the human immunodeficiency virus (HIV). Objectives: Multiple behavioral interventions have been conducted to promote adherence behaviors. However, adherence to HIV medications and medical appointments is still an issue of global concern. Method: The Health Promotion Model (HPM) is a nursing adaption of the health belief model. The HPM focuses on individual characteristics and experiences, as well as behavior specific cognitions and outcomes. Integrating the HPM in addressing adherence behaviors could be one of the building blocks of success in changing health behavior. Results: A search of the literature turned up no studies that applied the HPM in adherence behavior studies conducted among HIV-infected populations. Conclusion: This paper presents the reader with the availability of current adherence-behavior interventions and strategies that align with the HPM model components. It further proposes the need for medical treatment team members to adopt the HPM in current clinical practice settings so as to effectively address adherence behavior issues. doi: 10.5294/aqui.2016.16.4.2

Aim: The purpose of this study is to report the results of an integrative review of use of the International Classification of Functioning, Disability and Health (ICF) by nurses. This review is justified by the need for further investigation into use of the ICF in nursing, by nurses, so as to contribute to dialogue for the development of global, interdisciplinary and classification system comprehension. Methods: This integrative review included studies from 2001 to May 2015 in peer-reviewed journals. Two reviewers independently screened titles and abstracts for inclusion and completed data extraction. The papers were sourced from a number of electronic databases: MEDLINE/NML/PubMed, Web of Science, LILACS, CINAHL and SCIELO. Results: Twenty-six articles were included. Three themes on use of the ICF were identified: clinical nursing practice; teaching nursing students and nurses; and nursing research. Conclusion: This review helps nurses to visualise how and in what care environments the ICF is being used. It demonstrates the ICF has been used specifically in the rehabilitation nursing field, but offers some evidence of its use in occupational health as well. DOI: 10.5294/aqui.2015.15.3.4

This paper presents an explanation of a three-step process for the use of a conceptual model to guide nursing research and/or practice. Step 1 involves learning the content of the conceptual model and its research and practice guidelines. Step 2 requires a review of literature about the use of the model as a basis for research or practice. Step 3 focuses on construction and communication of a conceptual-theoretical-empirical structure for diverse research topics and practice situations. Emphasis is placed on application of the process to one conceptual model of nursing: the Roy Adaptation Model.

Las competencias laborales de enfermería permiten disminuir inequidades, acceso limitado a los servicios y favorecen una atención segura y de calidad. La formación de enfermeras/os es fundamental en la composición y dinámica de la fuerza de trabajo en enfermería, la calidad y pertinencia de los cuidados y el desarrollo de la capacidad institucional en salud. Objetivo: responder dos interrogantes: ¿Cuáles son los requerimientos actuales para el/la enfermero/a en el ámbito de la gestión y administración?, y ¿cuáles son las competencias de gestión de enfermería más importantes y su relación con la gestión de cuidado? Método: revisión de la literatura que parte con una exploración de campo desde los contextos y desafíos actuales de la profesión. La búsqueda bibliográfica se realizó en las bases de datos Cochrane Library, Lilacs, CINHAL, Dialnet, Universidad de la Rioja, ProQuest, Web of Science, Science Direct, PubMed, Business Source Premier, SciELO, Scopus. Se encontraron 4.697 artículos. Precisando la búsqueda se seleccionaron 51 artículos y documentos técnicos. Resultados: los estudios que priorizan las competencias de gestión coinciden en colocar entre las más importantes: comunicación, trabajo en equipo, resolución de conflictos y habilidades interpersonales y de liderazgo. En liderazgo priorizan dominio personal, efectividad interpersonal, gestión financiera y de recursos humanos. Conclusiones: las habilidades de gestión y liderazgo se deben basar en conocimientos sólidos y en la experiencia. Asimismo, la concepción de gestión y liderazgo ocupa un lugar complementario al rol de cuidado que tiene la enfermera/o. El liderazgo transformacional y relacional son necesarios para mejorar la satisfacción de las/os enfermeras/os. The job skills involved in nursing make it possible to reduce inequalities and limited access to services, and to support safe and quality care. The training nurses receive is fundamental to the composition and dynamics of the nursing work force, the quality and appropriateness of nursing care, and the development of institutional capacity in the health sector. Objective: The study was designed to answer two questions: What are the current requirements for nurses in the field of management and administration; and what nursing management skills are most important and how do they relate to health care management? Method: The literature review began by exploring the field in terms of current contexts and challenges to the profession. The bibliographic search involved several the databases; namely, the Cochrane Library, Lilacs, CINAHL, Dialnet, the University of La Rioja, ProQuest, Web of Science, ScienceDirect, PubMed, Business Source Premier, SciELO and Scopus. In all, 4,697 articles were found. When the search was narrowed, 51 technical papers were selected. Results: The studies that prioritize management skills coincide in that the most important ones are: communication, teamwork, conflict resolution, and interpersonal and leadership skills. As for leadership, the priority is on personal mastery, interpersonal effectiveness, financial management and human resources. Conclusions: Management and leadership skills must be based on sound knowledge and experience. Moreover, the concept of management and leadership is complementary to the role of nurses in providing care. Transformational and relational leadership are needed to improve the satisfaction of nurses. As competências laborais de enfermagem permitem diminuir inequidades, acesso limitado aos serviços e favorecem um atendimento seguro e de qualidade. A formação de enfermeiras/os é fundamental na composição e dinâmica da força de trabalho em enfermagem, na qualidade e pertinência dos cuidados e no desenvolvimento da capacidade institucional em saúde. A liderança transformacional e relacional são necessárias para melhorar a satisfação das/os enfermeiras/os.

Objetivo: el incremento en la producción de conocimiento representa para los científicos una preocupación, dado que diariamente se enfrentan a la decisión de elegir la literatura de mayor mérito científico para respaldar la toma de decisiones para la investigación y la práctica. La presente investigación tiene por objeto construir dos instrumentos registrados4, con propiedades psicométricas, validez facial y validez de contenido. Materiales y métodos: para el Instrumento de Criterios de Elegibilidad de los Artículos de Investigación para el Metaanálisis (ICrESAI) se proponen los criterios que se deben evaluar para determinar cuáles artículos de investigación deben ser elegibles para una revisión sistemática (RS) o metaanálisis (MA); así mismo, se calculó la fiabilidad interevaluador. El segundo es el Instrumento de Criterios para Evaluar el Mérito Científico de los Artículos Seleccionados para el Metaanálisis (IMeCI), el cual propone criterios que se deben evaluar para calificar el mérito científico y la calidad metodológica de los artículos seleccionados para adelantar una RS. Los cálculos para validez facial y de contenido se hicieron con tres investigadores expertos en metodología de la investigación y en medición. La validez de contenido se calculó con el Índice de Lawshe Modificado (Tristán-López, 2008), y la fiabilidad intercodificador se midió para variables cualitativas con el Índice de Kappa de Cohen. Resultados: el instrumento ICrESAI obtuvo un índice de validez de contenido (CVI) de 1,0. Para el instrumento IMeCI, el CVI fue de 0,75. En el instrumento ICrESAI los valores para el índice de fiabilidad o concordancia intercodificador estuvieron entre 0,547 y 0,973, considerados buenos de acuerdo con la interpretación propuesta por Cohen. Conclusiones: los dos instrumentos generados cuentan con validez (CVI y Lawshe) para realizar la selección y evaluación del mérito científico de las piezas de investigación. Estas dos herramientas son útiles en el estudio de cualquier área temática y al orientar el uso de la información para la toma de decisiones en la investigación y la práctica basada en evidencia. Materials and Methods: The Research Article Eligibility Criteria Instrument (ICrESAI) proposes standards to be assessed to determine which research papers should be eligible for a systematic review (SR) or meta-analysis (MA). Inter-reevaluator reliability was calculated as well. The second instrument involves Criteria for Evaluating the Scientific Merit of Articles Selected for Meta-analysis (IMeCI), which proposes standards to be evaluated to rate the scientific merit and methodological quality of the articles selected for a SR. The estimates for face and content validity were made with three researchers who are experts in research methodology and measurement. Content validity was calculated using the Modified Lawshe Index (Tristán -López , 2008) and inter-codifier reliability was measured for qualitative variables using Cohen's Kappa Index. Results: The ICrESAI instrument obtained a content validity index (CVI) of 1.0. The CVI for the IMeCI instrument was 0.75. With the ICrESAI instrument, the values for the inter-codifier reliability or concordance index were between 0.547 and 0.973, which is considered good according to the interpretation proposed by Cohen. Conclusions: Both instruments are valid (CVI and Lawshe) for selecting and evaluating the scientific merit of research articles. These two tools are useful to the study of any subject area and to guide the use of information for decision-making in research and evidence-based practice. Objetivos: o aumento na produção de conhecimento representa para os cientistas uma preocupação porque diariamente lidam com a decisão de escolher a literatura de maior mérito científico para respaldar a tomada de decisões para a pesquisa e prática. Esta pesquisa tem por objetivo construir dois instrumentos registrados6, com propriedades psicométricas, validade facial e validez de conteúdo. Materiais e métodos: para o Instrumento de Critérios de Escolha dos Artigos de Pesquisa para a Meta-análise (ICrESAI), propõem-se os critérios que se devem avaliar para determinar quais artigos de pesquisa devem ser eleitos para uma revisão sistemática (rs) ou metaanálise (ma); além disso, calculou-se a fiabilidade interavaliador. O segundo é o Instrumento de Critérios para Avaliar o Mérito Científico dos Artigos Selecionados para a Meta-análise (IMeCI), o qual propõe critérios que se devem avaliar para qualificar o mérito científico e a qualidade metodológica dos artigos selecionados para realizar uma rs. Os cálculos para validade facial e de conteúdo foram feitos com três pesquisadores especialistas em metodologia da pesquisa e em medição. A validade de conteúdo foi calculada com o Índice de Lawshe Modificado (Tristán-López, 2008), e a fiabilidade intercodificador foi medida para variáveis qualitativas com o Índice de Kappa de Cohen. Resultados: o instrumento IcrESAi obteve um índice de validade de conteúdo (cvi) de 1,0. Para o instrumento IMeCI, o cvi foi de 0,75. No instrumento ICrESAI os valores para o índice de fiabilidade ou concordância intercodificador estiveram entre 0,547 e 0,973, considerados bons de acordo com a interpretação proposta por Cohen. Conclusões: os dois instrumentos gerados possuem validade (cvi e Lawshe) para realizar a seleção e avaliação do mérito científico das peças da pesquisa. Estas duas ferramentas são úteis no estudo de qualquer área temática e na orientação do uso da informação para a tomada de decisões em pesquisa e prática baseada em evidência.

El presente trabajo tiene el propósito de construir una escala de evaluación de la conducta ética del personal de enfermería en el cuidado de los pacientes basada en las tres premisas fundamentales de la teoría del cuidado de Jean Watson. La versión final del cuestionario consta de 13 reactivos tipo Likert que reflejan los contenidos de las premisas fundamentales de la mencionada teoría. La validez del cuestionario se determinó por medio del análisis factorial con extracción de componentes principales y rotación varimax. Del análisis surgieron cuatro factores que explican el 64% de la varianza. Tres de los factores corresponden a las premisas fundamentales de la teoría, mientras que el cuarto factor está relacionado con la comunicación y la empatía. La adecuación de la muestra para el análisis factorial se confirmó mediante el cociente de Kaiser-Meyer-Olkin (KMO = 0,848), la prueba de esfericidad de Bartlett (p < 0,001) y el cálculo de la determinante de la matriz de correlaciones (0,006). La contabilidad por consistencia interna de la escala global es alta (a = 0,85). Se concluye que el instrumento es válido, confiable, y representa un medio conveniente para evaluar el comportamiento ético del personal de enfermería al proporcionar cuidado al paciente hospitalizado. The purpose of this paper is to construct a scale to evaluate ethical conduct in nursing care, based on the three fundamental premises of Jean Watson's caring theory. The final version of the questionnaire consists of 13 Likert-type response categories that reflect the contents of the basic assumptions of that theory. The validity of the questionnaire was determined through factor analysis with principal component extraction and varimax rotation. The analysis suggested four factors that explain 64% of the variance. Three of the factors pertain to the basic premises of the theory, while the fourth is related to communication and empathy. Aim: The purpose of this study is to report the results of an integrative review of use of the International Classification of Functioning, Disability and Health (ICF) by nurses. This review is justified by the need for further investigation into use of the ICF in nursing, by nurses, so as to contribute to dialogue for the development of global, interdisciplinary and classification system comprehension. Methods: This integrative review included studies from 2001 to May 2015 in peer-reviewed journals. Two reviewers independently screened titles and abstracts for inclusion and completed data extraction. The papers were sourced from a number of electronic databases: MEDLINE/NML/PubMed, Web of Science, LILACS, CINAHL and SCIELO. Results: Twenty-six articles were included. Three themes on use of the ICF were identified: clinical nursing practice; teaching nursing students and nurses; and nursing research. Conclusion: This review helps nurses to visualise how and in what care environments the ICF is being used. It demonstrates the ICF has been used specifically in the rehabilitation nursing field, but offers some evidence of its use in occupational health as well. DOI: 10.5294/aqui.2015.15.3.4

Objetivo: el presente estudio se propone comprender la percepción de la acogida de los enfermeros que actúan en la Atención Primaria a la Salud. Método: es una investigación cualitativa realizada con entrevistas con 21 enfermeros de ocho municipios del interior del estado de São Paulo. El análisis de los datos se fundamentó en la vertiente fenomenológica, modalidad estructura del fenómeno. Resultados: se reveló que la comprensión de los enfermeros sobre la acogida, en algunos momentos, se aproxima de los principios y directrices del Sistema Único de Salud (SUS). Conclusiones: por otro lado, esos profesionales manifiestan dificultades con la estructura, organización y gestión de los servicios de salud y expresan acciones y creencias que se acercan al modelo tradicional de cuidado. Para la concreción de la acogida necesitan unir sus esfuerzos de los diferentes atores involucrados. Objective: This paper is intended to explore how nurses working in primary health care perceive acceptance. Method: It is based on a qualitative study conducted through interviews with 21 nurses from eight municipalities in the State of São Paulo. A phenomenological approach was used in data analysis. Results: It was found that nurses’ understanding of acceptance, at certain points in time, comes close to the principles and guidelines of the Unified Health System (SUS). Conclusions: Nursing professionals manifest difficulties with the structure, organization and management of health services and express actions and beliefs related to the traditional model of care. The various stakeholders need to join forces if acceptance is to be achieved. Objetivo: o presente estudo propõe-se a compreender a percepção de enfermeiros que atuam na Atenção Primária à Saúde sobre o acolhimento. Método: pesquisa qualitativa realizada a partir de entrevistas com 21 enfermeiros de oito municípios do interior paulista. A análise dos dados pautou-se na vertente fenomenológica, modalidade estrutura do fenômeno. Resultados: revelou-se que a compreensão dos enfermeiros sobre acolhimento, em alguns momentos, se aproxima dos princípios e diretrizes do SUS. Por outro lado, manifestam dificuldades com a estrutura, organização e gestão dos serviços de saúde, além de expressarem ações e crenças que se aproximam do modelo tradicional de cuidado. Conclusões: diante dos resultados, constata-se a união de esforços dos diferentes atores envolvidos no processo.
