Abstract Background It is well established that social exclusion is a key social determinant of health; however, such association between social exclusion and health outcomes among older people remain a relatively under-researched area. This paper explores the effects of four dimensions of social exclusion on self-rated health and depression among older people in China. Methods This paper includes 8038 individuals aged 60 and over from the first wave national multi-stage probability sample (2014) from the China Longitudinal Aging Social Survey (CLASS). Descriptive univariate information for individual variables and four dimensions of social exclusion are presented. Multinomial and binary logistic regression models are used to examine the associations between social exclusion and self-rated health and depression. Results Older people who were in the lower level of exclusion from social relationships or subjective feelings of exclusion were significantly less likely to report fair or poor self-rated health than people in the higher level of exclusion (lower level of exclusion from social activities was significantly associated with being less likely to report poor SRH only). Older people who were in the lower level of subjective feeling of exclusion or exclusion from financial products were significantly less likely to report depression. Conclusions Different dimensions of social exclusion have different effects on self-rated health and depression. Social policies need to reflect this and efforts of services could usefully be oriented to prevent multi-dimensions of social exclusion. Highlights • The standard positive correlation between religiosity and well-being is found in an elderly Japanese sample. • However, multivariate analyses that include agreeableness and conscientiousness as rival predictors radically alters the findings — religiosity's unique well-being is substantially weakened and often statistically annulled. • Personality and sociodemographic variables are consistently found to be more robust well-being predictors than religiosity. • These findings raise considerable doubt over the claim that religiosity contributes to successful aging in Japan. • Recent calls for the faith-status of patients utilizing gerontology services to be assessed for diagnostic purposes appear pre-mature. Abstract Recent findings suggest that the belief-as-benefit effect (BABE) — the positive association between religiosity and health/well-being — is a spurious correlation voided by personality traits. The current paper investigates the cross-sectional relationships among personality, religiosity and psychological well-being in an older adult sample randomly-selected from Tokyo, Japan. Correlation and Hierarchical Regression Modelling (HRM)—with a two one-sided test (TOST) of equivalence—is utilized. The standard BABE correlation is reproduced. However, HRM utilizing a meaningful benchmark of effect (β ≥.15) largely neuters the result after controlling for trait agreeableness and conscientiousness. Religiosity does remain statistically related to the purpose in life and positive relationships sub-scales; though it explains just a sliver of variance in both instances. Compared to religiosity, agreeableness, conscientiousness and education level were more substantial and consistent well-being predictors. Whether religiosity auspices the psychological health of older Japanese adults thus remains to be established. [ABSTRACT FROM AUTHOR]

Abstract Background Social exclusion (SE) is a process that limits participation in society across life domains, and is associated with poor quality of life. Neighbourhood exclusion has been identified as particularly important for older adults. This paper examines the association between SE and well-being in older adults from urban and rural areas, focusing on neighbourhood exclusion. Methods Using a cross-sectional survey design with a stratified sampling frame, participants (aged 65+) from rural (n = 628) and urban (n = 627) areas of Barnsley, United Kingdom, completed a questionnaire containing indicators of five SE domains: civic activity, material resources, social relationships, services and neighbourhood. Sequential multiple regression models were developed for 1) total sample; 2) rural areas; and 3) urban areas, with well-being regressed on SE indicators after controlling for self-reported health. Results SE indicators explained 13.4% of the variance in well-being in the total sample (of which neighbourhood exclusion explained 1.2%); corresponding figures for the rural model were 13.8% (3.8%) and for the urban model 18.0% (1.7%); the addition of neighbourhood exclusion significantly improved all three models. Five SE indicators were significant in the rural model, compared with seven in the urban model, with four common to both. Discussion Neighbourhood exclusion explained more variance in well-being in rural than urban areas, whereas exclusion from services explained more variance in urban than rural areas. Area characteristics and the role of neighbourhood should be considered in policy initiatives to reduce SE and promote well-being. [ABSTRACT FROM AUTHOR]

• Research on unmet care needs lacks contributions based on qualitative data. • Only practical or physical care needs are recognized whereas psychosocial needs are ignored. • With content analysis of interview data, the psychosocial nature of care needs is revealed. • Informal carers may act as buffers and meet unmet needs on behalf of care users. Unmet care needs are usually defined in terms of receiving sufficient help in instrumental activities and activities of daily living. Research on unmet needs is mostly based on quantitative data. Older persons' and informal carers' views and experiences have received less attention. In this paper, we rely on a definition of unmet needs which includes both unmet needs due to insufficient care and those situations where informal carers experience undue strain. Using theory-driven content analysis, we examine community-dwelling older adults' and their informal carers' experiences of unmet needs: what kind of unmet needs they have, why and in which ways these needs are left unmet and what would they want to do to improve the situation. The data consists of interviews gathered in Austria, Finland and Slovenia. Results of the analysis reveal that unmet needs are largely psychosocial in nature. The predominating task-oriented care systems often do not consider these as care needs. Using methods of qualitative content analysis, we conclude that care users' unmet psychosocial needs are related to lacking a personal relationship with care workers; means to maintain or develop social contacts and pursue activities and interests; and adequate home care services or respite care. Excessive responsibilities are put on informal carers as they top up and fill in the insufficient care. This study contributes to a broader understanding of unmet care needs: the relational aspects of care and the universal nature of psychosocial care needs should be addressed in care services.
