No comprehensive review of Bayesian networks (BNs) in healthcare has been published in the past, making it difficult to organize the research contributions in the present and identify challenges and neglected areas that need to be addressed in the future. This unique and novel scoping review of BNs in healthcare provides an analytical framework for comprehensively characterizing the domain and its current state. A literature search of health and health informatics literature databases using relevant keywords found 3810 articles that were reduced to 123. This was after screening out those presenting Bayesian statistics, meta-analysis or neural networks, as opposed to BNs and those describing the predictive performance of multiple machine learning algorithms, of which BNs were simply one type. Using the novel analytical framework, we show that: (1) BNs in healthcare are not used to their full potential; (2) a generic BN development process is lacking; (3) limitations exist in the way BNs in healthcare are presented in the literature, which impacts understanding, consensus towards systematic methodologies, practice and adoption; and (4) a gap exists between having an accurate BN and a useful BN that impacts clinical practice. This review highlights several neglected issues, such as restricted aims of BNs, ad hoc BN development methods, and the lack of BN adoption in practice and reveals to researchers and clinicians the need to address these problems. Background: The application of artificial intelligence to interpret the electrocardiogram (ECG) has predominantly included the use of knowledge engineered rule-based algorithms which have become widely used today in clinical practice. However, over recent decades, there has been a steady increase in the number of research studies that are using machine learning (ML) to read or interrogate ECG data.Objective: The aim of this study is to review the use of ML with ECG data using a time series approach.Methods: Papers that address the subject of ML and the ECG were identified by systematically searching databases that archive papers from January 1995 to October 2019. Time series analysis was used to study the changing popularity of the different types of ML algorithms that have been used with ECG data over the past two decades. Finally, a meta-analysis of how various ML techniques performed for various diagnostic classifications was also undertaken.Results: A total of 757 papers was identified. Based on results, the use of ML with ECG data started to increase sharply (p < 0.001) from 2012. Healthcare applications, especially in heart abnormality classification, were the most common application of ML when using ECG data (p < 0.001). However, many new emerging applications include using ML and the ECG for biometrics and driver drowsiness. The support vector machine was the technique of choice for a decade. However, since 2018, deep learning has been trending upwards and is likely to be the leading technique in the coming few years. Despite the accuracy paradox, accuracy was the most frequently used metric in the studies reviewed, followed by sensitivity, specificity, F1 score and then AUC.Conclusion: Applying ML using ECG data has shown promise. Data scientists and physicians should collaborate to ensure that clinical knowledge is being applied appropriately and is informing the design of ML algorithms. Data scientists also need to consider knowledge guided feature engineering and the explicability of the ML algorithm as well as being transparent in the algorithm's performance to appropriately calibrate human-AI trust. Abstract: Objective: This paper introduces a modified artificial immune system (AIS)-based pattern recognition method to enhance the recognition ability of the existing conventional AIS-based classification approach and demonstrates the superiority of the proposed new AIS-based method via two case studies of breast cancer diagnosis. Methods and materials: Conventionally, the AIS approach is often coupled with the k nearest neighbor (k-NN) algorithm to form a classification method called AIS-kNN. In this paper we discuss the basic principle and possible problems of this conventional approach, and propose a new approach where AIS is integrated with the radial basis function – partial least square regression (AIS-RBFPLS). Additionally, both the two AIS-based approaches are compared with two classical and powerful machine learning methods, back-propagation neural network (BPNN) and orthogonal radial basis function network (Ortho-RBF network). Results: The diagnosis results show that: (1) both the AIS-kNN and the AIS-RBFPLS proved to be a good machine leaning method for clinical diagnosis, but the proposed AIS-RBFPLS generated an even lower misclassification ratio, especially in the cases where the conventional AIS-kNN approach generated poor classification results because of possible improper AIS parameters. For example, based upon the AIS memory cells of “replacement threshold=0.3”, the average misclassification ratios of two approaches for study 1 are 3.36% (AIS-RBFPLS) and 9.07% (AIS-kNN), and the misclassification ratios for study 2 are 19.18% (AIS-RBFPLS) and 28.36% (AIS-kNN); (2) the proposed AIS-RBFPLS presented its robustness in terms of the AIS-created memory cells, showing a smaller standard deviation of the results from the multiple trials than AIS-kNN. For example, using the result from the first set of AIS memory cells as an example, the standard deviations of the misclassification ratios for study 1 are 0.45% (AIS-RBFPLS) and 8.71% (AIS-kNN) and those for study 2 are 0.49% (AIS-RBFPLS) and 6.61% (AIS-kNN); and (3) the proposed AIS-RBFPLS classification approaches also yielded better diagnosis results than two classical neural network approaches of BPNN and Ortho-RBF network. Conclusion: In summary, this paper proposed a new machine learning method for complex systems by integrating the AIS system with RBFPLS. This new method demonstrates its satisfactory effect on classification accuracy for clinical diagnosis, and also indicates its wide potential applications to other diagnosis and detection problems. Objective In this paper, we propose a novel algorithm to extract the active intervals of intracardiac electrograms during atrial fibrillation. Methods First, we show that the characteristics of the signal waveform at its inflection points are prominent features that are implicitly used by human annotators for distinguishing between active and inactive intervals of IEGMs. Then, we show that the natural logarithm of features corresponding to active and inactive intervals exhibits a mixture of two Gaussian distributions in three dimensional feature space. An Expectation Maximization algorithm for Gaussian mixtures is then applied for automatic clustering of the features into two categories. Results The absolute error in onset and offset estimation of active intervals is 6.1 ms and 10.7 ms, respectively, guaranteeing a high resolution. The true positive rate for the proposed method is also 98.1%, proving the high reliability. Conclusion The proposed method can extract the active intervals of IEGMs during AF with a high accuracy and resolution close to manually annotated results. Significance In contrast with some of the conventional methods, no windowing technique is required in our approach resulting in significantly higher resolution in estimating the onset and offset of active intervals. Furthermore, since the signal characteristics at inflection points are analyzed instead of signal samples, the computational time is significantly low, ensuring the real-time application of our algorithm. Background: Identifying transcription factors binding sites (TFBSs) plays an important role in understanding gene regulatory processes. The underlying mechanism of the specific binding for transcription factors (TFs) is still poorly understood. Previous machine learning-based approaches to identifying TFBSs commonly map a known TFBS to a one-dimensional vector using its physicochemical properties. However, when the dimension-sample rate is large (i.e., number of dimensions/number of samples), concatenating different physicochemical properties to a one-dimensional vector not only is likely to lose some structural information, but also poses significant challenges to recognition methods.Materials and Method: In this paper, we introduce a purely geometric representation method, tensor (also called multidimensional array), to represent TFs using their physicochemical properties. Accompanying the multidimensional array representation, we also develop a tensor-based recognition method, tensor partial least squares classifier (abbreviated as TPLSC). Intuitively, multidimensional arrays enable borrowing more information than one-dimensional arrays. The performance of each method is evaluated by average F-measure on 51 Escherichia coli TFs from RegulonDB database.Results: In our first experiment, the results show that multiple nucleotide properties can obtain more power than dinucleotide properties. In the second experiment, the results demonstrate that our method can gain increased prediction power, roughly 33% improvements more than the best result from existing methods.Conclusion: The representation method for TFs is an important step in TFBSs recognition. We illustrate the benefits of this representation on real data application via a series of experiments. Pressure injuries represent a tremendous healthcare challenge in many nations. Elderly and disabled people are the most affected by this fast growing disease. Hence, an accurate diagnosis of pressure injuries is paramount for efficient treatment. The characteristics of these wounds are crucial indicators for the progress of the healing. While invasive methods to retrieve information are not only painful to the patients but may also increase the risk of infections, non-invasive techniques by means of imaging systems provide a better monitoring of the wound healing processes without causing any harm to the patients. These systems should include an accurate segmentation of the wound, the classification of its tissue types, the metrics including the diameter, area and volume, as well as the healing evaluation. Therefore, the aim of this survey is to provide the reader with an overview of imaging techniques for the analysis and monitoring of pressure injuries as an aid to their diagnosis, and proof of the efficiency of Deep Learning to overcome this problem and even outperform the previous methods. Clinical lab tests play an important role in disease diagnose and medical treatment, the test results have been utilized widely in predictive modeling tasks in healthcare. However, in most existing works, the loss function implicitly assumes that the value of the sample used to be predicted is the only correct one. This assumption fails to hold for lab test data, which usually are within respective tolerable ranges or imprecision ranges. In addition, the historical lab test data is always organized based on their sequential position, the timestamps between the data are often neglected. In this paper, we study the issue of building robust models while simultaneously taking imprecision and timestamp of the data into account with better generalization. In particular, "IR loss" is proposed in which each data in imprecision range space has a certain probability to be the real value, participating in the loss calculation. The loss is then defined as the integral of the error of each point in the impression range space. The sampling and discretization methods are proposed for loss calculation. A heuristic learning algorithm is developed to learn the model parameters. We further apply IR loss for disease progress prediction while the input data is organized as sequence. We reformulate the prediction task with timestamp based on Long Short-Term Memory (LSTM) network. At the same time, the timestamp is readily combined with the proposed IR loss to avoid the change of predicted result caused by the change of the test values in small time range. We conducted the experiments based on two real world datasets. Experimental results show that the prediction method based on IR loss can provide more accurate prediction result for different kinds of task and diverse learning methods. Abstract: Objectives: Balancing the trade-offs between solution quality, problem-solving efficiency, and transparency is an important challenge in medical applications of conversational case-based reasoning (CCBR). For example, test selection in CCBR is often based on strategies in which the absence of a specific hypothesis (e.g., diagnosis) to be confirmed makes it difficult to explain the relevance of test results that users are asked to provide. In this paper, we present an approach to CCBR in medical classification and diagnosis that aims to increase transparency while also providing high levels of accuracy and efficiency. Methods: We present an algorithm for CCBR called iNN(k) in which feature selection is driven by the goal of confirming a target class and informed by a measure of a feature''s discriminating power in favor of the target class. As we demonstrate in a CCBR system called CBR-Confirm, this enables a CCBR system to explain the relevance of any question it asks the user. We evaluate the algorithm''s accuracy and efficiency on a selection of datasets related to medicine and health care. Results: The performance of iNN(k) on a given dataset is shown to depend on the value of k and on whether local or global feature selection is used in the algorithm. The combination of these parameters for which iNN(k) is most effective in addressing the trade-off between accuracy and efficiency is identified for each of the selected datasets. For example, only 42% and 51% on average of features in a complete problem description were needed by iNN(k) to provide accuracy levels of 86.5% and 84.3% respectively on the lymphography and SPECT heart datasets from the UCI machine learning repository. Conclusion: Our results demonstrate the ability of iNN(k) to provide high levels of accuracy on most of the selected datasets, while often requiring the user to provide only a small subset of the features in a complete problem description, and enabling a CCBR system to explain the relevance of any question it asks the user. Summary: Objective: AutoNRT™ is an automated system that measures electrically evoked compound action potential (ECAP) thresholds from the auditory nerve with the Nucleus® Freedom™ cochlear implant. ECAP thresholds along the electrode array are useful in objectively fitting cochlear implant systems for individual use. This paper provides the first detailed description of the AutoNRT algorithm and its expert systems, and reports the clinical success of AutoNRT to date. Methods: AutoNRT determines thresholds by visual detection, using two decision tree expert systems that automatically recognise ECAPs. The expert systems are guided by a dataset of 5393 neural response measurements. The algorithm approaches threshold from lower stimulus levels, ensuring recipient safety during postoperative measurements. Intraoperative measurements use the same algorithm but proceed faster by beginning at stimulus levels much closer to threshold. When searching for ECAPs, AutoNRT uses a highly specific expert system (specificity of 99% during training, 96% during testing; sensitivity of 91% during training, 89% during testing). Once ECAPs are established, AutoNRT uses an unbiased expert system to determine an accurate threshold. Throughout the execution of the algorithm, recording parameters (such as implant amplifier gain) are automatically optimised when needed. Results: In a study that included 29 intraoperative and 29 postoperative subjects (a total of 418 electrodes), AutoNRT determined a threshold in 93% of cases where a human expert also determined a threshold. When compared to the median threshold of multiple human observers on 77 randomly selected electrodes, AutoNRT performed as accurately as the ‘average’ clinician. Conclusions: AutoNRT has demonstrated a high success rate and a level of performance that is comparable with human experts. It has been used in many clinics worldwide throughout the clinical trial and commercial launch of Nucleus Custom Sound™ Suite, significantly streamlining the clinical procedures associated with cochlear implant use. Paralyzed patients were increasing day by day. Some of the neurodegenerative diseases like amyotrophic lateral sclerosis, Brainstem Leison, Stupor and Muscular dystrophy affect the muscle movements in the body. The affected persons were unable to migrate. To overcome from their problem they need some assistive technology with the help of bio signals. Electrooculogram (EOG) based Human Computer Interaction (HCI) is one of the technique used in recent days to overcome such problem. In this paper we clearly check the possibilities of creating nine states HCI by our proposed method. Signals were captured through five electrodes placed on the subjects face around the eyes. These signals were amplified with ADT26 bio amplifier, filtered with notch filter, and processed with reference power and band power techniques to extract features to detect the eye movements and mapped with Time Delay Neural Network to classify the eye movements to generate control signal to control external hardware devices. Our experimental study reports that maximum average classification of 91.09% for reference power feature and 91.55%-for band power feature respectively. The obtained result confirms that band power features with TDNN network models shows better performance than reference features for all subjects. From this outcome we conclude that band power features with TDNN network models was more suitable for classifying the eleven difference eye movements for individual subjects. To validate the result obtained from this method we categorize the subjects in age wise to check the accuracy of the system. Single trail analysis was conducted in offline to identify the recognizing accuracy of the proposed system. The result summarize that band power features with TDNN network models exceed the reference power with TDNN network model used in this study. Through the outcome we conclude that that band power features with TDNN network was more suitable for designing EOG based HCI in offline mode. [ABSTRACT FROM AUTHOR]

Introduction: Machine learning capability holds promise to inform disease models, the discovery and development of novel disease modifying therapeutics and prevention strategies in psychiatry. Herein, we provide an introduction on how machine learning/Artificial Intelligence (AI) may instantiate such capabilities, as well as provide rationale for its application to psychiatry in both research and clinical ecosystems.Methods: Databases PubMed and PsycINFO were searched from 1966 to June 2016 for keywords:Big Data, Machine Learning, Precision Medicine, Artificial Intelligence, Mental Health, Mental Disease, Psychiatry, Data Mining, RDoC, and Research Domain Criteria. Articles selected for review were those that were determined to be aligned with the objective of this particular paper.Results: Results indicate that AI is a viable option to build useful predictors of outcome while offering objective and comparable accuracy metrics, a unique opportunity, particularly in mental health research. The approach has also consistently brought notable insight into disease models through processing the vast amount of already available multi-domain, semi-structured medical data. The opportunity for AI in psychiatry, in addition to disease-model refinement, is in characterizing those at risk, and it is likely also relevant to personalizing and discovering therapeutics.Conclusions: Machine learning currently provides an opportunity to parse disease models in complex, multi-factorial disease states (e.g. mental disorders) and could possibly inform treatment selection with existing therapies and provide bases for domain-based therapeutic discovery.
